Getting to the Bottom of Your Digestive Troubles Anyone who has suffered digestive problems knows that symptoms can range from mild to severe pain and bloating to diarrhea or constipation, none of which sounds like Ronnie Pimentel, MD much fun. EUS provides very high resolution images, because it does not have to go through barriers such as skin and bone, as compared to surface ultrasound.

Your digestive system is a complex and extensive part of the body. It starts at your mouth, down your esophagus to your stomach, small intestine, colon, rectum, and ends at your anus. It’s responsible for helping your body absorb essential nutrients and then getting rid of the waste, which are very important functions. So, if you continue to suffer from digestive disturbances, it’s probably time to make an appointment with your doctor. Not only will you be more comfortable once you’ve gotten to the source of the issue, more importantly, early diagnosis and treatment can reduce damage to your esophagus and GI tract.

EUS is very useful in helping physicians evaluate cancers or other tumors of the stomach, esophagus, pancreas, colon, liver, gall bladder, digestive tract and rectum, as well as to plan the most effective treatment strategies. It can even provide good images of smaller masses that may not be detected using other technology. And, preliminary results are available immediately, allowing for timely staging and treatment of gastrointestinal cancers. In fact, in the past 5 years EUS has become utilized more and more in this capacity.

Today, gastroenterologists are using advanced technology that make diagnosis and treatment

According to Cleveland Clinic Florida gastroenterologist Ronnie Pimentel, MD, “the EUS procedure is typically performed in an outpatient setting, and usually takes close to 45 minutes.” He adds that “patients do not experience discomfort during the procedure, and are able to go home the same day.”

How EUS works EUS is much like a regular endoscopy procedure in that a probe is inserted down the esophagus and into the stomach.

The difference, however, is that the probe’s tip has an ultrasound camera on it that provides a very clear view of the stomach and nearby organs. By getting so close to the organs, EUS allows doctors to take a biopsy through the stomach wall, by removing a small amount of tissue for testing. Getting a biopsy this way is more comfortable for patients than a traditional needle biopsy through the skin, and more critical is the fact that it reduces the risk of “seeding,” or spreading the tumor through the skin or to other parts of the body. Dr. Pimentel states that, “another important use for EUS is to perform nerve blocks for severe pain in cancer patients. We also can insert markers (known as fiducials) to guide future radiation treatments so that they target only the tumor, not the healthy tissue around it.” In addition, EUS is sometimes used to help find answers for patients with abdominal pain that doctors have been unable to explain.

ayor Wayne Messam, Commissioner Darline B. Riggs and Haitian Consul Guy Francois, Jr. jointly hosted a reception on Monday, May 23rd in recognition of Haitian Heritage Month at the Miramar Cultural Center. Approximately 280 guests attended the reception that featured Twoubadou Mania with Yvon “Kapi” Andre of famed Haitian band Tabou Combo, the Nancy St. Leger Dancers and Singer Saskya Sky. Guests were also treated to spoken word poetry by

Marnino Toussaint and Hillary Nomes of the Jason Taylor Foundation, an honor guard from Miramar High School as well as artwork by David Beaubrun and Jocelyn Telson. The event MC was actress and former Miss Haiti, Sandra Justice. Haitian Consul Francois extended a heartfelt thank you to the event hosts for their commitment to the Haitian Community and for recognizing the community’s rich heritage. Darlene Riggs, the first elected Haitian American

in Broward County talked about how much it means to her to be an elected official in the great City of Miramar and Mayor Messam talked about how special it was to see so many different people involved in the event. “This celebration was not only important to the Haitian Community, it was heartwarming to see the numbers of residents and guests from other countries who joined with our Haitian brothers and sisters in this joyous event,” said Mayor Messam.

Miramar Student and Mayor Win Big at Louder Than A Bomb Poetry Festival BY JILL SHARPE

M

iramar was the big winner in this year’s Louder Than a Bomb Florida Poetry Festival presented by Nova Southeastern University and UPS. Miramar Mayor Wayne Messam partnered with Desiray Baptiste, a senior at Miramar High School, to win first place in the first annual Mayor’s Slam event on April 12th. Then a few days later, Baptiste won the overall individual competition. Dubbed the “Super Bowl of poetry,” Louder Than a Bomb Florida, a Jason Taylor Foundation arts and education project, features a friendly spoken-word competition between school-based poetry teams representing local high schools from Broward, Miami-Dade, Palm Beach and St. Lucie counties. Funding from Nova Southeastern University, UPS and a host of other community partners has assisted the Jason Taylor Foundation support the schools’ efforts by providing year-round, in-class and after-school instruction, culminating in the 12-day Louder Than a Bomb Festival. The purpose of the Mayor’s Slam is to engage civic leadership with students from the community,” said Seth Levit, Executive Director of the Jason Taylor Foundation. Baptiste, who was part of Miramar High School’s winning team last year, was chosen by the poetry coach at her school to partner with Messam in the Mayor’s Slam. The Mayor and the student participated in a 90-minute workshop and wrote a powerful poem about the city which they performed together on stage in front of a large crowd at the Signature Grand in Miramar Mayor Wayne Messam during Davie on April 12th. City of Miramar Turns City Vehicles Into Mobile Mascots for Stormwater Matters Campaign BY JILL SHARPE

n Miramar, stormwater workflow vehicles like the F-150 pickup truck and the streetsweeper are not only getting the job done but drawing attention to the important work involved in stormwater management in the city. Each vehicle is wrapped with the public information campaign message, Only Rain in the Drain; intentionally turning them into Mobile Mascots for the City’s Stormwater Matters! campaign. “The campaign’s slogan sums up how important it is to keep pollutants like debris and pet waste away from street drains and gutters that lead to underground pipes and channels,” said Thomas Good, Public Works Director. Mobile Mascots are seen daily by over 55,000 drivers along the routes they frequent within the City. “They are helping present an active visual of what’s

Most recently, on April 27th, the U.S. Army Educational Outreach Program announced that the 6th grade team “JNS” from Glades Middle School was one of the finalists in the 14th annual eCYBERMISSION STEM Competition. The team, comprised of Noah Aguirre, Scott Kuang and Jason Torres, worked with team advisor/ Cougar Path Gifted Science teacher Mr. Michael Large on how to solve the problem of agricultural runoff and the protection of our Everglades aquatic and coastal marine life. As a finalist, the team won $1,000 in U.S.EE Savings Bonds, valued at maturity. “I congratulate team JNS and Mr. Large for a job well done,” said Principal Santana. Glades Middle is also mighty proud of several

special programs the school offers such as the EPICS Program from Purdue University and The Cougar Path Program. “The EPICS Program enables students to connect engineering and computing design with people and local community needs, “ said Principal Santana. “The Cougar Path Program is a project base curriculum that instills higher level critical thinking while developing 21st century skills. Glades Middle also offers Robotics, TV Production and more. “These programs present real life obstacles in which the student must create real life solutions,” Santana said. “ We can certainly say the future looks bright at Glades Middle as the pillars of passion, purpose and pride are what embodies the quality of education at our school.”

y Fellow Miramar Residents, I hope you’re having an awesome summer. Now is that time to switch gears with the kids out of school in camp, playing sports, enjoying family time and perhaps catching up on things you hoped to do during the year but just didn’t get a chance. It’s the same here for us as City employees. We continue to work diligently, ensuring we are focused on all things important to our residents and business owners. Here’s an update: During a recent meeting, the Commission voted unanimously to approve an initiative I sponsored called the P.A.C.E Program for Home and Commercial Wind & Energy Efficient Improvement Financing. P.A.C.E. stands for Property Assessed Clean Energy. Home and business owners are eligible to equip their properties with wind and energy efficient improvements through special financing with approved industry leaders. The programs provide upfront capital to property owners that allows improvements to their properties with refinancing paid through a special, non ad-valorem assessment levied on the property and repaid annually on the property owner’s tax bill. P.A.C.E financing can be repaid without penalty in advance or in increments of five to twenty-five years at the home or business owner’s consent, depending on the lifespan of the improvements made to the property. I will be holding a series of meetings in the community with the approved lenders to put the two groups together. During the meetings, property owners will be able to get a better understanding of who is offering the program, qualifications needed, how to complete the forms and more. C

olor blindness is a disorder affecting the capability to see colors under typical light or to discern colors as they are normally seen. Generally, the condition is present at birth, but it can also result from old age or a number of diseases of the eye. The discernment of color depends on cones located in the eye. People are usually born with three kinds of pigmented cones, each of which perceives various wavelengths of color. When it comes to pigment, the size of the wave is directly linked to the perceived color tone. Short waves produce blue tones, middle-sized waves are perceived as green tones and longer waves are seen as reds. Which type of cone is affected impacts the spectrum and severity of the color blindness. Because it is a gender-linked recessive trait, green-red color deficiency is more frequent in men than in females. Still, there are a small number of females who do experience some degree of color blindness, particularly yellow-blue deficiencies. Color vision deficiencies are not a debilitating condition, but it can impair educational progress and work performance. Not having the ability to distinguish colors as friends do can quickly and negatively impact a student’s confidence. For working people, color blindness could become a disadvantage

when competing against peers trying to advance in a similar industry. There are several examinations for color blindness. The most common is the Ishihara color test, named after its inventor. For this test a patient views a plate with a circle of dots in various sizes and colors. Within the circle appears a digit in a particular tint. The patient’s ability to see the number inside the dots of contrasting colors indicates the level of red-green color sight. Even though inherited color vision deficiencies can’t be treated, there are some steps that might improve the situation. Some evidence shows that wearing tinted contacts or anti-glare glasses can help people to see the distinction between colors. More and more, computer applications are becoming available for regular personal computers and even for smaller machines that can assist users to distinguish color better depending upon their specific diagnosis. There are also exciting experiments being conducted in gene therapy to correct the ability to distinguish colors. How much color vision problems limit a person is dependent upon the type and severity of the condition. Some individuals can adapt to their condition by familiarizing themselves with alternate cues for colored objects or signs. For example, many individuals are capable of learning the shapes of traffic signs (rather than recognizing red) or contrasting items with paradigms like a blue body of water or green trees. If you notice signs that you or a child might be color blind, we recommend that you see an optometrist. The sooner you are aware of a problem, the sooner you can help. s the U.S. continues to lick its economic wounds from the recession’s aftermath, millions of consumers are still faced with that ugly 4 letter word — DEBT. Nearly eight out of every ten Americans are in debt, i.e., they owe far more than they own. Debt collectors have created a multibillion dollar industry preying upon the financial hardships of others. While some collection companies are legitimate, many abuse the process daily by harassing debtors, calling multiple times a day, even on weekends, attempting to collect debts. These unscrupulous debt collectors live by the philosophy that if they harass you enough, at some point you’ll simply pay the debt just to get rid of the nuisance. This article, and the articles for the next two months, will address how consumers, through the assistance of knowledgeable counsel, can fight back against debt collectors, and win big money. There are at least three different avenues by which consumers can fight back against harassment and the never ending phone calls by debt collectors -- The Telephone Consumer Protection Act (“TCPA”), the Fair Debt Collection Practices Act and the Florida Consumer Collection Practices Act. Each of these, individually or together, when properly presented can result in huge damages in favor of consumers and against debt collectors. In 1991 Congress passed the TCPA. Among other things, this Act restricts debt collectors from using an automated telephone dialing system (“robocalling”) to place calls, or texts to a debtor’s cellular telephone without their consent. Since 1991, however, debt collectors have become more sophisticated, and as a result, new laws have been implemented with the assistance of the Federal Communications Commission (FCC), in order to combat the ever-evolving tactics debt collectors use to harass consumers daily. For example, Congress has enacted severe penalties for debt collection practices which allow consumers to collect between $500.00 and $1500.00 per collection call or text! In a recent federal case, the Court ordered a debt collector to pay a consumer $363,500.00 for robo-calling 727 times without express consent. In another case, the same federal court determined that the debt collector should pay $1,500 per each phone call considered “willful and knowing.” In both of these cases, the consumer prevailed, because they hired a knowledgeable attorney in order to assist them with their claims. ne of the interesting facts about South Florida is that it has one of the most electrically active atmospheres in the world. I’m not talking about the nightlife. Lightning strikes this fun area of the country thousands of times every year, unfortunately causing several deaths. Having great experience with lightning, most Floridians have learned how to decrease their chances of being struck. During a thunderstorm common sense takes over, we get indoors, we don’t stand under a tree, some even skip the 18th hole. Why? Because we have, in some crude way, noticed the statistics of lightning strikes. Then I wonder, why do people risk their lives every day undergoing cosmetic surgery procedures in mass production surgery centers? I’m not pointing fingers here, I’m just pointing out that cosmetic surgery can be a rewarding experience, but all surgery carries risk. Cosmetic surgery complications are rare, and those resulting in death are even rarer. Thousands of people are flocking to South Florida from around the world to experience the boon of cosmetic surgery. Most of these beauty pilgrims are attracted by the amazingly low prices here. The product of warring cosmetic surgery centers that continue to stab at each other’s hearts with yet a more affordable price. Some board certified plastic surgeons are violating the code of ethics using questionable marketing strategies in order to attract patients. Mass production, performed by detached physicians, guided by unscrupulous businessmen is a veritable lightning rod. In the constant effort to control costs, many centers start cutting back on a very important part of the equation, safety. Just think of it this way, when you have gotten the best price anywhere for your procedure, just imagine where the center has cut costs in order to provide you with that amazing price. Here is a little advice: Do your homework. Just because a friend had a procedure at a high volume center, and did OK, doesn’t mean you will be as lucky. Go to the ASPS website ASPS.org, and get educated on who should perform your procedure and what certifications the facility should have. Do a bit of research on their track record, and read reviews (the good, the bad, and the ugly). An infection can set you back thousands of dollars in lost work time, and further medical expenses. A botched job can cost you triple to fix. So now that really cheap surgery has become very expensive. Unfortunately, some have paid with their lives. So by all means come to South Florida to enjoy more affordable plastic surgery, just be very, very, careful where you stand.

And, in the same year the song was released, tragedy struck. Lennon was shot and killed in front of his New York apartment. Life doesn’t always follow a plan. As an estate planning attorney, I frequently see the sad consequences of a family facing an unexpected death, made even more difficult when minor children are involved. Does having an estate plan lessen the pain of an expected loss? Of course not. But it does take some of the burden off surviving family members: difficult decisions have already been made, priorities set, helpers designated. An important component of a parent’s estate plan is addressing issues regarding the upbringing of your children. What happens if both you and your spouse die simultaneously, or within a short duration of time? A contingency plan should identify the persons you’d like to manage your assets, as well as the guardian you’d nominate to finish raising your children. In many instances, you may purposely designate different persons to deal with your money and your children so as to maintain a system of checks and balances. You don’t want the probate court deciding who will manage your finances, or to guess whom you’d prefer to raise your children. Other issues to consider are whether you’d like your heirs to receive an inheritance directly, or whether you’d prefer to have the assets placed in trust. Funds from a trust can be distributed based on a number of factors that you designate such as age, need, and/or even incentives based on behavior and education. All too often, children receive substantial assets before they are mature enough to handle them properly, with devastating results. Most importantly, give careful thought to your choice of guardian, ensuring that he or she shares the values you want instilled in your children. One wrong move and the patient could end up without sight.” As a positive role model and mentor, Dr. Greenbaum encourages students to understand the medical field, as well as other fields. “I advise all students to work hard to become well-rounded adults,” said Dr. Greenbaum. “While most pre-med students like to focus on science courses, I encourage them to take additional courses that will broaden their knowledge base.” Dr. Greenbaum, who is a native of Southeast Michigan, attended the University of Michigan in Ann Arbor, Michigan, earning a Bachelor of Arts degree with High Distinction, with a major in Spanish language & History. She continued her education at Wayne State University School of Medicine in Detroit, Michigan. She completed her residency training in Ophthalmology at William Beaumont Hospital in Royal Oak, Michigan, where she served as Chief Resident. She is fluent in Spanish. Forty students, including some from Coconut Palm Elementary School and Silver Lakes Elementary School, recently took part in the fourth annual Autism in Flight event at Fort Lauderdale-Hollywood International Airport. The students had the opportunity to navigate the airport experience firsthand in an effort to help reduce stress on an actual flight.

When patients’ medical information is kept electronically, providers can make more effective diagnoses, communicate with one another more easily and reduce medical errors. And patients can be more confident that the care they receive is efficient and coordinated. Here in South Florida, Memorial Healthcare System has been a leader on the electronic health records front, providing patients and families with timely, accurate medical information they need via Memorial MyChart. MyChart is a free service that Memorial offers to patients of Memorial MyChart doctors, patients who have been discharged from Memorial hospitals, emergency rooms or urgent care centers, and patients who have received care from South Broward Community Health Services. MyChart provides direct online access to portions of the information that providers have entered in their records — including lab and imaging results, allergies, medications, immunizations and medical histories — all stored for quick retrieval. Records are also available on your iPhone or android device. At MyChart, you can view your: •

One of the best features of MyChart is that you can view your family records with proxy access, which allows a parent, guardian or designee to log in and retrieve information for both children and adult patients. So if your child is being treated for an illness, or if you’re caring for an adult family member, you not only can track their treatments but also can easily contact a provider with any questions or concerns you have. MyChart uses encryption software to help ensure secure storage of health records. And if you have a technical question about using MyChart outside of normal business hours, you can contact Memorial Healthcare System’s Help Desk at 954-276-7676.
