Kate and the girls from 3 East ward were raising money on their pink hair fund-raising day for the Leukaemia Foundation. Modbury Hospital was founded in 1975 and is part of the Central Northern Adelaide Health Service. Over this time the hospital has provided thousands of families across the north-eastern suburbs with a wide variety of health care services, including maternity and paediatrics, mental health and emergency. My Saudi Adventure by Emily
S
o why Saudi Arabia? That was the question on everyone’s lips when I told them I was going to throw in my “normal” life in Australia for a year in a far off country shrouded in exotic mystery and allure.
I had come to a time in my life where I needed to break free of the humdrum routine of the everyday and satisfy my craving for adventure and excitement. Flipping through a nursing careers magazine I came across the CCM Recruitment advertisement for contracts in Saudi Arabia. I called CCM and the recruitment officer gave me a complete and thorough overview of what to expect from my Middle Eastern sojourn, including details about the work involved, lifestyle, and cultural changes.
CCM’s continuous assistance with the application, medical, and visa processing enabled the process to be quick and hassle-free. So within three months I was waving goodbye to Oz and embarking on my Big Adventure. Almost three years later (and with no
plans to leave!) I can confidently affirm that my time here in the Middle East has been the most rewarding of my life. It isn’t unusual to be at one of the multitude of parties, balls, and functions that are held every weekend and be introduced to twenty different people, all different nationalities, from engineers to doctors, corporate managers to military personnel. Being literally in the ‘middle’, travel to Europe, Asia, and everywhere else from Saudi is easy and accessible. The social scene here is thriving, and I have been fortunate enough to make amazing lasting friends, whom I have travelled with and can share my experience with me. I came to Saudi Arabia knowing no-one and perhaps a little nervous of what lay beyond the horizon, but when I leave will take with me an irreplaceable experience, lasting friendships and a broader and wiser outlook on life and culture. 13

See the world from the Arabian Gulf An occasional contribution from Chris Kohleis, Recruitment Director of Promesse Hi everyone I’ve often said that working in the Arabian Gulf offers great opportunities to travel but some nurses have asked me why it’s so great. After all, wouldn’t the same apply if you worked in London for example? Both places are a lot closer to attractive holiday destinations in Europe than Australia or New Zealand. There’s a combination of reasons that so many nurses are able to have such great holidays while working in the Arabian Gulf states. The first is that when working in the Gulf you can retain around 80% of your salary after you’ve paid all your day-to-day expenses – there’s no tax and the hospitals provide for free most of your high expense items. You just can’t do that working either at home or in the UK or US. The money you retain is yours to dispose of as you please, to use for travelling, for some special purchases, for saving or any combination. In other words you can splurge out on a fabulous holiday and still have money left over. It’s great to have the funds to spare, but if you can’t get the time off, you still can’t go on that fabulous trip. In the Gulf, you’ll generally have around a hefty 7 weeks of annual leave per year. Also bear in mind that when you’re working, the shifts for most nurses are 12 hours which means you end up working about 7-8 days every fortnight. This means that you can take short breaks of 3-4 days without touching your annual holidays. You have to be sensible though – getting the days off you want obviously depends on your roster so you need to plan in advance. All the same, many of our nurses use this time to take regular short break holidays. I even went to the South of France for a long weekend. Flight times from Gulf locations to the Eastern Mediterranean are about 3-4 hours , Rome 4-5 hours, Paris & London about 7 hours, Nairobi 5 hours and Mumbai 3 hours. And short trips to holiday resorts such as to Jeddah for diving on the Red Sea are often only an hour away . The Gulf is a very competitive travel hub for connecting Europe, Africa and Asia and offers low cost fares and holiday packages in local currencies which are unaffected by exchange rate fluctuations – you are paid in the local currency and buy your travel in this currency. Finally the other interesting thing about working in the Gulf is that you will be working with a highly international workforce with colleagues from across the globe. Many of our nurses have developed life-long friendships with these nurses and joined them on their visits home – there’s nothing like a local to show you around. The breadth of your travel options is huge and where you go is only limited by your imagination. Most people have opportunities to travel like this only very few times in their life so it’s important to make the most of it. Regards and happy travelling

Remember to register with the Nursing and Midwifery Board As of the 1st of July 2010, all Nurses and Midwives across Australia will have to meet the same requirements to be registered. Following this date, a single registration will be recognised in all States and Territories within Australia. The NMBA has provided an FAQ available to download from their web site which we have reprinted below. More detailed information is available on the Nursing and Midwifery Board of Australia website (www.nursingmidwiferyboard.gov.au).
FAQ: Registration as a Nurse or Midwife 1. Are there one or more registers of nurses and midwives? Under the national scheme, there is a register of nurses, and a separate register of midwives. On the register of nurses there are two divisions: • Registered nurses (Division 1) • Enrolled nurses (Division 2) 2. What endorsements are available in the new scheme for nurses and midwives? There are a number of endorsements available for nurses and midwives in the new scheme. A summary of these endorsements is as follows: • Scheduled medicines: This endorsement identifies practitioners qualified to obtain, supply and administer schedule 2, 3,4 & 8 medicines for nursing practice in a rural and isolated practice area. • Nurse practitioner: This endorsement is available to registered nurses (Division 1) based on experience in advanced nursing practice in a clinical leadership role in the area of practice in which the registrant practises as a nurse practitioner. • Midwife practitioner: Any midwifery practitioner registered today will transition into the new scheme. 3. I am an enrolled nurse and currently have an endorsement or authority to administer medications. Why don’t I have an endorsement in the new scheme? In the new scheme, you have authority to administer medicine, unless there is a notation on your registration to say that you are not qualified to undertake this practice. 4. I currently have an endorsement for an area of practice. Will that appear on my registration in the new scheme? If you currently have an endorsement for one of the following areas of practice, you will no longer have a specific endorsement in the new scheme: • Maternal and child health nurse • Sexual health nurse • Psychiatric nurse

• Disability nurse • Critical care nurse There is no specific provision for these areas of practice to be recognised in the new scheme. However, you may continue to practise as you have before. 5. I currently have an endorsement or authority to administer immunisation medicines. Will I have an endorsement in the new scheme? You will be able to continue to administer immunisation as you do now in accordance with relevant State and Territory legislation, but there is no specific endorsement for this purpose in the new scheme. 6. I note that there is a requirement for my principal place of practice to appear on the public register. Is this essential? It is a requirement of the National Law that the suburb and postcode of the registrant’s principal place of practice appear on the public register. The principal place of practice is defined as the address from which you predominantly practice the profession, or if not practising, or not practising predominantly from one address, then your home address. For nurses and midwives practising through an agency, it can be the address of the agency. 7. I have conditions on my registration, but they have never before appeared on the public register. Is it essential they appear on the public register under the new scheme? It is a requirement of the National Law that conditions appear on the public register. However, a National Board may decide in the case of a health or impairment condition that there is no overriding public interest in the condition appearing on the public register. Any registrant in this situation should simply write to the National Board to request that a health or impairment condition not be published on the national register.

BEFORE SURGERY When a patient undergoes surgery, physiological and psychological stress levels rise significantly. As a result, nutritional needs increase. It is important to prepare a patient ahead of time to meet the increased requirements for nutrients that will occur during and after surgery. Protein status, in particular, affects the body’s ability to heal from surgical wounds, stave off infection and maintain general well being. Providing sufficient energy to deal with the demands of surgery is also of critical concern.
PROTEIN
Protein Energy Malnutrition (PEM) is very common in surgical patients, so it is important to ensure adequate protein intake prior to surgery. In the period following surgery, patients require adequate body protein in tissue and plasma to replace blood loss from surgery and to repair surgical wounds. Protein is also needed to prevent tissue catabolism following surgery.
ENERGY
Adequate energy intake is also an essential part of nutritional care before surgery. Since the body will require extra protein to repair tissue, overall energy requirements must be met by supplying carbohydrates and fats. Protein can then be spared to rebuild tissue.
VITAMINS AND MINERALS
If a person has any vitamin or mineral deficiency, the deficiency should be corrected before a patient undergoes
surgery. This will ensure that energy is used for healing rather than to compensate for the effects of a nutrient deficiency. For example, if iron deficiency anemia is present or an electrolyte imbalance exists, these nutrients need to be brought to normal levels prior to surgery.
ZINC
Protein metabolism depends on the presence of hundreds of enzymes that contain zinc. Collagenase, an enzyme that breaks down collagen, is especially important for wound healing. Increasing zinc intakebefore surgery can help the body heal from surgical wounds more efficiently.
VITAMIN C
During the postoperative period, large amounts of free radicals are released during the inflammatory phase of wound healing. Therefore, antioxidants are of critical importance.Vitamin C is particularly significant because it has both a strong antioxidant effect and a significant role in the processes that repair surgical wounds and prevent them from becoming infected.
SPECIAL CONSIDERATIONS
In most cases, surgical patients should be instructed to fast for at least eight hours prior to surgery.This is to ensure an empty stomach during surgery so that complications such as aspiration of food do not occur. Food in the stomach may also interfere with the procedure itself.

Bariatric Education Day - Management process for the admission & discharge of a Bariatric patient Health care professionals provide care for an increasing number of bariatric patients (previously known as obese or morbidly obese) who require specialised equipment and work practices. To celebrate this historic milestone, the 2010 IYNurse is planned as a sustained public awareness initiative to actively involve the world’s nurses — estimated to be more than 15 million — in a celebration of commitment to bring health to their communities worldwide.
