Insert requirements available from publisher, and a sample of the insert must be sent to the publisher for approval. • Reprints or polybagging with issue — Rates available upon request. • Preferred positions — Preferred positions are granted upon availability, at the approval of the publisher. • All ad insertions confirmed by email are final unless written cancellation received 14 days before printing.
Billing & Payment Procedures • Invoices are rendered with tearsheets from each issue as published. Invoices are mailed on the issue’s distribution date and are payable upon receipt. • Payment — All payments are due in full within 30 days of billing. We accept all major credit cards. A service charge in the amount of 1.5% per month will be charged on the outstanding balance of all past due accounts. Please adjust your payment schedule to comply, as our service charge policy will be enforced. • If charges due publisher from agency for ads placed pursuant to the rate card are not paid promptly, the publisher may, at his option, collect said charges from advertiser, and agency shall execute all necessary assignments.

General Conditions: No cancellations or changes in orders after space reservation date. No cancellations or changes will be considered in effect unless written confirmation is received and acknowledged by the publisher. If an advertiser cancels its advertising schedule prior to completion of the schedule, the advertiser will be billed a short-rate as an adjustment for the number of insertions that actually ran before written cancellation was received and acknowledged by the publisher. If new copy instructions are not received by the copy deadline, the publisher reserves the right to repeat a previous advertisement or other discretionary copy of the same size, and the advertiser or
agency will be liable for the space cost at the original contracted rates. Advertisements are accepted upon the representation that advertiser and its agency have the right to publish the contents thereof. In consideration of such publication, advertiser and its agency agree to indemnify and hold publisher harmless against any expenses or loss by reason of any claim arising out of publication. Conditions, other than rates, are subject to change by publisher without notice. The publisher reserves the right to insert the word “Advertisement” above or below any copy that may be misconstrued as editorial material. Positioning of advertisements is at the discretion of the publisher except where
a request for a specific preferred position is acknowledged by the publisher in writing. The publisher shall have no liability for errors in key numbers, the Reader Service Section, or the Advertiser’s Directory. The publisher shall not be liable for any costs or damages for failure to publish an advertisement. No conditions other than those set forth in this rate card shall be binding on the publisher unless specifically agreed to in writing by the publisher. The publisher will not be bound by conditions printed or appearing on insertion orders or copy instructions which conflict with provisions in this rate card. As used in this section entitled “General Conditions,” the term “publisher” shall refer to MAG, Inc.
