The continued collaboration of Pen Bay Medical Center and Waldo County General Hospital has strengthened the foundation required to build a resilient and sustainable healthcare network in the Midcoast. As we celebrate Coastal Healthcare Alliance’s second anniversary, we look forward to working together to provide the best patient-centered care, close to home in Knox and Waldo counties.
With a focus on primary care, our regional team of physician recruiters and practice managers are working to build the strongest network of providers right here in our community. Developing our primary care program will provide a basis for a healthy community and healthcare system for years to come. In 2017, we welcomed 17 highly-qualified providers in specialties ranging from family and emergency medicine to geriatric medicine and midwifery. In just the last few months, we have welcomed three new family medicine providers to Pen Bay Medical Center. Our recruitment efforts will continue, with more primary care physicians and specialists scheduled to join us in 2018 and beyond. The changing face of healthcare requires that we prepare to take on new challenges. When faced with the growing opioid epidemic, we worked
with MaineHealth to assemble a team to address the unique needs of those patients and families. We could not have undertaken a project of this magnitude without the leadership and support of our colleagues across the MaineHealth network, as well as our local leaders in behavioral health and primary care. We are proud to share the details of this system-wide effort within this report. The strength of our network rests in the capable hands of over two thousand colleagues across Knox and Waldo counties. We are thankful every day for their commitment to providing high-quality, patient-centered care. We want to take a moment to thank them for their warmth, patience and tireless dedication to our patients and to each other. For the past several months, the Coastal Healthcare Alliance Board of Trustees has been exploring a closer relationship with its MaineHealth partners. Under the proposed plan, the members of MaineHealth would create a new system Board of Trustees that would have authority over a system-wide budget and would be the governing body for Coastal Healthcare Alliance and other MaineHealth member organizations. To provide the community with details, as well as give the community an opportunity to provide feedback, we have attended numerous community meetings and hosted several town hall forums across Knox and Waldo counties. The conversations have not always been easy, but we can all agree - a stronger healthcare network is the best thing for
patients from North Conway, New Hampshire to Belfast, Maine. For many years, MaineHealth has been a trusted partner; providing Pen Bay Medical Center (PBMC), Waldo County General Hospital (WCGH) and our affiliated services with the leadership, purchasing power and strategic guidance to help us build the strong and vibrant healthcare system you have come to rely upon for your day-to-day healthcare needs. MaineHealth Shared Values In 2017, colleagues from across MaineHealth came together to create a set of system-wide values. Opioid Update In the winter of 2016, MaineHealth embarked upon a system-wide effort to address the opioid epidemic ravaging our communities. A group of 31 physicians and administrators from across our system spent seven months developing a multi-faceted approach to address prescribing, education and treatment. One outcome of these efforts was the development of the hub-and-spoke treatment model. This model uses regional hubs, staffed by psychiatrists and addiction experts, to provide screening and evidence-based Integrated Medication Assisted Treatment (IMAT) for patients with opioid use disorder in need of intensive treatment levels. IMAT combines medication and behavioral health treatment. The spokes are Patient Centered Medical Homes that provide IMAT in a primary-care setting for patients who have reached a particular level of stability.
In a collaborative effort between Maine Behavioral Healthcare, Mid Coast Hospital and Pen Bay Medical Center, EMBARK began seeing patients. EMBARK is a substance use treatment program that offers access to any level of care, using a combination of counseling and medication-assisted therapy to provide successful addiction treatment. Based on eligibility and clinical need, individuals can access three levels of care: acute treatment at the Pen Bay Medical Center PARC Unit, Intensive Outpatient and Medication Assisted Treatment at Mid Coast Hospital’s Addiction Resource Center (ARC), and intensive outpatient and IMAT treatment, as well as substance abuse counseling, at the Maine Behavioral Healthcare Rockland office. The EMBARK program is just the first step in the development of a comprehensive addiction treatment model for our communities. We are developing services at our primary care practices in Knox and Waldo counties. Coastal Healthcare Alliance, in partnership with Maine Behavioral Healthcare and MaineHealth, are committed to providing the highest quality addiction services for our local communities. 12
Waldo County General Hospital Emergency Department With plans nearing completion, we are closer to making the Waldo County General Hospital emergency department expansion a reality. With the goal of breaking ground in the spring of 2018, this expansion incorporates green design and operations in a facility with state-of-the-art patient spaces. Care, transport and discharge of patients will soon be even more convenient, safe and private. With special care areas created for psychiatric patients, patients and their families can rest assured knowing their unique healthcare needs will be met in the newly-renovated space. We are proud to welcome the future of emergency healthcare to the Midcoast.

Advances in Technology at Pen Bay Medical Center As the largest healthcare provider in the Midcoast, we know navigating the healthcare system and finding the right treatment, close to home, can be an emotional experience. With the financial support of the Pen Bay Waldo Healthcare Foundation, we can invest in state-of-the-art technology that allows patients to stay here, close to their family and friends, while seeking diagnosis and treatment.
In 2017, Pen Bay Medical Center invested in a fully automated, maximum speed, low-dose nuclear spect camera. Nuclear spect technology is used primarily to diagnose and track the progression of heart disease through blocked coronary arteries. The technology can also be used to detect bone disorders, gall bladder disease, intestinal bleeding and all types of cancer-related imaging. With the support of MaineHealth and the Pen Bay Waldo Healthcare Foundation, we are proud to be at the forefront of imaging; providing technology that makes the diagnosis of disease easier, faster, safer and more convenient for our patients.

Ann Hooper Center for Women’s Imaging at Waldo County General Hospital The opening of the Waldo County General Hospital Ann Hooper Center for Women’s Imaging marked a step forward in accessing an expansion of state-of-the-art, personalized care, close to home in the Midcoast. The state’s premier boutique imaging suite, the Ann Hooper Center for Women’s Imaging offers 2D/3D mammography, ultrasound, bone density and needle biopsies in a comfortable, private setting.
State-of-the-art 3D breast health technology is now available across the Midcoast at Pen Bay Medical Center and Waldo County General Hospital. This specialized care allows for better, earlier breast cancer detection and greater peace of mind. 5K Ready Suzanne Hall
After the long, chilly Maine winter, Suzanne Hall decided she needed to take charge of her health. The 57-year old mail carrier and mother of two grown children said “robust living” and several surgeries – including, knee, shoulder and carpal tunnel – had left her in a “non-athletic” state.

Reading the local papers, Suzanne came across “Get 5K Ready!”, an eight-week training program through the Journey to Health program at Pen Bay Medical Center and Waldo County General Hospital. Led by Rachael McCormick, a personal trainer and the regional manager of community health and wellness at PBMC and WCGH, the program is designed to prepare participants of
A few weeks into the program, Suzanne heard a story on Maine Public Radio about Kipchoge Keino, a Kenyan runner who triumphed over gall bladder disease to medal in the 1968 Olympics in Mexico. Suzanne said Keino became a hero for her and his story became a driving force for her to succeed. On June 3, Suzanne joined her teammates for the Coastal Mountains Land Trust 5K on the

“It was a terrific experience and one I’m completely grateful for.”

Despite feeling “sick with doubt,” she completed it, and ran the entire course. “Not walking, but running, and finishing it with people who supported you the whole journey, who listened to your fears and doubts, who suffered alongside, who showed up in the drizzly cold after a long day of work... It was a terrific experience and one I’m completely grateful for,” said Suzanne. Not long after that race, Suzanne joined a dear friend as she completed her first 5K and said she plans to run a few more. Every time she completes a race she sends $100 to the Kipkeino Foundation, in honor of the runner who inspired her to succeed.

Inspiring Others Orin Flagg
Last December, Orin Flagg began to experience shortness of breath with exercise. Due to his history of hypertension and diabetes, a stress test was done at Waldo County General Hospital. The stress test was abnormal and he was referred out for a heart catheterization. In late January 2017, a stent was placed in his heart which helped decrease his shortness of breath and increase blood supply to his heart. In early February, Orin’s doctor referred him to the WCGH cardiac rehabilitation department with the goal of helping him achieve optimal physical, psychological and social functioning through exercise and lifestyle changes.
Orin was part of a 12-week cardiac rehab program where he developed a long-term plan and began his road to recovery. Orin was impressed by the high quality, compassionate care shown by his healthcare team. This empowered him to take control of his health and make a commitment. During his rehab, Orin was a model patient and an ambassador of the program, motivating others with his dedication to wellness. The support he received from nurse director Mary Hanrahan, RN, CCRP and Susan Driskell, RN, BSN gave him the confidence to make changes. As a two-time cancer survivor, Orin is a fighter. At 75 years of age, he is now fully committed to his health and wellness. An Army veteran motivated by his family, Orin has a goal of living to 100 years + 2 days to experience the 100th anniversary of Pearl Harbor. Orin redoubled his efforts and is dedicated to a physician-approved, six day a week exercise plan.

“Orin’s enthusiasm and light-hearted nature had a beneficial effect on those exercising around him in Phase II Cardiac Rehab. This spilled over to his daily workouts at the YMCA where he mentors some of our former patients,” said Mary. “Every morning is a new day to wake up, get out of bed and work toward improving my health,” said Orin. “Each and every morning makes a difference.”

Passion for Providing Care Kurt McCollett
After starting and running his own roofing business for a few years, Kurt McCollett decided it wasn’t the right career choice for him. Kurt made a living, but something was missing. “I can make money doing anything and this particular career just wasn’t fulfilling or leaving me feeling like I was making a difference,” said Kurt. Setting out to find something new, Kurt came across the Maine State-approved CNA Apprentice Program run by Pen Bay Medical Center, in collaboration with the Mid-Coast School of Technology’s CNA Certification Program. The program piqued his interest and he decided to apply. Kurt was accepted in the inaugural session of the CNA Apprentice Program in March 2016 and completed the training in June 2016. The “earn while you learn” program pays students for their time and covers their tuition fees and books.
Upon completion, all apprentices are hired as CNAs at Quarry Hill or the Knox Center, and Kurt has been employed as a CNA at the Knox Center ever since. To date, 30 students have taken part and completed the CNA Apprentice Program. “My favorite aspect of the CNA program was the affiliation with PBMC. Knowing that I was working toward a job in a field that I already fell in love with was very gratifying,” said Kurt. His passion for the medical field, along with his positive experience in the CNA program, gave Kurt the drive to continue his nursing education. Kurt began taking classes toward a nursing major and was officially invited to join the University of Maine at Augusta’s Nursing Program in the fall of 2017. Once Kurt receives his Bachelor of Science in nursing, he intends to remain at the Knox Center.
“Nursing is some of the most rewarding and satisfying work an individual can perform,” said Kurt. “But it is also very challenging both physically and emotionally. I am honored and humbled to provide care for our residents with such a highly qualified and compassionate group of people.”
The CNA Apprentice Program is offered three times a year, in the fall, winter and spring based on availability of state funding. Operational Excellence Pen Bay Medical Center and Waldo County General Hospital are proud to be part of the MaineHealth Operational Excellence program that was honored with the 2017 Quality Improvement Leadership Award from Maine Quality Counts (QC), a nonprofit organization working to transform health and healthcare. The award recognizes an individual or organization that has introduced innovations, used creativity and embraced risk-taking in their leadership of demonstrably-successful quality improvement initiatives. The Operational Excellence program at MaineHealth brings leaders and team members together to engage in work focused on improving safety, quality and the patient experience. Beginning with Pen Bay Medical Center in 2013, Operational Excellence has rolled out at almost every organization across the MaineHealth system. One aspect of the Operational Excellence program is the “Gemba walk� where

leaders visit numerous departments, every day, to talk directly with teams about the work being done there. Department personnel collect data on the issues they wish to improve (referred to as Key Performance Indicators or KPIs) and share this information at the Gemba walk. PBMC celebrated completing more than 1,000 KPIs in 2017, while WCGH celebrated completing more than 500. KPIs range from safe sleep initiatives, timely patient communication and referral accuracy, to standardizing supply rooms and waiting area improvements. Working together to find solutions for process improvement is vital for our hospitals. PBMC and WCGH look forward to the continued growth of the program in the coming years. MaineHealth Cancer Care Network Cancer patients in the Midcoast now have access to world-class, coordinated cancer treatment thanks to an innovative collaboration between MaineHealth and its member hospitals. The recently launched MaineHealth Cancer Care Network, in affiliation with Boston’s Dana-Farber Cancer Institute, delivers comprehensive, patient-centered care with assistance from a $10 million grant from The Harold AlfondŽ Foundation. Most cancer patients receive a combination of therapies that can include surgery, chemotherapy and radiation therapy. These treatments are best performed by specialists, those with the highest level of training in their field. Not every hospital or private practice has the expertise or
technology to deliver all types of care but by working together, the MaineHealth Cancer Care Network provides comprehensive cancer treatment services to everyone in our communities. For patients and their families, this means less time and travel to obtain top cancer care. When patients access the MaineHealth Cancer Care Network for care, they access the services they need as close to home as possible. Patients can be referred to larger centers for advanced care, second opinions or clinical trials, and then return to their local communities for follow-up care. The network is committed to coordinating the patient’s journey as they access the best services for their individual needs. 28
Joint Ventures - LifeFlight of Maine Pen Bay Medical Center and Waldo County General Hospital are proud to support and partner with LifeFlight of Maine, a nonprofit air medical transport organization that provides patients with high-level critical care and lifesaving equipment by helicopter, airplane or ground ambulance. Being the second most rural state by population density in the United States, the services LifeFlight of Maine provides are critical to bringing the care of trauma and specialty centers directly to a patient’s side when they are far from a local hospital. Knox and Waldo counties utilize the services of LifeFlight of
Maine almost every other day as critical care nurses and paramedics answer calls for local residents and summer visitors. In 2016, 71 patients were transferred from PBMC and 68 were transferred from WCGH by LifeFlight helicopter, with others being transported by plane and ground ambulance. The scope and range of LifeFlight’s ability to deliver care for critically ill and injured patients is evolving continually as advances in time-critical treatment, changes in the healthcare system and Maine’s demographics continue to progress. Pen Bay Medical Center and Waldo County General Hospital are committed to a partnership and the mission of LifeFlight of Maine, and look forward to growing the relationship in years to come. 30
Healthy & Well Women’s Conference In May, Pen Bay Medical Center, Waldo County General Hospital and LincolnHealth hosted the inaugural Healthy & Well Women’s Conference, sponsored by the Pen Bay Waldo Healthcare Foundation at Point Lookout Resort & Conference Center in Northport. Studies have shown that women make approximately 80% of family healthcare choices—they choose their children’s doctors, make and take family members to appointments and ensure their family members and friends get the recommended care. Pen Bay Medical Center and Waldo County General Hospital are committed to keeping our patients, colleagues and communities healthy and well, and women are one of the most important groups to help achieve that goal. The free half-day conference was open to women of all ages and included breakout sessions focused on a variety of women’s health and wellness topics. The event provided more than 160 women with information, tools and resources to help them live healthy lives and be ambassadors of health for their families, friends and community. Junior Volunteer Program In 2017, Pen Bay Medical Center launched an eight-week Summer Junior Volunteer Program. Seven students from Camden Hills Regional High School, Medomak Valley High School and Oceanside High School with an interest in healthcare and a desire to help others were accepted into the program. Ranging in age from 16 to 18, the students were assigned shifts in radiology and the medical surgical in-patient unit at the hospital. At the completion of the program, all the students said they had an increased interest in pursuing a healthcare career. Pen Bay Medical Center is proud to offer this opportunity for local students to explore careers in the healthcare industry and give back to their community.

Pen Bay Waldo Healthcare Foundation With charitable donations from generous individuals, businesses and foundations, the Pen Bay Waldo Healthcare Foundation provides critical funding to keep quality care close to home. The foundation helps fund institutional priorities at Pen Bay Medical Center, Waldo County General Hospital, Waldo County Dental Care, Knox Center, Quarry Hill, Belfast Public Health Nursing Association and the Sussman House. All donations to the Pen Bay Waldo Healthcare Foundation, a 501(c)(3) organization, are tax-deductible to the fullest extent of federal and state law. The foundation works with community partners like Waldo County General Hospital Aid and the Rockland Women’s Golf Association, as well as our colleagues and local community members, to help raise funds for healthcare. These partners are working with the foundation to provide quality, patient-centered care to all those in the Midcoast. Our generous donors include grateful patients and families, individuals, board members, hospital and physician staff members, businesses and foundations. Donor Investments In Fiscal Year (FY) 2017 the foundation helped secure $814,606 in new gifts and pledges. Here is an overview of investments made possible with donor contributions in 2017:
Educational Opportunities Here at Pen Bay Medical Center and Waldo County General Hospital, healthcare teams participate in rigorous, on-going professional training to ensure they are up-to-date on best practices in patient care, healthcare trends and treatments. In 2017, donor funds helped to support many professional development opportunities, including: • Nurse Training Scholarships • Tufts University School of Medicine/Maine Track Program • Respecting Choices and the Serious Illness Conversation Project • Sexual Assault Nursing Examiner Training • Research Internship
Equipment & Technology In 2017, donor funds supported a range of major equipment purchases and critical upgrades, including: • Equipment to allow for use of nitrous oxide as an option for pain relief during labor • Nuclear SPECT camera to improve scanning for cardiology and oncology patients • •
Ultrasonic probe to allow for safe delivery of anesthetic pain relief Emergency department equipment including a wheelchair, isolation cart, sexual assault examination cart and OB/GYN stretchers

Oncology Patient Assistance Funds Each year, about 500 individuals are diagnosed with a new cancer at PBMC or WCGH. All too often, cancer treatment comes with significant out-of-pocket expenses, even for those with comprehensive health insurance. For many, fighting cancer is only half the battle—paying for it is the other half. Fortunately, generous community donations are available to provide financial relief to oncology patients at PBMC and WCGH, including: • PBMC Breast Cancer Classic Fund • PBMC Cancer Care Fund • WCGH Mammography Fund • WCGH Oncology Fund
Community Health and Wellness PBMC and WCGH are committed to increasing the number of people in our community who are healthy at every stage of life. To this end, we are focusing staff and resources on programs that support community health and educational initiatives to prevent disease before it happens. As residents gain knowledge, skills and attitudes to take better care of their health, we will see improvements in our community’s overall health and reduced use of higher-cost medical services. In 2017, donor funds have supported important community health initiatives, including: • Healthy & Well Women’s Conference • Improvements to the PBMC Community Wellness Trail • Increased colorectal cancer screening and education • Increased childhood immunization clinics
Access to Care Everyone should be able to see a healthcare provider when needed. As a nonprofit community based healthcare system, PBMC and WCGH are committed to providing access to care for all community members, regardless of their ability to pay. Each year, the Pen Bay Waldo Healthcare Foundation raises funds to support a range of safety net health care programs for uninsured, low-income community members, including: • CarePartners Program • Waldo County Dental Care • Charity Care
