On Monday 6 September 2010, SJOG staff turned blue to help raise awareness for Prostate Cancer. This photo was the second submitted. Well done for winning the competition twice and for your efforts in promoting awareness for a worthy cause. Safe Patient Care for NSW NSW public health system nurses and midwives took industrial action On the 24th November to push their 1 Nurse 4 Patients campaign.
The NSW Nurses Association (NSWNA) committee, along with nurses around Australia is claiming nurse shortages and poor skill mix has hindered the public health system for far too long, risking patient safety. The committee is calling for a major reform of the state’s public healthcare system, through the introduction of mandatory, minimum nurse-to-patient ratios in public hospitals and community healthcare services. This will be the first state-wide nurses strike since 2001. General Secretary of the New South Wales Nurses Association, Brett Holmes, says it is a last resort. The question is will it make a difference? Just ask most nurses following a typically busy shift and they will complain of feelings of frustration and dissatisfaction. Several nurses I’ve spoken with this week however, say the strike will make no difference and the action won’t even affect them. ‘We’ve chosen to be nurses, because we want to make a difference, we wouldn’t want levels of care compromised’ This statement although relating to the strike itself sums up the reason things have got so far. Nurses feel they can’t deliver the care they want to. There are some exceptional nurses working in our health facilities across Australia today, but as the workload increases and nurses try to cover a never ending list of tasks, orders, interventions, interactions, interruptions, prioritisation’s and documentations…quality is often the first thing to go amiss. All wards are pushed to maximum with several acutely unwell people who require continuous and careful assessment, frequent interventions, and constant vigilance. ‘Years ago patients would be in ICU, now they’re on the ward, these patents have very complex cases. It’s a constant juggling act’

Mr Holmes said the ratio had worked well in Victoria and would help overworked nurses to cope. ‘The overwhelming feedback from NSW nurses is that ratios, with the right skill mix, are the way to get hospital management to fill vacancies and provide the staffing levels required to provide quality, safe patient care,’ We have to be positive and hope that mandatory lower nurse:patient ratios will be enforced because it really makes sense for the health department as a whole. It’s depressing however, to think that the hope for lower ratios will likely never be realised, unless some advantage is shown in the hospital’s accounting books. Unlike the Victorian Labor Government, which last week announced improvements to its mandated nurse to patient ratios, the NSW Labor Government refuses to guarantee nurse staffing levels. The NSW Government is also refusing to employ 1,000 Registered Nurses who will graduate from NSW universities this month and forces hospital managers to restrict recruitment of nurses and midwives to meet budgets rather than patients’ needs. During Wednesday’s strike, nurses will hold a Special General Meeting of the NSWNA to hear a report back from NSWNA officers on the current state of negotiations with the State Government and endorse the next major steps in the campaign. Striking nurses in regional and rural areas, who are unable to get to Sydney, will hold activities in their local area. Let’s hope the nurses’ dream to ‘have the time they want and need available to spend with each patient and not feel like they have to run from one person to the next in order to get all “tasks” done’ can make it one step closer to being realised. Ampilatwatja - Central Australia RAHC is currently seeking an experience registered nurse to take up a short-term (3-12 weeks) paid placement in the Central Australian remote Indigenous community of Ampilatwatja. You will work in partnership with a multi-disciplinary remote health team, treat illness and promote well-being, maintain health systems, provide education and training, and encourage community health in action in order to contribute to better health outcomes. Services to be performed include emergency management of trauma, chronic disease management including tests and medications and implementation of the primary health care programs run by the health service. Skills required are venepuncture, cannulation, airway management, basic life support, patient assessment and cross-cultural communication. RAHC provides cultural training and clinical orientation prior to the start of a placement. In addition, RAHC will arrange all travel to and from the Northern Territory. Accommodation is also provided. Requirements: Unrestricted registration to practice as a registered nurse with two years broad based nursing experience. Fjordland - a great Arabian short break Hi everyone, When you’re working in the Arabian peninsula whether it be Saudi Arabia, the UAE, Qatar or other countries, the travel opportunities really are fantastic. You have plenty of excess cash, lots of holiday time and there are great travel bargains. (You retain a huge chunk of your salary - about 85% - because there’s no tax and so many large expenses are covered for free.) Among all the travel stories you’d expect, like trips to Paris, Rome, the Greek islands, Kenya, Istanbul, etc, a number of our nurses have been reporting back to us some lesser known spots in the region that they really enjoyed and have gone back to several times. So I thought I’d share two of these locations with you. Fjordland is usually a name associated with Norway or the NZ South Island but it applies equally as well to a region called the Musandam Peninsula in the Gulf. Until relatively recently the area has been quite isolated despite being adjacent to one of the busiest shipping lanes in the world and within easy driving
distance of Dubai or Abu Dhabu in the UAE. The peninsula forms the north-east tip of the Arabian peninsula that juts north and almost closes the Arabian Gulf from the Indian Ocean, leaving just a narrow strip of water, the Straits of Hormuz, separating Arabia from Persia (or Iran). What is really remarkable about the Musandam peninsula is the filigree pattern formed by enormous number of bays and inlets that together form a pristine, unspoiled marine environment. Mountainous cliffs drop precipitously into the deep aquamarine fjords and bays while sparse, ancient whitewashed villages cling to the water line. Formerly the setting for the tales of Sinbad, pirate lairs and smugglers’ caves, the area has opened up to eco-tourism. Yachts, pleasure cruisers and dive boats now travel to the area regularly from Dubai and other UAE ports for dolphin watching diving and swimming in totally secluded spots. Some smuggling across to Iran still does occur with super fast speedboats setting out from Musandam to ply their trade and return. Actually the Musandam peninsula is part of Oman despite being separated from the main body of Oman by a strip of UAE territory. Our nurses have taken enjoyable short breaks on their days off to the Musandaman various charter boats . Some other really memorable Arabian short breaks are to the Farasan Islands in the Red Sea, a marine park frequented by Jacques Cousteau, to Madain Salah in north west Saudi for the 2000 year old massive rock temples of the Nabateans and to the ancient towns, beaches and boltholes of Oman, to list just a few. Of course you can always pop over to Paris for the weekend. Working in Arabia can offer a lot if you go the right places – career progression due to fast expansion of hospitals, educational opportunities, great clinical experience, a top social life and fabulous travel. It’s also nice to know that you can also skip off to some quiet, magical spot for a short break when you feel like it. Best regards

e can all see it comingmany of us have greying hair, sore backs and other signs that retirement is not very far away. We wonder, and we worry, that there are not enough nurses coming up behind us to fill the ranks. What can be done? For those nurses who are nearing retirement age but are not quite ready to leave nursing entirely, organizations are focusing on ways to encourage and assist an ageing workforce to stay on the job. The Aging Workforce: Implications for Health Care Organizations (Hart, 2007) examines strategies to encourage older nurses to stay in the profession. Some of these strategies include: • creation of flexible shifts, including job sharing • “smart” patient assignments to reduce the need for walking • improving the design of nursing units to maximize efficiency
• ergonomics training (i.e. proper lifting techniques) • use of mechanical devices, such as patient lifts, to decrease workload • use of patient lift/transport teams • redefining and redesigning roles for older workers • improving lighting on units • wellness programs for all staff • stress reduction training for all staff • flexible benefits • elimination of overtime/floating for older nurses • phased or flexible retirement options • knowledge transfer It is unlikely that any organization will be able to implement all of these suggestions, but implementing even a few may persuade mature staff members to stay in the profession longer. Knowledge transfer is a particularly valuable concept, as mature nurses have a wealth of information and knowledge to share with their younger colleagues.

Spiritual Care in Nursing M
ost nursing education programs have expanded to include spirituality as a necessary component of nursing care. According to O’Brien (2008), “all nurses have the responsibility to be aware of and sensitive to their patients’ spiritual needs as a dimension of holistic health care”.
Comfort levels with using spirituality as a tool in healing vary from nurse to nurse; although we may agree that spirituality is an important part of providing nursing care, some may be less comfortable with outward manifestations of spirituality, such as praying with patients. Spirituality does not necessarily equate to being religious; spirituality can mean different things to different people. Nurses should not feel compelled to involve themselves in situations in which they do not feel comfortable, but should do what they can to help their patients in their spiritual quests. Regardless of our personal belief system, patients often need to engage in spiritual practices, which may include prayer, reading the bible, attending church or faith gatherings and other activities allowing them to express their
feelings and beliefs. During times of illness, some patients rely heavily on their faith to bolster their spirit, while others turn their backs on beliefs that they have held their entire lives. In both situations, patients may be in need of spiritual counselling. As nurses, part of our job is to recognize the spiritual needs of our patients and provide them with a means to meet these needs. This may include praying with a patient if asked, arranging for a priest, minister or other spiritual guide to visit the patient or arranging for a patient to attend church or other activities as desired by the patient. Whatever our personal beliefs, spirituality is of utmost importance to some, even when their health is threatened (or sometimes because of it). We must not forget, in a time of staff shortages, mandatory overtime and job stress, that the simplest gestures can mean the most to people and can provide them with comfort, hope and strength, an integral, yet often invisible part of our daily practice.
