More than a year into the pandemic, I find myself imagining all of the little ones who have been born during this scary and uncertain time — many have already turned 1! I think of the moms and dads who have stepped (or will soon step) into new parenthood in the midst of this terrifying marathon, forced to cobble together a whole new blueprint for normalcy and social support. An isolating experience plagued with its share of garden-variety fears in the best of circumstances, caring for a newborn is presently complicated by the threat of an actual plague. I reflect on the existential threats that terrorized me when my first child was born. There was the Nisqually earthquake that jolted us off our literal foundation when she was just a few weeks old. Seven months later, another thunderbolt, as we awoke to surreal televised scenes of the 911 terrorist attacks. Our sense of the world as a frightening place becomes acutely heightened when we have a child — but correspondingly, we are somehow made all the braver for it. The essential skills of this long-haul gig of parenting are rapidly alloyed, building through our response and servitude toward an indestructible power, a limitless endurance and an empowering trust in a wellspring of intuition that is born along with our actual child. But the wisdom and support of the wider parenting village is there for all of us, even (and maybe especially so) in this time of the coronavirus. Coping With Crying Tips from a parent educator for soothing your baby (and yourself) By Malia Jacobson Caring for a wailing newborn who seems inconsolable is exhausting, frustrating and isolating. That’s been especially true during the pandemic, which has increased fears about exposing infants to disease, squeezed child-care resources, slashed social support and compounded financial insecurities. With fewer opportunities for social and emotional support or for outside help with child care and household chores, frustration may give way to anger and increase the likelihood of shaking a crying baby. Research shows that rates of child abuse and abusive head trauma — the type of brain injury that happens when an infant is shaken — escalate during economic recessions and natural disasters. During the national recession that began in 2008, Seattle Children’s Hospital and Harborview Medical Center saw the number of such cases more than double, mirroring a trend occurring nationwide. In response to the increase in abusive head trauma cases locally and nationally, Seattle Children’s established its Period of PURPLE Crying caregiver education program (purplecrying.info). Christine Baker, coordinator for the Period of PURPLE Crying program, says that when COVID-19 began impacting new parents’ access to social support and child-care resources last year, child abuse researchers feared the worst. “Last year, at the beginning of the pandemic, we were fearing that there would be a similar effect with rates of child abuse, but it was hard to predict,” she notes. “There is research showing these effects after recessions and natural disasters, but there wasn’t data on the effects of a global pandemic like this one.” Now, more than a year into the COVID-19 pandemic, research has shown a similar spike in cases of child abuse. According to the Centers for Disease Control and Prevention, the percentage of emergency department visits and hospitalizations related to child abuse and ne20

glect for children and adolescents increased significantly from 2019 to 2020. “At this moment, we have a combination of isolation and virtual-connection fatigue, particularly for new parents who may be literally stuck at home with an infant,” says Baker. “People are tired of connecting online and they aren’t leaning into the virtual parties and gatherings the way they were a year ago. The supportive ‘We’re all in this together’ messaging is just not there anymore, because people are just over it.” Reaching caregivers about PURPLE crying Over the past year, COVID-19 has complicated efforts to educate caregivers about the dangers of abusive head trauma (sometimes called shaken baby syndrome). With more families delaying well-child checkups and community events temporarily curtailed, Period of PURPLE Crying program leaders have fewer opportunities to help new parents learn ways to cope with crying. Getting information to caregivers in the early weeks of parenthood is critical, because it only takes one shaking incident to permanently alter a child’s brain function, says Baker. “A shaken brain will never be the same brain again.” Shaking causes injury instantly, with effects that can compound over time with repeated episodes. Shaking any part of a baby, even


CHILD CARE the arms or legs, can cause learning disabilities; behavior disorders; problems with hearing, vision and speech; seizures; cerebral palsy; permanent disability; and even death. Using the acronym PURPLE (peak of crying, unexpected, resists soothing, pain-like face, long-lasting and evening), the program helps spread the message that a phase of intense crying is developmentally normal, temporary and not the result of genetics, parenting or your child’s personality. While not all babies go through such a phase, many do, crying for as long as five hours in the afternoon or evening during their second and third months. Crying generally tapers off around months four and five, but not before caregivers’ nerves are thoroughly frayed. Coping tips for caregivers Knowing that this phase is normal and doesn’t reflect poorly on their child or their parenting can help parents cope, says Baker. Working with families in Pierce and King counties and surrounding areas, Baker counsels caregivers to “just pause.” “We tell parents to ‘take five,’ because taking a pause or a break is so important to keep frustration with crying from escalating. We tell parents to just pause, take a minute, set your baby down gently someplace safe and walk away for a few minutes.” Parenting isn’t about perfection, says Baker. “It’s about doing what you need to do in the moment and handling a situation in a way that you’ll feel good about tomorrow.” Caregivers should know that periods of intense PURPLE crying are normal for babies, and so are feelings of anger — even fury — for parents. During tough moments, calling or texting a trusted friend or family member, listening to music, or taking a few deep breaths can keep frustration from escalating to rage — and keep babies safe from unintended outcomes. “A lot of parents don’t feel prepared to deal with the anger they feel, because it’s so unexpected,” says Baker. “They love their baby, they wanted and planned for this baby, and then they feel this anger that can be very isolating. Those are really normal feelings. We help parents expect and plan for that, so when those feelings show up, they know what to do.”

What is the Period of PURPLE crying? Peak of crying. Your baby may cry more each week, the most during month two, then less in months three through five. Unexpected. Crying can come and go, and you don’t know why. Resists soothing. Your baby may not stop crying, no matter what you try. Pain-like face. A crying baby may look like they are in pain, even when they are not. Long-lasting. Crying can last as long as five hours a day, or longer. Evening. Your baby may cry more in the late afternoon and evening. The word “period” indicates that the crying has a beginning and an end.

Rash Guard: When to Worry About Baby’s Skin By Malia Jacobson Spring is here, and local little ones are enjoying the outdoors in strollers, baby swings and backyards. For some, though, peeling away winter’s heavy layers reveals angry, irritated skin. If your baby’s skin seems unusually irritated these days, they’re not alone. Markus Boos, M.D., Ph.D., a pediatric dermatologist with Seattle Children’s Hospital, says his infant patients are coming in with more severe cases of common rashes, most likely because families are waiting longer than usual to take children to the doctor. “I think families are delaying care because of concerns about the safety of going out in public with their infant during COVID,” he says. Delaying care for skin irritations is understandable, particularly during the past year. After all, rashes on babies are sometimes dismissed as harmless, even unavoidable, childhood nuisances. But skin rashes can sometimes signal more serious health conditions, reactions to medication or even a serious COVID-19 complication, so it’s a good idea to check in with your child’s health-care provider if a rash appears suddenly, seems painful or infected, or is accompanied by cold or flu-like symptoms, such as fever, vomiting, changes in appetite or diarrhea. Here’s a primer on common rashes, along with information about when to worry about them and how to care for irritated skin at home.

children outgrow it by the time they reach kindergarten age. Heat rash As temperatures rise during spring and summer, caregivers may notice clusters of tiny pink or red bumps or water blisters on their child’s upper body, mainly the neck, chest or upper back. Caused by blocked sweat glands, heat rash can be itchy and uncomfortable, but it generally doesn’t signal an underlying illness and it is not accompanied by a fever. To treat heat rash, cool the affected skin with a damp washcloth or a cool (not freezing!) bath without soap and allow the skin to air-dry whenever possible. Psoriasis Scaly, thickened patches of skin covering a red, inflamed rash underneath could signal psoriasis, a frustratingly persistent condition that affects babies, children and adults throughout their lives. This condition has a genetic component, and while psoriasis can’t be cured, it can be managed with treatments that include topical steroids and oral or injectable medications. Psoriasis may be a manifestation of a systemic inflammatory disorder, notes Boos, and treatment may involve working with specialists in immunology, gastroenterology and rheumatology.

Atopic dermatitis

Multisystem inflammatory syndrome (MIS-C)

This hereditary form of eczema, which causes red, scaly patches near the joints and on the cheeks, is the most common condition Boos treats. Although atopic dermatitis has a genetic basis (meaning it can only be managed, not cured), certain factors make flare-ups more likely. Fragrances, harsh soaps and some chemical cleaners or detergents can result in itchy, uncomfortable rashes on babies and young children. Fortunately, the condition is often treated successfully with topical steroids, and many

In children with MIS-C, internal organs, including the brain, lungs, skin and stomach, become inflamed. “Things to look for include conjunctival injection [bloodshot eyes], bright red cheeks, swelling and redness of the palms and around the eyes, and a nonspecific rash that can look like hives, fine bumps or just

red spots and bumps. Sometimes the tongue can be bright red or white, as well,” says Boos. “In general, the skin findings occur along with or after a fever, but rarely do the skin findings precede fever.” Because of MIS-C’s potential effects on vital internal organs, this is a serious diagnosis, he notes, so prompt evaluation by a healthcare provider is necessary when a combination of these symptoms is noticed in babies and young children. Hand, foot and mouth disease (HFMD) This highly contagious illness may have made the rounds in your family, day care or group of friends. Caused by an enterovirus, hand, foot and mouth disease causes small, circular red spots and blisters on the hands, feet, mouth and buttocks. Babies with HFMD may drool more than normal and have a lackluster appetite, along with a fever, according to Boos. Because HFMD spreads easily, it’s important to keep children at home until their fever subsides and their blisters scab over. And don’t worry if your child loses their fingernails the following month — this is a

common HFMD aftereffect, and your baby’s nails should grow back good as new. Rashes are uncomfortable and generally no fun, especially when they make for a cranky, irritable infant. But most of the time, rashes are easily treated and shouldn’t cause alarm, Boos notes. See your child’s health-care provider if you notice any of the following: • Rashes that involve the eyes, mouth or nose • Infections with blisters and pus • Rashes that seem painful • Rapid changes in the rash’s appearance • Rashes accompanied by fever, vomiting or lethargy

Tiny Tummy Trouble The scoop on poop and other first-year digestive drama By Malia Jacobson Babies, especially newborns, spend most of their waking hours feeding and pooping. So, it’s understandable that digestive issues are a top concern for new parents, says Uma K. Pisharody, M.D., a specialist in pediatric gastroenterology with Swedish Medical Center. “New babies mostly eat, poop and sleep, so anything that makes them fussier than normal often leads to an assumption that there’s a problem with the baby’s digestion.” Happily, most babies have healthy, functional digestive systems — even those who experience an occasional bout of gas, diarrhea or tummy pain. But because common digestive issues such as spitting up, reflux, diarrhea, gas, constipation and changes in bowel movements top the list of concerns for new parents, they’re well worth clearing up. From the first poop to the first birthday, Dr. Pisharody answers your questions about first-year digestive dramas. What are the top digestive issues you see in babies? The top issues I see and treat in babies are feeding problems, difficulty gaining weight, vomiting, changes in bowel movements, like diarrhea or constipation, and reflux. Conditions like digestive tract birth defects or bowel obstructions are much less common.

issues are present, there could be something going on. Does a breastfeeding parent’s diet create gassiness in her baby? While by-products of some foods that make a mother gassier can also occasionally cause gassiness in her infant, this type of gas typically shouldn’t cause severe pain. The gas produced within a mother’s own digestive tract after she eats certain foods doesn’t get passed through her breast milk to her baby. I tell mothers that it’s wise not to eat too much of any one food, but to eat a varied and healthful diet while breastfeeding.

When it comes to spitting up, what’s normal? How much is too much? There is actually no “normal” or “abnormal” amount of baby spit-up, as each and every baby is different, and it’s best not to compare babies to each other. What I look for first and foremost is whether or not there is any forceful vomiting. Second, does the spit-up contain bile or blood? And third, is the baby gaining weight?

A newborn’s poop is already so watery — how can a parent tell if their baby has diarrhea? When I examine diarrhea in newborns, I am looking specifically for a significant change in frequency of bowel movements more than a change in consistency. If a baby who was having about six watery bowel movements each day suddenly begins having 10 to 15, that’s probably diarrhea and worth a call to your pediatrician.

If a baby spits up dozens of times per day but the spit-up isn’t forceful, doesn’t contain blood or bile, and the baby is happy and growing well, I don’t consider that a problem. On the other hand, a baby could spit up hardly a few times each day, but if those

FAMILY HEALTH + WELLNESS ments will vary in frequency from many per day (for example, after each feeding) to as long as one every two weeks for breastfed infants; the latter is not typical, but certainly can be normal. So we look for changes in each individual baby’s routine: If a baby is having three to four stools each day and then starts going a few days between stools, or if the stools suddenly become more firm, that’s something to bring to your health-care provider’s attention. Until solids are introduced to a baby’s diet, we want bowel movements to be fairly runny, not hard or firm. What’s a common myth about baby tummy trouble? One big misunderstanding is that all babies with reflux are experiencing acid reflux in the same way adults do. Reflux in babies is different from the acid reflux experienced by adults. When infant reflux is treated with medication to reduce acid, the medication may end up harming a baby’s digestive process in the long run, instead of helping, since natural digestive acid is essential for normal, healthy digestion. Another myth is that mucous in the stool is always a sign of a problem. In fact, mucous in stools can be normal for babies, particularly those who are breastfed. The amount of mucous depends on the baby’s specific microbiome, or types of bacteria present in the digestive tract. What is colic and how common is it? Colic is inconsolable crying that starts around 1 to 4 weeks of age. Around one in four babies have it. It’s thought to result from immaturity of the nervous system, rather than digestive problems or gas, as many parents assume. What are signs that a baby is experiencing gas and how can caregivers help? Any adult can tell you that gas can certainly be uncomfortable. One way to tell that your baby’s discomfort is from gas is that the pain seems to be relieved by burping or passing gas. I haven’t found OTC [over-the-counter] gas drops to be particularly helpful, but I don’t have a problem with them, if parents want to use them or find them helpful. For babies, gas in the digestive tract is usually caused by swallowed air. Along with

regular burping after feeds, I recommend working with a lactation consultant to improve a baby’s latch during breastfeeding to reduce air swallowed during feeding. Gentle abdominal massage can help a baby who is experiencing gas pains. For a new baby, you can place the baby tummy down on your lap, with the infant’s knees tucked in towards the chest, and gently massage the abdomen with your hands. This has the added benefit of lifting the baby’s bottom up, so gas rises upward and can more easily pass. This can help the baby pass a gas bubble. For older babies, laying them on their back and gently bicycling the legs can help. When should a caregiver seek help for a digestive problem? What are signs that something is wrong? Feeding aversion — when an infant seems disinterested or actively opposed to feeding — is a red flag that needs medical attention. When a baby is not gaining weight or growing, that’s something to bring to your healthcare provider’s attention right away. Prolonged jaundice — or jaundice lasting longer than two weeks — is a reason to see your pediatrician. We also worry about stools that lose their normal brown or yellowish color and become white or creamy, since that might signal a problem with the baby’s liver and bile ducts. Belly distension, extreme or sudden fussiness, forceful and/or bilious or bloody vomiting, or bloody stools are other reasons to contact your health-care provider. When in doubt, contact your provider with questions about your baby’s digestion. If you do head online to search for answers, Dr. Pisharody recommends the GIKids website (gikids.org).
