Welcome from the CAO President Welcome to the sunshine capital of BC, Kelowna. If you are a regular Annual Scientific Sessions attendee, you don’t want to get behind because we are sprinting across Canada quicker than a flock of our beloved Canadian geese. We started out on the East coast in St John’s Newfoundland 2 years ago, then jumped half-way across our country to Winnipeg (the “Gateway to the West”) last year, before again leaping over 2 more provinces to our western most province of British Columbia this year, and finally completing the crossCanada blitz next year on the West Coast in Whistler BC. Although I am a stubble-jumper by birth (from Saskatchewan, my eastern friends), I have spent many memorable holidays in the Okanagan Valley (from Sicamous in the north, down through Vernon, Kelowna, Summerland, Penticton, and on to Osoyoos, the desert capital of Canada, in the south). For the past 15+ years I have joined a group of 15-20 guys for a 5-day golfing “holiday” in Kelowna each spring (18-36 holes per day!), and many years I have returned to the Valley with my family for summer holidays: camping, swimming, house-boating, tubing, more golfing, and of course the Winery tour(s)! I have not suffered many “rain-days” in the Okanagan Valley. CAO headquarters (Diane Gaunt, Alison Nash, and our staff), our Scientific Chairperson (Dr. Richard Marcus), and our local arrangements committee (Chaired by Dr. Cliff Moore) have had to become very creative to respond to the larger-than-expected registration for the meeting. Cliff and Richard have lined-up First-Class speakers, including: Dr. Jay Bowman, Dr. Lysle Johnston, Dr. Maurice Corbett, Dr. Richard Bell, Mr. David Roberts, and Ms. Rosemary Bray. Several special speakers have been added to further tweak your interest, for example: Mr. William Hiltz on How to Steal from a Dental Office, Ms. Michelle Kelly (a Canadian skeleton racer) and Mr. John Rae to report on the Winter Olympics in Whistler in February 2010, our own Dr. Iain Allan speaking on “Tomorrow You Will Smile” – You Too Can Make a Difference. The Round-Table Breakfast presentations are always over-subscribed, so it’s likely a good idea to be there early! The two noon-hour meetings on Friday, the New & Younger Members Lunch, and the Insurance Guidelines Lunch are almost filled to capacity again. Richard and I are again requesting that you not bring rotten eggs and tomatoes to throw at us during the Insurance presentation! Our Orthodontic Graduate students will again present their research areas of expertise with the CFAO Graduate Student Poster Program – what an opportunity for us wet-fingered Orthodontists to learn some good stuff! The Social Schedule is going to be demanding because you are not going to want to miss any event, so get rested-up ahead of time. The Golf Tournament and Tours are scheduled on Thursday, with the President’s Welcome Reception in the early evening, followed by a Tailgate Party at which you should wear your favourite team colours (Go Riders, Go!). Adopting the “flavour” of the local area, our Friday evening dinner will be at the Summerhill Winery for a “Wine & Dine Experience”. Saturday evening, after the Alumni Receptions, is purposely left open for individual plans. Our 61st Annual Scientific Session is going to be one of the best, and on behalf of your Board of Directors, I want to “Welcome You West”. Thanks so much for your support in attending our meeting, participating in our activities, and contributing to our association.

Welcome from the 2009 CAO Conference Chair It’s September 2009 and here we are, welcoming the CAO and its members to the sunny Okanagan for the CAO 61st Annual Scientific Session. We know that you will be very pleased that you've come to Kelowna and hope you will enjoy every part of the meeting. Our slate of lecturers is outstanding. They will provide a broad overview of concepts, protocols and complexities in dealing with Cl II malocclusions--from the straight-forward to the extreme--from the traditional to the leading edge. As well we will see the impact of untreated CLP in the developing world and how Rotoplast is making a difference. With the 2010 Winter Olympics just around the corner, you'll get to hear from a competitor (a skeleton rider!) and a municipality of Whistler expert (the before, during and after effects of setting up Olympic facilities in a community). The Auxiliary program will keep everyone engaged as they learn--it will be a very stimulating session. Our meeting has come together smoothly with a great effort from my staff, from our mentor, Dr. Richard Marcus and of course, our heroes at the CAO office (special thanks to Diane Gaunt and Alison Nash). Dr. Rick Odegaard has arranged a solid Round Table session and Dr. Iain Allan has provided me with great ideas and expertise. We thank the exhibitors who have supported our meeting with their participation on the trade floor. Please 'cruise the floor', say 'hi' to you reps, see some new products and thank all the exhibitors for being an important part of our meeting. I would like to give a personal 'salute' to our president, Dr. Bob Cram, for all his hard work over the years on our behalf and it is exciting to be the local chair for his meeting. He is the king of tailgate parties so we have set up his own personal tailgate party here in Kelowna for you to enjoy. And remember, despite Bob's suggestions to the contrary, you WILL be able to attend even if you are not wearing a Saskatchewan Roughriders jersey!! What should you do while you are here? You've come to one of Canada's premier wine regions so make sure you get out and do a little wine tasting. We have some fantastic golf courses to be enjoyed. Okanagan Lake is fabulous and the water should be perfect for swimming and boating while you are here. (but keep your eyes peeled for our lake monster/mascot named Ogopogo--he's been known to nibble on the odd toe!!). Hiking, biking, shopping, touring, dining, and havin' a good time should be on your schedule. Your organizing team is excited to welcome you to our little corner of the world and we hope you have the BEST time while you are here!! Kelowna and BC welcome you!!

University of Alberta EFFECT OF LOW INTENSITY PULSED ULTRASOUND ON A HUMAN TOOTH SLICE ORGAN CULTURE Saleh Al-Daghreer*, Michael Doschak, Alastair J Sloan, Paul Major, Giseon Heo, Yeng Tsui and Tarek El-Bialy University of Alberta Aim: To investigate the effect of therapeutic Low Intensity Pulsed Ultrasound effect on the human dentin pulp complex in an in vitro tissue culture model. Materials and Methods: Ninety-two premolars were extracted from 23 adolescent orthodontic patients [mean age was 14y-5m ± (2y-3m)] whose orthodontic treatment required premolar extraction. All the premolars were cut transversely into 600 µm thick slices (8-11 slices per tooth). The slices were divided into two main groups according to the frequency at which LIPUS was applied (single application and daily application). The groups were further subdivided into five subgroups (5, 10, 15 and 20 minutes and one control group). The tooth slices were cultured at 37°C in an atmosphere of 5% CO2 in air, in a humidified incubator, with the medium changed after 24 hours and then every 48 hours. LIPUS was applied using a 2.5 cm2 transducer that produces incident intensity of 30 mW/cm2 of the transducer’s surface area. After 5 days of culture tooth slices were fixed, demineralised and processed for histomorphometrical analysis (to measure the Predentin layer thickness PD, and odontoblast cell numbers OD) and real time PCR to investigate changes in expression of genes of interest (Collagen I, DMP1, DSPP, TGF β1, RANKL and OPG). Results: The histomorphometric analysis revealed that the predentin thickness was higher in the single application group (10, 5 and 15 minutes) respectively than in the daily application LIPUS treatment group and the control groups. These 3 groups (10, 5 and 15 minutes/ single application) were not significantly different from each other. The odontoblast cell count was higher also in the single application groups mainly in the 5, 10 and 15 minutes respectively than in the control and other treatment groups. Also, they were significantly different from each other. Real time PCR demonstrated no statistical significant difference between the groups in the expression of Collagen I, DMP1 and TGF β1. All groups expressed DSPP, RANKL and OPG at a low level which were not significantly different. Conclusions: • LIPUS appeared to increase in the predentin layer thickness and odontoblast cell numbers following a single application for 5, 10 and 15 minutes. • No significant difference in expression of Collagen I, DMP1 and TGF β1 was observed in these LIPUS stimulated cultures when compared to controls. • Very low expression of DSPP, RANKL and OPG from cells within dentin pulp complex in this in-vitro model after 5 days of culture.

A SYSTEMATIC REVIEW OF THE EVIDENCE BEHIND DENTAL ARCH EXPANSION WITH THE DAMON PHILOSOPHY Hussam Al-Fakir*, Carlos Flores-Mir University of Alberta Introduction: : Depending on the purpose of the expansion and the stage of dental development, arch expansion can be done using various conventional expansion appliances; for example, Hyraxes, Quad helixes, Wilson arches and bonded RMEs. However, in recent years, a new philosophy has been developed for the accomplishment of dental arch expansion, involving more bodily movement of the teeth rather than just tipping the teeth. Objective: In this systematic review the evidence behind dental arch expansion with the Damon philosophy was sought. Method: Several electronic databases were searched for relevant articles and reference lists of pertinent articles were also searched. Results: Only two articles were found that fulfilled the minimal set of criteria set in advance. One article had a sample of 19 patients with two groups (9 patients treated with RME expansion then full fixed conventional appliance, 10 patients treated only with the Damon system). The second article had a sample of 54 patients with certain selection criteria then divided randomly into two groups (one treated with conventional appliance and second with Damon system). They found that dental arch expansion is possible with Damon philosophy; however, the expansion is mostly tipping in the premolar and molar area. Arch expansion was measured and quantified using dental cast models and posteroanterior cephalometric radiographs. Conclusion: The amount of research that has been done on this topic is far from sufficient and neither supports nor dismisses the premise. A more precise tool to measure and quantify arch expansion for the whole maxilla is needed.
ACCURACY OF COMPUTER PROGRAMS IN PREDICTING ORTHOGNATHIC SURGERY SOFT AND HARD TISSUE RESPONSE 1 Neelambar Kaipatur, DMD, MSca *, Yousef Al-Thomali BDS, Cert. Orthob, Paul Major DDS, MSc.c, Carlos Flores-Mir, DDS, DSc, FRCD(C)d University of Alberta
Purpose: The aim of the present systematic reviews is to investigate the accuracy of computer programs in predicting hard and soft tissue responses following orthognathic surgery. Materials and Methods: A systematic computerized database search was conducted using several electronic databases. The reference lists of all the selected articles were also searched for any potential articles that might have been missed in the electronic search and additional information not available through the articles was obtained directly from the source (by contacting the author). Results: Out of the 40 and 79 initially identified articles only 7 and 9 articles fulfilled the final selection criteria for soft tissue and hard tissue changes respectively. All 7 articles, in general, showed accurate prediction outcomes of soft tissue response (less than 2 mm) when compared to actual post surgical results in both horizontal and vertical directions. The most significant area of error in soft tissue prediction was the lower lip. The computer programs were unable to precisely predict all the skeletal changes. Most of the prediction inaccuracies with skeletal changes were within 2 mm/degrees except for Witts analysis possibly due to difficulty in locating the functional occlusal plane on cephalograms. Conclusion: These systematic reviews demonstrated that computer programs cannot consistently predict the skeletal or soft tissue changes following orthognathic surgery but their results may be considered inside a clinically acceptable range. Although the individual errors are almost always minimal (less than 2 mm or degrees); the composite addition of these minimal errors could have clinical implications. Pediatric patients with obstructive sleep apnea have several treatment options available including nasal continuous positive airway pressure (nCPAP). The nasal mask used to deliver positive airway pressure is secured to the paranasal area using elastic headgear straps, which are adjusted to obtain a tight seal of the mask to the face. The pressure exerted by the mask on the growing bones and soft tissues of the midface may be a potential cause of mid-face hypoplasia in growing children who are on long term CPAP or BiPAP therapy during their peak growth years. Methods: A systematic review of the literature was conducted to examine the evidence concerning long term nCPAP use in pediatric patients and any associated changes in craniofacial growth and development. Electronic database searches of six databases were conducted, along with manual searches of retrieved abstracts and articles. Results: Only one article was identified that met the selection criteria for the review. The article was a case report describing severe midface retrusion in 15-year old child associated with nCPAP therapy over a ten-year period. Conclusion: There is insufficient evidence to determine the association between long term nCPAP use and changes in craniofacial growth and development in pediatric patients. The mechanisms, relative risks, and potential consequences of midface hyploplasia associated with long term nCPAP use in growing pediatric patients are discussed.
LONG-TERM CHANGES USING A BONE-ANCHORED MAXILLARY EXPANDER (BAME) IN ADOLESCENTS – RANDOMIZED CONTROL TRIAL Manuel O. Lagravère Vich*, Jason Carey, Giseon Heo, Paul W. Major Graduate Orthodontic Program, Faculty of Medicine and Dentistry University of Alberta Objective: The purpose was to compare skeletal and dental long-term changes assessed by digital volumetric images produced after maxillary expansion between a bone-anchored expansion appliance and a conventional tooth borne RME. Methods: A sample of 62 patients ranging between 11-16 years of age, all needing maxillary expansion were randomly allocated to either of three groups (traditional Hyrax tooth borne expander, bone-anchored expander and control). BAME consisted of two stainless steel onplants (8.0 mm x 3.0 mm), two miniscrews (12.0 mm length, 1.5 mm diameter), and an expansion screw. Records (CBCT) were taken at baseline and at 12 month follow-up. A repeated measure MANOVA was applied to the distances and angles measured to determine the statistical significance in time. Bonferroni post-hoc tests were used to identify specific significant differences between the treatment groups. Results: Long term changes showed that none of the three groups presented statistically significant differences in symmetry angle. Both treatment groups had significant long-term expansion at the level of the upper first molar crown and root apex, upper first premolar crown and root, maxilla alveolus in the first molar and premolar regions and central incisor root. The tooth-borne expansion resulted in significantly more long-term expansion at the upper premolar crown and upper premolar root than the bone-borne expansion appliance. Conclusion: Both expanders presented similar results. Greatest changes happened in the transverse dimension while changes in the vertical and antero-posterior dimension were negligible. Dental expansion was also greater than skeletal expansion.

THE VALIDITY AND RELIABILITY OF MIXED DENTITION ANALYSIS METHODS: A SYSTEMATIC REVIEW Nghe S. Luu*, N. Kaipatur University of Alberta Objectives: The purpose of this systematic review was to determine the best technique to perform a Mixed Dentition Analysis with regards to validity and reliability as well as any relevant factors which could influence the prediction. Methods: Papers were indentified from a search of seven online databases and evaluated based on technique modality: study models alone, radiographs alone, or combinations of study models and radiographs. Results: 460 papers were identified and ultimately distilled to 32 papers after application of specific inclusion criteria. Conclusions: Our findings indicate that all mixed dentition analysis techniques and modalities have high reliability. All modalities have positively correlated validity, but there appears to be no difference in terms of overall validity between modalities. The predictions based on combinations of study models and radiographs have the lowest variability in overall validity; radiographs alone have higher variability in overall validity; study models alone have the most variable overall validity. Dimorphisms based on ethnicity, jaw, gender, or side do not appear to have any meaningful influence on the validity of mixed dentition analysis methods.
TOOTH LENGTH MEASUREMENT ACCURACY WITH FILM AND CBCT PANORAMIC RECONSTRUCTIONS Mark Rosenblatt*, Paul Major, Giseon Heo, Jason Carey, Carlos Flores-Mir University of Alberta Objectives: 2-D panoramic images (film and digital) have been used to identify and monitor changes in root length and morphology during orthodontic treatment. Magnification, overlapping anatomy, and variations in patient and tooth position lead to unreliable results. Cone beam computed tomography (CBCT) images correct the aforementioned limitations when viewed in 3-D volumes. Third party imaging suites also can also reconstruct CBCT data into panoramic images. This study compares the accuracy of panoramic images from film and CBCT reconstructions with direct caliper measurements for tooth length. Methods: Prior to extraction of 48 premolar teeth, CBCT images were taken and panoramic radiographs reconstructed using Dolphin Imaging. Film panoramic radiographs previously taken for orthodontic screening, were scanned and calibrated for digital tooth length measurement. The extracted teeth were measured for length by micro-calipers to provide a “true” measure of length for comparison. Results: Panoramic radiographs produced images with tooth sizes that were clinically (≥1mm) and statistically significantly different in length compared to their true length (p<0.001). Panoramic reconstruction from CBCT volumes resulted in tooth lengths 1.6mm (95% C.I.: 1.1, 2.0) longer than direct caliper measurement, while film panoramic images resulted in measurements 6.3mm (95% C.I.: 5.6, 7.1) shorter. Conclusions: Although more accurate than 2-D imaging, panoramic reconstructions from 3-D volumes represent significantly different length measurements compared to extracted teeth measured directly. Clinicians must exercise caution when attempting to infer tooth length changes from panoramic images from 2-D and 3-D sources.

A SYSTEMATIC REVIEW Matthew Witt*, Mohammed Korayem, Carlos Flores-Mir University of Alberta Aim: The objective of this review is to evaluate the magnitude of aesthetic impairment that the general public is able to discern. Methods: A systematic computerized search of electronic databases was undertaken in Medline, Pubmed, EMbase, Cochrane Library and Web of Science until April of 2009. Abstracts of the retrieved results were scrutinized by two independent reviewers and papers that investigated variables that impact anterior dental aesthetics from the layperson’s perspective were identified, selected, and assigned methodological scores. Results: A total of 36 articles met the inclusion criteria. Layperson preferences and sensitivities were determined for tooth shape, tooth wear, midline deviation, incisogingival display, smile arc, buccal corridor, incisor angulation, incisor-to-lateral step, overbite, diastema, tooth size, and tooth proportion. Conclusions: Some of the conclusions are that laypersons do not discriminate tooth shapes when displayed on female models, but prefer square-round incisors and flat canines (when paired with round incisors) in male models. Laypersons can detect dental midline deviations of less than 3.0 mm but when intentionally made aware of midlines, prefer midlines that are coincident with each other and the facial midline. Laypersons prefer unworn dentitions, zero gingival display and minimal buccal corridors. There appears to be aesthetic range of incisogingival display that laypersons tolerate, from 2.0 mm maxillary central incisor coverage to 2.0 mm maxillary gingival display. Laypersons are able to detect variations in crown width or height among individual teeth, especially if the variation is unilateral. Further studies are required to validate conclusions that were based on limited studies.

CFAO Graduate Student Posterboard Abstracts – Sponsored by University of Manitoba RELAPSE OF THE LOWER INCISORS POST ORTHODONTIC TREATMENT: WHEN IS RE-TREATMENT PERCEIVED AS CLINICALLY NECESSARY, ACCORDING TO DENTAL PROFESSIONALS? Dr. Christie Laberge*, Dr. William Wiltshire, Dr. Doug Brothwell, Dr. Igor Pesun University of Manitoba Introduction: Relapse is defined as the return, following correction, toward the pre-existing condition and presents mainly as mandibular anterior re-crowding post orthodontic treatment. The purpose of this study was to examine the perception, importance, necessity and desirability of various perspectives regarding the subjective need for orthodontic re-treatment among dental professionals. Methods: A questionnaire was distributed to orthodontists across Canada (Canadian Association of Orthodontist members) as well as to dentists registered with the MDA (Manitoba Dental Association). The orthodontists were randomly assigned to two different groups; they answered the questions based on the cases as being: 1) their own patients and 2) as transfer patients. Results: Dentists and orthodontists found a significant difference between mild, moderate and severe relapse of the lower incisors (p < 0.001). Dentists recommended re-treatment similarly between mild and moderate relapse cases, however, there was a significant difference between the recommendation for re-treatment between mild and severe cases, as well as between moderate and severe cases (p < 0.001). Orthodontists in both groups recommended re-treatment between mild and severe cases and between moderate and severe cases (p < 0.001). Orthodontists in group 1 recommended re-treatment for mild and moderate cases (p < 0.05), however, orthodontists in group 2 had no significant difference between the recommendation for re-treatment between mild and moderate relapse cases (p > 0.05). Orthodontists in group 1 recommended the need for retreatment the most for all categories (p < 0.001). The dentists and orthodontists of group 2 were similar in their acceptance of mild and moderate relapse (p=0.794), whereas the orthodontists in group 1 recommended re-treatment significantly more (for mild and moderate relapse) than both dentists and orthodontists of group 2 (p < 0.05). Dentists recommended re-treatment less than both orthodontist groups for severe relapse cases (p < 0.001). Conclusions: Both consistencies and variations between dentists and orthodontists were noted, as well as, between the two groups of orthodontists in regards to recommendations for re-treatment of relapse cases. Orthodontists were more critical if they perceived the work as their own.
ORTHODONTIC TREATMENT TIMING – A SURVEY OF ORTHODONTISTS AND PEDIATRIC DENTISTS IN CANADA Eileen Lo, Charles Lekic*, William Wiltshire University of Manitoba Introduction: The ideal timing to initiate orthodontic treatment is an important, yet controversial issue. The purpose of this study was to investigate the provision of orthodontic care for 7 types of skeletal dysplasia by paediatric dentists and orthodontists in Canada. Methods: A questionnaire was distributed to randomly selected orthodontists (N=140) and paediatric dentists (N=132) throughout Canada. Surveys returned within 8 weeks were included for c2 statistical analysis. Results: The response rate was 59% for orthodontists and 54% for pediatric dentists. Orthodontists and pediatric dentists differed significantly in the timing of their first orthodontic consultation (p < 0.01). More pediatric dentists used to the dental age to determine the appropriate time to initiate treatment (p < 0.01), whereas more orthodontists relied on the pubertal indicators (p < 0.01). More orthodontists would intervene in the early mixed dentition for moderate mandibular prognathia (p < 0.01); mid-mixed dentition for severe mandibular retrognathia (p < 0.01), late mixed dentition for moderate mandibular retrognathia (p < 0.01) and permanent dentition for skeletal openbite and severe mandibular prognathia (p < 0.01). Most pediatric dentists would intervene in the early and mid-mixed dentition for the specified cases of skeletal malocclusions (p < 0.05). Conclusions: The results of this investigation indicate both consistencies and variation between orthodontic and paediatric practitioners with regard to preference in treatment timing, and the factors that influence these decisions.

COMPARISON OF MANDIBULAR ARCH DIMENSIONS BEFORE AND AFTER NON EXTRACTION ORTHODONTIC TREATMENT Cédric Cardona*, Athéna Papadakis, Claude Remise, Pierre Rompré Université de Montréal One of the objectives during orthodontic treatment is to make every effort to minimize the phenomenon of post-treatment relapse. Many scientific articles have hypothesized that one of the cornerstones of post-orthodontic treatment stability is the maintenance of the original arch form. It has been suggested that of all the dimensions characterizing the arch form, mandibular inter-canine distance is the one that should be changed the least during treatment. In our study, we compared the dimensions of 45 pairs of mandibular plaster models before and after non extraction orthodontic treatment. For our group of patients whose mandibular crowding was less than 5 mm, we found that the inter-canine distance was modified less than either the inter-molar distance or arch length. It would be interesting, in future studies, to assess the post-retention stability of this group of patients. Key words: Plaster study models, Inter-canine distance, Mandibular expansion, Stability.
PERIODONTAL EFFECTS OF SARPE: CLINICAL AND CONE BEAM COMPUTERIZED TOMOGRAPHY EVALUATION. 6-MONTH PRELIMINARY RESULTS. Section d’orthodontie, Faculté de médecine dentaire, Université de Montréal, Montréal (Québec), Canada In skeletally mature patients, transverse maxillary deficiency can be treated with surgically assisted rapid palatal expansion (SARPE). Forces delivered by the expander produce areas of compression in the periodontal ligament, which could lead to alveolar bone resorption and possible changes in the attachment level. The aim of this prospective clinical study was to evaluate the periodontal effects of SARPE, by means of a complete clinical evaluation and Cone Beam Computerized Tomography (CBCT) evaluation. Methods: The sample included 14 patients (5 males, 9 females), with a mean age of 23.0 y. ± 1.9 years (range: 16 y. 4 to 39 y. 7). All patients were treated using a bonded Hyrax-type expander and the mean expansion was 9.82 mm (7.5 - 12.0 mm). All patients had a one-year retention period. CBCTs were taken and periodontal charts completed at each time point: T0 (initial), T1 (6 months post-expansion) and T2 (1 year post-expansion). Results & Discussion: SARPE seemed to have little detrimental clinical effects on the periodontium. Radiographic data demonstrated statistically significant changes: a significant decrease in the buccal alveolar bone thickness on most teeth, a significant increase in the palatal alveolar bone thickness on most teeth, a decrease in the buccal alveolar crest level of all canines and posterior teeth, and a tendency towards a decrease in the interproximal alveolar crest level on the mesial aspect of both central incisors. These changes could eventually have a significant clinical impact on the periodontium.

The objective of this clinical prospective comparative study was to evaluate the effects of SARPE three-dimensionally on the maxillary teeth, their supporting alveolar bone as well as the basal bone of both maxillae. Materials and Methods: The sample consisted of 14 subjects (mean age: 23.0 ± 1.9 years). The patients were treated using a Hyrax expansion appliance and a retention period of one year without any other orthodontic treatment was observed. A cone-beam computed tomography (CBCT) evaluation was performed before and 6 months after the end of expansion. Thirty-seven hard tissue parameters were measured on the three-dimensional volumes. Methods: Subjects were 604 patients aged 7 to 17 years presenting for orthodontic screening in a university clinic. The parents completed a health and sleep behaviour questionnaire prior to the child being evaluated in the orthodontic clinic. Results: Radiological results have demonstrated a significant increase in dento-alveolar inclination and in dental, alveolar and skeletal widths of the maxilla, with a tendency towards a gradually increasing expansion supero-inferiorly and postero-anteriorly. No significant sagittal displacement of anatomic landmarks located of the maxillae, nor any significant correlation between expansion screw activation and radiological parameters were found. Conclusions: SARPE causes buccal dentoalveolar tipping in the posterior superior region and significant transverse expansion on all hard tissues of the maxilla. However, SARPE does not induce any relevant sagittal movement of the maxilla and the mandible. The changes that were observed do not have a correlation with expansion screw activation.
MANDIBULAR MORPHOLOGY AND GROWTH RELATED TO HYPODONTIA AND RESPIRATORY DISTRESS IN SUBJECTS WITH PIERRE ROBIN SEQUENCE Christos Sideris*, Hicham El-Khatib, Claude Remise, Louise Caouette-Laberge, Pierre Rompré Université de Montréal Pierre Robin Sequence (PRS) consists of a triad of distinctive symptoms that are expressed concurrently: micrognathia, glossoptosis – causing respiratory distress – and cleft palate. The hypothesis of this study is that mandibular morphology and growth in subjects with PRS varies with the severity of respiratory distress and mandibular hypodontia. The experimental group consisted of 32 patients with the non-syndromic form of PRS (15 males, 17 females). Cephalometric tracings were performed at T1 (mean 7.8 years) and T2 (mean 12.6 years). Statistical comparisons were made using one-way ANOVA; the significance level was set at p< 0.05. Patients with PRS and mandibular hypodontia when compared to those with PRS and normal dentition, have a shorter posterior cranial base (observed at T2 only), a shorter mandibular body, a larger ANB angle, a shorter lower facial height, a higher facial convexity, a higher A-B(HP) value and a longer Y-axis. All these clinical characteristics don’t normalize with growth. Patients with a severe respiratory distress compared to those with a mild respiratory distress observed at birth have a shorter ramal length and a higher gonial angle. These clinical characteristics are inexistent at T2. In conclusion, mandibular morphology and growth in subjects affected by PRS and mandibular hypodontia is different than those affected by PRS with normal dentition. This results in a more severe Class II jaw relationship that does not normalize with growth. In addition, the severity of respiratory distress observed at birth affects future mandibular morphology but only in a transitory manner.

CFAO Graduate Student Posterboard Abstracts – Sponsored by University of Toronto INTERCENTER COMPARISON OF TREATMENT OUTCOME IN PATIENTS WITH COMPLETE UNILATERAL AND BILATERAL CLEFT LIP AND PALATE: ANALYSIS OF CRANIOFACIAL FORM Gregory S. Long, Jr. Discipline of Orthodontics, Faculty of Dentistry University of Toronto Background: Several different surgical treatment protocols have been advocated for the management of patients with complete unilateral cleft lip and palate (CUCLP) and complete bilateral cleft lip and palate (CBCLP), and little evidence exists on comparison of the outcomes of each protocol. Objectives: 1) To compare craniofacial morphology among individuals with repaired CUCLP treated at four North American centers, through cephalometric analysis. 2) To compare craniofacial morphology among individuals with repaired CBCLP treated at three North American centers, through cephalometric analysis. This is meant to serve as a means of treatment outcome assessment, as well as an effort to provide cleft lip and palate (CLP) teams with a basis from which to perform clinical audits. Methods: A retrospective study was conducted examining the lateral cephalometric radiographs of 148 individuals with repaired CUCLP (average age = 8y 8m, M/F = 99/49, cleft L/R = 106/42) as well as the lateral cephalometric radiographs of 93 individuals with repaired CBCLP (average age = 8y 10m, M/F = 61/32). Main outcome measures included 16 angular, 7 linear and 2 ratio cephalometric parameters. The group means for the different centers per cephalometric measurement were compared using analysis of variance to assess statistical significance. Specifically, pairwise comparisons of means were performed using the Tukey-Kramer method. Results: For the CUCLP sample, significant differences were found in the sagittal maxillary prominence among the four centers. The most significant differences were seen between centers 1 (largest maxillary prominence) and 3 (lowest maxillary prominence). The differences between the two centers in the mean SNA and ANB angles were highly statistically significant (p<0.001); whereas for the mean Ba-N-ANS’, and ANS’-N-Pg angles the differences were significant at the p<0.01 and p<0.05 level, respectively. Similarly, for the CBCLP sample, the most significant differences were observed between center 1 (largest maxillary prominence), and center 3 group A (lowest maxillary prominence), and center 4. The differences between the three centers in the mean SNA and Ba-N-ANS’ angles were highly statistically significant (p<0.001); whereas for the mean ANB and ANS’-N-Pg angles and WITS appraisal the differences between center 1 and center 3 group A, and center 1 and center 4 were significant at the p<0.001 and p<0.01 level, respectively. Conclusions: 1) Significant differences in craniofacial morphology, specifically hard and soft tissue maxillary prominence, exist among patients with CUCLP treated according to different surgical protocols at the centers investigated in this study; 2) Significant differences in craniofacial morphology, specifically hard and soft tissue maxillary prominence, exist among patients with CBCLP treated according to different surgical protocols at the centers investigated in this study; 3) For both the CUCLP and CBCLP groups, center 1 patients had the greatest hard-tissue maxillary prominence and soft-tissue convexity; 4) Patients from center 3 and center 3 group A, the only center to perform primary alveolar bone grafting, had the least prominent maxilla and straightest soft-tissue profile for both the CUCLP and CBCLP groups, respectively.

DEVELOPMENT OF A THREE-DIMENSIONAL IN VITRO MODEL SYSTEM TO STUDY ORTHODONTIC TOOTH MOVEMENT A.J. Heckler*, I.J. Pereira, P.J. Brooks, S-G. Gong, and C.A. Simmons University of Toronto Few three-dimensional (3D) models exist to study cell and molecular regulation of orthodontic tooth movement (OTM). The aim of this work was to develop a 3D in vitro model to study mechanical loading of human periodontal ligament fibroblasts (hPDLF). Methods: hPDLFs were seeded into fibrillar collagen gels constrained in culture wells. The viability, proliferation, and function of the hPDLFs were characterized over 14 days. hPDLF-seeded collagen gels were then cast in a device with movable end plates and subjected to static tensile, compressive, or no loading conditions. Cells were characterized within the model and the strain distribution was assessed. Results: Within the well-based cultures, cell viability was uniform throughout the gel. The number of cells increased five-fold by D7 (p<0.05) and remained similar until D14. Collagen fibre alignment and the degree of contraction of released gels was highest in the D7 and D14 gels (p<0.05). In the 3D model, viability was greatest in hPDLFs subjected to tension (p<0.05). Finite element modelling of the 3D model predicted strain magnitudes of approximately 12% in both the compression and the tension cases. Conclusions: The collagen gel supports hPDLF proliferation, viability, and the emergence of a contractile phenotype, replicating the constrained condition of the human PDL in vivo. The 3D model mimicked PDL strains similar to those observed during OTM, and can be used for future studies to better understand the effects of OTM on PDL fibroblasts. CONE BEAM CT STUDY OF RELATIONSHIP BETWEEN FACE HEIGHT AND DENTOALVEOLAR POSITION. Bn Laski*, Bd Tompson, A Metaxas, Ewn Lam Discipline of Orthodontics, Faculty of Dentistry, University of Toronto. University of Toronto Background: The intraoral findings of patients presenting with differing lower anterior face heights are variable. Cephalometric and model studies on the subject have elicited contradictory results. By analyzing the pre-treatment cone beam computed tomography (CBCT) records of orthodontic patients, it is possible to assess dental and skeletal features of interest that are relevant to orthodontic treatment planning. Objective: To determine differences between the dentoalveolar positions of maxillary and mandibular central incisors and first molars in subjects classified into three groups: long, normal and short lower anterior face height. Methods: Upper anterior face height/lower anterior face height ratios (UAFH/LAFH) were determined from 111 pretreatment iCAT® (Imaging Sciences International, Pennsylvania) CBCT scans and analyzed with AMIRA® (Mercury Computer Systems, Germany) imaging software. Upper and lower central incisor and first molar vertical heights and buccolingual inclinations were measured relative to fixed reference planes. Mean differences in tooth heights and inclinations between the groups were assessed using an analysis of variance. Results: Clinically significant differences in dentoalveolar heights, but not buccolingual inclinations, between the three face height groups were found. Conclusions: Face height and dentoalveolar position can be determined using cbCT image data. Vertical height, not buccolingual inclination, contributes to differences in lower anterior face height. CFAO Graduate Student Posterboard Abstracts – Sponsored by University of Western Ontario IN-VITRO COMPARISON OF THREE INDIRECT BONDING ADHESIVE SYSTEMS AND A DIRECT BONDING CONTROL Dr. Mitch Miller, Dr. Timothey Foley, Dr. Antonios Mamandras, Dr. Lesley Short, Dr. Amin Rizkalla, Dr. David Banting, Dr. Sherri Leung* University of Western Ontario Aim: This in-vitro study investigated shear peel bond strength (SPBS), bond failure upon transfer tray removal, adhesive remnant index (ARI), and enamel fracture incidence for three indirect bonding adhesive systems123 compared to a direct bonded control4. Materials and Methods: One hundred ninety-two human bicuspid teeth were arranged in acrylic bases mimicking human archforms, with four arches per group. Teeth were etched, bonded, stored for 100 days at 37°C, thermocycled and subsequently debonded with an Instron universal testing machine. Results and Conclusions: All adhesive groups demonstrated sufficient in-vitro SPBS values. Significant differences (p<0.05) were found among the four adhesive groups. The Direct and Light-Cured indirect groups had significantly higher bond strengths than the other indirect groups. The lowest SPBS values observed in all groups were clinically acceptable; however, the lowest values in the Sondhi and Thermacure groups less optimal. Two bond failures were noted upon transfer tray removal, one in the Sondhi group and one in the Light-Cured indirect group. ARI values showed the Direct group experienced mainly adhesivebracket interface failures, while Indirect groups demonstrated cohesive bond failures, with a disproportionately high number of enamel-adhesive failures in the Sondhi indirect group. Despite having the highest SPBS, the Light-Cured group exhibited the lowest incidence of enamel fracture. IMPACTED CANINES TREATED WITH CLOSED VS OPEN SURGICAL-ORTHODONTIC APPROACH: A SUBJECTIVE EVALUATION Dr. Nadia Nizam, Dr. Lesley Short, Dr. Antonios Mamandras, Dr. Bruce Hill, Dr. Sahza Hatibovic-Kofman, Dr. David Banting, Dr. Alison Stefanuto* University of Western Ontario Purpose: This study examined differences in esthetic outcome of palatally impacted maxillary canines treated with either open or closed surgical exposure techniques. Materials and Methods: Sixteen subjects in each group had a unilateral impacted maxillary canine of equal severity. The contralateral untreated canine was used as a control. Final esthetic outcome was assessed by orthodontists, dentists, and lay persons using a 100mm visual analog scale. Features evaluated included crown height, attached gingiva, mesiodistal tip, buccolingual torque, rotation, and overall esthetics. Results: No statistical or clinical difference in overall esthetics between the closed and open surgical techniques was found, as rated by each examiner group. Orthodontists and dentists found torque unsatisfactory regardless of surgical exposure technique. Orthodontists found the amount of attached gingiva unsatisfactory for both surgical techniques compared to the untreated side, although open exposure results were generally preferred over closed exposure. Orthodontists also found the closed technique led to unsatisfactory crown height, rotation, and tip. Dentists found the closed technique led to unsatisfactory tip, rotation, attached gingiva and crown height when comparing treatment to control. Lay persons were unsatisfied with tip, torque, and rotation in both treatment groups when compared to control, while attached gingiva and crown height were less satisfactory for closed surgical exposure vs. controls. Conclusion: Closed surgical exposure of palatally impacted maxillary canines led to less satisfactory esthetic outcomes than open surgical exposure. Although the difference was statistically significant, no clinical significance was established between open and closed treatment groups or between treatment and control canines.

Dr. Dolly Bharwani, Dr. Antonios Mamandras, Dr. Lesley Short, Dr. John Murray, Dr. David Banting, Dr. Harinder Sandhu, Dr. Zach Williams* University of Western Ontario Introduction: Degenerative joint disease is mildly prevalent in a pre-orthodontic population and can contribute to jaw pain and skeletal relapse post-orthodontic treatment. Purpose: To determine whether craniofacial form or particular treatment modalities are related to TMJ condylar degeneration. Materials and Methods: The cephalometric radiographs of 61 subjects with moderate-severe condylar degeneration (as diagnosed from panoramic radiographs) were traced and treatment factors were recorded. Results: Cephalometric findings of significance in this study included increased facial convexity, increased ANB, decreased SNB, decreased Pg-NA perpendicular, increased mandibular plane angle, increased gonial angle, increased overjet, and upright lower incisors when compared to cephalometric standards. Treatment modalities of significant prevalence are orthognathic surgery and Class II or Cl III elastics. Conclusions: Results from this study are inconclusive without a control group but the following observations can be made: 1. The prevalence of moderate to severe TMJ condylar degeneration, in a pre-orthodontic population was 3.02% 2. Subjects with moderate to severe TMJ condylar degeneration demonstrated: a. Mandibular retrognathia b. Upright mandibular incisors c. Dolichofacial pattern and vertical growth pattern 3. Females are at higher risks for developing TMJ condylar changes than males 4. Subjects requiring orthognathic surgery may be at risk fro developing TMJ condylar changes 5. The use of Class II or III elastics may increase the risk of developing TMJ condylar changes
