Happy Valentine’s Day

W

hile it seems like just yesterday we were

ringing in the New Year, now we turn our attention to the ones we love with Valentine’s Day just weeks

away. The stores are stocked with sweets and treats for our sweethearts. While we contemplate which gift we should purchase or which restaurant we shall visit for that romantic night out, we need to remember that the monetary value of a gift is a secondary notion. The most important point is that the gift comes from the heart -- and there is no better gift than spending valuable time with the ones we love. This Valentine’s Day, give from your heart and enjoy each other’s company. Happy Valentine’s Day!

Cooper City Couples to Say I Do…Again By Debby Teich

V

alentine’ s Day is a time for

love and romance. More than 25 Cooper City couples are expected to celebrate the holiday by renewing their vows during the “Say I Do…Again” ceremony on February 14, at 6:30 p.m., at Cooper City Hall. The event is free for Cooper City couples and their guests. Former Cooper City Mayor Suellen Fardelmann officiated the first “Say I Do… Again” event in 1989, when four Cooper City couples re-affirmed their vows in a group ceremony in front of friends and family. Over the years the number of couples in attendance has grown. Some come to celebrate their first anniversaries, while others come back every year and plan their Valentine’s celebration around it. “Say I Do…Again allows our resident couples to share a very special day of their life with others in their community,” said Lynda Bragg Good, Director Cooper City Recreation Department. “The love, laughter and memories made during this event make it a success!” The complimentary ceremony includes a walk down the aisle, flowers, photo under

the wedding arch, customary cutting of the cake, refreshments, coupons and prizes from local businesses, and a certificate of re-affirmation. There will be a table filled with each couple’s wedding photo. During the registration process, each couple provides information about themselves, such as how many years they have been married, where they met, and how many children they have. Cooper City Mayor Greg Ross will preside over the evening and perform the Re-Affirmation Ceremony. As each one is called up individually to “walk the aisle,” he will share some of this information with others at the event. Participants must be residents of Cooper City and sign up by February 11. For information, call 954-434-4300, ext. 233 or visit www.coopercityfl.org.

The parade will begin at 10am on Saturday and will proceed down Davie Road between Orange Drive and SW 39 Street. Learn about Davie’s Pioneers at the Old Davie School Historical Museum By Debby Teich

Volunteers reenact pioneer life during the 2013 Orange Blossom Festival.

I

f you have ever wondered how Davie’ s

pioneers lived without all of today’s modern conveniences, visit the Pioneer Village area of the Old Davie School Historical Museum. During the Orange Blossom Festival, volunteers will give visitors a taste of what life was like in the early 1900’s. Reenactments will be held on February 22 and 23, from 12 to 2. Dressed in pioneer clothing, volunteers will take visitors on a tour of a replica of a one room house from 1909. They will explain how the pioneers lived without electricity, shared one bed, used an outhouse, pumped their own water, sewed quilts, and more. Visitors can also sample food that was cooked on the fire pit, such as pineapple upside down cake, coffee, green beans and soup. “When the kids visit the house, they want to know how to turn on the lights and where the pioneers shopped for food,” said Leslie Schroeder, Director of the Old Davie School Historical Musuem. “Visiting the village is a great way to get an understanding of the trials and tribulations of living as a pioneer.” During the Festival, free guided tours of the Old Davie School Historical Museum will also be available each day at 12noon, 1pm and 2pm. The museum contains Indian relics, old gas pumps, a mounted 14 ½ foot alligator and other wildlife from the Everglades, photos from the Great Flood, and more. Visitors can also explore a replica of a 1920’s classroom.

The Old Davie School Historical Museum is located on the south side of Griffin Road. To reach it from the Orange Blossom Festival, use the footbridge to cross the canal. The Davie School Foundation was established in 1984 to protect and restore the Davie School to the integrity of its earlier appearance. The Foundation, in conjunction with the Town of Davie, the Broward County School Board, the Soroptimist International of Davie, The Davie Historical Society, and the community at large has worked tirelessly to preserve this important piece of Broward County’s history. The Old Davie School Historical Museum is a 501(c)3 non-profit organization located at 6650 Griffin Road. For information, visit www.olddavieschool.org.

raising funds to enhance the lives of less fortunate children in South Florida. On February 10, however, it will focus on bringing together more than 250 people for the Touch A Heart Foundation’s Live, Love and Give Back Day. For a $100 donation, guests will have an opportunity to relax, shop, eat, and learn, as they help make the Foundation’s mission a reality. “This year’s event will focus on health, wellness and enrichment,” said Deborah Perlman, co-founder of Touch A Heart. “We believe our guests will be inspired by the wonderful speakers throughout the day, and enjoy the company of good friends, great food and a shared passion of giving. Anthony Recovery Center and more. During Back to School, the Foundation adopts approximately 100 students who receive backpacks filled with school supplies. It also assists agencies with items for Easter baskets and provides gift cards for Passover meals. During the last holiday season, the Foundation was able to assist over 100 families, 8 under privileged daycares, and 2 group homes. The donors shopped and wrapped the gifts. They were also able to visit with the families and daycare centers they were helping. “We believe that one of the things that makes Touch A Heart so special is that donors can experience one-on-one time with the family or daycare they have chosen to assist,” said Lori Samuels, co-founder of Touch A Heart. “They get to enjoy the many smiling faces.” Everyone at Touch A Heart is a volunteer; there are no paid positions. Many of the Foundation’s volunteers are youth, who volunteer in return for service hours. They get involved with the wrapping, shopping and delivering, as well as many other projects. Members of the business community have also adopted a family(s) as a corporate donation. To participate in the retreat, call 954-566-0644/954-802-0562 or send an email to bonmcnair@aol.com. Since June 2012, Andrew has spent most weekends working on the boat with the help of some Coast Guard Auxiliary members, his two brothers who are in Boy Scouts, and some members of his own troop. Together they added fire extinguishers, buoys, life jackets, a first aid kit, an antenna, and a new GPS for navigation. They also replaced the seats, painted and did some mechanical work on the engine. “The boat is now seaworthy,” said Andrew. “It can be used for navigation, training and patrols.” Andrew lives in Davie and is currently a senior at Archbishop McCarthy High School. Davie Mom Raises Awareness of Concussion Injuries through Daniel’s Dash 5K Run/Walk edit author

M

any young athletes are reluctant to report symptoms of

brain trauma out of fear that it will keep them on the sidelines. They often play through a concussion to avoid letting down their teammates, coaches, schools and parents. On March 8, at 7:30am, hundreds will gather at Markham Park for the 3rd Annual Daniel’s Dash for Concussion Awareness 5K Run/Walk to learn more about the dangers of concussion. The event was started by Davie resident Diana Brett in memory of her son, Daniel. In May 2011, Daniel took his own life as a result of disabilities he suffered from concussions, as well as delayed diagnosis and treatment. Diana has been sharing his story to prevent the same thing from happening again. Daniel started playing football when he was 11. Although he was not very big, he was tough and fearless. He believed in the motto “no pain, no gain.” Like many teenage boys, Daniel dreamed of playing football in high school and college. On August 24, 2009, he was excited to learn that he would be the starting linebacker for his high school’s JV team. Unfortunately, it was also the last day he would play football, because concussion-related injuries forced him to quit the game “All he wanted to do was play,” said Diana. “If he had a headache, he would take Tylenol and blame it on a tight helmet. He did not understand the dire consequences of hiding or underreporting concussions just to stay in the game. There is a common misconception that concussions are only caused by big, hard hits, but smaller hits can also cause concussions.” Diana wanted to find a way for something good to come of Daniel’s death. She started the Daniel Brett Foundation, a 501 (3) (c) organization dedicated to raising concussion awareness and to benefiting student athletes. Through the Foundation, she has been fighting for a standardization of protocols for injured, young athletes

in Florida. She has been working to help institute concussion training, awareness, procedures, and testing in our communities and schools. The 3rd Annual Daniel’s Dash for Concussion Awareness In March 2012, 5K Run/Walk will be held on March 8. the School Board of Broward County adopted a Resolution mandating baseline concussion testing for all high school athletes, making it easier to diagnose concussions on an individual level. Before participating in sports, all Broward public high school athletes are now required to undergo ImPACT (Immediate Post-Concussion Assessment and Cognitive Testing). In April 2012, the state adopted a bill mandating that all young athletes suspected of sustaining a concussion or head injury be removed from practice or competition immediately. They cannot return until they get written medical clearance from a health-care provider. “Daniel was an amazing young man, with a kind heart,” said Diana. “He loved life, music, friends and family, and had a real zest for football. It is a parent’s worst nightmare to see their child suffer. We cannot let it happen again.” For information, visit www.danielsdash.org or “Daniel’s Dash for Concussion Awareness” on Facebook.

Hormones and Marriage By Dr. Jean-Claude Nerette

W

hen you said “I D o,” you never imagined that years later you would lose all interest in a sex life and find your “soul-mate” unattractive and irritating. You find yourself constantly battling fatigue, as everything you do requires so much effort. You don’t even know who you are when you hear yourself talking, as the mood swings consume your whole being. How can you possibly maintain a happy marriage when you’re not even happy with the person you are becoming? Can you control this? Or is this simply a normal progression of married life and you just have to deal with it? I am here to say “NO.” You absolutely don’t need to live this life. There are options. A lot of the symptoms stated above I hear on a daily basis, and many women and men are suffering unnecessarily. I am here to tell you that you can recoup that basic marital foundation – intimacy. You can get rid of the fatigue and mood swings and become the person you were when you first married.

Bio-Identical Hormone Replacement Reverse The Signs of Aging

Lose Weight, Decrease Blood Pressure & Improve Health…The Natural Way The solution is simple….it’s all about hormones. As we age our hormones start decreasing and as a result symptoms appear. The most common symptoms are lack of sex drive, fatigue, depression, weight gain in the mid-section, unable to multi-task and think clearly, mood swings, and hot flashes. All these symptoms can not only impact you as an individual, but obviously your relationship. A simple blood test is all that is needed to determine if you are lacking hormones. Once the deficiencies are identified, an individualized plan should be created to maximize your hormone levels. The type of hormone chosen - synthetic versus bio-identical - is imperative. Bio-identical hormones mimic exactly your own body’s molecules versus their synthetic counterparts, with known cancercausing agents within their chemical structure. The methods of bio-identical hormone delivery can include sub-lingual liquids, troches, creams and pellets. More important is that the delivery method must be constant - with no high peaks and lows. All methods of delivery, except the pellet, unfortunately give you an inconsistent absorption of hormones. When you place a cream on the skin in our hot humid climate of South Florida, how much are you exactly absorbing and what is wearing off? When you place the liquid or troche hormone underneath the tongue you have immediate absorption, but what is left to be absorbed the remainder of the day? The absorption rates with these methods are extremely unpredictable, making it really hard to manage the symptoms and regain control of your life. The bio-identical hormone pellet provides a consistent flow of hormone delivery. The pellet, which is less than the size of a tic-tac, is inserted underneath the skin. Therefore, the body has 24 hour access to the hormone, using it as needed. The pellets last up to six months, thus avoiding the hassle of taking daily medications. The absorption is predictable and monitored with lab values. The benefits are seen as quickly as five days. Some of the testimonials I have heard are: “Thank you for giving me my wife back,” “I am more centered as a human being,” and “I haven’t been this intimate with my husband since our honeymoon.” So you decide: Is bio-identical hormone pellet therapy right for you? Can your marriage be improved? Call us to schedule a free consultation 954-384-8989. Dr. Jean-Claude Nerette is Board Certified in Internal Medicine and Stephanie Muniz Nerette is Board Certified as a Family Nurse Practitioner. They both specialize in Functional Medicine, Bio-Identical Hormone Pellet Therapy and IV Nutrition Therapy. Their practice Bellissimo Medical is located at 2665 Executive Park Drive, Suite #3, in Weston.

Before I talk about my confession, though, let me say a few other things first.

Let me start by explaining the photo in this letter. You know, when I meet people in town they usually say, “Oh, yeah, I know you, you’re Dr. Rick. I’ve seen your ad with that picture of you and your family.” Well, I’m the guy in the back. Twenty-five years ago something happened to me that changed my life forever. I was a junior in high school and I started getting migraines. I had suffered from intense headaches since childhood. Over the years, I consulted many medical doctors, and specialists. I went through all kinds of tests, MRIs, and treatments. I tried every drug in the market; our medicine cabinet looked like a hospital’s infirmary. And still, nothing helped. But there’s more…

Several times a day patients thank me for helping them with their health problems. But I can’t really take the credit. My confession is that I’ve never healed anyone of anything. What I do is perform a specific spinal adjustment to remove nerve pressure, and the body responds by healing itself. We get tremendous results. It’s as simple as that!

Scholarship Fund Created for NSU’s Mailman Segal Center for Human Development By Debby Teich

N

ecessary therapies and educational

programs for children with autism can be very expensive. A new scholarship fund will allow more families to send children to early intervention programs at Nova Southeastern University’s Mailman Segal Center for Human Development. On behalf of young children with autism and their families, Interiors by Steven G. owner Steven Gurowitz recently donated $50,000 to the university to create the Interiors by Steven G. Scholarship Fund. The fund will provide financial support for students with autism who are enrolled in two early intervention programs at the university: the Starting Right parent/child program and the Academy school-based program. “The university is very grateful for Steven’s incredibly significant and meaningful donation,” said Dr. Roni Leiderman, the dean of Nova Southeastern University’s Mailman Segal Center for Human Development. “His scholarship fund will help make a positive difference in the lives of the university’s children and their families.” Starting Right is an early intervention, parentchild class for children, between the ages of 18 months and 3 years, that exhibit delays in language

and social skills. The program is designed to increase communication, social and school readiness skills. It also provides caregivers with techniques and strategies to use in the classroom and at home. The Academy program is for children between the ages of 3 and 7. It is designed to provide a private school, kindergarten option for students who require an individualized education program that focuses on social-communication, behavioral self-regulation, and independent functioning. Although Steven G. does not have any direct ties to someone with autism, he was inspired to make a generous donation because he feels that all kids deserve a chance. He believes that John F. Kennedy said it best, “One person can make a difference, and everyone should try.” “We seldom stop to think about how our day-to-day lives could be so challenged if we were in the shoes of so many that are affected by autism,” said Steven G. “I hope and pray that my contribution can contribute to the growth of the educational programs and therapies that are available.” For information, visit www.nova.edu/humandevelopment.

Unite Broward By Sheriff Scott J. Israel

A

s sheriff of the 1.8 million residents of

Broward County, I recognize that our community is diverse. It is comprised of people from many races, religions and cultures, and each needs to be heard. Broward is a minority-majority county, and in order to better protect and represent every community, I am committed to extending the lines of communication to effectively respond to any unmet needs. I understand that in many cultures and in certain instances voicing concerns to a deputy with a badge and a gun can be intimidating. To bridge the gap between law enforcement and the community, BSO must earn your trust. We have made significant strides in achieving this through a change in the culture at BSO, particularly through our Department of Community Outreach. Community Outreach and the chaplain’s office are essential in facilitating meetings with many diverse religious groups. Aside from my family’s own blended faith and our experiences with Christian and Jewish services, I attend many religious and cultural ceremonies to gain a better understanding of other faiths. These experiences provide me the opportunity to realize what matters to each community. In turn, secular members express how thankful and grateful they are to see the Broward Sheriff’s Office take such an active approach to working together. Many faith communities have remarked that trust between BSO and their congregation has grown and is flourishing because they know their opinions matter. Although BSO has always had an open-door policy, not every voice was heard. Today, BSO is an agency committed to listening, which has resulted in all faiths being represented. I continue to encourage and invite all religious institutions to discuss what matters and how we can work together to achieve those goals. 2013 marked a crucial turning point in the culture and traditions of the Broward Sheriff’s Office. I was determined that BSO play a noticeable and active role in many events for organizations such as NAMI, HANDY, the YMCA and the NAACP — organizations that voice the concerns of Broward County. By joining the Jimmy Cefalo Radio Show every Wednesday morning on WIOD, I was also able to communicate what the men and women of the Broward Sheriff’s Office were doing for every community, and spread the word that BSO is here to help. Today, community members are more at ease; they bring matters to our attention instead of waiting for them to escalate. BSO has always been an agency of proud tradition. Now a cornerstone of that tradition is to make a positive impact — one that encourages trust, reliability and dependability through the establishment of a two-way street of communication. Community leaders recognize a noticeable cultural change has taken place at the Broward Sheriff’s Office, and I look forward to continuing this in 2014. Another facet of BSO’s cultural transformation is that among our personnel, we encourage ethnic pride, religious heritage and cultural identity, and we do so in a manner that honors inclusiveness. The men and women of BSO will continue to reflect and represent the individuals we protect and serve because they are hired and promoted based on their talents and abilities. When you hire the right person for the right position, diversity and morale in the work place take care of themselves. BSO has become an agency where supervisors mentor and leaders lead by example. BSO personnel are encouraged to rely on these values and the spirit of inclusion whenever they interact with the public. Public safety requires involvement from the community. I want every member of our community to know that they have my attention, my ear and my respect. At the Broward Sheriff’s Office, your voice will be heard and understood. We are all one community and must work together. To stay up-to-date about BSO events, crime trends and safety tips, sign up for Sheriff Israel’s e-Alerts at www.sheriff.org/e-alert or join our Facebook fan page at Broward Sheriff’s Office (official) or follow us on Twitter @browardsheriff.

Dental decay is the most common chronic disease in children—five times more common than asthma. Yet with good oral hygiene habits, such as brushing and flossing, cavities are largely preventable. Recent research shows that children with poor oral health are more likely to have below average grades than kids who have good oral hygiene. According to a recent study in the American Journal of Public Health, researchers found students who experience tooth pain are “four times more likely to have a low grade point average – below the median GPA of 2.8” than their classmates. February is Children’s Dental Health Month. There are some simple things you can do to keep your children’s teeth from interfering with their education: ▸▸ Brush your child’s teeth yourself until age 6 and supervise brushing until age 8. Children may not have the dexterity to do it on their own effectively. ▸▸ Make sure your child brushes twice a day for at least two minutes--top, bottom and sides--not just the front teeth. ▸▸ Use a toothbrush with soft bristles. Remember to replace your child’s toothbrush every 3-4 months. ▸▸ Be sure to clean between their teeth once a day with floss to remove plaque and food where a toothbrush can’t reach. ▸▸ Visit the dentist twice a year beginning at age one. ▸▸ The American Association of Orthodontists recommends that all children receive an exam by an orthodontic specialist by age 7. This evaluation will help to diagnose any problems with their bite. The goal is for every child to have a healthy and beautiful smile for a lifetime.

What Role Does Orthodontics Play in Oral Health?

Straight teeth make for a beautiful smile and they are also easier to keep clean. Crowded and improperly positioned teeth are more difficult to brush and floss. This can lead to a higher risk for gum disease, tooth decay, and tooth loss. Orthodontics is often necessary to help improve the stability, function, and health of an individual’s teeth. Children, who receive compliments on their beautiful teeth, smile more. More smiles translate into a friendlier demeanor and higher self-esteem. According to the American Association of Orthodontists, “It has been clinically proven that a beautiful smile leads to higher self-esteem. Confidence generates success.” I am a firm believer that children are our highest priority. All children need and deserve a good smile. An uncorrected problem can adversely affect an individual’s speech, general health and self-esteem. A person’s self-confidence almost always increases when his or her smile and appearance are improved. The value of a beautiful smile cannot be underestimated.

Dr. Michael Maruri and his staff at Clear Orthodontic Solutions understand your child’s smile is just as important as their performance in school, which is why they offer convenient early-morning, after-school, and Saturday appointment times. Dr. Maruri offers a variety of treatment options to meet the unique needs of his patients, including traditional metal, clear ceramic braces, lingual braces, Invisalign, and Invisalign Teen. ▸▸ Faded walls and worn woodwork must be painted and/or refinished. Torn or dated wallpaper must come down. ▸▸ Open the drapes and curtains. Clean the windows. Home buyers are drawn to bright, cheery interiors. ▸▸ Dripping water suggests worn out plumbing. Clean those rust-stained sinks. Make sure the drains are running free and clear. ▸▸ Loose door knobs, sticking drawers, wobbly hinges and stuck windows can cost you a sale. ▸▸ Keep stairways and corridors clear and free of clutter. Cluttered areas are not only unattractive, they are dangerous. ▸▸ Inspect your garage – be sure that it is neat and organized. ▸▸ Give your storage spaces a clean coat of paint. Empty all trash receptacles.

▸▸ Get rid of piles of clothes and other clutter. ▸▸ Make bathrooms sparkle. Clean sinks and bowls. Re-caulk where needed. Make sure towels and area rugs are freshly washed. Use white towels only for a spa-like effect. Bathrooms are big sellers.

▸▸ Remove excess furniture. Use attractive and colorful bed linens and spreads; however, white will brighten any room regardless of the colors you have on the wall. Sandstones and earth tones soften the room. Open the drapes and let the light in. Remember: this is where your new buyers spend one-third of their lives. ▸▸ Buyers love the light. By day, let the sun shine in. By night, turn on all your lights – inside and out. Landscape lighting gives any home the look of luxury. ▸▸ Refrain from selling your prospect old furniture, appliances, lava lamps or anything besides your house. ▸▸ You never know when your agent may need to show your home at a moment’s notice. Make your beds and tidy up each morning, just in case. Be prepared. ▸▸ Refrain from engaging your prospects in conversation. Let your sales professional do his or her job – and let your buyers inspect without interruption. ▸▸ Make your pets disappear when your home is being shown.

section Ask the Expert

Timing Edit Headline of Tooth Eruption By Dr. Maria Grosman

O

ne of the most common anxieties parents

share with us concerns the timing of the eruption of teeth. We are often asked: “When are the first teeth scheduled to enter the mouth?” or “Which is ‘better,’ late vs. early erupting teeth?” The truth of the matter is that although there are published milestones, there is still a normal variation/range that teeth may be expected to erupt. In terms of which is better, there is no real benefit for early or late eruption. However, extreme deviations may cause a practical challenge, as we will discuss. Let’s start with the primary (or “baby”) teeth. The first primary tooth to erupt is usually the lower central incisor between 5-8 months of age. The teeth will then follow the “rule of twos,” which is that two additional teeth will erupt every two months until around 2-2.5 yrs old. There are a total of 20 primary teeth equally divided between upper and lower arches. After the eruption of the lower central incisors, we will see the eruption of the upper central incisors, the lower lateral incisors (as the name suggests, next to the central incisors), followed by the upper lateral incisors. The pattern will continue from the center laterally, with the lower erupting earlier than the upper counterparts.

Premature eruption of primary teeth occurs in 1:2000. This can occur in two ways. “Natal” teeth are present at birth and “prenatal” teeth erupt in the first 30 days of life. When teeth erupt prematurely, it is important to consult with your healthcare provider (pediatrician or pediatric dentist), as strategies for feeding will need to be addressed, especially if breastfeeding is planned. There are also structures which are present at birth and resemble premature teeth, but are actually not the primary teeth. One of the most common examples of this is “Epstein pearls.” They are visible cysts located in the upper arch in the front close to where the primary teeth are expected to be positioned. There is no treatment recommended for this, as long as there is no interference with feeding and no observed discomfort for the child. As with all published milestones, it is important to acknowledge the fact that these are just averages and that “normal” has a range. We find variation in gender, with females being ahead of males. It is also important to note that deviations in one area may have no bearing on others. For example, there are many children who are either “ahead” or “behind” in dental development, but are at the opposite ends of the spectrum in skeletal growth. They do, however, give us an idea of what to plan for and some guidance on timing of interventional treatment, whether dental or orthodontic, when indicated. Drs. Maria and Alon Grosman are Board Certified Pediatric Dentist and Orthodontist. Their office is located at 4401 S Flamingo Rd on the NW corner of Griffin and Flamingo Rd. When “Leaky” Valves Are at the Heart of the Problem Your heart has four valves. They open during a heartbeat so that the blood can flow from one chamber to another and close to make sure that blood can’t leak back out. Mitral regurgitation occurs when the valve between the upper and lower chambers on the left side of the heart does not close properly. Mitral regurgitation can be caused by progressive structural changes in the leaflets of the mitral valve, certain infectious diseases and some heart attacks. Mitral valve prolapse is a structural abnormality of the valve where the leaflets have extra tissue and are displaced from their normal position (too far backward). A small percentage of patients with mitral valve prolapse will eventually develop regurgitation. “Symptoms of mitral regurgitation include shortness of breath or difficulty catching your breath (especially with exertion), palpitations that feel like rapid or irregular heartbeats, and swelling of the feet and ankles, said Edward Savage, MD, cardiothoracic surgeon at Cleveland Clinic Florida. “Other symptoms are weakness or dizziness, and increasing fatigue.”

Edward Savage, MD Cleveland Clinic Florida Cardiothoracic Surgeon

An important part of the diagnosis is recent medical history. During a physical examination, your doctor may look for fluid in the lungs, an enlarged heart, or a heart murmur, which is the sound made when blood moves through a stiff or leaky valve. Your doctor may suggest further testing for evaluation. The test to diagnose mitral regurgitation is an echocardiogram. Sound waves are aimed at the heart allowing a detailed picture of the structure of the valves, which aides in the assessment of leaks. The strength of the heart muscle can also be assessed. Sometimes a higher resolution echocardiogram is needed to accurately assess the mitral valve. This is performed by passing a sound-generating transducer into the esophagus. This study is a TEE or transesophageal echocardiogram. Not all leaky valves require surgery. If the leak is mild or moderate, it is usually treated with medication and close follow-up. However, if the leak is severe, even if there are no clear symptoms, surgery is often needed. Sometimes patients who believe they have no symptoms actually do, but have adapted to their limitations. The only treatment currently available for severe mitral regurgitation is surgery. It is important to treat the problem before the heart has lost significant functional capacity. In most cases the valve can be repaired (>80%). Studies show that mitral valve repair results in better long-term survival, more preservation of heart function and lower risk of complications. If the valve can be repaired, no long-term use of blood thinners (anticoagulants) is required. Some valves cannot be repaired and require replacement. Whether repaired or replaced, most patients experience dramatic improvement in symptoms and functional status. Nearly all isolated mitral valve repairs or replacements can be performed using a minimally invasive approach through a 2-3-inch incision on the right side of the chest. Cleveland Clinic Florida specialists are experts with extensive experience. They specialize in the diagnosis and treatment of patients with all levels of heart disease, from the most simple to the very complex.

he issue of Alimony R eform will once again be

addressed by the Florida Legislature in 2014. Both the Florida House and Senate passed sweeping changes last year, only to have Governor Scott veto the bill at the 11th hour. The primary focus of the new law was to end what we now call permanent alimony. This classification of alimony can be required in long term marriages, which are defined as marriages 17 years or greater. Permanent alimony will terminate upon the death of either party, or the remarriage of the receiving spouse. In addition, it can be modified (increased or reduced) if there a substantial change of circumstance occurs. A substantial change of circumstance usually refers to a change in the finances of either party. If either spouse loses his or her job and a new job results in a lower income; that would be considered a substantial change of circumstance. The change, however, cannot be voluntary in nature. A person cannot quit his/ her job and then ask the court for some relief due to this intentional act. Permanent Alimony can also be terminated or modified if a “supportive relationship” exists. The question is, “what exactly is a supportive relationship?” Florida Statutes does in fact define such a relationship, but it is certainly open to interpretation. It exists when a couple, who is not legally married, do in essence hold themselves out as a married couple. Many individuals who receive permanent alimony will avoid re-marriage, since it will end alimony by operation of law. The Supportive Relationship Statute was enacted to deal with couples who are in essence married, but not by operation of law. The Courts are to consider the following in determining whether a supportive relationship exists: ▸▸ The extent which the couple has held themselves out as a married couple or otherwise conducted themselves in a manner that evidences a permanent supportive relationship. ▸▸ The amount of time the parties have lived together. ▸▸ Have the parties pooled their income or assets or otherwise exhibited financial interdependence?

▸▸ The extent that either party has supported the other. ▸▸ The extent of which either party has performed valuable services for the other party. ▸▸ Whether the parties have jointly contributed to the purchase of real property. ▸▸ Whether the parties have an express or implied agreement concerning support or property sharing. The above criteria clearly show that there is much room for interpretation by a judge. It is believed that the Florida legislature will in fact address this statute in an attempt to clarify the intent. If you are someone who is either receiving or paying alimony, it would be a good idea to keep up with news from Tallahassee, once the legislature is back in session. The law firm of Evan H. Most Common Mistakes by Homeowners Attempting to Obtain a Loan Modification By Omar J. Arcia, Esq. Foreclosure Defense and Consumer Bankruptcy Protection Attorney

A

s a result of various nationwide class actions and lawsuits

filed against mortgage companies by private individuals and government agencies, lenders have become more receptive and willing to assist homeowners in modifying their loans. However, lenders have also become more demanding of certain documentation to support loan modification requests. These are the most common mistakes we’ve seen homeowners make when processing a loan modification on their own. Provide Too Much Information. Most Homeowners believe that the more information they provide to their lender about their finances, the more likely their loan modification will be approved. Just the opposite is true. Most lenders are looking for specific information regarding a homeowner’s income and expenses, specific reasons to support a financial hardship for a loan modification, and they require a specific order and timeframe to submit the required documents. Overwhelming your lender with the same unnecessary documents time and time again will only place your file farther back in line for consideration. The Arcia Law Firm has represented homeowners that have been trying to modify their home loan for years, and within less than 30 days after retaining the Firm and submitting the appropriate documents, a loan modification is achieved. Refusing to Seek Legal Assistance. We have counseled hundreds of clients in mortgage related cases through the years, and the most significant mistake that homeowners make is to wait until the last minute to hire a knowledgeable and experienced attorney. Many homeowners think that they can easily defend themselves in a foreclosure proceeding, process their own loan modification, and save their home. However, some of the legal defenses to a foreclosure are highly technical, constantly changing, and require specialized knowledge. The attorneys representing banks and the judges overseeing in these cases are primarily interested in achieving an end result -- obtaining a final judgment of foreclosure, sale of the property at a public auction and closing the case. In more simple terms, this means you are out of your home before you can finish your loan modification negotiations. The key to prevent this end result is for every homeowner either in foreclosure or at risk of foreclosure, to stop playing “Russian Roulette” with their family home, and immediately hire a highly qualified law firm to assist them. The attorneys at the Arcia Law Firm have more than 40 years combined legal experience, and we are privileged to successfully represent hundreds of homeowners in our community with foreclosure defense and bankruptcy cases. If you are struggling to make your monthly mortgage payments, if you are in the middle of a frustrating loan modification process, or if you just received a foreclosure summons, contact the Arcia Law Firm today at 954-437-9066 to schedule a free consultation, or visit www.arcialawfirm.com to schedule your appointment online. Detailed explanations of different options available to homeowners are also discussed in a new DVD developed by the Arcia Law Firm entitled “Fight For Your Home.” If you mention that you heard about the DVD through this magazine, you will receive a complimentary copy during your free consultation to discuss your case in detail with a member of our highly experienced and determined legal team. ask the expert

Arthritis in the Foot and Ankle By Robert H. Sheinberg, D.P.M., D.A.B.P.S., F.A.C.F.A.S.

A

rthritis is a common ailment, especially as

people age. Here are answers to some commonly asked questions about arthritis: ▸▸ What is arthritis? Arthritis is a degenerative process that may take place in any joint in the body. Cartilage which allows the joint to move smoothly starts to wear down. Bone spurring and bone cysts may occur as the joint wears abnormally. The joint starts to move out of its normal position creating pain with movement. A loss of joint movement also follows the arthritic process. ▸▸ What causes arthritis? Most often arthritis is caused from an injury to the joint. The injury may occur as a child and it may manifest itself later in life. A bad injury to a bone or joint (fracture) may start the development of arthritis almost immediately after the injury. Arthritis is very common when a bone fractures into the joint. Malpositioning of the joint will cause abnormal wear and tear. It may also be seen in people who are excessively flatfooted, knock-kneed or bowlegged. It is also very common in people with bunions. ▸▸ Will the arthritis get worse over time? In almost all cases arthritis in any joint will worsen. The joint will wear abnormally causing pain, limited motion of the joint, bone spurring and swelling around the joint. The joint will become excessively stiff causing pain with many activities. Difficulty with certain shoes may also be present. ▸▸ Will medication help to lessen the pain and joint deformity? Oral medication will help to diminish some discomfort, but it will not change the degenerative process, which takes place slowly over time. ▸▸ Will physical therapy be of any benefit? Physical therapy will help retain some movement in the joint and keep swelling to a minimum. It may also help to improve muscle strength. Over the long term it will be of limited benefit as the degenerative process will continue to take place. ▸▸ Can surgery be of benefit if the arthritis is found early? Many procedures can be performed to assist in treating the degenerative process if found early. After a thorough history and physical examination, x-rays are taken to assess the degree of joint damage. Occasionally, if the damage is mild, an arthroscopic or small open procedure may be necessary to remove abnormal bone and soft tissue. Cartilage that is lifted away from the bone is removed to allow the joint to function more normally. If a joint is out of position removing bone spurs and soft tissue may be of limited benefit. In this case, the bones may need to be repositioned to allow more normal joint alignment. ▸▸ Can anything be done if the arthritis is severe? In some cases the arthritis is severe and joint replacement or joint fusions may be necessary. Joint replacements are an excellent option and may be necessary in older, more sedentary patients. Fusing the joint that is arthritic and not allowing it to move any longer is also an excellent approach to the treatment of arthritis. This eliminates any painful movement allowing a return to work and activities of daily living with minimal modifications. Some joints can be fused and still allow an athlete to resume professional sports. ▸▸ Will fusing the joint cause any long-term problems? Joints that are fused will be pain free. Other joints adjacent to the fused joint will need to work a little bit harder and in most cases remain asymptomatic, especially if they are healthy at the time of the surgery. ▸▸ Will orthotics be of any benefit? Orthotics will assist in maintaining normal joint position. Support of the foot and placement of the foot in better anatomical position may redistribute stress more evenly in the foot and ankle region. This may help to lessen pain and possibly delay the need for surgery. They provide great benefits in people with big toe joint (turf toe, bunions) problems. Orthotics are also very useful in patients who are flatfooted, knock-kneed, bowlegged and in those with lower back pain. Robert Sheinberg, DPM, is a Board Certified Foot and Ankle Surgeon, Fellow American College of Foot and Ankle Surgeons, Diplomat American Board of Podiatric Surgery.
