Hui No Ke Ola Pono offers free checks for breast, cervical cancer omen who don’t have insurance or who are underinsured can find comfort in knowing they have Hui No Ke Ola Pono to turn to for important health screenings. The Women’s Health Program at the nonprofit health center based in Wailuku offers free cervical cancer screenings and mammograms to those who qualify. The program aims to complete the screenings in a culturally sensitive setting, providing education on the importance of these preventive steps and information on how to do self breast exams, among other services. Dr. Heather Haynes, the organization’s medical director, said screenings for cervical and breast cancer are vital, as these cancers can be caught before symptoms arise, giving patients a better chance of survival. “Once people find out about the program, they’re thrilled that the service is there for them,” she said. In addition to the Women’s Health Program, the organization focuses on overall health enhancement and disease prevention and also provides primary care services at its general clinic. According to the organization’s website, staff members work within a standard of conduct that reflects Hawaiian values. The organization is one of five Native Hawaiian Health Care Systems created under the Native Hawaiian Health Care Act of 1988 and was reauthorized under the Native Hawaiian Health Care Improvement Act of 1992. Along with educational opportunities, Hui No Ke Ola Pono offers health monitoring and wellness classes, and Simply Healthy Cafe is under its auspices. The free annual health screenings for women are made available through the state Department of Health and the Breast and Cervical Cancer Control Prevention program. Haynes said that, thanks to more women gaining insurance through the federal Affordable Care Act, fewer are hav-

Treating the treatment: Physical therapy can help after cancer id you know that there are expected to be 1,658,370 new cancer cases in the United States in 2015? Among those, 231,840 will be female breast cancer cases, and of those, 1,140 will be in Hawaii, according to information at www.cancer.org. If this is hitting home, just know that you’re not alone. There are many side effects that can arise from the treatment of cancer. You probably think it’s pretty common to have physical therapy after a rotator cuff or an ACL (anterior cruciate ligament) surgery, right? It is just as important to seek out specialized physical therapists who treat the type of injuries that are common to the lymphatic system when the system is overwhelmed by cancer treatment. The body goes through the same type of healing stages, and early intervention and treatment can help decrease swelling and assist the lymph system to begin its own healing, even as soon as a few weeks post surgery. Women may go through so many different treatments to get rid of the cancer, such as surgery, radiation therapy, chemotherapy, hormone therapy, targeted therapy and bonedirected therapy. Most women with breast cancer usually have some type of surgery, either a lumpectomy or a mastectomy. Sometimes, lymph nodes need to be removed as part of this surgery, depending on how far the cancer has spread. Normally, women have 30 to 40 lymph nodes in their axilla, or armpit area. Some options for checking the nodes to see if they have been invaded by cancer include a sentinel lymph node biopsy and an axillary (armpit) lymph

the usual healing process. Early treatment also assists the lymph system to begin its own healing. There is no better time to optimize the healing of both the tissues and the lymphatic system than during the early stages of recovery. Another treatment that may be used, usually in combination with the other treatments, is radiation therapy. Radiation therapy is treatment with high-energy rays or particles that destroy cancer cells. Radiation to the

breast is often given after breast-conserving surgery to help lower the chance that the cancer will come back in the breast or nearby lymph nodes. During radiation therapy, there is often tissue trauma, as well as inflammation of the tissues. Local inflammation eventually leads to the formation of scar tissue, which can then cause feelings of tissue tightness and

node dissection. The surgeon will then take out any nodes that have cancer spread to them, and sometimes more depending on what the surgeon believes is necessary. When lymph nodes need to be taken out, the lymph system may not be able to function as well as it did before. Because the lymph system has a vital role in the defense of the body’s immunity, this can affect the normal healing processes in the area of surgery. Early intervention and treatment can help reduce the local inflammation that often slows down

In the arms and upper body, this can lead to range-of-motion problems and postural dysfunction. If a patient has radiation to the lower body, this can lead to rangeof-motion difficulties, as well as functional difficulties with walking, running, balance, etc. Physical therapy can start even in the early stages of radiation. This will help reduce inflammation and in turn, lessen scar tissue fibrosis. Another significant side effect of cancer or cancer treatment is lymphedema. Everyone who has had cancer treatment should know what it is, and know what they can do to minimize the risk of getting lymphedema. Initially, the lymph system can be affected when part of it is damaged or blocked, such as dur-

ing surgery to remove lymph nodes or radiation therapy. Cancers that block lymph vessels can also cause lymphedema. A challenge to the cancer survivor is that lymphedema may not appear right away, but at times, even years after treatment. Lymphedema is defined as an abnormal collection of high-protein fluid called lymph, which is made up of white blood cells, especially lymphocytes, which can attack bacteria in the blood. Lymphedema usually is first seen as mild swelling that starts anywhere in an arm, breast or chest wall (if the patient has had breast cancer) and can progress to severe swelling if untreated. The gold standard of treatment for lymphedema is therapy with a certified lymphedema therapist who is skilled in teaching the patient how to improve and decrease the swelling and give them strategies to minimize the risk of swelling returning. Chemotherapy is another treatment with

cancer-killing drugs that may be given intravenously (injected into a vein) or by mouth. The drugs travel through the bloodstream to reach cancer cells in most parts of the body. Chemo is given in cycles, with each period of treatment followed by a recovery period. Sometimes, it can be given before surgery. To be most effective, more than one drug is usually used. Physical therapy can help with the effects of chemo-toxicity. Chemotherapy affects the body in so many ways, including decreasing the quality of your heart’s contractions, its oxygen and nutrient delivery to tissues. It causes breakdown and atrophy of muscle tissue leading to weakness. It decreases your coordination and motor function. It also decreases your total lung capacity and your intake of oxygen and removal of carbon dioxide. It can affect the blood by causing anemia and making the heart work harder due to decreased hemoglobin. Exercise can counteract all these problems. It increases contractility of the heart and also increases oxygen and nutrient delivery to the tissues. It can increase protein synthesis to help rebuild tissues, and it stimulates the release of hormones that can increase

Yoga for mind, body and belly Gentle poses can relieve discomforts of pregnancy, ease day-to-day ailments regnancy is a time of great excitement and anticipation, but unfortunately for most women, it is also a time of terrible discomfort and potential disease. It’s easy to give in to lethargy and just let go, but “infanticipation” is no excuse not to stay fit. For one, exercise helps you maintain your weight even as your belly grows. It also helps you to strengthen your muscles, especially your back and legs, which most certainly take a beating during childbearing and birth. While lifting weights and other rigorous activities like aerobics and running are not advised, there are other lowimpact exercises you can

choose from: walking, swimming and yoga. Yoga provides many benefits for women in general, not only easing the way for those who are pregnant but also assisting with day-to-day issues. For pregnancy Yoga is most especially favorable for pregnant women not only because of the physical benefits one can get from it, but also because of the mental advantages it offers. Breathing and relaxation are two key points to master for childbirth, and these are principles taught in the practice of yoga. (They will come in handy, too, all throughout motherhood.) During labor, while in

pain, the body has the tendency to shut and tighten, which may delay the birthing process and stress both mother and baby. Learning the breathing techniques of yoga and doing them properly will ensure that you can ward off whatever fear you have of pain or of childbirth itself. The stretches and movements of yoga also tone and strengthen your body, and if you’re a first-timer, you should have no problem with the activity, as headstands and other inverted positions are turf of the advanced student. If you’ve long been doing yoga and are capable of such movements, know that pregnancy is a time to modify your routine. Steer clear of

poses that may lead to an accident — no sudden, jerking movements. You should also avoid poses that require laying on the back or belly. Yoga can also minimize common pregnancy symptoms like morning sickness and constipation, and can help ensure easier labor and smooth delivery by relieving tension around the cervix and birth canal and by opening the pelvis. After childbirth, yoga can help in restoring your body shape, uterus, abdomen and pelvic floor, and in relieving upper back tension and breast discomfort.
