Know Your Options I

f you’ve been trying to conceive for a year or more and you’re not pregnant, it’s time to see your GP. If you’re a woman over 35, or if you think either of you may have a fertility problem, see your GP after six months of trying to conceive. Your GP may refer you to a fertility clinic. This will usually be in a hospital and will provide fertility treatments.

The fertility treatment that’s right for you depends on a range of factors. Staff at your fertility clinic can help you choose the treatment that will give you the best chance of conceiving.

Types of fertility treatment No single fertility treatment is best for everyone. The right treatment for you will depend on your circumstances, including the cause of your fertility problems, the age of the female partner and your medical history. Broadly speaking, fertility

•  Metformin is particularly beneficial for    women with polycystic ovary syndrome   (PCOS).

Medicines often used to assist fertility are listed below. These are usually prescribed for women, although in some cases they may also be prescribed for men. •  Clomifene helps to encourage ovulation    (the monthly release of an egg) in    women who do not ovulate regularly or    who cannot ovulate at all. •  Tamoxifen is an alternative to clomifene    that may be offered to women with

•  Gonadotrophin-releasing hormone and    dopamine agonists are other types of    medication prescribed to encourage    ovulation in women. However, medication that stimulates the ovaries should not be given to women with unexplained infertility, as it is not thought to be an effective treatment in these circumstances.

Eligibility For Treatment

You should find out:

Fertility treatment funded by the NHS varies across the UK. In some areas, waiting lists for treatment can be very long.

•    which clinics are available •    which treatments are offered •    the success rates of treatments •    the length of the waiting list •    the costs

The criteria that you must meet to be eligible for treatment can also vary. Your GP will be able to advise about your eligibility for treatment.

Ask for a personalised, fully costed treatment plan that explains exactly what is included, such as fees, scans and any necessary medication.

If your GP refers you to a specialist for further tests, the NHS will pay for this. All patients have the right to be referred to an NHS clinic for the initial investigation.

If you decide to go private, you can ask your GP for advice, and make sure you choose a clinic licensed by the Human Fertilisation and Embryology Authority (HFEA). The HFEA is a government organisation that regulates and inspects all UK clinics that provide fertility treatment, including the storage of eggs, sperm or embryos.

Going Private If you have an infertility problem, you may wish to consider private treatment. This can be expensive and there is no guarantee of success. It is important to choose a private clinic carefully.

Surgical Procedures These include fallopian tube surgery, which can be helpful if the fallopian tubes, which lead from the ovaries to the uterus (womb), become blocked or scarred, preventing pregnancy.

Assisted Conception This can include intrauterine insemination (IUI), in which sperm is placed into the womb using a fine plastic tube. This can be helpful in cases of mild sperm problems. Assisted conception also includes IVF (in vitro fertilisation), in which sperm and eggs are mixed outside the body and put back into the womb. This can be helpful for a range of fertility problems, including more severe sperm problems and cases of unexplained infertility.

Amniotic fluid During pregnancy, your growing baby is cushioned inside a fluid-filled bag (amniotic sac) in your uterus (womb). The wall of the amniotic sac is made up of two membranes: the chorion and the amnion. These membranes keep your baby safely sealed in his bag of amniotic fluid.

Q. What is a show? Will I bleed? A. Bloody show: this will be a small amount of blood and mucus. This is a very good sign that the cervix has started to dilate. This is what we call the barrier in your cervix which helps prevent germs getting to the baby.

Q. I have very little control down below I only need to sneeze and I leak urine so how do I know my waters have gone and its not urine? A. The difference is the smell. you will have a very sweet smell which will be inevitably different to the smell of urine.

Q. I am afraid of the pain, what would you suggest. A. As a birth coach I suggest to all my clients they switch off from everything around them, this will give you nothing to concentrate on apart from your contractions and breathing. A laboring woman is at her best and at ease most when she is not being questioned. Basically we say don't stimulate the neocortex of a laboring woman, this means in everyday life you are thinking, in labor you switch your thinking brain off, if you are then left, labor will be much much more manageable, the problem occurs when you are then asked a question, you then have to think and then realise the pain your experiencing.

Q. How will I know i’m in active labor? A. Active labor is when your contractions are less than 5 minutes apart. Established labor is when the contractions are less than 3 mins apart, this is when you need to go to hospital

Are you Ready for a Baby? Career, life goals, holiday plans… is there ever a right time to have a baby? Kerry Potter examines the biggest decision you’ll ever make. When a woman takes a pregnancy test in a film and sees a thin blue line, one of two things happen: she whoops with joy or her world falls apart. She either wants a baby or she doesn’t – it’s black and white. Real life, however, is more complicated. I remember sitting in my bathroom looking at my very own thin blue line with mixed emotions. I was mainly thrilled – I was 32 and married, we owned a house, this was our plan – a plan discussed and dissected with a large number of my friends at that time (to the point where four of my five university friends got pregnant within six months of each other). But even so, I was also slightly scared. I couldn’t even keep my plants adequately fed and watered, how would I cope with a real live baby? And there was a third thing too – had I done everything I wanted to do with my life yet? Should I have got another promotion under my belt? Should I have tried harder to fulfill my ambition of working in New York? Should I have gone to Glastonbury one last time? Had I completed enough of Project Life to begin Project Baby? Difficult stuff to ponder on a normal day – let alone one when you’re

so pumped with hormones, the breakfast news makes you cry and you’ve got such bad morning sickness you vomit when you brush your teeth. Like most women my age, since turning 30, I have had many, many conversations with female friends about when to start trying for a baby; all of us increasingly spooked by the ‘fertility time-bomb’ headlines, accusing professional young women like us of putting our careers before motherhood. A GP friend was busy saving up to cover future maternity leave. A party-girl friend who had just got married described how her clubbing crew begged her to do another summer in Ibiza before she had babies. Another was experiencing the choice between IVF or continuing to try and conceive naturally after two unsuccessful years of trying.

A survey of 3,000 women by Centrum last year showed that some women take up to five years to plan motherhood, with a third aiming for certain career goals, a quarter saving £5,000 and 58% aiming to own a property. Meanwhile, the bucket list – once restricted to things to do before you die – now pops up on parenting websites, in the guise of ‘things to do before you get pregnant’, on the premise that there won’t be a lot of bungee jumping going on once you’ve given birth.

There is a sense that we need to get our (metaphorical and literal) houses in order – and live a little – before we take the plunge. The career sabbatical for the gap year you didn’t have at 18, finally running a marathon, writing that novel, going on holiday on your own, travelling to three continents – all things you want to tick off before you settle down to a life of designer pushchairs and Ofsted ratings.

perfect timing So when is best to have a baby? And can we really plan and control this aspect of our lives? Statistics show that we’re leaving it later, sometimes to our cost. The average age of women giving birth now is a year older than it was a decade ago; 29.5 in 2010 compared with 28.5 in 2000, according to the Office for National Statistics (ONS). The Department of Health research shows the abortion rate for women aged 30-34 increased by 10% between 2009 and 2011 too, indicating more women choosing to delay motherhood until their late 30s.

There’s no getting away from the fact that the biological reality is stark. “The aim should be to have a baby before 35, certainly by 37,” says Dr Magdy Asaad of London Fertility Centre. “It becomes much more difficult after 35. The number and quality of your eggs are reduced. It’s difficult for women because life expectancy is increasing – it’s now 80-85 years – so a 40-year-old still feels young. But it’s a different story with your ovaries. If circumstances allow, you should be considering babies in your late 20s or early 30s.” Ah, yes, “if circumstances allow” – and that’s the crux. When do any of us feel the timing is 100% right? There is no completely perfect time to become a mum. And, of course, it’s not always a simple weighing up of whether your job, relationship and experiences are where you’d like them, there’s also the very real possibility that pregnancy won’t come when you want it. On average, 84% of women will fall pregnant within the first year of trying and one in seven couples have fertility struggles, adding yet another dimension to the “when is the right time” question.

a certain amount of experience and credibility, so you know how to juggle and delegate, and your firm trusts you to work flexibly.” Then there is your relationship to consider. It sounds obvious, but you need to make sure your partner definitely wants a baby too – one US study showed 100% of couples who’d had a baby when the husband wasn’t initially keen had divorced within six years. “If you can’t talk about it and decide when a good time is, how on earth are you going to raise a child?” says psychology consultant Dr Sandra Wheatley. “It’s about coming to a joint decision that’ll make you both happy.” It helps if you’re married too – ONS statistics showed that 80% of married couples in 1991 were still together a decade on, compared to 60% of cohabiting couples.

“Women can feel overwhelmed by everyone else’s opinion. Listen to your own voice and feel confident in it” The cost of having a baby is also on the rise – so can you afford to pay for childcare? If you’re working in London and need a full-time nanny, it could cost you £24,000 per year (which means you’d need to earn almost £30,000 gross to break even). According to Daycare Trust figures, a part-time nursery place (25 hours a week) in London costs on average £6,340 per year, but the most expensive ones go up to £15,000. It’s a bit cheaper in Manchester at £4,592 for 25 hours

care but in Edinburgh it rises again to £5,178. Wherever you live, and whatever you earn, it will be a significant financial factor in your life. A house with your name on the title deeds and a spare room is also a prime concern for potential mums. But the average age of the UK first-time buyer was 29 years old in 2012, rising to 32 in London. Given the average age of a woman giving birth is 29.5, the figures don’t really add up. This may be a case of just having to change your mindset – in the current financial climate, renting a family home is perfectly normal – a new study from Shelter says by 2025, 27% of households will be renting UK-wide. But do the maths first – experts say you shouldn’t be spending more than 35% of the family income on rent to be on an even keel financially. You might not think you have to worry about your friendships, but it can be lonely when you’re the first among your peers to have a baby. A CLIC Sargent survey showed that 40% of expectant mothers worried about being socially isolated after giving birth. So if at least two or three friends are already mums, you’ll have advice and support on tap. You’ll also make new friends – mothers in your NCT group or antenatal class – to help you through those first few years. All in order? Congratulations! You are ready to have a child. But not everything in life fits neatly on a spreadsheet. We are a generation used to being in full control of our destiny, but pregnancy and motherhood simply aren’t like that – they’re unpredictable, wonderful and frustrating in equal measure.

“I don’t think there is a ‘right’ time,” says Nifa McLaughlin, editor of parenting website gurgle.com. “There will always be something else going on: an amazing career opportunity, your house isn’t big enough, you have that holiday planned. I don’t think you should be ruled too much by life itself, you should just go on your instincts.”

The best starting place is to work out which barriers are surmountable, practically and emotionally, in your current situation. First up for many women comes career. “Maternity packages differ but usually you need to be in your job for 18-24 months to reap the full benefits,” says Kirstie Burn from recruitment firm PSD Group. Burn believes the best time to have a baby is seven to eight years into your career. “You should be at a certain level of seniority so that when you come back you won’t have to start all over again. You’ll have

The right choice It’s something Beth Follini from Ticktock Coaching sees a lot. She’s a life coach who helps women decide whether or not they want children. “If work is your passion and you don’t have a strong desire to have a child, do you have to give up your life’s work for motherhood? My job is to show people their choices – it might be difficult but they can make it work.” Follini asks clients to identify senior female role models in their industry and analyse how they manage career and children. She also cautions against caring too much about what everyone else says or does. Since becoming a mother I know I’ve been guilty of urging friends wavering on the issue of parenthood to go for it – I love it, so they will, right? But doing it before you are ready could cause resentment towards your new lifestyle, partner and baby. “I would never tell people to just do it,” says Follini. “Women can feel overwhelmed by everyone else’s opinion. Listen to your own voice and feel confident in it.”

I had no inkling as to how much becoming a mother would change my life. Start by pressing your lower back into the floor so that the tailbone lifts up and away from the floor. Gradually lift the lower back from the floor, one vertebra at a time, until you are resting on your middle or upper back. Buttocks should be lifted a few inches from the floor. Weight should not be on your shoulders and neck. 4. Hold for 5 to 10 seconds and begin to slowly lower the back from the upper portion one vertebra at a time. Visualize the spine unrolling. Relax with the legs outstretched or hug the knees gently to the body.

Benefits: - Increases flexibility in the back and shoulders - Stretches and releases tension in the neck - Strengthens leg muscles. As long as comfort allows, women in the second trimester may also do this pose. Note: When getting up from a back-lying position, turn to your left or right side first and gradually work up to a sitting or kneeling position. This avoids any straining or dizziness.

5. Modified Bellows Breath Exhale completely as you tuck chin and tailbone, round your back, and bend at the waist and knees. Hands rest on thighs. Repeat 2 or 3 times.

Lower arms, rest, and breathe normally. 6. Move legs to the left side and repeat by curving to the left.

Benefits: - Tones and strengthens muscles along the sides of the body - Aids digestion Increases flexibility of the spine.

7. Firm Pose Sitting Position 1. Kneel on a mat with a cushion or folded blanket between your legs. You may need to lean forward slightly to avoid losing your balance. Tops of feet are flat against the floor, pointing straight back. 2. Supporting yourself with your hands, slowly and carefully lower your buttocks between your heels onto the cushion. 3. If you feel any discomfort, adjust the position of the feet or add more height to the cushion. 4. Hands may be placed behind you for support or may rest on thighs. Chest is lifted. Do not sit in Firm Pose if you have symptomatic (i.e., painful, throbbing) varicose veins.

9. Tension-Relieving Breathing Stand with feet shoulder-width apart, arms held in front of you, palms together at chest height. Shoulders are down and away from the ears. Open mouth and inhale as you stretch your arms to a T position. Lift your chin.

8. Knee Rocking 1. Sit in a cross-legged position with your left leg on top. If necessary, lean against a wall to support your back (see Healthy Sitting). 2. Cradling the left leg with both arms, lift it toward your chest and abdomen as far you comfortably can. 3. Keeping your back straight, hold the leg as high and close to you as possible without straining. 4. Moving from the hip joint, gently rock the leg back and forth as though rocking a baby. 5. After rocking one leg several times, switch leg position and repeat with other leg.

Benefits: - Helps alleviate discomfort in the hip joints caused by your growing baby’s pressure on the nerves and joints in the hip area, buttocks, and thighs.

10. Hands-to-Wall Pose

1. Slowly bend the elbows so that gradually and with control, your upper body moves toward the wall as you slowly exhale. 2. Bring your face as close to the wall as you comfortably can. The body remains straight. Do not bend at the waist or knees. 3. Hold for a few moments, breathing normally. 4. Inhale and slowly push away from the wall until your arms are straight and your body is erect. Lower your arms. Relax and breathe normally. Repeat 2 to 3 times. Second part: 1. Stand erect with arms extended in front of you, palms against a wall. Place the body so you are at arm’s length from the wall, leaning at a slight angle. 2. Fingers of both hands point toward each other, touching but not overlapping. 3. Shoulders are relaxed, feet flat on the floor. Buttocks are tucked to prevent lower back from arching.

The buggy squat is a postnatal exercise designed by Pushy Mothers, the Buggy Workout creators, for our sister publication Practical Parenting & Pregnancy magazine. If you’re keen to get your pre-baby body back, grab your baby, the buggy, and try this one out... What it does: Defies gravity and makes buns of steel! Strengthens the bum and backs/fronts of thighs. Get set: Stand behind the buggy, feet hip-width apart, knees aligned in parallel, hands shoulder-width apart, holding the buggy handle. Slide shoulder blades down and draw the tummy lightly in. Action: Breathe IN to prepare, and as you breathe OUT, pull your tummy in, bend your knees and push your bum back as if you’re sitting down on a chair. Keep your knees over your ankles and don’t arch your back. Breathe IN and smoothly return to the ‘Get set’ position, fully straightening your knees. Do 2-3 sets of 8 repetitions.

The UK’s first womb transplant is set to take place next year as part of a clinical trial in which 10 women will get the chance to carry their own babies.

Following the birth of a baby boy last year after a successful procedure in Sweden, the Health Research Authority has granted ethical approval for 10 transplants. The first British baby born from a transplanted womb could arrive as soon as late 2017 or 2018.

“Donor retrieval is a bigger operation than transplanting the uterus into the recipient,” the surgeon said. “We don’t want to subject a live donor to that operation.”

More than 100 women have been identified as potential recipients of transplants for a team of surgeons to be led by Dr Richard Smith. About one in 5,000 women – or 50,000 of childbearing age in the UK – were born without a womb, while some cancer sufferers have had theirs removed.

Smith said using deceased donors reflected the complexities of the operation.

Organ donor coordinators have suggested that about five wombs annually could be made available. After 12 months on immunosuppressant drugs and close monitoring, each woman will be implanted with one of her embryos, with the hope of achieving a successful pregnancy. A baby would be delivered by caesarean section to prevent the donor womb suffering the trauma of labour.

Smith said the technique would offer hope to women who could only have children through adoption or surrogacy. The consultant gynaecologist at the Queen Charlotte’s and Chelsea hospital, who has been working on the project for almost 20 years, said he was “really, really pleased” to obtain ethical approval for the transplants.

Six months after giving birth, each woman can try for another child, or the womb will be removed. That would minimise the risk of keeping women on immunosuppressant drugs for the rest of their lives. However, the trial needs to raise £500,000 before any operations can take place. “I’ve always been an enormous optimist,” Smith said. “The project has run with no money from the start. Somehow or other, somebody has always turned up and given us enough money to keep it going.” Just over £40,000 has been donated to the Womb Transplant UK project and contributions can be made here.

“For many couples, childlessness is a disaster. Infertility is a difficult thing to treat for these women,” he said. “Surrogacy is an option but it does not answer the deep desire that women have to carry their own baby. For a woman to carry her own baby – that has to be a wonderful thing.” To take part in the trial a woman must be aged between 25 and 38, have functioning ovaries and their own eggs, a long-term partner and be a healthy weight. Only a third of the 300 women who approached the Womb Transplant UK team met the criteria. Before the trial starts, embryos will be created and frozen using each woman’s eggs and sperm from her partner. They will then undergo a six-hour operation to receive a womb from a donor who is classed as

A 36-year-old Swedish woman gave birth to a baby boy in September 2014 after receiving a donor womb from a 61-year-old family friend who had given birth to two sons. She and her partner - both competitive athletes - named the baby Vincent, which means “to win” in Latin. “As soon as I felt this perfect baby boy on my chest, I had tears of happiness and enormous relief,” the mother said. She had been told at the age of 15 that she did not have a womb.

The Powerful Story Behind

sassy bloom Sassy Bloom was founded in 2012, after Hollie was diagnosed whilst 29 weeks pregnant with a very rare, incurable and life threatening progressive lung and heart disease called Pulmonary Arterial Hypertension.

After passing out several times during her pregnancy and being rushed to hospital by ambulance on numerous occasions – only to be dismissed each time as the doctors were confused by the symptoms that Hollie was presenting – Hollie saw a cardiologist who luckily recognised the severity of Hollie’s symptoms and immediately called for an ambulance. Darren, her husband was secretly told that his wife had a 50% chance of survival and that things were very serious and that the baby had to be born within 48 hours. “She looks very well on the outside, but your wife is EXTRMELY sick” he recalled. Hollie was transferred to a specialist hospital with an anaesthetist present as they feared she would die en route! “It was scary but I don’t think I really realised the seriousness of the condition,” says Hollie. “It wasn’t until I opened my eyes at about 2am in the morning and found my parents, siblings and husband sat round my bed, all with ashen faces, that it started to sink in just how critical my condition was.” “When the care team explained that I needed to get all my affairs in place as I might not get through the delivery, it really hit home just how ill I actually was” After a month in hospital, lots of medication and many unpleasant tests including a permanent infusion to Hollies heart – the couple’s baby was scheduled to be delivered with a large team present. The delivery date kept moving as the team had to juggle Hollie’s health and delivering a premature baby. Most mums previously in Hollie’s situation have died during labour or shortly after as the body re-adjusted. It was incredibly scary for Hollie, Darren and all their family and friends. At 34 weeks and after 5 hours in surgery – the couple’s new baby was delivered and taken to Special Care whilst Hollie was rushed to intensive care. Darren spoke to specialists all around the world, tracked down patients and put Hollie in touch with a mum who had

successfully come through the other side and did everything within his power to see Hollie through this chaos that turned the couple’s world upside down. “In Intensive Care I had to tell Hollie everything was going to be ok whilst watching people dying as their relatives were breaking down behind her”. Can you imagine a new mum going through all of this and then not being able to be with her baby for days?! Darren spent the coming days running from Special Care to Intensive Care to be with his new baby and his wife. On day 5 in Intensive Care day the panic alarm went off and the entire team came running, Darren turned white as they shouted at team members to get him out of the room – he thought that Hollie had died just like all the other woman that he had read about on the internet! Luckily Hollie had just passed out and after a medication increase. Hollie learned to walk again – she was downgraded to the Cardiology unit – a big milestone.

Project Baby:- Can you tell us what inspired you to team up with My 1st Years? I’m still breastfeeding and its not easy but it is easier to take him than it would be for Billie to take Nelly. Billie:- If I had Nelly here it would be chaos, It just wouldn’t be fair on her. She has just started nursery and our mum is always on hand to help. Project Baby:- Do you have any tips for new mums and

working life?

Billie:- Be orgainised, Last night I had everything laid out and ready to go this morning. It makes everything so much less stressful. Sam:- Yes organisation is key. Even if your not working. Single to duo as easy as ABC There is no doubting how easy it is to transform the Donkey from single to double, you just unclick a couple of catches and pull the frame to create a wider frame. You can have the seats in a variety of positions - Twin mode, two carry cots, seat and carrycot, parent and world facing. One of the key features for me is the size of the underneath basket, its huge and when in single mode you have an additonal basket which is really handy, but my little one kept leaning over and grabbing things, all fun and games. Tires do require a pump, so you are risking getting a flat whist walking the dog. The wheels and suspension do both deserve a mention, If your looking for a pram that is suitable on all terrain and is ideal for twins or a pram that will grow with your family this might just be what you are looking for. In single mode it was great for getting on public transport but in double it was a little more tricky. I had to leave the house and set up outside for duo mode, fine in the Summer months but maybe a nightmare in the Winter. The seats are more narrow than most standard prams but still plenty of room. Edward 9 Months and Polly-Ann 2 both were very comfy.

How to Help a Choking Child Recently 2 year old Jacob Jenkins died after choking on a grape at Pizza Hut. This incredibly sad story has highlighted the need for more people to learn First Aid. Choking is frightening and difficult to know how best to help. If the child is coughing, give them a second or two to see if they can cough the obstruction up themselves. If they are struggling to breath and unable to make a sound –their airway is blocked and they need assistance fast! To prevent choking: keep small objects out of reach, cut up food into very small pieces and supervise children while they’re eating, especially if they are under five years old.

For a baby under 1 year •  First check in the baby’s mouth and if there is something obvious, remove it with finger tips. DO NOT put your fingers down a baby or child’s throat, or finger sweep the mouth, as this can make matters worse by pushing the obstruction further down or by causing swelling. •  Lay the baby downwards on your forearm, across your legs, supporting them under their chin and using the flat of your hand, give a firm back blow between the shoulder blades. •  Give up to five back blows and check between each blow to see if the blockage has cleared. If the obstruction has not come out – get an ambulance on the way. •  If the obstruction hasn’t cleared, lay the baby on their back, place two fingers in the centre of the chest just below the nipple line and give up to five chest thrusts. (the same place as you push when doing chest compressions on a baby) •  Check to see if the blockage has cleared between each chest thrust.

•  Give up to 5 back blows checking each time if the blockage has cleared If the back blows haven’t helped get an ambulance on the way •  If the blockage hasn’t cleared after five blows, the next stage is an abdominal thrust/ Heimlich manoeuvre: •  Stand behind the child and place one hand in a fist under their rib cage. Use the other hand to pull up and under in a J shaped motion to dislodge the obstruction. Perform abdominal thrusts up to 5 times, checking each time to see if the obstruction has cleared. Anyone who has received abdominal thrusts must be seen by a doctor.

Warning: Never do an abdominal thrusts on a baby under a year as you could cause damage – chest thrusts for a baby.

•  If the child is still choking, call 999 (or 112) and alternate five back blows and five abdominal thrusts until emergency help arrives. If at any point the child becomes unconscious, commence CPR. Once the obstruction comes out: If they are unconscious and breathing – put in the recovery position If they are unconscious and not breathing start CPR If they seem absolutely fine – ensure that they don’t have problems swallowing, check there is no pain or bleeding – it is always advisable to have them checked out by a medical professional. If it is not your child, ensure that you have contacted their parents. It is strongly advised that you attend a First Aid course to understand what to do in a medical emergency.

Bird Nest One Way to Deal with Divorce A mothers story

The children simply live at “home” and the separated or divorced parents take turns living with them there, but never at the same time. The core element of this arrangement is that each parent maintains a separate residence where they live when it is not their turn at the “bird’s nest”. When my ex-husband and I were considering divorce, there were many things that paralyzed me with fear. My absolutely most terrifying concern was for my children’s adjustment. Early on I had lunch with a friend who is a family law attorney. She offered some really good advice. She said, “Everybody is different. Your family is not like any other family, your divorce doesn’t have to be like anybody else’s either nor does your custody arrangement. Get something you can both agree to on paper with the mediator, and then do what works and makes you guys happy. If it doesn’t work long term then you revert to what you agreed to on paper. The paper work can be a ‘just in case you stop agreeing about the children’ plan.” That lunch opened up a world of possibilities for me. Although my Ex and I didn’t agree on much, we did agree that the children should have as easy a time of it as possible. It wasn’t their fault after all. I told him of my lunch with this friend and what she said. We both agreed to consider alternative custody arrangements. While googling ‘alternative custody arrangements’ I stumbled on Birdnesting Custody. I was interested and found a few articles, but most were negative. After scouring around the internet a few days, I did find a few actual stories from people that had done it successfully.

Birdnesting custody is an arrangement where the family keeps the marital home, both parents paying into a household account to cover basic household expenses (in lieu of child support) and you rotate time in the ‘nest’. The children never leave, the house has custody of the dog even, and the house becomes the children’s home and source of stability.

Why We Chose to “Nest” Prior to learning about the possibility of Birdnesting my heart would clench inside my chest every time I considered moving the kids or that they would go back and forth from me to their dad’s. We had only recently moved across town into a house that was in the ‘right’ school district. Our children were getting older and it was urgent to be in the right district if we hoped to avoid private schools. We had moved in the nick of time for our oldest to start middle school. We still had boxes that weren’t unpacked. I hated to think of moving them again. Additionally, at the time, the housing market had just crashed to rock bottom. We had no equity in the short time in this house and would be lucky to get what we paid just months before. Also, we have one child with ADHD. Bringing the appropriate things back and forth from school to home is a daily challenge for her. I had no idea how she would function with another home and bag. My ex-husband agreed to try this arrangement based on these factors. Our mediator was initially intrigued but became a great cheerleader over the course of our sessions. She had never heard of it by name or the concept of Birdnesting as anything more than a transitional arrangement, but she encouraged us to try. She wisely counseled us to have specific rules and to never forget that the house is the safe place for the children. In my travels on Google I had found some ideas that other people used to keep boundaries clear in the nest. My ex-husband and I have 50-50 custody. Financially it works like this: we created a house account that for nest expenses. No cash withdrawals are ever made from that account. Mortgage, utilities, cable, groceries, kid expenses, braces, our life insurance policies, etc., are paid from this account. On paydays, we each pay the nest first. It was determined the percentage that each of us made of our household income (60/40). We added up the money required to run the nest and he puts in 60% and I put in 40%. Anything that comes up that we haven’t accounted for falls under the 60/40 rule. Running the nest is less expensive than running our household while married because we each pay for our own cars, insurance, credit cards, and basic life expenses etc. out of our own money.

Our Nest Rules: 1.  We respect the privacy of the other’s spaces in the nest. 2.  Leave the nest like you found it. The parent that rotates on shouldn’t have to come in and clean up the other’s mess. 3.  No overnight guests of the opposite sex in the nest. They may visit but may not stay past bedtime. 4.  The person on nest duty is responsible for groceries in the house, kid laundry, etc.

Some couples that Birdnest share an apartment where the off parent goes and they rotate between the apartment and house, but neither of us thought we could manage that. I have family in town that graciously offered me a guest room when I’m off of nest duty. I try to be as unobtrusive as possible. My ex-husband has a studio apartment for his off times.

Initially we were trying to figure out how we could possibly have money enough after paying for the nest to afford nice apartments for the children and then we realized that the beauty of this is that the kids never go there, so both of us do it as cheaply as possible. They’ve been to his apartment for short visits but they’ve never spent the night.

How Others React I get mixed reactions when I tell people about our custody arrangement. Some cheer me on and commend me for our unique arrangement. I love those people. Equally, some people are very negative and say that it will never work. Well, it has worked for almost 3 years. Some say that it will never work when we have other partners. So far that hasn’t been a problem either. When we date we just tell them up front about our arrangement. Some are confused until they realize that we are never at the nest at the same time. We cross paths several times a week, but we never stop walking as we pass. I did read about several families that Birdnested for many, many years and in that time both spouses remarried. They simply kept the nest going like before and at bedtime their new spouse went back to their shared home. Their kids grew up and then they sold the family home.

The Downside to “Nesting” There are downsides, I won’t lie. It is expensive. But until the housing market improves we are unlikely to be able to sell the house without losing money. Also, I get tired of living out of a bag. When I go back on nest duty I take my dirty clothes out of my bag, add them to the laundry room floor and pack again when I leave. It is an inconvenience. But then I remember that I’m not a child and I made my own decisions about my marriage and divorce. None of which my kids got a vote on. I remind myself that every day I pack the bag is a day my children don’t have to do it and feel proud that at this time they are spared that experience. The Advantages •  Said to be child-centric: it focuses on the needs of the children. •  Children are able to continue to live in their home for a greater sense of stability. •  Minimizes the difficulty for children of living in two places, like forgetting homework, toys, clothing, or other belongings at the other parent’s home. •  Parents might be better able to handle the experience of moving back and forth between their residences than the children might be. The Disadvantages •  It is expensive. Unless the parents are able to share the residence in which the children do not live, they are looking at having three residences, one for the children and one for each parent when they are not living with the children. •  It requires a large degree of civility between the parents and the ability to get along and make decisions together. In general, the absence of conflict is the best indicator of resiliency for children, both in a marriage and post-divorce. •  It does not take into consideration what would happen when one or both parents enter a new relationship. Even before this aspect of “moving on” occurs post-divorce, both parents often need to individuate from each other and establish separate lives in order to disentangle from the marriage relationship they had with each other. Bird nesting does not adequately foster this aspect of divorcing. •  House rules: in order for parents’ movements to occur in a nondisruptive manner, both parents would need to have similar rules and house cleaning sensibilities. As a parent it’s often difficult to know when a child’s development falls within the ‘normal’ range and when there is cause for concern. Just like learning to walk, children develop the ability to communicate at different speeds, and some will encounter difficulties along the way.


DEVELOPMENT

What is the difference between speech and language? Speech is how we produce or articulate the sounds that form words. Language refers to the way we receive, understand and express information through all forms of communication - verbal and non-verbal. Although speech problems and language problems are different, they often overlap. A child with a language problem for example, may be good at pronouncing words but be unable to join more than two words together. A child whose speech may be difficult to understand may still use words and phrases to express ideas. And another child may speak clearly but have difficulty understanding instructions.

As a very rough guide, and remembering that each child is different, your toddler should have an expressive vocabulary of around 50 words by the time they are 2 years old, and you should be able to understand about half of what they say. If your little one only uses certain sounds or words repeatedly, and is unable to use spoken language to communicate more than their immediate needs by this age, he or she might have speech or language problems, or a combination of both.

To help a child with a lisp, a therapist will demonstrate the correct production of the ‘s’ and ‘z’ sounds, highlighting to the child how the tongue is hiding behind the teeth. The child is given lots of opportunities to listen to the production of the correct sounds, and then helped to produce ‘s’ and ‘z’ correctly from a sound to use in conversation. For children who stammer, the therapist will try to establish any factors that may be contributing to the problem, and work with the parent on strategies to help the child to talk more easily, including overcoming fear and anxiety.

Things are going to get messy! Baby-led weaning is, it must be said, a somewhat cheesy term for just letting your infant self-feed. You cut food up into manageable sticks and offer it, they eat. It’s really pretty simple.

development The key difference between BLW and traditional weaning, when you think about it, is in the order that children learn to eat. With a puree, they learn to swallow first and then chew, which works fine until they meet a lump. With BLW, the babies learn to chew first and swallowing might come some time later. It’s ‘baby-led’ in the sense that you let them do what they need to do while they’re learning, and as the parent you resist the urge to get wound up in knots about how much they’re eating, whether they like the food you thought they’d like and whether it’s smushed into the nearest curtain. The main thing is… it’s all good clean (messy) fun.

Top Tips for BabyLed weaning 1. Have a good trawl on the internet for blogs, info and in particular video clips of BLW babies. Seeing little tiny 6-month-old babies demolishing their food and hearing the gasps of admiration from the proud parent behind the camera (and by parent I mean Dad. It’s always the Dad), will do your confidence the power of good. 2. Next, forget ‘baby food’. Food’s food, as long as you’re not adding salt. To start off with, think chip-sized because it’s an easy shape for little 6-month-olds to grip, but you’ll soon move on to smaller pieces as it’s more interesting for a child developing a pincer grip.

3. As a first food most people steam carrots (to about the degree that they can be smushed ‘twixt your thumb and finger), cut up cucumbers, make toast fingers or crinkle cut bits of mango, that sort of thing, but remember if there’s no reason whatsoever why your baby can’t have a pile of Spaghetti Bolognese or mashed potato to dig into if that’s what the rest of the family is having.

8. (Slightly bitter) experience suggests that the more effort you put into making something special for the baby, the less likely they are to eat it. Give them what you’re having. If they hate it, fine, they’re getting their calories from milk anyway. 9. Of course it would be perfect if we ate every meal as a family, just like the Waltons but this isn’t always possible. Try to keep your ‘social activity’ head on, though, even if it’s just you and your baby sharing a sandwich at lunch. Keep smiling, keep enjoying, keep paying attention. It’s just good manners at the end of the day, something it’s never too early for a child to learn. 10. Don’t get too hung up on three meals a day, it may take a while to work up to that. Whatever’s convenient and enjoyable for you is best. 11. And don’t put too much on the highchair tray at the one time, just a couple of pieces of food will stop them feeling overwhelmed. 12. Actual hunger can be frustrating for the babies when they’re still getting to grips (quite literally) with things. Timing ‘meals’ to between milk feeds seems to be best, and because it’s just finger food you aren’t limited to staying in. There’s no reason why you can’t pack a wee Tupperware with some carrot or cucumber, buy a banana when you’re out or just pull some bits out of an undressed salad. 13. Never put food into a child’s mouth, let them put it in by themselves so that they can control it as it moves backwards. If the baby gags, remember that it’s their way of moving food around in the mouth and don’t panic. Some parents have found that making exaggerated chewing faces and noises reminds the child to get back on track. 14. Nappies and their contents will soon fascinate you in ways you never thought possible. Raisins rehydrate, little pieces of still-green broccoli sneak through the digestive system and bananas produce poo with strange black threads. Look and learn, ladies. 15. Have a camera ready to capture those first gummy, carroty smiles because as daunting as it may seem, weaning is a very short time in your child’s life. So remember to enjoy it…

4. No bowls, they’re just asking to be flung heavenwards. Put the food on the highchair tray or table and remember, it’s all a learning experience for the baby at this point. They really don’t care whether the experience is ‘oooooh, mango is in my mouth’ or ‘ooooooh, a bowl is flying across the room’. 5. As an experienced eater yourself, you already have all the ‘equipment’ you’ll need to feed your child, but there are some things to consider. An easy-to-clean highchair is a must, so head to Ikea for a fifteen quid Antilop, which will even fit in the shower for a hose-down on a bad broccoli day. 6. There will be mess, oh yes there will, so if you are weaning in summer don’t be afraid to eat outside or semi-naked (and the baby too, if you like, hem hem) and for winter Ikea and Tommee Tippee make great cover-all and pelican bibs. 7. Putting a wipe-clean tablecloth under the highchair is a good idea if you have carpets and some people find that a crinkle cutter is handy to make food extra-grippable.

Breakfast Wake-Up Bars A healthy cereal bar is perfect if you don’t have time for a sit-down breakfast. olive oil or coconut oil, for greasing 125g/4½oz/heaped ¾ cup cashew nuts 150g/5½oz/1½ cups rolled oats 60g/2¼oz/heaped 1 cup unsweetened coconut flakes 1 tbsp maca powder (optional) 1 tbsp shelled hemp seeds 1 tbsp sunflower seeds 1 tbsp flaxseeds 60g/2¼oz/¼ cup chopped ready-to-eat dried apricots 60g/2¼oz/⅔ cup goji berries a pinch of sea salt 60g/2¼oz/¼ cup almond or cashew nut butter 60g/2¼oz/scant ¼ cup honey 200g/7oz/heaped 1 cup soft pitted dates, chopped 1 teaspoon vanilla extract

1. Preheat the oven to 180°C/350°F/Gas 4 and grease a shallow 20cm/8in square baking tin with oil. 2. Put the cashews in a food processor and process until fine. Add the oats and coconut flakes and pulse to break up slightly. Transfer to a mixing bowl and stir in the maca

powder, if using, seeds, apricots, goji berries and salt. 3. In a food processor, blend together the nut butter, honey, dates and vanilla extract to form a paste. Mix the paste into the dry ingredients. Spread the mixture into the prepared baking tin. Bake for 20–25 minutes, or until slightly golden.

Roasted Banana Caramel Mouse Roasting bananas with their skins on creates a wonderful sweet caramelized flavour. The flesh is puréed with a healthy caramel-style sauce to create a thick mousse. Sweetened only with dates, this makes an irresistible healthy dessert. The pecans can be made in advance and stored in an airtight container. They make a delicious snack. One banana has an impressive 34 per cent of the recommended amount of vitamin B6, which is important for hormone balance, lowering homocysteine.

Serves: 4 Preparation time: 5 minutes, plus 2-4 hours soaking Cooking time: 40 minutes

150g/5½oz/heaped ¾ cup soft pitted dates 2 bananas 2 tbsp almond butter 100ml/3½fl oz/generous ⅓ cup coco-

nut milk 1 tsp vanilla extract Toasted Nuts 50g/1¾ oz/½ cup pecan nuts

1 tbsp maple syrup 1 tsp vanilla extract ½ tsp ground cinnamon a pinch of sea salt

1. To make the toasted nuts, put the pecans in a bowl, cover with water and leave to soak for 2–4 hours, then drain.

4. To make the mousse, put the dates in a bowl, cover with water and leave to soak for 20 minutes, then drain.

2. Put the maple syrup, vanilla extract, cinnamon and salt in a bowl and mix well to combine. Add the drained pecans and toss them with the mixture to coat well.

5. Meanwhile, preheat the oven to 200°C/400°F/Gas 6. Put the bananas on a baking sheet and bake for 20 minutes, until the skins are black. Leave to cool, then peel away the skins and place the flesh in a food processor or blender.

3. Preheat the oven to 160°C/315°F/Gas 2–3 and line a baking tray with baking parchment. Spread the pecans on the prepared baking tray and bake for 20 minutes until crisp. Leave to cool, then roughly chop.

6. Add the drained dates to the food processor, along with the remaining ingredients and purée to form a thick mousse. Serve topped with the toasted pecans.

Vietnamese Turkey in Lettuce Wraps

This dish is ideal for an energizing lunch or light evening meal and is so quick and simple to prepare. Spoon the spiced turkey into crisp lettuce leaves and drizzle over a little chilli sauce for an extra kick. A high-protein, low-fat food, turkey is also rich in tryptophan and tyrosine which the body converts to serotonin and dopamine chemicals that play a role in the production of babymaking hormones.

Serves: 2 Preparation time: 10 minutes Cooking time: 8 minutes 1 tbsp olive oil 200g/7oz minced turkey 1 small red onion, diced 1 tbsp peeled and grated root ginger 1 red chilli, deseeded and diced 1 handful of coriander leaves, chopped 2 tbsp fish sauce juice of 1 lime juice of 1 lemon 1 tbsp honey 1 tbsp soy sauce sea salt and ground black pepper 2 Little Gem lettuces, leaves separated, to serve Thai sweet chilli sauce, to serve (optional)

1. Heat the oil in a large frying pan or wok and stir-fry the minced turkey until it begins to turn light brown. Add the onion, ginger and chilli and stir-fry for a further 2–3 minutes. Remove from the heat and stir in the coriander leaves. 2. Combine the fish sauce, lime and lemon juice, honey and

HappyEating A survey, launched by leading baby food brand Organix, reveals that one of the biggest barriers to introducing new foods during weaning is baby’s facial expression in response to new tastes. It can be challenging to introduce new foods during weaning and the faces babies pull can be interpreted by parents as disgust or dislike at what they are trying, which can make it all the harder. The Organix weaning survey reveals: •    More than half (55%) of mums say a baby’s facial expression puts them off trying to     encourage their baby to like a new taste and flavour. •    49% say it’s a struggle to keep babies interested in food. •    42% say they find it hard to get babies to try new foods during weaning. From sweet carrot to sour lemon and bitter broccoli, weaning is the beginning of a taste adventure, the first opportunity for your baby to explore and experience taste sensations. Evidence shows that it can take between 10 and 15 times to introduce a new food or flavour and that learning about food is all about familiarity and repetition. So by understanding more about their babies’ facial expressions, parents are likely to feel more confident about introducing new foods. Baby faces and understanding what they mean … But what is the meaning of that face your baby pulls as they try a new food for the first time? What’s your baby really trying to tell you?

“For example, what mums and dads might interpret as a look of shock or disgust, doesn’t necessarily translate as not liking the taste, what’s more likely is that baby is puzzled by the intensity of a new flavour. A surprised expression could be curiosity about the depth of flavour, the chances are they think the flavour is good but it’s very different to what they’ve eaten before – it’s all part of them experiencing and learning new tastes on their weaning journey.” It can be challenging to keep offering new foods and coming up with new and inspiring ideas to tempt little ones, but it’s important as those first tastes during weaning help define a child’s future relationship with food.
