“A perfect example of how best practices can truly help us take better care of our patients is how we faced the challenge of Georgia Rule 480-48 in 2014,” said Olsen.1 In late 2014, the Georgia State Board of Pharmacy issued the rule requiring pharmacies to ensure that any drug requiring storage at “anything other than room temperature” that is being delivered or mailed have packaging with a “USP-recognized method by which the patient can easily detect improper storage or temperature variations.” “We knew we had to make some changes in how we operated,” he stated. This new regulation came only two years into the young company’s founding, but Encompass Rx faced the challenge head on—as an opportunity. 1

Moundsville Pharmacy becomes model for synchronization and adherence Moundsville Pharmacy is a retail pharmacy in Moundsville, West Virginia, about 75 miles southwest of Pittsburgh. Founded in 1981, the pharmacy today is a model for prescription synchronization and Medication Therapy Management (MTM). Additional services include compounding, DME, immunizations, diabetes education and a few long-term care contracts. Owner Jason Turner, Pharm.D., began his training with pharmacy-mentor Roger Cole, R.Ph., at Moundsville Pharmacy where he realized his passion for independent pharmacy practice and business ownership. Today, the operation of Moundsville Pharmacy and the legacy of Cole continue through the efforts of Turner and his team of pharmacists and technicians.
Preceptor Recognitions
The pharmacy utilizes QS/1®’s NRx® Pharmacy Management and Point-of-Sale Systems to manage operations. In 2014, after expanding their prescription synchronization program and recognizing the importance of efficient pharmacy workflow, Turner’s goal was to take the pharmacy to the next level. “At the time, I thought we may need a new pharmacy management system – one with more bells and whistles,” said Turner. “But after looking at some other systems and talking with QS/1, I learned it wasn’t the software that lacked the bells and whistles, it was the way we were using the system or, I should say, not using the system.” With some support from QS/1, Moundsville Pharmacy developed a strategy for better workflow performance.

The L60 robot fills the traditional blister (bingo card), typically containing a 28/30/31 medication day supply. The output of the L60 is patient labeled and ready to deliver to your customer. The L60 is the most flexible single-medication automation on the market, handling calendar filling and skip dosing, as well as splitting out “day packs.” The C60 robot fills a half-card, which is a 15/16-day supply often used for schedule two to four medications or short-cycle filling scenarios.

The U60 is a fully automated single medication packaging robotic solution. Within the robot, for each card produced, each individual blister cup is labeled on the back of the card. About Encompass Rx Encompass Rx is a locally owned and operated independent specialty pharmacy based in Atlanta, Georgia. Founded by the owners, John Olsen, Maney Mazloom, and Dee Renade, they are focused on patient care for individuals with chronic diseases, believing that their role as clinical pharmacist does not begin or end with the dispensing of a prescription. In just over 4 years, Encompass Rx has grown beyond their wildest dreams. What started as a regional specialty pharmacy focused on the greater Atlanta area is now a fully URAC-accredited, 50+ person company licensed in over 30 states. The three owners have over 25 years combined experience in specialty pharmacy. John Olsen, PharmD., attributes this growth and success to utilizing industry best practices.
Challenged by Regulation “A perfect example of how best practices can truly help us take better care of our patients is how we faced the challenge of Georgia Rule 480-48 in 2014,” said Olsen.1 In late 2014, the Georgia State Board of Pharmacy issued the rule requiring pharmacies to ensure that any drug requiring storage at “anything other than room temperature” that is being delivered or mailed have packaging with a “USP-recognized method by which the patient can easily detect improper storage or temperature variations.” “We knew we had to make some changes in how we operated,” he stated. This new regulation came only two years into the young company’s founding, but Encompass Rx faced the challenge head on—as an opportunity. 1

Finding Opportunity in the Solution
Implementing Change
With the introduction of the new regulation, Encompass Rx began to seek out an appropriate temperature indicator. They evaluated several visual temperature indicators, and made a decision to use Temptime’s LIMITmarker®.
During the evaluation phase of temperature indicators, Encompass Rx focused on minimizing steps.
“We had not previously investigated temperature indicators, simply because we thought they’d be cost-prohibitive,” said Olsen. “But at the same time, our patients are our number one priority and the last thing we want is to have a patient concerned about their medication.” After investigation, Encompass Rx felt even more strongly that implementing a temperature indicator was the right thing to do, regardless of the regulation. “We loved the idea, and wanted to make it happen,” he said. “We work hard to make sure our patient’s medications are carefully delivered, in a timely manner, and their peace of mind is important to us. It was almost serendipitous that the rule and our desire to do this happened at the same time.”
“We have specific protocols and a fine-tuned shipping process,” John described. “When testing the various indicators, we quickly determined that implementing use of the LIMITmarker indicator added one simple step of sticking the indicator on the product bag before it goes into the cooler.” Other products required extra steps like pulling a tab for activation, or opportunities for what Olsen described as “more human error.” In the end, they determined that LIMITmarker was more user-friendly and they were more confident about implementing it into their processes. “Implementation went smoothly for us,” he added. “The Temptime team was great to work with—they helped us understand how the indicators work and where to place them. We had no hiccups at all.”

Addressing Fears Encompass Rx’s primary “fear of the unknown” was that incorporating temperature indicators could have a negative impact on their business. Instead of improving reliability and shipment integrity, what if there were false indications causing customer complaints and costly reshipments that would negatively affect the bottom line? “We were honestly concerned that false positives might result in a negative ROI, so we did a range of internal tests of our own,” said Olsen. “Plus, Temptime provided us with white papers and product quality details and in the end, we figured out that LIMITmarker was a safe bet, a good product.”
Expanding Use for Patient Satisfaction While it’s not mandated, Encompass Rx includes a temperature indicator with every refrigerated product that they send out. Many drugs have extended stability data and are stable at room temperature or even higher temperatures for up to 30 days. Some pharmacies wouldn’t find it necessary to use indicators on these types of drugs, but Encompass Rx has learned that indicators go a long way in helping their patients feel comfortable and confident about their medications. And, it’s working. Encompass Rx has earned the trust of their patients, with a solid patient satisfaction rating of 97%.
Justifying ROI “The amount of reshipments has decreased probably tenfold since we started using indicators,” exclaimed Olsen. “We rarely reship products anymore. Previously during the hot summer months here in the southeast, we would get at least one call a day regarding product integrity due to concerns about heat exposure.” “The indicators pay for themselves quickly,” he added. With an average shipping cost of $20-$25 plus the average medication product cost of $4000, the elimination of one reshipment pays for an entire quarterly order of indicators. Olsen described that all patient concern phone calls are monitored closely and reviewed quarterly. With two years of indicator use under their belts, Encompass Rx has deemed

the indicator use ”very effective, and incredibly impactful on our business of taking care of patients.” Additional ROI is realized since Encompass Rx employees are taking dramatically fewer patient calls—that in the past consumed resources and increased costs.

Preceptor Recognitions
The Mission of Roosevelt College of Pharmacy is to embrace a patient centered approach to learning that will produce graduates who are Competent, Committed and Compassionate. The Office of Experiential Education greatly values the time and dedication of all of our preceptors who play a critical role in providing quality pharmacy experiences for our students. Each year our students submit their nominations for Introductory Pharmacy Practice Experience (IPPE), and Advanced Pharmacy Practice Experience (APPE) Preceptor of the year. The 2016 recipients received their award this year in May, at the Class of 2016 Awards Banquet. The IPPE Preceptor of the Year was awarded to Dr. Brandy Cooper-Kirby, from Sinai Health System. Student evaluations of her acknowledge her extreme commitment and passion towards the profession of pharmacy and pharmacy students. It was noted that Dr. Cooper-Kirby, along with the whole pharmacy staff at Sinai Health System, are very welcoming, which leads to a very positive learning environment. The APPE Preceptor of the Year was awarded to Dr. Nora Flint, from Rush University Medical Center. Dr. Flint has precepted students from Roosevelt College of Pharmacy since we sent our Inaugural Class out on their IPPE’s in 2011. Student evaluations of her have volumes of accolades to share about her and the learning environment that she creates for them. Her interactive style of teaching helps the students to apply what they learn in the classroom to real life situations. She was noted as being an exceptional preceptor who truly prides herself in wanting to help prepare future pharmacists to be exceptional as well. MCPHS University Thanks Saumil J. Patel., PharmD. Saumil J. Patel graduated from MCPHS University with his PharmD degree in 2007 and has developed a successful Medication Therapy Management (MTM) service for Walgreens in Roslindale, Massachusetts. He is currently the District MTM Leader for Walgreens in the Boston district. He is energetic and passionate about providing MTM throughout his tenure at Walgreens and has been an advocate of changing our culture to relational from transactional. The formation of Medication Therapy Management has been the niche of his practice. He realizes as the pharmacy profession continues to evolve, and as pharmacists keep pushing to become recognized as health care providers, MTM will very likely prove to be at the foundation of such growth. Saumil is offering MCPHS University students, both IPPE and APPE, the opportunity to be involved in MTM delivery while completing community rotations. He also offers a specialty APPE elective rotation in MTM and depending on the ability of the APPE student, to be involved in the completion of the entire MTM case. Utilizing pharmacy students and offering the students the opportunity to practice MTM will benefit not only a pharmacy and its MTM-eligible patients, but it will also help pharmacy students learn and be able to utilize MTM in their future. Moundsville Pharmacy becomes model for synchronization and adherence Moundsville Pharmacy is a retail pharmacy in Moundsville, West Virginia, about 75 miles southwest of Pittsburgh. Founded in 1981, the pharmacy today is a model for prescription synchronization and Medication Therapy Management (MTM). Additional services include compounding, DME, immunizations, diabetes education and a few long-term care contracts. Owner Jason Turner, Pharm.D., began his training with pharmacy-mentor Roger Cole, R.Ph., at Moundsville Pharmacy where he realized his passion for independent pharmacy practice and business ownership. Today, the operation of Moundsville Pharmacy and the legacy of Cole continue through the efforts of Turner and his team of pharmacists and technicians.
The pharmacy utilizes QS/1®’s NRx® Pharmacy Management and Point-of-Sale Systems to manage operations. In 2014, after expanding their prescription synchronization program and recognizing the importance of efficient pharmacy workflow, Turner’s goal was to take the pharmacy to the next level. “At the time, I thought we may need a new pharmacy management system – one with more bells and whistles,” said Turner. “But after looking at some other systems and talking with QS/1, I learned it wasn’t the software that lacked the bells and whistles, it was the way we were using the system or, I should say, not using the system.” With some support from QS/1, Moundsville Pharmacy developed a strategy for better workflow performance.

Using Technology Tools Turner said a key to Moundsville’s success was activating QS/1’s Workflow platform. “The Workflow feature created dramatic changes in our pharmacy operation. It made us more accountable and much more efficient in tracking and solving problems, checking prescriptions and ensuring prescriptions are handled properly and efficiently.” Additionally, the staff quickly came to rely on QS/1’s Pharmacy at a Glance dashboard that allows monitoring of pharmacy workflow at any workstation. The tool enables you to quickly spot bottlenecks in quality assurance, processing, error resolution – and a number of other pharmacy functions you have the ability to monitor. Turner also commented that the pharmacy’s increased use of QS/1’s Tickler feature, to schedule prescription processing and patient related tasks, has centralized the random patient and prescription notes in their pharmacy. This has also allowed them to reduce the amount of clutter on their counters. And to tie it all together, for over 10 years, Moundsville Pharmacy has relied on the seamless integration of QS/1’s NRx Pharmacy Management and Point-of-Sale Systems. “I’ve found this integration to be very beneficial to the business in general, and to reporting in particular,” Turner offered. “It makes operating and managing a pharmacy very simple.” Improving Outcomes With Star Ratings and quality measures becoming increasingly important in the pharmacy, Turner set a goal for improved adherence for the patients of Moundsville Pharmacy, convinced the key was prescription synchronization. He was right.
As his prescription synchronization program expanded to more patients, it became obvious to Turner that synchronization created favorable outcomes for both the patient and the pharmacy. “It’s clear that medication synchronization is the foundation to better patient adherence AND pharmacy growth. It seems every month we find more third parties looking for pharmacies to help improve patient adherence scores,” he said. For the patient, you are offering a better model for the prescription refill process which results in healthier outcomes, quality of life and convenience, as well as greater customer satisfaction. As he said, “Imagine the experience patients would have if conversations about lack of refills, out-of-stock medications, prior authorizations and insurance issues were replaced by recommendations for therapies, suggested immunizations and education regarding their health and disease states?” For the pharmacy, it means greater efficiency and lower operating costs per prescription – specifically, fewer incoming calls, reduced checkouts, consolidated deliveries, improved scheduling of staff, and more effective inventory management. Even better, it also generates greater prescription volume. With prescription synchronization focused on strategic workflow, the pharmacy is also able to spend more time with the patient to review medications for chronic conditions and help them identify meaningful opportunities for improving their health. Creating a Successful Program Turner said that one of the keys to a successful prescription synchronization program is to have the proper pharmacy workflow tools enabling the

C A S E S T U DY pharmacy to process greater prescription volume more efficiently. Moundsville relies heavily on the comprehensive tools of its NRx software to manage synchronization and the ensuing workflow. Two tools are HealthMinder® and InstantFill®. “Health-Minder tells us which refills are due, then InstantFill enables us to process those fills efficiently,” stated Turner. With QS/1 technology handling workflow, Turner said his focus is on adherence scores and processes that will help improve prescription adherence. They are expanding their services as well as honing their counseling techniques to help patients experience the highest level of adherence and the maximum benefits from their medications. To improve Five-Star Ratings, they review medication possession ratio (MPR) percentages in the pharmacy system’s patient record. The system provides reports on patients with less than optimal adherence. The Tickler feature enables scheduling of refills when they are due and Health-Minder contains a number of tools to monitor adherence, detect late refills and aid inventory management. The system also helps identify patients eligible for MTM.

Seeing Results After three years of focus on medication adherence, prescription synchronization and pharmacy workflow, Moundsville Pharmacy is a model for performance-driven workflow. In 2013, Moundsville’s monthly prescription volume was 7,300. Today, its monthly volume has increased 22 percent to 8,800. By the summer of 2016, 850 patients were enrolled in the synchronization program. These patients account for 55 percent of the pharmacy’s prescription volume. The pharmacy’s workflow has become a well-oiled machine. The staff processes synchronizations on Monday, completes Syncs on Tuesday, with delivery on Wednesday. The pharmacy was recognized by OutcomesMTM® in 2014 as the “Top MTM Center in West Virginia” and completed 90 percent of Mirixa® CMRs in 2015. Moundsville’s success led to the acquisition of a second location in the neighboring town of Sistersville in the fall of 2014. In the past year, Sistersville’s prescription volume increased 11 percent to 3,750 per month as it enrolled 340 patients in synchronization, representing 54 percent of its prescription volume.

With the introduction of this licensed version, in addition to research and water testing, Pyrosate is ideal for dialysis centers, compounding pharmacies, and pharmaceutical and medical device companies.
Performing the Test
The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.
Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage.
