It’s summertime, which for most of us, means fun outdoor activities in the sun. If you’re bothered by allergies, however, most likely your sunny days are marred by watery eyes, sinus congestion, runny nose, sneezing, coughing, headaches, sore throats, or itchy skin. Fortunately, you don’t have to continue to suffer from these irritating symptoms. A qualified allergy doctor can discover what you’re allergic to, and offer treatments that provide easy relief.

WHAT CAUSES ALLERGIES? Allergic ailments are caused by an «overreaction» of the immune system to various environmental substances. The immune system perceives some elements of the environment as a threat to the body and tries to «defend» against them. The major form of defense is an inflammatory response, which is why clinical symptoms of allergic ailments and infectious illnesses are very similar and tend to overlap.

Allergens can be divided into two large groups: 1) those that we encounter outside our homes, such as pollen from various plants, and 2) those that are present inside our homes, such as dust or mildew. Different plants produce pollens at particular times of the year. As a result, people sensitive to tree pollen will experience an “acute exacerbation,” or flare-up, of symptoms when trees blossom. In the New York area, this period stems from April until early June. Likewise, people with hypersensitivity to autumn grasses, such as wormwood or ragweed, will begin to feel allergic symptoms in the fall, between August and October. Very often, individuals are sensitive to several allergens, and thus the duration of their symptoms is considerably longer. This pertains particularly to people who are sensitive to allergens that are present inside houses and apartments, ranging from domestic dust and mold (fungi), to hair and saliva of various animals (so-called indoor allergens). Such people may show continuous allergic symptoms all year long. Symptoms may include headaches, sore and burning throat, itching and irritation of the eyes, and coughing. People who experience mild continuous symptoms due to household allergies

often fall victim to flare-ups in the spring and fall, as well. Allergic diseases are not limited to symptoms connected with upper airway passages, however. Other prevalent allergic ailments include bronchial asthma (with symptoms of wheezing, shortness of breath, chest tightness, and/or coughing), and asthmatic bronchitis. Of those who suffer from bronchial asthma, 50% of adults and 80% of children have allergies.

IDENTIFYING TRIGGERS WITH SKIN TESTS The most common method of identifying allergens to which an individual is sensitive through various types of skin tests. Conducted by doctors who are allergy specialists, these tests are safe and take a minimal amount of time. Results are usually available within fifteen minutes. Knowing the substances which a person may be sensitive to enables the allergist to use effective preventative measures and treat allergic diseases. Preventative measures and treatments, in general, are carried out using a combination of different medications selected by a doctor to meet the particular needs of each patient.

TREATMENTS CAN HELP YOU FEEL BETTER For those who suffer from allergy symptoms three months or more throughout the year, and want to be cured permanently, doctors might recommend injections of allergy provoking substances (such as tree pollen, animal hair, or mold), which are highly diluted in water. Patients following this therapy become “immunized,” or desensitized, to the allergens. This method is called immunotherapy, or «allergy shots.» Numerous studies have shown that the success of immunotherapy exceeds 80%. If you suspect you have allergies, you can make an appointment with a board-certified allergist, and, in many cases, walk out of the office with the mystery of your ailments instantly solved. There may be things you are exposed to that you can simply avoid. An allergist will often suggest how you can make changes in your home and work environment, so that symptoms are relieved. Whooping cough is a kids’ disease, right? It may seem so, especially if you’ve seen the advertisements warning parents to get vaccinations for their children. Whooping cough is particularly dangerous for infants, as it can cause death in small, fragile bodies. But anyone can get it, which means that adults are at risk, as well. If you haven’t had a whooping cough booster within the last ten years, here’s why you should talk to your doctor about it right away.

WHAT IS WHOOPING COUGH? Whooping cough is a respiratory infection caused by Bordetella pertussis bacteria. Also called “pertussis,” the infection is highly contagious, and can cause severe coughing fits that make it difficult to breathe. The bacteria actually produce toxins that paralyze respiratory cells, which causes the immune system to react and creates inflammation in the lungs. It starts out sort of like a cold or flu, with symptoms like sneezing, a runny nose, a mild cough, and a mild fever—all symptoms you’ve probably experienced before. But gradually, things get worse, until the cough becomes more severe, and you start to suffer “fits” where you cough and cough and can’t stop. In between, in an effort to breathe, you suck in air, creating a “whooping” sound that characterizes the infection. You can see how these symptoms would be very unpleasant even for adults. Children can

suffer even more, actually turning blue during a coughing fit because of an inability to get enough oxygen into the system. Other symptoms may include vomiting, exhaustion, and apnea (a pause in breathing). Some adults will get over it on their own, after about 7-10 days, but others, and some children, will suffer with it for weeks to months. In China, the disease is known as the “100-day cough.”

DISEASE SHOWING UP AGAIN Before the whooping cough vaccine, the pertussis bacteria caused a large number of deaths. After the vaccines came out in the 1940s, the disease was reduced more than 157-fold. Unfortunately, it’s not gone, and has been renewing its attack in recent years in certain parts of the country. In 2012, nearly 50,000 cases of whooping clothes were reported in the U.S.—the most since 1955. Twenty died because of the infection. Fifteen of those were infants who were too young to receive the vaccine. This is why doctors advise parents to be vaccinated, so they don’t pass the infection onto their vulnerable children. In June 2014, Mercury News reported, “California’s growing whooping cough epidemic has claimed the life of a third infant.” The baby was only a year old. The number of cases for the year numbered 4,558 at the time—almost double the number for the entire year of 2013. Once a person has contracted the infection, treatment is usually limited to support-

ive care. Antibiotics may be used to remove the bacteria from the respiratory tract, but it usually doesn’t shorten the duration of the disease. Infants are at risk for complications like pneumonia, seizures, ear infections, and dehydration. Adults may experience coughing fits so severe they fracture a rib.

RECOMMENDATIONS FOR VACCINATION Current recommendations for whooping cough vaccinations are: ■ Children should receive the diphtheria, tetanus, and pertussis (DTaP) vaccination at 2, 4, 6, and 15 through 18 months and 4 through 6 years. ■ Adolescents should receive the tetanusdiphtheria-pertussis (Tdap) between 11 and 18 years of age (preferably at age 11-12 years). ■ Children who weren’t fully immunized against pertussis should receive the Tdap vaccine between 7 and 10 years of age. ■ Adults 19 years and older who didn’t get the Tdap booster should get one right away. ■ Pregnant women should receive a Tdap booster during each pregnancy. ■ Adults 65 and older should receive the Boostrix (GSK) vaccine or the Tdap. If you haven’t received a whooping cough booster, or you’re not sure when you last received it, check with your doctor. A simple shot could save you from having to suffer the symptoms of infection, and could protect the children in your life as well. www.4health.net ww.4h w.

ARE YOU USING THE WRONG TOOTHBRUSH? SOME BRUSHES MAY CAUSE SENSITIVITY AND BLEEDING ■■■ Gordon Barclayy Buying a toothbrush is no big deal, right? You go to the store and you pick one off the shelf. Done. Except if you do it that way, you could be causing yourself problems. The bristles could be too hard, creating sensitivity. If the head is too big for your mouth, you could be leaving food particles behind to cause plaque buildup and cavities. If you get one that you just don’t like, you may be less motivated to brush—increasing your risk of gum disease down the road. You don’t need a Ph.D. in toothbrush design to pick up the right brush for you, but you can give yourself a better, more efficient brushing experience if you know a few things.

WHAT TO LOOK FOR IN YOUR NEW TOOTHBRUSH Believe if or not, you could be using the wrong toothbrush. Ask yourself—do your teeth feel clean? Do you like to brush? Are you having good checkups with the dentist? Do you enjoy comfortable, strong gums with no sensitivity? If you answered “no” to any of these questions, you may have the wrong toothbrush for your mouth. It’s time to go shopping. Before

you part with your hard-earned cash for a brand new brush, consider the following: Bristles: Most brushes offer soft, medium, and hard bristles. If you have sensitive teeth or gums—cold drinks and sugary items hurt— choose the softest bristles you can find. A lot of dentists advise soft bristles no matter who you are, because they reduce risk of sensitivity. They also bend easier, so are more likely to get in between teeth for more thorough cleaning. A 2011 study published in the Journal of the International Academy of Periodontology found that those who brushed with stiffer bristles had an 11 percent increase in gum bleeding after eight weeks. Size: How big is your mouth? You may not have thought about it before, but it’s important. If your brush is too big, it’s going to miss those tight spots between teeth and in the back. Imaging the space between your cheek and your back teeth. Will the brush fit easily there? Use that as a gauge when choosing the size of the head. Manual or power: Used to be a brush was a brush, but not anymore. Now you have a choice between a manual or powered brush, and if you choose a powered brush, you have even more choices. Does it matter? There have been many studies on the subject, and so far,

the consensus seems to be that both are good for cleaning, but that rotating/oscillating electric toothbrushes may be better at getting to those hard-to-reach spots. The difference is small, however, so the important thing is to get what you’re most likely to enjoy using. Manual brushes are less expensive, easier to travel with, more flexible in the hand, and potentially better for those with sensitivity (since you can apply less pressure). Electric toothbrushes may do better at cleaning some areas of the mouth, can be fun to use, and may also be ideal for those with arthritis or other conditions that make brushing difficult. Handle: This is all about personal preference—do you like a more padded handle, or just a standard, smooth one? This may seem a small thing to think about, but again, it’s about making the brushing experience pleasant so that you’ll do it more often. Choose a handle that feels good in your hand and that you can easily move around. Should you go shopping for a new toothbrush? If yours is older than four months, the answer is yes. If you’re experiencing sensitivity, bleeding, or other dental problems, you probably also need to find a new model. If you have diabetes, you probably already know that losing weight can help. Shedding a few pounds can help lower your blood sugar, improve your heart health, and reduce your risk of other complications like kidney disease, depression, and eye problems. It’s not so easy, however, to succeed at weight loss, particularly when you have type 2 diabetes. Studies have shown that even when patients are put into diet and exercise programs, their weight loss is modest. Blood sugar variations, sleep problems, medications, thyroid problems, and more can all make weight loss difficult. A recent study suggests another alternative—gastric bypass surgery. Those with type 2 diabetes who underwent the surgery reported a higher quality of life afterwards. Might this be a healthy option for you?

WHAT IS GASTRIC BYPASS SURGERY? During gastric bypass surgery, doctors reduce the size of the stomach so that patients can’t eat as much as they usually do. They do this by stapling off a large part of the stomach so that it is no longer available to store and digest food. What is left is a small pouch that can handle only a few ounces at a time. That pouch is then connected to the small intestine. When the surgery is com-

pleted, the stomach is reduced by as much as 90 percent. Patients who undergo gastric bypass usually experience dramatic weight loss. Does it last? A 2013 study looked at a similar, but different surgery—gastric banding (where a band clamps off part of the stomach). Researchers found that all patients experienced a mean of 47 percent excessive weight loss after 15 years. Another 2013 study found that bariatric surgery led to greater weight loss and better blood sugar control than nonsurgical treatment among diabetes patients who were mildly obese, though evidence for lasting weight loss was limited. Now, a new 2014 study indicates that gastric bypass may not only produce weight loss for diabetics, but may improve quality of life as well.

STUDY FINDS GASTRIC BYPASS IMPROVES QUALITY OF LIFE For the study, researchers looked at the effects of weight loss on 38 patients with type 2 diabetes. A total of 19 of them went through gastric bypass surgery, and the other 19 went through a medical diabetes and weight management program that involved exercise, diet, and meal replacements. Researchers followed up with all the patients at 18 and 24 months. Results showed the following:

■ Two years after treatment, those who

went through surgery had lost an average of 64.4 pounds. Those who had gone through the weight management program lost only 11 pounds. ■ At 18 and 24 months after treatment, those who had gone through surgery reported a 60 percent greater reduction in problems managing their diabetes. These problems included emotional distress, eating behaviors, and trouble with selfmanagement. ■ Up to two years after treatment, those who went through surgery had nearly twice the improvement in the adverse effects of weight on their quality of life.

IS GASTRIC BYPASS FOR YOU? You may be a candidate for gastric bypass if you: ■ Have a body mass index (BMI) of 40 or more. ■ You have a BMI of 35 to 39.9 and you have type 2 diabetes. Though there are clear benefits to gastric bypass for type 2 diabetes patients, there are risks as well. Talk to your doctor about your options. If you’ve struggled with obesity and diabetes for awhile now, and lifestyle modifications haven’t helped, this surgery may be the treatment you’re looking for.

WHAT’S CAUSING YOUR BELLY FAT? YOU MAY HAVE INSULIN RESISTANCE! During the past 20 years, there has been a dramatic increase in obesity in the United States. More than one-third of U.S. adults (35.7%) and approximately 17% (or 12.5 million) of children and adolescents aged 2–19 years are obese. Another 33% of all adults are in the overweight category. Though there are a lot of things that can cause weight gain, I find the following four factors often play a crucial role in expanding my patients’ waistlines.

THREE THINGS HAVE CHANGED ABOUT HOW WE EAT Hippocrates, father of medicine, is quoted as saying, “Let food be your medicine, medicine be your food.” Over the past several decades, however, we’ve moved away from this concept, and now see food merely as “calories in, calories out,” which has changed our relationship to what we eat. Three things happened to the modern diet that now contribute to the high levels of inflammation we currently see in the population at large, leading to increased health problems and disease. Understanding these three fundamental principles forms the foundation for a healthy and long life. Belly fat and insulin resistance: Insulin resistance, or metabolic syndrome, affects one in three Americans and leads to excess belly fat. Unlike the subcutaneous fat that is protective, studies have shown that belly fat produces cytokines and other inflammation-causing chemicals that lead to the increased risk of cardiovascular disease, diabetes and other conditions. In fact, waist circumference of over 40 inches in men and 36 inches in women (lower in Asian Populations), is one of the five criteria for metabolic syndrome. This leads to belly fat which increases the risk of diabetes, heart disease and even cancer risk! Lack of antioxidants: Before the modern food processing era, our diet used to have more than 50 percent unprocessed and uncooked food items such as colored fruits, vegetables, and other natural ingredients that are high in antioxidants such as polyphenols. THE FOURTH FACTOR More recent research has revealed a fourth factor in this list—lack of quality sleep. According to the National Institutes of Health, 50 to 70 million Americans are affected by chronic sleep disorders and intermittent sleep problems. The Centers for Disease Control and Prevention (CDC) adds that sleep insufficiency is linked to motor vehicle crashes, industrial disasters, and occupational errors. Lack of proper duration and quality of sleep also contributes to insulin resistance, which in turn leads to weight gain. A 2012 study, for example, found that sleep deprivation impaired the ability of fat cells to respond to insulin, the hormone that regulates blood sugar. Unfortunately, this can set in a vicious cycle where sleep problems lead to weight gain and weight gain in turn—particularly if it leads to sleep apnea—can make sleep problems worse!

Jackie was 69 years old, and she was worried. For the last few weeks, she’d been experiencing pins and needles in her left shoulder and arm. It would come on all of a sudden, last for a few minutes, and then go away. She was trying not to be paranoid, but couldn’t help but be concerned. Could it be an early sign of a heart attack, or a symptom of multiple sclerosis? Symptoms in the left arm are always a little scary, because they can signal an impending heart problem. There are a number of other potential causes, however, so it’s important to record your symptoms, stay calm, and talk to your doctor as soon as possible.

WHAT CAN CAUSE SYMPTOMS IN THE LEFT ARM? Most people are aware that pain/tingling in the left arm can signal a heart attack, but it can also be a symptom of other conditions. Here are ten of them. 1. Heart attack: This is the one you know. You experience pain or tingling in the left arm, and you may also have pain in the chest or left jaw. 2. Stroke: This is the other most serious possible cause of symptoms in the left arm. If you feel a tingling in the arm, it could signal impaired brain activity, or impaired blood flow to that part of the body.

3. Circulation problems: A tingling sensation accompanied by cold and clammy hands may indicate that you have blood circulation problems, but not necessarily a stroke. Your blood vessels may be narrowed because of atherosclerosis, or you may just be wearing clothes that are too tight. 4. Parasthesia: This is the medical name for that feeling of tingling, pricking, or burning. It may happen and then go away forever, or it may become chronic, returning again and again. Chronic parasthesia may be a symptom of a neurological disease or traumatic nerve damage. 5. Carpal tunnel syndrome: You may think of this condition as occurring in the hand and wrist, but it can affect the blood flow for the entire arm. If you feel a tingling or numbness in the wrist and arm, this may be the cause. 6. Pinched nerve: This is one of the most common causes of that tingling feeling. If you have a pinched nerve somewhere in the arm, shoulder, or neck, it could cause tingling and numbness in your arm. 7. Spinal problems: If you have a herniated disc or other problems in the spine, it can affect your arm. 8. Other diseases: Diabetes, alcoholism, multiple sclerosis, metal poisoning, and other disorders can cause nerve problems in the arms. 9. Thoracic outlet syndrome: This occurs when nerves or blood vessels that go from

your neck to your arm are compressed because of trauma, overuse injuries, obesity, pregnancy, and more. A common symptom is left arm tingling and/or numbness. 10.Vitamin B12 deficiency: Especially in seniors, a vitamin B12 deficiency may affect the nerves to the point that they tingle or become numb. Seniors have a more difficult time absorbing vitamin B, and often need supplements.

MEDICAL MARIJUANA AS A POTENTIAL TREATMENT FOR GLAUCOMA WHAT THE STU UDIES TELL US SO FAR

■■■ Colleen M. Storyy You may already be aware that medical marijuana (cannabis) can help ease pain, soothe nausea and vomiting, and tame anxiety in some individuals. But can it help treat glaucoma? There was some evidence from the 1970s that it could, but the studies were limited. Here’s what today’s research has shown us so far.

WHAT IS GLAUCOMA? Glaucoma is a disease of the eye that causes damage to the optic nerve. This nerve is responsible for carrying images from the retina at the back of the eye to the brain. Over time, glaucoma increases pressure on the delicate nerve fibers of the optic nerve, causing blind spots. Over time, the disease can lead to progressive, irreversible vision loss. Glaucoma is the second leading cause of blindness.

CONFLICTING RESULTS FOR MEDICAL MARIJUANA Current treatments for glaucoma include medications (usually in the form of eye drops) that help reduce pressure on the nerve, laser surgery, and operative surgery. With proper attention, patients with glaucoma can usually slow down the progression of the disease and prevent further loss of eyesight. Some patients, however, have trouble with side effects or fail to respond to current treatments. Might medical marijuana provide patients with another alternative? According to the American Academy of Ophthalmology

(AAO), the idea came to light in the 1970s, when preliminary studies found that smoking marijuana, or administering it orally or intravenously, did lower intraocular pressure (IOP), which is the pressure that causes optic nerve damage in glaucoma patients. A later 1980 study also found that marijuana inhalation was accompanied by a decreased IOP, but also a general reduction in blood pressure. This is a concern because if the overall blood pressure goes too low, it can reduce blood flow to the optic nerve, leading to vision loss. A 1998 study reported that it is “undisputed that smoking marijuana plant material causes a fall in intraocular pressure (IOP) in 60% to 65% of users.” The concern, they went on to state, was that long-term smoking could cause other effects in the body that could potentially make the disease worse, or worsen the patient’s overall health.

DRAWBACKS OF MEDICAL MARIJUANA FOR GLAUCOMA Another concern is that though medical marijuana does lower IOP, it does so for only about three to four hours, meaning that patients would have to smoke it around the clock to maintain the effects— impractical for most. While under the influence of the drug, patients would also be advised not to drive or operate heavy machinery, which could interfere with their regular activities. Unlike some other uses for marijuana, such as to ease the pain of cancer treatments, glaucoma would require regular and consistent

use for the rest of the patient’s life, or until it stopped working. This sets up a number of additional problems, as long-term use of marijuana increases risk of several other health problems. These include lung damage, memory impairment, schizophrenia, panic attacks, heart disease, infertility, and a weakened immune system.

WHAT IF YOU DIDN’T HAVE TO SMOKE IT? What if patients didn’t smoke the marijuana—would that help? In a 2002 study, researchers had participants either smoke marijuana or take marijuana tablets for treatment of their glaucoma. All had an initial decrease in IOP, but these decreases were not sustained—pressure went up again after awhile, indicating the patients may have adapted to the drug. All patients elected to stop the treatment within one to nine months for various reasons, including a desire to avoid side effects. We don’t have any marijuana eye drops yet. Scientists have looked into it, but they still haven’t formulated a solution that can treat the eye with high enough concentrations to be effective. This is an area of ongoing research.

BOTTOM LINE Though marijuana can reduce IOP, at least in the short term, it remains a poor option for long-term treatment because of side effects and other potential health hazards. Patients are advised to stick with proven treatments unless new research provides a workable alternative. www.4health.net ww.4h w.

Once diagnosed, it can be successfully treated with injections and/or laser treatments as indicated by the latest protocol. “There are some hidden diseases like glaucoma that a person may not be aware of, and yet it can threaten their vision. screening—at least once a year to determine if there is any pathology. Also, some people have a family history of glaucoma and retinal disease. Because these diseases are hereditary, it’s important these patients have regular eye exams.”

ADVICE FOR PROTECTING YOUR EYES When it comes to eye care, Dr. Appel says it’s very important to wear sunglasses. The ultraviolet light of the sun can accelerate cataracts and some retinal diseases. Additionally, a healthy lifestyle, including regular exercise and eating a nutritious diet, helps keep your eyes functioning optimally. When she was only a child, Dr. Appel immigrated with her parents to the United States from Kiev in the former Soviet Union. Today, she practices what she preaches. She is a regular road runner and has completed 15 marathons. She runs 5K races to raise money for the eye bank. She also strongly believes in giving back, and has traveled to St. Kitts and Grenada where she has performed laser eye surgery and treated diabetic eye disease. Her interest in ophthalmology began while she attended New York University School of Medicine. All students there must study the various medical specialties. “I fell in love with the instrumentation and loved the micro surgery involved in ophthalmology,” she said. If you’re taking birth control pills, you may be a little bit concerned. There are a lot of messages out there about them. Do they cause depression? Could they increase your risk of cancer? Are they affecting your weight?

MYTHS & TRUTHS ABOUT BIRTH CONTROL PILLS Rumors may begin with a kernel of truth, but they often get blown out of proportion. Here are several rumors about birth control pills that aren’t entirely true. They cause depression. Studies have shown that some women have an increased risk of depression and/or mood swings when taking the pill, but the levels are low, meaning that most women don’t experience these mood problems. Those more likely to experience depression were often those who had a history of depression prior to using the pills. One recent study actually showed that women on birth control pills were less likely to be depressed than those using other forms of contraception. They cause cancer. In truth, birth control pills reduce the risk of many cancers, and

may increase risk of only two—breast cancer and cervical cancer. Even this isn’t certain, as studies have been mixed. Some have shown a slightly increased risk of breast cancer, while others have shown no increase at all. There is absolutely no evidence that the pill causes breast cancer, however. It may slightly increase risk of cervical cancer, but again, this may be because sexually active women have a higher risk of becoming infected with the HPV virus, which is what causes cervical cancer. At the same time, birth control pills reduce the risk of ovarian, uterine, and endometrial cancer, and may also protect against bowel cancer. They cause weight gain. Some women experience some water retention when taking the pill, which may make a pound or two difference in your weight. A review of 44 studies found no evidence that birth control pills result in weight gain for most women. Those that do gain a pound or two when starting birth control will typically lose it within a few months. They affect your future fertility. There are some rumors that taking the birth control for a few years or more could make it harder to get pregnant later. The truth is

that it may take a few months for your body to adjust after you go off the pill, to give your hormones a chance to stabilize again. In fact, most women tend to recover quickly, which is why it’s always recommended that you use another form of birth control after forgetting to take the pill as you have a chance of becoming pregnant. A large study of over 2,000 women who had taken the pill for about seven years and then went off it reported that over 20 percent were pregnant within a month, and nearly 80 percent were pregnant within a year. You need to take a break from them. Some women believe that they need to give their bodies a “break” from the pill every now and then. This can actually be a bad idea, as it may lead to an unwanted pregnancy. It can also put your body through unnecessary stress, as when you go off the pill, your body needs to start producing hormones again—in essence, it goes through a major shift. If you then go back on the pill, it has to adjust again. It’s actually easier on the body to remain on the pill to keep things consistent so your body doesn’t have to go through so much adjusting. SHOULD YOU GET A BUTT LIFT? WHAT’S INVOLVED IN ENHANCING YOUR BACKSIDE y Merrell ■■■ Lynn

When you think of plastic surgery, you may think of breast enhancement or facelifts. But according to a recent American Society of Plastic Surgeons report, it may be time to turn around and look at your backside. Last year, doctors performed over 10,000 butt lifts—more than in any previous year. Also called “buttock augmentation,” “butt implants,” or “gluteoplasty,” butt lifts are catching on, with 16 percent more people going for it in 2013 than in 2012. What’s the big deal, and could you be a candidate for backside enhancement?

WHAT IS A BUTT LIFT? The butt lift is a procedure intended to create youthful, perky buttocks that enhance the body profile. The basic surgery tightens the appearance, removing any excess skin and repositioning the surrounding tissue to create a smoother, more rounded look. If the patient wants a little more junk in the trunk, so to speak, doctors may also extract fat from other parts of the body through liposuction, purify it, and inject it back into the buttocks to create a fuller appearance. (The extra fat is typically taken from the stomach, thighs, and lower back.)

The Brazilian butt lift, which you may have also heard about, is a particular type of butt lift that involves adding some padding. The surgeon will either add in extra fat, as noted above, or will use implants to create the more rounded, lifted look. Fat redistribution is often preferred for creating overall body sculpting, and surgeons like working with it because it is pliable and gives them more control over shaping.

WHAT ARE THE RISKS? The risks of a butt lift are similar to the risks for any surgery, and include infection, an allergic reaction to the anesthesia, a blood clot, and excess bleeding. Potential complications include poor wound healing, asymmet-

ric of flattened buttocks, fat creating lumps in the backside, swelling and fluid accumulation, and bleeding under the skin. Most of these risks are rare, however, as long as patients go to an experienced, legitimate plastic surgeon. There have been some horror stories of women actually dying after going through a butt lift with the wrong doctor. In March 2014, for example, police charged Tamira Mobley of Wood-Ridge, New Jersey, with manslaughter and felony assault after she gave a young woman a botched butt lift that resulted in her death.

The surgery itself can take several hours, depending on the extent of fat repositioning needed. TOENAIL FUNGUS— SHOULD YOU TRY LASER TREATMENTS? STUDIES QUESTION WHETHER LASERS ARE COMPLETELY EFFECTIVE ■■■ Gordon Barclayy Jennifer had tried everything, but nothing was having any effect on her toenail fungus. Three of her toes had been infected for over a year. Her doctor had given her medications, but the side effects were too difficult for her to maintain treatment. Home remedies had been ineffective and she didn’t know where to turn. Don was in a similar situation. He had suffered from toenail fungus for most of his life, going on and off treatments and spending a fortune on medications and creams. Nothing would get rid of it for good

and he was worried it was something he would have to live with the rest of his life. Both Jennifer and Don heard about laser treatments, and wondered if they would do the trick. Could lasers finally destroy the fungal infections?

WHAT IS TOENAIL FUNGUS? Toenail fungus is caused by a fungal infection that goes deep into the nail bed. Also called “onychomycosis,” it affects between six and eight percent of the population. The infection occurs when a person comes into contact with the microscopic fungi—

The fungi that cause toenail infections live in warm, damp areas, like swimming pools, showers, and locker rooms. They can get into the skin through small cuts or through a small separation between the nail and the skin underneath.

WHO’S AT RISK? Some people are more at risk for a toenail fungal infection than others. Those who perspire a lot, for example, may have moist feet, which fungi prefer. Those who walk barefoot through public areas, have a skin condition like psoriasis, have a reduced blood flow to the feet, have diabetes or a weakened immune system, or who work in a humid or moist environment are also more at risk.

STANDARD TREATMENTS If you go to your podiatrist with a fungal infection, he’s likely to recommend medications as a first line of treatment. There are several options, including oral antifungal drugs, medicated nail polishes, and medicated nail creams. There have been substantial improvements made in these treatments over the years, but they may cause uncomfortable side effects in some people, and in others, may not be completely effective. If they don’t work, it’s time to move to other options. This is the point at which some patients may consider laser treatments.

DO LASER TREATMENTS WORK? During a laser treatment, the doctor applies the heat of a laser to the nail to destroy the harmful fungi while sparing healthy

tissue. The Food and Drug Administration (FDA) approved the treatment about four years ago, after some small studies showed promise that it could help cure toenail fungus. A recent study, however, cast doubt on the efficacy of this treatment. Researchers tested laser treatment on toenail fungus, applying it in five treatment sessions. Results showed no improvement in the severity of the fungal infection. Another 2013 report published in Dermatology Online Journal also looked at laser treatments for toenail fungus, and concluded that while some doctors have experienced success with the treatment, overall the evidence has not yet shown it to be effective for most people, and that more studies are needed. Dr. Bershow, co-author of the report, noted that there are drawbacks to laser treatments as well—namely, that they are expensive, that most insurance companies won’t cover them, and that after paying a lot of money, patients may still be saddled with an infection.

SHOULD YOU TRY LASER TREATMENT? If you have the money, you may elect to try lasers if you haven’t had luck with other treatments. Just be prepared as the infection may come back. Practicing careful hygiene, using anti-fungal spray, changing socks daily (or more often), and refraining from going barefoot may all increase your odds of success. If you’d rather not do laser, or if you find it ineffective, there is still another option—the complete removal of the nail, which is usually very effective.

INFERTILITY IS NOT FOR SISSIES STUDIES SHOW WOMEN SUFFER STRESS; AT RISK FOR DEPRESSION

■■■ Colleen M. Storyy Cathy and Jeff had been trying for years to get pregnant. They wanted desperately to have a family, but after giving it everything they had, nothing was working. They were out of money, and out of endurance. Jeff said there were other options. They could think about adopting. They could volunteer. They could find other ways to bring children into their lives. Cathy tried to be reasonable. He was right. It was time to move on. But she couldn’t do it. She and Jeff went through a very difficult year. At the end of it, Cathy tried to commit suicide. Fortunately, Jeff found her before it was too late. According to a recent study, Cathy’s behavior is not abnormal at all. Many women who suffer from infertility are at risk for serious psychiatric illness. The unfortunate thing is that they are often not aware of it.

INFERTILITY IS STRESSFUL In today’s world, even if a couple doesn’t have the ideal biological circumstances to produce a baby, they can undergo infertility treatments to hopefully help them start a family. These treatments aren’t easy, however, and can cause a lot of stress and strain. In 2012, researchers looked at 14 studies that included a total of over 3,500 women who were going through fertility treatments. They found that many women experienced significant emotional distress and anxiety,

though it didn’t seem to affect their ability to become pregnant. A later 2014 study found similar results in that infertility caused stress. This time, however, that stress did affect the women’s ability to conceive. Researchers at Ohio State University examined saliva samples from over 400 women who wanted to get pregnant, and found that those with higher levels of stress hormone in the samples were twice as likely to be diagnosed as infertile—unable to get pregnant after a year of trying.

WHAT HAPPENS WITH ALL THAT STRESS? All that stress has to go somewhere. If women eventually conceive, the energy can go into a positive place, but if they don’t, then what? Danish researchers analyzed data from over 98,000 women who had seen a doctor about infertility between 1973 and 2003. They then followed them for an average of about twelve-anda-half years, and found nearly 4,700 who were hospitalized with mental health problems. Just over 2,500 had not been able to conceive. Just over 2,100 had experience success with fertility treatments and had at least one child as a result. Results showed the following: ■ Women who remained childless had an

18 percent higher risk of being hospitalized for mental illness than those who had at least one child. ■ Potential mental illnesses included anxiety, obsessive-compulsive disorders,

schizophrenia, depression, and eating disorders. ■ Infertility was also linked to a significantly higher risk for alcohol/substance abuse. Researchers noted that the results were probably understated, as other women may have had similar mental health problems, but had not been hospitalized for them. An earlier study found similar results. Researchers studied data from over 50,000 women who had been referred to hospitals or private fertility clinics between 1973 and 1998. They found that women who did not have a child after an initial fertility evaluation had an over two-fold greater risk of suicide than women who had at least one child after a fertility evaluation.

ASK FOR HELP These studies illuminate how much women who are diagnosed as infertile may suffer both while trying to get pregnant and after. If you or a loved one is going through fertility treatments or trying to recover from the realization that you may not be able to conceive, it’s important to be aware the dangers. Realize it’s not your fault, you’re not alone, and that help is available. Talk to your doctor about potential support groups, personal therapy, couples counseling, and stress relief. AFTER A STROKE WHO CARES FOR THE SPOUSE? STUDY FINDS CAREGIVERS AT RISK OF DEPRESSION

Surviving a stroke is a hard thing to do. Just ask anyone who’s done it. “Depression really hit me when I returned home after rehab,” Thomas, a stroke survivor, told the American Stroke Association. “I soon found I would have to depend on other people for things as simple as changing a light bulb.” The Centers for Disease Control and Prevention (CDC) states that stroke is the leading cause of death in the U.S., killing nearly 130,000 Americans each year. Yet more than 795,000 people suffer a stroke each year, leaving hundreds of thousands to cope with recovery when they get back home. But it’s not only the victims themselves who struggle— caregivers, too, can undergo a lot of stress while trying to help a loved one and adapt to their new situation. According to a recent study, if the patient and the caregiving spouse disagree on how the stroke victim is doing in his recovery, the caregiv-

ing spouse may be at significant risk for emotional stress and depression.

STUDY SHOWS RISK OF DEPRESSION IN STROKE SPOUSES For the study, researchers from the University of Cincinnati interviewed 35 couples in which one spouse had suffered a stroke within the past three years. They talked to each person individually, asking questions about the stroke survivor’s recovery, including how they were doing at everyday tasks, how their memory was performing, and how they were managing any problems they came up against. Results showed the following: ■ In general, the spouses felt like the stroke survivor’s recovery was

going much slower than the stroke survivor did. ■ The degree to which the spouses’ opinions differed correlated

4HEALTH ■ In other words, the more the spouse’s opinion of recovery dif-

fered from the survivor’s opinion, the more likely the spouse was to experience depression. Researchers explained that if the wife was the one who had suffered a stroke, she may have said that she believed she could still drive, or that she was recovering enough to do more tasks around the house. If her husband disagreed, saying she couldn’t drive or he had noticed little difference in her abilities to do everyday tasks, he would be more at risk for depressive symptoms.

OTHER EVIDENCE ADVISES CAUTION FOR CAREGIVERS This isn’t the only study to find risks for stroke caregivers. Earlier research from 2001 found that spouses suffer psychological effects immediately after their mate suffers a stroke. “Our research indicates that it is very important for health professionals to pay attention to the spouses’ psychological health as well as the patients’,” said lead author Forsberg-Warleby. According to a 2013 report by the U.K. Stroke Association, twothirds of caregivers experienced difficulties in their relationships with a partner as a result of a stroke. One in ten actually broke up with their partners, or considered doing so. For many, the situation doesn’t get better—while nearly 50 percent said they were stressed by caring after three years, nearly 70 percent said they were after seven years.

TIPS FOR CAREGIVERS Unfortunately, caregivers rarely have anyone to care for them, so they have to watch out for their own well being. That may not always

be easy—according to the report above, three-quarters of them put the needs of the stroke survivor over their own, which is typically a recipe for burn-out and health problems. If your spouse has suffered a stroke, here are some tips to take better care of yourself. Don’t forget—if you’re not feeling well, you’ll have a hard time caring for your spouse. Look for support. Ask your doctor about caregiver groups. No one knows what you’re going through unless they’ve gone through it themselves. Talking to others in the same boat can be extremely helpful. Educate yourself. Caregivers often lack the knowledge they need to adapt to their new life with their spouse or loved one. Keep a running list of questions to ask the doctor. Ask for materials that explain what has happened and what to expect. Check online resources. Get help. Don’t expect that you can handle all of your spouse’s needs by yourself. Look into at-home care for assistance, or look for other friends and family members that may be willing to step in to handle some tasks, like grocery shopping, lawn care, pet care, or travel to doctor’s appointments. Focus on your own health. You still need to eat a healthy diet, get daily exercise, and get regular rest, if you want to stay healthy. Maintaining these habits will also boost your emotional health. Don’t neglect your own life. Many caregivers stop doing the things they enjoy to take care of their spouse. This increases risk of depression and other health issues. Plan time away to meet up with friends and do things you enjoy so you can rest and recharge.
