SERVE SAFETY PLAN 2021
During the COVID-19 Pandemic Updated February 26, 2021
STANDARD PRACTICE Ministry Partners must practice physical distancing (maintaining 6 ft distance from others) and wear a mask/face covering during interactions on campus. Our goal is to collaboratively ensure a safe working environment, while abiding by government bylaws due to COVID-19 restrictions.
Here are some things we are implementing to help keep our facility safe: More frequent sanitation of commonly touched surfaces or areas (doors, handles, chairs, equipment) Cleaning washroom facilities regularly Easy access to hand sanitizer around the workplace
Limited access to the elevator Limited access to main Kitchen Restricting Access to the Electric room and Cleaning supplies to custodial staff only
More frequent communications on our ONLINE events, new policies and requirements for your health and safety, and measures we are taking to support the community. Posting signage around the building that covers physical distancing guidelines & building closure notices (visit PORTICO.cc/notices for updates) Communicating building closures on: PORTICO.cc/notices
Communicating health measures & guidelines on PORTICO.cc/COVID-19 Requiring all Visitors to complete a Contact Tracing Form to provide details in case follow-up is needed.

PRESCREENING QUESTIONS Ministry Partners are required to complete the following selfassessment before entering the PORTICO campus:
Do you have any of the following new or worsening symptoms or signs of the COVID-19 coronavirus? Fever or chills Cough or barking cough (croup) Shortness of breath Sore throat Difficulty swallowing Decrease or loss of smell or taste Pink eye Runny or stuffy/congested nose Headache that's unusual or long lasting Digestive issues like nausea/vomiting, diarrhea, stomach pain Muscle aches that are unusual or long lasting Extreme tiredness that is unusual Falling down often Have you travelled outside of Canada or had close contact with anyone who has travelled outside of Canada in the past 14 days? If yes, please do not enter the workplace until you have completed the required quarantine period, or have been cleared by a medical professional to return to the workplace.

PRESCREENING QUESTIONS (CONT.) Has a doctor, health care provider, or public health unit told you that you should currently be isolating (staying at home)? If yes, please stay home to self-isolate and contact your healthcare provider or Telehealth Ontario (1-866-797-0000) to find out if you need a COVID-19 test. In the last 14 days, have you been as a "close contact" of someone who currently has COVID-19? If yes, please stay home to self-isolate and contact your healthcare provider or Telehealth Ontario (1-866-797-0000) to find out if you need a COVID-19 test. In the last 14 days, have you received a COVID Alert exposure notification on your cell? If yes, please get tested for COVID-19, and stay home until you have received a negative test result.
If you answered NO to all the above questions, you can enter the PORTICO campus building. If you answered YES to any of the above questions, you should not enter the PORTICO campus building. Please inform your Ministry Lead, stay home to self-isolate, and contact your health care provider or Telehealth Ontario (1-866797-0000) to find out if you need a COVID-19 test. If any of the answers change during the time you are serving, inform your Ministry Lead and go home to self-isolate immediately, and contact your health care provider or Telehealth Ontario, as outlined above.

CONTACT TRACING As outlined by Ontario's Office of the Chief Medical Office of Health in the COVID-19 Screening Tool for Business and Organizations (Screening Workers and Screening Patrons): Businesses and organizations must maintain a record of the date/time that workers were in the workplace and their contact information. This information may be requested by public health for contact tracing. These records should be maintained for a period of at least a month. These records will comply with any applicable retention and privacy requirements and may be requested by the local public health unit to support case and contact tracing in the event of an outbreak.
Self-Assessment: All Ministry Partners must register for an event and answer NO to all of the Prescreening questions before arriving to serve. Ministry Partners will be scheduled on PCO Services, and will be checkedin upon arrival, to maintain record whenever they serve onsite. Upon arrival to serve, Ministry Partners must confirm at the MP Check-In Desk that they have not answered YES to any of the prescreening questions.
