Ready for Spring? Asthma Season is Approaching Asthma affects 22 million Americans. Anyone can develop asthma, and it may occur at any age. However, people with a family history of asthma are at Frank Eidelman, MD increased risk, as well as individuals with allergies. With spring bringing allergy season, those afflicted with asthma may suffer increased symptoms, including; chest tightness, difficulty breathing, frequent coughing, shortness of breath and wheezing. According to Frank Eidelman, MD, Cleveland Clinic Florida allergy and immunologist, “Not every person with asthma experiences symptoms in the same way, and you may have different symptoms at different times.” Asthma is a problem in the airways. “Inflammation is the main cause for the

airways of a person with asthma to become very sensitive, and as a result react to “triggers.” Contact with these triggers brings on asthma symptoms,” states Dr. Eidelman. An important part of asthma control is avoiding common triggers, such as; viral infections, allergens (dust mites, pollens, pets, mold spores, etc.), tobacco smoke, air pollution and changes in weather. Asthma can be controlled, but not cured. Having frequent symptoms or difficulty completing tasks is not normal, and Dr. Eidelman urges “good asthma control will stop these symptoms and end visits to the emergency room.” In addition to avoiding triggers, properly using medication prescribed by your doctor is critical for good asthma control. There are two main types of asthma medications; anti-inflammatories, which should be taken on a regular basis, as they reduce swelling and mucus production in the airways and

Let us direct you to the best care. Access. Anytime. Anywhere.

bronchodilators, which should be taken on an as-needed basis, as they relax the muscle bands that tighten around the airways, which improves breathing. Based on the severity of your asthma, an action plan will be developed by your physician. This plan will include use of asthma medications (with a number one goal of reducing inflammation), actions to take when asthma worsens, and when to seek emergency care. By learning to manage your asthma, you take an important step toward living free of symptoms and maintaining a normal, healthy life.

Children Get the Royal Treatment at Mayor’s Kids’ Day community news

others and fathers have their own special day. In Pembroke Pines, so do the children. On June 24, more than 1,000 children are expected to join in the fun at the 7th Annual Mayor’s Kids’ Day. They will have an opportunity to eat lunch with the Mayor and visit participating businesses that will be offering them special deals. “Mayor’s Kids’ Day is an event that families and businesses look forward to all year,” said Francie Novo, Mayor’s Kids’ Day organizer and Pembroke Pines Special Events Coordinator. “Businesses are able to increase their foot traffic, as they showcase their products and services to a captive audience. Parents and kids can enjoy a day filled with free fun. This event makes all the kids in Pembroke Pines feel like a VIP.”

The free event is for children ages 15 and younger. Pre-registration is required. Residents can register at several locations throughout the city, including the Pines Recreation Center, Rose G. Price Park, Town Gate Park, and Pembroke Lakes Tennis Center. Kids will receive a special wristband that will entitle them to hunt for deals and freebies from local businesses throughout the city. Some of the things they might find include free bowling, swimming, haircuts, ice cream, mini golf, cookies, breakfast, and more. Businesses will also offer discounts on purchases, food samples, raffles and more. From 12:30pm to 1:30pm, the Mayor will host a fun-filled lunch for the children at the Charter High School, 17189 Sheridan Street. They can challenge the Mayor in a City Trivia game for prizes. They can visit with fire and police personnel, as well as mascots from several of the sponsors. They can also try their luck in the BrightStar money machine, which will be filled with valuable prizes. The City will provide a free lunch and Publix will provide dessert. “The Mayor values the families and businesses that have made Pembroke Pines their home and this event makes sure to introduce them to each other,” said Novo. “His effort to unite them makes Pembroke Pines the best place to work, play, shop and live.” For information, call 954-435-6525 or visit www.ppines.com.

pproximately 400 runners, walkers and strollers participated in the 6th Annual Pill Box Pharmacies Flamingo Strut 5K and Kids Fun Run. The race, which took runners through the tree lined streets of the Pembroke Lakes neighborhood, started and finished in front of Pill Box Pharmacies and Medical Supplies, which was the presenting sponsor. It raised $10,000 for the City of Pembroke Pines Charter

School Foundation. “This is a very competitive run with a great social element,” said Kathleen Kubler, event organizer. “This race is community-based, so many people know each other and encourage one another throughout the year to prepare for the ultimate fitness fun-fund raiser.” Participants received a warm predawn welcome from Pembroke Pines Mayor Frank C. Ortis and Commissioner Jay Schwartz and were cheered on by the Pembroke Pines Charter School Jaguar Cheerleaders. They enjoyed entertainment prior to the race, as well as awards and breakfast at the conclusion. Prizes were awarded to the top three male and female in the Overall and Masters

Some of the winners were: ▸▸ Top Overall Male: Gabriel Castro (19:19) ▸▸ Top Overall Female: Emily Daycock (24:11) ▸▸ Fastest City Employee: Bob Aguirre (24:17) ▸▸ Fastest Charter School Employee – FSU Campus: Patty Correa Time (24:19)

hildren and teachers were the ultimate winners of Mayor Frank C. Ortis’ 14th Annual Golf Tournament, held at Pembroke Lakes Country Club. The sold out tournament raised $80,000 for the Pembroke Pines Charter School Foundation. Since the tournament was established in 2002, it has raised over $1.25 million for the Charter School System “Our teachers are the foundation of our award-winning schools and they are dedicated to the academic success of our students,” said Mayor Frank C. Ortis. “This results in corporations, local, businesses and residents wanting to support our schools.” Golfers played on the 18-hole, John Stanford designed golf course. The day included a shotgun start/four man scramble, dinner and live auction. This year, the winning team was made up of Radio Personalities from Y100 - Footy, Froggy, Ivan Rosado and Austin Rumaner. “This is a challenging course,” said event organizer Kathleen Kuebler with the Pembroke Pines Recreation and Cultural Arts Department. “There was no hole in one; however, a few skilled golfers got close.”

Pembroke Pines Mayor’s Chess Challenge Is A Success community news

By Debby Teich

C

hess has long been regarded as a game that can have beneficial effects on learning and development, especially when it is played from a young age. Chess players of all skill levels and ages recently participated in the monthly traveling Mayor’s Chess Challenge at Club Nineteen in Pembroke Pines. The event is way for cities in Broward County to help bring awareness to the importance of supporting scholastic chess. It included Mayors, elected officials and community leaders throughout Broward County. “Chess is a great game,” said Pembroke Pines Mayor Frank C. Ortis. “The Mayor’s Chess Challenge promotes critical and creative thinking in our young people.” At the event, 125 players competed on 50 regulation chess boards and two garden chess boards, as well as on a life-size chess board. They joined Pembroke Pines Mayor Ortis, Pembroke Pines Commissioner Jay Schwartz, Sunrise Mayor Michael Ryan, School Board Member Patti Good and Broward County Schools’ Dr. Mark Strauss. Also present was Wendi Fischer, Executive Director of Americas Foundation 4 Chess, who is also known as “The Chess Lady.” “It was an honor to have The Chess Lady join us,” said Mayor Ortis. “She flew all the way from Seattle to talk chess in Pembroke Pines.” The Mayors’ Chess Challenge is a First Move event, sponsored by the nonprofit Americas Foundation Photo (l to r): School Board Member Patti Good, Pembroke Pines Mayor Frank Ortis, The Chess Lady, Sunrise Mayor Michael Ryan 4 Chess. and Pembroke Pines Commissioner Jay Schwartz with some players Internationally, the at the Chess Challenge.

Approximately 140,000 students also participate in the program in 26 states in the U.S. The Chess Lady teaches chess lessons via streaming video into the classroom, where teachers reinforce the student chess/learning activities. Locally, the monthly challenge rotates to different cities in the area. Recent challenges have been held in Hallandale Beach, Wilton Manors, Cooper City, Coral Springs, Lauderhill, Tamarac, Davie and Weston. Kids receive a passport that gets stamped for each month they attend. Many of the students are members of elementary, middle and high school chess clubs throughout Broward. Alex Davis of Davie has not missed a Mayor’s Chess Challenge. “Chess players of all ages and skill levels had a wonderful morning learning the game or challenging one another,” said Francie Novo, special events coordinator for the City of Pembroke Pines. PPCHS Students Perform Trauma Talk Prior to Prom By Debby Teich

H

igh school graduation and prom are supposed to be happy occasions. Pembroke Pines Charter High School Student Government Association and senior class organized Trauma Talk to encourage the school’s graduating seniors to make good choices as they celebrated these milestones. Eight days prior to prom, students gathered to watch a graphic, original, one-act play created to bring the devastating reality of celebrations gone wrong to the forefront. “As school officials, teachers and parents, we cannot reinforce enough to our students the importance of making good decisions,” said Shannon Torres, the PPCHS 2016 Class Sponsor and Curriculum Specialist. “One bad decision ruins more than one life. We have 403 graduating seniors in the class of 2016 and hope they left Trauma Talk with new insights into the importance of acting responsibly and potential consequences which affect family, friends and themselves.” Trauma Talk opens with a happy scene of nicely dressed dates, a prom corsage presentation and parents taking photos. The play moves rapidly to the nervous laughter of the teens, who are passengers in a car being driven by a date that is drinking. Fast forward and the stage is dark, except for the red light of emergency vehicles illuminating the horrific image of the four prom dates lying

bloodied and scattered near car wreckage. One girl is pronounced dead and three others are treated at the scene and transferred the hospital. The parents, who were laughing five hours before, begin discussing a funeral. A bright star was extinguished. The driver lost a baseball scholarship and is sent to jail. The friends are left devastated and forever questioning why they let him drink and drive. The cast of Trauma Talk included PPCHS senior class representatives, teachers, Pembroke Pines firemen, paramedics, police officers, and undertakers. Students heard from Pembroke Pines Mayor Frank C. Ortis and Vice Mayor Iris Siple, trauma surgeon Dr. Seong K. Lee from Memorial Health Care System, and Pembroke Pines Paramedic Captain Vinny Scardino and Pembroke Pines Traffic Homicide Investigator Jamie O’Sullivan. All of them shared messages about making good choices, not being a follower, keeping friends safe and the inability to erase memories of extricating bloodied twisted bodies from wreckage. Nurses from Memorial Healthcare System also attended and spoke with the students following the program, as did PPCHS teachers Carrie Hyacinth and Andrew Curry, who assisted in the developing the program.

Franklin Academy Charter School Band Receives High Ratings for Musical Performances By Debby Teich

F

or the first time since Franklin Academy’s middle school band program was formed in 2011, students were awarded an overall Superior rating on their Music Performance Assessment (MPA), which was hosted by the Florida Bandmaster Association. The MPA provides opportunities for students and directors to receive critical evaluation by noted experts in the field and experience a formal concert setting. It also serves as a motivational goal, as well as an opportunity to listen and learn from others. There are 41 students in Franklin Academy’s band. After performing three musical pieces “March for a Festival” by Edmondson, “Appalachian Morning” by Sheldon, and “Flourish” by Feldstein/ Clark – the band received a Superior rating from all three judges. It also received an Excellent rating in Sight Reading, when both the band director and students had three to five minutes to review two unfamiliar music

pieces and perform them. “It took a lot of hard work, listening skills, selfrecording, team work and motivation for us to accomplish this big achievement,” said Monique Green, band director. “This 2015-16 band group taught me a lot of things professionally and personally throughout this journey.” The band’s next step was to begin preparing for District’s Solo and Ensemble Assessment and the students excelled once again. Thirty-five students,

in grades six through eight, performed musical selections individually and in groups, depending on the instrument they play. Thirteen students were rated Superior, eight were rated Excellent, and six were rated Good for their solo and ensemble performances. “I am so impressed with the talent of my students this year,” said Green. “It has been a pleasure to serve as their mentor.”

S

chool is ending and summer is almost here. That means many of us will be swimming in pools and in the ocean to cool off. I’d like to take this time to remind you about the #SaferBy4 Campaign that the City of Pembroke Pines is participating in to reduce preventable child deaths. The #SafeBy4 Campaign is designated to help children reach their fourth birthday. One of the components of the #SafeBy4 Campaign is Water Safety.

Here are just a few tips to remember:

▸▸ Make sure that someone is always actively watching the water when a child is in the pool. ▸▸ Assign a “Water Watcher” to ensure that a responsible adult is supervising the child. ▸▸ Children should never have accessibility to water without being accompanied by an adult. ▸▸ Have several (in case one fails) to physically block a child from a pool. Barriers include door alarms, pool fences, self-latching gates, child proof locks and pool covers. ▸▸ Early activation of EMS, CPR and Rescue Breathing can mean the difference between life and death. Keep a phone by the pool and learn CPR and Rescue Breathing. The City of Pembroke Pines offers swim classes to our residents. Register your 1-4 year old for one of our parent and child or Preschool Level 1 Swim Classes at one of the following locations: Pembroke Lakes Pool, Price Park Pool or Towngate Pool. For more information on swim classes call (954) 4506895 or go to www.ppines.com/FireDepartment/SaferBy4.

I’d also like to invite you to join me at the “Mayor’s 7th Annual Kids Day” event for children 15 and younger being held on June 24, from 8am to 8pm. More than 1,000 children participate each year in this free citywide scavenger hunt. Following a set schedule, local businesses open their doors to participants wearing wristbands they receive from the Recreation and Cultural Arts Department to provide special discounts or free promotional items. I’ll also be hosting a fun filled lunch for the children at the Charter High School from 12:30pm to 1:30pm. Please check the city’s website for registration information and locations. There are many other exciting activities planned throughout the summer so be sure to check our website and facebook page for details and announcements. It’s a great time to enjoy all our city has to offer. Wishing you a very safe and fun summer! As always, if you have any questions or concerns, please do not hesitate to contact my office at 954-450-1020 or by e-mail at fortis@ppines.com.

MARILYN LOST

T

he term comes from the Greek words “gyne” (meaning “woman”) and “mastos” (meaning “breast”). In practical terms, this means abnormally large breasts on men. The condition is relatively common in adolescent boys, and 90% of the time symptoms disappear in a matter of months, or as adolescence wanes a few years later. But the remaining 10% are burdened with a social handicap that can cause shame, lowered self-esteem and confidence. There are many potential causes, including puberty, steroid abuse, obesity, marijuana use, side effects of medicine, and certain genetic diseases. Gynecomastia can be emotionally devastating. A man or boy with gynecomastia struggles with anxiety over such simple acts as taking off his shirt at the beach. In cases of obesity, weight loss can alter the gynecomastic condition, but for many it may not totally eliminate it. For all other causes, surgery is the only known physical remedy, and often the best solution. Once the contour of the chest is restored to normal, the patient can begin to feel more confident and self-assured with his physical body image. Plastic surgery to correct gynecomastia is technically called reduction mammaplasty and reduces breast size, flattening and enhancing the chest before AFTER contours. In severe cases of gynecoDr. Messa’s patient mastia, the weight of excess breast tissue may cause the breasts to sag and stretch the areola (the dark skin surrounding the nipple). In these cases the position and size of the areola can be surgically improved and excess skin may be reduced. The procedure can be performed under intravenous sedation or general anesthesia depending on each individual’s medical condition. In cases where gynecomastia is primarily the result of excess fatty tissue, liposuction techniques alone may be used. There are various liposuction techniques that may be used; the technique most appropriate in your case will be defined prior to your procedure. Excision techniques are recommended where glandular breast tissue or excess skin must be removed to correct gynecomastia. Excision also is necessary if the areola will be reduced, or the nipple repositioned to a more natural male contour. Incision patterns vary depending on the specific conditions and surgical preference. Sometimes gynecomastia is treated with both liposuction and excision. Any surgical treatment to correct gynecomastia will require incisions. While most incision lines are concealed within natural contours, some may be visible and are a necessary result of breast reduction surgery. After surgery, dressings or bandages will be applied to your incisions and an elastic bandage or support garment may be used to minimize swelling and support your new chest contour as it heals. The procedure duration is typically one to two hours and most men are able to return to work within 4 to 5 days depending on how much lifting is required. Dr. Messa is board certified by both the American Board of Plastic Surgery and the American Board of Surgery, an active member of the American Society of Aesthetic Plastic Surgery, American Society of Plastic Surgeons, and a Fellow of the American College of Surgeons. He is a recognized expert in cosmetic plastic surgery and Dr. Messa has achieved Chairman status with Medicis and Mentor Corporation. This award honors distinguished leaders in cosmetic surgery for demonstrating expertise and excellence through a high volume of aesthetic product procedures. Do you have a thoughtful and comprehensive estate plan which includes:

Celebrity Estate Planning Blunders BY BRUCE IDEN, ESQUIRE

T

his month, we take a trip down memory lane to highlight a few “celebrity estate planning blunders.” While you may not immediately relate to these specific celebrity situations, there are lessons that apply to current estate planning choices. The legendary actress Marilyn Monroe is making more money than ever although she died over 50 years ago! Marilyn’s will left most of her estate to her beloved acting coach Lee Strasberg, who died in 1982, leaving his interest to his third wife Anna. Anna Strasberg hired a company to license Monroe’s products, and auctioned off many belongings, including the dress she wore to JFK’s birthday celebration for more than $1 million. Although she received millions of dollars from Marilyn’s estate, the two had never met. Marilyn’s plan should have provided for alternate beneficiaries if her chosen one did not survive. Joe Robbie, first owner of the Miami Dolphins, built a stadium with $115 million of private funds. Robbie died just three years later, having created an estate plan laying out his intentions for his business, wife and nine children. In Robbie’s plan, ownership of his enterprises would remain in his trust, providing his wife with an income for the rest of her life. His trust named three of his nine children as successor trustees, which surprised and infuriated Robbie’s wife and the other six siblings. Siblings filed suit against siblings. Mrs. Robbie demanded her lawful 30% share of his estate as a bargaining tactic to maximize leverage to expand the appointed trustees. Unfortunately, Mrs. Robbie died before she could withdraw the 30% demand, which precipitated a mandatory appraisal of all estate assets necessitating the liquidation of assets to pay the 30% share. Her unexpected death added to the demand for cash to pay estate taxes due nine months following her death. Because of a lack of liquidity, the team and stadium had to be sold to pay approximately $47 million in estate taxes. Had Joe Robbie explained his intentions and business succession plan to his family there would likely have been a different outcome and less chance of a family feud. We first met Sonny Bono as part of Sonny and Cher. Singer, songwriter, restauranteur, producer, former mayor of Palm Springs, U.S. congressman, husband, and father, Bono never got around to creating a will. When he died in a skiing accident at age 62, his estate was valued at approximately $1.7 million but also contained royalty rights and residuals from his music career. His wife, Mary Bono, spent years in court battling to be named executor of his estate and have control of his music rights. Adding to the drama, Cher, his former wife, sued the estate for $1.6 million for unpaid alimony, child support and attorney’s fees from their 1974 divorce. Another twist was the claim by a 35-year-old man that he was Bono’s son. Bono’s failure to plan left an unexpected legal and financial mess for his family. Estate planning is a deliberate, premeditated act of love. Life can change in an instant. What are you waiting for? Bruce Iden earned his J.D. from George Washington University National Law Center and his L.L.M. in taxation from the University of Miami School of Law. He has been practicing in the area of estate planning and probate and trust administration for over 30 years. For more information, or to schedule a complimentary estate-planning consultation with Mr. Iden, please contact Iden Law Offices at (954)885-0085.

Alimony Reform? Not This Year! By Evan Baron

U

ntil approximately one week ago, I was expecting to write this month’s article on the changes in Family Law that would be taking effect as of October 1, 2016. For the second time in four years, the Florida Legislature had passed a bill which would have modified the current alimony laws in this state; and for the second time in four years Governor Scott has vetoed the bill. The result is that everything remains the same and the proposed laws that were passed by the legislature are forgotten, at least until 2017; when they reconvene. The reason the Governor gave for vetoing the alimony reform bill four years ago was due to the fact that it contained a retroactive clause. This would have allowed individuals, who were either ordered to pay or agreed to pay permanent alimony, to go back into court and seek either a termination or reduction based upon the change in the law. The Governor indicated that if in fact that provision was removed in the future he would in fact pass an alimony reform bill. This year, the retroactive provision was in fact removed, but the legislature added a time sharing provision, which would have resulted in an equal time sharing presumption when there are minor children involved. Currently, there is no such presumption, although it is very common that Mothers and Fathers do have equal time sharing with minor children. In spite of what many individuals believe because of their independent research on the internet, there is no such equal time sharing presumption in Florida today. Each case is considered independently and the Court is to consider the best interests of the children. In exercising his veto power, Governor Scott indicated that he felt the equal time sharing provision did not protect the interests of the children. Obviously, the issue of alimony reform is controversial. Individuals who are not directly involved in an alimony case may not have an opinion about this matter. Those that are receiving permanent alimony or may potentially fall within those who might be required to pay such alimony if divorced may see it differently than those who are receiving or might receive permanent alimony. Those within the legal community are themselves divided as to whether alimony reform is a positive or a negative. I can only advise a client as to what the current law is and not what it may be or should be. As I stated before, every case is different and it is difficult to structure a law which applies to all situations. The one issue which I thought would be beneficial to prospective clients was a formula to calculate the amount of alimony to be paid or received. Currently, there is no alimony guideline in Florida and therefore it is sometimes difficult to advise someone how much alimony may be involved in a particular case. I suspect that this issue will be raised again next year by the legislature, as it seems that some type of reform is inevitable. The law firm of Evan H. Baron and Associates is located at 1655 North Commerce Parkway, Suite 201, in Weston. If you have any questions concerning this issue or any other family law matter, call the office at 954-385-9160.

Most Common Mistakes by Homeowners Attempting to Obtain a Loan Modification By Omar J. Arcia, Esq. Foreclosure Defense and Bankruptcy Protection Attorney

A

s a result of various nationwide class actions and lawsuits filed against mortgage companies by private individuals and government agencies, lenders have become more receptive and willing to assist homeowners in modifying their loans. However, lenders have also become more demanding of certain documentation to support loan modification requests. These are the most common mistakes homeowners make when processing a loan modification on their own.

Provide Too Much Information

Most Homeowners believe that the more information they provide to their lender about their finances, the more likely their loan modification will be approved. Just the opposite is true. Most lenders are looking for specific information regarding a homeowner’s income and expenses, specific reasons to support a financial hardship for a loan modification, and they require a specific order and timeframe to submit the required documents. Overwhelming your lender with the same unnecessary documents time and time again will only place your file farther back in line for consideration. The Arcia Law Firm has represented homeowners that have been trying to modify their home loan on their own for years, and within just a few months after retaining the Firm and submitting the appropriate documents, a loan modification is achieved. Last year, the Arcia Law Firm successfully modified over 130 loans, and so far we have modified more than 120 loans this year.

Refusing to Seek Legal Assistance

We have counseled hundreds of clients in mortgage related cases through the years, and the most significant mistake that homeowners make is to wait until the last minute to hire a knowledgeable and experienced attorney. Many homeowners think that they can easily defend themselves in a foreclosure proceeding, process their own loan modification, and save their home. However, some of the legal defenses to a foreclosure are highly technical and require specialized knowledge. The attorneys representing banks and the judges overseeing in these cases are interested in obtaining a final judgment of foreclosure, sale of the property at a public auction and closing the case. In more simple terms, this means you are out of your home before you can finish your loan modification. The key to prevent this end result is for every homeowner either in foreclosure or at risk of foreclosure, to stop playing “Russian Roulette” with their family home, and immediately hire a highly qualified law firm. The attorneys at the Arcia Law Firm are privileged to successfully represent dozens of homeowners in our community. We have foreclosure defense, loan modification and bankruptcy departments. You can view videos and testimonials of our satisfied clients at our website under “Success Stories.” Detailed explanations of different options available to homeowners are also discussed in a new DVD developed by the Arcia Law Firm entitled “Fight For Your Home.” If you mention that you heard about the DVD through this magazine, you will receive a complimentary copy during your free consultation to discuss your case in detail. If you are struggling to make your monthly mortgage payments, if you are in the middle of a frustrating loan modification process, or if you just received foreclosure papers, contact the Arcia Law Firm today at 954-437-9066 to schedule a free personal consultation with Omar Arcia, or visit www.arcialawfirm.com to schedule your appointment online.

Gait Disturbances in Children By Dr. Carlo A. Messina

G

section Ask the Expert head

ait disturbances are one of the most common concerns of parents. Children may begin walking with their feet turned in or out, leaving parents with many questions. The problem may be coming from any level between the foot and hip. Some deformities occur at one level (i.e. foot, leg, knee, or hip), while others are more complex. Some children may “grow out of it”, while others will not. Newborns and infants are born with their legs bowed and the lower leg turned in relative to the upper leg (medial tibial torsion). This represents the fetal position of the lower limbs. With the beginning of walking the lower limbs start to straighten as normal physiologic development takes place.

There are many causes of gait abnormalities. Genetics is often the main cause. Rotational problems often run in families. When parents and siblings have grown out of it, the presenting child usually will as well. In utero position during pregnancy may cause the foot to turn in. Sleeping or sitting postures may not cause the problem, but will delay the resolution of them. Compensating mechanisms for limb deformities can cause abnormalities. They may also be caused by tight muscles and ligaments at any level, as well as bone deformities in the hip, femur, knee, tibia, or feet.

EXAMINATION OF THE CHILD

A thorough examination of the child is necessary, including physical exam and gait analysis. Observations of the child are made at rest, standing, walking and running. The doctor will also check hip range of motion with the hip flexed and extended as well as knee structure and the rotation of the tibia on the femur (lower leg bone relative to upper bone). The position of the ankle and the leg bone to determine bone deformities weightbearing and nonweightbearing will also be checked. Observations of foot posture to check for curvatures and elevation or flattening of the arch as well. Neurological problems must be ruled out.

TREATMENT

To effectively treat rotational problems, the level and severity of the deformity must be determined. If sleeping and sitting position perpetuate the deformity, they must be changed and in some cases, splinting or bracing is necessary. There may be a need for casting the foot, ankle and leg (every two weeks) to put tight ligaments and muscles into a continuous stretch. This may be necessary for anywhere between two and eight weeks. Physical therapy to improve muscle flexibility and to provide gait training to the child may also be recommended. Patients may also use in-shoe custom molded orthotics or gait plates to help encourage normal walking. This is especially beneficial in the flatfooted child or ones who are intoed. Night braces or splinting to encourage and maintain correction may be needed. In rare cases, surgery may be necessary if the problems persist and walking becomes difficult or if the child continues to fall or trip. In most cases, continued observation of the child may be all that is necessary. Carlo A. Messina, DPM, is a Board Certified Reconstructive Foot and Ankle Surgeon, Diplomate American Board of Foot and Ankle Surgery. He is a Foot and Ankle Surgeon with the Foot, Ankle and Leg Specialists of South Florida and specializes in treating infants, children, teens and adults.
