Affidavit The undersigned affiant, being duly sworn, deposes and says:

1. That Affiant, Sergeant Steven Cifone #375 , being duly sworn , does depose and state that I am a member of the Southington Police Department and have been for approximately nine (9) years and five (5) months prior to this date. At all times mentioned herein , was acting as a member of said department. The following facts and circumstances are stated from personal knowledge and observations, as well as information received from other police officers acting in their official capacity, and from official police reports and statements made by prudent and credible witnesses.

2. That the name and address of the victim are not being disclosed in this report in accordance with C.G .S. 54-86d and 54-86e. The Victim will hereinafter be referred to as Victim within this report. Victim is described as a 36 year old white female. The accused is identified as Gregory Smith with a DOB of 12/21/1976. 3 . That on 1/21/2020, Victim came to the Southington Police Department to report that over the past several months, Gregory Smith, has violated a Protective Order on multiple occasions by visiting her residence in Southington, CT. 4 . That Victim provided photos of Smith's vehicle in the parking lot of her complex as well as a sworn written statement regarding her relationship and twelve (12) separate violations of the protective order.
