ryan Mineo grew up swimming. “I was on the swim team from the time I was around 4 or 5 years old,” he notes. But at the age of 18, while swimming in a small lake in Texas, he discovered he had an intense fear of open water. “It was terrifying; I never knew I had this fear,” Bryan says. Now he’s an open-water swim coach—plunging into the Pacific Ocean 365 days a year for two to three hours a day. How did he turn his anxiety into expertise? He faced his fear head-on. It all started with Bryan trying to impress a girl. “She was training for a triathlon,” he says. The plan was for the two of them to head out to the lake and for Bryan help her train for the competition. “I swim out, maybe a couple hundred yards, and I start panicking,” Bryan remembers. “I swallow a couple of gulps from the wind and the chop. I’m treading water and hyperventilating. At this point I knew I couldn’t do this.” The jig was up; quickly it became clear that Bryan was the one who needed help. “She swam over to me and mouthed the words, ‘Just breathe.’” Those two words would stick with the young swimmer and later become a big part of Bryan’s coaching approach. This challenge to overcome his fear was new—and a little inviting for Bryan. “I’ve had an amazing life and a really fortunate childhood,” he explains. “I really had no hardships, no family deaths at that point, so I really didn’t know what struggle was. This was the first time I was like, ‘Whoa, this is a challenge for me. Maybe I should approach this; maybe this is a good opportunity.’” So rather than backing down from his fear, Bryan began driving 30 minutes to the lake, four to five times a week. Day one began with him standing, chest deep in the lake. By day two he was treading water. “I didn’t realize it [at the time], but I was creating a progression and a methodology. I’ve refined it, of course, but how I teach now is how I learned to overcome my own fear,” Bryan says. Today, whether he’s coaching one-on-one, leading one of his weekly, year-round Swim Mechanic Ocean Groups (SMOG) of some hundred-plus swimmers in Redondo Beach, instructing a smaller SMOG squad in the pool or coaching remotely online, Bryan is constantly evolving the Swim Mechanic brand. “I knew I wanted to create a community within my field. [Open water swimming] is a really niche thing as it is,” he says. “It started with just me and a few clients. Then it kind of blew up … it’s still so humbling.” With a wide range of clientele—from athletes training to obtain peak performance and master technique to ocean newbies looking to shake themselves of the fear of what’s below—Bryan’s focus is individualized with each swimmer. “Everyone moves really uniquely in the water. For me, it’s about trying to pick up on that and connect the cause and effect—which is not intuitive and not a given with swimming.” But there’s more to it than the physicality. “There’s a deep bond with swimmers,” Bryan explains. “It likely has to do with the lifestyle and the kind of commitment it takes.” Never one to be competitive against others, Bryan is no stranger to challenging himself … be it testing the limits of his temperature tolerance with ice baths, cold showers and year-round ocean swims sans wetsuit, or pushing the distance of each swim from five miles to 10 and beyond. “My addictions are more about the mental thresholds,” he explains. “That’s been the most rewarding thing—to find these thresholds and raise the bar. The biggest takeaway is that it bleeds over into other facets of my life.” This is true for his clients as well. Like a form of swim therapy, Bryan gives people the techniques to overcome their fear … and that doesn’t go away once they’re back on dry land. n

Bryan’s Year in Health Weekday Itinerary: “My life is awesome by the way. You’re going to hate me after I tell you this. I sleep in till 7:00 or 7:15, get a water, juice or coffee and head to the beach. That’s my favorite part of the day. I work with two or three hours of clients. From 11 a.m. to 5 p.m., basically, every day I have open. I go to a lot of coffee shops, I read, I write and then I’ll typically do something active. We [he and his massive Great Dane, Joey] go for a hike every day in PV. And then I may have one or two nighttime clients.”

Daily nutrition: “I’ve been vegan for about four years, and I’m gluten-free. I eat clean, but my vice is baked goods.”

Workout routine: “I’m always moving. I used to be much more regimented, and I didn’t even realize how unhappy it was making me. So now I’m much more about approaching it in a fun way, seeing how can I move my body differently. So I climb, I do Pilates once a week, I do yoga and I hike every day.”

Guilty pleasures: “Bourbon and cookies. (Best answer ever.)”

A fitness craze you want to try: “I feel like I need to try CrossFit. Another thing I really want to do: It’s not really a fitness thing, but I want to go skydiving.”

alvin Vachon was ice skating by the time he was 3 years old, but that’s not surprising since his father and grandfather have long National Hockey League (NHL) associations. Calvin’s dad, Nick Vachon, is a coach and the general manager of the Los Angeles Junior Kings— a considerable step taken in 2016 by the NHL L.A. Kings to oversee the development of the sport in Southern California. “The decision by Kings president of business operations and alternate governor Luc Robitaille and L.A. Kings GM Rob Blake to create a formal association with the Los Angeles Kings is a tremendous opportunity for our youth as hockey grows in Southern California,” says Nick. The Los Angeles Junior Kings now have 28 teams with more than 420 players in the program. Nick grew up in the world of professional hockey as the son of legendary goalie Rogie Vachon, who played for the Montreal Canadiens, Detroit Red Wings, Boston Bruins and L.A. Kings. Nick was drafted by the Toronto Maple Leafs and played for the New York Islanders before transitioning as a coach for the Junior Kings. Now Calvin, age 12, is following in the family footsteps as a goalie for the Junior Kings Pee Wee AAA Major team. When Calvin started playing hockey at age 5, Nick put him in various positions including a forward and a defensive player. “However, by the time he was 8 he had a natural ability and was extremely quick in accomplishing things at the net that kids that age aren’t developed to accomplish, such as splits in saves,” says Nick. Calvin replies, “I always thought that defending the goal was more fun.” When Calvin’s not in school, he’s on the ice. He

Even though they’re playing hockey every day, the players have time to see the city and experience a different lifestyle. “When I go, I try to learn a few new words and try a few new foods,” says Calvin. He might only be in middle school, but Calvin speaks like a seasoned player when he discusses the psychology of playing in elite tournaments. “Hockey is fun, so when I’m on the ice, I’m focused and I think about all the good things that are going to happen.” “Calvin maintains his composure on the ice like a much older player,” Nick says, “because he’s able to give both verbal and physical direction to his defense.” So how does Calvin stay grounded with such a family legacy and so much time committed to hockey? “My mom says if I don’t do well in school, then I can’t play hockey,” says Calvin. Nick and his wife, Renee, spend a lot of time around the sport, but they balance that with daughter Chloe’s dance and gymnastics involvement as well as family time in the South Bay. “In the spring I play baseball, and in the summer I like to hang out with my friends at the beach,” says Calvin. As hockey in Southern California heats up, the paths to the NHL become varied through both junior hockey and college programs. The NHL has had six draft picks from Southern California in the past few years, including three former Junior Kings. “We are fortunate to have a nationally recognized program that develops student athletes at all levels, and we’re committed to instilling teamwork, commitment and a passion for the game, because those are life skills that translate off the ice and are more important than winning and losing,” says Nick. Southern California junior hockey has become so strong, it hosts showcase exhibition weekends. However, under the guidance of Nick as the GM with the Kings involved in the decision-making and development, junior hockey in Los Angeles is being cultivated under an umbrella that is committed to developing young athletes both mentally and physically. “There’s always a lot going on, but it’s all good and fun,” says Calvin. Watch the NHL roster in a few years; the name Vachon will likely be on the back of a jersey.” n

Her priority is simply becoming the best Muay Thai fighter she can be. “I’m focused on self-improvement,” she says. “I want to be an example for younger girls that want to get into fighting”. Janet aims to continue fighting as much as possible before retiring into the next chapter of her life: having a family of her own. n

Born in the Philippines, Charlene grew up in a close-knit extended family where relatives lived in six houses on the same street. Always fascinated with American culture, she moved in with an aunt living in Glendale in 2002 and soon landed a job in Orange County. “In the Philippines, you stay with your parents until you get married,” says Charlene. “It was a big transition to live alone.” She struggled for the first two years but eventually settled in—making friends and working in online sales, search optimization and managerial positions. After getting engaged, Charlene moved from Culver City to her fiancé’s Manhattan Beach home. Running a business keeps Charlene busy, but she makes time for civic engagement. She serves as president of the South Bay Bike Coalition, which works to make biking, walking and driving safer in the South Bay. After receiving the PCOS diagnosis, Charlene focused on her health. She works out regularly and still treks to Culver City for class in Capoeira—a combination of martial arts and dance. She maintains a keto (highprotein, low-carb) diet. PCOS increases sugar levels, and her lifestyle changes have helped bring her levels down. Charlene wants to increase awareness of PCOS, which she had not heard of before being diagnosed. “It’s more common than I realized, but most women only learn they have it when they’re wanting to start a family. Women who are struggling should know they’re not alone.” n

Peter Borden, MD, began his career in 2001 when he joined his father, Donald Borden, MD, who had been practicing orthopaedic surgery in the South Bay for 30 years. With the love and determination of one of our employees, we managed to get him a 10-year VISA to come to the United States and get the orthopedic and medical help he needed. Geral’s smile shines brightly to this day.

WHAT IS LIPEDEMA? Lipedema is an often misunderstood fat disorder in which pathologic fat accumulates in the legs and arms. This fat is resistant to standard weight loss methods and can be very painful. Lipedema patients may also have a compromised lymphatic system (part of the immune system), which is a different disorder called lymphedema.

I have found that lipedema patients are frequently misdiagnosed and poorly informed about their treatment options. Through in-depth

Earlier this year my daughter was diagnosed with Angelman syndrome, a severe genetic disorder. Karen Alfonso, MD, pediatrician, is a mother of two boys. She chose pediatrics because she loves children, and their innocence and honesty constantly remind her of the more important values in life. In our fast-paced, technologically advanced world, it is easy to move forward quickly to address immediately the patient concerns. There are certainly visits that require more succinct, straight-to-the-point evaluations, but then there are moments when it becomes vital to stop the typing, put the computer away, look the patient in the eye and listen. I believe this is still an important part of our physical exam and vital to ensuring we are treating our patients as a whole and addressing their needs appropriately.”

HOW WOULD YOUR PATIENTS DESCRIBE YOUR APPROACH?

WISE WORDS There aren’t many patients who enjoy flossing, so when you get in there it’s critical that your technique is correct. String floss is preferred because it allows better adaptation to remove plaque in between teeth. Once you get past the contact between teeth, contour the floss like the letter C and wipe up and down a few times. Then reverse your C and wipe the next tooth. And if you have something stuck in between your teeth often tying a double knot in the floss will help pass stubborn things like popcorn kernels.

“I’m very easygoing at work. Patients are usually nervous when they go to the dentist; the last thing they need is intimidation or someone telling them what they need without explanation. My approach with patients has always come from the perspective of being their friend. I try to find a way to connect with everyone when I first meet them, and I want them to know I’m their advocate.”

All of our team enjoys giving back to our community in whatever way we can. We have all worked at CDA Cares, a volunteer dental program through the California Dental Association that provides no-charge cleanings, fillings, treatment of infection and oral health education. Locally we donate dental supplies to families in need through community groups such as Sandpipers and Rainbow Services. “My favorite outreach is visiting classrooms for hearing impaired children at Howard Wood Elementary School and teaching them about oral hygiene,” shares Michelle Luciano.

“We view the radiographs/photos/proposed treatment on a TV monitor with the patient. Having a screen to view this on sounds simple, but it is the clearest way for patients to understand what is going on in their mouth. Going into a surgical appointment where the patient knows why we are doing a certain procedure is, above anything else, my best aid in patient education and communication.”

DO YOU FEEL IT’S IMPORTANT TO GIVE BACK AND SUPPORT THOSE IN NEED?

“We do. We support several organizations and events, including the LA25 Foundation for the Arts, CDA Cares and the Neptunian Women’s club. Dr. Brian also makes mouth guards for his alma mater, Mira Costa High School.”

We provide a multidisciplinary and integrative approach for those suffering with acute and chronic pain, addressing their needs on the physical, social, psychological and spiritual levels. Dr. Ripu Arora is board-certified in pain management and anesthesiology. He uses targeted nerve blocks, radiofrequency techniques and cost-effective advanced therapies, like spinal cord stimulation. Our physician assistant, Sheila Arora, holds a Master of Science degree from USC and is trained in pain management. Florentina Alden, our certified integrative nutrition health coach, holds a master’s degree in spiritual psychology. “I became a volunteer at an indigent primary care clinic after a cheerleading injury during my senior year at Berkeley ended my professional dance career. I realized how fortunate I was that my pain limited a very small aspect of my life when I saw patients whose pain prevented them from being an active participant in their own lives. I was excited to learn that there was a specialty that focused on regaining function by decreasing pain with specific procedures for different types of pain.”

I am an anesthesia-trained pain management physician who performs interventional procedures with the goals of increasing function and quality of life, decreasing pain and weaning off narcotic medications. “In the past few years there have been many advances in the field of pain management. I feel the most promising are the technological advances in spinal cord stimulation and the research conducted in regenerative medicine. Stimulators have MRI compatibility, are smaller in size, and the stimulation delivered is much more comfortable for the patients. Studies are being conducted in regenerative medicine throughout the world demonstrating anti-inflammatory effects and possible healing properties of platelet rich plasma (PRP) and stem cells when injected into joints, intervertebral discs and soft tissue like tendons and ligaments.”

I want to educate more people about Botox injections for migraines. It is very effective in decreasing the number of migraines per month so the patient is not stuck balancing under-treating headaches with medication-overuse headaches. For patients who experience more than 15 headache days a month with headaches lasting more than four to six hours who have tried other preventative medications, this could really give them a huge piece of their life back. I love that it is a clinic procedure that takes 10 minutes or less and there is no down time for recovery.

GOOD TIMES My husband and I love exploring the South Bay with our son. We have fallen in love with the area; there are so many amazing parks and playgrounds perfect for play dates. I am constantly on the go and strive for some “me time” at the gym three times a week for yoga, Pilates and weight training.
