Introduction The Department of Education has devoted itself into making the Philippine Educational System level up to global standards. Don Carlos Village Elementary School (DCVES), our beloved school, has always supported the goals of the Department. All our efforts from the crafting of our programs, policies, guidelines and standards all the way up to the actual delivery of knowledge - are aimed at bringing out a positive change in the current educational setting especially in the implementation of the new K to 12 program. With the fast changing times and the broadening globalization, the challenges that we now face in realizing these goals have become more complex that no single sector can resolve alone. Being co-equal stakeholders, all of us must now work together in the true spirit of multi-sectoral cooperation in order to overcome these obstacles. We have painstakingly compiled this School Report Card covering the years 2013-2017 to provide all of us a snapshot of the school’s current condition and performance. It is our fervent hope that you will share with our successes and understand our predicaments. With a good realization of the facts, we want you to be even more engaged in making our school a better learning place for the students.
Projects FEEDING PROGRAM DCVES School Feeding Programs such as SBFP (School Based Feeding Program) & SFP (School Feeding Program) was designed to provide children good nutrition, health services and support their academic and non-academic performance all year round. The school believes that Feeding Programs will help improve the nutritional status of the learners who were undernourished as well as learners who are at risk, players or athletes and representative on different academic and non-academic which will help improve their performance in the school. Inline with these goals, DCVES achieved 80.00% of their POST Nutritional Status or (1,470 out of 1,839) of the learners who fall under the Normal health status for the S.Y 2016-2017. These achievements were made possible because of the continued support of the DepEd fund or SBFP in which they’ve continued the fund started on June –September 2016 and SFP Canteen Fund in which they’ve started on first week of July – March 2017. Moreover, DCVES Feeding Program will still be in action in supporting the learners in their needs through the cooperation of the different feeding organization and the guide of our Almighty God.
Let us learn from the lessons of the past. Let us work together for an even better tomorrow. KAYANG KAYA PAG SAMA SAMA! MABUHAY TAYONG LAHAT! MABUHAY ANG DCVES! Page 2

DEWORMING Deworming helps the children for better absorption of nutrients in the body, avoid anemia and more active physically and mentally. Deworming was conducted by our school nurse Jenifer L. Lago. There were 1246 or 65.75% pupils dewormed out of 1895 school children for the first dose while the second dose that was conducted by school nurse Esnairah T. Diamla, there are 1221 or 64.43%. There is 1.31% decreased from the first to second dose.

It can be gleaned from the table that there was an increase of about 4.39% in the enrolment of 2015 over that of 2014. An increment of 6.32% can be seen in the following year. The latest year recorded a .99% of increase in the number of learners enrolled. The increase in the enrolment can be attributed to the new school building, announcement & campaign during graduation, parade around the community for early registration and strong partnership with the barangay / community.

Projects ECO-SAVERS PROGRAM To lessen the garbage waste in our school, we segregate the waste through Materials Recovery Facilities. Every room have their trash can for biodegradable and non- biodegradable. The recycled materials were pet bottles, paper, carton and sell it to junk shop for our fund and have a project like urban gardening and gulayan sa paaralan. The project BMW of the Grade IV teachers as the lead team has contributed a lot in sustaining a clean learning environment at the same time the children were able to learn from the sale of the materials recovered.

The table shows that at the start of the school year, the number of Severely Wasted and Wasted learners were 83 & 134 respectively. By the end of the School Year the SW was reported to be 60 and the W was 12. There is significant decrease in the number of malnourished learners as reflected in the pre and post weighing of the children. The feeding program – SBFP and SFP sponsored feeding caused this better improvement in the nutritional status of the learners. Out of 217 learners who were malnourished 128 of them were being fed by SBFP (DepEd fund) and SFP Canteen fund) wherein 78 were under the continued funds of SBFP last school year and 50 were sponsored by the canteen fund. In the primary grades 48% (889 out of 1839) of the learners fall outside the normal health status. On the other hand, only 31% of the Grades 4-6 learners fall outside the normal health status. REMEDIAL READING PROGRAM The Remedial Reading classes at DCVES has been organized on July, 2016. The program was designed to help slow-reader and non-readers children and those who belong to the frustration level in their reading ability. They were classified according to the level of their ability by using different assessment tools such as dolch basic sight words, letter recognition, Phil-IRI and through recommendation by their respective adviser. There were 90 pupils enrolled in this program. To motivate the pupils to attend the sessions they were made beneficiaries of the School Feeding Program. It was successful program because every grading period the pupils improved their reading abilities. At the end of the year, the pupils were already readers. Most of them fall under the C2 and C3 classification.
A number of manipulative materials were provided by the LSB through the SEF for the kindergarten learners. This year, each kindergarten teacher received a projector with a white screen for ready use in their daily delivery of instruction. Page 6

The drop out rate of the school has decreased from 4.88% last year to 3.17 this year. Through LIS the children were properly monitored if they continue schooling or not. Drop outs was reduced through parents meeting, home visitation, monitoring of pupils attendance and early identifying of PARDOS. The main cause of dropping out from the school was family problem. It greatly affects the education of the children. CI interventions must be employed to attain 0% drop-out rate in the succeeding years.
