You Earned It One of the principal attractions to the field of dentistry is the ability to run your own business and the independence it promotes to support a lifestyle that is unique to you. Independence is a powerful draw, pursuing passions outside of dentistry or within dentistry itself. CE warriors, world travelers, sports enthusiasts, and family jockeys all find benefits in the small business motif. However, running a small business is not without disadvantages. Small businesses are often referred to as “our babies” for reasons. Long nights and stressful hours are part of the small business dream! To grow and nurture a small business requires a lot of love and dedication.

Small businesses are essential for the person(s) they support, but, more importantly, they provide jobs to the community and contribute to the economy in general. Dentistry has a unique position to be able to open and provide services in almost any community. We are an economic driver in both urban and rural areas. For instance, the dental practices in my small hometown area of Perry, Oklahoma, support our community by providing solid good-paying jobs and valuable services. It took becoming a dentist in an urban setting for me to truly understand the flexible nature of small businesses and the support they give to the community. According to the US Small Business Association, small businesses with fewer than 500 employees account for 99.7% of all businesses, and 99.7% have paid employees. New jobs in the US are 62% small business driven, accounting for 44% of the total US economic activity. Small businesses are the backbone of the US economy, employ more people collectively than big businesses, and are more resilient during times of strife. In 2021, following the COVID-19 pandemic, small business applications rose to record levels. While large corporations struggled, small businesses were for those entrepreneurs with the American dream in their sights. Small businesses give back to the community by keeping local dollars in the community economy. Promoting other small businesses in the community and keeping it local supports

jobs and community sources for improvements in infrastructure and growth. Small businesses are leaders in innovation and in generating new ways of doing things. Many large businesses once started out as small businesses. Furthermore, big businesses are now trying to acquire small businesses in order to gain those attributes for their benefits. Small businesses drive the innovation that our economy needs to meet the challenges of a global economy. Whew! That is a lot of weight for all small business owners to take on! Keep in mind that you are not alone. Small businesses supporting one another is the best way to keep the US economy from crashing. An Open Letter to ADA Members: The Value of a United Tripartite and the ADA The following letter was submitted for publication in the ODA Journal by F. Lynn Means, ODA Executive Director, and proud member of the American Society of Constituent Dental Executives (ASCDE). August 14, 2023 The American Society of Constituent Dental Executives (ASCDE), representing state dental association executive directors and CEOs, believes that a unified tripartite is crucial for advancing the dental profession and its values. As Executive Directors/CEOs of state dental associations, we often focus on the value of organized dentistry at the state and local levels. However, the American Dental Association is also an important, valuable partner in the tripartite structure of organized dentistry. A dentist’s membership in organized dentistry means the individual gets the benefit of the tripartite – the local dental society, the state dental association, and the ADA – all working together to support dentists and their practices, while also protecting dentistry as a profession and promoting the oral health of the public. The strong tripartite of organized dentistry helped to build the modern dental profession that we have today – a profession that is compassionate, inclusive, trusted, and respected. Many ADA programs that provide direct assistance to dentists, regardless of practice modality, are highly visible and well known. Other ADA programs are less visible but no less valuable to dentistry, the public, and the tripartite. This includes critical support services to state dental associations and local dental societies. A discussion of some of the amazing benefits provided by the ADA can be found below. Not every state dental association and local dental society uses all the support services offered by the ADA, but most have used several of these benefits at some point. Whether it is a grant to help advance a legislative issue, financial assistance with litigation, or an invitation to the ADA to weigh in on an important scientific issue, we all benefit from having access to ADA’s resources. And similarly, we all benefit every time the ADA assists one of our sister states or local dental societies as it strengthens our collective body. We know that issues that are not adequately addressed in one state can blossom into problems in other states, which is why we stay unified in support of the tripartite and the dental profession wherever issues arise. While dentistry’s challenges continue to evolve so does organized dentistry as we rise to meet those challenges together as a strong, influential, and unified profession. Organized dentistry’s engagement and membership market share are the envy of nearly all other professional associations, especially in the health care professions. The ASCDE strongly believes unity is critical as we address the unique challenges and opportunities facing dentistry today and build on our strong foundation to achieve continued success into the future. We are better together. ESSENTIAL ADA BENEFITS: The Voice of the Dental Profession The ADA is the public authority on all dental topics. When the national media outlets cover dental issues, they seek input from the ADA. The ADA’s team of trained spokespeople and subject matter experts speak to the collective wisdom of the profession to the media, policymakers, and the public. For state or local level inquiries, we often turn to ADA resources or messaging to add validity and heft to our own communications. The ADA communications efforts protect and promote the dental profession and oral health from a position of credibility and authority unmatched by any other organization. A Reliable Advocate The ADA’s federal advocacy has had a significantly positive impact on dental care, dental practices, and oral health. The ADA defends and promotes the profession and patients before Congress and federal regulatory agencies. The ADA routinely and successfully advocates to eliminate or reduce the impact of burdensome regulations on the practice of dentistry. No other organization can do that with such credibility. The recent passage of the Competitive Health Insurance Reform Act, which limits the antitrust exemption available to health and dental insurance companies under the McCarran-Ferguson Act, and the development of the dental licensure compact and other efforts that promote licensure portability, are recent examples of how the ADA’s advocacy benefits patients and the profession. ADA Standards Program Setting standards is one of the most crucial roles of the ADA. From the specific torque at which dental handpieces spin to the wavelength at which dental curing lights cure, the ADA Standards cover almost every aspect of dentistry. These standards promote safety, reliability, and efficacy for dentists and the public. The U.S. Food and Drug Administration encourages dental product manufacturers to use FDA-recognized ADA consensus standards in their product submissions. No entity other than the ADA has the national presence and credibility to provide this crucial and valuable service.

Maintaining Strong Ethics The ADA maintains the “ADA Principles of Ethics and Code of Professional Conduct.” This is the universally accepted dental code of ethics in America and it serves as a publicly accessible reminder that patients come before commercial or financial interests. The ADA is the only organization that has the credibility to promote a code of ethics for the entire profession. The code is integral to ensuring dentistry remains a profession that is trusted by the public. Creating Universal Codes The ADA maintains the Code of Dental Procedures and Nomenclature (the CDT Code). The CDT code is the universally accepted standard for documenting dental treatment and ensures a level of consistency for payment of dental services. Only the ADA has the credibility that ensures these codes are universally accepted by payers, the government, and dentists. The ADA also ensures these codes are regularly reviewed and updated – keeping up with changes in technology and dental practice. Science and Research The ADA Science and Research Institute is crucial for advancement of dentistry through scientific research and provision of information that is practical, useful, and free from outside bias. The ADA’s research allows for development of evidence-based best practices and clinical practice guidelines that drive innovation and support the delivery of optimal oral health care. The ADA is regularly called upon by state and local dental societies to provide science and evidence-based information to regulatory bodies about dental amalgam, water fluoridation, and other dental-related issues. The ADA is the recognized preeminent resource for the science of issues related to dentistry. Health Policy Institute The ADA Health Policy Institute (HPI) conducts innovative studies on a wide range of topics impacting the U.S. dental economy, including access to dental care, the dental workforce, utilization and benefits, dental education, health care outcomes and more. A recent example of the importance of HPI is the COVID-19 Economic Impact on Dental Practices polling that was instrumental in providing valid data and guidance to policymakers and industry stakeholders during the pandemic. ADA Credentialing Service The ADA Credentialing Service allow dentists to avoid the repetitive, slow, and cumbersome method of submitting error-prone traditional paper applications for credentialing and re-credentialing. Through the ADA Credentialing system, dentists are able to retain ownership and control over access to their data while simultaneously reducing the administrative burden of filling out repetitive information for multiple dental plans. More than 100,000 dentists have used the ADA Credentialing Service, clearly illustrating its value. Additional ADA Benefits and Programs We could continue to go on about invaluable ADA programs and benefits like diversity leadership training, wellness and mental health support, and the ADA seal of acceptance program for consumer products that helps the public make informed decisions about dental-related products. There simply isn’t enough space to list all that the ADA does to benefit the profession, the public, and oral health. ADA Support for the Tripartite One of the least visible aspects of the ADA is its support for state and local dental societies. Just as providing roads, electrical service, and water and sewer lines are important to our communities, the ADA provides important infrastructure that sustains and enhances the work of state dental associations and local dental societies across America. These “public works” may not be as visible as the direct benefits like advocacy and dental practice support, but they are no less vital to the success of organized dentistry and the dental profession. For example, the ADA aggregates best practices and provides toolkits for various issues, including membership recruitment and retention and in-office dental plans. The ADA also provides expert information about dental insurance reform legislation, which encompasses dental loss ratio, noncovered services, prior authorization, and virtual credit cards. All of these tools can be modified to fit specific specifications and help state dental associations save time and resources while advancing issues that benefit member dentists. Perhaps the most underpublicized aspect of the ADA’s support for the tripartite is its investment in technology to provide local dental societies and state dental associations with a shared membership management database and software that enhance our ability to meet and track member dentists’ specific needs, streamline processes, assist with governance, and aid meeting planning. The ADA’s investment in this technology saves state and local resources and enhances our effectiveness.

Report from the ODA Board of Trustees The Board of Trustees met on August 4, 2023 to conduct regular business of the ODA. The Board voted to contribute all 2024 dues paid by Active Life Members to the Oklahoma Mission of Mercy Legacy fund. The Board of Trustees determines annually to what altruistic fund Active Life Member Dues shall be donated. The Board appointed Dr. Matt Bridges to serve as the ODA Board representative to the DENPAC Board of Directors. Dr. Tamara Berg nominated, and the Executive Committee and Board of Trustees approved, the addition of the following members to the Shobe Memorial Tablet in recognition of their outstanding, unselfish leadership at all three levels of the Tripartite and for their pioneering ideas that have helped further dentistry locally, statewide, and nationally: Dr. Ray Cohlmia

Important Information for Your Public Employee Patients The ODA recommends that you display a poster in your waiting area or at your front-desk checkout window to help better inform your patients who are public employees about dental insurance and options that may be available to them and their families. Below are some “talking points” that may assist you and your office staff when discussing dental insurance with your public employee patients. •

DENTAL INSURANCE IS NOT LIKE MEDICAL INSURANCE. Dental insurance is more like a prepaid gift card or phone card. Your dental insurance has a maximum allowable per year, and when it is spent, it is spent. So it is very important to familiarize yourself with all of the dental insurance options available to you in order to get the most “bang for your buck”. LOOK CLOSELY AT ALL OF YOUR OPTIONS! Sometimes, for just a few dollars more, you can get $500 or $1,000 more in benefits per year. Ask questions and make an informed decision!

BE SURE AND MAKE THE RIGHT CHOICE REGARDING COVERAGE FOR YOU AND YOUR FAMILY. For example, regarding referrals to specialists for orthodontics or root canals, some dental insurance plans don’t cover these services, and some do.

MAKE SURE YOU EVALUATE YOUR COVERAGE EVERY YEAR. Your enrollment/renewal period is in October, and you can make changes during that time. Benefits and coverage within the same plan may change from year to year, and if you don’t stay informed, your benefits could decrease or even change so much that they no longer meet your and your family’s dental needs.

STAY INFORMED ABOUT HOW MUCH OF YOUR DENTAL INSURANCE YOU HAVE USED. Don’t let your dental benefits go to waste by not using them all before the year ends. OkMOM 2024 will be a two-day event dedicated to serving the uninsured and underinsured population in need of dental treatment. Volunteer registration will open October 15, 2023 at okmom.org. Yes. Dentists can earn CE credit hour by hour up to 10 hours. Hygienists can earn CE credit hour by hour up to 5 hours. CE certificates will be sent to volunteers following the event.
I have a dental license from another state. Can I still participate?
Yes. An out-of-state dentist or hygienist can apply for a temporary license to volunteer at this event. Out-ofstate dentists and hygienists need to complete a Special Volunteer License form and send it to the Oklahoma Board of Dentistry to receive their temporary license. This form can be found at okmom.com/volunteers.
What should I bring to the event?
You can bring your own dental equipment to OkMOM, but it must be clearly labeled. Please note that every chair of the clinic needs to be fully operational at all times, so there is a possibility that your equipment may be used by another dentist if you step away for a break, etc. You are responsible for your own equipment. Otherwise, please only bring necessities! We cannot store your items, so leave what you will not be using during volunteer hours either in your car or at home.

BILL COULD PROVIDE PATH TO DENTAL CAREERS IN RURAL AREAS The ADA is championing the Rural Health Workforce Pathway Act which could help alleviate dental workforce shortages and would encourage elementary and secondary school students in rural communities to explore healthcare careers via career guidance, mentoring and help enrolling in training programs. "Because these students have a higher likelihood of staying and practicing in the community they grew up in or a similar rural area, your legislation will help to alleviate geographical disparities and ensure access to care for rural patients," ADA President George R. Shepley, DDS, and Executive Director Raymond A. Cohlmia, DDS, wrote in a letter thanking the legislation’s sponsor, Sen. Tim Kaine, D-Va.

Clinical Differential Diagnosis: Soft Tissue Mass By: Glen D. Houston, DDS, MSD (gdhdds@heartlandpath.com) HISTORY This 68 year old male presented with a chief complaint of “a growth involving my left lower jaw.” The patient noted that he had not been examined by any health care professional since his dentures were delivered approximately 20 years earlier. He wears his dentures day and night and noted that he occasionally takes aspirin for “denture pain.” He also believes that he has a "yeast infection." His social history reveals smoking and smokeless tobacco use for over 50 years. QUESTION #1 An appropriate clinical differential diagnosis for this lesion involving the left mandibular vestibule and buccal mucosa might include: a. Inflammatory fibrous hyperplasia (epulis fissuratum)

Candidiasis, an infection with the yeast-like fungal organism Candida albicans, typically presents intraorally as adherent white plaques that resemble curdled milk or cottage cheese clinically. These plaques can be wiped off or removed by scraping them with a tongue blade or rubbing them with dry gauze. Likewise, an aspirin burn (chemical trauma) produces mucosal necrosis from aspirin being held in the mouth. With short exposure, the affected mucosa exhibits a superficial, white, wrinkled appearance. This necrotic epithelium can also be removed leaving an area of hemorrhagic submucosa.

Epulis fissuratum, defined as a "tumor-like" proliferation of fibrous connective tissue, is typically observed in association with the flange of an ill-fitting denture. It usually presents as a single fold or as multiple folds of hyperplastic, reactive tissue involving the vestibular area. Verrucous carcinoma also presents as a papillary soft tissue mass that may resemble epulis fissuratum. Condyloma acuminatum (b) (venereal wart) is a virus-induced proliferation of stratified squamous epithelium of the genitalia, perianal region, oral cavity, and larynx. This is considered to be a sexually transmitted disease with lesions developing at the site of sexual contact. This lesion is much more common in the anogenital 28 journal | September/October 2023

Choices (a), (b), (d), and (e) are incorrect. As mentioned earlier, (a) inflammatory fibrous hyperplasia (epulis fissuratum) is a "tumor-like" proliferation of fibrous connective tissue. The histologic description of the present case is
inconsistent with this diagnosis. In order to support the diagnosis of (b) candidiasis, fungal elements (hyphae) must be observed microscopically. Since the lesion is not a white patch clinically and does not demonstrate a thickened keratin layer of the surface epithelium microscopically, (d) hyperkeratosis can also be ruled out. Finally, the diagnosis of (e) squamous papilloma cannot be supported based upon the described histologic features. DISCUSSION Verrucous carcinoma is a form of squamous cell carcinoma of the oral cavity that was first described by Ackerman in 1948. It is a "low-grade" malignant neoplasm, most commonly observed involving the mandibular buccal sulcus and the alveolar mucosa of the mandibular ridge. A smaller number of cases are found on the maxillary alveolar mucosa and occasionally on the tongue. Verrucous carcinoma represents less than 5% of all oral cancer cases. There is a striking correlation between the presence of this neoplasm and the use of tobacco products (particularly smokeless tobacco). This neoplasm is generally observed in elderly patients, the mean age of occurrence being 60 to 70 years, with nearly 75% of the lesions developing in males. The clinical appearance of this lesion is that of an exophytic "piled-up" growth or large papillary mass with a micronodular or mammilated surface that is attached by a broad base to the oral mucosa. The degree of keratinization (often considerable) determines the color of the lesion, white or reddish-white.
Microscopically, this entity is described as an exophytic, papillary epithelial proliferation with prominent, bulbous rete ridges. The epithelial proliferation is characterized by well-differentiated cells that lack the cytologic atypia observed in invasive squamous cell carcinoma. Additionally, the surface epithelium exhibits clefts between the papillary projections which are filled with keratin. A significant lymphocytic infiltrate in the underlying connective tissue may be present. Verrucous carcinoma, because of its exophytic nature, superficial growth, and unique histology, offers the patient an excellent prognosis. It is only superficially invasive and metastasizes infrequently. The 5-year survival rate for treated verrucous carcinoma approaches 90%. It can be treated successfully by simple excision with adequate surgical margins. Most recurrences are directly related to inadequate surgical excision at initial presentation. Radiation therapy is generally felt to be contraindicated in the treatment of verrucous carcinoma since there have been some reports of
transformation from well-differentiated verrucous into aggressive, anaplastic, invasive squamous cell carcinoma. AEGD Case Studies Case #1
By: Aziz Pradhan, DMD A 67-year-old male presented to the University of Oklahoma Advanced Education in General Dentistry Program in December 2020 for a comprehensive dental exam and treatment plan. The patient’s chief concern was that he would like to take care of his oral health after not seeing a dentist for several years. He is taking medications to manage high blood pressure, high cholesterol levels, and an enlarged prostate. A carotid artery stent was placed in December 2022. The patient presented with missing teeth #1, 3, 16, 17, 19, an FPD 2-x-4 with recurrent caries on #2, coronal fracture of #12, recurrent buccal caries on #15 under an existing amalgam restoration, and a vertical root fracture on #19. The periodontal diagnosis for the patient was localized Stage I Grade A periodontitis. Initial treatment consisted of localized SRP, sectioning of FPD 2-x-4, extraction of #2, and extraction of #19. Implants were placed and restored to replace #2 and #3. Tooth #15 was restored with a gold crown. Due to the extent of the coronal fracture on #12, the tooth was deemed non-restorable (Figure 1). A CBCT scan showed adequate palatal bone for an immediate implant.

The implant on #12 was placed using a fully guided protocol. Intraoral scans were taken and uploaded as STL files to Dentslpy Sirona’s inLab CAD software. Tooth #12 was digitally removed, and the edentulous space was replaced with a digital wax up of #12 (Figure 2). REALGUIDE ™ was used to fabricate the surgical guide. STL files of the maxilla with #12 digitally removed and of the digital wax up were uploaded into the REALGUIDE ™. Additionally, the CBCT file was uploaded into the planning software. An Astra 3.6X9mm implant was planned to ensure adequate engagement of palatal bone and acceptable prosthetic positioning (Figure 3). After planning was completed the guide design was uploaded to the InLab CAM Software and printed using Dentsply Sirona’s Primeprint and guide cylinder was added. Tooth #12 was atraumatically extracted with an elevator and forceps technique. The surgical guide was seated (Figure 4) and the Astra guided implant sugery kit was used to prepare the osteotomy. Symbios demineralized corticated and cancellous bone was placed into the osteotomy followed by placement of the implant to depth achieving a primary stability of 15 Ncm. A cover screw was placed, and the site covered with additional graft. A collagen plug was used to cover the site followed by placement of Figure-8 and simple interrupted chromic sutures. A post-operative periapical radiograph was taken (Figure 5) and the patient was dismissed with post-operative instructions.

Case #2 By: Kurren Virk, DMD This case study highlights a unique approach to the preparation, temporization, and restoration of a difficult restorative situation. A 55-year-old female presented to the University of Oklahoma AEGD program stating that she had a tooth that was prepared and temporized for a crown, which fell off about a year ago. The pre-operative bitewing radiograph (Figure 1) shows tooth #15 appears to have been prepared in a way that largely spared the distal cusps, but in the interim after losing the provisional, the tooth had supra-erupted leaving minimal inter-occlusal space (<0.1 mm). Furthermore, the distal gingiva extends nearly to the occlusal surface of the tooth, whereby a traditional crown preparation would require significant gingivectomy or crown lengthening to gain adequate ferrule for proper resistance and retention form. To combat these issues, the restoration planned was a base metal (non-precious alloy) onlay. The advantages of this material are that significantly higher bond strength can be achieved (up to 2x) compared to noble/high-noble metals, a minimal occlusal thickness of only 0.3 mm allows significantly less tooth reduction, and a more economical lab bill relative to gold. This allows for conservative preparation needing fewer retentive features. The patient’s consent was obtained to have a silver-appearing metal restoration, and it was confirmed that she does not have a nickel allergy, which can be a contraindication for use of this material. The old core build-up material was removed, and the tooth was minimally prepared to give a smooth, flowing surface to the prep.
the mesial embrasure, then a light-curable liquid dam was placed into the mesial embrasure, preventing the tooth from drifting during the provisional period, and then the Teflon was removed (Figure 3). On seating day, the patient reported no sensitivity in the interim, the liquid dam was removed. Liquid dam and an acorn instrument were used to create a holder on the occlusal surface of the onlay to allow easier positioning during the appointment due to difficulty of handling. The bonding protocol for metal is the same that would be used to bond a zirconia restoration. Intaglio of restoration: Air abrasion, cleaner, MDP Primer. Tooth: Air abrasion, total etch (since dentin is sealed), bonding agent. Cemented with adhesive dual-cure resin cement. Figure 4 shows the final restoration on a lab model. After preparation, immediate dentin sealing (IDS) was done. This is ideally done with a 4th or 6th generation bonding agent. Air abrasion was used to clean and micro-mechanically prepare the tooth surface, then 4% pure Chlorhexidine for further disinfection and long-term inhibition of MMPs to maintain bond strength. After this, a self-etching primer and bonding agent was placed on dentin only, followed by 0.5 mm layer of flowable resin also on dentin only. This serves to reinforce the hybrid layer and complete the IDS procedure. Benefits of IDS include higher dentin bond strength, longer maintenance of dentin bond strength, less bacterial leakage, and minimal to no post-operative sensitivity. Glycerin application prior to curing is then completed to remove the oxygen-inhibited layer, followed by air abrasion to leave the surface dry and clean. Figure 1: Pre-op BW Margins were marked with a pencil intraorally to aid the lab, and an iTero digital final impression was made and sent to Express Dental Lab, Norman, OK (Figure 2). Light contacts and occlusion were requested as base metal is very hard, and thus difficult to adjust.

Case #3 By: Thanh Truong, DDS A 65-year-old Caucasian female presented to the University of Oklahoma Advanced Education in General Dentistry in September 2020 as an external referral from the Stephenson Cancer Center for an evaluation for comprehensive dental care. Medical history includes a cancer history of radiation and chemotherapy, bifrontal craniotomy and lateral rhinotomy to remove an olfactory neuroblastoma with removal of retropharyngeal node metastases. The patient’s last round of radiation therapy was in 2010, with the maxillary left posterior area receiving more than 55 Gy, and the mandible also received more than 55 Gy. The patient’s last round of chemotherapy (IV Cisplatin) was in 2019. The patient’s current medications included Sudafed, Aleve, and Clinpro 5000ppm fluoride toothpaste. The patient had multiple decayed teeth due to severe xerostomia (see Figure 1&2). The patient’s chief complaint was “I just want my teeth to be taken care of. I don’t want to keep losing my teeth.” The patient also stated that she had been unable to find a dentist to treat her due to her medical history for ten years. After thorough discussion with all treating parties, the proposed treatment plan included extracting #2, 3, 4, 9, 10, 13, 18, 30, 31 due to non-restorability. The extraction would be done by the OU Oral Surgery Department, with a plan to remove only one tooth at a time to reduce soft tissue trauma. Therefore, any development of osteoradionecrosis might possibly be manageably taken care of. While these teeth are awaiting extraction, they will receive silver diamine fluoride to stop decay progression. Restorative treatments included composite fillings on #5-B, 7-MDL, 8-ML, 29-MO, and a maxillary immediate partial denture to replace #4, 9, 10, and 13. The patient will have adequate function with a shortened dental arch (up to 2nd premolars).
procedures. Removable appliances can cause ORN from tissue trauma and may need to be adjusted frequently. Besides reducing tissue trauma, other methods can be used to reduce the chance of patients developing ORN. Hyperbaric oxygen (HBO) therapy is recommended before and after surgery. These patients typically receive 20 pre-extraction HBO sessions followed by 10 post-extraction sessions. Other considerations include use of local anesthetics without vasoconstrictor, and prophylactic antibiotics for dental procedures. Interestingly, a shortened dental arch (with all teeth up to 2nd premolars) has been shown to provide enough function for most patients.

As of April of 2023, the patient had #2, 13, 18, 30, and 31 extracted, fillings on #5-B, 7-MDL, 8-ML, 29-MO, and SDF application on #9-D and 10-M. The patient had excellent soft tissue healing and bone healing without any sign of osteoradionecrosis (see Figure 2). Ongoing treatment includes extraction of #3 and 4. Immediate delivery of maxillary immediate partial denture will follow extraction of #9 and 10. It should be noted that the clinician’s efforts for the patients while they are undergoing radiation therapy should be focused on managing side-effects from treatment such as mucositis and dry mouth as well as educating patients about the importance of routine dental hygiene procedures to maintain a healthy dentition. After patients’ complete radiation therapy, the clinician’s priority shifts to minimizing soft and hard tissue trauma, as it could lead to osteoradionecrosis (ORN). ORN occurs in approximately 9% of patients who receive more than 70 Gy of ionizing radiation to treat head and neck malignancy, 2% of patients who receive 60 – 70 Gy, and almost 0% in patients who receive less than 60 Gy. Furthermore, the mandible is 7 times more likely to develop ORN than the maxilla due to mandibular bone being denser and less vascular. Non-surgical procedures are safe and are recommended to avoid tooth loss which may include scaling and root planing, non-surgical endodontic treatments, and all restorative 32 journal | September/October 2023

FROM AEGD TO PRIVATE PRACTICE By: Meredith Turbeville, DDS Every dentist remembers the nerves and excitement felt after dental school. I remember having this feeling when I had completed my doctoral program in San Antonio, Texas, and was on my way to joining the AEGD program at the University of Oklahoma College of Dentistry. During my long trek from San Antonio to Oklahoma City, I remember contemplating my goals. I wanted not only to become more confident and efficient in the skills I had already gained, but also to learn as many advanced procedures as I could. Once the AEGD was in full swing, I realized what an amazing opportunity it was. Within a month, I started doing implant cases, esthetic cases, and complex restorative rehabilitations. I saw more difficult cases in one month than I would likely treat in a year of private practice. I had expert faculty who taught me an immense amount and guided me toward confidence. The OU AEGD leadership created the perfect balance of independence while holding a safety net to catch me, if needed. Throughout my residency year, I
learned the value of collaboration. My co-residents and I would often be huddled in our resident room, swapping stories and advice based on our experiences. Relying on colleagues in this industry is a lifeline for better Dr. Meredith Turbeville with her boss, Dr. Melanie Emerson patient care. When I left the AEGD in June 2022, I entered private practice in Midwest City, OK, with my now mentor, Dr. Melanie Emerson. Entering a busy private practice comes with many challenges. I had to adjust to running more than one dental chair and learning to work with multiple assistants, all while keeping track of the many hygiene exams. While the transition was challenging, I never had to worry about my clinical skills. I arrived confident that I could make a difference in this practice and help my patients by applying the skills I learned in my AEGD. The beauty of completing my residency is that it allowed me to focus all my learning on these new aspects of dental practice. I remember that the most difficult part of my transition was conforming to the running of an office. From my year in practice, I learned from every member of my team. From the assistants, I learned smoother techniques and patient management, and from our office managing staff, I learned the importance of documentation for insurance and treatment presentations. My mentor/boss has taught me countless ways to improve my skills and the importance of self-care.

Instagram Posts to Grow Your Practice By: David H. Wong, DDS When it comes to Instagram and social media, the word “influence” gets thrown around quite often. With an estimated 127 million Instagram users in the US in 2023 (source: Statista), this represents a huge market for dentists. While not all of us desire influencer status, the ability to influence people and to affect their behaviors and actions without direct contact is a useful skill when it comes to growing your dental practice. Regardless of whether you favor Instagram, Facebook, YouTube, or TikTok, here are some tips to help create effective posts and content in order to set your practice up for social media success.
4. Expertise: These posts are important so that people remember that you are, in fact, a legit dentist, not just some dancing TikTokker or office prankster. These types of posts will include your certificates, accomplishments, awards, recognition, or publicity. If you have been in the newspaper or on TV, Instagram is a great place to show it. You can also demonstrate your expertise by sharing how-to or how it’s done videos. People love tips and tricks (how to get whiter teeth, how to save money at the dentist, etc.).
Having lectured on this topic of influence, I often hear from dentists that the main challenge they face is that they do not know what to post. They have worn out all the holiday posts, staff birthdays, dental awareness month, and so on, and they need more content. When creating content for my practice, I try to focus on four areas of influence: authenticity, attraction, proof, and expertise. 1. Authenticity: Create your own content. Yes, you can buy stock photos, repost other accounts, or hire a company to post for you, but in order to capture your audience (i.e. patients or colleagues), your content has to be yours. Patients, customers, and clients like real people, and real people have personalities, hobbies, families, and quirks. Real people share. They confide in one another. In your posts, share a little about yourself besides where you went to dental school. Brag on your staff. Show people what your day is like. Share a secret about yourself or maybe an industry secret that people may not know. When we confide in others and display authenticity, we build trust, and people like dentists they can trust. 2. Attraction: Pretty posts are nice. Pretty pictures are also important. But this is not the type of attraction I am talking about. Here, we are talking about the laws of attraction. Specifically, if you want people to like you, you have to like them. In your posts, do not be afraid to tag or promote your friends, staff, or colleagues. Tag your favorite dental organization, such as the ODA (@okdentassoc). Being attractive on social media means you are engaging and actively participating with others (hence “social media”). The more you like, comment, and share, the more that this will come back to you. The bottom line is that people like people who like them. That is the law of reciprocation. 3. Proof: Social proof. This is where you get to display your work for all the world to see. Post some “before and after” photos of the beautiful smiles you have created. If you do not take pictures of your work, now is a great time to start. Until then, you can also share a patient testimonial or maybe your best online review. Here is a secret: sometimes, I post my worst one-star Google review (authenticity, remember?).
There are many more types of Instagram posts out there, such as giveaways, but this is a great, simple place to start. Besides creating influential content, remember to post regularly and use pertinent hashtags, such as #tulsadentist, #whiteteeth, or #brightsmile, to maximize your reach. Do not forget to engage with other accounts and use all of Instagram’s other features, such as stories or reels. The Aftermath- A Testimonial (Cornerstone Dentistry, Shawnee)
Since 2006, I have called Cornerstone Dentistry at 1601 Airport Drive, Shawnee, OK, my work home. But on the evening of April 19, 2023, my town and work home were severely affected by a devasting tornado. I watched as the warnings came from the news that a tornado was headed toward Shawnee and that everyone needed to take cover. The Cornerstone team continued to watch the news and see the damage across our beloved town, including the cross streets where our office was located. On April 20, in the light of the morning sun, the staff arrived to see windows blown out, trees split down the middle, the roof needing serious repair, and vital business equipment destroyed from water and wind. Immediately, the staff and I went to work, notifying patients that the office would be closed until further notice, and trying to figure out how we could quickly get back to serving our beloved patients. After a full week of the office being completely inoperable, we finally got the green light! Electricity had been restored, and the team was able to start treating a limited number of patients. There was still no phone service or internet, and boards were placed where windows once were, but being able to serve patients who needed immediate attention was our team’s top priority. During the chaos of cleaning up and figuring out how to quickly get back to seeing a full patient load, I realized that there was no way our team had the time to attend the ODA Annual Meeting the following weekend. I called the ODA to cancel my meeting registration and proceeded to tell the woman on the phone about the tornado damage at our office. She recommended that I email Lynn Means, ODA Executive Director, with the information about our office, along with some photos. Lynn immediately responded, got me in touch with Shelly Frantz, and informed me of a program called the Oklahoma Dental Relief and Disaster Grant Program (RDGP). Honestly, RDGP was not even on my mind at the time! That one call to my dental association changed so many things and gave our team even more hope after that terrible event. Shelly was able to send us some immediate funding to get the work started, and we provided the paperwork to apply for additional funding. As of today, most of the larger repairs are complete. We needed a new roof, multiple windows, a new fence, computer equipment, etc. We are still completing finishing touches, like window coverings, flower beds, and exterior building cleanup. The quick action of the ODA team and their kindness helped us tremendously to get back on our feet and do what we love the most—dentistry. I would like to personally say how very thankful I am for the ODA for their support and willingness to help us. They were so thoughtful to suggest that we apply, and then they helped us through the process. The ODA has truly been a blessing to our office!
