round this time of the year , we take a deep
breath and wonder where the time went. The sweltering summer heat is slowly becoming an item of the past, the awesome summer vacation pictures and memories have moved to the back of our minds, and our kids have settled back into their school and sports routines. Now that the Halloween sugar rush has worn off, we are already in full preparation for the Thanksgiving holiday. Wow! Thanksgiving already? In the blink of an eye the entire year seems to have gone by. Another fantastic year of enjoying time with family, friends and neighbors, of being there for each other through good times and bad, and of enjoying each other’s companionship. As we sit down this year with our loved ones from near and far to indulge in a delicious turkey feast, let's give thanks for all the little blessings we have received throughout the year, let’s be generous to those who need our help, and let’s continue to enjoy each other. Weston Miracle League Kicks Off Baseball Season in Pembroke Pines By Debby Teich
O
n N ovember 2, the Weston M iracle League kicked off its 8th
baseball season in Pembroke Pines, giving South Florida children with disabilities the opportunity to play baseball. Once again, the stands were been packed with proud parents and volunteers shouting words of encouragement like “great hit,” “great catch” and “you can do it.” Players can join the league throughout the season, which ends on January 25. The Weston Miracle League serves any disabled child, between the ages of 5 and 18, who has a desire to play baseball. Since baseball diamonds were not exactly designed with wheelchairs and crutches in mind, the Miracle League removes the barriers that keep children with disabilities off the baseball field. There is no cost to participate and baseball experience is not needed. All practices and games take place Sundays, from 4 to 5pm, at Rose G. Price Park’s Miracle League Baseball Field in Pembroke Pines. The park has a field with a special rubberized surface that makes it accessible for individuals with wheelchairs or other assistive walking devices. The atmosphere at the games is very low key and every player leaves a winner. Parents have an opportunity to take a break and relax, while their kids have fun on the field. Some of the players are focused on the game, and others simply want to play catch on 2013 baseball season
the sideline. Each inning all of the players have an opportunity to bat, remain safe on the bases, and score a run. For many of the kids, it is the highlight of their week. To help the athletes, the Miracle League uses a “buddy” system – pairing each player with an able-bodied peer, age 13 or older. The volunteers attend a variety of middle and high schools throughout Broward County and earn service hours for their work. Pembroke Pines parent Barbara Kopec has two daughters involved in the Weston Miracle League. Jackyln, 12, plays baseball and Jenna, a high school senior, volunteers as a buddy. “This is our second year involved with the Miracle League,” said Kopec. “Jacklyn enjoys playing baseball and having a ‘buddy.’ This type of atmosphere makes the kids feel comfortable and have fun. There is no pressure and each child plays at their own level.” Jenna says that she really enjoys volunteering. “Volunteering for the Weston Miracle League is such a rewarding experience,” said Jenna. “That is why everyone is so excited to participate. Imaginations Come Alive at the StoryBook Festival By Debby Teich
M
any think of the library as a
place to read a book in silence. On November 15, some of these books will come alive at the 13th Annual StoryBook Festival, which will be held from 10am to 4:30pm, at the Broward County Southwest Regional Library, located in the Pembroke Pines Academic Village. The free family event offers a day of music, dance, storytelling, storybook characters, crafts, and activities for youngsters and their families. “This community event is designed to bring books to life for young minds and to tell stories in new and dramatic ways that get youngsters hooked on books, while making the public library a dynamic community destination,” said Susan Hill, Youth Services Supervisor. More than 4,500 people are expected to attend the StoryBook Festival, which is sponsored by Broward County Southwest Regional Library, the Broward County Library Foundation and the City of Pembroke Pines. The first 1,000 children to arrive will receive a free book. Children are encouraged to come dressed in their favorite storybook costumes. This year’s theme is “Home Sweet Home Florida.” Storytellers from throughout Florida will share stories from Florida’s folk tradition, such as cracker tales, Native American tales of the Seminole, and bi-lingual tales from the Spanish-speaking communities that settled Florida. Entertainers will also share music, history and experiences of growing up in Florida. Pembroke Pines Needs Participants for Inside Out Project By Debby Teich
T
he City of Pembroke Pines is always
looking for unique ways to make art affordable and accessible to the general public. It is currently searching for members of the gay community who want to be photographed for its newest project, Inside Out, which shares the message of “Coming Out to a Network of Support” and shows the younger generation that there is support across age and gender. Renowned Parisian street-artist JR created the Inside Out photographic collage project in 2007. He invited groups to come together to share messages with their local communities and the world by showcasing large-scale black and white portraits to tell their stories. More than 120,000 people from over 108 countries have participated. As part of Broward County’s 100th birthday celebration, local groups are encouraged to join the effort. Pembroke Pines is the third city to sign up, after Miramar and Fort Lauderdale. Jill Slaughter, Curator of Special Projects for the city, came up with the idea for the Pembroke Pines project. While working on “Which Way Out,” an exhibit at Studio 18 in the Pines about keeping secrets, she learned some disturbing facts. “According the statistics, gay teens are more likely to commit suicide, run away, or become
addicted to alcohol and drugs than their straight peers,” said Slaughter. “This project is another opportunity to show the community that, regardless of our sexual preference, we all have more commonalities than differences. The faces of all groups look the same. It is our opinions that separate us.” The Pembroke Pines Inside Out project needs a minimum of 25 people to participate. It will include members of the Pembroke Pines police department, Broward college students, students from Broward high school Gay-Straight Alliance groups, members of the Fort Lauderdale Gay Men’s Chorus, local artists and many others. There is no cost to join the project and it is open to the community. Participants will have their 3’ X 4’ black and white portraits taken in January. In February, the portraits will be wheat pasted to the façade of the Pines Recreation Teen Center, located at 7400 Pines Boulevard. Hollybrook Residents Create Recycling Program By Debby Teich
O
n N ovember 15, H ollybrook will be among the thousands of
communities across the country that will celebrate America Recycles Day, a nationally-recognized day dedicated to promoting and celebrating recycling in the U.S. The residents of the recreational condo located in Pembroke Pines recently started their own recycling program and it has been very successful. Harold Parke, who helped to lead the effort, says that many condos already recycle bottles, cans, and newspapers, and most textile recycling programs are limited to donation boxes in shopping centers or once- or twicea-year clothing drives. Hollybrook wanted to take recycling to the next level. “Hollybrook has taken the initiative to become the first condo complex in Broward County to go the next step in recycling and launch a pilot program of recycling clothing and other textiles,” said Parke. Hollybrook’s program runs throughout the year. There is a textile collection bin permanently located in the administration building, as well as several textile bins that rotate throughout the complex in a three-month cycle. The bins are located in the second floor laundry rooms. They are emptied every other week and then moved to another place in the Hollybrook complex. “This system is designed to make textile recycling convenient,” said Parke. “There is a bin in your building every three months, and there is always a bin in the administration building if you don’t care to wait for a bin to cycle back to your
building.” The Hollybrook program is designed to recycle more than just clothing. It also includes bedding, towels, curtains, table cloths, throw rugs and many other household items. It recycles all textiles, including good quality clothing that can be resold, as well as ripped or stained clothing and other textiles that can be reused in industry. “Everything made from fabric can be recycled,” said Parke. “It is preferable to recycle these items than to discard them to a landfill. In landfills, fabrics slowly decompose and account of about 5% of the volume of landfills. Recycling adds a new value to used textiles.” The collected textiles are distributed in flea markets in Central America. They are also converted to wiping cloths or shredded into fibers, which can be used for insulation, pillow stuffing, speaker cones, and more. Hollybrook will accept donations from anyone in the community. Donated textiles should be clean, dry, and placed in a plastic bag to keep them dry. Parke says that Hollybrook residents should feel good about their efforts, since the used clothing has helped someone live a better life, and the clothing kept out of landfills will help their grandchildren live a better life. participating in the 5KRun/Walk Sallarulo’s Race for Champions and they would like the community to join them. It will be held on November 8, at 8am at Nova Southeastern University - Davie Campus. Proceeds will benefit the Special Olympics of Broward County. “You do not need to run,” said Mary Palacios, CTRS, CPRP Special Population Coordinator for the City of Pembroke Pines. “You can walk, cheer for others, or just help us out with our fundraising efforts for Broward County Special Olympics. Sallarulo’s Race for Champions was created by Paul, Patrick and Samantha Sallarulo to raise money for Special Olympics Broward County. Paul is a member of the NSU Board of Trustees, Chairman of the Wayne H. Huizenga School of Business and Entrepreneurship, Dental School – Advisory Board, and President of the International NSU Alumni Association. He has been a strong supporter of Special Olympics and has been working with people with special needs since he was 16 years old. In 2006, Paul decided to complete a half marathon in Orlando to mark his 50th birthday. At the same time, he raised money for Special Olympics by collecting donations. He finished the race and surpassed his fundraising goal, generating $130,000 in donations. This motivated him to create an event where the community could join him in his support of Special Olympics. It was the start of the 5K Run/ Walk. Special Olympics Broward County provides a free program that enriches the lives of children and adults with intellectual and developmental disabilities. These programs provide the athletes with the invaluable gifts of self-confidence and self-image, which carries over into all aspects of their lives. Pembroke Pines Included on Money Magazine’s “Best Places to Live” List By Debby Teich

ore than 160,000 residents call Pembroke Pines home .
According to Money magazine, they have made a good choice. Money recently revealed its annual “Best Places to Live in America” 2014 list and the City of Pembroke Pines was on it. It was ranked #32, and the only city in Florida to be among the top 50. “We’ve often been described as a city with a friendly small-town feel, but with big city amenities, events and activities,” said City of Pembroke Pines Mayor Frank C. Ortis. “It’s this unique blend of both that makes our city a great place to live, work, retire and visit. We’re very proud to be the only city in Florida to hold this special ranking of being one of the Best Places to Live in America.” According to the magazine, the list looked at 781 cities across country with populations 50,000 to 300,000 and ranked them based on more than 50 factors, such as: economic strength, job growth, affordable housing, safety, schools, accessible healthcare, arts and leisure, and the ease of living. Money cited a variety of reasons for ranking Pembroke Pines so high. While Pembroke Pines has no real city center, Farmers Market in Pembroke Gardens no one lacks for shopping; the city contains 8.5 million square feet of retail space. Those who come seeking warmth won’t be disappointed, as the city has the warmest winter temps of any on our list. But they’ll also need to prepare for wet weather—Pembroke Pines ranks high in terms of annual rainfall. Mona Davis was not surprised to find Pembroke Pines on Money’s top 50 list. “I have lived in Pembroke Pines for 25 years and would not consider moving anywhere else,” said Davis. “I love the people, culture, and climate. Glass Gallery Announces Art Competition Winners By Debby Teich

Pembroke Pines Glass Gallery 10th Annual Art Competition. Seven winners were selected. Through November 17, the community is invited to view the free exhibit, located at the Glass Gallery, 10100 Pines Boulevard. The juried art competition features approximately 200 pieces of beautifully displayed art. It represents a wide variety of art mediums, including: watercolor, oil, pencil, sculpture, mixed media, ceramic and photography. “This is our 10th Annual Art Competition and every year is better than the previous one,” said Benoit Menasche, City of Pembroke Pines Art Curator and Administrator. “Needless to say this year’s competition is the best ever. Visitors will be amazed at the quality of work exhibited. It shows that we are ready for our own museum in Pembroke Pines. I am very proud of our artists and I am honored to be associated with #2 them. Artists are special people; they are the creators and the soul of our city.” The winners of the competition include: #1. Seniors Test Their Spanish Spelling Abilities By Debby Teich

n recognition of National H ispanic H eritage month , seniors
once again competed for the title of “Spelling Bee Champion.” The 3nd Annual Senior Spanish Spelling Bee was held at the City of Pembroke Pines Southwest Focal Point Community Center. “The Spelling Bee helps to keep their minds young,” said Suzy Felix, Activities Coordinator. “It doesn’t matter their age. They always enjoy a little competition with their friends.” Pembroke Pines Director of Community Services and Housing Jay Shechter, who loves to play Scrabble, came up with the idea for a Spelling Bee several years ago. After holding the first two in English, the Spanish-speaking seniors wanted a turn. The next contest was in Spanish, in celebration of National Hispanic Heritage Month. The recent Spelling Bee was open to those 18 and older. Participants ranged in age from 53 to 84. The tension was high throughout the competition, as they spelled a wide range of words using the pronunciation from the Spanish alphabet, including any special punctuation. The contest
started with easy words, such as “blusa” and “hablar” and progressed with more difficult ones. Once contestants misspelled a word, they were eliminated. Teresa Eyrich, who won 3rd place in 2013, was declared the winner after she correctly spelled “alféizar,” which means “the inner ledge of a window.” She received $50. Magie Urquijo won second place ($30) and Luz Pardo won third place ($20). All three received a Certificate of Recognition. PPCHS Students Help Digitize 50+ Years of Historical Documents By Debby Teich
W
hen the new Civic Center opens in Pembroke Pines , it will
offer residents an opportunity to learn about the history of their city, which was incorporated nearly 55 years ago. Students from the Pembroke Pines Charter High School History Hunters Club have teamed up with the City of Pembroke Pines Historical Museum to digitize thousands of historical documents. The project will be displayed at the Civic Center. Approximately 25 students from different grades are working with City Historian Gerry Witoshynsky and Assistant Ellen McNamara on the extensive project that is comprised of three phases. The students have already begun working on Phase I, which requires sorting and sifting through mounds of newspaper articles provided by the Historical Museum. They are organizing them into categories, such as City’s Infrastructure, City Commission, Parks and Recreation, Education, Music, Culture, Minority Influence, Housing, and more. “The kids and I brainstormed the best ways of separating the newspapers into topics,” said English teacher Marta Cabeza, who is overseeing the effort. “We started out with approximately 15 Students from the Pembroke Pines Charter High School History Hunters Club are working with the City of Pembroke Pines Historical Museum to help preserve the City’s history.

PPCS Academic Village Middle School Campus Celebrates Peace Week By Debby Teich

tudents at the Pembroke Pines Charter School Academic
Village Middle School recently joined others around the world to celebrate Peace Week 2014. They participated in a variety of activities and organized events centered on the theme of “peace.” “These activities were an excellent way to get all of the student body involved in establishing positive peer relations,” said Jennilee Abolafia, guidance counselor. “Being that we are a new school, all of our students have come from various backgrounds, with vast experiences. Their interactions throughout the week provided them with a safe environment to make new friends that will last at least through high school.” Thirty students in the school’s leadership class organized the effort with help from Deborah Carvajal, drama teacher. The students made Pinwheels for Peace and placed them on the campus grounds, drew peace symbols on the walkways, and signed a Peace Pledge banner. They also wrote “Pass the Peace Notes,” which were messages of peace that they shared during lunch and in the hallways. The notes contained compliments or had quotes from famous peacemakers, such as “You are beautiful just the way you are,” “You have a fantastic smile,” or “Be the

change you wish to see in the world.” In addition, each student and staff member was given a tag to write their wish for peace. “All the tags were placed on the peace tree that grows outside our front doors as a daily reminder that peace begins with us,” said Abolafia. “All of the students participated and were excited to share their wishes.” Eighth-grade students Janiya Russell and Nicholas Sarbello participated in the effort. “During peace week, while making the pinwheels and writing down my peace wish, I realized that the world is going to change,” said Russell. “Someday someone, or all people, will make that change.” “During peace week, it felt great to work with fellow leaders, surprise everyone with our creative works and instill a peaceful feeling among students at our school,” said Sarbello. Three Reasons Why You Should Hire a Law Firm to Negotiate a Successful Short Sale By Omar J. Arcia Foreclosure Defense and Consumer Bankruptcy Attorney

ecently our law firm has been retained by numerous clients
who are being sued by their former lender after completing a short sale of their homes. They received what is called a deficiency judgment action. In other words, the lender (or some third party that purchased the debt) sued the homeowner for the difference between the amount owed under the loan and the amount of the short sale.
In every one of these cases, the homeowner was not represented by an attorney during the short sale process, but instead, chose to handle it themselves, or to allow a realtor to manage the entire process. Most realtors, however, are solely interested in “closing the deal” and receiving their commission, not in the most complete and favorable outcome for the seller/homeowner. Even though a realtor must be involved in listing the property and securing a purchaser, here are at least three reasons why you should hire a law firm to negotiate a successful short sale. ▸▸ Avoid the dreaded deficiency judgment. The principal reason to hire a law firm to handle a short sale is to avoid any deficiency judgment action by your lender after the short sale is completed. Nutrition and Plastic Surgery By Charles A. Messa, III, M.D., F.A.C.S.
W
hen discussing cosmetic enhancements of
the face and body, patients often focus on the procedures and products available and ignore the significance of nutrition. At the cellular level, the aging process is characterized by mitochondrial damage, inflammation, and glycation. Plastic surgery addresses the external signs of aging. Proper nutrition, including diet and supplements, can not only slow the aging process, but can also ensure that the patient heals properly after surgery. Adequate Vitamin C is necessary for collagen production, which strengthens a wound and fights free radical damage. Other less often mentioned antioxidants, which are more potent, are resveratrol (found in red wine) and pycnogenol (pine bark extract). Both have demonstrated clinical efficacy in reducing inflammation in the body and reversing the signs of aging. Patients often prefer nonfat supplements, which at times are actually higher in sugar and more damaging to the body. Too much sugar in your diet leads to increased levels of insulin and can progress to fat infiltration in the liver. Unsaturated fatty acids, such as Omega-3 and Omega-6, are essential fatty acids that are good for the body. They are often found in plant foods, such as vegetable oils, nuts (almonds, walnuts) and seeds (flax), as well as in fish, such as salmon. It is the saturated fats and trans fats that should be avoided as much as possible. These are found in butter, red meat, and high fat dairy products. Recently, scientists have presented conflicting evidence as to the benefits of Omega-7 fatty acids. Palmitoleic acid is not an essential fatty acid, meaning our bodies can manufacture it from other nutrients and we don’t need to get it from food. Experimental studies have shown it to be beneficial in reducing heart disease and triglyceride levels. More controlled clinical trials are necessary to determine the health benefits of this omega. As we age, our metabolism changes and hormone levels decrease. The effects of years of cellular damage from poor dietary habits and lack of proper nutrition become more evident. Conjugated linoleic acid (CLA) has been shown in studies to reduce fat and preserve muscle tissue. CLA helps you lose weight faster by reducing fat storage introduced by your daily diet and accelerating fat metabolism within the existing adipose (fat) tissue. Similarly, L-carnitine is a naturally occurring amino acid which plays a vital role in the metabolism of fat. It functions as a transporter of fatty acids into the mitochondria, the metabolic furnace of the cell. Carnitine boosts energy by stimulating the body’s burning of triglycerides as fuel. L-carnitine allows the body to burn more fat. No pill or supplement is a substitute for a balanced, healthy diet, including some degree of calorie restriction, low-glycemic foods, coupled with both cardiovascular and weight-bearing exercise. Patients should know that not all supplements affect each individual in the same manner. hese days , many individuals going through a
divorce try to handle the matter without the assistance of an attorney. They feel that it is better to try and settle the matter without spending large sums of money on legal representation. In all honesty, simple divorces can easily be handled by the parties. Simple divorces are cases in which there are no minor children and no real assets or liabilities to divide. There is a great deal of assistance through the courthouse or the internet to “walk you through” the process. Sometimes, however, people can be as they say “penny-wise and pound foolish.” In an effort to save money on attorneys’ fees, they may wind up giving up too much or perhaps not receiving his/her share. An individual who represents him/herself is held to the same standards as a licensed attorney. A judge will not give an individual the benefit of the doubt, just because an attorney is not sitting by his/her side in court. This can often present a problem in a more complicated case. I have often reviewed Marital Settlement Agreements which were entered into without the assistance of counsel. Often these individuals come to see me after the fact, to request assistance in attempting to “get them out” of the Agreement. Unfortunately, it is extremely difficult to undo what has already been done. A Marital Settlement Agreement or a Mediation Agreement has the same validity as any binding contract. In addition, as I previously stated, individuals are expected to know the law. It is not an excuse to later say “I wasn’t represented by an attorney.” I have often been told by individuals who had initially attempted to represent themselves that judges don’t seem to listen to them in court. Now this could simply be a matter of perception or a true statement. It may also be because non-lawyers are not familiar with the rules of evidence. As such, certain testimony may not be permissible. A court proceeding is controlled by rules of evidence and rules of procedure, which are in many ways illogical and foreign to the average individual. Many attorneys offer free initial consultations and those that don’t usually charge a nominal fee for the initial meeting. If you have any doubts about the law as it pertains to your situation, it would be in your best interests to consult with an attorney. However, an attorney cannot ethically represent both parties in a divorce and therefore can only advise one of the parties. Therefore, initial consultations should take place without your spouse. Lastly, if you have considerable assets, such as real estate or a business, or you anticipate a time sharing issue, you should definitely consult with an attorney to discuss these matters. t has been slightly over four years since South
Florida homeowners gained the upper hand in the once “red hot” real estate market when choosing to sell. Fueling this cycle were major institutional investors who entered the market. They vied for and “gobbled” up a vast majority of the affordable real estate with cash purchases, causing multiple offer scenarios and increasing the annual appreciation rate of homes to within double digits. This made it increasingly challenging for the average home buyer with a minimum down payment to secure the American dream of homeownership at near bargain prices. These institutional investors that once found South Florida real estate vastly attractive have slowly disappeared, exiting the area within the preceding months. I have been observing the gradual slowing of the market for the past eight plus months. During this interval, the absorption rate of the local housing inventory reflected a trend towards a balanced real estate market. Simultaneously, the local media unremittingly touted the progression as being a seller’s market; therefore, leading the general public to believe that this was truly the case. I am here to tell you differently! The housing market has since shifted and is now balanced, with neither buyers nor sellers having the upper hand in all but a few cases. Are homes still selling? Of course they are! Homes, varying by location and priced correctly at fair market value from the onset, are still receiving multiple offers from the “pent up” demand of traditional buyers that were not priced out of the market during the period of hefty price increases. Homes for sale that are overpriced are “sitting” on the market for longer periods of time and not selling until the owners reduce the price to the level that the market can bear. What is interesting to note is that while the housing market was trending upward, a staggering number of homeowners were, and still are, going through the seemingly never ending process of foreclosure. With the housing inventory steadily increasing along with gently diminishing demand, it will only be a matter of time before buyers will have the marginal advantage, albeit just slightly. How much time you ask? No one really knows since cycles generally don’t really have a start or a stop. There are no magic indicators that can point us to the beginning or end of a cycle. This specific editorial emerged as a result of a recent trend I have been observing involving a considerable amount of homeowners who aspire to market their homes for sale with unrealistic expectations in reference to time it takes, and, the pricing that will get the property sold. My advice is to be rational in your decision-making process, since the window of opportunity for your highest net profit after the sale is being reduced as time keeps wandering by. ACL Reconstructive Surgery By Dr. Al DeSimone
T
here are many views regarding indications for
ACL Reconstructive Surgery such as age, activity/demand level and degree of instability. Most individuals, especially moderate to high demand athletes, regardless of age, who have a complete tear of their anterior cruciate ligament, as well as clinical and functional evidence of instability, are candidates for ACL Reconstructive Surgery. When considering the pediatric or adolescent population, there has been extensive debate in the literature over the last ten years. Initially, treatment was consistent with bracing, rehabilitation and activity modifications (cessation of sports); however, due to the noncompliance of this patient population and their persistent knee instability with daily activities, these patients will often have further injury to the chondral and/or meniscal surfaces, resulting in degeneration and functional impairment. Non-surgical treatment in this patient population is not advised. When considering ACL Reconstructive Surgery in pediatric and adolescent patients, age and amount of growth remaining becomes vital, as this may tailor the technique used to reconstruct the child’s knee. The major consideration is the potential risk of injury to the growth plate/physis resulting in growth disturbance before skeletal maturity. One surgical option is a physeal (growth plate) sparing procedure, which is an allsoft tissue technique considered in the younger athletes because of their amount of growth remaining. This procedure is often non-anatomic but does improve functional stability and completely avoids the growth plates. Another physeal sparing technique in this age group describes the use of drilling superficial bone tunnels within the epiphysis. With the use of fluoroscopic guidance during arthroscopy, this technique secures the graft in a relative anatomic position, but still avoids the growth plates. In a slightly older child, since most of the growth comes from the femur, one may consider a partial transphyseal approach. In this procedure, the graft is brought through a transtibial (physeal) tunnel and is fixed to the femur in a near anatomic position avoiding the physis. A surgeon will often use an all-soft-tissue graft and fix the graft away from the femoral and tibial growth plates, thus, avoiding growth disturbances. Transphyseal anatomic ACL Reconstructions is considered in adolescents and adults, as well as those children who have minimal growth left. This allows for anatomic graft placement by creating tunnels at the native origin and insertion sites of the original ACL. With this arthroscopic technique the surgeon drills through the growth plates and depending upon the age of the patient, one may consider soft tissue (hamstring) graft or bone patella tendon bone graft to reconstruct the ACL. The type of tissue used and fixation technique will depend upon the patient’s skeletal maturity and growth remaining. My gold standard for skeletally mature teenage athletes and young adults is bone patella tendon bone autograft with intraphyseal fixation. When there is a contraindication to using intraosseous fixation I will use hamstring autograft and fix the graft away from the growth plates. These two techniques provide excellent stability with no growth disturbance. I would advise against the use of cadaveric (allograft) tissue in this younger population as there is a higher incidence of graft failure with return to high demand sports. There are few indications where non-surgical treatment is advised. First is in the low demand sedentary individual who doesn’t describe functional instability in their day-to-day activities. Next is the young athlete who sustains a partial injury to the ACL but still presents with good clinical stability. In these patients I closely monitor and follow their progress and often advise for the use of a custom de-rotational brace when returning to their sport. The last patient is that patient who is unable, for medical reasons, to pursue surgery because of the potential inherent risks of anesthesia. For these patients I will often advise a physical therapy program, bracing and activity modifications. Alfred A. The Fountain of Youth – Bioidentical Hormones By Michael F. Augustino, MD, FACOG
I
t has been nearly thirteen years since the over -
publicized and over-exaggerated risks of hormone therapy hit the news. This has been a serious disservice to women, creating a fear of hormones which has been blown out of proportion. Out of concern for my patients, many of whom suffer from changes associated with their ovaries in approach to menopause or in reaching full menopause, and a genuine interest in finding a safe and suitable solution, I spent nearly a year researching medical literature with regard to bioidentical hormones. I discovered that those studies, which caused all the fear, had to do with synthetic hormones. These risks do not pertain to bioidentical hormone therapy. It is also a viable treatment for menopausal women. Lifespan has increased over time and now women can spend over one-third of their lives in menopause. Realizing the effects of hormone deficiency is the key to understanding the many benefits of hormone therapy; once the knowledge is there, this therapy becomes self-explanatory. At some point in a woman’s life, menopause is inevitable. Bioidentical hormones can alleviate the symptoms associated with lack of estrogen and testosterone as these levels fall with age. Quality of life is enhanced as skin tone, hair, nails, energy, libido, mind clarity, sleep, hot flashes, night sweat, and memory are all improved. Bioidentical hormone therapy also offers many health benefits. After using natural hormones, women experience an increase in bone density which protects against osteoporosis. Muscle metabolism is also improved leading to an increase in muscle mass and reduction of body fat. Testosterone protects the heart and heart disease is the number one medical problem in this country. It is important to point out that bioidentical hormone therapy is not a one size fits all treatment. In other words, every patient has their therapy individualized, so it is custom prepared to fill their body’s deficiencies and put the body back into balance. Patients sleep better, therapeutic levels are more consistent, and everything improves over time. Patients who are using bioidentical hormone therapy have a high degree of satisfaction. I am consistently told: “I should have done this sooner,” “my children want to be around me again,” “I have more energy,” and “my relationship with my husband has been enhanced and is getting back to the way it used to be.” Men can also benefit from bioidentical hormone therapy. As men age, testosterone levels decrease. With hormone therapy men notice increased muscle strength, more energy, and a better libido. Last month, the City of Pembroke Pines was notified that Money magazine’s annual “Best Place to Live in America 2014” list ranked Pembroke Pines #32 in the top #50, and the only city in Florida to make the list. They reviewed 781 cities across America with populations 50,000 to 300,000 and ranked them based on more than 50 factors including economic strength, job growth, affordable housing, safety, schools, healthcare, arts and leisure and ease of living. We couldn’t be prouder. It’s not government, however, that makes a city great. It is residents and businesses working together and supporting each other through challenges and prosperity. We are equally excited that Pembroke Pines has also been named 2014’s Best City of Hispanic Entrepreneurs in a recent study conducted by WalletHub, a personal finance outlet. In celebration of National Hispanic Heritage Month, they assessed the minority business climate within the 150 largest U.S. cities and examined 19 key metrics, including Hispanic entrepreneurship rates, corporate tax systems and the share of businesses owned by Hispanics. We work hard to ensure diversity and equality throughout our city. And, we celebrate the hard work and dedication of all entrepreneurs who face today’s economic challenges.
