PIEDMONT MEDICAL CENTER EARNS BABY-FRIENDLY DESIGNATION by Baby-Friendly USA, Inc.

I

n September 2015, Piedmont Medical Center became the third Tenet hospital to receive international recognition as a Baby-Friendly USA designated birth facility. The BabyFriendly Hospital Initiative was launched by the World Health Organization and the United Nations Children’s Fund in 1991. Its goal is to encourage hospitals and birthing centers to provide excellent care in the areas of infant feeding as well as mother and baby bonding. Rachel Miller, Labor and Delivery Nurse and Certified Lactation Consultant, championed the project. “It became more obvious to us that the Baby-Friendly designation was based on what research shows leads to the best health outcomes,” says Miller. “This designation for Piedmont Medical Center demonstrates that we employ the best evidence-based practices in infant care.” In order to receive the designation, facilities implement “Ten Steps to Successful Breastfeeding.” This includes having a written breastfeeding policy, and helping mothers initiate breastfeeding within an hour of birth. As a result, PMC has changed how infants and mothers are cared for right after a baby is born. The first change is that once a baby is delivered, it goes “skin to skin” with mom, and remains there through the first feeding. “This means we don’t weigh them first, or bathe them first,” says Miller. Instead, the baby is laid on a warm blanket on the mom’s chest, where it is dried with a towel. Then the baby is put skin to skin with mom, and a warm blanket is laid over them both. Research shows that being cradled skin to skin is the most effective way to stabilize a baby’s temperature. The initial assessments of the baby’s movements, color, temperature, and heart rate are performed while the baby

is skin to skin. The second big environmental change is the practice of “rooming in,” which means that the baby stays in the room 24/7. “This is wonderful for families,” says Miller, “because they get to see everything the doctors do, and they can ask questions while the physicians are treating their babies.”

A message from Bill Masterton Chief Executive Officer

L

ooking back at 2015, it was a phenomenal year for Piedmont Medical Center. We increased our ability to serve patients by opening a neurosurgical ICU and growing that team. We expanded our open heart program and worked to open a new Imaging Center in York. We also began renovations of our patient rooms in the main tower of the hospital and saw major growth in our newly renovated Emergency Department! Since I arrived here in 2012, Piedmont has doubled the number of Urgent Care Centers and Imaging Centers serving this community with BEFORE four locations of each now. We have grown our physician network from 23 physicians to over 70 physicians in our local market to serve you and your family. AFTER We’ve also grown from a 20,000 square foot Emergency Department to over 40,000 square feet! While the numbers and statistics are good to share, one of the most important indicators to me regarding the state of the hospital is shown by looking at the physicians who are making the decision to join our network. Just this past year, we welcomed Dr. Brandon Sick, Dr. Michael Curran, Dr. James Jordan, Dr. Kevin Zacour and Dr. Melodie Harrison to our primary care network. At the beginning of 2016, we’ll see Dr. George Barron relocate to Piedmont Family Practice at Rock Hill, Dr. Kristine McGruder join Family Medicine Associates in York and the physicians and staff at Rock Hill GYN/OB will also join the PMC Physician Network! We look forward to having Drs. Joe Robinson, Norman Taylor, Ansley Hilton, Barbie Rowland, Craig McClelland, Kelly Wormer and Tyler Pierce onboard! I’m excited about the continued momentum we’ve gained throughout last year and I am very optimistic that 2016 will prove to be just as monumental.

2

We are proud to enter the New Year with our fifth consecutive grade of “A” on the Hospital Safety Scorecard from the Leapfrog Group. Similar to how we 5 TIMES IN A ROW! were all graded in school, this award indicates to our staff, patients, and community our outstanding commitment to patient safety.

3NICU

I would like to congratulate all members of Level our maternal-child team on their newest distinction as a Level III NICU (neonatal intensive-care unit). This recognition allows PMC to care for babies born at less than 32 weeks gestation as well as babies born with critical illness, at all gestational ages. Along with becoming a Level III NICU facility, PMC has also earned the coveted Baby-Friendly facility designation. This title is the direct result of the compassionate care and dedication this department delivers to every patient and family that enters its doors.

The end of 2015 presented a number of opportunities for our PMC team to connect with those in our community. We hosted our sixth blood drive of the year benefitting the Community Blood Center of the Carolinas. As the Presenting Sponsor of Rock Hill’s 10th annual Christmasville celebration, we were pleased to see many new faces enjoying downtown Rock Hill! We also had more than 1600 participants at the eighth annual Reindeer Romp 5K benefitting Girls on the Run of the TriCounty. PMC team members helped spread warmth and happiness during the holiday season through coat collections with the City of Rock Hill, toy collections with WRHI through Toys for Happiness, and a record-setting Angel Tree adoption program with Pilgrim’s Inn providing gifts to children and families in our community. As we look forward to all that 2016 has in store, PMC will continue our commitment to our mission to provide exceptional healthcare to every person we have the privilege to serve!

PHYSICIAN FEEDBACK leads to capital improvements for hospital rooms.

E

very year, physicians at Piedmont Medical Center are asked for their input on decisions regarding capital investments in the hospital. At the 2014 Physician Business Planning Retreat, they suggested that we renovate our hospital patient rooms in our main tower. As expressed by Dr. Ansley Hilton, the doctors realized that hospital room aesthetics are something that patients care about. The administrative team set about the task objectively. Each room on the second and third floors in the 33-year-old main tower underwent an audit. The condition of each room was graded against 12 criteria, after which each room was assigned a score. Based on the scores, it was decided that the Progressive Care Unit would be renovated first. The next step was to survey the nursing staff about what was most important to them. They expressed a desire for more convenient workstations for themselves, and rooms with attractive interior design for their patients.

In June 2015, the renovations in two sample rooms were completed. Nurses were invited to rank various aspects of each room. In addition to an updated look, dry-erase boards will be replaced by new digital signage, and a bedside computer workstation will be installed in each room. We’re excited to report that renovations of 26 Progressive Care Unit patient rooms began in October 2015 and are well underway.

Piedmont Medical Center Earns Baby-Friendly Designation!

The designation process was a group effort by the management team and the nursing staff. It takes most facilities three to five years to achieve the Baby-Friendly designation. Piedmont earned it in three years. “Choosing your healthcare provider is important, and it’s important for you to know what you are looking for,” says Miller. “It’s important to show our patients that we have chosen to go through this designation process. It is not just us saying we are utilizing these techniques, but it is

recognition by a third party that we are doing it with excellence and we are all very proud of that.”

TWO PATIENTS, one satisfied family.

"It began when I started having contractions just shy of 33 weeks with our second son. The staff in labor & delivery was amazing! They took care of me, and stopped the contractions that night. I returned a few days later to give birth to our sweet boy. On the day I was to be discharged, my husband was in severe pain from a kidney stone. My nurse set up a transport to the ER – no questions asked! After his discharge later that day, we left our sweet boy in the NICU to head home. Fast-forward several days and my husband is still in pain from his kidney stone that hasn't passed. We called his doctor, and they decide that surgery to remove the stone is best. The doctor set everything up for a quick SATURDAY NIGHT surgery to remove the stone. The NICU staff was amazing during our son's 2-week stay. They knew of my husband's kidney stone, and asked EVERY TIME we came to visit our son how he was feeling. They went out of their way to make us feel important! Dr. Magura performed the surgery on my husband and was very understanding of our unique situation. He took the time to talk with us and explain what to expect. We have nothing but love and appreciation for these wonderful people that helped us both during a very scary – but joyful – time in our lives. Thank you to the NICU staff, Labor & Delivery staff, and Dr. Magura's surgical team on Oct. 17, 2015."

s his tenure serving as Chief of Staff approaches its end, Dr. Sushil K. Singhi is excited about the many initiatives taking place at Piedmont Medical Center. “The most important thing for the hospital is the increased bonding between the medical staff and the administration,” he says. “It improves how our patients feel about the hospital, and how our staff feels about the hospital.” As Dr. Singhi puts it...

“ We are big enough to serve you, and small enough to know you.”

F

or the past two years, Nicole McMillion has been knocking down walls, metaphorically and literally, as the Director for the Center for Psychiatry at Piedmont Medical Center. Not only has she transformed the perception of mental illness for those who interact with her department, she has led a makeover of the adult inpatient facility. The makeover included 17 beds within the unit and five pods in the new Emergency Department, all designed to increase safety for patients with mental health issues. “It is good for the community to know that we are here to keep you safe no matter what your needs are,” stated Nicole. “At the hospital, we want to make sure we are meeting all patient needs, and not just the obvious medical needs.” Located in the main tower of Piedmont Medical Center, the Center for Psychiatry is the only inpatient psychiatric unit serving Rock Hill, Chester, and Lancaster county patients. Over the course of one month, they typically assess over 1600 patients.

What many may not know is that 1 in 4 people are affected by mental illness, though not all will need inpatient care. For those who are admitted, Nicole and her team have created a comfortable and colorful environment. A new 120-page patient journal created by the team allows staff members and patients to work together to create daily goals, a safety plan and a discharge plan. Patients spend time in group therapy, doing recreational activities and exercising. One wall of the Center for Psychiatry is even decorated with patient artwork completed while in group therapy. The staff ’s main goal is to give patients the tools to live productively when they leave PMC. This is done by helping patients learn their unique triggers and providing them with coping mechanisms they can use outside the hospital. We want to thank this dedicated department for the work they do to serve our community.

WHAT IS A CERTIFICATE OF NEED (CON)?

Y

ou’ve heard about Piedmont’s pursuit of a Certificate of Need (CON), as it relates to efforts to build a new hospital in Fort Mill. Many have asked what exactly a CON is and what it allows us to do. CON laws require government permission before a healthcare facility can offer new services, expand a current facility, or purchase particular pieces of equipment. These laws were originally enacted to help control medical costs, by making sure that no single community overinvested in medical services to the detriment of another. In South Carolina, the Department of Health and Environmental Control (DHEC) oversees the CON application process. In 2014, The Administrative Law Court upheld DHEC's decision to award

6

the CON to Piedmont Medical Center to build its new 100-bed hospital in Fort Mill, a decision that is currently under appeal by Carolinas HealthCare System. Two additional Piedmont Medical Center CON applications were approved in 2015. The first allows us to convert five enhanced perinatal bassinets in our neonatal internal care unit (NICU) from Level IIE to Level III. The second allowed us to open our highly anticipated York Imaging Center. We look forward to offering these new services to our patients. And, we will continue to provide updates on the status of the Fort Mill Medical Center CON, as they become available.

hen Deb Myers leads seminars on bariatric surgery, it’s personal. The Bariatric Program Coordinator at Piedmont Medical Center is a Registered Nurse who had gastric bypass surgery in 2007. “I had struggled with weight my entire life,” says Myers. She was on medication for high blood pressure and high cholesterol. She tried diets and various lifestyle changes, but her weight continued to affect her health. At the time of her surgery, PMC provided two options: gastric bypass or adjustable gastric banding. Studies show that after five years, patients who underwent either surgery had similar results. Myers was interested in the fastest path toward her goal of losing 100 pounds. “I grinned the whole way to the hospital,” she says. “It’s a big decision, because it’s a huge lifestyle change, but by the time I made the decision, I was ready to go.” Dr. Alex Espinal performed the surgery, with assistance from Dr. Kamran Borhanian. Myers achieved her goal weight in 18 months. Two years after the surgery, her laboratory results greatly improved, and Myers describes her blood pressure as “perfect.” For Myers, the most surprising after effect was how people in the community related to her.

“There’s still a bias against people who are overweight,” says Myers. “To me, it’s one of the last acceptable biases in this country. There’s the perception that if you are overweight, you are lazy or you just eat the wrong things, but evidence shows there are a lot of genetics involved.”

Deb Myers, rn, cbn

As the coordinator of the PMC Bariatric Program, Myers feels she can make a difference. “People really relate to me when they find out I have been in their shoes, and that I do understand,” she says that understanding and compassion increased her confidence. Prior to her surgery, Myers had a fear of public speaking. Now, she regularly conducts seminars for people who are considering bariatric surgery and hosts health and wellness events throughout the community. Myers recently earned the special designation of Certified Bariatric Nurse from the American Society for Metabolic and Bariatric Surgery. When she talks to potential surgical candidates, she tells them that changing their lifestyle is a process. It’s one she’s grateful to have gone through. “This is absolutely the best thing I’ve ever done for myself,” she says. “My only regret is that I didn’t do it sooner.”

e are thrilled to share that our first Nurse Betty Scholarship Award Winner Grea Boyd is a hard-working member of our Emergency Department. She used the $20,000 scholarship to attend Clemson University where she graduated Summa Cum Laude in May 2015. “The scholarship gave me the opportunity to pursue a degree from the school of my dreams,” said Grea. “Not only did Piedmont support me financially, but I was in constant contact with representatives from the hospital making sure I had all the tools to succeed in college.”

Grea says she became a nurse because she wanted to make a difference in the lives of others. “I have learned that just a smile, an encouraging word or holding a hand is sometimes the best medicine we can give.” When asked what advice she would give an aspiring nurse, Grea shared that you just have to ‘stick with it.’ “Nursing school is difficult and can be overwhelming at times, but it is worth it in the end when you can see the impact that you make daily in the lives of your patients!” Thank you for your commitment to excellence, Grea! We congratulate you on your success and look forward to seeing you grow at PMC!

here have been great advances in medicine since the first abdominal hysterectomy was performed in 1853. Today, there are many less invasive options than open incision, including the Single Incision Laparoscopic Surgery (SILS™) hysterectomy. Dr. R. Craig McClelland was the first Charlotte area doctor to perform a SILS™ hysterectomy. To date, he has performed the procedure more than 600 times. The surgery is highly reproducible, which means the same technique can be performed the same way to achieve the same results. The SILS™ hysterectomy involves one incision that is hidden in the belly button, and barely visible after recovery. It uses one device; a flexible laparoscope with a camera built into it. Depending on the pathology, the surgery takes about 90 minutes. Patients typically stay in the hospital for one night, and are then advised to rest for at least 3 or 4 days. Most patients can drive within the first week, and are back to work within two weeks. Because there is only one incision, patients are more likely to experience less pain and have a diminished chance of infections, bleeding or hernias. Recent advances in the field include a second generation high definition flexible laparoscope that is only 5mm.

SILS™ PROCEDURE offers minimally invasive options for hysterectomy. Dr. McClelland’s goal is to educate as many patients as he can about SILS™ and to train more doctors to be able to perform the procedure. “Piedmont Medical Center has been very accommodating to us by getting this equipment,” says McClelland. From 2009 to 2012, a monthly course was held at the Women’s Center, during which almost 200 surgeons from around the country were trained on the procedure.

He advises women who are considering surgery to seek more than one medical opinion, and to ask friends who have had similar issues for their advice. “I’m always available for a second opinion, and happy to talk to anyone, any time,” says Dr. McClelland.
