The first Thursday of every month is “nerds day Thursday” and we encourage staff to dress as nerds for fun and staff morale. Also next month we plan to ask for a gold coin donation from everyone who dresses up and send the proceeds to a hospital in Somalia in which I volunteered last year.

New health program for Indigenous Women offers Hope Queensland University of Technology (QUT) has begun a new nurse-led research project that aims to offer new solutions and hope for Indigenous women’s health in Australia. The project has offered a rare opportunity for Aboriginal and Torres Strait Islander women from Brisbane’s north-side and Cherbourg, to come together to discuss and learn about new ways to overcome the alarmingly poor health statistics, life expectancy and chronic illness of Indigenous women. The official name for the new program is Women’s Wellness Health Program. Professor Debra Anderson from QUT’s School of Nursing and Midwifery stated that, “in a previous research project about women’s health, I identified that Aboriginal and Torres Strait Islander women were not being catered for within standard women’s health prevention programs.” “So that was the basis for our current research project, which was supported through Diabetes Queensland and conducted by a team of QUT researchers, including Associate Professor Bronwyn Fredericks, and PhD student Melissa Walker.” Both Ms Walker and Professor Fredericks are Indigenous women who have attained extensive experience working with Aboriginal and Torres Strait Islander communities in the areas of human services, wellbeing and health. In the beginning stages of the research project, Indigenous women had come together the 26

brainstorm the most appropriate and effective ways to reach Indigenous women with their health and wellbeing program. “The research participants suggested that a health program should be delivered through a notfor-profit organisation and advocated the formation of Indigenous women support groups to represent, advise and advocate for Indigenous women in the area of diabetes management,” Professor Anderson stated. She also included that plans for the development of the not-for-profit organization, and support groups, were now well underway. “Typically diabetes issues are handled through a process of individual consultation with a health professional. However, the research participants have indicated that a more suitable way of educating Indigenous women about managing their diabetes issues would be through the process of yarning,” Ms Fredericks said. “Yarning describes the coming together of a group of Indigenous people to discuss issues and share knowledge,” Ms Walker said. “The research participants felt yarning circles would provide a valuable opportunity for other Indigenous women to discuss mental health, and social and emotional wellness issues, all of which have an effect on diabetes.” One of the elders in Brisbane’s north side, Aunty Honour Cleary stated that, “we believe that having our own women’s group and women’s programs will help reduce the health inequalities between Indigenous and non-Indigenous women.”

Northern Territory Indigenous communities benefit from a 1000 health professional placements The Remote Area Health Corps (RAHC) has now filled over 1,000 short-term health professional placements into remote Indigenous communities across the Northern Territory (NT) since beginning operations less than 3 years ago. RAHC was established to recruit urban-based health professionals, including registered nurses, midwives, general practitioners, dental and allied health professionals to provide primary health care on short-term paid assignments in remote Indigenous health centres in the NT. RAHC health professionals have been providing significant support to the hard working permanent staff in the remote health centres

as well as expanding the health workforce in remote Indigenous communities throughout the NT. Are You Culturally Aware?

W

e live and work in a culturally diverse society, caring for people from varied ethnic and linguistic backgrounds. Within Australia we have our culturally rich Indigenous population, as well as many migrants from countries as diverse as Russia, Greece and Korea.

This presents a challenge for nursing and midwifery staff to provide care that is both clinically safe and culturally sensitive. Many hospitals have protocols designed to aid staff in understanding and delivering culturally appropriate care, yet in reality it can be difficult to do this. In this post, we’re going to have a look at some theory behind culturally appropriate care, as well as examining a couple of relevant case studies. 34

Transcultural care: cognitive approach

Across Australia it is known that Indigenous people have higher rates of adverse health outcomes compared with non-indigenous people. This is similar to other countries such as New Zealand and Canada. It is therefore important that Indigenous people are not discriminated against in any way and that their culture is accounted for when they present for health care. The cognitive approach to culturally appropriate care takes into account the customs and traditions of various cultures. Health professionals are usually educated about different cultures and taught their beliefs, practices and values, especially those of indigenous peoples, and how to


incorporate these into appropriate care. Unfortunately this fails to address the broader social and political factors that can impact on an individual’s access to health care and may lead to stereotyping. An individual is no longer assessed as someone unique, but within the clinician’s framework of what that person’s culture represents to them (the clinician) – a type of “cookbook” approach.

The individual approach to dealing with people of different cultures has been developed as a method of acknowledging how colonisation and migration can affect an individual’s cultural values and practices. Unfortunately this requires health professionals to have a greater insight into individual cultures and how the society they are living in has influenced both the culture and the individual. Theorists acknowledge that the individual approach, while resulting in a superior service than the cognitive approach, is a long way from being universally practiced in multi-cultural societies such as Australia. The best we can do at the moment is treat each individual with respect, and acknowledge that although we try our best to be culturally sensitive, this may not always be achievable.

Case Study 1

A nurse entered an Iranian patient’s room to find him on the ground, seemingly muttering incoherently to himself. The patient could speak very little English and the nurse was concerned that the patient had fallen out of bed and was unable to right himself. She went over to try and help sit the patient up, however he became visibly agitated and the nurse was left unsure of what to do. Later the nurse found that the patient was a devout Muslim and he had been praying to Mecca. • Should the nurse have done something

Case Study 2

A Cambodian woman was rushed to the emergency department by her family who explained she was feeling extremely dizzy. Upon examination the nurse found the patient’s body covered in dark, red welts. Feeling concerned about the possibility of self harm, the nurse informed both her supervisor and the doctor and an investigation was undertaken. It turned out that the patient had been practicing a traditional form of healing called “coining”, where a coin is rubbed vigorously on the patient’s body. • Should the nurse have done something different in this situation? • What would you have done in this situation? • Is there any way the nurse could have demonstrated cultural sensitivity in this situation? Working as health professionals in a multicultural society can present many challenges to the practicing nurse. We need to remember to treat each patient as an individual, instead of categorising them by the culture we assume them to hold. While there may be cultural norms that apply to the majority, we need to always look at the wider picture in an effort to provide the highest level of care possible. COSA’s 38th Annual Scientific Meeting ‘Partnerships against cancer – bridging gaps, breaking barriers’ November 15-17th 2011, Perth Convention & Exhibition Centre

The COSA Annual Scientific Meeting is the leading gathering of cancer health professionals in our region. We welcome clinicians and researchers working in cancer from medical and radiation oncology, surgery, nursing, pharmacy, and allied health. This year’s scientific program will focus on urological and prostate cancers, as well as the role of primary care in cancer. In 2011, COSA is partnering with the Australian and New Zealand Urogenital and Prostate Cancer Trials Group (ANZUP) which will help ensure the program includes state of the art presentations on prostate and other urogenital cancers. Program highlights include: • Partnerships against cancer, with a presentation from David Weller titled “What role for primary care in earlier cancer diagnosis?” • Working with General Practitioners to care for the carers of advanced cancer patients • Next generation sequencing – cancer genetics, with Andrew Biankin presenting on the International Cancer Genome Consortium • Closing the gap – consumers in cancer care, with presentations from Jessica Corner and Karl Lorenz on the international perspective, and Helen Zorbas on the Australian experience • The true cost of running trials for cooperative trials groups, including a presentation from Mitch Kirkman on the Clinical Trials Action Group report
