PRESIDENT'S MESSAGE Dear Friends and Guests: On behalf of Board of Directors and Staff, I would like to welcome our distinguished guests and friends, specially who traveled from India at the 19th fund raiser dinner of Metropolitan Asian Family Services. ADVOCACY and RESOURCE DEVELOPMENT has been our primary focus to meet our commitments to this dynamic organization and it's noble mission. It gives me great pleasure to share that despite of the tough economy and decreasing state funds we have managed to expand our services to new locations in Naperville, Vernon Hills and Algonquin to make our services more accessible to suburban communities. We will continue to build partnerships with local community groups and seek their support to meet our short term and long term goals and stretch dollars through community collaboration and expand our volunteer base. Our new members have brought and attracted additional linkages to achieve desired outcomes in our fund raising efforts. Our networking has helped to connect with young professionals who are pumping great energy and enthusiasm to the worthy mission of keeping our seniors safe and secure in their homes. We still need required matching funds to keep providing services. We have taken new initiatives of assisted Housing and Medical clinics to meet ever increasing needs of our target population. I welcome you and ask you to contribute to help us meet our share in this uncertain economy. I thank you for joining us today and make this a successful and enjoyable event. Sincerely,

EXECUTIVE DIRECTOR’S MESSAGE Dear Friends & Distinguished guests: I welcome you all at our 19th fundraiser dinner anniversary. I am very excited to have our special guests from India & Chicago. The fiscal year 2011 was a very challenging year, we found ourselves at crossroads when the needs of our aging populations were increasing and the state and federal funding for social services were decreasing. Our journey during the last 19yrs. was filled with struggle and overcoming of challenges with ongoing support from government agencies, community organizations, board of directors and individual donors. We sincerely thank our sponsors and patrons for their continued support at every milestone of our journey. Starting from the modest office in Chicago in 1992, we have consistently expanded our programs and services through identifying the needs of our clients, mobilizing public, private and community resources and creating community-based solutions with effective networking and inter agency collaboration. We are serving our community in 11 counties and 91 cities in Illinois through various programs and services. We have also started community health clinic on an experimental basis by recruiting local physicians to meet our goal of comprehensive and integrated services offered to our community. I urge you to read the report to get acquainted with our programs & services and get involved in whatever capacity you can. Your contributions of time and talents are the blessing to us at any moment and at any location. Call us at the numbers given in this report. “Be generous and you will be prosperous. Help others & you will be helped” Thank you for coming and I hope that you have a great time with us today. Sincerely,

More than ever your organization is needed, providing services from citizenship training and immigration counseling, to employment training and health screenings, to translation and interpretation services. Our 49th Ward is one of the most diverse in Chicago, and our community is the home to many of the people you serve – coming from India, Pakistan, Bangladesh, and Eastern European countries. Every day, their lives are improved through your work. And in turn, my entire ward enjoys greater health and happiness. Your hard work and dedication are much appreciated. Thank you for another year of service to our community! Yours sincerely, Joseph A. Moore Alderman, Ward 49

Executive Director’s Annual Report It gives me a great pleasure to present our agency‟s annual report for the year 2010-11. This year was full of challenges due to the depressed economy and reduction of funding for social service programs. We had to work harder to maintain the existing services for our targeted population. I am happy to report to you that in spite of difficult times, we have expanded our services in the Lake, McHenry and DuPage counties. Senior centers were opened in the Village of Algonquin and Vernon Hills to provide services to the far north and north west suburbs. We have also opened a Medical clinic in Naperville to provide the basic health care services to the community. Historically speaking, MAFS owes its existence to Universal Industries, a Chicago based business, which applied for Home Care Services, which is now known as Universal Metro Asian Services (UMAS) and MAFS was formed to deliver all other social service programs. Metropolitan Asian Family Services (MAFS)/UMAS have come a long way. It was incorporated as a not-for-profit Tax-Exempt 501 © (3) multi-cultural organization in 1993 with a mission to provide comprehensive and integrated services to immigrant population of India, Pakistan, Bangladesh, Nepal, Sri Lankan and East European communities facing linguistic and cultural barriers. MAFS/ UMAS is a multi-cultural and multi ethnic organization and has, at its core, a commitment to educate, counsel and assist and empower immigrants to become productive member of American Society. Starting from Des Plaines office in 1992, our agency has now expanded its services to seven fully equipped senior centers strategically located in Chicago, Niles/Des Plaines, Roselle/Schaumburg, Carol Stream, Naperville, and most recently opened centers in Algonquin and Vernon Hills. These centers are accessible to the targeted population currently serving 91 cities and neighborhoods over eight counties of Cook, Suburban Cook, Lake, DuPage, McHenry, Kane, Kendall and Will in the greater Chicago land. Our staffs are recruited from the targeted population and are highly trained professionals. Our agency maintains confidentiality and respects the privacy of the participants and fully complies with rules and regulations, of State of Illinois. Our programs and services have grown consistently with our mission over the years.

Studies say that seniors stay happy and healthy when they spend their aging years with their families which is specially true for immigrant elderly population…… COMMUNITY CARE PROGRAMS: MAFS/UMAS got its first contract with Illinois Department on Aging to administer Homecare Services to ethnic senior population in 1993 via Universal Industries. This program provides services to the elderly in their home, which allows them to stay in their own home rather than being institutionalized in a nursing facility. These services are provided by our homecare aides who are recruited from the ethnic communities reflecting the client‟s cultural background. These services are available at all our locations. Homecare aides assist the elderly with daily-chores, such as cleaning, cooking, shopping, personal grooming and escorting them to

Executive Director’s Annual Report medical appointments, when necessary. They assist the elderly in their homes anywhere from one to seven days a week, depending on the needs of the senior. Their needs are determined by an independent Case Management Agency who assesses the senior‟s disability to do their daily tasks. These services are ofered with full dignity and confidentiality of the participant. An intensive pre-service & in-service training are mandatory requirements of Illinois Department on Aging. Our Supervisors have to complete Certificate of Achievement training offered by Illinois Department on Aging and they also attend mandatory workshops every year. Our homecare aides receive quality trainings from trainers who have expertise in their field. Examples of topics covered include Alzheimer‟s, Depression, Safety, Nutrition, Hygiene, Physical and Mental Health. Topics related to health issues such as diabetes, heart disease, healthy diet and stress management are also covered in trainings. Home Care program is an anchor program of our agency. MAFS/ UMAFS is a recognized leader and biggest employer providing inhome services to ethnic elderly population. Home Care program offers an opportunity for career development to persons who may not be suitable for main stream jobs due to their language and cultural barriers. The unique feature of this program is mandatory preservice and in-service trainings which assist Homecare Aides in all aspects of preventive eldercare and codes of conduct ensuring client‟s health, safety, and well being. IMPACT OF THIS PROGRAM: This program has an amazing impact on the well-being of the families served. The home care services result in fewer trips to hospitals and emergency rooms saving thousands of dollars of the State. The additional benefit is that elderly can stay in their own homes with their families instead of nursing home placements, which is a tremendous financial burden on the State and Federal government. Moreover there are no ethnic Nursing Homes available for ethnic targeted population to accommodate their cultural, food and linguistic needs. Most of the immigrant seniors are lonely, depressed and suffering with aging complications and are culturally isolated while their family members are at work. Our employment and training services have created an army of multilingual home care workers who provide culturally and linguistically appropriate services to the elderly population we serve. Our trained home aides take better care of themselves & their own families. This will have tremendous ripple effect on the economy and future work force that is required to take care of aging population in America.

Executive Director’s Annual Report “Happy aging is not adding years to life but adding Life to years” ADULT DAY SERVICE [ADS]: Adult Day Care Service program provides opportunity for the elderly to get involved in structured and fun-filled activities. ADS is funded by the Illinois Department on Aging. Its purpose is to assist elderly clients who need care in a safe, secure and home-like environment outside of their home. This program provides breakfast, hot lunch and snacks to the participants, on-site medical monitoring is available by trained nurses, yoga & fitness sessions, health education and screening seminars, re-creational activities and round trip transportation services from their home to the centers by experienced drivers. This program is currently available at Chicago, Niles, Carol Stream and Naperville centers. IMPACT OF THIS PROGRAM: This program is very helpful to family members who have to go to work, therefore unable to look after their aging parents or in-laws. It promotes a healthy life style and allows the seniors to socialize with same age seniors to break their isolation in a home like environment. This program also offers opportunities for physical and mental health well being. It promotes new friendships and reduces depression and social isolation in many cases. These centers also provide an open door policy for seniors as ‘DROP-IN’ facilities for those who want to enjoy the various services offered at the day care center but cannot be approved under the state guidelines. Adult Day Care programs keep seniors happy and healthy through preventive health education seminars and screening services. Seniors are mentally, physically, socially stimulated in a home-like environment. PUBLIC BENEFITS ASSISTANCE PROGRAM: IMMIGRATION AND CITIZENSHIP ASSISTANCE Immigration and citizenship assistance was provided to South Asian immigrant families since1992 from a law office facility in Des Plaines. MAFS began these services in 1993 to ensure that clients fully understand the US immigration process and are able to utilize the immigration services and existing benefits available to them. It provides accurate information, counseling and referrals and full assistance in completing the immigration forms for citizenship and family petitions. MAFS provides assistance to telephone callers and “walk-in” clients that require assistance with applications. MAFS also helps the community members in obtaining OCI Card & VISA from the Indian Consulate and is now working for cancellation of Indian Passport for those who have attained US citizenship. People are expected to call for their OCI card application.

Executive Director’s Annual Report INTERPRETATION & TRANSLATION SERVICES: MAFS is providing interpretation and translation services on behalf of the seniors in Public benefit offices, such as Department of Human Services, Social Security, Immigration and Naturalization services in Chicago and suburbs. Our staff speaks more than 15 languages and they assist the community/seniors with their applications for citizenship and any interpretation services whenever required. Staff translate English forms into Hindi, Gujarati, Urdu and Bosnian languages for the State of Illinois and other Agencies. Funding comes from the Coalition of Limited English Speaking Elderly (“CLESE”) for translation services. SOCIAL SERVICES AND OUTREACH: TITLE III (Old American Act) Seniors with linguistic barriers usually do not have easy access to services provided by state and federal agencies. This program is funded by Age Options and is provided at Suburban Cook County by Social Service Specialists who provide information and assistance about the benefits available to the elderly. MAFS provides similar services at other counties, even though we are not funded. The demands for these services are consistently increasing each year at our targeted communities. Our staff assists clients to fill out applications for the appropriate services and escort them to the public benefit offices such as Social Security Administration, Immigration and Naturalization, and Department of Human Services. Interpretation services for limited English speaking seniors are also provided as needed. The social workers link them with various service networks and help them to receive benefits available to them. The daily callers and visitors also get assistance without appointments. Government forms in Hindi, Urdu, and Gujarati and Bosnian language are also available to facilitate the application process . ENERGY ASSISTANCE PROGRAM (LIHEAP) The Light & Heat Energy Assistance Program (LIHEAP), funded by CEDA, is designed to help eligible low-income households to pay for their light and heat bills. Clients are assisted in processing their applications for this program to help them receive the funds. They are guided to bring necessary documents to complete their applications. Thousands of eligible elderly have taken advantage of this program every year at Chicago and Niles locations. NUTRITION PROGRAMS: Most seniors miss their traditional ethnic food from their homeland when there is nobody at home to cook for them and serve a hot meal during the day time. This program provides 50% of their daily nutritional requirement. Meals are prepared daily at our facility by trained staff who have passed the State Food and Sanitation licensing exam. The purpose of the con-

Executive Director’s Annual Report gregate meal program is to provide nutritionally balanced ethnic hot meals in a group setting to break the isolation and to increase socialization. These meals are hygienically prepared and served at our centers. This program has been very successful, seniors are able to enjoy meals in a home like environment and develop new friendships. The meals are served from 12 pm to 2:30p.m. GOLDEN DINERS PROGRAM (CHICAGO) MAFS started the Golden Diners program on October 1, 2001 in its main office with a fully equipped kitchen. Hot meals are prepared by trained cooks in strictly hygienic conditions. This program is in its 11th year of successful operation and is funded by Chicago Department of Family and Support Services. We have also added a Box-Dinner program for the last two years to fulfill the need of seniors who live alone and are unable to cook for themselves. These programs are an effort to break the isolation of seniors and to solve the malnutrition problem faced by ethnic seniors. Over 150,000 meals were served at this location. SUBURBAN HOT MEAL PROGRAM In 2003, MAFS opened its second congregate meal program at the Niles/Des Plaines center, with the funding from the Age Option (Suburban Area Agency on Aging). A specially-equipped commercial kitchen was added to the senior center to prepare meals on-site. We have served over 100,000 meals over these years and have given 10,000 lunch-boxes for ARRA program in the last fiscal year. This program is funded by Age-Option. DUPAGE SENIOR COUNCIL HOT MEAL PROGRAM In 2006, MAFS opened its third congregate meal program at the Roselle, Lombard and later in Carol Stream centers with funding from Northeast Area Agency on Aging/DuPage Senior Citizen Council. These meals were provided three days a week to fulfill the need of seniors. Currently the food is being prepared at the newly constructed kitchen in Naperville. Recently Lombard center was merged in Naperville and Carol Stream is in Roselle center.

“An ounce of prevention is worth more than a pound of cure” Health Care programs: Heath Promotion & Disease prevention program-TITLE III D The purpose of these programs is to educate and encourage seniors and their families to adopt healthy life styles to stay healthy. This goal is achieved by providing information and necessary tools to the elderly and their family members who learn preventive health care strategies through educational seminars and screening.

Executive Director’s Annual Report HEALTH PROMOTION & DISEASE PREVENTION SEMINARS MAFS conducts health screenings and health education seminars in Chicago, Niles, Schaumburg, Arlington Heights, Hoffman Estates and the Village of Palatine. Health seminars are made possible by funding from Age Options. MAFS is working with various healthcare organizations to promote health awareness among the South Asian Community. Free seminars and screenings cover topics such as: Healing with Nutrition, Bone Density, Cholesterol check-ups, Chronic disease prevention, Cancer, Aging Eyes, Kidney & Diabetes, Loss Matters, Depression, Fall Prevention and Medication Management. Flu Shots are administered in the Fall by networking and collaborating with existing community Health Care providers. Following organizations collaborate with us in these programs: · White Crane Wellness Center · UIC Collage of Pharmacy · Asian Health Coalition · Northwestern University · Coalition of Limited English Speaking Elderly (CLESE) · Arthritis Foundation

If the cost of education is expensive, think about the cost of ignorance…... EDUCATIONAL PROGRAMS Educational programs are designed to improve the knowledge and technical skills of targeted population and community members to become productive members of our community and build their communication skills to better function in the American society. MAFS has been providing ESL classes since its inception 19 years ago as a tool to prepare immigrants for citizenship examination. Following programs have been developed & implemented over the years. ENGLISH AS SECOND LANGUAGE [ESL: Bright Ideas] Bright Ideas is a program offered by CLESE to low-literate seniors since 2004. The curriculum focuses on practical speaking and listening skills. The class meets at the MAFS Roselle Senior Center two days a week. In the past year, we have trained new seniors in English speaking skills for daily life, thus, making them more functional in the American society. We also provide citizenship classes to seniors at Chicago, Niles, Roselle and Naperville to help seniors pass the US citizenship test. COMPUTER TRAINING The agency has continued to offer computer training to its immigrants since 1997. We provide computer training at our centers in

Executive Director’s Annual Report Chicago, Niles and Roselle. Funding received from the Retirement Research Foundation enabled us to upgrade our computers and improve our training from 1997 –1999. Currently we have no grant to support the program but we are continuing the program due to the community‟s need for education. Currently we have a computer training program at Chicago and Niles centers. We take a nominal donation of $35 for learning basic computer skills for 3 months. We have also introduced numerous advanced courses in computer learning, such as computer languages, web designing, web development, graphics, and even certification, all for a nominal charge. Our agency is trying to find new source of funding in order to continue and provide computer literacy classes for job seekers. Future Plans We are planning to launch an assisted living housing facility for our aging seniors and senior Home assisted living project. At this time, this project is our top priority. Currently, we are doing research on efficient and successful models. It's truly a huge multi-dimensional endeavor which needs public, private and community support to be accomplished. We are also planning to develop Adult Day Care programs in the Vernon Hills center in Lake County and Algonquin center in Mchenry County. In December 2010, we started a Community Health Clinic in Naperville on an experimental basis by recruiting local physicians to meet our goal of comprehensive and integrated health care services for our target population and we are now planning to start the same service at our Chicago location. At the end, the cost of providing and maintaining the quality of social services comes from many sources. The reduction in funding for social services has added extra burden on our organization as we continue to serve our ethnic seniors. As we are forced to generate matching funding for our existing programs, we are looking for monetary donations from the community members and business organizations. Therefore, we hope you will extend your whole –hearted support to our organization and help us continue much needed services to the ethnic elderly. “Be generous and you will be prosperous.
