The eventual introduction, in stages, of a NHS, is certain to absorb much of the media attention and bring long-awaited changes to local medical services. When appointed this summer, new Health Minister George Pamboridis made restarting NHS talks a priority. At the same time, the long-term campaign to attract medical tourism is continuing, based on a local league of experienced and highly qualified doctors, modern medical centres with ER facilities, cost-effective prices, convenient geographic location and fine weather. Recuperate in the sun Recuperating after surgery on a sandy beach, whilst sipping local drinks and receiving a full-body massage, is an attractive and affordable option in Cyprus. Services range from check-ups to cosmetic surgery, cardiac care, dental procedures and infertility treatment. With the exception of specialised procedures (which are only 5% of all treatments and are extremely rare) almost all medical treatments, including surgery, angioplasty and coronary stenting (primary PCI) can now be done in Cyprus. This means fewer patients now have to go abroad for treatment, while the creation of medical schools has given added momentum to an already well-established health sector. In recent years, Cyprus has also emerged as one of the Mediterranean’s leading spa destinations, thanks to the world-class facilities available at the island’s resorts. Here, pampering has been raised to an art form and spa services and rejuvenating treatments - either as part of a package or a la carte - add a wonderful dimension to any holiday. Current facilities offer spa experience that can last from a few hours at an urban day spa to a few weeks at a health resort, where a whole range of treatment, exercise and special diet are on the menu. Corporate pampering is certainly also an option

for all executives and a relaxing spa gift for an employee or client is a treat that is appreciated all the year round. Wellness holidays can take place at wonderfully preserved historical spaces, or at state-of-the-art wellbeing centres. The spas provide holistic wellness using the broadest range of popular techniques for the mind, body and soul including mineral spas, which make use of the natural qualities of thermal springs with healing properties, as well as hydrotherapy centres where the properties of water are used for therapeutic purposes. The island also offers thalassotherapy centres where use is made of the natural properties of sea water and its products including mud and seaweed.

Apollonios of Kition Apollonios of Kition was a physician (c. 60 BC) belonging to the Empiric school of thought. He studied medicine in Alexandria under the surgeon Zopyrus, but he lived in Kition - modern-day Larnaca. One theory states that he studied medicine in Kition, although it is not clear whether a medical school existed at the time. Apollonios was known all over the Ancient Greek world and was considered to be the most important of all Cypriot physicians. His medical opinions were considered valid and authentic according to Herodian. Important personalities including Strabon, Cicero and Dioscorides have also written positive comments on Apollonios. Although primarily a physician, Apollonios had an interest in surgery and trauma and wrote several books. One that survived is ‘Peri Arthron’ (On Joints).

This week, he outlined some of the current obstacles to the full implementation of Cyprus’ National Health Scheme (NHS) and also revealed what we can expect once it is introduced. Today, a scientifically and country technocratically tried-and-tested healthcare system has been designed, based on wider social cohesion and moulded on the Cyprus reality. The implementation of the NHS is an imperative social need. There are no obstacles to its implementation beyond indecisiveness and a fear of change. It was also included in the Troika’s Memorandum of Understanding (MoU) with the Republic of Cyprus. In terms of the HIO, we are prepared to provide any necessary support to the ministry in this effort, as well as to private hospitals in their effort to reform and prepare for functioning within the NHS environment. The reformation, modernisation and autonomy of

hospitals cannot take place from one day to the next. It is a process and the implementation of the NHS will have the role of a catalyst in its success. For this reason, the process of the autonomy and modernisation of hospitals must be simultaneous with the implementation of the NHS.

CW: Has there been progress on the issue of the involvement of private insurance companies? TA: As it has been designed, the NHS includes the management of a fund which will be used for the collection of contributions and compensation of providers through one organisation - the HIO. This is the plan which was unanimously voted in by the House of Representatives and confirmed after a long social debate. Beyond this, many studies by internationally acclaimed institutions, most recently the World Health Organisation (WHO), have shown this is the strongest and most efficient plan under conditions such as those we have in Cyprus. This, of course, does not mean that there will not be a place for private insurance company activity in the health sector. On the contrary, it is our conviction that insurance companies will be able to offer supplementary health insurance packages for services that will not be covered by the NHS. This is the case in most European countries with a national health system in place.

CW: What can you reveal in connection to amounts people can expect to pay once the NHS is in place? TA: Today, citizens of Cyprus spend 5-6% of their income on healthcare which is triple the amount paid by the average EU citizen. The cost for certain groups of the population such as the elderly, those with chronic illnesses, the unemployed and the poor is even greater. It would be good to mention that the NHS will not add, or indeed subtract, from today’s healthcare cost but will achieve its fairer distribution. Based on the study undertaken by Mercer in 2013, the contribution percentages related to the 10-year funding of the NHS is as follows:

CW: At what stage is the IT infrastructure that will play such an important role in the NHS? TA: The implementation of the IT system is of vital importance for the smooth functioning of the NHS in Cyprus. The system will link those entitled to services with those who will provide them. The HIO will allow the direct issuing of referrals, provider compensation, the issuing and filling of prescriptions, the briefing of entitled users, and more. The HIO is at the stage of issuing a call for the final tender for obtaining the IT system for the NHS as it has now completed dialogue with the chosen parties. We will await the tenders’ submission and then evaluate them, with a decision to be made within the first three months of 2016. CW: Is there a new timetable for the full implementation of the NHS? Are we under pressure from our obligations to the Troika or EU? TA: It is not our obligations to the EU or Troika that make the implementation of the NHS urgent but the need springing from the collapse of the current system. This has resulted in the people of Cyprus not

TA: It has been acknowledged by all that the current healthcare system is collapsing. Public hospitals are overburdened and understaffed with the result of long waiting lists. Private hospitals are underperforming, with the result of their having to overcharge to remain viable. People are then trapped between high costs and long waiting lists. It is therefore, universally acknowledged that the implementation of the NHS, as it has been planned and agreed on, is the only way out and the only solution to the dead-end in the health sector. The health of our people is not and cannot become a commercial commodity. It is a social and political right that can only be realised with the implementation of the NHS.

rom enhancing quality of life, to helping protect against several health consequences linked to unaddressed hearing loss, the case for early treatment is strong. Considering that we ‘hear’ with our brain, when we have a hearing loss the connections in the brain that respond to sound become reorganised. Hearing aids provide the sound stimulation needed for the brain to restore the normal organisation of connections to its “sound centre” so it can react to and process the “missing” sounds. For many years, experts have known the positive impact that addressing hearing loss has on quality of life. People with hearing loss who use hearing aids see an improvement in their ability to hear in many settings; and many see an improvement in their relationships at home and at work, in their social lives, and in their ability to communicate effectively in most situations. Many even say they feel better about themselves and life overall. Moreover, researchers are discovering a significant link between hearing loss and other health issues, such as cognition, dementia, depression, falling, hospitalisation, mortality, and overall physical and mental health. Then latest findings support early treatment of hearing loss with indications from several health areas. Cognition: Unaddressed hearing loss not only affects the listener’s ability to “hear” the sound accurately, but it also affects higher-level cognitive functioning. Specifically, it interferes with the listener’s ability to accurately process the auditory information. For instance, older adults with mild-to-moderate hearing loss performed poorer on cognitive tests than those of the same age who had good hearing. Researchers state that: “The sharpness of an individual’s hearing has cascading consequences for various aspects of cognitive function. Even if you have just a mild hearing loss that is not being treated, cognitive load increases significantly. You have to put in so much effort just to perceive and understand what is being said that you divert resources away from storing what you have heard into your memory.” Risk of dementia: Seniors with hearing loss are significantly more likely to develop dementia over time than those who retain their hearing. Older adults with hearing loss are more likely to develop problems thinking and remember-

ing than older adults whose hearing is normal. Brain shrinkage: Although the brain becomes smaller with age, the shrinkage seems to be fast-tracked in older adults with hearing loss. Use of MRI to look at the effect that hearing loss has on both brain activity and structure revealed that people with poorer hearing had less grey matter in the auditory cortex, a region of the brain that is necessary to support speech comprehension. Researchers hypothesise that when the sensory stimulation is reduced due to hearing loss, corresponding areas of the brain reorganise their activity as a result. Risk of falling: People in middle age (40-69) with even just mild hearing loss were nearly three times more likely to have a history of falling. The intensive listening effort demanded by unaddressed hearing loss may take cognitive resources away from what is needed for balance and gait, experts have suggested. Increased hospitalisations: Older adults with hearing loss were 32% more likely to have been admitted to a hospital than their peers with normal hearing. Older adults with hearing loss were 36% more likely to have prolonged stretches of illness or injury (lasting more than 10 days). Mortality: Hearing loss is tied to greater risk of dying for older men from any cause and particularly from cardiovascular causes. Men and women who used hearing aids, although they were older and had more severe hearing loss, had significantly lower mortality risk compared with hearing impaired men and women who did not use hearing aids. Depression: Several studies have found a link between depression and hearing loss. Older adults with hearing loss were 57% more likely to have deep episodes of stress, depression or bad mood than their peers with normal hearing. Hearing loss is associated with an increased risk of depression in adults of

all ages, but is most pronounced in 18 to 69-year-olds. Working adults – aged 35 to 55 - with untreated mild to moderate age-related hearing loss were more prone to depression, anxiety, and interpersonal conflict than those with no hearing problems. Treating hearing loss early offers substantial improvement in quality of life and long-term prognosis. The healthcare providers under the NHS meet minimum requirements related to qualifications, infrastructure and equipment. How are health services accessed? The beneficiaries of the NHS gain access to health services once they have registered with the General Practitioner (GP) or paediatrician of their choice. Beneficiaries navigate the health system through referrals from their GP. The referral defines the specialty of the provider and is valid for a specified period and for a specified number of visits. The selection of a provider is always made by the patient. With the referral, the patient may undertake repeat visits to a specific specialist or obtain a second medical opinion from another specialist. It can also affect the family and society, causing tension and disruption, social isolation and a breakdown in social communication and interaction. Early identification and rehabilitation is essential to try to avoid these various potential complications by maximising a child’s ability to hear, and facilitating normal speech and language development and communication.

Adult and Paediatric Hearing Solutions Today, computer-based digital technology and scientific methods are used for diagnosing hearing loss and programming hearing aid systems. Solutions for people who suffer from hearing loss or tinnitus are now discreet and sophisticated with unrivalled performance. They do not whistle, unless poorly fitted, and they can provide the user with effortless hearing. The most expensive hearing aid is not always the best for everyone. There are many affordable suitable models to choose from, ranging from discrete, custommade digital aids to sophisticated behind-the-ear hearing aids. Completely-in-the-canal These are also custom designed. They fit deep into the ear canal so they are not easily visible. They may also reduce the problem of wind noise. Additionally, due to exposure to moisture and ear wax, CIC need good care and cleaning.

Behind-the-ear (BTE) These are now much smaller and neater than in the past and fit comfortably over the ear with a slim thin tip or a custom earmould worn in the ear. Because the components are housed outside of the ear, BTE aids tend to be the most durable, require less repairs and last longer. They can be worn easily, even with spectacles. Open ear These are housed in a tiny case that fits over the ear coupled to an ultra-thin tube that fits in the ear. These are appropriate for people with normal to nearnormal hearing in the low pitches (bass tones), and a mild to moderately severe loss in the high pitches (high tones). This is normally a result of noise-induced hearing loss (e.g. ex-military, factory workers, pilots) or ototoxicity. They are very comfortable, cosmetically appealing and reduce the occlusion effect some people may get by hearing their own voice too loud. In-the-canal (ITC) and In-the-ear (ITE) ITCs are custom designed, small in size, and fit deeper into the ear canal than the in-the-ear hearing aids. They fit comfortably and securely in the ear. CROS/BI-CROS hearing aid This is the best way to fit mono-aural hearing loss. This allows the person to pick up sound on the poor ear side and hear it on the better side making the person hear from both sides and localise sound easily! Invisible-in-the-canal (IIC) These are the latest technology in size! They are smaller than the CIC as they are deep in the canal making it difficult for someone to detect them. This type is heavily based on the hearing loss degree and the size of the ear canal, as it does not fit in all ears.

Robotic surgery Operating without a scalpel in Cyprus By Kika Kasinidou

R

obotic surgery, a specialised medical service, is a new concept for Cyprus, offered only by a handful of medical centres in the private sector. The first operations began in 2008 on patients with urological ailments and some years later extended to gynaecological cases while they now cover all types of general surgery. Robotic surgery is the evolution of laparoscopic Less traumatic and endoscopic surgery. These are inserted into the patient’s body through tiny openings in the skin, thus avoiding the need for large and painful incisions. During robotic surgery, as it is carried out today, the surgeon sits in front of a surgical robotic control panel on a computer. The surgeon can see 3D and magnified images of the site of the operation onto a large screen. The surgeon then carries out the operation with the use of two specialised controls that look somewhat like videogame joysticks. The movements carried out by the surgeon with the help of these controls are digitally transferred to a state-of-the-

art computer, processing and transferring with unprecedented accuracy, the relevant movements onto the operation site where they are carried out by the articulate hands of the robot. These movements are entirely controlled by the surgeon and if the surgeon as much as turns his head away from the screen, the movement stops. The robot cannot move on its own or be programmed but acts only according to the surgeon’s movements. Surgeons are specially trained to be able to use surgical robots. Significant advantages The advantages of robotic surgery are significant and include the follow-

ing: Least possible surgical trauma to the tissue, better aesthetic result and small scars, and quicker recovery and patient mobility. They also include minimal blood loss, fewer blood transfusions, reduction in post-operative pain, quick exit from the hospital, lower cost of time spent in hospital, and a faster return to everyday activities and work. Other advantages include the ability to magnify an image 10 to 15 times, better lighting availability, minimal postoperative complications connected to trauma such as infection, decay, hernias, chronic pain, scar tissue deformation. This type of surgery also has less impact on the heart and lungs and can

be used on patients with a difficult medical history. Also, 3D visuals, colour images and the ability to magnify an image 10 to 15 times results in complete control of the situation. Because the surgeon’s movements on the console are faithfully replicated by the mechanical arms, natural hand trembling is eliminated. This ensures the best possible accuracy. The laparoscopic tools making up the robotic arms can not only carry out every movement of the human hand (seven degrees of freedom of movement), but also rotate almost 360 degrees within the surgery site, surpassing even the capabilities of the human hand.

Private insurance has been developed to provide a series of insurance products aiming to cover the health protection needs of a large proportion of the local population within the conditions of the specific market. The high aims set on a financial and cultural level have been in place for some time and are very clear: Comprehensive insurance coverage of the population through the system’s adequate and permanent social funding. In general, the theory of funding a social-insurance system, which in this case is connected to the health of the population, requires social, macroeconomic, financial goals and coordinates that fall beyond the horizon of the current financial crisis. Some choices, out of necessity, can be characterised as political and mainly keeping in mind the priorities created by the social and financial implications of the insurance system’s function. Whatever the choices made by the political leadership, the main elements in creating Cyprus’ new health system must create the vital conditions for the viability of health insurance system. These are financial balance, flexible coverage of the needs of the population, constant encouragement for the improvement of the quality of health services provided, policies for limiting and/or wiping out improper use, and an adequate level of satisfaction on the part of the insured population. Under the weight of the responsibility of ensuring the above and the effective operation of the new framework set out by Cyprus’ Health Insurance Organisation, important decisions have to be made on

the following: - The structure and character of the insurance system. Here, the basic question is whether a cumulative or multi-insurance health system is introduced. - The types and extent of insurance risk that will be covered by the public health system. - The means and timeframe for implementing all the necessary institutional interventions to the highest possible detail. - The pre-required actions on a level of infrastructure, know-how, education and training, organisational adjustment. - The level and form of action of all those making up the health system of Cyprus, including the private insurance companies. In parallel to decisions being taken on the above, the Health Ministry has implemented a series of interventions with which it hopes to transform the country’s health sector and put in place new models of healthcare provision. These include the introduction of the Diagnosisrelated group (DRG) system to classifying hospital cases, the planning of ‘Gatekeeping’ in primary care reducing patient referrals to specialists, formalising medical protocols etc. The size and depth of all these interventions and changes signal Cyprus’ health sector reform. They also set out the new role which the private insurance sector will play in Cyprus. New conditions Private insurance companies will have to adjust their products and services to fit new needs including possible new categories of clients. Pricing will also have to be adjusted as well as the accessibility of services such as being able to get insurance coverage online. Insurance companies may also be able to combine coverage with that provided by the public sector to the benefit of all involved. Schemes tailored for chronic and serious ailments can be planned, including coverage of disease management programmes. Private insurance companies can also play an important role in transferring know-how or even providing services to the public system, something which has been done for some years now in Greece. iannoukas Medical Laboratories (YML) was established in 1975 by Dr Nicos Yiannouka and over the next 40 years, it has managed to become one of the island’s largest accredited laboratories. Our laboratories are comprised of our main premises in the centre of Nicosia, seven sampling centres and a nationwide number of mobile units, making us the only laboratory able to provide services throughout all of Cyprus. Today, the laboratory is managed by Mr Kyriacos Yiannouka (FIBMS) and Dr Harris Stefanou, together with another 15 university graduates.
