CASE STUDY Make finding the right injection product fast and easy. The BD Easy Dispensing Packaging System is designed to simplify a complicated product category. Work smarter, not harder.

THE CHALLENGE Patients with diabetes inject insulin using a pen needle or insulin syringe. Both types of injection devices are available in various needle lengths and gauges. Barrel capacities are unique to insulin syringes and add another variable to the process of identifying the correct device for the patient. For busy pharmacists, however, having so many variations and SKUs can make it challenging to locate the exact pen needle or insulin syringe your patient requires. Note: ADA recommendations for patients are to inject with shorter needle lengths (4mm pen needle/6mm insulin syringe needle)1. BD Easy Dispensing Packaging System

BEFORE

THE SOLUTION Medical technology company BD has redesigned their insulin syringe and pen needle packaging to help pharmacists readily identify and dispense the appropriate product for each patient. The BD Easy Dispensing Packaging System organizes pen needles and insulin syringes using bold colors to indicate pen needle length and insulin syringe barrel capacities. The packaging features a combination of color-coding unique to device type and prominent numerals indicating needle lengths. This packaging design was made to help easy product recognition and may help to simplify complexity when arranging pharmacy shelves by lengths of BD Ultra-Fine™ Pen Needles and barrel capacities of BD Insulin Syringes with BD Ultra-Fine™ needle. In a single case study, BD representatives visited Gerard O’Hare, pharmacist at Jeffrey’s Drug Store in Canonsburg, Pennsylvania who dispenses between 30-35 boxes of pen needles and insulin syringes every week. • O’Hare was asked to retrieve from stock, a box of BD insulin syringes with a 0.5 mL barrel and 8-millimeter length needle. What he didn't know was he was being timed. (see image BEFORE) • After doing so, BD reps showed him the BD Easy Dispensing Packaging System and he agreed to have his shelves reorganized to match the image he was shown. (see image BD Easy Dispensing Packaging System) • O’Hare was then asked again to retrieve the same box of BD Insulin Syringes. Once again, he was unaware he was being timed. This time locating the BD insulin syringes took nearly one-third as long2 with the BD Easy Dispensing Packaging system in place than it did the first time. (see image AFTER)

With O'Hare's permission the product was put back on the shelves in the original position they were found. O'Hare was then asked to reset the shelves according to the BD Easy Dispensing Packaging System Image so he could fully understand how the products should be arranged, which he did. The change took just a few minutes. The BD Easy Dispensing Packaging System may be helpful to pharmacy staff when searching for a product on shelf. Efficiency is important to the operation of most pharmacies and providing the appropriate product to the right patient is always a top priority. O’Hare said, “The system looks good, it’s a no-brainer.”

Case Study Driving Operational Efficiency Through Innovative Solutions Operating a pharmacy in today’s healthcare space can be challenging and time consuming. Managing ever changing drug pricing and payment issues add to the challenges. Operational efficiency is the key to achieving success.

The Situation A high volume pharmacy engaged Net-Rx™ to provide better visibility into their claims reimbursement and reconciliation process in an effort to increase claims accuracy and reconcile payment issues.

» Managing the Software and Improving Visibility The pharmacy was using reimbursement tools and a separate reconciliation software that they lacked confidence in, and which were not meeting their needs. The software had no user interface and it was difficult to draw actionable insights from the monthly reports. The pharmacy was looking for a solution that would provide them with a clearer, simpler view of their data, and a timely report out of actionable items.

» Increasing Claims Accuracy to Prevent Lost Reimbursement Opportunities The pharmacy noticed discrepancies in their claims data and lower than expected reimbursements. They became concerned that some or all of their claims were not submitted accurately and/or were not being reimbursed according to current and accurate industry prices. In addition, they needed assistance keeping track of price changes. » Identifying Missed and Partial Payments As pharmacy operations took precedence, the pharmacy had limited time to reconcile third party payments. They needed a solution that could quickly and accurately identify missed and partial payments.

» Dedicated One-on-One Customer Support The pharmacy met with a dedicated Net-Rx analyst bi-weekly to review their claims data and ask questions. The analyst helped the pharmacy understand, identify and correct billing issues and database errors recovering funds and preventing future losses. » Exception Based Reporting Easily Identifies Billing Discrepancies Net-Rx’s EditRx claims analysis service helped the pharmacy by analyzing each of their claims and providing them with exception based reporting a detailed list of claims organized by the specific billing error and action item - which they are able to use to rebill and recover lost revenue.

» AWP Verification and Retro Reports Track Price Changes The pharmacy used the AWP (Average Wholesale Price) Verification report to keep track of AWP changes and update their drug records when automated processes fail. The report minimized losses and ensured that future claim submissions, even for non-electronic claims, were correctly priced. Additionally, eligible claims identified by the analyst were rebilled for higher reimbursement.

» Data Visibility and Accuracy Leads to Increased Confidence The pharmacy owner was pleased with Net-Rx’s dashboard and the various reports available. While initially reviewing all of the reports personally, and relying on the Net-Rx analyst, now members of the pharmacy staff run and analyze reports independently and require limited support from the Net-Rx analyst.

The AWP Retro Report identified claims not paid according to the most recent AWP due to a lag by the payer in updating their internal records. These claims were tracked by Net-Rx and when the payer updated their files, the pharmacy rebilled them to capture the additional reimbursement.

» Increased Reimbursement, Reduced Losses In a two year period, the pharmacy was able to rebill over 400 claims and recover almost $30,000 in revenue. With each month’s reports, the corrected billing issues prevented future losses on refills. Corrections to claims that had refills led to a projected revenue increase of nearly $70,000.

» Aging Report Highlights Payment Discrepancies The pharmacy regularly viewed the Aging Report, an automated, transaction level report that identified unpaid and underpaid claims as well as funds taken back by the payer. It gave the pharmacy better insight into their cash flow, and made it clear which claims needed to be addressed and why.

» Reduced Lost Revenue, Improved Cash Flow Management Net-Rx’s aging report improved the pharmacy’s visibility into payments by helping them to track payments and reduce lost revenue. As a result, the pharmacy was better able to manage their cash flow and make informed spending decisions.

Q. Why did Walberg Family Pharmacies start using medication packaging?

When I graduated from pharmacy school, I knew there had to be a better way to help my patients adhere to their medication regimens. I started by filling my patient’s medications in pill organizers, but many patients required multiple organizers, which caused more problems and confusion. Strip packaging seemed like the best option for an adherence program because it drastically reduces the margin for error while taking medications.

Q. Can you tell us more about Care-Fill Custom Medication Packaging?

Care-Fill is a medication management service that packages medications into dose packs labeled by day, time, and dose. My wife, Stacey, and I co-founded Care-Fill® in 2012 when I bought my first medication packaging robot. Care-Fill isn’t just the packaging of medications, it’s the process of managing those medications to make sure our patients adhere to their medication schedule and stay healthy. We do everything needed to make sure our patients get their medications on time. We call the patient every month to check for changes to medications or dosage, we contact their doctors if they need refills, and we will work with their insurance company if needed. Our medication management service is free, and delivery or shipment is also free. Our Care-Fill service has allowed us to realize efficiencies by synchronizing patient medications. And, fewer prescriptions are left unfilled due to medication mismanagement. Care-Fill promotes increased adherence, so we see more fills per calendar year.

Q. What features/benefits were important to you when looking for the right automation/robotics?

1. ROI: We needed to see significant improvement in staffing ratios – specifically prescriptions per staff hour. 2. As we added more machines, it was crucial that the individual units were able to communicate and work together seamlessly – to act as one cohesive system.

Q. What was the process of implementing the technology into your pharmacy? How did the staff adjust to onboarding?

We started small, with only a couple of staff members working out of our Jamestown Pharmacy location. We began packaging medications for close family and friends to get their feedback. It was a slow process at first. We were constantly refining our procedures to find the best way to do things. Once we began marketing the service to our patients and our community, Care-Fill grew significantly. Since 2012, we have outgrown our space and relocated three times due to the need for additional staff and equipment. We are now preparing for our fourth move this summer. Our new 7,000 square foot facility has a customized floorplan to increase efficiency and manage workflow.


Currently, we are focused on synchronization and central fill. With the new facility’s completion on the horizon, we are also working on expanding Care-Fill into new markets and long-term care.

Q. Have you seen the use of multipackaging technology increase your patient’s medication adherence?

Absolutely! By managing and packaging their medications, there is little room for error. Between good to go calls and syncing, Care-Fill ensures our patients don’t go without their medication.

Q. How do you remain competitive in the industry?

Our drive and proactive approach to technology give us a competitive edge. We never stop looking for new ways to improve efficiency and workflow. We believe we have the right pharmacy system and the right robotics to acquire new patients. We also need the right staff to retain our current patient numbers. The way we treat our customers is what separates us from the larger pharmacies. Since our reputation is dependent on the decisions and actions of our staff, they are a critical component of our success and ability to compete. What we do (and how we do it) matters.

Q. What impact has COVID-19 had on your business?

It has been a fluid process at our retail pharmacies. We have seen a considerable drop in prescription volume and are constantly creating and modifying our procedures to make sure that our customers feel safe and comfortable receiving their medications. We have implemented new services, such as curbside pickup, and adapted our free delivery service to be contactless. Adherence packaging, on the other hand, never skipped a beat and is continuing to grow. We had already established the process to ensure proper procurement, packaging, professional care, and delivery, so Care-Fill has been impacted very little throughout the pandemic.

Keeping medication in proper temperature range — winter, spring, summer and fall

ltru Specialty Pharmacy is a part of Altru Health System, which serves people in the city of Grand Forks as well as much of North Dakota and extending into Minnesota. What began seven years ago as a grassroots effort with one technician working in partnership with the PharmD has grown to include three full-time technicians and three full-time pharmacists. Still, the specialty pharmacy’s mission has remained the same — to enhance care and improve the overall health and wellbeing of their patients.

on Joint Commission accreditation requirements in addition to US Pharmacopeia (USP) best practices for good storage and distribution of drug products, which many accreditation agencies use as a guideline. “We live and work in a climate that can be -40ºF in the winter and 100ºF in the summer,” explains Rosemary. “To be sure we are giving our patients the best possible care, it’s important to know we are sending drug products in such a way that they remain both effective and safe.”

Package Performance Qualification The specialty pharmacy did its first round of testing in 2019. “We started with our refrigerated packouts, which need to be kept between 2ºC and 8ºC. We tested in cold weather to make sure packages were staying in range for the amount of time we anticipated shipments to take,” says Rosemary. In Phase 1, Altru included two different wireless sensors. An M-300 measured temperatures inside the medication packaging at regular intervals and an S-400 monitored the external ambient temperature in the outer box. Test packages were sent to Temptime, who analyzed the data captured. “We learned a lot in that initial evaluation of our packaging,” Rosemary says. Temptime gave Altru guidance on coolers and other packing materials best for cold chain shipments to help improve their packout design. “We made a few tweaks with the intent of giving patients even more confidence in the medications we deliver — and Temptime was a great help through the entire process.” Altru then moved on to Phase 2 for further testing of the passing Phase 1 package design. In Phase 2, test packages were prepared in the same way, but this time shipped to various geographies using different modes of transportation such as courier and FedEx. Again, Temptime collected the time-temperature data, using the information to prepare a PPQ Report that validates the qualification of the packouts.

Aside from ensuring packages performed the way they needed them to, Rosemary says one of the most significant benefits has been instituting best practices throughout the shipping processes. “Now every shipment of refrigerated medication is packed the same way,” she says. “In the past — there were no specific guidelines. So one person might include one icepack in a shipment where another might put in three, which could cause medication to freeze.”

Altru is currently finishing its second round of PPQ testing. “We’re in the process of testing controlled room temperature (CRT) and refrigerated shipments during the summer to ensure packouts are keeping medication from getting too hot,” says Rosemary. “With all we learned in our previous testing, the process this time around was much smoother and we successfully passed Phase 1 on the first try.”

In the end, she says, PPQ testing just delivers peace of mind – for the pharmacy as well as for patients.

Results demonstrate that the Altru Retail Pharmacy shipping boxes are capable of providing the desired temperature environment for shipment of temperature sensitive products, maintaining temperature within 2°C to 8°C for a minimum of 33-hours. All recorded temperatures inside the shipping containers were within the lower limit of 2°C and an upper limit of 8°C for up to 33-hours.

The specialty pharmacy was also pleasantly surprised to find that the PPQ testing would help them with their URAC accreditation, which they began working toward in October of 2019. “A lot of URAC’s requirements are centered on providing documented proof that medication temperatures are monitored and maintained, and everything we’re already doing will apply.”

Next Steps Altru’s next step will be to test CRT packages during the colder seasons and retest refrigerated packouts. “Regular, seasonal PPQ Testing is just so important,” Rosemary says. “Our patients trust us — and we need to continually earn that trust with every single shipment that goes out.” More than that, she adds, most of the drugs they deliver — such as cancer medications — are critical to patients. “They have to arrive on time and in good condition, it’s imperative.”

In addition to research and water testing, Pyrosate® is ideal for dialysis centers, compounding pharmacies, and pharmaceutical and medical device companies.

Performing the Test

The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.

Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage.
