Call for mental health support for kids with asthma A new study has found that young children with severe or persistent asthma are at higher risk of developing many common mental health problems. The research, a collaboration between researchers at Perth’s Telethon Institute for Child Health Research and Columbia University in New York, has been published online in the top international journal Psychological Medicine. Telethon Institute psychologist and report co-author Dr Monique Robinson said the findings build on previous studies which have found that as the severity of asthma increases, so do problems such as anxiety and depression. “We were interested in understanding the link between asthma in early childhood and mental health problems later on as little is known about the relationship,” Dr Robinson said. “We looked at whether the link was present for mild as well as severe asthma, and whether the link depended on asthma symptoms being persistent throughout childhood as opposed to asthma that lessens as the child grows older.” The study used Western Australian data from the Raine Study to determine whether children who had asthma at age 5 were vulnerable for later mental health problems through to the age of 17 years. The research team found that having asthma at age 5 was associated with a higher vulnerability for the later development of problems such as anxiety, conduct problems and affective problems.

When the children with asthma were separated into groups depending on the severity of their condition, children with mild asthma were no different to those without asthma in terms of mental health outcomes, but children with severe or persistent asthma were seen to be the most at risk of future mental health problems. “The link with mental health was not present when children had asthma early in life but grew out of it by later childhood. However, children whose asthma developed later in childhood were at risk for internalising mental health problems such as anxiety, depression, somatic problems but not externalising problems like behavior issues,” she said.


“We did find that as children got older, the likelihood that they would experience a mental health problem decreased, perhaps indicating that as children get older they are better able to adjust to their asthma without experiencing psychological difficulties.” Dr Robinson said that it probably wasn’t asthma itself that caused mental health problems, but rather the added challenges for the child of dealing with a chronic disease. She said the study supports the need to assess psychological functioning as part of routine care for children with a chronic or severe disease, including for those with severe and persistent asthma throughout childhood. “It also indicates the difficulties involved for children trying to manage and adjust to asthma, and it may be useful to provide social and emotional support or coping skills training for these children to help maintain positive mental health despite their illness,” Dr Robinson said. “This may also lead to better asthma control given good mental health is linked with better self-management of asthma.” The Raine Study The Raine Study is jointly conducted by the Telethon Institute for Child Health Research and The School of Women’s and Infant’s Health at the University of Western Australia. The study started in 1989, when 2900 pregnant women were recruited into a research study at King Edward Memorial Hospital to examine ultrasound imaging. The mothers were assessed during pregnancy when information was collected on the mother and the father, for example diet, exercise, work, health, etc. The research team at the Telethon Institute for Child Health Research have assessed the children at 1, 2, 3, 5, 8, 10, 14, 17, 20 and now 23 years of age. myCompass Self-help for mental health problems is in the palm of your hand Mental health problems are more prevalent in the community than most people realise. Anxiety-related and mood problems are particularly common, affecting approximately 20% of adults in any given year, and 50% of adults during their life time. Over and above their psychological impact, people with anxiety-related and mood problems experience significant physical, personal, social and work-related impairment. Not surprisingly, mental health in the workplace is increasingly a concern for employers and employees in Australia. This is particularly relevant for people working on the front line of healthcare where there is potential for stressful or traumatic situations. Although many people are able to manage their problems without them affecting their work, some people will require support for a short 10

period of time and may benefit from strategies aimed at building resilience and well-being. As the primary caregivers in frontline medicine, nurses are positioned to assist patients with both their physical and mental health problems. It is common for people experiencing illness or injury to have a corresponding decline in mental health. In these situations, it is important for nurses to have an understanding of what resources are available. What is myCompass? myCompass is the Black Dog Institute’s new and innovative self-help program for people with mild-tomoderate depression, anxiety and stress. The myCompass


program combines mobile phone and internet technology to help people better self-manage their symptoms. It does this by assisting people to actively self-monitor the thoughts, feelings and behaviours that are troubling them, and providing 24-hour access to activities and resources designed to build resilience and promote positive wellbeing. myCompass is an interactive self-help service that aims to promote resilience and wellbeing for all Australians. myCompass is a guide to good mental health – it points you in the right direction. You can track your moods, write about them and view information and tips. You can also choose to do one of the modules designed to help you manage mild to moderate stress, anxiety and depression.

moderately depressed and anxious are ideal candidates for myCompass. What equipment does a person need to have? Ideally, to reap the greatest benefit, a person should have access to a personal computer (with their own e-mail address) and an internet-enabled mobile phone. This will mean that a person will be able to receive self-monitoring reminders, motivational snippets, and useful tips and facts to both their e-mail and mobile phone, as well as complete their self-monitoring on either device. Nevertheless, myCompass can be used on either a computer or internet-enabled mobile phone only.

How is myCompass accessed?

What are the benefits of using myCompass?

myCompass can be found at www.myCompass.org. au. Registering to use myCompass is easy and involves setting up a unique access password (that helps ensure the security and privacy of a person’s information) and answering a brief profile questionnaire. On the basis of responses to the questionnaire, myCompass will make recommendations about the thoughts, feelings and/or behaviours that a person is likely to benefit from monitoring, and will suggest appropriate interactive modules for a person to complete. Alternatively, myCompass users can select their own monitoring dimensions and modules.

myCompass is accessible 24/7, via a mobile phone or desktop computer. The program provides assistance and support where and whenever it is needed.

Who will benefit from myCompass? myCompass is not a “quick fix”. For the best results, a person needs to commit to daily self-monitoring of their thoughts, feelings and or behaviours (this takes about 1 minute and can be done on a computer or mobile phone), and complete skill building modules and activities on their computer at least twice-weekly for 7 weeks (about 10 minutes each time).

People who commit to using myCompass for a period of 7 weeks can expect to better understand the contributors to or “triggers” of their low mood, stress and/or worry and be able to more effectively manage these. They will also experience a reduction in their mental health problems. Sticky Situation Professor Peter Little targeting the way cholesterol sticks Professor Peter Little AM has set his sights on a big target: reducing the number of deaths claimed each year by the world’s biggest killer – cardiovascular disease. Atherosclerosis, which leads to blocked arteries, heart attack and strokes, is the chronic inflammatory process that underlies most cardiovascular disease, the largest single cause of premature death in developed – and now developing – countries around the world. Little hopes his team’s groundbreaking research into targeting ways to stop cholesterol from sticking to blood vessel walls will eventually be turned into drugs to treat and prevent heart attacks and strokes. But he admits it’s a long road from their promising mouse studies, which show drugs reducing the development of cholesterol deposits by 50 per cent, to getting the drug on to shelves. Little says turning a promising compound into a manufactured drug costs about $800 million and takes at least 10 years, involving extensive human trials. Still, he’s encouraged by a study published in February by New York’s Stony Brook University showing that treating people who have not yet had a heart attack with drugs for high cholesterol and blood pressure is just as effective as implanting stents through an angiogram to open up blocked heart arteries – now the major activity of all cardio hospital departments in Australia and the US. Each year 22,350 stents are performed around Australia at the cost of about $18,300 each. Professor Peter Little, Founding head of RMIT’s new pharmacy department 12

Little says the current gold standard for drugs in this area – cholesterol-lowering statins – prevents only 30 per cent of heart attacks.


But if these were used in conjunction with drugs developed out of his team’s research, many more patients would be kept from the operating theatre with a treatment that would be cheaper and just as effective as stents.

Since the Vietnam War, studies have discovered that young men are already showing early signs of atherosclerosis, particularly in the West with its growing trend towards sedentary lifestyles, obesity, high cholesterol and blood pressure. More cholesterol deposits on vessel walls over time, increasing the chance of these deposits rupturing, which causes a blockage and a heart attack. If this happens in a blood vessel in the head, it will cause a stroke. Little’s research lab team, funded by the National Health and Medical Research Council, has been investigating how cholesterol in the bloodstream deposits onto vessel walls due to the sticky nature of molecules in the wall called proteoglycans. “We are targeting the initial response – the cholesterol sticking to the vessel wall – so if you can slow that down you’ll slow down the later stages of the disease and you might then delay a heart attack by 10 years,” he says. Professor Wenhua Zheng, who runs a neuro pharmacology lab at Sun Yat-sen University in Guangzhou, China, works on neurological diseases such as Alzheimer’s. He has been collaborating with Little over the past four years and their shared interest in signalling pathways has led to them authoring eight papers together.

“Little’s work is important for understanding, and eventually for treating and preventing, atherosclerosis,” Zheng says. “This is critically important for the new treatment of cardiovascular diseases.” Little spent 20 years running a research lab at the prestigious Baker Institute and was awarded an Order of Australia in 2007 for his work in diabetes. He is also the founding head of RMIT’s new pharmacy department, which opened its doors early last year. 13

Carers WA Educational workshops supporting family carers Carers Australia is the national peak body representing Australia’s 2.6 million unpaid family and friend carers. Carers Australia advocates on behalf of Australia’s unpaid carers to influence policies, programs and services at a national level. They also manage a range of national programs that support unpaid carers in their caring role. Part of the National Network of Carers Associations, Carers WA is a not for profit, community based organisation and registered charity dedicated to improving the lives of the estimated 310,000 family and friend carers living in Western Australia. Carers WA is recognised by both State and Federal governments as the voice of carers. 14

Who is a carer? A carer is a person who provides ongoing unpaid care to someone with a disability, chronic illness, mental illness or who is frail. Carers may range in age from as young as eight through to people well into their nineties. More than one in eight Australians are carers; of these, most are providing either substantial or full time care. Two thirds of carers are women and most carers provide care for a parent, partner or child. Carers WA help carers in any caring situation, including support for people who may have a loved one who lives separately or who is in a care facility.


The role of Carers WA is to work in active partnership with carers, persons with care and support needs, health professionals, service providers, government and the wider community to achieve an improved quality of life for carers as well as offering services to carers. Membership is FREE and carers can call 1300 227 377 (Western Australia) with no referral required. Alternatively, service providers and health care professionals can refer carers to Carers WA (with the verbal consent of the carer) by phoning 1300 227 377, emailing referral@carerswa. asn.au or visiting the website www.carerswa.asn.au. On the website there is an ‘interactive PDF referral form for hospital staff,’ under the ‘Publications’ heading. Carers WA provide a number of services; please note that these services are for people living in Western Australia. Some programs are national, please see the website, www.carersaustralia.com.au for specific programs for your state or territory or call your local carers association for further information. Carers WA Services Counselling from Carers WA is offered in three different ways; face to face, phone and email. Face to face sessions are held at the Perth head office and in some outer suburbs and larger regional centres; they are low cost. Phone counselling (between 8am-5pm Monday-Friday) and email counselling are free and can be accessed by calling 1800 007 332 or emailing chat@carerswa.asn. au. We also offer ‘care and share’ morning teas for group sessions to talk about particular topics. Need advice on a caring issue? If carers are feeling overwhelmed by the services available to them, our Advisory team may be able to assist with their options and point carers in the right direction in getting their issue/s solved. This is a free service, Monday-Friday, 8.30am4.30pm.

Finally, Carers WA has a hospital based program called ‘Prepare to Care’, which is funded by Home and Community Care. Carers WA staff (who have a nursing background) provide education to hospital staff about the Carers Recognition Act 2004, how to better engage with carers and also provide resources for hospital staff to distribute to carers. A hospital admission can be a very stressful time for patients and carers alike and the ‘Prepare to Care pack’, has a place for carers to record information they receive and things to consider before leaving hospital so that a discharge plan can be made with the carers input. It also contains useful information and contacts for during and after the hospital stay. To assist nursing and allied health staff understand the importance of including and engaging with carers, Carers WA has developed a study day. The study day is entitled ‘Family and friend carers; A holistic and practical approach for hospital staff.’ The study day looks at the ‘Prepare to Care’ program in depth and also explores the realities of carer’s lives through a hospital admission and beyond. This day is open to any clinical or allied/other health staff who have the ‘Prepare to Care’ program in their workplace. Previous participants have said; “Fantastic day. Best study day that I have been on!” “Even though I was well aware of this program I found it very informative and now feel very energised to ensure my carers are fully informed also.” This study day is also endorsed by the Royal College of Nursing Australia and earns nursing staff six continuing education points for the low cost of $25. The next study day will be held on Wednesday 14th November 2012 at the Carers Centre in Perth. Educational Workshops for Carers Carers WA run a series of FREE education workshops throughout the year, both metro based and in regional areas. The workshops provide essential information to assist people in their caring role. The University of Queensland Offering CPD to Nurses, Midwives and other Health Professionals What do Nurses, Midwives and all other Health professionals have in common? We all need to undertake continuing professional education. As the need for continued education and training continues to impact registration requirements for Australian nurses and midwives, education providers are tackling the challenge head on. The University of Queensland School of Nursing and Midwifery is responding to recent legislative changes requiring all nurses and midwives to engage in a minimum of 20 hours of Continuing Professional Development each year. In late 2011, the School launched the first in a series 16

of professional development workshops for health professionals. These courses now range from one day seminars, clinical skills workshops, and short courses which once completed, will enable you to meet the requirements for CPD and registration and may earn you points for credit towards postgraduate qualifications. At The University of Queensland there is a vast array of CPD activities that will meet your needs for mandatory professional education. We even offer a course that shows you how to turn all your learning activities into CPD hours, and some of these activities do not require you to leave your own home! Fiona Wake, RAHC Clinical Manager, says the online e-learning modules are available free of charge, and are an important part of RAHC’s contribution towards improving the health outcomes of Indigenous Australians. “The e-learning modules are available to any health professional that’s interested in Indigenous health, not just those going to work in remote communities,” said Ms Wake. 18

“The beauty of our e-learning modules is that they’ve been developed by a range of remote health specialists to provide high quality learning materials and a realistic overview of what really happens in the field.” The e-learning modules available include an introduction to Indigenous health, chronic conditions management, paediatrics, maternity, mental health, medical emergencies, trachoma, ear health and oral health. Importantly, they can be used by any health professional, not just RAHC employees, as part of their ongoing professional development. “We get great feedback on our training. People come away so grateful for the knowledge gained which they can


apply to their scope of practice either in an urban setting or remote. “The training RAHC provides is unique to anything else that’s available in the health sector, and the fact that it’s free is such a bonus.” In addition to online training, RAHC placements starting work in the Northern Territory take part in a two-day cultural and clinical orientation when they first arrive. Conducted in Darwin and Alice Springs, the orientation covers vital information about the key cultural and clinical issues experienced in remote Indigenous communities.

“It’s very hard to prepare someone for the cultural experience that they’re going to have, as well as the complexities of the clinical challenges they might be faced with, so these training days along with the various online training modules provide a great deal of information and support,” Ms Wake said. Healthy recipes... Banana, Oatmeal & Chocolate Chip Cookies What you will need: • • • • •

2 ripe bananas 110g/ 4 ounces rolled oats 80g/ 3 ounces chocolate chips 55ml/ 2 fl ounces flavourless oil i.e. canola, vegetable 1 tsp vanilla extract * Makes about 15 cookies

Method: (1) Preheat oven to 175 degrees C/ 350 degrees F. Line baking tray with a baking paper and set aside. (2) Mash bananas in a large mixing bowl with a fork. Then add chocolate chips, rolled oats, vanilla extract and vegetable oil and mix until well combine with a fork. Set aside for 15 minutes. (3) When you just mix the ingredients together, the mixture is a little watery. By letting the mixture sit for at least 15 minutes, it will allow rolled oats to soften and the mixture to become thicker and stickiers – to be able to shape the cookies. (4) 15 minutes later, use a dessert spoon and the fork to drop teaspoonfuls of mixture onto the baking tray. Use the back of the spoon to flatten the cookie dough slightly as the mixture doesn’t expand much. (5) Bake for 15 -20 minutes or until slightly brown. The cookies are very soft when they are removed from the oven. Cool on tray and the cookies will harden when they are completely cool. 34


Smoked Salmon, Avocado and Rocket Salad What you will need: • • • • •

Rocket leaves (arugula), about 60g (2 oz) An avocado 1/2 of a full sized lebanese cucumber 100g (3 oz) smoked salmon (used Tassal brand) 1 tbsps sesame seeds

Dressing: • • • •

1 lime about 1/2 cup of olive oil (adjust to your preference) Salt to taste A drop of sesame oil

Method: (1) ) In a small dry pan, toast 1 tbsp of sesame seeds over medium flame until lightly brown. Set aside. (2) Wash and dry the rocket leaves. Set aside. (3) Slice cucumber. Set aside. (4) Halve a lime and squeeze the juice into a small bowl. Then add olive oil, sesame oil (if using), salt to taste and 1/2 tbsp of the toasted sesame seeds. Whisk until it emulsifies. (5) In a salad bowl, add rocket leaves, cucumber and hand torn smoked salmon. (6) Use an avocado slicer to slice the avocado. To avoid the avocado from turning brown, I dip slices of the avocado in the dressing and add into the salad bowl. Scatter the remaning toasted sesame seeds as a finishing touch. (7) Serve with the dressing separately.
