If you are confused by the controversy surrounding cholesterol, you are not alone. You may have recently heard that dietary cholesterol doesn’t seem to impact serum cholesterol. In other words, the cholesterol you eat may not have an impact on your cholesterol levels! Now, everyone’s confused…and experts are no longer in agreement. Basically, the waters have been muddied by 1) research that was not representative of “typical American eating patterns” and 2) a change in the Dietary Guidelines for Americans.

So, what now?

Here are some of the most important things for you to know: Cholesterol is a waxy, fat-like substance found in all of our bodies’ cells. It is used to make things like hormones and Vitamin D. Since our bodies make adequate amounts of cholesterol, there is no need to obtain cholesterol from food. However, our favorite animal foods to eat also produce their own cholesterol, resulting in our consumption of cholesterol. The 2015 edition of the Dietary Guidelines for Americans did not include a dietary recommendation on cholesterol as it did in the past. The key recommendation from the 2010 Dietary Guidelines to limit consumption of dietary cholesterol to 300 mg per day is not included in the 2015 edition, but this change does not suggest that dietary cholesterol is no longer important to consider when building healthy eating patterns. It is recommended that individuals should eat as little dietary cholesterol as possible while following a healthy eating pattern. In general, foods that are higher in dietary cholesterol, such as fatty meats and high-fat dairy products, are also higher in saturated fats. Experts cannot agree as to whether the cholesterol itself raises serum cholesterol or if it’s the saturated fats that accompany cholesterol that raise serum cholesterol levels. However, saturated fats and more importantly, trans fats, are known key components to an increased risk of cardiovascular disease. It is safe to say that more research is needed regarding the relationship between dietary cholesterol and blood cholesterol levels. Adequate evidence is not available for a quantitative limit for dietary cholesterol specific to the Dietary Guidelines. Dietary cholesterol is found only in animal foods such as egg yolks, dairy products, shellfish, meats and poultry. A few foods, notably egg yolks and some shellfish, are higher in dietary cholesterol but not saturated fats. Eggs and shellfish can be consumed along with a variety of other choices within and across the subgroup recommendations of the protein foods group.

Useful Care Tips Every Care Provider Should Know

Dementia is not a specific disease. Many dementias are progressive, meaning they start out slowly and gradually get worse. The National Health Institute estimates that 3.4 million Americans age 71 and older – 1 in 7 Americans in that age group – have dementia. These facts make it apparent that all senior care providers will at one time or another work with and provide care for patients suffering from dementia. Caring for seniors with this disease can be difficult, frustrating and stressful. It is important to know what works and what doesn’t work when addressing a resident who suffers from any stage of dementia. NEW RULES FORCING CHANGE The new Requirements of Participation that the Centers for Medicare and Medicaid Services released in the fall of 2016 are forcing changes on the healthcare industry, specifically with dementia residents. While delivering quality of care is a goal that everyone strives to accomplish, some of the regulations now force care providers to prove they are putting residents first. A brand new section added to these regulations is the requirement to provide necessary “Behavioral Health.” This requirement states: 483.40 Behavioral Health “The facility must have sufficient staff who provide direct services to residents with the appropriate competencies and skills sets to provide nursing and related services to assure resident safety and attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident, as determined by resident assessments and individual plans of care...” This regulation includes implementing non-pharmacological interventions and giving the appropriate treatment for any resident who “displays” or is diagnosed with dementia. How can you help your staff accurately diagnose behaviors, determine appropriate treatment and identify alternatives to drugs as a means to deal with dementia residents? TRAINING! Building caregivers’ skills not only gives them confidence, it also helps with the quality of life you are providing for your dementia residents. These 5 quick tips can make a significant difference in the way your staff approaches residents suffering from dementia and will also assist in providing more positive outcomes in their interactions.

1. DON’T QUIZ SENIORS WITH MEMORY LOSS Imagine how upsetting it would be to not remember basic information you have known your whole life. Irritating, right? Now, imagine how it would feel to constantly be asked questions to test your ability to remember. It would be like throwing gas on a fire. Instead of asking a resident “Do you remember her name?” or “Can you tell me what you had for lunch?”, try a different approach. What about “Oh, look over there. It’s Anna.” or “The pasta at lunch today looked really good.”? Being helpful rather than potentially causing frustration can go a long way with your residents. 2. DON’T TALK TO DEMENTIA RESIDENTS LIKE THEY ARE CHILDREN No adult wants to have someone approach him or her and talk in a song-like tone with child-like phrases. Getting really close and invading personal space only compounds the situation. Even when seniors are struggling with memory, they can become very irritable and upset when they are treated like children, and understandably so. 3. CHOOSE DIGNITY OVER DISRESPECT Just because a patient suffers from dementia doesn’t mean that he or she has lost the ability to hear. Talking about a resident while he or she is sitting right there is not only disrespectful but it can also be embarrassing and upsetting to the resident. If you need to address a matter regarding your resident with another staff member, find a time to do it in private. 4. NOT EVERY DAY WILL BE THE SAME Dementia can be very misleading. One day a resident may not be able to recall anything, and the next day he or she may remember specific information. Approach every day as a new day with new opportunities. Some days are sure to surprise you. 5. THINK BEFORE YOU ACT When it comes to providing residents with the best possible care, the best thing you can do is try to put yourself in their shoes and ask yourself “How would I like to be treated?” If you are always thinking about your residents’ needs first and using careful consideration when you interact with them, you can’t go wrong.
