Fremantle Hospital’s ED wins 4-Hour waiting time challenge
Western Australia’s Fremantle Hospital has managed to successfully decrease its ED waiting times down to a fourhour benchmark despite the 30 per cent increase in patient admissions, according to new statistics released last month. Nicola Roxon, the Minister for Health and Ageing stated that, “when work started here at Fremantle Hospital in March 2009 only 53.8% of patients were admitted, treated or discharged within 4 hours. This increased to 84.4% in June 2011.” “This improvement has come about because of a change in how the whole hospital works together… It is important that this happened in a safe way. We know that as the waiting times reduced, so did the mortality rate from emergency admissions.” 12
The new Four-Hour Rule programme that was initiated by WA’s Government, has only managed to be implemented thanks to the $56.4 million funding from the Commonwealth Government back in 2008. And now, a further $750 million has been donated to the project throughout Australia until 2015. Ms Roxon continued to state that, “this is an example of a successful programme being delivered and now expanded nationwide. It is a credit to the hard work of doctors and nurses here in WA.” Melissa Parke, a Member for Fremantle, said that an additional $630,000 is being invested for the hospital’s emergency department to create better treatment options, patient privacy and mental healthcare.

New Emergency Department Designs Win Top Award for their Efforts At this year’s Tableland Cluster Quality Awards, the staffs from Tenterfield Hospital have been officially recognised as being the best in their region. The award acknowledges the high level of commitment that the staff had demonstrated at the Hunter New England health district in a successful attempt to improve the quality of patient care across a range of categories including improving patients’ experience health promotion, quality, safety and access. Michael Moore, the Health service manager at the Tenterfield hospital, and registered nurse John Brown were the two recipients of award on behalf of the staff, all of whom were recognized for improving the patient experience. “It was an honour to receive the award, it was really nice to have our efforts recognised” Mr Moore said. Tenterfield was nominated for the award after the hospital redesigned its emergency department. This involved utilizing a new triage system where patients were ranked from one to five depending on the severity of their condition. “It has really helped us improve the experience for all patients. I have received
a lot of positive feedback about the waiting room, in particular,” Mr Moore said. “The system has really helped us be more efficient.” Mr Moore stated that his staff don’t do the work that they do to only win awards, but it does help to maintain good morale amongst them. “These awards are given out each year and I would hope that Tenterfield would be nominated for at least one every year,” he said. “We were the project managers but it really was a team effort from all members of staff.” Richard Torbay, a Member for the Northern Tablelands, presented the awards in Glen Innes to winners.

Healthcare professionals plan for industrial action
Hundreds of health professionals are expected to take out industrial action from Victoria’s hospital emergency department as the crisis continues to deepen.

According to reports released, there are still bed shortages occurring, whilst women are being forced to give birth in emergency departments, plus the nurses’ strike is continuing with news that the hospital may be forced to close down beds and cancel elective surgeries. Western Health’s doctors were granted the right to proceed with industrial action by Fair Work Australia. If the doctors did decide to continue with the strike, this would throw the EDS from Sunshine, Williamstown and Western Hospitals into complete mayhem for everyone involved. The action taken out over improved pay and conditions is expected to go ahead, with other emergency doctors expected to show their support during the strike, a source stated. Some topics that are expected to arise during the protest include making patient care a priority
in Victoria for the critically ill, as well as suspending all procedures not “limb or life-threatening”. Doctors are planning to refuse treatment to patients when numbers exceeded safety limits, suspend the use of short stay units, refuse to write medico-legal reports and stop teaching medical students. Meanwhile, nurses are threatening to close one in three hospital beds, as well as stop one in three elective surgery operations in Victoria because they claim the Bailieu Government wants to remove minimum nurse-to-patient ratios. Lisa Fitzpatrick, the state secretary for the ANF stated that the work bans, which are scheduled to be voted on by 30,000 nurses, will demonstrate that nurses are willing to fight to provide quality patient care. Ms Fitzpatrick added that nurses are asking for a pay rise, and demanding that low-skilled health assistants not be used to replace fully trained nurses. A report from the Victorian Auditor-General revealed that maternity services in Melbourne’s growing suburbs were so poorly fitted and insufficient, that women were forced to give birth in hospital emergency departments. A report issued a few weeks back titled The Maternity Services : Capacity Report, claimed that in the week the audit team attended one outer metropolitan hospital, three women were forced to give birth in the emergency department. “Birthing in the ED presents clinical risks, as the midwife must leave patients in the labour ward to attend the woman in emergency,” the report stated. The service also failed in providing women and their families with adequate privacy. Associate Professor Hannah Dahlen, the spokeswoman for the Australian College of Midwives stated that this was “not good enough”.

Australian hospital staff bring smiles to Bangladesh Staff from a theatre suite at a teaching hospital, Western Sydney, will be heading to Bangladesh this month as part of their mission to raise funds and pay for surgical supplies and treat patients, including children. They will be performing cleft palate and burns surgery on the children, as well as attempting to operate on as many patients as they can within the two week timeframe that they will be in Bangladesh for. The mission, titled The Aussi Bangla Smile Appeal is made up of nurses and doctors who have taken the time out of their annual leave, as well as paying for themselves to make the mission possible. Barbara Mitchell, a theatre nurse and surgeon Hasan Sarwar, initiated the mission together back in 2007, when they named it The Aussi Bangla Smile Appeal. Their aim is to treat as many patients as they can within the two weeks they are there. Part of their mission will also include training local surgeons and nurses by conducting hands-on teaching sessions, plus operating local treatment centres. The equipment will be taken to Bangladesh, or purchased over there from India. All unused and surplus equipment will be donated to the local theatre staff towards the end of their trip. Current statistics indicate that there is 1 plastic surgeon 18
to 70,000 patients in Australia, as opposed to 1 plastic surgeon to 8,000,000 patients in Bangladesh. “We aim to perform 90 more surgeries in November. The conditions are poor and we work from 8am until 11pm to treat as many kids as we can,� said Barbara. It is Dr Sarwar’s dream to potentially open a speciality hospital that will conduct cleft lip and palate surgery for the underprivileged in Bangladesh. In the meantime, the team is planning to continue the Aussi Bangla Smile Project by planning the next trip once they return from this one.

The use of Webinar technology for Continuing Professional Development in Nursing There has been much debate in nursing circles about the various ways continuing education can be developed and delivered. With the introduction of APHRA, and the Continuing Professional Development Registration Standard from 1st July 2010 there has been a flurry of activity. Continuing professional development “... is the means by which members of the profession maintain, improve and broaden their knowledge, expertise and competence, and develop the personal and professional qualities required throughout their professional lives.”1 The concept of continuing professional development is not about training and testing for new skills but the recognition that the professional should keep up to date with the best practice guidelines that underpin their area of practice. 24
The problem with education is getting from the workplace to the education session. Money is tight and staffing levels reflect this – staff may want to attend the session but are unable to leave their work area to do so. The Nurses for Nurses Network was one of the first organisations to utilise webinar technology to provide education sessions for its members. A webinar is “...used to conduct live meetings, training, or presentations via the Internet. In a web conference, each participant sits at his or her own computer and is connected to other participants via the internet.” 2. The feedback from nurses has been very positive:

In order to achieve ongoing registration as a nurse, individuals are required to have evidence that identifies that they have met the continuing professional development standard set by AHPRA.
