The Drug Store Project is in its 9th year and is an event where the community of Atwater comes together to sponsor what could be the largest collaborative prevention activity that continues to be conducted in our area.

Humankindness Retreats’ Generosity Store The complexes on the EKG tracings are created by electrical activity within the heart muscle. However, it is possible for things other than cardiac activity to interfere with the tracing. Some common causes of interference (aka: artifact): muscle tremors, shivering, patient movement, loose electrode or electrical equipment/60 cycle interference. These deflections are considered to be artifact and important to recognize and interpret them as such rather than arrhythmias. Winner of the last rhythm: Ofelia Aquino, RN

the Dignity Health Way was implemented at our sister hospital in Mark Twain. She has also been a great support to our Quality Department, building the mini MOR to report our core measures effectively and providing meaningful use tools to our staff. She also helped the QM director build a template to use for QMC. She has been pulled in many different directions to help build stories with our data, such as our ER and OR team to find improvements despite what time of the day or night the request is received. She always makes time to get the work done. On top of her PE role, she is completing her bachelors degree in the summer of 2016. She will also be in the next cohort starting this month in the Six Sigma Black Belt Program through the Dignity Health Academy.

costs associated with transfusion complications. Leukocyte reduction is clinically effective in reducing the incidence of transfusion-related complications, including post-operative infections. Pall filters are used to prevent febrile and non-hemolytic transfusion reactions due to donor white cell cytokines released during blood storage. MMC Policy PC 78 - Blood or Blood Products addresses when a Pall Filter should routinely be used: PALL FILTER (SQ 40S – orange in color and rectangular in shape), a PHYSICIAN ORDER IS NOT REQUIRED to use the filter. Pall filters are NOT to be USED for general blood transfusions but should be used in the following situations: 1. After two or more units of transfused blood and/or where long term hemotherapy is anticipated. 2. Chemotherapy - patients who require multiple transfusions. 3. Hemodialysis when receiving blood other than during dialysis treatment. 4. Difficult cross-match – check for the Blood Source Red Blood Cell Antigen Typing indicator tag on unit of blood. Antigen Typing indicators tags (tan in color) are attached to the unit with a rubberband.
PigTail Catheters (AKA multipurpose drainage catheter by Cook Medical) – inserted under local anesthesia with ultrasound guidance to treat Malignant Pleural Effusion. Malignant Pleural Effusion is a Proximal and distal drainage catheter condition in which an abnormal amount of fluid collects between the parietal and visceral pleura in the chest cavity. Lung and breast cancer account for about 50-65% of malignant pleural effusions. Pigtail Catheter Insertion Kits contain: the PIGTAIL catheter, a three way stop cock, a short drainage connection tube and a 2000 ml collection bag. Once inserted, the Pigtail is connected to the 3 way stopcock, which is connected to the collection bag by the collection tubing. Generally, after the collection bag has approximately 1500 ml of pleural fluid it is removed from the system and sent to the laboratory for cytology examination. The nurse now needs to make certain that the three way stopcock on the Pig tail catheter is NOT REMOVED/ DISCARDED. To continue collection of drainage the nurse will setup the following: pigtail is connected to 3 way stopcock, which is connected to new drainage tubing (Boston Scientific CE 0197 available only from Ultrasound) to which a sterile Foley drainage bag is attached. Of concern is that the staff have been discarding the stopcock on the pigtail not realizing that the stopcock may be needed for the physician to instill medication. If the stopcock is accidentally discarded obtain another from the medication room so the system can be set up appropriately. Thanks to Nic Paskin RN 7th floor for bringing this to our attention.
Pall filters are found in the Warehouse and can be obtained after hours by the Administrative House Supervisor. Per Corry Yokley – Distribution Supervisor – Pall filters are being added to the Medication Room stock supplies.
