Less invasive option for valve disease treatment offers benefit with fewer risks Aortic valve stenosis is one of the most common and serious conditions affecting the heart’s valves. It Robert J. Cubeddu, MD is a narrowing of the aortic valve opening, which is the passage for oxygen-rich blood to leave the heart and go out into the body. When the valve opening is narrowed, the flow of this blood is blocked or reduced. This condition is most common in people 65 or older, severely affecting nearly 500,000 people in the United States each year. It can cause shortness of breath, chest pain, congestive heart failure, and, ultimately, death if left untreated. A revolutionary approach to treatment Replacing the diseased valve is the standard treatment for most patients with aortic stenosis. Until recently, however, the replacement had to be done through a surgical procedure in which the patient’s chest was opened in order to access the heart. In recent years, the transcatheter aortic valve replacement (TAVR) procedure has revolutionized treatment for aortic stenosis. It is minimally invasive, allowing the surgeon to replace the valve through a catheter placed in the femoral
artery, which runs up the thigh, through the abdomen and to the heart. A multitude of studies have shown TAVR to be a safe and effective alternative to traditional valve replacement, offering patients minimal risk and discomfort, a shorter hospital stay (usually only one or two days vs. five to seven days with the open procedure), and a prompt recovery. “Aortic stenosis is a condition that offers a malignant course. As with open heart surgery, this procedure cures the condition,” says Robert J. ebruary is typically the month we shower those we love with gifts of chocolates and flowers, and maybe a piece of jewelry. This Valentine’s Day, why not give your loved one a gift that truly shows how much you care about them – the gift of life. February is American Heart Month and it’s the perfect time to help your family adopt a heart-healthy lifestyle and learn lifesaving CPR. Heart disease is the leading cause of death for men and women in the United States. According to the American Heart Association (AHA), someone in this country suffers a heart attack every 34 seconds. This is when blood flow to the heart is blocked. During a heart attack, seconds count. It is critical to keep the blood flowing until paramedics arrive. That’s why the AHA is encouraging everyone, even children, to learn hands-only CPR (Cardiopulmonary Resuscitation). Throughout the month of February, local municipalities, fire departments, and other agencies and health care providers will be offering hands-only CPR classes. It consists of two simple steps – call 9-1-1 and then push hard and fast until help arrives. Immediate CPR can double or triple a person’s chances of survival after cardiac arrest. Take the time to learn this lifesaving technique, because the life you save will likely be that of someone you know. This is also the month to adopt a heart healthy lifestyle, if you haven’t already. That means eating nutrient-rich foods, exercising more (with at least 150 minutes of moderate physical activity each week), and living a tobacco-free life. So, this Valentine’s Day, show your loved ones how much you really care – take the time to learn or refresh your CPR skills and commit to living a heart-healthy life.

Achilles’ Tendonitis in the Athlete BY MICHAEL M. COHEN, DPM FACFAS
I
t is said that the heart and soul of every running athlete is in his Achilles’ tendon. In ancient mythology, Achilles is described as the swiftest and fastest of heroes whose one exception was his heel. Many athletes, especially runners, fall victim to the same crippling malady. If not dealt with rapidly and properly, the condition frequently becomes debilitating and, at times, life altering. The calf muscle originates at the knee and converges into the Achilles’ tendon which is anchored into the back of the heel bone. Its principal function is to pull up on the heel and push the foot in a downward direction. This allows the athlete to rise vertically and propel forward. It’s second unappreciated function is to decelerate the leg and stabilize the knee to absorb shock during impact. One in ten runners will suffer from Achilles tendinitis and 16% will be forced to end their careers due to chronic pain and recurrence. Due to its compromised blood supply, scientific
research reveals that the Achilles lacks true inflammation. Therefore, the value of anti-inflammatory medications is controversial. The first sign of Achilles tendinitis usually comes when getting out of bed or a chair. There is sudden pain and stiffness in the back of the ankle causing one to limp during the first few steps until the calf muscle and tendon have stretched out and warmed up. Most athletes recognize Achilles’ tendinitis as discomfort that is well localized in the midsection of the tendon or at its insertion at the back of the heel. Treatment is predicated on the degree of injury and level of damage. Activity modification and at times immobilization is warranted to allow the tendon time to heal. Stretching the calf and controlling pronation and supination with orthotics and a heel lift are beneficial. It’s important that one refrains from running or walking with shoes that have zero or negative drop heels, as this may cause or aggravate the symptoms. Although uncomfortable at first, the value of eccentric strengthening accelerates the repair of damaged tendon cells and its use cannot be understated. Ice therapy, particularly after a workout, may reduce the extent of tendon degeneration. PRP and stem cell injections have proven to be valuable in recalcitrant tendinitis, often preventing the need
for surgery. Shockwave therapy is a useful technique when tendinitis is insertional. The ultimate fear is a recurrent or chronic Achilles’ tendinitis which fails to heal and/or results in a partial rupture which at times may require surgical attention. Broward County Schools to Commemorate MSD Victims with Day of Service BY JOHN DUKA
T
his Valentine’s Day, students and faculty across Broward County will observe a Day of Service and Love in commemoration of the tragedy at Marjory Stoneman Douglas High School. Rather than attending classes, students have been given the option to participate in a number of service projects as a way to give back to the community in honor of the 17 souls that were lost two years ago. In collaboration with HandsOnBroward, the City of Parkland, the City of Coral Springs and the Interfaith Council, schools across the district will participate in activities and service projects for their schools and local communities. “A Day of Service and Love honors our fallen eagles and our families and communities impacted by this tragedy,” said BCPS Superintendent Robert W. Runcie. “We invite our entire community to be a meaningful part of the ongoing process of healing by pausing at 10:17 a.m. to honor and remember.” There will be no academic activities during the day of service. After participating in the programs, students will be released from school early. In addition, there will be a county-wide moment of silence at 10:17 AM across the school district. District schools

Davie Singer Competes in South Florida’s Kids Got Talent Show BY STACEY BOMSER
C
amila Pena-Torres says she’s been singing as long as she’s been alive. Her father jokes that as a baby, Camila would sing, not cry. The 15-year-old performer is now preparing to compete in the South Florida’s Kids Got Talent singing competition, which takes place during Tri-Rail’s “Rail Fun Day” on February 8th. Camila is one of 13 talented young singers chosen to compete in South Florida’s Kids Got Talent. The first-place winner will receive $500, a one-year contract inclusive of a photo shoot with composite cards courtesy of Posche Models & Talent, and professional studio and music video recording sessions with MikeSoperMusic and Newborn Studios. Camila has put a lot of thought into her song selection for the competition. “I want to sing something that resonates with me and has had
an effect on my life and the lives of many others.” She plans to sing Waving Through a Window from her favorite Broadway musical, “Dear Evan Hansen”. Camila confesses she has another personal reason for choosing that particular song. “I butchered the song in my eighth-grade talent show. No note was the right note. more convenient to use public transit. Local Mom Honored as 2020 Florida Mother of the Year BY TERRY JAILLET
J
ennifer Cohen of Davie was recently announced as the 2020 Florida Mother of the Year®. She is the 48th woman in Florida history to hold this honor. Jennifer is a single mother to twin teenage daughters, both of whom were born 10 weeks premature. She was recognized for her work as a philanthropic leader. Jennifer has raised millions of dollars for a variety of organizations that benefit our community. She, along with honorees from states across the US, will be recognized during the 85th National Convention of American Mothers in Washington, DC, on April 19th – 21st. She will also serve as an ambassador for Florida mothers during visits with members of Congress. Jennifer has devoted her entire life to helping others. Over her career, Jennifer has raised millions of dollars for a variety of organizations that are important to her. Ever the champion for the underdog, Jennifer has spent her life helping animals and people. She does this all while being a single mother to her twin teenage daughters, Sydney and Alexandria, who are amazing young ladies. In an effort to show her extreme gratitude after her daughters were born 10

They spent two months in the NICU, and it was a terrifying experience,” commented Jennifer, “Thanks to the amazing care they received at Memorial Hospital West, my daughters did not have any lifelong health challenges. Should I have My Child’s Wisdom Teeth Extracted? BY DR. MARTA GROSMAN
T
his is a question I am asked frequently, especially at the end of orthodontic treatment. The question is very logical. We have been working to align the teeth for 2 years and everyone is excited to unveil their new smile to the world. The last thing that anyone wants is to leave the wisdom teeth to spoil our great work. On the other hand, the extraction of third molars is usually carried out by an Oral Surgeon using some form of sedation. The decision to refer must be thought out and based on science. According to the American Association of Oral and Maxillofacial Surgeons (AAOMS), the decision to avoid extracting the Third Molars (AKA Wisdom Teeth) depends on their meeting the following criteria: completely erupted and nonimpacted, completely functional, painless, free of decay, disease-free, capable of being properly cleaned. As can be seen from this list, the academy of oral surgery leans heavily toward extraction. This list fails to aid in my decision making in most cases as the average teen completes orthodontic treatment between 13-14 yrs old and the wisdom teeth erupt between 17-21 years old. What I prefer to use as a guide is the anatomy of the mandible (lower jaw) and the developmental stage of the Third Molars. For example, there is a nerve that runs the length of the lower border of the mandible called the inferior alveolar nerve. Among other things, this nerve provides for our feeling in the lower lip. The Third Molars develop right over this nerve and it is not until around 15-16 yrs old that the roots of the teeth tend to approximate and sometimes even wrap around the nerve. I allow the third molar to grow and hopefully find a reasonable eruption path prior to the completion of development of its roots. If I find that there is too much pressure being placed on the 12 year molars, I will refer for evaluation prior to the full development of the roots. What this accomplishes is the removal of the Third Molars with a much reduced chance of damage to the nerve. As in most situations, timing is everything. Our protocol consists of consistent retainer wear as soon as braces are removed. We then monitor the third molars until they either come in or are removed. Consistent retainer wear is more than sufficient in maintaining that beautiful smile through the process. In fact, we are recommending long term night time wear of the retainer even if the Third molars are removed. As it turns out, the eruption of the Third molars is the most well known, but not the only factor which can cause shifting of the teeth over time. What Does Child Support Cover? BY EVAN H. BARON, ESQ. ’m often asked, “what expenses does child support actually cover”? The answer is very general in nature. It is a contribution to all monthly household expenses, such as rent/mortgage, utilities, food, etc. It is a contribution towards the regular monthly expenses of the parent’s household. A parent cannot designate the expense his/her support payment should be used for. It cannot be deposited into a separate account solely for the benefit of a child. In addition, child support does not cover some normal and customary expenses for the children such as extracurricular activities, uncovered medical expenses, school expenses, college funding. All of these expenses must be addressed separately as additional contributions from each parent. Private school tuition is also not credited against the monthly child support obligation, as private school is considered a privilege and not a necessity since a child can attend public school at no cost.

The laws in Florida generally allow a judge or the parties quite a bit of discretion when finalizing family law cases. However, there is one particular area of the law which requires strict compliance. This concerns the issue of child support. In either a Dissolution of Marriage or a Paternity action, child support must be addressed. The parents cannot waive a child’s right to financial support, nor can they deviate from the state-mandated monthly amount as determined by the Florida Legislature in their Child Support Guidelines. Even if both parents agree, a monthly support amount cannot be less than required by the Guidelines, although it can certainly be greater. The monthly amount is based upon a number of factors. The primary factor is the income of each parent. If a parent is not employed the court will impute income, generally equal to a minimum wage, unless said parent has the ability to be employed at a higher income. Once each party’s percentage of contribution is determined, a Guideline Chart will establish the minimum support payment. The child support amount is also impacted by the number of overnight visitations exercised by each parent. Generally speaking, the more overnights exercised by the paying parent, the less support he/she will probably pay. Additional considerations that are included in determining the monthly payment are the cost of daycare/aftercare and health insurance premiums for the minor child’s coverage. Ultimately, whatever child support obligation applies to your family, that support is available to contribute towards the everyday expenses necessary to provide a roof over the child’s head, food on their table and clothes on their backs. n The law firm of Evan H. have worked 30 years in healthcare with executive business positions with several major traditional medicine companies and I have to admit that my experience has not been impressive relative to patient satisfaction or outcomes. Florida Real Estate: The Heart of It All. BY ZONA HORTON harles Payne of Fox Business News stated – the housing market is experiencing a “boom!” He explained that millenials were starting to settle down and form households – finally! All have heard that the real estate market is expected to do very well this year as mortgage rates remain at historic lows. Now we are facing a lack of homes available for sale. Move.com released a report showing that 2020 is beginning with the lowest available housing inventory in two years. The report explains: “Last month saw the largest year-over-year decline of housing inventory in almost three years with a dramatic 12 percent decline, pushing the number of homes for sale in the U.S. to the lowest level since January 2018.” George Ratiu, Senior Economist at Realtor.com, explains how this drop in available homes for sale comes at a time when more buyers are expected to enter the market: “The market is struggling with a large housing undersupply just as 4.8 million millennials are reaching 30-years of age in 2020, a prime age for many to purchase their first home. The significant inventory drop…is a harbinger of the continuing imbalance expected to plague this year’s markets, as the number of homes for sale are poised to reach historically low levels.” “Florida has been showing signs of growth for a decade,” says Miguel Caminos, U-Haul Company of Orlando president. “Central Florida is really booming. I can’t think of any major suburb where there aren’t home developments or new shopping centers being built. There’s an expectation of comfort for people moving here. They know there are jobs. Plus, there are tons of attractions, and our state is familyfriendly. The weather is perfect, and no matter where you live, you’re less than an hour from the beach.” Fort Lauderdale, which had been ranked as number 8 in 2018, is not ranked 19. However, we are typically out of land in Broward County, making the demand for housing enviable by other areas of the state and the country. U-Haul published that “Florida arrivals of one-way U-Haul trucks increased 1% while departures were down 1% compared to the state’s 2018 numbers. Arrivals accounted for 50.6% of all one-way U-Haul traffic in the state. Kissimmee, Ocala, West Palm Beach, Port Saint Lucie and the Bradenton-Sarasota corridor lead Florida’s gains. Boca Raton, Ft. Lauderdale, Pensacola, Miami and St. Petersburg are among the other notable cities to see a net increase of U-Haul trucks.” If You’re a Buyer: It may take time to find a home you love. Once you do, however, be ready to move forward quickly. Get pre-approved for a mortgage, be ready to make a competitive offer from the start. Understand that a shortage in inventory could lead to the resurgence of bidding wars. Calculate just how far you’re willing to go to secure a home, if you truly love it. If You’re a Seller: You’re in the driver’s seat. Don’t be unreasonable, but understand you probably have the upper hand. Homeownership is still the heartbeat of your wealth portfolio. Act on it!

I was thrilled to attend and give opening remarks at the recent dedication of the Veteran Access Clinic at Nova Southeastern University. This is another great day for our veterans and is a reminder that every day is Veterans Day in Davie!!!! The Soroptimist International of Davie members presented their poster contest awards ceremony that increases awareness across generation of the scourge of human trafficking. The winners were: First place, Natalie Sesodia from Cooper City High, second place, Rachel Kuperman from Majorly Stoneman Douglas High and third place, Shelby Baker from Western High. The young women and their posters will be presented at the Feb. 5th council meeting. I will be presenting my State of the Town address at the Davie/Cooper City Chamber luncheon on Tuesday, Feb. 18 at noon along with Mayor Gregg Ross from Cooper City. MSD Shooting Anniversary Inspires a Call to Action BY SHERIFF GREGORY TONY he senseless mass shooting at Marjory Stoneman Douglas High School two years ago affected the lives of so many. The tragic loss of 17 innocent victims compelled law enforcement, grieving families, community leaders, and everyday citizens to look for lasting solutions to mass violence in our county. Many were determined to get involved to make Broward County a safer place to live, and they did. We’ve come a long way; however, there is more we can do. At the Broward Sheriff’s Office, we take all threats of violence in our schools very seriously. Since launching our Real Time Crime Center in early 2019, we have investigated hundreds of these threats and made more than 50 arrests. In December, BSO arrested three Broward County students who made violent threats against a school. Today, every threat received is presumed to be real, and we take immediate action to investigate each one.
