PRIMARY INTENDED OUTCOME Human drug compounding is an important branch of pharmacy practice that creates medications tailored to the health needs of specific patients. Compounded drugs improve and save lives by providing medicines that may be unavailable or not commercially sold in a particular formulation. However, compounded drugs are generally not required to meet federal approval and can be at risk for contamination. Charles River provides a range of microbiological testing services and equipment to support drug discovery, development, and manufacturing that can be tailored to a facility’s specific needs and ensure patient safety.

THE CHALLENGE In 2012, an outbreak of fungal meningitis caused numerous cases of serious illness and more than 60 deaths, according to the CDC. The public health tragedy resulted from contaminated injectable steroids that were produced by the New England Compounding Center (NECC). Confusion over state and federal oversight was called out as a contributing factor. The NECC was also found to be acting, erroneously, as a drug manufacturer, which would be subject to different regulations than a compounding pharmacy. Compounding pharmacies are now divided into two sectors. Designated 503A facilities are limited to dispensing small amounts of drugs and must follow USP and state pharmacy guidelines. Larger 503B pharmacies are allowed to use bigger batches and must validate every process with current good manufacturing practices (cGMPs). These 503B requirements include more frequent environmental monitoring, on-site inspections, and submitting products for testing, which can result in longer lead times.

When Dayton Children’s Hospital reached out to Comprehensive Pharmacy Services (CPS) for support in assessing compliance and regulatory issues for controlled substances, sterile compounding (USP <797> and <800>), The Joint Commission standards and 340B, CPS responded with custom solutions from its Compliance and Regulatory Services and 340B teams.

SOLUTIONS & RESULTS: Sterile Compounding (USP <797> and <800>) SOLUTIONS: In January 2018, the Compliance and Regulatory Services team provided a full assessment of compounding safety practices from procurement to administration in Dayton Children’s Hospital’s four compounding locations that included review of: •

Workflow observations

Equipment and engineering controls

Environmental sampling

Cleaning and disinfection

Policy and procedures surrounding sterile compounding

Following the assessment, the Compliance and Regulatory Services team drafted a Compliance Report and developed a plan for process improvement designed to optimize safety, efficiency and effectiveness, and compliance. At the request of the organization, due to a plethora of ongoing projects on the current Director of Pharmacy’s agenda, the Compliance and Regulatory Services team offered a custom approach to managing the execution of

the project plan remotely. Currently an ongoing project, the Compliance and Regulatory Services team is virtually managing implementation and execution of the line items in the plan, ensuring that timelines are met and freeing up the Director of Pharmacy for other responsibilities. RESULTS: As of March 2018, the project plan is approaching 25 percent completion. In February 2018, the Board of Pharmacy surveyed one of the organization’s four sterile compounding locations. Because some of the items identified in the project plan were being acted upon, the surveyor issued no further citations. At another location, guidance is currently being provided for a full remodel of the sterile compounding suite which includes working directly with the pharmacy team, architects, engineers and facility administration.

PRIMARY INTENDED OUTCOME Human drug compounding is an important branch of pharmacy practice that creates medications tailored to the health needs of specific patients. Compounded drugs improve and save lives by providing medicines that may be unavailable or not commercially sold in a particular formulation. However, compounded drugs are generally not required to meet federal approval and can be at risk for contamination. Charles River provides a range of microbiological testing services and equipment to support drug discovery, development, and manufacturing that can be tailored to a facility’s specific needs and ensure patient safety.

THE CHALLENGE In 2012, an outbreak of fungal meningitis caused numerous cases of serious illness and more than 60 deaths, according to the CDC. The public health tragedy resulted from contaminated injectable steroids that were produced by the New England Compounding Center (NECC). Confusion over state and federal oversight was called out as a contributing factor. The NECC was also found to be acting, erroneously, as a drug manufacturer, which would be subject to different regulations than a compounding pharmacy. Compounding pharmacies are now divided into two sectors. Designated 503A facilities are limited to dispensing small amounts of drugs and must follow USP and state pharmacy guidelines. Larger 503B pharmacies are allowed to use bigger batches and must validate every process with current good manufacturing practices (cGMPs). These 503B requirements include more frequent environmental monitoring, on-site inspections, and submitting products for testing, which can result in longer lead times.


CASE STUDY Reliable & Rapid Endotoxin Testing When gram-negative bacteria are destroyed, they leave behind endotoxins. These cell wall fragments are capable of causing fever and septic shock in human and animals. Bacterial endotoxins are ubiquitous and can contribute to the contamination of IV drugs and vaccines. Product release testing for endotoxins is required for all sterile products. As the leader in the evolution of endotoxin testing, Charles River has developed revolutionary products and technologies that have increased both the quality and speed of testing. Traditional endotoxin testing can be performed in a 96well titer plate, which requires a trained analyst, can be very tedious and complicated, and ultimately opens up huge potential for human error. “This is generally a test for a scientist to perform,” Patton said. The Charles River Endosafe® nexgen-PTS™ (portable test system), in contrast, is a simple, handheld instrument specifically designed for non-scientists to perform endotoxin testing. The instrument utilizes FDA-licensed LAL cartridges, which contain all the necessary reagents to produce quantitative endotoxin results in as little as 15 minutes, making it simple and affordable enough for even very small pharmacies. There is no need to prepare endotoxin standards and no microbiology training required to run a test utilizing the Endosafe® cartridge technology. In fact, the test is so user-friendly that even personnel from different departments can be relied upon to carry it out. “One large client wanted to test their water early in the morning, so that they could save time in getting their manufacturing lines up and running,” Patton recalled. “They taught the first person who arrived each day to run the test; that just happened to be the janitor.”

THE DECISION Erin Patton was a Charles River customer long before she was hired to work there as a senior product specialist. As a microbiologist and lab manager at a large analytical testing lab at the time of the NECC crisis, she performed and reviewed hundreds of thousands of tests, including potency and sterility testing. “I developed a very good understanding of the regulatory issues facing compounders today,” she said. At Charles River, Patton focuses on endotoxin testing, microbial detection, and microbial identification. These highly specialized tests often require expensive equipment and scientifically trained personnel to run them. It may not make sense for a compounder to take those functions on in-house. “A compounder’s expertise is in making drugs, not testing them,” Patton said. “We offer rapid and reliable technologies that remove the possibility of human error and increase operational efficiency, which ultimately saves money. Our purpose-built portfolio of progressive products and services are targeted toward large and small compounders as they deliver accurate, relevant, and measurable data to fuel confident decisions on product quality for release.”

THE RESULTS When it comes to environmental monitoring, Charles River supports routine dayto-day microbial monitoring as well as more urgent investigations. Clients who may be concerned about an organism found in their clean room or, more urgently, a product sterility test result that did not pass, need detailed and rapid results. Genetic sequencing is considered the gold standard in microbial identification. An organism’s DNA will be the same regardless of phenotype or stress state. “It’s not enough to look at a sample and say ‘oh, there aren’t too many organisms,’” Patton said. “You need to know what species are present to learn how the problem happened and to understand potential patient risk downstream.” Charles River’s Accugenix® microbial identification and strain typing services deliver powerful genetic sequencing. This level of specificity, combined with the industry’s largest reference library of organisms relevant to drug manufacturing, provides the most advanced method of microbial identification available. The Charles River ID lab processes hundreds of samples per day and offers same-day and up to 5-day results, depending on the customer’s time frame. “We can take a sample plate and use genetic sequencing as well as MALDI-TOF technology to get a detailed result back within the day, if that’s what is needed,”

CASE STUDY Patton said. Those turnaround expectations are met 99.6% of the time with a 98% identification rate. Rapid testing turnaround is especially crucial when it comes to sterility testing and the impact on beyond-use dating. Confirming that a sterile product is saleable typically requires travel time to and from a lab and a sterility testing period of 14 days. This waiting period results in a shortened beyond-use dating window that may already be quite compressed. Charles River’s Celsis® rapid microbial detection systems deliver sterility testing results in 5 to 7 days, effectively cutting the waiting time in half. “This extra wiggle room allows more time to get the product to the hospital,” Patton said, which ultimately affects not only the bottom line but also patient well-being.

THE CONCLUSION Some compounding pharmacies still fall under state pharmacy board jurisdiction, but they are experiencing increasing scrutiny from the FDA that is not likely to change. When inspections happen, they are usually unannounced and will require the facility manager to produce records to prove that cGMPs are being followed. “We know it’s very important to able to produce documentation to show that a facility’s quality situation is tight,” Patton said. “Inspectors want to know what you are doing to assure quality and sterility, and they expect compounders to build quality into every procedure. This is one of the reasons the NECC got into trouble – their facility was not under control.” Testing can tell you when a problem exists with a product; it can also act as a valuable surveillance system. “You may get something in your microbial identification results that is not going to show up in your product, but they can correlate. This kind of data allows you to take action before a big problem occurs. This is the point of what we do.” Charles River clients are able to generate targeted reports through a web portal at any time. “Customers can do a deep dive into their own data,” Patton said. When it comes to proving compliance and control, she said, “it’s all about using that data to tell the story of your facility.”

University of New Mexico’s College of Pharmacy would like to recognize an Outstanding Preceptor — Dr. Daniel Co Dr. Daniel Co received his PharmD from The University of Georgia, completed a PGY1 residency at Northeast Georgia Medical Center, and is currently the Director of Pharmacy Services at the Advanced Care Hospital of Southern New Mexico, where his areas of interest are infectious disease, medication safety, and pharmacy practice experience. As a preceptor, Dr. Co focuses his rotations on fostering student growth. He is mindful that “oftentimes APPE rotations are the only opportunity many students have to focus on developing solid strategies for researching drug information questions, applying clinical guidelines to actual patients, and,” Dr. Co says, “present to and educate other healthcare providers.” Though Dr. Co has only been precepting students here for less than a year, feedback from students proves he is an inspiring preceptor and role model to his students. Dr. Co is valued for his investment in student learning, for the time he spends mentoring and guiding students, and for the way he challenges students to develop their critical thinking. One may have to go far to find someone as excited as Tom Giovinazzo about the new School of Pharmacy and Pharmaceutical Sciences (SOPPS) at Binghamton University. A community and consultant pharmacist at The Medicine Shoppe on Vestal Avenue in Binghamton, N.Y., Giovinazzo can hardly wait to work directly with SOPPS PharmD students during their internships. “I’ve been very excited about it,” he said. “When students are around, they’re always excited, have a good attitude and their outlook is positive. It rejuvenates my passion for the profession every time. Plus, this is my hometown and to put the pharmacy school in a seemingly overlooked area of Johnson City, N.Y., is very inspiring. It’s why I feel so compelled to be involved.” As a community and consultant pharmacist, Giovinazzo divides his time between working with patients at The Medicine Shoppe and consulting in long-term care settings such as nursing homes and adult care facilities. Dr. Shoffner has been associated with the college since its inception, serving on the Board of Visitors for the Dean and precepting both IPPE and APPE students since 2011. n Deactivates and Deters The activated carbon as well as the deterrent will arrive in a dry form at the bottom of the containers contained in pods. Once the activated carbon and deterrent have been activated, the containers are now ready for controlled substance waste to be added. The activated carbon will deactivate all forms of controlled substances including pills, patches, tablets, and IV solutions. The deterrent is a precaution for the unlikely occurrence of someone attempting to drink the liquid content, as it will physically prevent the drinking of the liquid in the container. n Containers and Brackets Our containers were designed with a one-way disposal path which allows for medications to go into the containers but not be removed. In addition, the lid is designed to prevent a sharp vial or ampoule from entering the container. The medication will not be deactivated if the denaturant does not come into contact with the disposed medication. This design feature prevents possible diversion opportunities from occurring. Meeting Pharmacy Compliance and Regulatory Challenges Comprehensive Pharmacy Services drives compliance with an end-to-end process: complete assessment, strategy development and implementation.

THE CHALLENGE: Achieving and sustaining compliance across the entire healthcare organization and lowering regulatory risk while improving operational efficiency and effectiveness are critical goals for hospital and health system pharmacies. When Dayton Children’s Hospital reached out to Comprehensive Pharmacy Services (CPS) for support in assessing compliance and regulatory issues for controlled substances, sterile compounding (USP <797> and <800>), The Joint Commission standards and 340B, CPS responded with custom solutions from its Compliance and Regulatory Services and 340B teams.

SOLUTIONS & RESULTS: Sterile Compounding (USP <797> and <800>) SOLUTIONS: In January 2018, the Compliance and Regulatory Services team provided a full assessment of compounding safety practices from procurement to administration in Dayton Children’s Hospital’s four compounding locations that included review of: •

Facility design

Workflow observations

Equipment and engineering controls

Environmental sampling

Cleaning and disinfection

Policy and procedures surrounding sterile compounding

Following the assessment, the Compliance and Regulatory Services team drafted a Compliance Report and developed a plan for process improvement designed to optimize safety, efficiency and effectiveness, and compliance. At the request of the organization, due to a plethora of ongoing projects on the current Director of Pharmacy’s agenda, the Compliance and Regulatory Services team offered a custom approach to managing the execution of 42

the project plan remotely. Currently an ongoing project, the Compliance and Regulatory Services team is virtually managing implementation and execution of the line items in the plan, ensuring that timelines are met and freeing up the Director of Pharmacy for other responsibilities. RESULTS: As of March 2018, the project plan is approaching 25 percent completion. In February 2018, the Board of Pharmacy surveyed one of the organization’s four sterile compounding locations. Because some of the items identified in the project plan were being acted upon, the surveyor issued no further citations. At another location, guidance is currently being provided for a full remodel of the sterile compounding suite which includes working directly with the pharmacy team, architects, engineers and facility administration.

CONTROLLED SUBSTANCE COMPLIANCE SOLUTIONS: In August 2017, CPS’ Compliance and Regulatory Services team performed a complete controlled substance assessment at the request of hospital leadership. Utilizing the Controlled Substance Diversion Prevention Assessment tool in the Comprehensive Pharmacy Assessment Platform, the Compliance and Regulatory Services team identified opportunities across the healthcare organization for improvement in management of controlled substances processes and developed an action plan, timeline and tools to track and manage implementation of the overall strategy. RESULTS: From August 2017 when the initial assessment was made until to February 2018, the Compliance and Regulatory Services team worked with the hospital and pharmacy leadership to identify 105 opportunities for process changes, including several best practices. While still an ongoing project, the team has implemented process to address 63 of these opportunities. Of the remaining 42 opportunities, all but 9 are currently in the process of being addressed.

340B SOLUTIONS: In October 2017, CPS’ 340B team per formed an initial evaluation, looking at both compliance and business opportunities. After digging deep into the on-site processes, the 340B team identified a number of manual functions that had not been evaluated previously. From the assessment, they put together a road map defining what needed to be done in what timeframe. The team ensured that the organization was hitting timelines for establishing policies and procedures as well as internal audit processes. The plan also identified ways to optimize 340B benefits and save significant dollars for the hospital. RESULTS: CPS’ 340B team was able to take a good 340B program to a great 340B program and identified $500,000 in additional 340B opportunities. The hospital has engaged CPS with a 3-year Continuous Readiness program to help the organization maintain a state of continuous audit preparedness. The 340B team meets with the pharmacy leadership every quarter to update where they are and provide assistance as needed.

THE JOINT COMMISSION SOLUTIONS: In February 2018, the Compliance and Regulatory Services team conducted a comprehensive assessment and follow-up with a gap analysis, and then drafted a strategy for meeting The Joint Commission Accreditation Medication Management standards. RESULTS: The strategic plan has been delivered to the on-site pharmacy director, and the Compliance and Regulatory Services team is assisting with implementation of that plan.

Product Specifications

Snap-N-Go products are concentrated, sterile solutions that come in single-dose vials. Each vial delivers a precise dose or desired concentration, that is not available commercially, when introduced into a diluent bag of choice. In the forward positions, the vials can be stored in automated dispensing machines (ADMs) for rapid retrieval, dose tracking, and subsequent administration to a patient by a nurse utilizing a standard vial-to-bag adaptor/activation process. Dilute-N-Go products are concentrated, sterile solutions that come in single-use syringes. Each syringe delivers a precise dose or desired concentration, not commercially available, when its contents are introduced into a diluent bag of choice. Dilute-N-Go products reduce the number of error-prone steps and the time associated with manual compounding by up to 85%. For added safety, each Snap-N-Go vial and Dilute-N-Go syringe is barcoded and includes GS1 barcode for track and trace and TALLman lettering to distinguish sound-alike, look-alike drugs from one another to prevent medication errors. We proactively provide a certificate of analysis and certificate of conformity detailing each product's purity, potency, sterility, and stability prior to receipt of the product.

Company Background

Excelera started in 2013 when several health systems came together to create the network concept. As this group traveled the country recruiting members, it quickly concluded that few health systems operated specialty pharmacies. For this reason, Excelera created a consulting offering to help health systems get into the business, become accredited, and truly maximize the opportunity. As the early adopters came on board, it didn’t take long for manufacturers to take notice. Pfizer was an early subscriber to the Excelera network model, and agreed to provide access to its oral oncology medications to participating network members with the expectation that Excelera would satisfy the data reporting requirements.
