Launched in September of 2018 as an offshoot of the Florida Cancer Specialists & Research Institute, AON Pharmacy is a specialty pharmacy dedicated entirely to dispensing oral oncology medications. Based on their rapid growth, this type of “specialized” specialty pharmacy is clearly an idea whose time has come. “We started with just three employees, shipping medication to patients in about nine states via Fed Ex,” says Doug Braun, Pharmacy Manager for AON Pharmacy. “As we approached our first anniversary, we had 17 employees and plans in place to expand our reach to all 50 states.”

When a long-term care (LTC) or closed-door pharmacy partners with a healthcare facility, a federally mandated requirement obligates the pharmacy to provide medications to that facility within a certain amount of time. To remain in compliance, the LTC pharmacy must be able to respond at any time of day. The process of fulfilling this requirement can be challenging for these pharmacies, both financially and logistically. TracRx™, a secure, web-based portal by Care Services, parent company of MedCallRx and GeriScriptRx, allows LTC pharmacies to schedule backup medication fulfillment and delivery and provides visibility into the entire process – from order placement to point of delivery (POD) signature. Powerful analysis reporting identifies trends in drug use and provides opportunities to improve operational performance and manage costs.
THE CHALLENGE When a LTC pharmacy receives a stat or after-hours drug request, the need usually involves an urgent situation where a patient may be unable to wait until the next scheduled delivery to receive medications. In these situations, the drug may not be available in a facility’s on-site emergency kit or dispensing unit, and the industry practice for emergency medication delivery is less than four hours, or at the next scheduled dose.

The portal’s userfriendly dashboard makes managing and monitoring a request easy and documenting important data simple. TracRx allows long-term care and closed-door pharmacists to: • View backup medication status. • Schedule and track deliveries. • Set customized notifications. • Analyze all medications and deliveries down to a facility level. • Create utilization and fulfillment reports.
For LTC pharmacies serving facilities in rural areas, or for requests that occur during off hours, driving into the pharmacy to fill and deliver medications is onerous and costly. “Choosing to partner with a backup pharmacy provider is an important business decision for many LTC pharmacies,” said Jay Palin, president of backup pharmacy services at Care Services. “Pharmacies have to weigh the cost of staffing an on-call or around-the-clock pharmacist versus using a backup pharmacy partner for after-hours or emergency support.” With standard backup pharmacy processes, following up on a medication order to check whether it has been filled, or to confirm pickup and delivery, is most often done by phone calls to both the delivery service and the dispensing retail pharmacy. “Typically pharmacy or delivery notifications are delayed,” said Palin. “In order to track status during the fulfillment and delivery process, the LTC pharmacist has to make several phone calls, which is clearly inefficient and time consuming.” Further, all too frequently, when LTC pharmacies decide a backup pharmacy order needs to be placed, decision points are not typically documented. “If the reason for using a backup pharmacy is not documented, it’s difficult for LTC pharmacy leadership to make operational decisions to improve future outcomes,” said Palin.
THE DECISION Care Services realized there was an opportunity to streamline the management of backup services and create more transparency in the process. “Given all that is on their [LTC pharmacies] plates, creating a new backup pharmacy process was not on the top of anyone’s list,” said Joe Boures, chief executive officer of Care Services. Boures decided to convene a client advisory council to learn more about the challenges faced by the company’s clients and tailor the company’s new technology portal to solve previously unmet needs. “Our clients were not bashful in providing a lot of insight about what would help them be more efficient and capture the data they needed.” The goal was to find the best entry point requiring the least amount of effort for LTC pharmacies with limited resources. To that end, Care Services created a user-friendly digital interface in TracRx that is poised to make backup pharmacy spreadsheets and duplicative data entry obsolete.

CASE STUDY THE RESULTS A new learning curve is never appealing to pharmacists who are already busy. “The biggest challenge [to TracRx] is probably change management,” said Boures, “until our clients see how easy and intuitive the portal is to use, the benefits they receive through proactive notifications and reporting, and realize that it takes less than 10 extra seconds per order to save them multiple time consuming phone calls afterward.” The TracRx system automatically lists all pharmacies in network by proximity, eliminating the need to search manually. “We currently have 70,000 pharmacies in our network,” said Palin, “and the portal updates automatically to include any new ones. A spreadsheet can’t do that.” The TracRx system is also fully integrated with delivery services provided by Complete Delivery Solution, the medication delivery division of Care Services, enabling LTC pharmacies to schedule and track deliveries seamlessly. From the C-suite perspective, the ability to produce quarterly business reviews is invaluable. “Using old processes or systems, pharmacists wouldn’t be able to answer questions about why certain meds were being sent for backup or how long it was taking,” Palin said. “With TracRx, medication fulfillment and drug delivery data can be analyzed down to the facility level.”
THE CONCLUSION Following our initial roll-out to a small group of clients, upgrades to the TracRx system continue to happen in response to users’ priorities. “This is an ongoing process,” said Boures. “So far, we have been able to integrate the majority of requests.” Currently, the TracRx portal is available for clients of Care Services’ affiliate companies, GeriScriptRx and MedCallRx. “Our plans are to roll TracRx out to all of our clients, and simultaneously work with other companies to get this service out to another swath of pharmacies,” said Boures. Ultimately, Care Services sees the TracRx portal as a game changer that will provide big benefits to all of their clients. “ We want to make TracRx available to anyone [in the LTC pharmacy space] using backup pharmacy services,” said Palin.

Independent pharmacy is one of the most competitive markets in healthcare. In any town or city, you’ll commonly find two (or more) competing big-name chain pharmacies located on the same block, vying for consumer loyalty. Despite this intense pressure, independent community pharmacies are not becoming a thing of the past. Participants in this nearly $80 million sector simply need to work smarter in order to rise above the noise. Co-branding with a larger, recognized organization is a tactical approach to retaining a pharmacy’s independent identity, while benefiting from a powerful partnership. Cardinal Health offers independent pharmacies strategic co-branding opportunities through their Medicine Shoppe International, Inc. franchise which leverages both The Medicine Shoppe® and Medicap Pharmacy® national brands. THE CHALLENGE The Tri-State Medical Group comprises three independent pharmacies located in West Virginia and south eastern Ohio. “Our stores are within about 30 minutes of each other, of different sizes, and in very different locations,” said J.J. Bernabei, pharmacist and CEO/owner of Tri-State Medical. “Follansbee Pharmacy is in a strip mall, Tri-State Pharmacy is in a medical building attached to a hospital, and Value Leader Pharmacy is attached to a general practitioner and ob/gyn clinic.” All three

pharmacies are Members of The Medicine Shoppe® Family franchise network and are open to the public, and Bernabai employs 80 staff members, including 12 full-time pharmacists. In 2013, Tri-State’s contract with Leader Drug Stores (a network of independent pharmacies affiliated with Cardinal Health) was expiring. Bernabei was deciding whether or not to continue that connection as he opened his third pharmacy location. He knew that going it alone was not a real option. “To succeed, you need to really push your operation to the limit and take advantage of all the technology in the marketplace,” he said. Independent pharmacy owners do not usually retain specialized marketing or technical staff, however. “We did it all ourselves,” said Bernabei. “That’s nearly an impossible feat for one store, so when you have three, it’s even more difficult.”
THE DECISION When Bernabei looked into co-branding, Cardinal Health’s Medicine Shoppe franchise option stood out. “We did not have anyone appeal to us on the cobranding side the way Cardinal Health did,” he said. "The potential partnership showed itself to be a positive prospect in many ways, including what is usually one of the biggest concerns for any business owner – cost."

“We did our due diligence first, and found that the fees were actually a minimal part of the process.” Bernabei said. “Ultimately, the costs of the programs we purchased [as part of the Leader network] before co-branding were virtually the same after co-branding with The Medicine Shoppe® but we got so much more value.”

THE RESULTS Co-branding provides advantages that go beyond aligning with a national brand name that customers recognize and trust. According to the National Community Pharmacists Association, 91% of prescriptions are covered by third-party contracts, making contract management an important challenge for independent pharmacies to master. “Rarely do I come across a contract that The Medicine Shoppe® is not already involved with and has vetted,” said Bernabei. “It can be difficult for independent pharmacies to establish contracts, so co-branding delivered a robust advantage for us. We are still three independent pharmacies, and now we contract along with several thousand others through The Medicine Shoppe® PSAO.”

CASE STUDY The co-brand connection also offers a wealth of marketing support, from store signage to social media branding. “We utilize just about every marketing piece they afford us,” Bernabei said. “The co-branding for the appearance of the stores is seamless, and our co-branded electronic signs are a big driver. Their in-store radio program comes with templates for ads that play automatically in our stores and on our hold messages; I would have had to create these myself or go without and miss the opportunities.”
THE CONCLUSION Owners who join The Medicine Shoppe® franchise can retain their existing independent pharmacy names and simply add “A Member of The Medicine Shoppe Family,” to their brand identity. “People in our area recognize The Medicine Shoppe® as a credible brand,” Bernabei said. “They don’t see it as a mass merchandiser or an operation that is not independently owned.” If connecting to a recognized brand name boosts a community pharmacy’s visibility and trustworthiness, co-branding with The Medicine Shoppe® offers even more. “For us, the name was not the main benefit,” Bernabei said. “We saw the big advantages in what cobranding would do for our business model. It helps us operate more efficiently and lets us take advantage of programs that we otherwise wouldn’t have access to without being part of a bigger operation, including contract management. The Medicine Shoppe® is leading the pack on this.” Co-branding also makes his business more attractive to potential future owners. “Our plan is to expand our ownership [team] to a couple of our younger pharmacists, and their vision is to open even more co-branded locations,” Bernabei said. Through co-branding, Bernabei’s pharmacies all belong to the same PSAO, which allows for a simple purchasing model and consistent pricing. “These advantages are very attractive to younger pharmacists who want to be owners, but don’t have much experience on the business side. Performing the Test

The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.
Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Q. Is CBD a pain management alternative to opioids?
CBD or cannabinoid itself is non-psychoactive. Preliminary research indicates CBD or cannabidiol could be a successful opioid alternative. Recent studies have shown that states with medical marijuana laws (THC and CBD) have twenty-five percent (25%) fewer opioid-related deaths than states that have not implemented laws. Studies also show that CBD has a diminishing effect on drug cravings, which could impact the high relapse rates associated with opioids. While some studies are encouraging, it’s also important to understand why CBD shows so much promise to fight the opioid crisis. Education is key as CBD or cannabidiol continues to emerge as a pain management solution. CBD offers an opioid alternative for the treatment of chronic pain. Just last year, a study from the European Journal of Pain showed that (using an animal model) CBD applied on the skin could help lower pain and inflammation due to arthritis.[1]
Q. Will CBD alone get a patient high?
No. Scientifically and clinically, hemp plants contain less than 0.3% tetrahydrocannabinol (“THC”), the psychoactive chemical found in cannabis. This minute amount of THC will not make a patient feel high, reducing the risks of theft, addiction, and diversion associated with THC use.
Q. What is the FDA position on CBD?
To date, only one prescription medication containing CBD has been approved by the FDA – Epidiolex, a drug used to treat some forms of epilepsy. The FDA has historically issued several warning letters to firms that market unapproved new drugs that allegedly contain cannabidiol (CBD). Interestingly, the FDA has tested the chemical content of cannabinoid compounds in some of the products and many

were found to not contain the levels of CBD they claimed to contain. CBD products are not approved by FDA for the diagnosis, cure, mitigation, treatment, or prevention of any disease. Notices and warnings related to promised outcomes of the effectiveness of CBD products should be considered before promoting or offering any CBD product for sale.[2] The FDA held its first public hearing on how regulation should be implemented on May 31, 2019, and in the future, will regulate CBD products. The goal of the May 2019 public hearing was for the FDA to learn about the current status of the safety, manufacturing, product quality, marketing, labeling, and sale of products containing CBD.
Q. How do you know what’s in the bottle labeled CBD?
Currently the manufacturing of CBD is unregulated. According to the Centers for Disease Control (CDC), synthetic products marketed as CBD sickened at least 52 people in Utah last winter, sending 31 of them to emergency rooms.[3] This lack of regulatory oversight creates a certain level of ongoing risks.
Q. Why CBD or cannabidiol?
Many patients have reported pain management success when using CBD products. As research on CBD products and effects continues, owners must continue to monitor published research to determine if the CBD products offered remain safe and beneficial.[4] To date, there is no recognized research to support CBD products for use as an appropriate treatment for serious medical conditions like cancer, diabetes, and Alzheimer’s disease. However, CBD is commonly used to address anxiety and insomnia, with some supportive research for these ailments.
Before implementation of a CBD regimen, patients should consult his/her primary care physician for guidance. Once approved, patients should start usage of CBD in small doses, after first confirming the CBD product is a “quality product.”
Q. How do you determine whether a CBD product is a quality product?
Before offering a CBD product for sale, check the manufacturer, source, or grower of the CBD, and the amount of THC present. Additionally, it is important to check for a “Whole Plant” or “Full Spectrum” Label (another quality of a good CBD oil is that it is manufactured using the whole plant). Finally, verify the existence of thirdparty lab results. Consulting and offering CBD products can feel intimidating at first, but with due diligence you’ll be ready to advise and assist patients seeking a quality product.
Q. Should owners know the laws in their state?
All 50 states have laws legalizing CBD with varying degrees of restriction and, while the federal government still considers CBD in the same class as marijuana, it doesn’t habitually enforce regulations against CBD distribution and/or usage. THE DECISION
When interfaced with EverSpring’s pharmacy management system, Noritsu’s software helps the pharmacy generate a pill report that is continuously aggregated and monitored, helping to identify the fastest-moving medications that are in high demand across facilities and patients to better optimize production. The Xana strip packager then utilizes active cassette and tray management to better dispense medications in an uninterrupted workflow. In advance of a batch run, the number of NDC’s allocated as cassette-loaded drugs can be placed (expandable up to 1,000 unique NDCs), thereby minimizing the labor previously required to be delivered manually via tray.

Automated pharmaceutical packaging technology is a growing industry, aiming to address the needs of an increasingly large population of complex patients and the pharmacies that serve them. Numerous manufacturers are vying for prized pharmacy space. “I was in the market for new automation,” said Mick Blazier, Pharmacist-in-Charge at EverSpring Pharmacy in St. Louis. “There are a number of big players in the field, of course. Another pharmacy that I had a relationship with used Noritsu, so I also included them in my search.” EverSpring exclusively targets long-term care, including assisted-living facilities and developmentally disabled group homes, so Blazier’s interest in improving the pharmacy’s adherence packaging was strategic. During the course of his research, he met with a handful of pharmacy automation market leaders, including Noritsu. “I was most impressed with what Noritsu offered, both in terms of their machinery and their customer support.” EverSpring ultimately made the decision to purchase two Noritsu Xana 4001 strip packagers as well as their MDM-3 medication detection machine.
THE RESULTS The addition of Noritsu’s technology resulted in substantial improvements in EverSpring’s production time, workflow, and overhead costs. “We were running 16-hour days on average; sometimes we were so behind, we had to run 20 or 24 hours a day with the older machines,” said Blazier. “The new machines decreased our run time to an average of 10 hours a day and increased our output by 25%, while decreasing our payroll hours by about 50%.”

CASE STUDY EverSpring’s new packagers combine lower labor costs and improved features. Custom-labeled medication pouches provide important information in clear, crisp detail, making it easier for patients and caregivers to read. The Noritsu equipment also allows EverSpring’s staff members to make more efficient use of their work time. When the outdated machines filled pouches, they still had to be hand-checked by a pharmacist for safety. With the addition of medication and pouch detection through the MDM workstation, a digital image and associated data are archived for every pouch ever produced. “A pharmacist sees all of [the images] on screen and can visually inspect a blown-up image of each pouch if necessary,” Blazier said. The MDM workstation has reduced staffing needs by two FTE pharmacists and also allows seamless connectivity between EverSpring’s existing pharmacy management system and eMar administration. EverSpring’s accuracy has also benefited. “The combination of Noritsu equipment we selected has tremendously cut down on medication errors leaving the pharmacy,” Blazier said.
THE CONCLUSION Ultimately, customer service was the main driver in Blazier’s choice of Noritsu Pharmacy Automation, and he has not been disappointed. “They had an onsite presence here for two weeks after the installation, and we are always communicating with Noritsu about ways to make the system even better,” said Blazier. “If we ever have a problem, they either log in remotely to take a look immediately or send one of their dedicated field techs here within a couple of hours.” As pharmacy growth continues, Blazier acknowledged that EverSpring would turn to Noritsu again for additional packaging machines. “They are so efficient, and the support is that good.”

AON Pharmacy—Tools of the Trade for Keeping Patient Medication Stable

Launched in September of 2018 as an offshoot of the Florida Cancer Specialists & Research Institute, AON Pharmacy is a specialty pharmacy dedicated entirely to dispensing oral oncology medications. Based on their rapid growth, this type of “specialized” specialty pharmacy is clearly an idea whose time has come. “We started with just three employees, shipping medication to patients in about nine states via Fed Ex,” says Doug Braun, Pharmacy Manager for AON Pharmacy. “As we approached our first anniversary, we had 17 employees and plans in place to expand our reach to all 50 states.”

That’s why when it came time to conduct performance qualification (PQ) testing with their packaging, the pharmacy turned to Temptime for guidance.
Performance Qualification (PQ) Testing Adding new patients in states across the country means AON will be shipping to a greater variety of climates. “We wanted to be sure our shipments were maintaining proper temperatures so the medications we provide arrive within their stability profile,” Doug says. This was the impetus for their decision to conduct PQ testing.
Having already achieved their ACHC accreditation, AON began the process to attain URAC accreditation as well. URAC’s current guidelines require pharmacies to conduct PQ testing annually or any time a change is made to their packout. “The difference is that ACHC simply wants to know that you have processes and(3) 20.5 oz frozen gel packs pre-conditioned at room temp (laid out individually) prior Three shipment. CASE STUDY AON followed their current SOP for packing and included the EDGE sensors within their packouts to monitor both internal and ambient temperatures. The packages were then shipped to Temptime HQ for an initial performance evaluation. Sensors recorded temperatures every 10 minutes from the time they left AON’s facility until they arrived at their final destination. Temptime then downloaded and analyzed the data from the sensors and provided initial data and feedback to AON. All shipments must maintain the temperature range being tested for a specified duration in order to pass Temptime’s Phase 1 evaluation and move on to Phase 2.
In the second phase of PQ testing, Temptime personnel visited AON onsite to assist with testing and documenting procedures. “Our Temptime rep observed our refrigerated packout procedure and made sure that we were putting the temperature sensors in the correct spot,” says Doug. “She also made sure the sensors were active, programmed with the correct parameters and ready to start recording data so that we would get the information we needed.” The pharmacy sent several test packages out to various locations and climates. Even though they send temperature-sensitive medications overnight, AON requested Temptime provide performance data for 48 hours to see what might happen in the case of shipping delays.

Test results demonstrated that the AON Pharmacy “Small” shipping boxes are capable of providing the desired temperature environment for shipment of temperature sensitive products, maintaining temperature within 2°C to 8°C for a minimum of 24-hours.

“We found the EDGE sensors super straightforward to set up,” Doug explains. “We had no issues, no need to call tech support—everything was easy and worked well. That’s exactly what we were looking for.”
The Results The biggest surprise for the pharmacy was that their packouts were really getting the job done. “I thought we’d see medication start in a normal range and then warm up some,” says Doug. “That didn’t happen – and if anything, some packages actually got colder than I expected for brief periods.” Still, no medications ever fell out of range. “The PQ testing was a great way to validate that our packages were configured properly and didn’t need modifications,” he says.
Looking Forward “We do know that as we expand our footprint, we are going to have to be prepared for all kinds of temperature extremes,” says Doug. “We hadn’t thought about medication getting too cold or freezing when we were only shipping throughout the Southeast.” AON plans to add additional packout configurations that protect both refrigerated and room temperature medications shipped to colder climates, into Alaska for instance. The pharmacy has purchased EDGE M-300 and S-400 programmable temperature sensors from Temptime to use for future internal PQ testing since URAC doesn’t require that testing be conducted by a third party. “We found the EDGE sensors super straightforward to set up,” Doug explains. “We had no issues, no need to call tech support – everything was easy and worked well. That’s exactly what we were looking for.” They plan to wait until winter before testing any new packouts. They’ll also conduct PQ testing regularly, both to ensure they are meeting patient needs as well as to comply with URAC accreditation requirements.
Right now, the pharmacy only uses TransTracker for monitoring heat exposure. “I expect we’ll be evaluating if we want to monitor both heat and freeze events based on our expansion plans,” he says. It just makes sense to give patients that peace of mind since exposure to both excessive heat or freezing temperatures can be detrimental to medication stability. “We explain to our patients what they’ll be getting and what they need to look for before we send them their medication.” So far, they’ve had no patients calling in to say medication has gone out of range. Doug says this is critical to AON since the oncology medications they distribute can cost anywhere from $7,000 to $20,000. “Having to replace even one shipment would have significant financial consequences for us not to mention inconveniencing our patients.” Confirming that their packouts worked as needed wasn’t the only validation AON got out of the PQ testing. “Working with Temptime through this process just solidified my confidence in them,” Doug says. “We’re in this day and age where customer service is lacking. It’s become the new norm, but not with Temptime. When I call, I get a callback – usually the same day. If they say they are going to do something, they follow through. It’s refreshing.”
