Welcome New Patients! Speech & Hearing/Pediatric Physical and Occupational Therapy Pen Bay Medical Center


Thank you for choosing to receive your services at the Pen Bay Medical Center Speech and Audiology/Pediatric OT and PT department. Our front office is open from 8:00 a.m. to 4:30 p.m. Monday through Friday and you may leave a voicemail at any time. We are providing this brochure to share some helpful information about our practice.


Medical Records (Health Information) If you need a report from us about yourself or your child (including a report of the evaluation) you must sign a “Release of Information� form at the front window. You will also need to call this number: 207-921-8240 to request the documentation you need.

Scheduling The receptionist will schedule your first visit. You may be scheduling the rest of your visits with your therapist or audiologist. You do not need to check out at the window if you have already made a plan for your next visit with your therapist. All co-pays will be billed to you through the patient accounts office.


Cancellations and no shows If you are not able to keep your appointment we ask that you let us know as soon as possible. This helps us schedule that time slot for someone else. We understand that there are occasionally circumstances that do not allow for much notice, but please call early when possible. If you can only give same-day notice please call as early in the day as you are able. We often have a waiting list for services. If you do not show up for 10% of your appointments in the course of one month we will need to give away your time slot and offer you a different time or placement on the waiting list. Thank you in advance for your understanding.

Weather In the event that we have severe weather please try to call as early in the day as possible if you plan to cancel. If the power goes out or we are forced to close due to severe conditions we will call you as soon as the decision has been made to close so that we can cancel/reschedule your appointment. We usually remain open even if school is cancelled.


Cell Phone Use Cell phone use is discouraged if you are attending an appointment for yourself or with your child. Children are very distracted by cell phones. If you need to use your phone during your child’s therapy visit we ask that you step out into the waiting room to do so.

Pool If your child participates in aquatic therapy at the YMCA you can call our office to cancel appointments. Our number is 921-6380. If a thunderstorm is predicted it is a good idea to call the YMCA (236-3375) before you leave home. They close the pool in the event of lightening, and it often happens quickly so we might not have a chance to reach you before you leave your home.

CDS and School Services Some of our children are seen at schools and preschools. If school is cancelled we are not able to see the children on those days. We do not follow the school schedule here in the clinic, but we do need to cancel therapy for children who are receiving services in the clinic through CDS if school is cancelled.


Illness We ask that you call to cancel and reschedule your appointment if you are sick. If your therapist is sick he/she is expected to stay home and someone will call you to cancel your appointment. These guidelines can help you decide when you should cancel your or your child’s appointment. Please cancel if you or your child has: • A deep or uncontrollable cough • Fever: please wait until 24 hours after the fever has passed without the use of fever reducing medication. • Diarrhea or vomiting: please wait until 24 hours after the diarrhea and/or vomiting have passed. Please do not bring your child to the pool if he/she has had diarrhea in the past 24 hours.

• Bacterial infection including impetigo, strep throat, pneumonia, bronchitis, pink eye, etc.: please wait until 24 hours after you have seen the doctor and begun taking antibiotics. • Head lice: If your child has head lice please wait until 2 days after treatment. • Rashes: Please do not send a child with a rash of unknown origin until he/she has seen the doctor and has been given clearance.

• Colds: With no fever and mild symptoms you are welcome to keep your appointment as long as you feel you or your child will still be able to benefit, However, if the symptoms are uncomfortable with deep coughing and poor energy, it is best to cancel.

• Flu: Please wait until 24 hours after the fever is gone without the use of fever reducing medication, and symptoms are in the comfortable range.

Please do not bring sick siblings to your child’s appointment, and if you are sick please see if there is someone else who can bring your child or consider cancelling your appointment. We strive to protect the health of all our patients and employees.
