The Unions and the Government of Belize are making progress in their impasse as it relates to the Unions accepting a 10 percent pay-cut. Talks will resume on Monday April 5. Said pay-cut has been proposed by the government as a necessary move in terms of cost saving at this time given the dual cataclysm to have hit Belize in the last 13 years visà-vis the UDP and the Covid-19 pandemic. Details are scarce given the closed session of the talks between the Unions and the Government lead by the Deputy Prime Minister Cordel Hyde and the Hon. Francis Fonseca, and the Hon, Henry Charles Usher. The talks have a date which is a public and bank holiday is progress and is a definite show of good faith on both sides that seem that have been thoroughly divided

Belmopan. March 31, 2021. 4:30 p.m. The Cabinet, chaired by the Prime Minister Hon. John Briceño, met in regular session on March 30, 2021. • Cabinet received an update from its union consultation team on the progress of discussions with the Joint Union concerning fiscal adjustment efforts. Further consultations will continue on April 5, 2021. • Cabinet approved the protocols presented by the Ministry of Health & Wellness for the re-opening of casinos scheduled for April 15, 2021. • Cabinet approved the extension of the curfew to the April 15, 2021. • The Minister of Health & Wellness informed Cabinet that an additional 33,600 AstraZeneca vaccines would be arriving in Belize on Mach 31, 2021. This is the first shipment of vaccines through the COVAX facility. • Finally, Cabinet supported the call from the Minister of Home Affairs and New Growth Industries for adherence to all safety measures over the Easter weekend and endorsed the “Easter Safety Three Ws Campaign to wash your hands, wear your mask and watch your distance.” Ends

COPA AIRLINES RESUMES FLIGHT SERVICE TO BELIZE BELIZE CITY, BELIZE, Tuesday, March 30, 2021– The Belize Tourism Board (BTB)
welcomes COPA Airlines’ announcement this week that it will resume its weekly service to Belize effective June 2021.
COPA Airlines first introduced service to Belize from Panama in December 2015, but had to suspend operations in 2020 when the COVID-19 pandemic crippled the global travel industry. The BTB has maintained strong relationships with its international airline partners throughout the COVID pandemic, and continues to seek opportunities to further expand accessibility to the destination. COPA’s resumption of service will create new opportunities to promote and sell the destination to key markets within South America and Europe. Additional details regarding flight frequency will be shared in the coming weeks
31 March 2021 We are happy to be here today to receive this first instalment of vaccines through the COVAX Initiative. Through this delivery more Belizeans will be added to the list of those who have gotten their first jab. The more persons we vaccinate the better we protect ourselves and our families. Already research has shown that people who receive the vaccine are protected from getting so sick that they end up in the Hospital or worse end up dying from COVID-19. So yes, the vaccine saves lives! I think this is amazing, considering that one year ago we had thousands and thousands of people dying daily all over the world, and even here at home we lost over 300 Belizeans. This is why this COVAX facility is so special because it is an effort that has the world working together to ensure that countries large and small alike are able to get vaccines so that as one human family, we can stop the spread of this dreadful virus. I say this with confidence, my husband is grateful to all of you for your patience and understanding. Our Government is proud of you for the way you have cooperated by following the rules and guidelines that have kept the spread of the virus under control. Four months ago, over 5,000 Belizeans had contracted the Corona Virus. Today, working together, we have 37 Belizeans who have tested positive for the coronavirus. WE DID THIS TOGETHER and because of our efforts we are now moving yet another step closer to winning the war against COVID with the phased roll-out of the vaccines. This will allow us to come together with family and friends in thanksgiving and celebrate the holiday season. In the meantime, however, we must not let our guard down, we must continue to be vigilant together. This means that when it is your turn, you should get vaccinated! It also means that we should all encourage our family members and friends to get vaccinated as well, especially the most vulnerable among us. And we must continue with the three W’s: Wear your mask! Watch your distance! And wash your hands. Today is a good day, our work to rid Belize of the COVID-19 virus continues on pace and our government continues its work to defeat COVID which is one of the key pillars of PlanBelize. EDITORIAL
Every Great Sacrifice has its Just Reward
T
raditionally Easter for many is a time for sober reflection on sacrifices made and the joyful celebration of rebirth. Last year, we were preparing not for Easter and the traditions and celebrations, but rather we were preparing for what became known as the Great Lockdown. Every country has been affected and the casualties and losses are incalculable. The Pandemic has forced us to evolve and forever changed the way we live, work and play. Our children have been out of the classroom for a year now and many of us have been out of work for about the same time. Belizeans by and large have learnt to be kinder, more considerate and patient with one another. We have had to adapt merely to survive. After a year of great sacrifices, we are once again on the cusp of resurgence. The vaccines are being rolled out in phases and Belizeans are ready to get back to work safely and with the assurance that immunity brings. While the Covid-19 Pandemic will certainly become part of our history, the lessons it has thought us will remain. Certainly, not everything in our economy is as sweet as the Easter Basket treats. The teachers and public servants are unyielding in their demands, and it seems likely that they are willing to break us all rather than bend. Unemployment rates and extreme poverty are staggering, and we continue to borrow just to keep the social safety nets going. There is urgent need to instead focus on jump starting our economy, and for putting Belizeans to work and weaning them off the social programs, that keep adding to our national debt. Children must start school, and the phased-in program will ensure that it is done in a controlled and supervised manner to be sure that safety measures are implemented. Easter comes in Spring and just as it reminds us of sacrifices made, it also reminds us of rebirth and hope. Just as we are going through hardships and sacrifices will ultimately have to be made for the greater good, so too will this sacrifice allow for better times ahead. The spring ushers in new hope and with it new opportunities. Already there are signs of good things to come, as the Tourism Industry shows healthy promise already. Schools are already preparing to start either now or in September and our mainstays which are the agro-productive sector are being reinforced and nurtured. The much-needed infrastructure that all sectors rely on is being repaired and refurbished. Our roadways are the lifeline of farmers and commuters and contribute greatly to our productivity. There is much that has been done, and so much more that needs to be accomplished. As we go into the Holy Week we must remember to keep safe and follow all the established protocols. Every great sacrifice has its just reward.

Todo Gran Sacrificio Tiene Su Justa Recompensa

radicionalmente, la Pascua es para muchos un momento de sobria reflexión sobre los sacrificios realizados y la alegre celebración del renacimiento. El año pasado no nos preparamos para la Pascua y las tradiciones y celebraciones, sino que nos preparamos para lo que se conoció como el Gran Encierro. Todos los países se han visto afectados y las bajas y pérdidas son incalculables. La pandemia nos ha obligado a evolucionar y ha cambiado para siempre nuestra forma de vivir, trabajar y jugar. Nuestros hijos llevan un año fuera de las aulas y muchos de nosotros llevamos el mismo tiempo sin trabajar. Los beliceños, en general, han aprendido a ser más amables, considerados y pacientes entre sí. Hemos tenido simplemente que adaptarnos para sobrevivir. Tras un año de grandes sacrificios, estamos de nuevo en la cúspide del resurgimiento. Las vacunas se están distribuyendo por fases y los beliceños están preparados para volver al trabajo de forma segura y con la seguridad que da la inmunidad. Aunque la pandemia de Covid-19 pasará sin duda a formar parte de nuestra historia, las lecciones que nos ha enseñado permanecerán. Ciertamente, no todo en nuestra economía es tan dulce como las golosinas de la cesta de Pascua. Los maestros y los funcionarios públicos son inflexibles en sus demandas, y parece probable que estén dispuestos a llevarnos a la ruina a todos antes que doblegarse. Las tasas de desempleo y de pobreza extrema son asombrosas, y seguimos pidiendo préstamos sólo para mantener las redes de seguridad social. Es urgente centrarse en la reactivación de nuestra economía y en poner a los beliceños a trabajar y retirarlos de los programas sociales que siguen aumentando nuestra deuda nacional. Los niños deben empezar a ir a la escuela y el programa de introducción gradual garantizará que se haga de forma controlada y supervisada para asegurarse de que se aplican las medidas de seguridad. La Pascua llega en la primavera y al igual que nos recuerda los sacrificios realizados, también nos recuerda el renacimiento y la esperanza. Así como estamos pasando por dificultades y que al final habrá que hacer sacrificios por un bien mayor, también este sacrificio permitirá que vengan tiempos mejores. La primavera anuncia una nueva esperanza y con ella nuevas oportunidades. Ya hay indicios de que se avecinan cosas buenas, ya que el sector del turismo se muestra muy prometedor. Las escuelas ya se están preparando para empezar desde ahora o en septiembre y nuestros pilares que son el sector agro-productivo, se están reforzando y alimentando. Se están reparando y renovando las tan necesarias infraestructuras de las que dependen todos los sectores. Nuestras carreteras son las arterias vitales de los agricultores y de los viajeros y contribuyen en gran medida a nuestra productividad. Es mucho lo que se ha hecho y mucho más lo que queda para hacer. A medida que nos adentramos en la Semana Santa debemos recordar que debemos mantener la seguridad y seguir todos los protocolos establecidos. Todo gran sacrificio tiene su justa recompensa.

FUEL PRICES, THE TRUTH Any and all attempts to explain fuel prices in Belize let alone the world is probably a serious attempt at futility. The economic behemoth is one that touches the very heart of everyone’s pocket—rich and poor. Business men see their bottom line diminishing and the consumer sees their purchasing power shrinking at any price hike and ignore the as often price decreases. The tease cartoons of the past and the memes of today do not forgive any hike in fuel prices. The excuses of the past or those of today do not hold up a second. Still, let’s point out some. Belize spends millions of dollars in foreign exchange approximately every two weeks for fuel imported to Belize via large tanker ships. Export earnings pay for this. Every Belizean consumer directly or indirectly pays for this. All Belizeans consume imported goods. Not all Belizeans are involved directly in bringing in foreign exchange. With all that, one can see the dire importance of Agriculture and Tourism. In 2008, a fuel tax ‘formula’ already existed where the government gained revenue from fuel sales to Belizeans. This of course sustained the importation but still the US dollars
has to be spent. It is complex. The importer(we can all see the fuel silos at the Port) buys the fuels and thereafter expenses and taxes bloat the price to what the consumer sees and pays. These include importer profits, freight from port to gas stations, fuel evaporation loses, gas station expenses and profit, and government taxes are the main additions to the acquisition price. 2010 Dean Barrow added to the formula a dollar flat tax on every gallon of fuel from the consumer. Dean Barrow felt that this was innovative. This, ladies and gentlemen, was Dean Barrow showing early on that he was incompetent and lazy, at best. So this flat tax became and still is law. 2012 Fuel was at a world highest… in Belize during the first of UDP’s three recessions it was almost 13 dollars in the city and over that in Punta Gorda 2020 when the pandemic had been announced, Dean Barrow and the UDP add 50 more cents. He called it a surcharge. It is law at the moment. In the larger scheme of things, world petroleum prices plays some role. This is a perpetually fluctuating environment. The USA is now the biggest producer of petroleum in the world. But Saudi Arabia is the biggest exporter of petroleum. Complex? As this article is being penned, we see a headline: Oil Prices Fall As Bearish Sentiment Returns. This is as a result of the Suez
Canal opening up after the Ever Given cargo ship was dislodged and moving once more. Will the prices at Belize’s pump go down tomorrow? Remember the acquisition every two weeks. If there is any lowering it could be see then. The oil giants play a role in this but they are also at the behest of unseen economic forces that rule. It is argued that oil petroleum prices do NOT totally determine pump price. In 2020, oil prices dipped under ZERO but fuel at the pump was not free. Now that fuel is at 120 Belize per barrel and our prices are not exactly at historic highs. As this piece began, it is difficult to argue that the price of fuel is not too high when all of us feel it somehow, especially in pandemic times. On June 3, 2019, the world average price was 8.60 Belize dollars per gallon of premium. At the Belizean gas stations we were paying 11.30. This is a difference of exactly $2.70. On March 23, 2020, the world average price was 7.47 Belize dollars per gallon of premium. At the Belizean gas stations we were paying 10.90. This is a difference of exactly $3.43. On March 30, 2021, the world average price was 8.49 Belize dollars per gallon of premium. At the Belizean gas stations we were paying 11.64. This is a difference of exactly $3.15. Largely, the prices of fuel are seasonal. A large part of the world that determines the prices use fuel
05 5 to heat their homes in the northern hemisphere’s winter. This past February, the southern part of the United States experience frigid cold temperatures as they haven’t in their lifetimes. The cold front that this sent us might have also prevailed with fuel prices as well. When we see vehicles rolling all over Belize, we might wonder if the fuel prices affect them. Not all vehicles are pleasure. Many are for worker’s transportation and delivery of goods all over the nation. But, as we use God’s given petroleum it is still something to remember along with Covid protocols—conserve fuel. It helps Belize’s economy. Now that we are in austere times it is time to revive the idea of driving to conserve fuel. Greetings friends, We have much to be grateful for. This past week marked one year of living under the painful effects of the coronavirus pandemic. In that time, 317 of our fellow citizens died. In their memory, we must move forward in hope. Fellow citizens our government continues to make daily progress. Over the past few days alone we have seen improvements to the water system in Bullet Tree, a new bridge at Calla Creek, better living conditions for our soldiers at Price Barracks, fifty tons of cacao exported and hundreds of sheep farmers receiving technical support. These are actions tied to our plan Belize, to improve conditions in rural life and to take care of those who serve us. Our work and fight is for building Belize’s greatness. Tourism is the great job creator, and foreign exchange earner and I want to assure you that we are doing everything to bounce back and bounce forward with the greatest urgency. Last Tuesday Minister Mahler presented his Tourism Recovery Plan; one that is inclusive and will help get our tourism sector up and running in the shortest possible time. Using a technology dashboard for tourism businesses to help open our doors again, soon we will be able to tell the world – welcome back to Belize! In line with this, our health care team continues to perform the herculean task by effectively rolling out the vaccines so that we can continue on the positive path to getting to zero with every adult Belizean vaccinated. Finally, we are starting to see the light when our children can go back to school. Soon we will start a phased and safe opening of face to face learning. COVID-19 numbers remain low in Belize while Europe and many in our region are under lockdown. We cannot and must not let down our guard. The inviting of retirees and people that are vaccinated is another open move to add energy to the reopening of the industry. The tourist sector is next to be vaccinated and this will add impetus to Belize’s brand as a safe haven for foreigners that are literally a couple hours away. Already we are have news that the Central American carrier Copa will return to Belize in mid-summer. Cruise tourism is in the horizon as well. Casinos are to be opened this coming mid-April as well with due Covid protocols in place.
Belize is Afoot, 5-0 Win vs Turk’s & Caicos The Belize national senior men’s football team, the Jaguars, rebounded from their 0-2 loss to Haiti in the FIFA World Cup 2022 Qatar first round of qualifiers last Thursday, March 25 to deliver a crushing 5-0 win against the Turks and Caicos Islands in their 2nd game at the Pan Americano Stadium in San Cristobal on Tuesday afternoon. The Jaguars had possession for most of the first half of play, but the Turks & Caicos squad was also creating plays for attacks into the Jaguars’ backfield, keeping the defenders Everald Trapp, Asrel Sutherland, Colin Westby and Deshawon Nembhard on their toes. Jaguars’ keeper Woodrow West also came off his line to keep it a nil-zip ball game for the first 45 minutes. The Jaguars’ wingers Krisean Lopez and Nana Mensah worked the ball up the sidelines while midfielders Angelo Capello and Jordi Polanco tried to create scoring opportunities for the strikers Deon Macaulay and Carlos Bernardez. The Turks & Caicos defense held up despite this onslaught for 45 minutes, and it was not until 3 minutes of injury time was added on to the end of the first half, that Carlos Bernardez flipped over for a ‘chilena’ reverse kick in midair to put the finish on a long cross to center from Macaulay. There was nothing the Turks’ goalie Pendieno Brooks could do as the ball rocketed inside the left post and into the back of the net: 1-0! The Jaguars came out storming in 2nd half, 19-year-old Jesse “Beast” August came off the bench to substitute for Krisean Lopez, and he immediately drove in a 2nd goal in the 47th minute: 2-0! Bernardez scored his second goal of the match a minute later to make it 3-0! Jaguars’ technical director Dale Pelayo Sr. now sent in all his bench players: Nahjib Guerra, Desmond Wade, Jose Urbina and Ean “Gato” Lewis to relieve Capello, Nana Mensah, Bernardez and Polanco. Deshawon Nembhard overlapped from the defense to score a 4th goal in the 81st minute, and Belize’s top goleador Deon Macaulay scored his 27th international goal in injury time in the 91st minute for the 5-0 win. The Jaguars’ next opponent will be Nicaragua, who will have the home field advantage in Managua on June 5. The Jaguars next home game against St. Lucia at the FFB Stadium in Belmopan on June 8, has been forfeited by that island nation which has pulled out of the World Cup. THE BELIZE TIMES The agriculture sector and its Minister Jose Mai probably have not slept a wink since November 11. From Cattle, Pigs, Chickens, Onions, Cacao, Sugarcane, and now even Soursap have become a part of what Belize exports. With time the scale will increase. It is not lost that Minister Mai was instrumental in the speedy yet controlled opening of the Corozal Freezone. This opening
allowed over 1100 jobs and much needed pay checks for men and women who have been unemployed for a pandemic year. Of course, all other ministers had a hand in this. Foreign Ministry, Finance, Tourism and Health had to be working in unison. Plenty of what we can have is dependent not only on the efficacy but the eventual real world effectiveness of the vaccine in arms. We have a largely young population so herd immunity will not be as important for us as initially as the safety of our vulnerable. All our adults being vaccinated is part and parcel of our economic revival. The

2021 message to Belize is get vaccinated…your health and Belize’s economy depends on it. Back to the unions, there is hope that the prolonged, and am sure detailed, meetings will yield results that will be beneficial for the vast majority. This is definitely one of the interpretations of what making a Belize that works for everyone means. In this version of Belize, everyone is working and having a life of a much higher quality than Barrow/Faber’s UDP provided. It is about time and Belizeans are up for it. THINK ABOUT IT JOHNNY BRICEÑO We pay tribute to the extra ordinary patience of Belize’s new Prime Minister. A mere five months ago the people of Belize by overwhelming numbers kicked out the corrupt, incompetent Barrow/Faber UDP rulers. Again in March this year, three weeks ago, a virtual voters’ revolution chased all UDP’s from the City Council and Town councils in the country. Everything that looked or smelled of the rotten UDP’s were thrown into the political dirt box. Bravo for Belize! Bravo for Belizeans! Bravo for Briceño. This quiet, unassuming, successful politician has inherited a thousand and one slings and arrows of the outrageous legacy of the arrogant Dean Barrow, who not only broke the country, but almost broke the back of the suffering people of our country. Were Barrow to have still been in office today, Sharon Fraser would be on forced leave, facing disciplinary charges and guaranteed dismissal. Elena Smith would be pilloried by political propagandists and their foul-mouthed UDP media organs. She would be told to go fly a kite. As to Messrs. Henry and Flowers, they would have been told their careers and employment would crash into an administrative cul-de-sac. With the puppet PSC Chairman their days would be numbered. But the bully Barrow is gone. His show off, high flying, purple prose and press conferences days of arrogance are gone. His name is stink out da road. His administration is thickly tainted with the adjectives of waste, greed and the ugly terminology called corruption. Johnny Briceño, Prime Minister of Belize , has displayed patience in this haggling with the Teachers and Public Service Unions. The Joint Staff Relations Council, the regular mechanism to sort out union issues has been forgone in favor of direct involvement with Cabinet Ministers. Not one, but three top-gun newly elected Ministers, led by the Deputy Prime Minister Belize have been spending hours upon hours in conversations with the unions. Cordel Hyde, Francis Fonseca and Henry Charles Usher represent the respect with which the new government holds the union leadership and their members. The new government is a powerful government. It is a popular government. The men and women who sit on the government side in the House of Representatives and in Cabinet are qualified, intelligent and committed. Many are experienced and successful individuals. All represent a team of brain power which will carry this country forward. The new government is asking the unions to agree to a salary reduction to help fight the economic crisis which Barrow and the UDP’s left us in. We all know who are responsible for this crisis. And it ain’t the PUP. It is true the unions were silent during thirteen years of waste, abuse and corruption. It true the teachers unions stood up to Borrow, Faber and their gangster government. It is also true the present Senior Managers attacked the Teachers Union for standing up. It is also true Public Service Union was nowhere to be heard or seen during the corruption of those years. But! But ladies and gentlemen, we are facing catastrophe. There is an economic crisis which threatens ruin for all. We are all in the same boat. Well, we are not all in the same boat, but we are all in the same storm. Some have speed boats, some have sail boats. Some have doreys. Some have to swim. Many will drown if the rest don’t help them. Thousands of Belizeans need help. Urgently. They need groceries, food to eat. Every day, every week. And yes, government must demonstrate it is cutting out waste and fat. We would say government needs a liposuction. It must get rid of unnecessary vehicles. It must send home the hangers-on. We support an austerity program. A serious all-inclusive austerity program. In the interim, the government cannot delay its budget and the country cannot be on pause. Nor can the country be in uncertainly. Ladies and gentlemen, Prime Minister Johnny Briceño has shown much patience. It is time to resolve the situation. OUR BOLEDO What is happening with the people’s Boledo and Lottery? Millions of dollars have been siphoned away from the government revenue to enrichen Brads and an alleged family member of Dean Barrow. This is much needed monies that can help with thousands of grocery bags for the most vulnerable and needy Belizeans.

Dean Barrow is a worthless person for doing what he did with the people‘s Boledo and Lottery. We must recover our heritage. We must expose every detail of this corruption. In the meantime, increase the tax on Boledo and Lottery earnings three fold, immediately. At the next House Meeting… Introduce a National lottery with attractive prizes. All revenues to be used for groceries and assistance to the growing numbers of Belizeans fighting food in security. Urgency needed! “DOUBLE, DOUBLE TOIL AND TROUBLE” Am reading and re-reading some Shakespeare plays. Pure joy. Alcohol and weed can’t compare. “Double, double toil and trouble; fire burn and cauldron bubble”...” by the pricking of my thumbs, something wicked this way comes” Was thinking of southside Belize City. The cauldron the bubble. Need for a rescue plan. A development zone.,, A youth corp,,. A housing project and a form of de-densification,,. Too many poor people crowded up on one another in overcrowded conditions. We on the southside need music, money, dancing, drama, singing, more marching bands. We need poetry – poets and writers. We need to bring out the Bob Marleys and Beyonce in our people. Too much poverty, too much stress. Too much negativity. Hard times create, creativity. Release the artists. Free up the young people. This is like a message to the Chief Education Officer. While planning on the reopening of schools please plan for more, much more, of the above paragraph. Books and learning is boring. Make it attractive and a joy to be learning and doing creative stuff. What all this got to do with Shakespeare and Macbeth and Julius Caesar? Figure it out. BET YOU DID NOT KNOW THIS He said to them, “but now, he who has a money bag, let him take it, and likewise a sack; and he who has no sword, let him sell his garment and buy one. For I say to you that this which is written must still be accomplished in me…And he was numbered with transgressors”… Then they said “Lord look, here are two swords” and he said to them, “It is enough” Bet you didn’t know that those words come from JESUS. Don’t take my word for it. Look in the Bible at the gospel of Luke, chapter 22, verses 36 and 37. Yep! Jesus was numbered among the transgressors. He was saying sell your garment and buy a weapon. Use this long Easter weekend to read more about the amazing revolutionary name Jesus Christ. His words remain the most powerful weapon of all. Read and get yourself educated. MAURICE BISHOP WAS IN BELIZE In March, Assad Shoman and Said Musa could not believe their ears. Breaking news across the Caribbean, Central America and the USA was that the elected, corrupt government of Dean Barrow, oops, sorry, Eric Gairy had been overthrown. This was 13th March 1979. The tiny island of Grenada in the Caribbean had experienced a military coup. It was the first and only time in history that an English Caribbean, British system of government was overthrown by civilians, police and some soldiers. Assad and Said were the two major “communists” in Belize and 1979 would prove a boon for leftists and progressive elements in the region. Leading the revolution in Grenada was the charismatic and beloved Maurice Bishop. Maurice visited Belize in September 1981 and was housed at one of the ten buildings hastily constructed in Buttonwood Bay, Belize City. This was to accommodate the many dignitaries who came to Belize to celebrate our long awaiting Independence. Belizean intellectual Godfrey Smith has written an excellent biography on Maurice Bishop and his assignation. Released last year, copies are nowhere to be found. Brian Meeks, one of the Caribbean leading intellectual who visited Belize and was hosted on one of the Krem Wednesday night show, has also written a recent article on Maurice Bishop. Meeks was in Grenada just weeks before the American military invaded and took over control of the island. It is a fascinating and enlightening history. We need to “emancipate ourselves from mental slavery, none but ourselves can free our mind” Read my beloved brothers and sisters. Find time to read. Behind Cuba’s successful pandemic response
Cuba’s long-standing commitment to health has led to a successful COVID-19 pandemic response, but it is threatened by financial and supplier issues. Talha Burki reports. As The Lancet Infectious Diseases went to press, Cuba was due to launch a phase 3 trial of its subunit conjugate vaccine against COVID-19. Soberana-2 is one of four candidate COVID-19 vaccines in development in Cuba. It is produced by the Finlay Institute in Havana. On the basis of as-yet- unpublished results from early-stage clinical trials, Vicente Verez-Bencomo, director-general of the Finlay Institute, expects the vaccine to show an efficacy in the region of 80–95%. “We are very optimistic”, he said. If everything goes according to plan, Cuba could start a mass vaccination programme for its 11·2 million citizens sometime in the summer. After keeping SARS-CoV-2 at bay for most of 2020, Cuba has experienced a surge of infections in 2021. As of March 8, the country had reported 55 693 cases of COVID-19 and 348 deaths. 23 093 new cases occurred in February alone, almost twice as many as occurred in the whole of 2020. Cuba is still doing far better than the majority of other countries in the region, but a vaccine is urgently needed. A second phase 3 trial of Soberana-2 is planned for Iran, as part of a partner- ship between the Finlay Institute and the Pasteur Institute of Iran. A phase 2/3 trial has been scheduled for Soberana-1, which was also developed by the Finlay Institute. The Center for Genetic Engineering and Biotechnology (Havana, Cuba) is behind the other vaccine candidates. Abdala and Mambisa, a nasal spray, both entered phase 1/2 trials late last year. Soberana means sovereign in Spanish. Abdala is the title of a poem by a Cuban revolutionary, and Mambisa is named after the guerrillas who fought against the Spanish colonialists in the 19th century. All of which indicates that the vaccine drive is a matter of national pride. President Miguel Díaz-Canel has visited the Finlay Institute three times over the course of the COVID-19 pandemic. At home and abroad, post- revolutionary Cuban identity has always been bound up with health. In 1960, Cuba joined the relief effort after the Chilean earthquake. In 1963, it sent health-care workers to assist the newly independent state of Algeria. Cuba’s Henry Reeve Brigade was established in 2005. It has despatched cadres of health-care professionals all over the world to combat disasters and epidemics. Cuban doctors were on the scene in Haiti during the cholera outbreak that followed the 2010 earthquake; they arrived in west Africa during the 2013–16 Ebola crisis. And when COVID-19 spread to Europe, two Henry Reeve teams landed in Italy. By the end of April, 2020, more than 1000 Cuban health-care workers were helping foreign countries respond to COVID-19.
“The international health programme is about solidarity; Cuba believes that healthy populations are the bedrock of global society and they want to support that any way they can”, said Clare Wenham, assistant professor of global health policy at the London School of Economics and Political Science (London, UK). Malaria, polio, tetanus, and measles have been eradicated in Cuba. The island’s successful response to COVID-19 was largely a result of years of investment in primary care and assiduous attention to population health. The country has comprehensive universal health care and one of the highest doctor to patient ratios in the world. Doctor and nurse teams are embedded in the local community. “Everyone has a yearly routine check- up, and if you do not go, the doctor will come and find you”, Wenham told The Lancet Infectious Diseases. “It means physicians proactively identify problems; there is a real emphasis on prevention.” Disease outbreaks can be detected more or less immediately. Under a model known as CARE, patients are stratified into four categories: apparently healthy, at risk of disease, unwell, and in rehabilitation or recovery. Those at risk of disease include individuals who are overweight, have diabetes, or hypertension. When Cuba registered its first case of COVID-19 on March 11, 2020, it already knew the whereabouts of its most vulnerable citizens. In an interview with MEDICC Review, family physician Marta Gálvez outlined the advantages of the Cuban system: “The first thing any self-respecting doctor must know is the health situation of the population she serves”, she explained. “The main goal of a primary care physician is health promotion and prevention of diseases, so you have to know your community to design a strategy that suits their needs. CARE is a vital tool: it’s why I know that I have 658 older adults in a total population of 1093 people, and 42 of the elderly live alone.” Roughly one in five Cubans are over the age of 60 years. “The public health network is very strong in Cuba, but it comes at the cost of civil liberties”, said Wenham. “Cuba is a very specific context; not many countries are going to accept that kind of close medical surveillance, and most governments do not have such tight control over their citizens.” After SARS-CoV-2 entered the island, more than 28 000 medical students led an active screening programme that within a few weeks had reached 9 million Cubans. Cuba had started preparing well in advance of its first case of COVID-19. It quickly shut its borders and set up isolation centres and an efficient system of test-and- trace. But soon after Cuba opened up late last year, cases began to rise. The pandemic has been extremely expensive. Gross domestic product
shrunk by 11% in 2020. Instead of the usual 4 million tourists, Cuba played host to just 80000. The long- standing economic blockade imposed by the USA has taken a heavy toll. “Health centres and clinics face regular stock outs of basic drugs, such as paracetamol, and other equipment such as bandages”, notes Fiona Samuels, senior research fellow and honorary associate professor at the London School of Hygiene & Tropical Medicine (London, UK). “The staff are very well trained, but the health infrastructure is decayed and they often lack the basics to allow them to do their work effectively.” Cuba’s biotechnology industry sprang up in response to the US blockade. It consists of more than 30 research institutions and manufacturers, under the aegis of the state-run conglomerate BioCubaFarma. In the late 1980s, Cuba developed the world’s first meningococcal B vaccine. It produces eight of the ten routinely used vaccines in the country, and sends hundreds of

2021 millions of doses abroad. But obtaining raw materials is a constant struggle, especially in the aftermath of the hardening of the American sanctions during Donald Trump’s presidency. “You have situations where suppliers of important components for our industry for several decades have been obliged to suddenly stop; it makes everything more expensive and complicated, and it is real concern”, said Verez-Bencomo. Tourism brings in a flow of much-needed currency, especially since uban-Americans have been barred from sending remittances, but with the tourists comes the virus. The Cuban Government reckons that more than 70% of current cases of COVID-19 are linked to new arrivals in the country. If Soberana-2 proves successful, Cuba plans to export it at low cost after the national vaccination efforts have finished. The centralised health- care system means the domestic rollout is unlikely to be problematic, although pockets of the island are tricky to access. Verez-Bencomo reckons that by the end of the summer the country will have the capacity to produce 10 million doses of vaccine per month. Cubans are excited about the endeavour. “When we call for volunteers for clinical trials, we always have two or three times as many people as we need coming forward”, said Verez-Bencomo. “On the street, everywhere I go, everyone is asking about the vaccine.” Talha Burki
