In order to participate in 340B and continue receiving reimbursement, El Rio must meet a variety of exacting criteria and be recertified annually. “The two biggest requirements are meeting patient definitions and avoiding duplicate discounts,” Carzoli said. “We must have auditable records for every single medication we dispense to every patient, down to the tablet.”

THE DECISION El Rio’s previous pharmacy management software system was rife with frequent filing mistakes involving sound-alike or spelled-alike patient and drug names. These mistakes resulted in potentially dangerous dispensing errors. “It was death by 1,000 paper cuts. We filed alphabetically, and the previous system did not let us know if we had multiple patients in the same bag or alert us to check the patient’s identity,” Carzoli said. “The vendor was unable to make adjustments to safeguard our patients.” In addition, El Rio’s volume was increasing. “The system we had simply could not keep up. We would have needed to continually add more staff.” El Rio was also dealing with a manual prescription refill system that required a dedicated technician to spend an entire day listening to patients’ voice messages and typing prescriptions into the system. “Keeping our tech staff tied up with manual tasks like that meant that our most knowledgeable and expensive resources – our pharmacists – were spending time counting tablets and pouring liquids,” said Carzoli. These needs for technological efficiency, increased safety, and improved patient satisfaction drove an exhaustive search for a better software solution. “We required more robust clinical checking and were looking for ways to improve our adherence and star rating,” Carzoli said. “We also wanted better messaging and education for our patients.” El Rio’s information technology team was heavily involved in making the final decision, based strongly on their concerns around patient data safety and reliable functionality.

THE RESULTS Switching to Liberty Software in 2015 improved El Rio’s efficiency, while providing more comprehensive patient care. “The same number of pharmacists now do more clinical review and have time to work directly with patients to identify their needs and improve their health care,” Carzoli said. The new software included a custom bin management system that uses dates and slot numbers rather than patient names. “We went from having multiple wrong dispensations per year to a single one since using Liberty,” Carzoli said, “and that error only happened because a staff member overrode the safety system.” Liberty Software was also willing and able to customize El Rio’s patient keypad, adding four prompt questions that patients must answer when receiving medication to verify their identity and the name of the drug. “This is another huge advancement in our technology that is responsible for dropping our safety events to virtually nothing,” Carzoli said. In addition, the time wasting refill system has been replaced with an automated smart phone app and auto-refill features. “Now, patients enter an order and it goes directly into the system, to be safely processed right away.”

THE CONCLUSION One of the tenets of participating in the 340B drug program is the requirement for healthcare entities to provide patients with continuity of care. Every step, however, must be documented. “With Liberty Software, we can now access and review information seamlessly from our patients’ electronic health records,” Carzoli said, “This lets us make better clinical decisions while also meeting the program’s data collection requirements.” Looking ahead, Liberty Software continues building tools that help El Rio continue to improve and grow. They have developed an API to better connect the larger EHR system with Liberty’s platform. “This will take us light years toward integrating bidirectional information movement between the systems,” Carzoli said. “This was another customization that was a dream of mine for some time. We asked for it and Liberty agreed to make it work.”

Q. What makes an automated will call system turn-key and user-friendly?

I would say that it’s intuitive, just simply intuitive. What you want it to do and how you would want it done, the will call system does it. The sorting feature that our system has, makes it easy to identify prescriptions that haven’t been picked up in 10 days or more, bags that have low batteries, and prescriptions that need to be returned to stock. This feature saves my staff valuable time that we did not have when using a very basic will call system.

Q. How do you select an automated will call system which is expandable and continuously upgraded?

When I was looking to upgrade my will call system from a basic system to an advanced system, we looked at the following: • F unctionally: How the system works, features, and benefits. • Bags: How bags are powered (external vs. battery), size, and space. • Cost: Positive ROI, competitive price. • Partnership: A positive experience to get the system up and running, facilitate it, and troubleshoot it.

Q. Given Nebraska fills and dispenses a high volume of prescriptions every day, how did your choice of will call automation ensure patient safety and accuracy?

When updating our will call system, it was very important to have safety features. The current will call system we use has a required two point identification feature,

which verifies both the name of the patient and DOB. The patient must give this information to staff or the medication bags cannot be found. Although, our error rates are astronomically low, the most common problem was the wrong patient getting or almost getting the wrong medication. With an innovative will call system in place, our retrieval rates are quicker and we are saving a few seconds of time, hundreds of times a day.

Q. Will patients appreciate Nebraska’s investment in their safety, yet receive quick and exceptional service?

The response from our patients has been positive, but interesting enough – some patients haven’t noticed yet! However, the patients that do notice like the utilization of technology, they find it novel and state-of-the-art. We have tripled the size of our pharmacy and added multiple new technologies, and our will call system is by far the most effective and positive aspect. Our advanced will call system is something our patients and staff alike can appreciate.

Q. Why do you think it’s important for a high-volume pharmacy to have a will call system in place?

When a pharmacy hits a high-volume status it is extremely important to stay focused and organized. First and foremost, being a high-volume pharmacy has another level of organization. Organization is always key for efficiency, but I think at some point it becomes more about liability. Being a disorganized pharmacy is one thing, being a high-volume pharmacy that is disorganized, just is not feasible. Adding the will call system gave our pharmacy that few extra minutes to add

– drawing them to something new and innovative, and improved morale. I hope that when all is said and done and I get a chance to look back, the thousands of times we’ve saved 10 seconds here, 30 seconds there, we were net-positive in the investment.

I’d argue that purchasing a will call system is not an expense, but an investment. With the amount that you spend on the system, you get back more of the employee’s time and satisfaction. Installing an advanced will call system has helped with employee engagement

All items are boxed and sealed, and a FedEx pickup is scheduled for the next business day. Medical Arts Pharmacy is an independent community pharmacy that services patients of all ethnicities and backgrounds with chronic conditions and acute self-care ailments. He received his pharmacy training from the University of Delhi, India. Dr. Singh is an active member of the California Pharmacists Association (CPhA) and National Community Pharmacists Association (NCPA). He has also served as a board member for nine years with the Indian Pharmacists Association and two years as the secretary. Dr. Singh is the former President of South East Pharmacy Association. Sofia Gezalyan, PharmD, BS, RPh, APh (IPPE 2- Institutional Pharmacy Practice)

Dr. Gezalyan received her Bachelor of Science in Health Promotion and Disease Prevention from USC in 2002 then earned her Doctor of Pharmacy degree from USC in 2006. Upon completion of her PharmD in 2006, Dr. Gezalyan started her career at Hollywood Presbyterian Medical Center (HPMC) as a Clinical/Quality Assurance Pharmacist. In 2007 she was promoted to Clinical Coordinator of Pharmacy Services. In that position, she aggressively expanded the Clinical Pharmacy program. Dr. Gezalyan was promoted a dual role, as the Manager of Inpatient Pharmacy Services and the Clinical Coordinator where she developed and implemented the Bedside Pharmacy Program. This concept was well received and adapted by many hospitals since then. In 2017, she was further promoted to Director of the Inpatient Pharmacy Services. In 2018, she became a licensed Advance Practice Pharmacist.

Lawrence Thompsen, BSc, RPh, MBA (IPPE 3- Selective Elective Pharmacy Practice)

Dr. Thompsen is a licensed pharmacist in multiple states and has more than 40 years of experience in hospital practice, management, home infusion, hospice care, nuclear pharmacy, and state and federal drug law enforcement. He has been a pharmacy lecturer and preceptor for more than 35 years. Dr. Thompsen is a member of numerous national, state and local pharmacists associations. He is also a dedicated member of the California Disaster HealthCare volunteers, and LA County EMS Agency. He is currently the Sr. Clinical Director and Vice President of Operations at a national home infusion provider in Los Angeles. Dr. Thompsen received his pharmacy degree from the University of Illinois, and later earned his Nuclear Pharmacist certification at Purdue University. Dr. Thompsen went on to earn MBA in International Health Care Finance from Keller Graduate School of Management.

Dr. Villasenor graduated from Cal State University Long Beach in 2009 with a Bachelor of Science in Microbiology with a Minor in Chemistry, and a Bachelor of Arts in Spanish Language. She went on to work as a pharmacy technician for one year at County LAC+USC before returning to school to pursue her doctor of pharmacy. In 2014, Dr. Villasenor earned her PharmD from Loma Linda University with aspirations to enter into the ambulatory care field. Dr. Villasenor is also a certified pharmacist in the American Academy of HIV Medicine. She is currently working as the Pharmacist in Charge at Aids Healthcare Foundation. In her spare time, Dr. Villasenor teaches Pharmacotherapy in Infectious Disease as an adjunct professor at Charles Drew University in the Physician Assistant program. She joined West Coast University School of Pharmacy as an Advanced Pharmacy Practice Experience preceptor in 2016.


The College of Pharmacy at Roosevelt University partners with many Community and Ambulatory Care pharmacists and sites to provide quality Core and Elective pharmacy experiences for our Advanced Pharmacy Practice Experience (APPE) students. Our recipient for this year’s APPE Outpatient Preceptor of the Year Award has been an APPE Preceptor for Roosevelt College of Pharmacy for the past three years. She is a preceptor that students enjoy having, because she challenges them to think about the whole patient. Dr. Natasha Niedbalec is recognized by her students as someone who motivates and provides helpful feedback. After observing her confidence, clarity and compassion with patients for just one day, I immediately wanted to practice, and practice the way she did. Her instinct was to always hear my interventions and recommendations first, and would follow it up with her own reasoning and advice. She encouraged and empowered me to use knowledge and skills I gained in the classroom and gave me a comfortable platform to apply it. Regardless of how busy Dr. McCafferty’s day was, she never hesitated to stop what she was doing to answer any of my curiosities.
