Congratulations to the winners of this issues cover competition. The nurses in the photo are participants of some of St John of God Hospital (Murdoch) various programs, celebrating the completion of their courses at a ceremony held in the Hospital. Director of Nursing, Robyn Sutherland said Murdoch is one of very few hospitals to offer support to graduate Registered Nurses over three years, taking nurses from undergraduate to graduated nurse and beyond. We received an interesting email from RN Gina Baynham of Toronto Private Hospital about a weight loss program. Here’s what she said: “I thought you might be interested in knowing about a brilliant health challenge that we undertook in the Hospital recently. It involved staff from every department and was a great moral booster as well as being excellent for health promotion within our hospital”. Turn to page 15 to read what they did. Journeys through time in Arabia. Hi everyone, Many of our nurses who are thinking about a contract in the Middle East often remark that they know very little about the Arabian peninsula except for the modern cities like Dubai and Abu Dhabi. So I thought I’d try to give you a flavour of some of the journeys of discovery into the heart of Arabia (now Saudi Arabia) so that you’ll be inspired to go and explore on your own. Actually in a broader sense, that’s what living and working in the Middle East is really all about. Bounded by the Red Sea in the west and the waters of the Gulf in the east, the Arabian peninsula consists mainly of Saudi Arabia with the smaller Gulf states of the UAE and Qatar on the Gulf coast and Oman and Yemen in the south facing the Indian Ocean. A huge waterless expanse of shifting sands called Rub-al-Khali or the Empty Quarter in the southern interior formed an effective barrier between most of these states. But the latest research suggests it wasn’t always so. Geological evidence suggests that as early as 5000 years go there were lakes in the Empty Quarter. Also, Saudi archaeologists are reclaiming from the sands a large city complex dating from that period - clearly impossible without abundant water. From an historical-political perspective, Arabia lies at the fault line between a succession of Western European empires and various Eastern empires based in Persia or beyond. For millennia, these two forces have contested for dominance over territory and for access to lucrative trade routes, such as for incense from Oman, the India trade and for spices from East Asia. For example, Octavius Caesar after defeating Mark Antony and Cleopatra in Egypt, sent a Roman legion into Arabia to try to wrest control of the lucrative incense trade from the local Arabs. Aided by the Nabateans (of Petra fame), Aelius Gallus took his legion across the Red Sea to what is present day Saudi Arabia, crossed the mountains and headed south towards

Yemen. Unfortunately for Aelius, his force was decimated by thirst and disease before finding any frankincense and he was forced to return in failure to Alexandria. Jumping forward almost 2000 years, it is incredible how little Europeans knew about the interior of the Arabia peninsula until relatively recently - it had remained uncharted and inaccessible. Then, a succession of fascinating characters, mainly English explorers and adventurers, included some memorable women, opened the region to European eyes from 1862 onwards. For example, Lady Anne Blunt, who travelled widely in the region, purchased many Arab horses from the Bedu, and was instrumental in establishing Arabian bloodstock lines in the West. More recently, the accounts of Wilfred Thesiger’s crossings of the Empty Quarter in the 1950s in his book Arabian Sands make for inspired travel reading. The great thing is that a contract to work in Saudi Arabia and the other Gulf states) offers you unique access to explore this fascinating land. There are lots of great places to see and experience but I won’t go on about them – that’s for you to discover for yourself, as I did. Perhaps you can rekindle some of the romance and adventure of travellers past like some of our other intrepid candidates – maybe even write a book about your travels. Regards and happy travelling

Toronto Private Hospital staff are celebrating the end of their recent “12 Week Challenge� Staff from all over the hospital joined a competition to see who could get fit over a 3 month period. The aim was to have their Body Mass Index (BMI) assessed at the beginning and end of the challenge. Staff from all over the hospital entered a spirited battle to see who could do the most exercise and lose some excess weight in the process. They were advised on what their ideal weight was based on their height. Weekly weigh-ins and filling in an exercise diary kept motivation high. Staff moral was boosted as different departments interacted and shared healthy recipes and fitness tips.

At the final weigh-in the participants had lost a massive 41 kg and done 26,477 minutes of exercise! WOW! The group celebrated their success at a meal out last week at a local restaurant. The $10 entry fee created a prize fund of $350 which was shared out by the most successful participants. Management supported the challenge by donating bottles of wine and Beauty gift vouchers which every participant had a chance of winning through a lucky draw on the night. The trim and healthy Toronto Private staff enjoyed their health kick so much they are already discussing when to run the next one! 15

Jacob nurses: A family passion

L

eeming mother and son Tania and Jacob Jerovich share more than a surname; they share a passion for care giving. And the passion is all in the family. Both have graduated from Challenger Institute of Technology with a certificate in health services, and Jacob’s two aunts are now enrolled in the same course. Tania, a 45-year-old mother of three, said she was inspired to pursue a health career after watching her teenage son flourish in the Certificate III in Health Services Assistance course at Challenger Institute. She took long service leave from her office job at Western Power to study the course and, after graduating in June, was now planning a career in health science or enrolled nursing. “I saw how much Jacob matured and how well he performed in his training at Challenger,” Tania said. “I have always wanted to

work in a hospital and help make people’s lives better, so I decided to follow Jacob’s path. “It is never too late to change your career – there are avenues out there to do whatever you want to do.” Jacob, who is now studying a science degree, said he encouraged his mother to begin a new career and had enjoyed giving her support and advice during her studies. “I had no hesitation in recommending this course to my mum,” Jason said. “The course made me more responsible and I gained respect for the elderly, which is something not many young people know about. “I’ve always been interested in the human body. Working in the cancer ward at St John of God Hospital Murdoch is really interesting because you get to establish relationships with the patients.” Challenger Institute nursing lecturer Julie Manning said it was a pleasure to teach mother and son

and the course had given them both an opportunity to turn their passion into rewarding careers. “Jacob gained a lot of confidence and has used his health services assistance qualification to embark on a university degree,” Julie said. “And it’s wonderful to see how well Tania has followed on from Jacob. Tania’s commitment and achievements were outstanding. She performed extremely well in the wards and in the classroom and ended up receiving the Student of the Course Award.” Julie said the health services assistance certificate involved a partnership with St John of God Hospitality Murdoch. The course was unique because 50 per cent of the training was delivered in the hospital, providing students with a real-life learning environment. Since the course began in 2009, 19 students had gained employment in hospitals as patient care assistants.

The many tasks that the Indigenous health workers perform all have a common objective: to improve the state of health within the Indigenous community, by assisting them to take a strong role in controlling and managing their own health and lifestyles. One important responsibility health workers have is to enhance communication between the medical staff and the Indigenous patients. Feelings of discomfort, anxiety and confusion are common experiences for patients when visiting the doctor. For Indigenous patients, these feelings may be heightened by factors such as a lack of understanding of the respective cultures and values of practitioner and patient. They can also promote a better understanding of the cultural beliefs and medical practices of both parties. By helping to bridge the cultural differences that may exist, the role of Indigenous health workers is one of great importance.

Killing You? Anyone who works shift work knows how difficult this can be, physically, mentally, emotionally and socially. Nurses often work when everyone else is sleeping. We work night shifts, weekends, and holidays. We switch back and forth from days to evenings to nights with little time to adjust in between. Shift work impacts all areas of our lives because our patients need us during the night as much as they do during the day, and disease never takes a holiday. There have been many studies published on the issue of shift work and how it affects nurses physically. Studies have linked the following health conditions to shift work: • Increased risk of ischemic heart disease • Depression • Sleep disturbances • Poor pregnancy outcomes (low birthweight infants, premature labour) • Gastrointestinal dysfunction (ulcers, irritable bowel syndrome) • Menstrual irregularities • Obesity

These are just a few of the adverse health effects linked to shift work. More recently, studies have examined cancer as a possible outcome of years of shift work. “Several studies have found that women working at night over many years are more prone to breast cancer. Some research also suggests that men working at night may have a higher rate of prostate cancer” (Hazards Magazine, 2009). For example, researchers from Harvard University found that women who worked shift work were at higher risk (50%) of developing breast cancer, and that the longer women worked shift work, the higher their risk became (Megdala et al, 2005). Night shifts have been implicated as being the most problematic, particularly for workers who work night shifts for many years. However, rapidly alternating shifts with little time off to adjust is not much better, according to experts. There is more and more awareness of the negative effects of shift work on nurses. It is hoped that as awareness grows, experts will not only recognize the problem but will come up with concrete suggestions to ameliorate the problem.

Advocacy The Gift of Nursing

A

dvocacy can be defined as the act of pleading or arguing in favour of something. In terms of nursing, advocacy can be seen as the process we engage in when we speak on behalf of our patients when they are incapable of doing so themselves. Advocacy can take many forms. It can be political, as when nurses immerse themselves in the politics of healthcare in order to bring about needed change. It can also be social, such as when we attempt to create change in our society that helps to educate others on issues in healthcare. Advocacy also occurs on a more personal level when we argue for the best interests of our patients. As nurses, we advocate for our patients every day in a myriad of ways. We interpret their lab results, consult with other disciplines and specialties, discuss plans of care with physicians, make decisions based on minute-to-minute changes in patient condition, lobby our supervisors for changes that affect delivery of care at

the unit level, teach about side effects of treatments or medications, call for a spiritual advisor when we feel the patient is in need of comfort and hope‌there are so many examples of what we as nurses do to advocate for our patients. Perhaps the most important job we do is helping our patients to become their own advocates, encouraging them to take an interest in (and responsibility for) their own health. More and more patients are finding strength in advocating for themselves, asking questions of medical professionals and demanding answers, expecting the best from the healthcare professionals tasked with helping them to regain or maintain their health or, at the other end of the spectrum, experience a meaningful ending of life. As nurses, this is our strength and our gift. No other profession has the opportunity that we as nurses have to give our patients the ability and strength to be their own best advocates.

Finding your way around the Arabian Gulf takes a degree of ingenuity.

The astrolabe was used in ancient times to make astronomical measurements, typically of the altitudes of celestial bodies, and in navigation for calculating latitude. It consists of a disk with the edge marked in degrees and a pivoted pointer.

Early Arab navigators developed the astrolabe, an effective and beautiful instrument which enabled the user to ‘perform such diverse tasks as timekeeping at day and night, surveying, determining latitude, and casting horoscopes.’ One cast brass instrument like this one bears a maker’s stamp and date - Nastulus (Iraq) - 315, corresponding to 927CE - 139 years before the Norman invasion of Britain. If you want to work in Arabia today, you won’t need an astrolabe to find your perfect 44 job but you will need our navigational skills. We
