Safety and the pace of change Since Mark Carne took over as Network Rail chief executive in early 2014, safety has been at the core of every speech he has given and every press statement issued. Safety going hand in hand with performance is the mantra he’s sought to instil. Carne is attending the Rail Safety Summit for the first time to deliver the keynote address. Speaking at last month’s Rail Exec Club event, Mark said he felt he had made it clear from the start of his tenure that safety was what he stands for.

‘You go into a great factory and do you find bits and pieces of scrap left all over the floor? No, of course you don’t. They’re spotlessly clean and they’re organised and they’re planned like military operations and that’s the mindset we need to have in the railway.

He said, ‘I deeply believe that it is our obligation as leaders to create a culture which enables people to feel that they are obliged to work safely and feel they can work safely, that they’re not under pressure to do things in an unsafe way that puts them or passengers’ safety at risk in any way.’

‘You keep your place clean and tidy we organise our work properly, we plan it properly, we execute it brilliantly and that will drive efficiency and improvements in the way we work.’

This year’s Rail Safety Summit will begin by looking at the pace of change within the industry around safety, exploring Network Rail’s reforms around safe work planning and the way in which projects like HS2 are having to consider safety at the design stage. Additionally, the conference will look at the importance of leadership and how things like technology and the law are affecting safe working practices. Prior to joining Network Rail, Mark was executive vice president for Royal Dutch Shell plc in the Middle East and North Africa. Compared to rail, oil and gas has an enviable safety performance record. ‘It’s not altruistic entirely, it’s not just about because I don’t want people to hurt themselves, it’s because I want to run a great company and I know that this is one of the key tools we can use to run a great company,’ said Mark.

In the past couple of years, there have been a number of train accidents around the world which have had a devastating impact on human life. In Europe, Spain, France and Switzerland have all experienced fatal crashes which have claimed the lives of both passengers and railway staff. It is now eight years since the last passenger or workforce fatality in the UK which was caused by a train accident. It’s one area in which the industry in this country takes some sense of pride. Besides the traditional areas of concern such as level crossings and suicides, there are now new focusses - occupational health is one. Another area of concern is road casualties as highlighted by the RSSB’s 2014 safety performance figures.

Publishing the figures, George Bearfield, director of system safety at RSSB, said, ‘Road transport plays a crucial role in keeping the railway running, by enabling round-the-clock maintenance and development. However, this brings with it a risk to rail staff and others, highlighted by the loss of life to an infrastructure worker who was moving materials from one railway worksite to another. ‘A cross-industry project is underway to highlight the wider risks associated with driving for work and improve overall understanding and management. ‘Whilst there can never be zero risk, there is no room for complacency and active prevention, monitoring, and learning all play a role.’ Mark Carne Chief Executive, Network Rail

Rail Systems Director, High Speed Two (HS2) Ltd Whilst Professor Andrew McNaughton, Technical Director of HS2 acts as Principal Witness for the Hybrid Bill over the coming years, Giles has responsibility for the day to day leadership and running of the HS2 Technical Directorate. The Technical Directorate comprises operational, technical and environmental specialists specifying and assuring the HS2 design process. He is a Chartered Engineer. Giles started his railway engineering career commissioning Class 91 locomotives for the East Coast Mainline in 1988 after completing a degree in Electronic Systems and Control Engineering. He then worked in various design and project engineering capacities on light rail and underground projects in Manchester and London. After completing a Masters degree in Engineering Project Management in 1995 he worked as an engineering manager in the rolling stock and railway systems supply sector in America, Poland and France. In 2002, he returned to the UK to work for the Government as their European Technical Director. In this capacity he had responsibility for the UK’s European railway technical specifications for interoperability and implementation policies. He represented the UK in Europe on rail technical and safety matters and was the UK Board Member of the European Rail Agency. In 2007, Giles joined Network Rail where he developed the Train to Infrastructure Interface Specifications for Thameslink and Crossrail rolling stock procurement programmes. Before joining High Speed 2 in July 2014, Giles headed up Network Rail’s Systems Integration team working for the Department for Transport – technically integrating between the Thameslink rolling stock, infrastructure and franchise replacement programmes.

PLANNING AND DELIVERING Network Rail is in the midst of rolling out its newly devised planning process, Planning and Delivering Safe Work (PDSW). Much more than just a handful of tweaks, PDSW is a wholesale reform of how infrastructure projects are planned and delivered safely and, ultimately, it makes clear who is responsible. The East Midlands will be the testbed for the new process this summer before it is extended to all routes by the end of the year. But how exactly will PDSW change how projects are planned and what role will Safe Work Leaders (SWL) play in it all.

Safe Work Leaders PDSW will create the new role of Safe Work Leader (SWL) – a single, accountable person responsible for safe work, from planning through to delivery. An SWL will be put on every work site, on or near the track, and all safety

critical roles will report to their SWL. The role will address concerns over the lack of leadership that sometimes exists and make it clear who is in charge. The SWL will be an employee of either Network Rail or a principal contractor or a contractor who holds a Railway Contractors Certificate. SWL roles will be split into three levels to mirror the complexity of work being carried out. The Controller of Site Safety (COSS) role and the Engineering Supervisor (ES) role will remain.

Proscient PDSW will see the introduction of a new electronic permit tool (Proscient) which will allow the SWL to plan, risk assess, deliver and hand back work more efficiently. It will be the single system used to plan works across the UK.

Those who have attained the required competencies will be able to access Proscient through a web browser. The system will be used to issue electronic permits which will put the latest information at the fingertips of the SWL, and provide a single ‘version of the truth’ for greater clarity. Clear, site-specific information plotted on track schematics will give easy access to more detailed information than ever before. Planning works digitally will ensure the plan is fit for site conditions on any given day and enables re-planning on site if necessary. The system will work alongside the existing PPL and ELIPSE systems, not replace them. Steve Featherstone, Network Rail track programme director, will be giving a presentation on work staging and planning at the Rail Safety Summit on 30 April.

What the future holds for BTP Marc Johnson talks to BTP Chief Constable Paul Crowther British Transport Police Chief Constable Paul Crowther says he wants the organisation to remain an integral part of the railway and not be seen simply as a supplier or some kind of private security outfit for TOCs. And why wouldn’t it? BTP has existed in one form or another since the birth of the railways, its creation driven in part by the unusual death of William Huskisson at the opening of the Liverpool to Manchester railway in 1830. Huskisson was struck and killed by the Rocket after getting off the train halfway to speak to the Duke of Wellington while the engine took on water. Although crime figures are continuing to fall - for the 11th consecutive year this year - there are always areas for improvement.

Chief Constable Crowther said that upon taking the role in 2014, there were two areas he had hoped to focus on; prevention and the management of offenders. ‘I had a clear view that restructure was only a tiny part of the story and what we needed to do was completely transform the way that we operated, build on some great success that we had,’ said Crowther. ‘The end of our performance year at the beginning of April sees the 11th consecutive year of crime reductions.’ He added, ‘We’ve set ourselves some very ambitious further crime reduction targets as well as reducing disruption and increasing passenger confidence.’

Indoors stuff Crowther joined the Metropolitan Police when he was 19. Soon after, he joined BTP as a uniformed constable on the London Underground. From there, he was promoted to Detective and then again to Superintendent before eventually taking up the role of Deputy Chief Constable. He also spent a less enjoyable, but ultimately rewarding, period as head of training doing ‘indoors stuff’. ‘I’ve always been an operational guy who likes to be in the thick of it,’ says Crowther. In March 2014, Crowther was named as Andy Trotter’s successor, taking over the Chief Constable role. As Deputy Chief Inspector, he oversaw the consolidation of the force’s eight geographical areas into just three divisions. During his career, Crowther has also played a part in the investigations at Hatfield,

BTP has a very different set of priorities to other forces in the UK. In particular, it has to be acutely aware of how its actions impact on the running of the railway.

Chief Constable Crowther will be giving a talk entitled ‘How do you lead and motivate a successful team’ at the Rail Safety Summit on April 30. In 2013, he was awarded an OBE in the Queen’s New Year Honours for his 30+ years of service to the police force.

Says Crowther, ‘If we shut the railway down every time someone made a bomb threat or left an unattended item, the system wouldn’t run very often.’

‘I think 7/7 is probably just about the most extreme example of crisis management,’ says Crowther. ‘In many of the incidents I’ve dealt with, the thing that really comes to the fore is how one makes critical decisions which are appropriate, which don’t under react but don’t overreact. So in the case of terrorism, if we’re not careful, we actually do the job of the terrorist for them by overreacting to an incident.’

In the past, there have been reviews considering the merits of letting the Metropolitan Police take over BTP’s position in the capital. The conclusion was that BTP played too important a role. North of the border, Police Scotland is preparing to take over responsibility for policing Scotland’s expanding rail network. Not quite Scottish independence but a significant moment in BTP’s story.

‘We stood back and said where do we fit into this rapidly expanding rail industry,’ said Crowther. ‘Sometimes, I think that with some of the rail operators, we may have ended up in a sort of relationship where we were viewed as a provider, a supplier/provider type relationship, and actually I wanted us to be seen very clearly as a strategic partner, an integral part of the railway family.’ He added, ‘I firmly believe in the network-wide provision of policing and security – it’s a model that works, it’s a model that’s been reviewed probably about nine times by different bodies as this subject comes up from time to time and every single one of those reviews has concluded that it’s best delivered by a dedicated transport police. That is firmly my professional view.’

We hit the books and our research discovered that even though people are aware of these dangers, they never actually think an accident is going to happen to them. However, they noticed when others were at risk. We took this information to develop a campaign to encourage people to simply ‘Lend a Helping Hand’ and be considerate to fellow travellers. This could be something as simple as standing aside when people are alighting the train or not rushing and barging on the platform.

We don’t want to tell you things you already know. You know the potential risks around the platform, but we’d love you to reinforce safe behaviour by promoting this campaign and leading by example. Principal Human Factors Specialist, Health and Safety Laboratory Phoebe Smith is an associate fellow of the British Psychological Society and a chartered scientist with over 20 years’ experience in human factors. She was awarded an honorary professorship by Sheffield Hallam University in 2013. Phoebe worked for the Royal Air Force undertaking research into flight safety and supporting accident investigations. Since joining the Health and Safety Laboratory in 1998, Phoebe has worked on a wide range of projects from writing health and safety guidance for the call centre industry through to supporting HSE inspectors investigate safety culture in high hazard organisations. Phoebe has advised on the human factors aspects of decision making, team working, procedures, and competence and training in support of HSE, other public sector bodies and commercial organisations in a wide range of sectors including health, electricity, oil and gas, pharmaceuticals, rail and nuclear. Her particular areas of expertise are competence and safety culture. As well as initiating the development of and co-facilitating the ‘Human factors in accident investigation’ course, Phoebe trains on behaviour change and mindfulness in safety and has trained the Singaporean government health and safety inspectors on the fundamentals of human factors.

Senior Associate, Pinsent Masons LLP Sean is a regulatory lawyer within Pinsent Masons’ UK Regulatory team. He advises across the UK on both contentious and non-contentious regulatory matters but with a particular focus on health and safety related matters as part of a multi-disciplinary team that includes Chartered IOSH Safety Practitioners. His experience means that he appears regularly as an advocate in court proceedings and Coroner inquests following work related incidents. He provides advice on a wide range of health and safety issues across all sectors of industry; he has a particular interest in infrastructure issues and the law relating to directors’ health and safety duties. At his previous firm he was a member of the team dealing with investigations into the fatal incidents at Ladbroke Grove and Potters Bar. Sean has spoken widely on health and safety and corporate manslaughter and has hosted many seminars and lectures on aspects of health and safety and regulatory law. He is recognised by Legal 500 and Chambers legal directories as a “Star Associate” in the field of health and safety and is a founder member and secretary of the Health and Safety Lawyers Association. In 2011 he was appointed as a Director and Trustee of the Construction Health and Safety Group, a charitable training organisation based in Surrey. Sean will become a Partner at Pinsent Masons in May 2015.

Wing Award moves to Rail Safety Summit The prestigious Wing Award for Safety will be presented during the Rail Safety Summit conference at the Royal College of Physicians. The Wing Award for Safety is an annual award, presented to someone who has made an outstanding contribution towards the safety of staff working at the line side on the railways of Great Britain. It was introduced in 1994 by the Institution of Railway Signal Engineers (IRSE) to commemorate the life and work of Peter Wing, a former Fellow of the Institution. Though initiated by the Signals and Telecommunications (S&T) community, it was stipulated from the start that the award would be made to an employee of any profession or company, something quite important as the UK rail industry was in the initial stages of privatisation. Peter Wing began his signal engineering career in the Test Room in Reading where, on satisfactory completion of initial post school studies, he was offered a sandwich course engineering apprenticeship. On successful completion of his training and professional qualifications, he held various posts around the Western Region before being promoted to assistant S&T engineer, Norwich Division in 1973 and then the same at Liverpool Street. However, the allure of God’s Wonderful Railway held strong and in

1980 he returned, this time to Bristol as divisional engineer. After a spell at Reading as Regional Signal Engineer, he was promoted to Western Regional S&T Engineer. In 1991 he moved up to the Directorate of S&T Engineering at British Railways Board where he took considerable interest in lineside safety.

Dead Serious about Safety At the time, the level of rail staff fatalities was unacceptably high and that in S&T was disproportionately so. There had been four within the Department within 12 months. In this role, Peter’s care and concern for staff safety provided the driving force for the production of an extremely hard hitting and innovative video entitled Dead Serious about Safety which was instrumental in bringing about a culture change in safety awareness and won international acclaim. The video analysed four recent fatalities and included a very emotional interview with a brave widow of one of them. The reduction to virtually zero of lineside casualties over the next few years, with Dead Serious about Safety as a central pillar of the national campaign, was remarkable. Peter sadly died in 1993 at the tender age of 49 and has been greatly missed by his family and colleagues alike. His leadership, especially in matters of staff safety, was exemplary.

As a boss, he had an exceptional talent for bringing the best out of those who worked for him. The award is administered by the IRSE on behalf of the industry, notably: the Members of the Railway Group, the Office of the Rail Regulator (in particular Her Majesty’s Railway Inspectorate) and the Railway Supply Industry, which, amongst others, supported the creation of the award. Says Tom O’Connor, managing director of Rail Media, ‘We are delighted to welcome the Wing Award for Safety to the Rail Safety Summit. Peter Wing was an inspirational figure in the railway industry and it is fitting that his example and pioneering work is continuing in this way. The Rail Safety Summit at the Royal College of Physicians forms an increasingly helpful focus for our number one responsibility, railway safety. We look forward to a much higher profile for the Wing Award for Safety.’

He has extensive management experience gained at London Underground, Connex and with his current employer Govia. He has held Board level appointments with these companies and also worked in operations, change management, human resources and investment planning. Charles has a BA (Hons) and MBA. Following the crash, Pam became the leading spokesperson for improving rail safety. Her battle saw her lock horns with rail management executives and government ministers - eventually bringing down Transport Secretary Stephen Byers MP for lying in the House of Commons – as well as being smeared by Prime Minister Tony Blair. She was dubbed a trouble maker, but in fact her tenacity helped bring about massive changes in rail safety. Over a decade on, having suffered a turbulent marriage along with mental and physical scars from the crash, Pam no longer regards herself as a victim. She uses her experiences to help others who have been affected by disfigurement or burns and has become an inspiration to many.

Executive Director, Railway Chaplains, Liam is Senior Chaplain for BTP and is the Executive Director of the Railway Mission, which provides welfare support to all sectors of the UK’s rail industry. Beginning his railway career as chaplain for Birmingham, the West Midlands and Chiltern Rail, he has worked in partnership with the rail industry for many years, and now heads-up the Railway Mission’s national network of chaplains. The chaplaincy team sees, at first hand, the consequence of lapses of safety protocols, and Liam’s position in the industry gives him an insight into the humanitarian cost of these failures. The Railway Mission was founded in 1881 and, in offering an holistic approach to pastoral support, its service is sometimes described as ‘cost effective staff welfare’. “But it should be noted,” says Liam, “that chaplaincy support is not a replacement for the normal chain of care provided by a company. It is a complementary service that can help to reduce the levels of stress experienced by staff, and help them avoid reaching crisis point.” There are Railway Chaplains covering England, Scotland and Wales, engaging with people face-to-face at all levels and in all areas of the industry. Chaplains travel extensively, talking to staff and managers with the intention of building good relationships so that individuals feel able to approach them in their hour of need. “Our work ranges from having coffee and a chat with railway staff to supporting the family and colleagues of a rail worker fatally injured in a tragic incident,” says Liam. “Our chaplains are there to offer face-to-face support with empathy and care whatever the circumstances.”

Director of Transport and Investigations, National Audit Office Paul is a Director at the National Audit Office specialising in the audit of organisations in the transport sector and leading the National Audit Office’s programme of investigative activity. This investigative work has included a suite of reports looking at Government’s approach to how the concerns of whistleblowers are handled. During his time at the National Audit Office, Paul has also had responsibility for auditing organisations in the education and defence sectors. He has also completed two secondments at the Ministry of Defence and the Home Office working in senior finance positions. Prior to the National Audit Office, Paul studied mathematics at the University of Bath. Outside of the National Audit Office, Paul is a keen fan and player of rugby and enjoys spending time with his young family.
