MVMA QUARTERLY - WINTER 2018
From the President-Elect The past year as President-Elect has been packed with challenging issues and many miles of travel to represent our state at the national level. I have attended the Leadership, MVMA, Carol Ryan, DVM Heartland, and AVMA conferences and worked on committees to help analyze the problems at large. Our profession is taking a hard hit by the rising suicide rate among veterinarians. Compassion fatigue may engulf individuals that deal with high levels of compassion and empathy. There is stress from juggling work and family life, paying student loans, dealing with difficult co-workers and trying to sell a practice at retirement. Our profession needs a stress management intervention!!! I have made this the primary focus for our next convention. It is time we slow down, BREATHE and focus on our wellbeing. The 2019 MVMA Convention is quickly approaching. It will be held in Columbia January 24-27. It will focus on the

veterinarian, technician, staff and the family and friends that support them. We will have amazing continuing education lectures from nationwide speakers. The large animal and equine practitioners will be intrigued by the top notch lectures. Get your USDA accreditation hours on site. Companion animal lectures will embrace current issues in medicine. The University will help decrease stress to the veterinarian during anesthesia time by offering a dry lab. It will help simplify the use of machines and increase the understanding of principles. Lectures will focus on improving staff morale, increasing profits, decreasing stress and learn how to enjoy your life with your loved ones. Experts will ease the fear of buying in and of selling a practice. Retirement security and paying down student debt are two areas that can increase anxiety. Find out how to plan ahead. We will also host a huge job fair for veterinarians and technicians. Dr. Andy Roark, DVM will be our keynote speaker on Saturday. He is an international speaker and is quite entertaining as he can relate to all age
groups in the veterinary spectrum. His lecture series will hone in on your medical, communication and management skills. Dr. Rick Debowes, 2017 AVMA President’s Award recipient will present on Sunday. He has helped thousands of individuals develop the personal skills, including resilience, communication and conflict resolution that are essential in the day to day roles of a veterinarian He is an amazing, high energy speaker that will spark the positive outlook to help everyone reach their personal best. PLEASE connect with colleagues and classmates and plan to attend the 2019 Convention. There will be time to relax, laugh and learn to love our profession to the fullest. Good Advice from the AVMA & Your MVMA
Remember to Take Time for You. Compassion Fatigue
Stress Management
o you feel emotionally numb or drained at the end of the day? Do you enjoy your work but feel like you have nothing left to give? If so, you might be suffering from compassion fatigue – and if that’s the case, you’re not alone.
tress is normal in the day-to-day life of a veterinarian. From juggling the competing demands of work and family, to diagnosing and treating critically ill patients, to dealing with difficult co-workers or employees, to making ends meet financially while paying off a mountain of student debt and trying to save for the future, we face stressful situations every day. But it is well known that too much stress can have negative effects on health.
D
Undoubtedly, the veterinary medical field attracts individuals who demonstrate a high level of compassion, empathy, and drive to care for others. Overall, veterinarians and other animal caretakers report a high level of satisfaction in their work; the caregiving work that we do rewards us with compassion satisfaction – a joy or sense of achievement found in helping and caring for others. However, repeated exposure to traumatic events (such as abuse, illness, and euthanasia) can lead to compassion fatigue. Compassion fatigue and burnout are similar but not interchangeable. Compassion fatigue – also known as “vicarious trauma,” “secondary traumatic stress” or “secondary victimization” – is the result of a medical caregiver’s unique relationship with a patient, through which empathy allows the caregiver to “take on the burden” of the ill or dying patient. Veterinarians are very much at risk for compassion fatigue. Like other caregiving professionals, they deal with death and illness on a daily basis. They have to deliver bad news to clients, deal with animal cruelty, and see clients struggle to balance financial needs with the needs of their pets. Research shows that veterinarians face ethical dilemmas three to five times per week, and such moral stress is a primary contributor to compassion fatigue.
Symptoms of Compassion Fatigue
Feelings of apathy and isolation are at the top of the list of symptoms of compassion fatigue. But they are far from the only ones. This insidious disorder can cause problems both psychological and physical. Common symptoms of compassion fatigue can include: Bottled-up emotions • Sadness and apathy • Inability to get pleasure from activities that previously were enjoyable • Isolation • Difficulty concentrating • Feeling mentally and physically tired • Chronic physical ailments • Voicing excessive complaints about your job, your manager(s) and/ or co-workers • Lack of self-care, including poor hygiene and a drop-off in your appearance • Substance abuse or other compulsive behaviors such as over-eating or gambling
12
S
The Mayo Clinic suggests four strategies for coping with stress: Avoid • Alter • Accept • Adapt In some instances, you may be able to avoid stress altogether; in others, you might be able to alter your situation. But there may be other times when you simply need to accept things the way they are, or alter your own thinking or behavior.. Taking time every day to relieve stress also is vital. How you do that is up to you, and there are a wide range of possibilities. A good starting point is this stress checklist of basic stress-relief measures that anyone can utilize. Stress management can include meditation, yoga, expressive writing, mindfulness, or affirmations. Dr. Daniel J. Siegel demonstrated this simple breathing exercise proven to increase both physical and mental well-being during a keynote address at the 2016 AVMA Convention. Stress reduction also can come from physcal exercise, playing games with your family, spending time with friends, or finding 15 minutes of quiet time to be alone. Since most of us got into the veterinary profession because we love animals, it can mean taking 15 minutes extra each day to spend quality time with your pets. (We all know the value of the Human-Animal Bond; you can leverage it to help relieve stress during your day!) It doesn’t matter what approach you choose – as long as it works for you. One approach that can help throughout the day is to try to become mindful of moments when you start to feel stressed, and take immediate action to manage that stress. Breathe deeply, take a five-minute break from what you’re doing, or take a quick walk to give yourself a change of scenery. If you’re about to have a difficult conversation with a client or co-worker, take 15 seconds before you walk into the room, and just breathe deeply and prepare yourself mentally. If a situation or someone’s comment has made you angry, try to use positive thinking or the power of humor to reshape your attitude. Learn to recognize the symptoms of stress, and take immediate action when you find yourself becoming tense.
If you are showing signs of compassion fatigue or stress, talk to your peers who have experienced the same feelings, take time for yourself, try relaxation techniques and do a little research at www.avma.org by looking up “compassion” & “stress” in its search bar. There are many more suggestions to help with your well-being.

AVMA Delegate Report
By Carol Ryan, DVM - AVMA Delegate and Chuck Barry, DVM - AVMA Alternate Delegate
O
ur AVMA is a dynamic organization. The AVMA, founded in 1863, is one of the oldest and largest veterinary medical orga- nizations in the world, with more than 91,000 member veterinarians worldwide engaged in the art and science of veterinary medicine. It is hard to imagine where our profession would be without the guiding force of the AVMA. The AVMA Annual Convention was held in July in Denver, Colorado with over 9,000 veterinarians and technicians in attendance. The two days prior to each convention are packed full of organized veterinary medicine. Leaders scrutinize innumerable topics affecting our industry. The AVMA has a representative delegate and alternate delegate from each state. They attend the Regular Annual Session of the House of Delegates along with every allied group that you could imagine which enhances the framework of the veterinary profession. Many dignitaries are present from around the globe. The issues are of great relevance and they reflect our ever changing world. We are truly blessed to belong to such a wonderful profession. Here are a few policies that were laid into action: Foot and Mouth Disease: The AVMA will continue following the devastating effects of the disease and supports the United States government developing a vaccine bank to be utilized in the event of an outbreak. Resolution 5: Promotion of Responsible Breeding of Companion Animals with Inherited Disorders. A resolution was passed Dr. John Howe (center) with Dr. promoting responsible dog breeding and a reduction in heritable problems was adopted as amended Chuck Barry and Dr. Carol Ryan. by the House. Student Debt: Student debt will be analyzed in detail. A motion was approved to recommend to the Board of Directors that the AVMA investigate and promote strategies for private practitioners to assist students and recent graduates in managing the cost of their education with hiring incentive packages. The HOD approved a motion to recommend to the Board of Directors that the AVMA investigate methods to make available financial advisor(s) to help veterinary applicants, students, and recent graduates manage the cost of education. A motion was approved to recommend to the Board of Directors that the AVMA investigate the development of a low interest loan program for veterinary students that is financially sustainable for the AVMA. These were a few on the long list of topics. The delegation elected Dr. Grace Bransford at the AVMA Vice President, Dr. John Howe as the AVMA President Elect and Dr. John de Jong presides as the AVMA President. The AVMA will meet again in January 2019. If you have a concern, question, or idea regarding how your AVMA can better serve our profession, please contact us. Thank you for allowing us to represent the state of Missouri at the national level.
We're thankful for the pets in our lives, and the vets that take care of them.

MVMA QUARTERLY - WINTER 2018
Are you geng ready to sign an equipment lease? Read this first! It is becoming more and more common for vendors to offer various equipment leases and agreements. Some will give credit toward your lease payments if you purchase $XXX in supplies. Others will offer points to use against supply invoices. While some of these equipment leases appear to be a good deal, if you’re planning to sell your pracce during the lease term, big issues could be coming your way. Buyers (corporate consolidators AND private pares) typically purchase only the pracce assets. The debts must be seled at closing and this includes leases. In other words, the money payable to the seller will be reduced by the amount of those debts. In some cases, this can be a significant amount of money. In a recent transacon, my client had an equipment lease with a well-known industry vendor and wanted to sell his pracce. My client was faced with a lease buy-out totaling about $300,000. The lease was somewhat new with binding language unl 2021. Unfortunately, this means the seller will be reducing that total from the sale proceeds, a very significant hit. Deal Breaker? Well, that’s for the seller to decide.
What does this mean for you? Here are a few points to think about prior to signing an agreement: Do you have an exit strategy in place? If so, what is the esmated number of years before you plan to sell? What are the total costs to you if the contact must be cancelled? Did you read the fine print? Some agreements will automacally renew unless you give advanced noce to cancel. Is the equipment necessary? If rerement is in your near future, contrary to what you might think, upgrading your exisng equipment may not make your pracce more valuable. I would be happy to visit more about this subject if you have quesons. Give me a call!

MVMA Proposes Change to Compound Drug Rules Officers of the Missouri Veterinary Medical Association went before the Missouri Pharmacy Board on October 25, 2018 in Columbia to propose a change to compounded drug rules. The proposed rule would allow veterinary clinics to stock a supply of compounded drugs to be administered by the veterinarian in emergency situations.

The MVMA representatives presented support letters not only from the MVMA, but from 115 individual veterinarians, the UM College of Veterinary Medicine, and the KC Pet Project in Kansas City. The Missouri Veterinary Medical Board also supported the change as well as other organizations. At press time, the Missouri Board of Pharmacy was still in deliberation on the measure, but MVMA is very hopeful for a successful outcome. MU-CVM Class of 2020 Celebrates the Start of Clinics
We buy great practices. We hire great people.
(Edited from MU-CVM News)
20 YEARS
The University of Missouri College of Veterinary Medicine celebrated the Class of 2020’s transition into clinical rotations during the annual White Coat Ceremony, held Oct. 14, 2018, at the Missouri Theatre in Columbia, Missouri.
VetCor www.VetCor.com
The MVMA president, Cindy Penrod, DVM and Richard Antweiler, executive director, presented the newly coated clinicians a veterinary cadeusus lapel pin, and on behalf of the Missouri Veterinary Medical Foundation, a name badge for their white coats. MU CVM students spend the first two years of their professional curriculum learning the basic sciences needed to practice veterinary medicine. They then spend 19 months in the clinical portion of their training during which they rotate through a series of required courses and electives. Under the supervision of clinicians, they work in the Veterinary Health Center’s Small Animal, Food Animal and Equine hospitals and the Veterinary Medical Diagnostic Laboratory, as well as undertaking preceptorships in private practices or with public agencies on their way to completing their doctorate of veterinary MVMA president,Cindy Penrod, DVM presents a lapel pin to Kelly Bosco. medicine. The 113 members of the class selected a family member, friend or mentor to present the laboratory coat they will wear during some of their clinical rotations. CVM Dean Carolyn J. Henry could not attend this year’s ceremony, but recorded a video greeting for the event. “Regardless of whether your work uniform remains a white coat, or becomes coveralls, scrubs, a military uniform, or even business attire, it’s truly an exciting time to be a veterinarian, Henry said. “Our role in animal, human, and environmental health has never been more important than it is today and that role changes and expands every year. I’d encourage each of you to be ready to embrace change and the opportunities that change presents. Remember, every good thing that has ever happened in your life happened because something changed. “As part of your clinical experiences in Clydesdale Hall and in the hospital, public and farm settings outside of the CVM, you will also become the ambassadors for our college and of our profession. Never lose site of the importance of this role. 33

he 2018 Missouri Veterinary Technicians Association Conference was held at the Humane Society of Missouri this past October. Attendance was just under 100 technicians! Thank you to all the practices and companies that allowed their technicians to attend! We had several great topics that provided 5.5 CE credits for free to our members. Topics that were covered included “Contagious Disease Prevention”, “Blood Pressure Monitoring Under Anesthesia”, “Why Screen Healthy Pets? The case for Preanesthetic and Preventative Care Diagnostics”, “Alternatives to Extractions”, “Career Options for Veterinary Technicians” and the “Veterinary Nurse Initiative”. We also had several sponsors that helped make it all possible- IDEXX, Dechra, Rolling Acres and Loving Hearts. Planning for the 2019 conference has already begun. MVTA will likely be hosting the conference in the Kansas City area. Exact location in that vicinity is still to be determined. More informa-
The next event that MVTA will be attending is the Missouri Veterinary Medical Association convention. Please encourage your technicians to attend! We will be hosting a technician reception that provides snacks as well as a guest speaker.

There are also a couple Willis, RVT and Whitney Fahrendorf, RVT. of changes happening with MVTA. One of them is that our website is currently in the process of a major overhaul. We are redoing the entire site to make it more modern and to make it where accessing the information needed is as easy as possible. Therefore, in the next month or so the www.movta.org website may be down for that reason. If you have any questions during this transition feel free to e-mail me at whitbrowntf@gmail.com. It is a common clinical problem, with up to 1 in 5 cats over the age of 7 being affected. Hypertension is most often associated with other diseases, chronic kidney disease (CKD) being the most common. Up to 2 out of 3 cats with hypertension also have CKD; when one is not managed well, it could potentially worsen the other. To learn more about SEMINTRA, visit www.semintra.com. Zoetis today announced that the U.S. Department of Agriculture (USDA) has granted an expanded indication for Cytopointยฎ, a novel monoclonal antibody (mAb) therapy for the treatment of allergic dermatitis in dogs. Cytopoint is the first mAb therapy licensed to treat allergic as well as atopic dermatitis, an indication for which it was fully licensed by the USDA in 2016. Administered every four to eight weeks, one injection of Cytopoint works by targeting and neutralizing interleukin-31 (IL-31), a key protein involved in triggering itch in dogs, offering both convenience and sustained efficacy. โ This approval gives veterinarians much needed flexibility for personalized treatment of allergic and atopic dermatitis in dogs with two therapeutic approaches โ the injectable Cytopoint and the oral tablet Apoquel,โ said Thomas P. Lewis II, DVM. Addison Biological Laboratory, Inc is proud to announce the release of their next exclusive home care dental product, MAXI/GUARD Oral Cleansing Wipes. MAXI/GUARD Oral Wipes are both an effective pet oral care product and applicator all in one. Cleansing the oral cavity of pets is much faster and easier since the wipes are already infused with our unique zinc formulation. This taste free compound neutralizes offensive mouth odors, helps reduce the deposition of plaque and aids in gingival inflammation. The wipes are textured to assist in the mechanical removal of plaque, but not so rough to cause damage to the gum tissue. MAXI/GUARD Oral Wipes increase owner compliance and pet acceptance. To aid in the relief due to Hurricane Michael, Purina is sending more than 25,000 pounds of pet food and cat litter to local organizations helping with emergency response efforts for pets in the region. In times of need, Purina works with our national disaster relief partner Rescue Bank, a program of GreaterGood.org, to get pet food and litter to shelters and pet owners throughout affected areas. Purinaโ s donation will further assist with Rescue Bankโ s efforts in the southeast, as well as aid two other local organizations, including the Patriot Emergency Response Team in Panama City, FL and FloridaDisaster.org Division of Emergency Management in Jackson County, FL.

MVMA’s Quality Assurance Program What Is the Missouri Stocker/Feeder Quality Assurance Program? The Missouri Stocker Feeder Quality Assurance Program is designed to meet the quality improvement and pre-conditioning needs of producers, feeders, and consumers. This program educates participants in immunology, animal well-being and the economics of disease. Participation in the program increases producer accountability for quality and safety of the product they sell.
How Does the Program Work? Veterinarians and producers must be certified. Veterinarians are certified by MVMA MSFQAP Committee members. Producers are certified by their veterinarian. Veterinarians and producers learn how they can influence carcass quality and consumer confidence by monitoring where and when injections are administered. Once certified, the producer may participate in the program at one of three levels (white, red or blue tag). The three-level approach allows all producers the opportunity to match their level of management and facilities to the level of participation.
Level 1: Show-Me Select (White Tag) • Participating veterinarians and producers must be certified. • Calves must be properly identified with an approved ear tag. Tag numbers will be listed on an approved certificate. • External and internal parasite control is adminis- tered. • Calves will be dehorned and healed. Calves will also be castrated with a knife and healed or verified to be steers. • Vaccinations: 7-way clostridial, IBR, BVD, PI3,
BRSV, Mannheimia haemolytica with leukotoxoid, (Histophilus is optional.) • Cattle must be born on the producer’s farm. A range of birthdates or the birthdate of the oldest calf in the group must be recorded. • Bull in and out dates will be provided. • Individual calf treatments will be recorded to include date, product, dosage, route of administra- tion and injection site. • Vaccinations must be given no younger than four months of age and at least 21 days before sale.
Level 2: Show-Me Superior (Red Tag) • Weaned 45 days before offering for sale. • Meet Level 1 requirements - NOTE: Initial vac- cinations may be given at an age determined by an attending veterinarian. • In addition, a second round of vaccinations must be given using the following guidelines: • A 7-way clostridial as well as a modified live vaccine containing IBR, PI3, BVD and BRSV must be administer- ed; • Histophilus somni is optional; • Only one dose of Manheimia heamolytica is required if that dose is administered to calves 5 months of age or older; • The time frame between initial and second vaccinations must be a minimum of 21 days and all vaccinations must be completed at least seven days prior to sale; • If a veterinarian determines initial vaccinations should be given to calves less than 4 months of age then second vaccinations must not occur until calves are 5 months of age or older.
Level 3: Show-Me Supreme (Blue Tag) • Meet Level 1 and Level 2 requirements. • All cattle must be tested and be negative for persistent BVD infection. • Genetic information is optional.
For more information, visit the MVMA website at www.movma. org. An online MSFQAP Producer Manual, online veterinarian certification, and other materials are available.
