It’s summertime, which for most of us, means fun outdoor activities in the sun. If you’re bothered by allergies, however, most likely your sunny days are marred by watery eyes, sinus congestion, runny nose, sneezing, coughing, headaches, sore throats, or itchy skin. Fortunately, you don’t have to continue to suffer from these irritating symptoms. A qualified allergy doctor can discover what you’re allergic to, and offer treatments that provide easy relief.

WHAT CAUSES ALLERGIES? Allergic ailments are caused by an «overreaction» of the immune system to various environmental substances. The immune system perceives some elements of the environment as a threat to the body and tries to «defend» against them. The major form of defense is an inflammatory response, which is why clinical symptoms of allergic ailments and infectious illnesses are very similar and tend to overlap.

Allergens can be divided into two large groups: 1) those that we encounter outside our homes, such as pollen from various plants, and 2) those that are present inside our homes, such as dust or mildew. Different plants produce pollens at particular times of the year. As a result, people sensitive to tree pollen will experience an “acute exacerbation,” or flare-up, of symptoms when trees blossom. In the New York area, this period stems from April until early June. Likewise, people with hypersensitivity to autumn grasses, such as wormwood or ragweed, will begin to feel allergic symptoms in the fall, between August and October. Very often, individuals are sensitive to several allergens, and thus the duration of their symptoms is considerably longer. This pertains particularly to people who are sensitive to allergens that are present inside houses and apartments, ranging from domestic dust and mold (fungi), to hair and saliva of various animals (so-called indoor allergens). Such people may show continuous allergic symptoms all year long. Symptoms may include headaches, sore and burning throat, itching and irritation of the eyes, and coughing. People who experience mild continuous symptoms due to household allergies

often fall victim to flare-ups in the spring and fall, as well. Allergic diseases are not limited to symptoms connected with upper airway passages, however. Other prevalent allergic ailments include bronchial asthma (with symptoms of wheezing, shortness of breath, chest tightness, and/or coughing), and asthmatic bronchitis. Of those who suffer from bronchial asthma, 50% of adults and 80% of children have allergies.

IDENTIFYING TRIGGERS WITH SKIN TESTS The most common method of identifying allergens to which an individual is sensitive through various types of skin tests. Conducted by doctors who are allergy specialists, these tests are safe and take a minimal amount of time. Results are usually available within fifteen minutes. Knowing the substances which a person may be sensitive to enables the allergist to use effective preventative measures and treat allergic diseases. Preventative measures and treatments, in general, are carried out using a combination of different medications selected by a doctor to meet the particular needs of each patient.

TREATMENTS CAN HELP YOU FEEL BETTER For those who suffer from allergy symptoms three months or more throughout the year, and want to be cured permanently, doctors might recommend injections of allergy provoking substances (such as tree pollen, animal hair, or mold), which are highly diluted in water. Patients following this therapy become “immunized,” or desensitized, to the allergens. This method is called immunotherapy, or «allergy shots.» Numerous studies have shown that the success of immunotherapy exceeds 80%. If you suspect you have allergies, you can make an appointment with a board-certified allergist, and, in many cases, walk out of the office with the mystery of your ailments instantly solved. There may be things you are exposed to that you can simply avoid. An allergist will often suggest how you can make changes in your home and work environment, so that symptoms are relieved. WHAT DO YOUR CHOLESTEROL NUMBERS REALLY SAY? NEW GUIDELINES TAKE OTHER RISK FACTORS INTO ACCOUNT

If you’re 40 or over, most likely you’ve had your cholesterol levels checked, probably more than once. You may know your most recent numbers. What you may not know is exactly what those numbers mean. It used to be that a high cholesterol number signaled an automatic risk for cardiovascular disease, while a low number meant you didn’t have to worry about it. But recent scientific studies have shown us that it’s a little more complicated than that. Here’s what you need to know to make the right decision when it comes to treatment.

THE BASICS ON CHOLESTEROL NUMBERS Cholesterol has long been associated with heart disease because it can build up inside the walls of the arteries, causing “atherosclerosis,” or a narrowing and stiffening of the arteries. Over time, this cholesterol buildup can lead to a heart attack or stroke. Because cholesterol makes up a large part of the plaques that form in atherosclerosis, doctors and scientists believed that cutting back on the amount of cholesterol circulating in the blood helps to reduce the risk of heart disease. To that end, they aimed to help people attain the following numbers: ■ Total cholesterol: below 200 mg/dL ■ LDL “bad” cholesterol: below 100 mg/dL

■ HDL “good” cholesterol: at or above 50

FACTORS TO CONSIDER IN TREATMENT

mg/dL ■ Triglycerides: below 150 mg/dL Based on these guidelines, a person who tests high for LDL cholesterol may have been prescribed a cholesterol-lowering medication. But new guidelines show that doctors need to take other factors into consideration.

Cholesterol is actually good for us. It helps make hormones and vitamin D, insulates nerve cells, and maintains cell walls and structures. Too much, however, can increase risk of heart attack, but it depends on the person. Doctors recommend that everyone over the age of 20 get regular cholesterol screenings at least once every five years. If cholesterol levels are found to be high (as described below), drugs aren’t necessarily the answer. Instead, doctors now take into account other risk factors like smoking, diet, exercise, diabetes, blood pressure levels, and weight in determining the best approach. ■ Total cholesterol: high if 240 mg/dL or above ■ LDL cholesterol: high if 160 mg/dL or above ■ HDL cholesterol: low (unhealthy) if less than 40 mg/dL If you have levels that are considered high, talk to your doctor. If you’d like to try getting your numbers down without drugs, try these lifestyle changes: ■ Stop smoking ■ Maintain a healthy weight ■ Exercise for 30 minutes at least five days a week ■ Eat less saturated fat and more fiber ■ Eat fewer refined sugars and refined grains (like white flour)

ONE CHOLESTEROL NUMBER ISN’T ENOUGH According to the American Heart Association (AHA), “your LDL number should no longer be the main factor in guiding treatment to prevent heart attack and stroke.” Scientists have found that relying on this number alone can leave many patients unprotected. A 2009 study found that almost half of patients admitted to the hospital for heart disease had LDL cholesterol numbers below 100—considered low risk. The problem with relying on cholesterol numbers alone is that they tell you how much is circulating in your blood, but not how much is building up along the walls of your arteries. Things like genes, lifestyle, diet, and other factors can all affect the “stickiness’ of the cholesterol in your body. Someone who smokes, for example, is more likely to buildup cholesterol plaques in the arteries than someone who doesn’t, even if both people have the same LDL cholesterol number.

There are a lot of reasons to keep your teeth looking good these days. A 2013 study, for example, found that white teeth make you look five years younger, more employable, and more successful. A 2012 survey by Kelton Research also found that teeth are the first thing many people notice when they meet someone new. Keeping teeth clean also helps reduce bacteria, however, which is important to overall health. A recent study, for example, found that gum disease harms the immune system, leaving people more vulnerable to dangerous infections.

WHAT CAUSES GUM DISEASE? We are in a daily fight against bacteria. They always exist in the mouth, and like to feed on the food we eat and the beverages we drink. As long as we brush and floss regularly, we keep the bacteria in check. Without regular cleaning, however—or if other factors interfere, like illness and some medications— the bacteria can gain the upper hand, clinging together to form sticky plaque on the teeth. If you go to your regular dentist appointments, your hygienist will remove this plaque, getting it off the teeth so it can’t cause problems. If the plaque stays on the teeth, however, it will continue to grow and spread. This can cause inflammation in the gums, a condition called

“gingivitis.” You may notice bleeding when you brush. This indicates that the bacteria are winning, and it’s time to take serious action. Without it, the condition can progress to the more serious “periodontitis,” which causes pockets to form between the gum and teeth, leading to infections and potentially to tooth loss.

University of Florida found that the same bacteria that cause gum disease also promote heart disease. The National Health Service in the United Kingdom also notes that gum disease increase the risk of stroke and diabetes.

RISK FACTORS FOR GUM DISEASE GUM DISEASE BACTERIA HOBBLES THE IMMUNE SYSTEM While gum disease is dangerous to gums and teeth, it’s also dangerous for the rest of the body. In this recent study, researchers from the University of Pennsylvania identified P. gingivalis as a type of bacteria responsible for many cases of periodontitis. When they contaminated mice with the bacteria, they found that it interfered with the immune system, causing it to be much less efficient in knocking out infections. The bacteria also promoted inflammation. All in all, this one type of bacteria was responsible for “hobbling” the immune system so that it was less likely to do its job. Usually, when we have an infection, the body’s immune system goes to work getting rid of it. Without a strong immune system, we are more vulnerable to a number of health problems, including heart disease, cancer, and cold and flu viruses. People with gum disease have compromised immune systems. Other studies have found this to be true. In 2014, for example, researchers from the

systems, and therefore may have a harder time fending off mouth bacteria. Smoking: Tobacco use increases risk for gum disease. Genetics: If you have gum disease in your family, you may be more at risk for it. Stress: Stress weakens the immune system, which can make it tougher for your body to fight off the bacteria. Medications: Many of today’s modern medications can interfere with your oral health. These include oral contraceptives, antidepressants, and some heart medications. ■■■ Colleen M. Storyy Jane wondered what was wrong with her stomach. It seemed it was rarely happy anymore. She thought maybe she was suffering from colic, but the pain kept coming back. Sometimes it was really bad, causing cramping, gas, vomiting, and diarrhea. She finally went to the doctor. He told her she had a twisted bowel. If you imagine twisting up a garden hose so the water doesn’t flow anymore, you have the general idea of what a twisted bowel is. For those unlucky enough to have this happen, is surgery the only answer?

WHAT IS A TWISTED BOWEL? When someone has twisted bowel, it means that some part of the intestines has become twisted, resulting in an obstruction. Where the twist occurs is important: ■ Small intestine: This is the area of the

intestine that runs between the stomach and large intestine, or colon. A twist here is called “small bowel volvulus.” ■ Large intestine: This is the largest part of the digestive system, and runs from the end of the small intestine (starting at a pouch called the “cecum”) to the anus. A twist here is called “colonic volvulus.”

born with intestinal malformations, including twisting. These infants are also more at risk of twisted bowel in adult life. Your diet isn’t great: A poor diet can affect how the waste moves through the bowel, which may result in twisting in some people. Overeating: Overwhelming the stomach with too much food can cause the intestines to sag or droop, which can increase risk of twisting. Pregnancy: Expectant mothers are more at risk. Other health conditions: Patients who have had intestinal surgery in the past may suffer from scar tissue that can pull and tighten intestinal tissue, increasing risk of twisting. Diverticulosis, some cancers, Crohn’s disease, severe constipation, gallstones, and post-surgical recovery may increase risk. Endometriosis: Increases risk in women.

SYMPTOMS OF TWISTED BOWEL If you have a twisted bowel, that doesn’t mean you’ll necessarily experience severe pain right away. You may have a few cramps or feel a little stomach upset at first. But over time, the pain is likely to get worse.

WHAT CAUSES A TWISTED BOWEL? Why would part of the intestines become twisted? Doctors aren’t exactly sure why it happens, but there are several things that increase risk:

■ Nausea and vomiting ■ Swollen or distended stomach ■ Constipation and bloating ■ Difficulty making a bowel movement

Blood in the stool Intense fatigue Back pain Bad breath and fever

IS SURGERY REQUIRED? In most cases of twisted bowel, surgery isn’t necessary. Doctors typically choose between the following options depending on your specific case: ■ Time: In some cases, patients can be

treated in the hospital in such a way that the intestine is given time to heal on its own. Patients refrain from eating for awhile and receive fluids through an IV to take the pressure off the intestine. ■ Enema: Sometimes this encourages the bowel to straighten on its own. ■ Lighted instruments: Doctors may be able to use a proctoscope or sigmoidoscope to straighten the twisted section of bowel, or may insert plastic stents to keep the intestine straight and open. ■ Surgery: If the intestine is completely blocked and causing severe symptoms, surgery is the best option. The doctor may perform various types to fix the problem, depending on where the twist is. The important thing is to fix the problem. If a twisted bowel is left untreated, it can cause part of the intestine to die, and may lead to life-threatening infections. Always check with your doctor!

WHAT’S CAUSING YOUR BELLY FAT? YOU MAY HAVE INSULIN RESISTANCE! During the past 20 years, there has been a dramatic increase in obesity in the United States. More than one-third of U.S. adults (35.7%) and approximately 17% (or 12.5 million) of children and adolescents aged 2–19 years are obese. Another 33% of all adults are in the overweight category. Though there are a lot of things that can cause weight gain, I find the following four factors often play a crucial role in expanding my patients’ waistlines.

THREE THINGS HAVE CHANGED ABOUT HOW WE EAT Hippocrates, father of medicine, is quoted as saying, “Let food be your medicine, medicine be your food.” Over the past several decades, however, we’ve moved away from this concept, and now see food merely as “calories in, calories out,” which has changed our relationship to what we eat. Three things happened to the modern diet that now contribute to the high levels of inflammation we currently see in the population at large, leading to increased health problems and disease. Understanding these three fundamental principles forms the foundation for a healthy and long life. Belly fat and insulin resistance: Insulin resistance, or metabolic syndrome, affects one in three Americans and leads to excess belly fat. Unlike the subcutaneous fat that is protective, studies have shown that belly fat produces cytokines and other inflammation-causing chemicals that lead to the increased risk of cardiovascular disease, diabetes and other conditions. In fact, waist circumference of over 40 inches in men and 36 inches in women (lower in Asian Populations), is one of the five criteria for metabolic syndrome. This leads to belly fat which increases the risk of diabetes, heart disease and even cancer risk! Lack of antioxidants: Before the modern food processing era, our diet used to have more than 50 percent unprocessed and uncooked food items such as colored fruits, vegetables, and other natural ingredients that are high in antioxidants such as polyphenols. THE FOURTH FACTOR More recent research has revealed a fourth factor in this list—lack of quality sleep. According to the National Institutes of Health, 50 to 70 million Americans are affected by chronic sleep disorders and intermittent sleep problems. The Centers for Disease Control and Prevention (CDC) adds that sleep insufficiency is linked to motor vehicle crashes, industrial disasters, and occupational errors. Lack of proper duration and quality of sleep also contributes to insulin resistance, which in turn leads to weight gain. A 2012 study, for example, found that sleep deprivation impaired the ability of fat cells to respond to insulin, the hormone that regulates blood sugar. Unfortunately, this can set in a vicious cycle where sleep problems lead to weight gain and weight gain in turn—particularly if it leads to sleep apnea—can make sleep problems worse!

Storyy “I’m overweight,” Janelle told her friends, “but I’m healthy.” There are a lot of people who would agree with Janelle. “I’m fat, but I’m fit,” they may say. It’s not really surprising — a study published in 2005 reported that overweight people had a lower mortality risk than normal weight people, and that both overweight and obesity cause fewer deaths than was previously believed. Turns out there were flaws in that study, though. The Harvard School of Public Health pointed out that the information gathered on underweight people failed to include diseases that caused weight loss, and smoking — both factors that increase mortality. More recent studies have illustrated that not only is being overweight or obese bad for your health, it increases your risk of cancer and premature death.

WHERE THE CONFUSION STARTED The confusion started when researchers studied some overweight and obese people and found normal cholesterol and blood pressure levels. This seemed to indicate these people had no risks for heart disease or diabetes — the two most common consequences of weight gain. Scientists called these people the “healthy obese” for awhile. The group seemed to

make up about a third of all obese adults, but they confused researchers, as they seemed to go against everything we know so far about weight and health. Whereas fat cells usually encourage inflammation and unhealthy metabolic changes, in about a third of the overweight, they seem to just sit under the skin without causing any issues. According to a 2013 study in Diabetes Care, these healthy obesity types are more likely to be under the age of 40 and to carry their fat somewhere other than the belly — the hips and thighs, for example.

STUDY SHOWS EXCESS WEIGHT HASTENS DEATH AND DISEASE More recent research, however, has found that “healthy obesity” is a fantasy. Researchers from the University of Toronto reviewed the results of eight other studies looking at the differences between normal weight and overweight individuals. The studies included more than 61,000 people. They looked at that “special” group of obese people who seemed to be metabolically healthy. What they found: Even without high blood pressure, diabetes, or other similar health issues, those people who were obese were found to have a 24 percent increased risk for heart attack, stroke, and death after 10

years, compared to those who maintained a normal weight. “Sometimes the message changes when you look closer,” said Dr. Caroline Kramer, lead author of the study. “It’s not OK to be overweight. It’s not OK to be obese, even though you may not have metabolic abnormalities.”

MENTAL HEALTH ISSUES SERIOUS IN THE ELDERLY HOW LOVED ONES CAN DETECT SYMPTOMS AND GET HELP ■■■ Colleen M. Storyy Seniors often face difficult health issues. Their bodies may not work as well as they did when they were younger. Muscle strength wanes. Joints hurt. Physical activity can be exhausting. Digestion may be more difficult. On top of that, seniors often feel lonely and isolated. They’ve likely seen many people they care about pass on. Family members may be scattered far and wide. They may find it more difficult to take part in the social activities they used to enjoy. All this can take a toll not only on physical health, but on mental health, as well. A 2011 study reported that mental health issues in those 65 and older increased nearly 21 percent between 2007 and 2010. Meanwhile, psychiatrists specializing in geriatric issues are few and far between — about one for every 10,000 senior citizens. Worse, the suicide rate for the elderly is high. In 2010, those 65 and older made up nearly 15 percent of suicides, while that for the population as a whole was 12.4 percent. Fortunately, there are steps we can take to save our loved ones from this tragic fate.

WHAT MENTAL HEALTH ISSUES DO SENIORS FACE? As our parents or other relatives age, we may be on the lookout for symptoms of heart disease, cancer, and diabetes, but mental health issues can often go undetected.

Depression: Depression affects more than 6.5 million seniors, but is not a normal part of aging. Unfortunately, it often goes untreated, as the symptoms are confused with those caused by other illnesses, and with side effects of medications. Physical illness can increase the risk of depression, but that doesn’t mean it should be ignored. Depression can make an illness worse, raising the risk of death from disease and/or suicide. Anxiety Disorders: Sometimes called the “silent geriatric giant,” anxiety disorders are more prevalent even than dementia or depression among seniors. Seniors may suffer from generalized anxiety disorder, or from specific phobias, obsessivecompulsive disorder, post-traumatic stress syndrome, social phobia, and panic disorders. Dementia: This may be one of the mental illnesses we’re more likely to expect in the elderly, but what we don’t realize is that it can show up in a number of different forms. It can also be difficult to diagnose. Whereas some seniors may simply be forgetful, others may have difficulties communicating or even taking care of themselves. In fact, simply having memory problems isn’t enough to garner a dementia diagnosis. Real dementia involves symptoms that are severe enough to affect daily activities. Alzheimer’s: While dementia is a term associated with the symptoms of cognitive decline and aging, Alzheimer’s is a spe-

cific cause of dementia that involves the buildup of plaque in the brain. Dementia is not a disease, but more like a collection of symptoms, whereas Alzheimer’s is a disease that physically affects the brain. It typically shows up as confusion, impaired speech, difficulty thinking, and personality changes.

HOW CAN YOU TELL? Though loved ones may think they notice memory lapses in the elderly, there are other, more serious symptoms to watch out for: ■ Sadness or a “down” mood that lasts for

longer than two weeks ■ Unexplained fatigue or sleep changes ■ Changes in weight (up or down) or ap-

petite ■ Feelings of worthlessness or helplessness ■ Confusion and disorientation ■ Loss of interest in activities he or she

used to enjoy ■ Changes in appearance or dress ■ Problems maintaining the home or yard ■ Financial issues — problems working

ADVICE FOR PROTECTING YOUR EYES When it comes to eye care, Dr. Appel says it’s very important to wear sunglasses. The ultraviolet light of the sun can accelerate cataracts and some retinal diseases. Additionally, a healthy lifestyle, including regular exercise and eating a nutritious diet, helps keep your eyes functioning optimally. When she was only a child, Dr. Appel immigrated with her parents to the United States from Kiev in the former Soviet Union. Today, she practices what she preaches. She is a regular road runner and has completed 15 marathons. She runs 5K races to raise money for the eye bank. She also strongly believes in giving back, and has traveled to St. Kitts and Grenada where she has performed laser eye surgery and treated diabetic eye disease. Her interest in ophthalmology began while she attended New York University School of Medicine. All students there must study the various medical specialties. “I fell in love with the instrumentation and loved the micro surgery involved in ophthalmology,” she said. 10 WAYS TO PROTECT YOUR EYESIGHT AS YOU AGE MILLIONS OF PEOPLE SUFFER EYE PROBLEMS AFTER THE AGE OF 440

The 2010 U.S. Ceensus reported that over four million of people over 40 have some sort of visual impairment, w with over one million suffering from blindness, o over 24 million suffering cataracts, over two milllion suffering from glaucoma, and over 7.5 milllion suffering from diabetic retinopathy. Meanw while, just over two million over the age of 50 suffeered from agerelated macular degeneration. We tend to take our eyesight fo or granted, but this isn’t a good idea. Close your eyes for just a moment and you’ll realize what a p precious sense sight is. Fortunately, there are things you can do to protect your eyes as you get older.

have increased by 70 percent. Eye problems don’t just affect what you can see. In seniors, they can increase risk of falling, injuries, and depression. At their most severe, vision problems can rob older adults of their independence. You may already be thinking about eating right, exercising regularly, and reducing the stress in your life. All of these things will help your eyesight as well, but there are some additional things you can do to increase your odds of enjoying your vision for many years to come. 1. Get regular eye exams: Many eye diseases develop without you noticing them. Your eye doctor can detect them early, which increases your odds of preserving your vision. 2. Eat more fruits and vegetables: They contain antioxidants important to eye health. 3. Wear sunglasses: UV radiation is just as damaging to your eyes as it is to your skin. Wearing sunglasses can help prevent cataracts and retinal damage. Just make sure they offer broad-spectrum UV protection. 4. Take a walk: In the AAO study above, researchers discovered that those who got regular physical activity were 56 percent less likely to have vision problems. 5. Consider supplements: Certain vitamins and minerals have been found to help prevent some eye diseases. Make sure you’re getting enough vitamin A and E, beta carotene, lutein, zinc and copper. The

National Eye Institute reported in 2001 that these nutrients can help reduce the risk of age-related macular degeneration. 6. Eat more fish and walnuts: Omega-3 fatty acids are crucial for eye health, and can help you combat dry eye syndrome as you age. Add more fatty fish like salmon and anchovies to your diet, along with walnuts, flaxseed, and chia seeds. 7. Stop smoking: Smoking increases the risk of eye diseases, including cataracts, macular degeneration and optic nerve damage. 8. Give your eyes a break: We’re harder on our eyes these days than we used to be. Most of us stare at some kind of screen—computer, tablet, phone, television—for hours each day. Whenever we do this, we tend to blink less, leaving eyes dry and irritated. Get in the habit of looking up and out at least once every twenty minutes. Try looking out a window or across a room so you’re using your distance vision. 9. Check with your family: Most eye diseases have some sort of hereditary link. If one of your parents or grandparents had glaucoma, for example, your risk is automatically increased. This is important for you to know, so you can tell your eye doctor. Regular screening can help you stay o top of any disease developments. 10. Report on any changes in your vision: The earlier you detect vision problems, the better your prognosis will be. If you notice blurriness, hazy vision, halos around headlights, difficulty reading, floaters, eye pain or swelling, or other issues, don’t hesitate to talk to your doctor right away.

The American Migraine Foundation states that 36 million Americans—about 12 percent of the population—suffers from migraine headaches. Nearly a third of women will experience them during their lifetimes. With migraines being so common, it can be hard to tell when you should just take an aspirin, and when you should see your doctor about the condition. Unfortunately, for some people, migraines can indicate other health issues, including an increased risk for stroke.

WHAT IS A MIGRAINE?

■ They can be chronic: Migraines are likely to plague people for years.

MIGRAINES MAY INCREASE RISK OF STROKE In addition to being painful, migraines can also increase risk of certain types of stroke in some people. A recent 2014 study, for example, found that adults with migraines have a two-fold higher risk of stroke than those who don’t have migraines. An earlier study found similar results, with researchers concluding that migraines “may be a marker for patients at increased risk for ischemic stroke unrelated to a migraine attack.”

They last: Migraines can last anywhere from a few hours to several days.

The results of a 2014 survey by the American Society for Dermatologic Surgery showed that nearly half of the respondents—41 percent—wanted treatments that would help fade redness and scars and improve skin tone. LASER THERAPY HELPS WOUNDED WARRIORS RECOVER FROM BAD SCARS TREATMENT IMPROVES APPEARANCE AND RANGE OF MOTION g Rice ■■■ Morgan

A February 2014 Congressional Research Service report for Congress stated that between 2001 and 2014, nearly 7,000 U.S. soldiers were killed, and nearly 52,000 injured. Those that come home often bear serious scars that are so severe they are not only unattractive, but can restrict motion and flexibility, affecting a soldier’s ability to perform everyday activities. In response to the need for a solution for all these thousands of soldiers bearing scars, dermatologists turned to laser surgery. What they found benefitted not only soldiers, but other victims of scars and wounds, such as those in car crashes, fires, and work and home accidents.

WHAT KIND OF LASER SURGERY? It’s called “ablative fractional resurfacing,” a procedure that up until recently, was used mainly to treat acne scars and sun-damaged skin. A dermatologic surgeon applies the laser technology to the skin. It removes the damaged, outer layers, while also stimulating the underlying skin. This promotes new collagen production as the skin heals. The result is skin that looks tighter, smoother, and younger.

Recent research, however, has found that this same procedure can be very helpful when it comes to improving the look and feel of scars. In 2012, researchers from the Dermatology Clinic at the Naval Medical Center San Diego reported that they had been using ablative fractional resurfacing since 2009 to treat scars and scar “contractures” (scars that pull skin tight and make it more difficult to move easily). They found that the process worked well with few side effects, helping to improve the look and feel of traumatic scars.

RECENT RESEARCH SHOWS LASER THERAPY WORKS Now, a more recent study shows even more promise for the procedure. Dermatologist Chad M. Hivnor, staff dermatologist for the San Antonio Military Health System, wanted to improve the look and feel of scars that were causing psychological stress for his patients. He found that the laser therapy produced the following results: ■ Loosening of the skin around the scar. This helps improve mobility and flexibility. An example—patients with scars on their faces were able to open and close their mouths after the therapy. ■ Improved appearance. Treated scars had improved color and texture.

Dr. Hivnor noted that the same therapy could be used to improve any scar, new or old, caused by any trauma.

THERAPY HELPS RESTORE RANGE OF MOTION In July 2014, the Armed Forces Benefits Association reported on how ablative fractional resurfacing is helping our wounded warriors. Marine Gunnery Sgt. Brian Meyer, for example, who lost most of his fingers and a leg to a roadside bomb, has experience substantial improvements in his range of motion after going through the treatments. “When the job requires me to hook something and grab it,” he said in an AP video, “now, I can do it. Before, I couldn’t.” Dr. Hivnor received the Air Force Association’s Paul W. Myers Award for his work using lasers to improve skin texture and flexibility in wounded warriors, and is now studying these treatments in injured children, as well. In 2003, the Centers for Disease Control and Prevention (CDC) reported that about 60 percent of all leg amputations occur among people with diabetes. Of these, about 85 percent were preceded by a foot ulcer. The longer you have diabetes, the higher your risk of suffering from a foot ulcer. Diabetic patients need to take this very seriously, to avoid more serious complications down the line. Fortunately, proper care can help reduce your risk.

WHAT IS A FOOT ULCER? A foot ulcer is simply a wound or sore on the foot that resists healing. It may take a long time to heal or keep coming back after it seems that

it was getting better. These types of ulcers are more common in people with diabetes because of two main factors: ■ Vascular disease: Many diabetic patients have vascular diseases

that can affect blood flow into the feet. This can affect healing time. Wounds that do not get enough blood tend to take longer to heal, and can actually get worse over time or become infected. ■ Nerve problems: Diabetics are also at risk for nerve damage. They may lose sensation in certain areas of the feet, reducing their ability to feel when something goes wrong. QUESTIONS TO ASK YOURSELF For those who already have a gynecologist, the question is: Are you happy with him or her? Do you feel like you’re getting the care you need? What is 7-Star Needling? “7-Star,” or plum blossom needling, is a technique similar to gua sha that uses a specially designed needle with seven small tips. Like acupuncture needles, these are very fine, small needles. This technique is often used with early stage cold symptoms, trauma, and long term pain. The practitioner will use the 7-star needle to lightly tap on the affected area, rather than inserting a single needle into the area. There may be slight bleeding. Following treatment, you will be advised to keep the area covered and avoid windy conditions for a day or two. What is Moxibustion? (MOXA) Moxibustion is a heating method of treatment using the Chinese herb mugwort. Moxibustion is used for treating pain, trauma, and certain cold-related injuries or conditions.

The mugwort is compressed into a stick that is burned near the body or into small cones that are placed on the body with a protective barrier that prevents burning. Both these methods introduce heat into the affected area, providing pain relief. There is some limited smoke and odor when using moxibustion. Please advise your practitioner if you have asthma or other respiratory conditions, or if you are sensitive to odors. What is Cupping? Cupping is a technique that is most often used for musculoskeletal pain, such as back pain. In this technique, small glass or plastic cups will be placed on the affected area and suction will be created, drawing some of your tissue inside the cup. Your practitioner may apply a thin layer of oil to your skin and may move the cups around the affected area. There is no bleeding. You may feel a sensation similar to that of being massaged. You may

experience round bruises or bruising over the affected area that may last three to five days. Following treatment, you will be advised to keep the area covered and avoid windy conditions for a day or two. Are There Any Reasons Why I Should Not Be Treated with Acupuncture? While there are no contraindications to receiving acupuncture or other TCM treatment modalities, there may be certain conditions or circumstances that may prevent you from receiving a treatment on a particular day. Some conditions may mean that certain acupuncture points or treatments are cautioned or contraindicated, and your treatment plan will be adjusted accordingly. If you are pregnant or think you may be pregnant, please notify your acupuncturist immediately. Most acupuncture points are safe to use during pregnancy, but there are a few points that are avoided as a precaution.

BLOOD IN YOUR URINE— WHAT DOES IT MEAN? WHE EN YOU NEED TO SEE A DOCTOR ■■■ Colleen M. Storyy Seeing blood in your urine may be startling, but fortunately it’s not usually cause for major alarm. There are many reasons that the urine may be stained red, and most of them are not cancerous. Only a doctor can be sure, however, so it’s important to always get a checkup as soon as possible. If the problem is serious, early treatment can greatly increase the odds of success.

WHAT IS BLOOD IN THE URINE? Medically termed “hematuria,” blood in the urine is a sign that some blood cells have managed to leak into the urinary tract. Where this blood comes from, however, can vary widely. Sometimes, there may be blood in the urine that you can’t see. Called “microscopic hematuria,” it is detected only with urine tests or under a microscope. This type of trace amount of urine is actually considered normal, as an average person with a healthy urinary tract excretes about one million red blood cells via urine each day. “Gross hematuria” describes a condition where the blood can be seen with the naked eye. Usually this type turns the toilet water pale pink, bright red, or smoky brown (like tea), or you may see spots of blood in the water after urinating.

WHAT CAUSES BLOOD IN THE URINE? There are many conditions that can cause blood to appear in the urine. The most com-

mon for those younger than 40 years of age are kidney stones or urinary tract infections. According to the Mayo Clinic, other causes include: Urinary Tract Infections: These may occur as a result of bacteria that enter the body through the urethra and multiply in the bladder. In older adults, the only sign of the illness may be microscopic hematuria. Kidney Infections: If bacteria gets into the kidneys from the bloodstream, or moves up from the ureters into the kidneys, the kidneys may become infected. Bladder or Kidney Stones: The minerals in concentrated urine sometimes form crystals on the walls of the kidneys or bladder, creating small, hard stones. Only the large ones can actually cause blockage, resulting in extreme pain and sometimes blood in the urine. Enlarged Prostate: If this gland, which surrounds the top part of the urethra in men, becomes enlarged, it can partially block urine flow, which can cause hematuria. Kidney Disease: Inflammation in the kidneys can result in urinary bleeding. Inherited Disorders: Sickle cell anemia, a hereditary defect of hemoglobin in red blood cells, as well as Alport syndrome, which affects the kidneys, can both create hematuria. Medications: Some common drugs like aspirin, penicillin, blood thinners, and cancer drugs can also cause blood to appear in the urine. Strenuous Exercise: Athletes who exercise intensely may sometimes experience hema-

turia, perhaps due to dehydration or trauma to the bladder. Marathon runners are particularly susceptible. Cancer: Blood in the urine may be a sign of kidney, bladder, or prostate cancer.

DIAGNOSIS AND TREATMENT Sometimes you may experience hematuria with no other symptoms, but other times a burning sensation when urinating, fever, nausea or vomiting, weight loss, or pain in the side, back, belly, or groin may accompany the bleeding symptoms. It’s important to note all your symptoms, as this will help your doctor diagnose your condition. If you cannot get in to see your doctor right away and your symptoms are getting worse, go to a hospital emergency department. A combination of blood in the urine, fever, and side (flank) pain may indicate a kidney infection, and requires immediate care. Your doctor will ask about your symptoms, your medical history, and your current medications, and then will ask for a urine sample for testing. A urinalysis can show signs that will help diagnose your condition, such as the presence of certain proteins, which may signal a kidney problem, or the presence of white blood cells, which may signify an infection.
