Helping those who care for others
A carer is anyone who cares for a friend or family member, who due to illness, disability, a mental health problem or an addiction, cannot cope without their support. Care4Me is an exciting new scheme that provides a range of services to Gedling Homes residents who are caring for someone, or are cared for. Care4Me aims to provide a range of options to make the role of being a carer a little easier.

The services we offer broadly fall into one of five categories. Health & wellbeing support; home modifications; social inclusion; practical support and access support. If you are feeling isolated why not join a local health walk in the area to become more active? Or you could attend our Men in Sheds drop-in sessions to work on your carpentry skills over a cuppa. Perhaps you would like to become more IT literate so you can order shopping or do banking online? If you or the person you care for has a long term health condition such as stroke, COPD (chronic obstructive pulmonary disease) or heart problems, there are classes that can help. We can also help with simple home exercise and advice to help prevent a fall. The Young Person’s Positive Moves programme is also available for 8-15 year olds who are inactive and have conditions such as autism or ADHD (attention deficit hyperactivity disorder). Did you know that if you believe your home needs adapting to meet your needs, we can also arrange this?

We also have access to transport services and respite care (we can arrange to have a qualified care provider look after the person you care for) to enable you to access our services. We can provide manual handling training to help prevent injury.
You may be looking for some financial help or advice. We can also help you with this.
If you are interested in any of the services on offer, please complete and return the Care4Me referral form section of this leaflet. We will then contact you with the relevant information. Care4Me will be in touch to see how the service has benefitted you three, six and nine months after the first contact.

I am interested in Health & Wellbeing Support: This support focuses on services and information to help you as the carer or person being cared for with your own health and wellbeing. Home Modifications: This service looks at adaptations that you may need to make to your home to make things easier for you, such as grab rails or pull-cord alarms. Meeting People: By accessing this service you will be able to look at joining group activities, becoming a volunteer and attending events for carers. Practical Support: It’s important to get the right advice around money matters, so we can help put you in touch with the right people. This support includes information on carers assessments, personal budgets and respite breaks. We can also help you gain training and access courses which could make things easier and more practical for you. Anything else (please tell us)
I may need to access the following help in order to attend the Care4Me services: Transport
