Notify ‘black fungus’ under Epidemic Diseases Act: Centre tells State govts

NEW DELHI, MAY 20 (AGENCIES): India is now officially in the midst of two epidemics after the central government asked states to make mucormycosis a notifiable disease Wednesday. A notifiable disease means the government has to be mandatorily informed about any case that is detected. Among the other notifiable diseases in the country are tuberculosis, cholera and diphtheria. The Union Health Ministry has urged states and union territories to make ‘black fungus’ or mucormycosis a notifiable disease under the Epidemic Diseases Act, 1897, stating that the infection is leading to prolonged morbidity and mortality amongst COVID19 patients. The ministry, in a letter, said that in the recent times a new challenge in the form of a fungal infection namely mucormycosis has emerged and is reported from many states amongst COVID-19 patients, especially those on steroid therapy and deranged sugar control.

Aerosols carrying Covid virus can travel in air up to 10m, says Centre the spread of the novel coronavirus, it says “just as smells can be diluted from the air through opening windows and doors and using exhaust systems, ventilating spaces with improved directional airflow decreases the accumulated viral land in the air, reducing the risk of transmission.” It further advised introducing outdoor air in offices, homes and larger public spaces and said measures to improve ventilation in these spaces must be taken up on urgent priority in urban and rural areas alike. “Recommendations for hutments, homes, offices and large centralised buildings are given. Simple strategic placement of fans, open windows and doors, even slightly open windows can introduce outdoor air and improve the air quality inside. The introduction of cross ventilation and exhaust fans will be beneficial in curtailing the spread of the disease,” it stated. In buildings with central air-management systems, improving central air filtration or increased filtration efficiency is especially helpful when enhanced out-

door air delivery options are limited. The advisory recommended the use of gable fan systems and roof ventilators in offices, auditoriums, shopping malls and other closed public spaces. “Frequent cleaning and replacement of filters is highly recommended,” it said. The release stated that saliva and nasal discharged in the form of droplets and aerosols, by an infected person while exhaling, talking, speaking, singing, laughing, coughing or sneezing is the primary mode of virus transmissions, and an infected person who shows no symptoms also transmit the virus. Notably, the SARSCoV-2 virus infects a human host where it can multiply, in the absence of the host it cannot survive, and stopping the transmission of the virus from a person to another person will decrease the infection rate of the disease to a level where it can eventually die. “People without symptoms can spread the virus. People should continue wearing a mask, wear double masks or an N95 mask,” it added.

letter stated. Mucormycosis, more commonly known as black fungus, has increasingly been reported across the country, especially in diabetic patients who have been put on steroids after testing positive for the SARS-CoV-2 virus. There are concerns also about the quality of water used in oxygen concentrators that are being used in small medical establishments or at home. Many states such as Telangana and Rajasthan have already made it a notifiable disease. “In recent times a new challenge in the form of a fungal infection namely mucormycosis has emerged and is reported from many states amongst COVID-19 patients especially those on steroid therapy deranged sugar control. This fungal infection is leading to prolonged morbidity and mortality amongst COVID-19 patients,” noted Agarwal in the letter. Mucormycosis a rare infection: Mucormycosis is a serious infection but is

usually rare. However, the numbers of cases in India during the second wave of the pandemic are threatening to overwhelm hospitals, especially because when the disease enters a hospital with piped oxygen supply, infection in one patient may quickly spread to all those that are on the same gas line. The infection is often fatal when it spreads to the brain. The symptoms of mucormycosis infection in the lung are often the same as that of Covid-19 — fever, cough, shortness of breath and could aggravate the condition of a patient. “Disseminated mucormycosis typically occurs in people who are already sick from other medical conditions, so it can be difficult to know which symptoms are related to mucormycosis. Patients with disseminated infection in the brain can develop mental status changes or coma,” notes the Centers for Disease Control and Prevention, the national public health agency of the US.

NEW DELHI, MAY 20 (AGENCIES): Cautioning people to maintain social distance to prevent the transmission of the Covid-19 virus, the office of the principal scientific adviser to the Government of India on Thursday said that aerosols, one of the key transmission mode of the SARS-CoV-2, can travel in the air up to 10 metres. “Aerosols and droplets are key transmission mode of the Covid-19 virus, and while droplets fall within 2 metres from an infected person, aerosols can be carried in the air up to 10 metres,” an advisory said. The “Stop the Transmission, Crush the Pandemic - Masks, distance, sanitation and ventilation to prevent the spread of SARSCoV-2 virus” advisory highlights the important role well-ventilated spaces play in diluting the viral load of infected air in poorly ventilated houses, offices and other closes spaces and said ventilation can decrease the risk of transmission from one infected person to the other. “This fungal infection is leading to prolonged morbidity and mortality amongst COVID 19 patients,” Joint Secretary in the health ministry Lav Agarwal said in the letter. The treatment of this fungal infection requires multidisciplinary approach consisting of eye surgeons, ENT specialists, general surgeon, neurosurgeon and dental maxillo facial surgeon, among others, and institution of Amphotericin-B injection as an antifungal medicine. “You are requested to make mucormycosis a notifiable disease under the Epi-

demic Diseases Act, 1897, wherein all government and private health facilities, medical colleges will follow guidelines for screening, diagnosis, management of mucormycosis, issued by the Ministry of Health and Family Welfare (Gol) and the Indian Council of Medical Research (ICMR). “And, make it mandatory for all these facilities to report all suspected and confirmed cases to health department through district-level chief medical officer and subsequently to Integrated Disease Surveillance Programme (IDSP) surveillance system,” the
