New 3D I-Cat SCaN teChNology at NaIDu orthoDoNtICS Naidu orthodontics’ investment in 3D It shows the correct root angulations and finds supernumerary teeth dental imaging technology provides best and their exact locations to communicate clinically correct information to oral surgeons to facilitate precise extractions and avoid possible results for their patients. The initial task, of any orthodontic treatment, is to establish and understand the exact tooth position and relationship of the patient’s anatomy. This is traditionally done using tooth impressions and molds as well as regular X-ray technology. These methods are often extensive and perceived by many patients as less than pleasant. However, capturing this information is critical to developing treatment options through a clinically responsible approach, while at the same time it controls radiation exposure to the patient.
Dr. Kistama Naidu, DMD, MS. Board Certified Orthodontist
The office of Naidu Orthodontics recently introduced low dose I-Cat Scan technology – a first in Broward. This allows the precise capturing of 3D initial orthodontic workups and progress scans. At the same time this advanced technology reduces the applied X-ray dosage significant with a shortened scan time of just 4.8 seconds. why is accurate initial exploration important? Capturing precise accurate 3D views to analyze teeth, roots, TMJ, airway, and sinuses without magnification or distortion enhance the dental practice’s efficiency by capturing a complete workup in less than 10 seconds.
exploratory surgeries. It allows our office to replicate dental implant placements virtually using the convenient anchor pin library.
Quickly and easily all the available images are compiled into a virtual study model service complete with crowns, roots, developing teeth, impactions and alveolar bone. With just one click, 3D cephalometric analysis are created, which also yields full traditional 2D analysis. why is low dose I-Cat Scan technology important? Traditional dental scanning technology utilizes the unique properties of X-rays to create a 2D image of the patient’s facial bones and teeth. The advanced I-CAT scan technology allows us to not only create a 3D image, but it also reduces the amount or dosage of X-ray exposure. In short, I-Cat Scan technology allows Naidu Orthodontics to gather more information in order to map the most effective, and least invasive, treatment plan for the best possible tooth realignment while minimizing the patient’s exposure.

Happy Valentine’s Day
W
hile it seems like just yesterday we were
ringing in the New Year, now we turn our attention to the ones we love with Valentine’s Day just weeks
away. The stores are stocked with sweets and treats for our sweethearts. While we contemplate which gift we should purchase or which restaurant we shall visit for that romantic night out, we need to remember that the monetary value of a gift is a secondary notion. The most important point is that the gift comes from the heart -- and there is no better gift than spending valuable time with the ones we love. This Valentine’s Day, give from your heart and enjoy each other’s company. Happy Valentine’s Day!

He has become well known among children and adults in the community for his talks on Stranger Danger, cultural awareness and more. On February 12, Wilson will retire from the Community Affairs Unit. In 1991, he joined the Pembroke Pines Police Department as a patrol officer. It was not long before he moved to the Crime Prevention Unit, which ultimately became the Community Affairs Unit. Since then, he has been dedicated to developClarence Wilson ing a partnership with Pembroke Pines residents and businesses in order to help to identify and target the problems within the community. “We all want the same thing – to feel safe in Pembroke Pines,” said Wilson. “We need the public to be the eyes and ears of the police department. As long as we have a strong partnership, we will be able to have a positive impact on crime statistics and solving crimes.” Wilson has spent most of his career delivering the police department’s safety message to the public. He has been a facilitator for the Police Explorers program, Community Emergency Response Team, and Citizens Police Academy. He has conducted safety presentations for school children through Career Days, Safety Town, and the Stranger Danger program. He has given cultural awareness and diversity training for law enforcement, as well as for high school and college
students. He has met with homeowners’ associations and neighborhood watch programs. He has conducted security surveys for local residents and businesses. He has been involved in numerous service organizations, such as the Kiwanis Club, Rotary Club and Chamber of Commerce. And so much more. “I am thrilled that I have had the opportunity to impact so many lives in Pembroke Pines,” said Wilson. Cheryl Watters, police service aide, has been working with Wilson since 1997. “Clarence is a fantastic person,” said Watters. “He is willing to help anyone in need. When he speaks to children and adults, he knows how to get his message across. He always lights up the crowd and puts a smile on their faces.” Wilson will maintain his connection with the Pembroke Pines Police Department as a reserve police officer and plans to volunteer for 16 hours a month. He is looking forward to spending more time with his wife, who is a retired police officer, as well as his daughter, who runs track at the University of North Carolina at Chapel Hill. He also plans to travel more. Clarence Wilson delivers a safety message to a group of school children.

The PLC Law Group Gets Results By Debby Teich
extensive trial experience working as assistant public defenders and assistant state attorneys. Aside from being a multicultural law firm, our lawyers and staff have had a wide range of practical professional experience outside of the practice of law, which helps the firm have a real perspective on our clients’ needs. This goes above and beyond just analyzing the “letter of the law.”
Describe your staff. The culturally diverse staff has professional experience in banking, finance, mortgages, accounting and real estate.
What is your (Michael Samuda) background? I have represented hun-
dreds of clients in foreclosure matters. I have provided assistance to clients in real estate matters even while in Law School through the Immigration Clinic. Currently, my practice is concentrated in the areas of foreclosure defense, immigration, family law and general civil matters.
The PLC Law Group is comprised of highly skilled, experienced, professionals who work closely with each client to provide the highest possible standard of service.
T
he PLC Law Group focuses on Real Estate (Foreclosure Defense), Immigration, Family Law, Personal Injury, Criminal Law and Bankruptcy. The staff is comprised of highly skilled, experienced, professionals who work closely with each client to provide the highest possible standard of service. They develop the best strategy for each case, so clients get the results that they need and deserve.
Michael Samuda, Managing Partner, answered some commonly asked questions about the firm.
What are the main areas of practice at the firm? The main
areas of practice include: Real Estate (Foreclosure Defense), Immigration, Family Law, Personal Injury, Criminal Law and Bankruptcy.
The PLC Law Group helps clients obtain a positive result with loan modifications, short sales, or general defense of a foreclosure complaint.
How do you help clients with their real estate matters? One of
our main practice areas is foreclosure defense. We help these clients obtain a positive result with loan modifications, short sales, or general defense of a foreclosure complaint.
How do you help clients with immigration issues? We provide a wide
array of immigration services. They include, but are not limited to, family or business based petitions, deportation defenses, and asylum applications.
How do you help clients with Family Law? We help clients in dissolution of marriage (divorce) proceedings, as well as custody, support, and paternity issues.
How do you help clients achieve the best possible results? We
understand that each client brings a uniquely different case based on his or her unique set of circumstances. We carefully evaluate each client’s objective and then formulate a course of action to achieve that objective. We value each client’s perspective as a critical piece of the legal puzzle to insure that we do our best to advocate, negotiate and litigate for their best interest.
What is special about your attorneys? Our attorneys have gained
22
February 2014 | pembroke pines town times
What is Broderick Taylor’s background? Broderick Taylor is a former Prosecutor and handles most of our foreclosure defense and criminal cases.
What is Vene Hamilton’s background? Vene Hamilton is a mentor to
the firm. She has a vast amount of knowledge in multiple areas of practice. She was the first black to be president of Broward County Women’s Bar Association and the first female president of the T.J. Reddick Bar Association.
What differentiates PLC Law Group from other law firms in the areas of real estate, loan modifications and foreclosure defense? Our extensive involvement with banks, realtors, mortgage companies, title underwriters and the courts really separates us from most law firms who have just recently added real estate and loss mitigation departments to their practice. Since the early 2000’s, our firm has helped thousands of homeowners with their particular real estate needs and has also conducted hundreds of closings. We also work closely with of counsel attorneys in the areas of Personal Injury and Bankruptcy. We are a complete general practice law firm.
What are the key things that people should consider before hiring a lawyer? The hiring of an attorney should not be based solely on adver-
tising. Potential clients should have a lengthy one-on-one consultation with an attorney to see if that lawyer or firm is a good fit for their particular situation.
Does the firm offer free consultations? Yes. Our consultations are always free.
Do you have offices in other locations? We have had offices
in many different states. We are concentrating our current efforts in the South Florida market and still continue to service our New York and New Jersey clients. The PLC Law Group is located at 1806 N. Flamingo Road, Suite 280, in Pembroke Pines. For information, call 954-449-7144 or visit www.plclawgroup.com.

Pines Charter High School proved once again that community service and having fun are not mutually exclusive. Eighteen volunteers from the school served dinner to the seniors at the Southwest Focal Point Community Center and then cleaned up. At the end of the meal, the students and seniors had a great time together, as they kicked up their heels on the dance floor. This was the fourth year that members of the school’s National Honor Society participated in the event. `“Our eighteen PPCHS volunteers had as much fun as the seniors,” said Kolby Rudd, a Social Studies Teacher at the Pembroke Pines Charter High School and National Honor Society Moderator. “They are looking forward to volunteering at

Chapter membership not only recognizes students for their accomplishments, but challenges them to develop further through active involvement in school activities and community service. Each of the 80 NHS members at the school must earn a minimum of 20 NHS volunteer hours, in addition to the standard 40 service hours needed to graduate. y first two months as Mayor of Broward County serving as your District 8 Commissioner has been very busy, fast paced but exciting! I have enjoyed attending many wonderful events in the District and throughout Broward County. I had the pleasure of attending the 1st Annual “Broward’s Got Talent” youth talents showcase at the Southwest Regional Library in Pembroke Pines. I was blown away by the rich talent right here in South Florida and I cannot wait for next year’s program. The Parkway Academy youth choir and orchestra did a fantastic job at the tree topping ceremony for Rose Miniaci Arts and Education Center for youth in Broward County. Through a partnership with Pembroke Pines and Broward County I was proud to attend the grand opening of The River Grass Arts Park & Susan B Katz Theater in Pembroke Pines. The art work on the outside of the building is just as spectacular as the talent show cased inside. The ceramic tile public art project “All the Worlds a Stage” by artist Suzi K. Edwards. I encourage you to see this artwork in person, it is amazing! Last month, the BB&T Center in Sunrise announced record breaking ticket sales for Billy Joel – over 40,000 tickets were sold for the two night engagement – as a representative of Broward County I presented the Key to Broward County to Mr. Billy Joel. The venue improvements draw top rated artist to Broward County and boost ticket sales at future events which bring more culture (and fun) to South Florida.
I would be remiss if I did not mention that we should reach out to those that need our support. There are two events that I have focused on for January and February. The first is the 4th Annual Dan Marino Foundation Walkabout Autism and Expo, held on January 25th, at Sun Life Stadium. Autism is often misunderstood and I hope with this event and the Autism In Flight event (mentioned in last month’s article) will help bring awareness of those living with autism. The second event is the 5th Annual Tour de Broward. This consists of five fitness and recreational events for families and the community. It includes a 100K and 50K cycling challenge, 5K Timed Run, 3K Family Walk and Power of Play Kid Zone. It will take place at Miramar Regional Park on Sunday, February 23rd, 2014 at (7am). Monies raised for this event will go to the Pediatric Cardiac Center at Joe DiMaggio Children’s Hospital to purchase high-tech equipment and provide emergency assistance to families. I am the team captain for “Dream Team Broward”, and I encourage you to come out and support this event or form your own team. As you know, I launched “Broward Means Business” to not only showcase local businesses (big and small), but to bring assistance and to cultivate innovation and talent we have right here in South Florida. In order to facilitate entrepreneurs, Broward County’s Office of Economic and Small Business Development will host workshops to assist small businesses please call 954-357-6400 for more information. I am volunteering as a business mentor for the South Florida based Avanze Corp. and they will be running monthly seminars throughout Broward County. Avanze Corp assists all types of small businesses from incorpora-
tion, idea development, marketing, business plan writing and obtaining financing. To learn more about their services please call 888-848-7771. With the birthday celebration of Dr. Martin Luther King, Jr. and having February serve as African-American/ Black History Month, awareness is brought to AfricanAmerican history – it is a fundamental part of American history - and I am reminded how far we’ve come in a short period of time. As the first African American Female Mayor of Broward County it gives me pleasure to know that we as a community are coming together and bridging all gaps of divide to work in unity for the greater good of our County. 2014 marks the Golden Jubilee or 50th Anniversary of the 1964 Civil Rights Act. The Civil Rights Movement is typically thought of as an African-American movement. However, the Civil Rights Movement did lead to improvements in the legal rights of oppressed groups of people and, more specifically, the 1964 Civil Rights Act. It also outlawed major forms of discrimination against racial, ethnic, national and religious minorities, and women as well as ending unequal application of voter registration requirements and racial segregation in schools, at the workplace and by facilities that served the general public. Annually, my family and I participate in the Dr. Martin Luther King, Jr. parades and celebrations in Hallandale Beach, West Park, and Miramar. I encourage you to participate in this month’s events or take a moment to delve into the rich history of America. As Dr. King once proclaimed, “we are all made by history. ave you been thinking about a new look , a
new you? Maybe now is the time to do something for yourself that will give you that much needed boost you’ve been looking for. But what would that be? There are many reasons to want to improve something about your appearance. Maybe you are a mother whose body just isn’t the same after multiple pregnancies. Loose skin, stretch marks and sagging breasts can all be improved with surgery. A “mommy makeover” can include a tummy tuck, which tightens the underlying muscles and removes the redundant excess skin. When liposuction is done along with the abdominoplasty to contour the waist and hips, the end result is the flat stomach you had prior to pregnancy. It can also include a breast lift, with or without an implant, to lift and reshape your breasts to restore a more youthful appearance.
With the challenging economic times, more and more individuals are returning to the work place. Are you looking for a new job, or just trying to stay competitive in the job market? You might want to consider having a facelift. A facelift can take years away by improving the appearance of the nasal labial folds and jowls. This rejuvenating procedure will give you a more youthful look that matches how you feel inside. Or maybe you just need an eyelid lift. A blepharoplasty will take away that tired look and leave you looking refreshed and vibrant. Not ready for surgery? Think about a liquid facelift, where Botox and injectable fillers are used to restore volume in the face lost with aging and sun damage and decrease the lines and wrinkles around the cheeks and mouth. Noninvasive fractional laser resurfacing is another way to diminish fine lines, wrinkles, and improve skin pigmentation to make you look and feel your best. Skin tightening can also be achieved through other advanced, non-invasive laser techniques. If these procedures are not in your budget, you can still benefit from advanced techniques and skin rejuvenation through our medically supervised skin care program. At the Weston Cosmetic Surgery Center, our experienced skin care specialists, under my direction, perform superficial chemical peels, facials, and gentle crystal-free microdermabrasion to enhance and rejuvenate your skin. There is minimal downtime with these treatments and you will see an improvement in your skin texture and tone after one or two treatments. We also offer an individualized skin care regimen which you can continue at home including Obagi, TNS SkinMedica and Vivité, to name a few. There is no excuse today to not do something for yourself to improve your appearance. Give yourself a gift of beauty and confidence today. Arthritis in the Foot and Ankle By Robert H. Sheinberg, D.P.M., D.A.B.P.S., F.A.C.F.A.S.
A
rthritis is a common ailment, especially as
people age. Here are answers to some commonly asked questions about arthritis: ▸▸ What is arthritis? Arthritis is a degenerative process that may take place in any joint in the body. Cartilage which allows the joint to move smoothly starts to wear down. Bone spurring and bone cysts may occur as the joint wears abnormally. The joint starts to move out of its normal position creating pain with movement. A loss of joint movement also follows the arthritic process. ▸▸ What causes arthritis? Most often arthritis is caused from an injury to the joint. The injury may occur as a child and it may manifest itself later in life. A bad injury to a bone or joint (fracture) may start the development of arthritis almost immediately after the injury. Arthritis is very common when a bone fractures into the joint. Malpositioning of the joint will cause abnormal wear and tear. It may also be seen in people who are excessively flatfooted, knock-kneed or bowlegged. It is also very common in people with bunions. ▸▸ Will the arthritis get worse over time? In almost all cases arthritis in any joint will worsen. The joint will wear abnormally causing pain, limited motion of the joint, bone spurring and swelling around the joint. The joint will become excessively stiff causing pain with many activities. Difficulty with certain shoes may also be present. ▸▸ Will medication help to lessen the pain and joint deformity? Oral medication will help to diminish some discomfort, but it will not change the degenerative process, which takes place slowly over time. ▸▸ Will physical therapy be of any benefit? Physical therapy will help retain some movement in the joint and keep swelling to a minimum. It may also help to improve muscle strength. Over the long term it will be of limited benefit as the degenerative process will continue to take place. ▸▸ Can surgery be of benefit if the arthritis is found early? Many procedures can be performed to assist in treating the degenerative process if found early. After a thorough history and physical examination, x-rays are taken to assess the degree of joint damage. Occasionally, if the damage is mild, an arthroscopic or small open procedure may be necessary to remove abnormal bone and soft tissue. Cartilage that is lifted away from the bone is removed to allow the joint to function more normally. If a joint is out of position removing bone spurs and soft tissue may be of limited benefit. In this case, the bones may need to be repositioned to allow more normal joint alignment. ▸▸ Can anything be done if the arthritis is severe? In some cases the arthritis is severe and joint replacement or joint fusions may be necessary. Joint replacements are an excellent option and may be necessary in older, more sedentary patients. Fusing the joint that is arthritic and not allowing it to move any longer is also an excellent approach to the treatment of arthritis. This eliminates any painful movement allowing a return to work and activities of daily living with minimal modifications. Some joints can be fused and still allow an athlete to resume professional sports. ▸▸ Will fusing the joint cause any long-term problems? Joints that are fused will be pain free. Other joints adjacent to the fused joint will need to work a little bit harder and in most cases remain asymptomatic, especially if they are healthy at the time of the surgery. ▸▸ Will orthotics be of any benefit? Orthotics will assist in maintaining normal joint position. Support of the foot and placement of the foot in better anatomical position may redistribute stress more evenly in the foot and ankle region. This may help to lessen pain and possibly delay the need for surgery. They provide great benefits in people with big toe joint (turf toe, bunions) problems. Orthotics are also very useful in patients who are flatfooted, knock-kneed, bowlegged and in those with lower back pain. Pembroke Pines Update By Mayor Frank C. Ortis
M
any N ew Year resolutions were made and
for most of us they have probably already being broken. However, one of my resolutions that will consistently stand fast is my wish to ensure that you and our local businesses are always a top priority. Last year, we worked long and hard with city, county, state and federal officials to ensure the Pembroke Road overpass at I-75 becomes a reality. Construction will begin this year. We fought hard for educational funding and will continue to do so, becoming that squeaky wheel that gets results. Our children are just too important not to. The list goes on and we are determined to do our best at every turn. I do have optimistic hope for this to be a year of positive growth and change. It is an election year for our city, as well as for our Florida legislature. So it is extremely important that you vote and make your voices heard. There are many complicated challenges ahead regarding school budgets, education standards, public safety, economic development, transportation, and numerous other concerns. We are leaders in issues that directly affect you and local businesses, and as such, encourage everyone to make this a year in which you get involved.
I’d like to invite everyone to attend my Town Hall meeting on Monday, February 3, 2014 at 7 pm at City Hall in the Commission Chambers, 10100 Pines Boulevard, on the south-west corner of the intersection of Pines Boulevard and Palm Ave. This meeting will cover topics of interest and concern to everyone in our community. I look forward to seeing many new and old faces, so that we work together in keeping this the greatest city! As always, if you have any questions or concerns, please do not hesitate to contact my office at 954-4356505, or by email at fortis@ppines.com.

Most Common Mistakes by Homeowners Attempting to Obtain a Loan Modification By Omar J. Arcia, Esq. Foreclosure Defense and Consumer Bankruptcy Protection Attorney
A
s a result of various nationwide class actions and lawsuits
filed against mortgage companies by private individuals and government agencies, lenders have become more receptive and willing to assist homeowners in modifying their loans. However, lenders have also become more demanding of certain documentation to support loan modification requests. These are the most common mistakes we’ve seen homeowners make when processing a loan modification on their own. Provide Too Much Information. Most Homeowners believe that the more information they provide to their lender about their finances, the more likely their loan modification will be approved. Just the opposite is true. Most lenders are looking for specific information regarding a homeowner’s income and expenses, specific reasons to support a financial hardship for a loan modification, and they require a specific order and timeframe to submit the required documents. Overwhelming your lender with the same unnecessary documents time and time again will only place your file farther back in line for consideration. The Arcia Law Firm has represented homeowners that have been trying to modify their home loan for years, and within less than 30 days after retaining the Firm and submitting the appropriate documents, a loan modification is achieved. Refusing to Seek Legal Assistance. We have counseled hundreds of clients in mortgage related cases through the years, and the most significant mistake that homeowners make is to wait until the last minute to hire a knowledgeable and experienced attorney. Many homeowners think that they can easily defend themselves in a foreclosure proceeding, process their own loan modification, and save their home. However, some of the legal defenses to a foreclosure are highly technical, constantly changing, and require specialized knowledge. The attorneys representing banks and the judges overseeing in these cases are primarily interested in achieving an end result -- obtaining a final judgment of foreclosure, sale of the property at a public auction and closing the case. In more simple terms, this means you are out of your home before you can finish your loan modification negotiations. The key to prevent this end result is for every homeowner either in foreclosure or at risk of foreclosure, to stop playing “Russian Roulette” with their family home, and immediately hire a highly qualified law firm to assist them. The attorneys at the Arcia Law Firm have more than 40 years combined legal experience, and we are privileged to successfully represent hundreds of homeowners in our community with foreclosure defense and bankruptcy cases. If you are struggling to make your monthly mortgage payments, if you are in the middle of a frustrating loan modification process, or if you just received a foreclosure summons, contact the Arcia Law Firm today at 954-437-9066 to schedule a free consultation, or visit www.arcialawfirm.com to schedule your appointment online. Detailed explanations of different options available to homeowners are also discussed in a new DVD developed by the Arcia Law Firm entitled “Fight For Your Home.” If you mention that you heard about the DVD through this magazine, you will receive a complimentary copy during your free consultation to discuss your case in detail with a member of our highly experienced and determined legal team. Contact the Arcia Law Firm at 954-437-9066 to schedule your free consultation. T
he issue of Alimony R eform will once again be
addressed by the Florida Legislature in 2014. Both the Florida House and Senate passed sweeping changes last year, only to have Governor Scott veto the bill at the 11th hour. The primary focus of the new law was to end what we now call permanent alimony. This classification of alimony can be required in long term marriages, which are defined as marriages 17 years or greater. Permanent alimony will terminate upon the death of either party, or the remarriage of the receiving spouse. In addition, it can be modified (increased or reduced) if there a substantial change of circumstance occurs. A substantial change of circumstance usually refers to a change in the finances of either party. If either spouse loses his or her job and a new job results in a lower income; that would be considered a substantial change of circumstance. The change, however, cannot be voluntary in nature. A person cannot quit his/ her job and then ask the court for some relief due to this intentional act. Permanent Alimony can also be terminated or modified if a “supportive relationship” exists. The question is, “what exactly is a supportive relationship?” Florida Statutes does in fact define such a relationship, but it is certainly open to interpretation. It exists when a couple, who is not legally married, do in essence hold themselves out as a married couple. Many individuals who receive permanent alimony will avoid re-marriage, since it will end alimony by operation of law. The Supportive Relationship Statute was enacted to deal with couples who are in essence married, but not by operation of law. The Courts are to consider the following in determining whether a supportive relationship exists: ▸▸ The extent which the couple has held themselves out as a married couple or otherwise conducted themselves in a manner that evidences a permanent supportive relationship. ▸▸ The amount of time the parties have lived together. ▸▸ Have the parties pooled their income or assets or otherwise exhibited financial interdependence?

▸▸ The extent that either party has supported the other. ▸▸ The extent of which either party has performed valuable services for the other party. ▸▸ Whether the parties have jointly contributed to the purchase of real property. ▸▸ Whether the parties have an express or implied agreement concerning support or property sharing. The above criteria clearly show that there is much room for interpretation by a judge. It is believed that the Florida legislature will in fact address this statute in an attempt to clarify the intent. If you are someone who is either receiving or paying alimony, it would be a good idea to keep up with news from Tallahassee, once the legislature is back in session. The law firm of Evan H. Baron and Associates is located at 1655 North Commerce Parkway, Suite 201, in Weston. For information or a free consultation, call 954-385-9160.
When “Leaky” Valves Are at the Heart of the Problem Your heart has four valves. They open during a heartbeat so that the blood can flow from one chamber to another and close to make sure that blood can’t leak back out. Mitral regurgitation occurs when the valve between the upper and lower chambers on the left side of the heart does not close properly. Mitral regurgitation can be caused by progressive structural changes in the leaflets of the mitral valve, certain infectious diseases and some heart attacks. Mitral valve prolapse is a structural abnormality of the valve where the leaflets have extra tissue and are displaced from their normal position (too far backward). A small percentage of patients with mitral valve prolapse will eventually develop regurgitation. “Symptoms of mitral regurgitation include shortness of breath or difficulty catching your breath (especially with exertion), palpitations that feel like rapid or irregular heartbeats, and swelling of the feet and ankles, said Edward Savage, MD, cardiothoracic surgeon at Cleveland Clinic Florida. “Other symptoms are weakness or dizziness, and increasing fatigue.”
Edward Savage, MD Cleveland Clinic Florida Cardiothoracic Surgeon
An important part of the diagnosis is recent medical history. During a physical examination, your doctor may look for fluid in the lungs, an enlarged heart, or a heart murmur, which is the sound made when blood moves through a stiff or leaky valve. Your doctor may suggest further testing for evaluation. The test to diagnose mitral regurgitation is an echocardiogram. Sound waves are aimed at the heart allowing a detailed picture of the structure of the valves, which aides in the assessment of leaks. The strength of the heart muscle can also be assessed. Sometimes a higher resolution echocardiogram is needed to accurately assess the mitral valve. This is performed by passing a sound-generating transducer into the esophagus. This study is a TEE or transesophageal echocardiogram. Not all leaky valves require surgery. If the leak is mild or moderate, it is usually treated with medication and close follow-up. However, if the leak is severe, even if there are no clear symptoms, surgery is often needed. Sometimes patients who believe they have no symptoms actually do, but have adapted to their limitations. The only treatment currently available for severe mitral regurgitation is surgery. It is important to treat the problem before the heart has lost significant functional capacity. In most cases the valve can be repaired (>80%). Studies show that mitral valve repair results in better long-term survival, more preservation of heart function and lower risk of complications. If the valve can be repaired, no long-term use of blood thinners (anticoagulants) is required. Some valves cannot be repaired and require replacement. Whether repaired or replaced, most patients experience dramatic improvement in symptoms and functional status. Nearly all isolated mitral valve repairs or replacements can be performed using a minimally invasive approach through a 2-3-inch incision on the right side of the chest. Cleveland Clinic Florida specialists are experts with extensive experience. They specialize in the diagnosis and treatment of patients with all levels of heart disease, from the most simple to the very complex.

all created equal? No, they are not. Orthotics, sometimes referred to as “orthoses” or even “arch supports,” is a device inserted into a shoe. It can range from a felt pad to a custom-made shoe insert. It can correct an abnormal or irregular walking pattern. Orthotics allow people to stand, walk, and run better and with more comfort. While over the counter orthotics, typically seen in drug stores, are available and may help ease mild symptoms, they typically do not correct the wide range of symptoms that prescription foot orthoses can. Prescription orthoses are custom made to fit a person’s unique foot structure. Orthotics come in many shapes, sizes, and materials and fall into three general categories: those that change foot function, those that are mainly protective in design, and those that combine functional control and protection.

Rigid orthotics fall into the first category. They are designed to control function and are used normally for walking and dress shoes. They are often made of firm material like plastic or carbon fiber. They are made from a mold after a podiatric physician takes a plaster cast (or other image) of the foot. Rigid orthotics control motion in two major foot joints that lie below the ankle joint. They can improve or eliminate strains, aches, and pains in the legs, thighs, and lower back. Soft orthotics are mostly used to absorb shock, increase balance, and take pressure off sore spots of the foot. They are effective for diabetic, arthritic, and deformed feet. Soft orthotics are usually made of cushioned materials, so they can be worn against the sole of the foot. They extend from the heel past the ball of the foot, and include the toes. Soft orthotics, like rigid ones, are made from a mold after the podiatric physician takes a plaster cast. Lastly, there are the semi-rigid orthotics. They provide foot balance for walking or engaging in sports. They are typically made from soft material and reinforced with more rigid material. Sometimes they are prescribed for children with flat feet and in toeing or out toeing disorders.
