COLORECTAL CANCER Raising Awareness About Colorectal Cancer Which Colorectal Cancer Screening Test is Right for You? Colorectal cancer is the fifth most common cancer, with nearly 150,000 new cases diagnosed annually in the United States. Twothirds of the diagnoses affect the colon, while a third affect the rectum. And sadly, more than a third of those diagnosed pass away. Colorectal cancer remains the third most common cause of cancer death for women and the second leading cause of cancer death for men. The good news is, by spreading awareness, more people are surviving! Diagnosis typically occurs in one of three ways: • Suspicious signs or symptoms • Routine screening • Emergency admission with gastrointestinal complications Unfortunately, there are no symptoms in most patients with early stage colorectal cancer. Instead these patients are diagnosed as a result of screening tests, which is why it’s important to follow up on those recommended screenings with your primary care physician.
“Colorectal cancer screening is an integral part of every patient's healthcare screening and prevention strategy,” says Millennium Gastroenterologist Dr. Keith Hussey.
Understanding the difference between cancer prevention and cancer detection tests.
According to the American College of Gastroenterology, colonoscopy is the recommended preventive test. The Fecal Occult Blood Test (FOBT), which checks for blood in the stool, and the DNA Mutation test, which is a genetic test are the recommended detection tests. All patients should be offered a cancer prevention test (colonoscopy) first; however, if this test isn’t the right course of care, then cancer detection tests should be used. Colonoscopy is recommended every 10 years beginning at age 50 for patients with average risk, and screenings should begin at age 45 for African Americans. If you have a family history of colorectal cancer, talk with your doctor about further detection options. Patients often ask at what age can they stop screenings. Although there are no

strict rules, the authors of some studies believe once patients reach the age of 80, screening is no longer necessary. However, diagnostic colonoscopy for symptoms or other medical investigation purposes should be individualized to each patient at any age. Cologuard, another screening test, is actually two tests in one. It tests not only for blood in the stool but also for DNA mutation. But this combination test can be problematic because the results do not distinguish which test is positive, and patients with positive results may have occult blood for other reasons and not necessarily colorectal cancer or precancerous cells. * Complete Guidelines can be found online at www.gi.org Bottomline: Ask you primary care provider at your next appointment what screening options are right for you. Could My Diet Be Increasing My Risk of Colorectal Cancer?

Colorectal cancer is largely preventable with regular screening and lifestyle changes. According to the American Cancer Society, there’s no sure way to prevent colorectal cancer. But there are things you can do that might help lower your risk, such as changing the risk factors that you can control. To date, one of the most cited risk factors is diet — referring specifically to poor dietary habits that often lead to obesity. “In fact, research shows that 45 percent of colorectal cancers in the United States are preventable each year through diet, staying at a healthy weight, and being physically active,” says AICR Registered Dietitian Alice Bender. “That’s about 64,000 cases every year.”

So, what does a good diet for preventing colorectal cancer and for aiding cancer treatment look like? 1. Eat a diet rich in fruits, whole grains and vegetables including broccoli, cabbage, cauliflower, kale and other members of the cruciferous family. These foods tend to be high in folate, a B vitamin which has been shown in numerous studies to lower the risk of colorectal cancer. 2. Limit red meat, especially processed meat. Eating too much red meat — like beef, lamb, and pork — increases the risk of colorectal cancer. And processed meats — like bacon, sausage, luncheon meats and hot dogs that are smoked, cured, then treated with nitrates and preservatives — raise risk even more. Try to eat no more

than three servings each week. Less is even better. Get enough calcium and vitamin D. There is good evidence that getting enough calcium and vitamin D can help protect against colorectal cancer. Shoot for 1000 to 1200 mg per day of calcium and about 1000 IU per day of vitamin D3. Some groups recommend testing for vitamin D deficiency, especially in those at increased risk of low levels — such as those living in northern parts of the country as well as the elderly, very overweight, and people with darker skin. Drink alcohol in moderation – one serving per day for women, two per day for men. There is a strong link between higher alcohol intake and colorectal cancer. Heavy drinkers should try to cut down or quit. A serving of alcohol is equal to 12 oz of regular beer, 5 ounces of wine or 1.5 oz of distilled spirits. Chose magnesium-rich foods especially if you are overweight, obese or over the age of 55. Getting adequate magnesium in these groups may reduce the risk of colorectal cancer by 12 percent according to the American Journal of Clinical Nutrition (2012). Adding as little as 4 ounces of fish, a cup of dried beans, 2 ounces of peanuts or half a cup of cooked spinach is sufficient enough to supply those needs. Get regular exercise. Even in moderate amounts, exercise and weight loss is beneficial. Physical activity does not have to be expensive or difficult. The key is to do some form of physical activity that makes your heart beat faster and your lungs breathe more deeply. Lose weight if you are overweight. Being overweight or obese increases the risk of colorectal cancer in both men and women, but the link seems to be stronger in men. So, get to and stay at a healthy weight and avoid weight gain around the midsection to lower your risk.

Why So Tired? Time Change and its Effects on Sleep Cycles As the clocks change for daylight savings time, it is important to remember that almost all animals and plants have an internal body clock that regulates multiple biological processes that take place over the course of about a day. Our internal clock is housed deep in the brain in the hypothalamus. It is very sensitive to the effects of light and darkness. Studies have shown that some people have great difficulty adjusting to the switch to daylight savings time. Springing ahead by an hour is most difficult for those who tend to go to sleep late and wake up later in the morning. The time change in the fall is more difficult for those who tend to go to sleep early and wake up early. Although evidence is mixed, there are some experts who believe that even these minor alterations in the normal sleep-wake cycle can lead to an increased risk for motor vehicle accidents and accidents at work. There is even evidence that there is an increased risk for heart attacks the Monday after we spring forward for daylight savings time. In general, it takes most individuals about a day to respond to a time change of one hour.
Conditions related to alterations in the internal biologic clock are called circadian rhythm disturbances. Common disorders in this category include jet lag disorder when traveling and crossing time zones, work shift disorder in those who work the night shift, delayed sleep phase syndrome seen most commonly in adolescents who stay awake until the early morning and sleep late in bed, and advanced sleep phase syndrome often seen in elderly patients who go to sleep early in the evening and wake up well before they would like to because they have already had a full seven hours of sleep.

All of us require about seven hours of restorative sleep in order to wake up feeling refreshed and maintain good health. It is best to maintain a regular bedtime and set your bedtime seven hours before you expect to wake up. Make certain the room is dark, quiet and comfortable. Turn off all personal electronics for one hour before going to bed and read or watch television in a dimly lit room but not the bedroom for one hour before going to sleep. If unable to get to sleep or return to sleep within 10 or 15 minutes, leave the bedroom and read or watch television elsewhere until tired before returning to bed. Remember, a good night of sleep is vital for good health.

Our Community Partner the Neighborhood Health Clinic Celebrates Ribbon Cutting
On Thursday, February 28, the non-profit Neighborhood Health Clinic in Naples cut the ribbon on an 8,400-foot expansion that will bring advanced imaging capabilities to not only its clients but also Millennium’s patients! The expansion houses comprehensive imaging services, including a brand-new MRI, 64-slice CT scan and X-Ray. Millennium’s Naples’ Women’s Center mammography services will also move into the facility within the year. Millennium, in partnership with the clinic, will help run the imaging component of the expansion. Millennium patients now have access to state-of-the-art equipment, and Millennium radiologists will donate their diagnostic expertise to the clinic’s clients who need it.
