October is Breast Cancer Awareness Month
L
ast year more than 2.3 million women were diagnosed with breast cancer and 685,000 died, according to the Breast Cancer Research Foundation (ABCF). Breast cancer is the most common form of cancer in women. It accounts for about ¼ of all cancers in women and 12 percent of all new cancer cases across all sexes. In the United States, there will be about 280,000 new cases of invasive breast cancer diagnosed. Despite massive progress in diagnosis and treatment, about 15% of those diagnosed will eventually succumb to the disease. But, there is hope. Over the past 45 years, the five-year survival rate has significantly increased for breast cancer patients. In fact, people with localized disease at the time of diagnosis now have a 99% five-year survival rate, while the survival rate for women with metastatic breast cancer, that has spread beyond her breast, is just 28%. Early detection is perhaps the most important thing. The ABCF recommends women should regularly conduct self-examinations beginning in their 20s and receive annual mammograms beginning in their 40s. There are many new promising therapies being tested and developed. More important than a new miracle drug, however, is identifying a lump or change in your body. And, if you do detect something, seek medical help. It could make all the difference.

alloween is going to be spooky again this year! The South Florida Cloggers are sponsoring a 2-day scary haunted house event called "The Legend of the Lodge Returns!" The haunted house will be held at Robbins Lodge in Robbins Lodge Park on October 26th and 27th. It is not for the faint of heart! Have you ever heard the legend of the lodge? Many years ago, a young girl named Robbin lived in the lodge with her family. One Halloween night, a terrible accident occurred. Over time, the lodge became abandoned. Rumor has it, Robbins lodge has been haunted ever since, and every once in a while, on a cool October night, you can still hear Robbin's screams for help. Join
us to find out what horrors still lurk in the darkness. For those looking for a fun Halloween event with a little bit less fright, the group will also host a “G” rated event on Sunday, October 31, from 1-5pm at the Bergeron Rodeo Grounds. The “not so haunted” maze will be a great opportunity for the kids to show off their costumes and maybe get a little candy! Broward County Public Schools Launches Coronavirus (COVID-19) Dashboard

roward County Public Schools (BCPS) launched a webbased dashboard to give parents up-to-date information on the number of within their school community who have tested positive for COVID-19. The dashboard, which is updated daily, can be broken down by students and faculty and is sortable by school.
The data are reported based on the day the positive test is reported to the school. Confirmed cases are as reported to the school system’s Risk Management and Coordinated Student Health Services, based on the date tested as reported to the District. Parents are cautioned that there could be delays in the reporting.
BCPS created the dashboard to give as much transparency as possible to families with children in school. The district reopened with mask mandates at all schools. The school board has been in a public battle with Governor Ron Desantis over the board’s decision to require masks. Western High School Hosted 20TH ANNIVERSARY 9/11 MEMORIAL CEREMONY

estern High School Students came together to pay homage to the thousands who lost their lives on September 11, 2001. Since 2002, the school has marked this day with a student-constructed memorial, which has served as a symbol for Western High students to host the We Will Never Forget ceremony. The monument replicates the Twin Towers, surrounded by the Pentagon, with a flagpole placed in the outline of the state of Pennsylvania. Local fire departments and law enforcement members participated in the “We Will Never Forget” ceremony alongside Western High JROTC, choral singers, band, and the Student Government Association.
“Today Western High School kept its promise that it made 20 years ago,” commented Western High School Principal Jimmy Arrojo, “we treat this event with great reverence and respect. This monument was designed and built by students.” He highlighted the fact that this was the first time in three years the school was able to hold the ceremony because of covid restrictions. Davie Mayor Judy Paul was also on hand, “Through all that devastation and all that destruction, these colors waived brightly in the air,” she commented, “and gave us the hope and the strength to continue.” The memorial was built in 2002 by sixteen students from Davie, Weston and Southwest Ranches as part of an engineering class. The ceremony takes
place annually on the September 11th or the closest school day to that date. Every aspect of the monument has significance. The seating area is in the shape of a pentagon, with two concrete pillars centered inside representing the World Trade Center towers. Behind the towers is a flagpole, its base imprinted in the shape of Pennsylvania.

EXERTIONAL COMPARTMENT SYNDROME IN THE ATHLETE
A Commonly Missed Diagnosis

hen shin splints are not shin splints. There many causes of leg pain in athletes. All too often, they are lumped together under the term shin splints which most sports medicine physicians do not consider a true diagnosis. Stress fractures, tendinitis, medial tibial stress syndrome are all examples of maladies referred to as shin splints. However, one commonly missed diagnosis often confused with shin splints is exertional compartment syndrome (ECS). Exertional compartment syndrome (ECS), also known as exertional induced compartment syndrome or chronic compartment syndrome is a unique condition where an athlete experiences muscle pain in the legs with weakness of the ankle as a result of increased muscle pressure in one or all four compartments of the leg. Patients with this condition will typically note the onset of symptoms at a fixed point ranging from 5-30 minutes into an activity, at a specific level of intensity, with symptoms usually occurring in both legs. The most characteristic feature of (ECS) is that it is exercise induced and never present at rest. The vast majority of patients with ECS are runners who range from the occasional jogger to the elite Olympic level runner. It is not unusual to see unconditioned athletes develop these symptoms after beginning an exercise program. Occasionally the problem is recognized in high-level cyclists as well.
symptoms are reproducible with exertion and should be recreated in the office setting with an exercise challenge such as running on a treadmill or stair climbing. Specific muscle and sensory testing after exertion will aid in confirming the diagnosis and specifying the compartment affected. During strenuous activity blood flow increases to the leg causing the volume and weight of the muscle to increase by about 20%. As a result, the muscle fibers attempt to swell but are restricted by the tight surrounding fascia. Immediately after activity the leg may feel tense or tender with deep message or passive stretch. Runners may find that a bubble or several bubbles are visible in front of the leg which we refer to as fascial hernias. These are areas where the fascia may have become weak allowing the muscle to herniate through due to the excessive internal leg pressure. The physical exam during rest is otherwise generally unremarkable. X-rays, magnetic resonance imaging, vascular studies and other imaging exams are used to rule out other diagnosis but are not helpful, as they are always normal in ECS. To confirm the diagnosis the sports physician will need to measure the pressure in each compartment of the leg with a special instrument much like a tire gauge. In normal patients, the pressure difference between rest and activity is small, while those with ECS will have a dramatic increase in pressure readings after activity.
Why does this happen?
Exercised-induced compartment syndrome is not to be confused with acute compartment syndrome which is considered a surgical emergency. Typically, acute compartment syndrome is a result of a traumatic injury causing a similar increase in compartment pressures due to hemorrhaging. In this situation, the pressure cannot be controlled easily and the delay in treatment may cause permanent muscle and nerve damage. In contrast, people with exercise-induced compartment syndrome will find that the pressure is relieved completely with rest. ECS is generally not considered an emergency.
There are generally 4 compartments in the leg, each is surrounded by tight tissue resembling a thick saran wrap which holds the compartment in place and separates it from the adjacent compartments. Patients with ECS will complain of cramping, burning, tightness and weakness of the ankle with exercise. Some will also develop numbness in the foot and describe a foot slap due to ankle weakness during running. This may cause the runner to trip as a result of the foot not being able to clear the ground just before landing. Pain will force the athlete to stop at which time symptoms completely resolve after resting a few minutes. The

What is the treatment?
A period of rest and avoiding high-intensity activities can be tried. Often more experienced athletes will recognize the level of intensity provoking symptoms and attempt to exercise just below that threshold. Muscle retraining by running more efficiently may also improve symptoms. However, if the diagnosis of ECS is clear and the symptoms persist despite exhausting conservative treatment, surgery may be required if the athlete intends to continue the sport. The surgery involves releasing or cutting the tight fascia using a technique called fasciotomy. This allows the muscle to expand during exercise. Complication rates are rare when done by an experienced surgeons and may include bleeding, wound breakdown, and numbness. Also, recurrence of symptoms can redevelop several years after a fasciotomy in a small percentage of patients as a result of the fascia reforming. The results of the procedure however are generally very gratifying and has allowed many athletes to remain active in their sport.

s Hurricane Ida picked up speed and intensity it became clear that coastal communities along Lousiana’s gulf coast were again going to face the worst. Residents put up storm shutters, boarded up windows, and purchased supplies. They cautiously watched digital tracking maps and made plans for possible evacuations. We were reminded, yet again, how important a Hurricane Kit is for families living in the tropics. Fortunately for us, the storm stayed to the west. Our neighbors along the gulf coast and in the northeast, of course, were not as fortunate. Hurricane Ida will likely be remembered as one of the worst natural disasters in US history. The storm came ashore as a category 4 storm with winds up to 150mph. The powerful storm dumped record rains all the way to New York City, with significant flooding all along its path. The South Florida community, local char-
itable organizations, and local businesses stepped into action immediately following the storm. In fact, even before the storm left the islands, South Floridians were organizing supplies, raising funds, and dreaming up logistics plans. Several large donation drives raised money and supplies for impacted communities. Initial estimates project the cost of the storm’s damage at $95 billion or more. Much of that in Northeastern states hit by torrential rains, putting the storm into a league with other deadly cyclones that have crippled the region in recent years, according to analysts. Debunking Myths about Breast Cancer By Alia Abdulla DO
Myth 1: Wearing a bra (with or without underwire) can cause breast cancer. A study, published in 2014, looked at the correlation between breast cancer and bras. It found that the patients diagnosed with breast cancer were NOT RELATED TO BRAS. They did find that people who take hormone replacement therapy, have a first-degree family history of breast cancer, or who have not had children all are at higher risk for breast cancer, however.
Myth 2: Wearing antiperspirant or deodorant can cause breast cancer.
Some rumors say that underarm deodorants and antiperspirants, especially those containing aluminum and other chemicals, are absorbed into the lymph nodes and make their way into breast cells, increasing cancer risk. Shaving the underarms is said to make this worse by creating tiny nicks that allow more of the chemicals to enter the body. Some think that antiperspirants, by stopping underarm sweating, can prevent
the release of toxic substances from the underarm lymph nodes, also increasing cancer risk. There is no evidence of a link between antiperspirant use and breast cancer. Some studies have found that women who use aluminum products under their arms are more likely to have higher concentrations of aluminum in breast tissue.
Myth 3: Carrying your cell phone in your bra can cause breast cancer.
There is no evidence of a connection between cell phones and breast cancer, but the safety of cell phones is still being studied. The research isn’t there to support this claim but cell phone manufacturers often do recommend keeping your device away from your body as much as possible.
Myth 4: Consuming too much sugar causes breast cancer.
There is no evidence that sugar in the diet causes breast cancer. There’s a common myth that sugar can feed the cancer and speed up its growth. All cells, whether cancerous or healthy, use the sugar in the
blood (called glucose) as fuel. While it’s true that cancer cells consume sugar more quickly than normal cells, there isn’t any evidence that excessive sugar consumption causes cancer. Eating too much sugar can lead to weight gain and being overweight is an established risk factor for breast cancer. In addition, some studies have linked diabetes with a higher risk of breast cancer — especially more aggressive, later-stage cancers.
Myth 5: All breast cancer is treated pretty much the same way.
Treatment plans vary widely depending on the characteristics of the cancer and patient preferences. Many people have a vague idea that breast cancer requires some combination of surgery, radiation therapy, and maybe chemotherapy, but they don't know much beyond that. ig things are on the horizon for 16-year-old International Dance Academy (IDA) student Paula Zapata. The aspiring ballerina will be packing her slippers and moving to New York City to join the Jackie Kennedy Onassis School at the American Ballet Theater (ABT). The prestigious pre-professional ballet training program is available, by invitation only, to a limited number of student dancers, ages 12-17, from The United States and around the world. “It is a dream come true for a dancer who sets her goals at dancing professionally on the world’s finest stages,” exclaimed Mary Buttner, Director of International Dance Academy. “Since Paula walked through our doors, we immediately knew that she was special. We just knew we had someone so special and quite phenomenal. It is an honor of breathtaking proportions, and we are so proud!” Ten years ago, Paula’s parents never expected the “tutu” cute six-year-old to follow her dream and excel and achieve an honor of such magnitude. Like all great parents, they just wanted to give their daughter the very best opportunity to learn what she desired at that stage in her life. Fortunately, they found International Dance Academy and Paula found her second home. “The environment at IDA is amazing. It has been the best place for Paula to learn and train. She knew what she wanted at such a young age, and I watched her passion grow along with her love of the art here at the Academy,” explained Paula’s mother, Diana Zapata. The ambitious dancer was encouraged by her teacher, Deborah Buttner, Artistic Director of the International Dance Academy in Weston to audition for the ABT

She impressed the ABT talent scouts and teachers at the distinguished program so much that they invited her to join their program this fall at their impressive school in New York City. Paula has excelled in all aspects and gives 110% in every class. She also performs major roles on stage in IDA’s professional IBC productions such as Nutcracker, Little Red Riding Hood, and so many more. Additionally, throughout the years she has assisted Mary Buttner by demonstrating techniques to the younger pupils. The eager ballerina also attained the esteemed Advanced I Diploma in the Royal Academy of Dance examinations “We realized at that young age that Paula was going to shine, and she sure has!” said Deborah Buttner. “All these years she has shown her dedication, work ethic, and ability to focus and work extra work. Paula is a humble, kind, and beautiful young lady with a huge future ahead of her. By Bruce Iden, Esquire have a will and it states that my home is being left to my children. Should I be concerned about probate? Yes. The reality is that your home, even if left to heirs in a will, cannot be legally sold without permission of the court, which means that your property must go through a probate administration. It is common knowledge that probate administration is costly and time consuming. Unfortunately, the process only moves as fast as the court dictates. My partner and I own our home together but we have not executed wills. What will happen to my home after I am gone? Most married couples own their home via ” tenants by the entireties,” and ownership transfers automatically to the surviving spouse. Once the surviving spouse passes, the home is subject to probate with ownership determined by the surviving spouse’s will, or if none, by the court. If a home is owned via “joint tenants with rights of survivorship” the deceased’s share will pass to the other joint owner, who may not be their spouse. Your situation can be addressed by consulting with an estate planning attorney who is also familiar with real estate matters. My spouse and I created a trust, but our attorney did not discuss our home ownership. Why? Addressing a client’s assets and assuring that they are not subject to probate is part of estate planning, often referred to as funding. In my practice, offering a blueprint for funding, including home ownership, is part of the process. Unfortunately, there are attorneys that create trusts and do not provide funding assistance. I recommend that you reach out to your estate planning attorney to ask about funding, addressing all of your assets including any real property you may own. So, how do I guarantee that my home is not subject to probate? There are several types of deeds that assure a home transfers to a new owner without the participation of the probate court. This is certainly not a do it yourself project, and should be done by someone who not only has experience with real property but understands the estate planning process. Your goal is to prevent problems down the road. My cousin sold his mom’s house after she died. She didn’t have a special deed, and he did not go through probate. How was he able to do this? Your cousin may have owned the house jointly with his mom, or she may have created a life estate deed naming your cousin as the remainderman. My spouse and I each have children from previous marriages. I’d like my home to eventually go to all our children. After I am gone, can my spouse change the deed so it only goes to her children? Yes, she can. With so many blended families, this is a common concern. But with creative planning, you can be assured that all children ultimately receive the house as an inheritance, regardless of which spouse passes first.
