CHALLENGES FOR PATIENTS USING INTENSIVE INSULIN THERAPY Diabetes is a challenging disease for patients and caregivers to manage. For some patients, the use of insulin is required to achieve glycemic control but it can have significant adverse consequences if not used appropriately. Incorrect administration of insulin (too little, too much, or at the wrong times) can result in both transient and serious hypo- and hyperglycemia, severe hypoglycemia, and diabetic ketoacidosis.

Patients with Type 1 (T1D) and Type 2 diabetes (T2D) who are using intensive insulin, defined as three or more injections of insulin per day or the use of an insulin pump, may have frequent unrecognized episodes of hypoglycemia during the day and at night. These frequent episodes can result in a patient’s awareness of hypoglycemia becoming impaired, where they have trouble “feeling their lows” and leaving them more at risk for severe hypoglycemia and its potential consequences of seizures, coma, or even death. The American Diabetes Association recommends that patients using intensive insulin monitor their blood glucose levels at least 6 to 10 times per day in order to make diabetes treatment decisions regarding diet, lifestyle, and the safe and effective use of their treatment. Unfortunately, due to the pain, burden and inconvenience of fingerstick testing, many patients test far fewer times than recommended to successfully manage their disease.

Due to the stress and consequences of hypoglycemia, many patients on intensive insulin and even their caregivers may develop a fear of hypoglycemia and will test with fingersticks throughout the night to prevent a seizure or even death due to a hypoglycemic episode. Unfortunately, most patients and caregivers then decide to use less insulin than prescribed, leaving the patient hyperglycemic with essentially uncontrolled diabetes.

CASE STUDY The New Standard of Diabetes Care: Continuous Glucose Monitoring in Pharmacy Practice

CHALLENGES FOR PATIENTS USING INTENSIVE INSULIN THERAPY Diabetes is a challenging disease for patients and caregivers to manage. For some patients, the use of insulin is required to achieve glycemic control but it can have significant adverse consequences if not used appropriately. Incorrect administration of insulin (too little, too much, or at the wrong times) can result in both transient and serious hypo- and hyperglycemia, severe hypoglycemia, and diabetic ketoacidosis.

Patients with Type 1 (T1D) and Type 2 diabetes (T2D) who are using intensive insulin, defined as three or more injections of insulin per day or the use of an insulin pump, may have frequent unrecognized episodes of hypoglycemia during the day and at night. These frequent episodes can result in a patient’s awareness of hypoglycemia becoming impaired, where they have trouble “feeling their lows� and leaving them more at risk for severe hypoglycemia and its potential consequences of seizures, coma, or even death. The American Diabetes Association recommends that patients using intensive insulin monitor their blood glucose levels at least 6 to 10 times per day in order to make diabetes treatment decisions regarding diet, lifestyle, and the safe and effective use of their treatment. Unfortunately, due to the pain, burden and inconvenience of fingerstick testing, many patients test far fewer times than recommended to successfully manage their disease. Due to the stress and consequences of hypoglycemia, many patients on intensive insulin and even their caregivers may develop a fear of hypoglycemia and will test with fingersticks throughout the night to prevent a seizure or even death due to a hypoglycemic episode. Unfortunately, most patients and caregivers then decide to use less insulin than prescribed, leaving the patient hyperglycemic with essentially uncontrolled diabetes. For many patients and caregivers, monitoring with fingersticks is difficult and does not provide enough information to manage their diabetes with the stress affecting their sleep, quality of life, and productivity at school and work.

vs. INTERMITTENTLY SCANNED CONTINUOUS GLUCOSE MONITORING There are differences in commercially available continuous glucose monitors (CGMs), which are available as real-time or intermittently scanned. Real-time CGM does not require any action on the user’s part for glucose data to be transmitted automatically to the receiver. An intermittently scanned CGM requires users to take an action to actively scan the sensor for their glucose data to be displayed. The Dexcom CGM system is the only real-time CGM available for patients 2 years and older. The user or caregiver is able to make diabetes treatment decisions with zero fingersticks and no calibrations.* The Dexcom G6 lets patients see their glucose numbers on their compatible smart device or receiver, receive alerts when their glucose levels are heading high or low, and share their data with up to 10 followers.

OPPORTUNITY FOR PHARMACISTS Pharmacists are considered one of the most trusted and accessible health care professionals. For this reason, they are afforded the opportunity to identify patients and caregivers who may be struggling with diabetes management and offer solutions such as real-time continuous glucose monitoring (CGM). CGM is increasingly recognized as the standard of care for people with T1D and insulin-requiring T2D and one of the most important advances in managing T1D since the discovery of insulin. Ronald Simbulan, RPh, Director, Mail Order and Specialty Pharmacies at RWJBarnabas Health in West Orange, New Jersey conducted an outreach program to employees and dependents who were using mealtime insulin. Patients were informed that CGM could help them manage their diabetes and if they agreed, the pharmacy would contact their provider to obtain a prescription for CGM. “We needed to help our employees and dependents with diabetes manage a difficult disease and, based on the evidence, we felt that recommending real-time CGM to those who could benefit was important,” Simbulan noted. The education and outreach by the pharmacy was appreciated, with many patients agreeing to have the pharmacist discuss CGM with their provider and to request a prescription. “Don’t assume that the patient’s doctor discussed CGM – we found that most patients who could benefit from CGM weren’t even aware that it was available as an option for them,” Simbulan advised.

DEXCOM REAL-TIME CONTINUOUS GLUCOSE MONITORING The Dexcom G6 is the only real-time CGM that is FDA approved for patients ages 2 and older. The user or caregiver can make diabetes treatment decisions without confirmatory fingersticks or calibrations.* The Dexcom G6 is approved for patient self-start and uses a simple one-touch auto-applicator to insert a small water-resistant sensor just beneath the skin. The sensor continuously measures glucose levels and sends data wirelessly to a display device through a transmitter. The display device is either a small touch screen receiver or a compatible smart device, which displays realtime glucose data with rate and direction 24 hours a day. The system alerts patients when it is time to change sensors and transmitters; the sensor is replaced every 10 days and transmitters are replaced every 90 days. The glucose readings with rate and direction help Dexcom CGM users proactively manage glucose highs and lows, plus give added insight into the impact that meals, treatment, exercise, and illness may have on their glucose levels. Dexcom CGM users are also notified with customizable alerts and alarms if their glucose is heading high or low, allowing them to take action to prevent and/or treat the high or low glucose levels. The CGM data can also be shared with up to 10 followers who will receive the user’s glucose data in real time and can then check in or assist if needed. Users can also allow their providers access to the last 90 days of their glucose readings via CLARITY software reports. The diabetes management application, CLARITY, lets providers and patients identify glucose trends and patterns to assist in making changes to the diabetes management plan.

CASE STUDY Who Should Use Continuous Glucose Monitoring? Continuous glucose monitoring (CGM) is recommended for patients with type 1 and type 2 diabetes who use any form of mealtime insulin (with or without a pump). Pharmacists can expand their patient-care relationships to identify, educate, and advocate for those who may benefit from CGM, as conversations and screening of hypoglycemia and suboptimal compliance can occur at the pharmacy quite readily.

Pharmacy profile-related indicators to identify patients who may benefit from CGM include: • Use of multiple daily insulin injections • Use of mealtime insulin • Use of an insulin pump • Fills for glucagon or glucose tablets • Excessive test strip use • Low test strip use • Erratic insulin fills • Reduced insulin fills

IN CONCLUSION There have been tremendous advances in CGM technology; in fact, CGM is an emerging standard of care in diabetes management, particularly for patients with T1D and T2D using three or more insulin injections per day or receiving insulin through a pump. Pharmacists are uniquely positioned to identify patients with diabetes who are struggling with the challenges of using their insulin safely and effectively and could benefit from continuous glucose monitoring. The value of real-time CGM glucose data cannot be understated, as studies have shown that CGM use can result in lower A1c values and reductions in hypoglycemia and hyperglycemia for patients of all ages, education levels, and mathematical ability. The Dexcom system is available through all pharmacy wholesalers, and access and coverage are increasing, with many insurers covering the technology through the pharmacy benefit. Additionally, Medicare currently covers CGM for patients with T1D and T2D who are using intensive insulin treatment. Q. What is the Hazardous Waste Pharmaceutical Final Rule and what does it do?

with their hazardous waste pharmaceuticals, which will keep more pharmaceuticals out of municipal waste landfills and sewers.

On February 22, 2019, EPA finalized the new Resource Conservation and Recovery Act (RCRA) regulations for managing hazardous waste pharmaceuticals. The final rule consists of three primary components:

Q. Who is affected by Subpart P of this final rule?

1.) Amends the hazardous waste listing for nicotine (Waste Code P075) such that FDA-approved over-the-counter nicotine patches, gums, and lozenges will no longer be considered hazardous waste when discarded. 2.) Creates new Subpart P in 40 CFR Part 266 (“Subpart P”), which contains new regulations that have been tailored specifically to the healthcare sector (including pharmacies and retail stores that sell pharmaceuticals) to improve the management of hazardous waste pharmaceuticals and thereby enhance protection of human health and the environment. Subpart P addresses the regulatory status of prescription pharmaceuticals going through reverse distribution and non- prescription pharmaceuticals going through reverse logistics. 3.) Establishes EPA’s Reverse Logistics Policy, which addresses the regulatory status under RCRA of all other unsold retail items going through reverse logistics. This final rule provides solutions to many of the hazardous waste management issues facing healthcare facilities, including retail stores that sell pharmaceuticals. Healthcare workers and retail pharmacy employees are often unfamiliar with RCRA hazardous waste management requirements, leading to inadvertent mismanagement of hazardous waste pharmaceuticals as municipal waste or medical waste. Subpart P streamlines the hazardous waste regulations for these wastes, reduces the compliance burden on healthcare personnel and healthcare facilities, and ensures that more hazardous waste pharmaceuticals are managed properly. Subpart P provides a practical system for the management of hazardous waste pharmaceuticals that is easier to comply with than the industry-oriented hazardous waste generator regulations. Accordingly, EPA encourages healthcare facilities to manage their non-hazardous pharmaceutical waste under the final rule, along

Healthcare facilities (including retail pharmacies) and reverse distributors will follow these new, tailored standards for managing their hazardous waste pharmaceuticals. Subpart P does not apply to pharmaceutical manufacturers (unless they act as reverse distributors), production facilities, or other generators of hazardous waste pharmaceuticals (e.g., farms or ranches). Those facilities remain subject to all other applicable hazardous waste regulations. In addition, the amendment to the P075 hazardous waste listing for nicotine applies to anyone (i.e., not just healthcare facilities or reverse distributors) who generates or manages discarded FDAapproved over-the-counter nicotine replacement therapies (i.e., patches, gums, and lozenges). Finally, the sewering prohibition in Subpart P is effective on August 21, 2019. This is the one provision in Subpart P that applies to ALL healthcare facilities and reverse distributors, including healthcare facilities that are classified as Very Small Quantity Generators (VSQGs). VSQG healthcare facilities are not required to comply with the other provisions of Subpart P.

Q. How will discarded over-the-counter nicotine replacement therapies be regulated? This final rule exempts FDA-approved over-the-counter nicotine replacement therapies (i.e., nicotine patches, gums, and lozenges) from the RCRA Hazardous Waste Code P075 for nicotine. Thus, under the federal hazardous waste regulations, generators (e.g., retail stores, hospitals) may discard these wastes as nonhazardous. EPA recommends, however, that facilities take steps to prevent unauthorized access to the waste containers, dumpsters, etc., to prevent illicit diversion (i.e., dumpster diving). Nicotine continues to be a listed acute hazardous waste with the waste code P075, so any nicotine waste, other than nicotine patches, gums, and lozenges, is still regulated as P075 acute hazardous waste (e.g., liquid nicotine in electronic cigarettes, nicotine pesticides).


Subpart P protects human health and the environment by improving compliance at healthcare facilities (including hospitals, clinics, and retail stores with pharmacies) and reverse distributors that generate and manage hazardous waste pharmaceuticals. Specifically, the rule decreases the risk of illicit diversion of unused prescription hazardous waste pharmaceuticals by requiring basic tracking of these pharmaceuticals from healthcare facilities to reverse distributors. Subpart P will reduce the negative impacts caused by hazardous waste pharmaceuticals that enter the environment through flushing down the sink or toilet, and is projected to prevent the flushing of 1,600 to 2,300 tons of hazardous waste pharmaceuticals annually.

Q. What is the definition of healthcare facility under Subpart P of the final rule? In Subpart P of the final rule, a healthcare facility is defined as any person who is lawfully authorized to: (1) provide preventative, diagnostic, therapeutic, rehabilitative, maintenance or palliative care, and counseling, service, assessment or procedure with respect to the physical or mental condition, or functional status, of a human or animal or that affects the structure or function of the human or animal body; or (2) distribute, sell, or dispense pharmaceuticals, including overthe-counter pharmaceuticals, dietary supplements, homeopathic drugs, or prescription pharmaceuticals. This definition includes, but is not limited to, wholesale distributors, third-party logistics providers that serve as forward distributors, military medical logistics facilities, hospitals, psychiatric hospitals, ambulatory surgical centers, health clinics, physicians’ offices, optical and dental providers, chiropractors, long-term care facilities, ambulance services, pharmacies (including long-term care and mail-order), retailers of pharmaceuticals, veterinary clinics, and veterinary hospitals. This definition does not include pharmaceutical manufacturers, reverse distributors, or reverse logistics centers.

Q. What is the definition of pharmaceutical under Subpart P of the final rule? In Subpart P of the final rule, pharmaceutical is defined as: any drug or dietary supplement for use by humans or other animals; any electronic nicotine delivery system (e.g., electronic cigarette or vaping pen); or any liquid nicotine (e-liquid) packaged for retail sale for use in electronic nicotine delivery systems (e.g., pre-filled cartridges or vials). This definition includes, but is not limited to, dietary supplements, as defined by the Federal Food, Drug and Cosmetic Act; prescription drugs, as defined by 21 CFR 203.3(y); over-the-counter drugs; homeopathic drugs; compounded drugs; investigational new drugs; pharmaceuticals remaining in non- empty containers; personal protective equipment contaminated with pharmaceuticals; and clean-up material from spills of pharmaceuticals.

This definition does not include dental amalgam or discarded needles (“sharps”).

Q. Which pharmaceuticals are considered hazardous waste? A solid waste is considered hazardous waste if it meets a hazardous waste listing or exhibits a hazardous waste characteristic described in title 40 of the Code of Federal Regulations Part 261. It is the generator’s responsibility to determine whether the waste that they generate is hazardous under RCRA. There are approximately 30 commercial chemical products listed on the P and U hazardous waste lists (see 40 CFR §261.33) that have pharmaceutical uses; however, this number does not completely represent the total number of brand name pharmaceuticals that may actually be listed hazardous wastes when discarded. Waste pharmaceuticals may also be hazardous because they exhibit one or more of the four characteristics of hazardous waste: ignitability, corrosivity, reactivity, and toxicity.

Q. What is the difference between Reverse Distribution and Reverse Logistics? Subpart P adopts the terminology commonly used in the industry. “Reverse distribution” is used to refer to the reverse flow of prescription pharmaceuticals. Unused prescription pharmaceuticals are sent to reverse distributors to determine if they are eligible to receive manufacturer credit. Because they are not redistributed by the reverse distributor for reuse, the prescription pharmaceuticals are considered waste at the healthcare facility before they are sent to a reverse distributor. That said, these wastes have value in the form of potential manufacturer credit, which has allowed EPA to take a more tailored and flexible regulatory approach for this particular type of hazardous waste pharmaceuticals.

Q. What benefits does EPA expect from the new management standards for hazardous waste pharmaceuticals in Subpart P?

“Reverse logistics” is used to refer to the reverse flow of nonprescription pharmaceuticals and other unsold retail items. Reverse logistics centers evaluate whether items can be sold on the secondary market, donated or recycled. If non-prescription pharmaceuticals and other unsold retail items have a reasonable expectation of being legitimately used/reused (e.g., sold or donated) or reclaimed, then they can be sent to a reverse logistics center without being considered waste at the healthcare facility. On the other hand, if the non-prescription pharmaceutical or other unsold retail item does not have a reasonable expectation of being legitimately used/reused or reclaimed (e.g., it is broken or leaking), then it is considered a waste at the healthcare facility and may not be sent to a reverse logistics center. If the waste is a hazardous waste pharmaceutical, then the healthcare facility must manage the non-prescription hazardous waste pharmaceutical under the new Subpart P regulations for pharmaceuticals. Other unsold retail items that are considered hazardous waste (i.e., non-pharmaceutical hazardous waste) at the healthcare facility must be managed under the existing applicable hazardous waste regulations.

The easy-tofollow, step-by-step illustrated instructions allow the user to set up assays within minutes. The Pyrosate® Kit is available in sensitivities of 0.03 EU/mL, 0.125 EU/mL, and 0.25 EU/mL, in both 10-pack and 30-pack formats. In addition to research and water testing, Pyrosate® is ideal for dialysis centers, compounding pharmacies, and pharmaceutical and medical device companies.

Performing the Test

The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.

Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage.
