DOSIS Systems’ Month-to-Month Subscription – Autonomous Efficiency Guaranteed
CASE STUDY Grow Your Family of Pharmacies With Medicine Shoppe International, Inc. Smart independents achieve successful expansion through the power of co-branding with a respected name.
PRIMARY INTENDED OUTCOME
DOSIS Systems offers autonomous workflow that partners with pharmacies to ensure improved efficiency, productivity, and bottom-line by automating over 75% of their production.
Despite an overall trend showing an increase in the number of independent pharmacies that close their doors every year, new standalone stores continue to appear in communities across the U.S. The challenges these independent shops face include the effect of reimbursements on revenue, the cost of managing inventory, as well as difficulty parsing and acting on crucial profit and loss metrics.
Jerry Barnette Managing Partner
Owners looking to expand their footprint have several options with Medicine Shoppe International (MSI), whether they choose to convert an existing pharmacy, acquire a pharmacy, or open a new store or tele-pharmacy, they can utilize a proven franchise model with a full brand or co-branding plan that works for them. MSI Franchisees benefit from a variety of business solutions that maximize marketing, tap into advantages of scale, and ultimately improve patient engagement and care.

THE CHALLENGE
David Ruth
There was a time when independent pharmacy owners found it fairly easy to be successful. Today, the environment is much more challenging, according to Jerry Barnette, managing partner of the Med-Save family of pharmacies and independent owner since 1994. “We’ve gone from competing against the chains to competing against the insurance companies and PBMs. We have to watch everything carefully to determine where we will be six months down the road regarding reimbursements and retroactive adjustments.”

Because hospitals and health systems are uniquely aligned to deliver the care that these patients need, every hospital should have a specialty pharmacy strategy. That strategy must recognize the impact each key stakeholder will have on the success of the specialty pharmacy, including patients, providers, clinical staff, payors and pharmaceutical manufacturers and how to develop and manage those relationships for success.
•
Patient education regarding treatment and side effects before and during treatment
•
Adherence monitoring using patient surveys
•
Adverse effect management to address side effects
•
High cost of specialty medications, most over $2,000 per fill
•
Fewer than two percent of patient populations requires these drugs
•
Complex administration and compliance issues
•
Manufacturers may choose to limit specialty drug distribution
PROVIDERS Communication is often fragmented in the healthcare environment where the fax machine and landlines are still used to communicate and coordinate care. A landmark study by Biologics found that the average cancer patient

CARDIOVASCULAR RISK THE ROLE OF THE PHARMACIST
PHARMACISTS CAN HELP PREVENT THE NEXT CARDIOVASCULAR EVENT VASCEPA® (icosapent ethyl [IPE])—not your typical omega-3 fatty acid drug—could help millions of high-risk patients, including statin-treated patients and statin-intolerant patients, to reduce their cardiovascular (CV) risk.1,2 Multiple professional societies have updated guidelines or issued advisories recognizing IPE, including the American Diabetes Association, the European Society of Cardiology/European Atherosclerosis Society, the National Lipid Association, and the American Heart Association.3-6

Pharmacists are among the most trusted and most accessible of all healthcare professionals (HCPs).7,8 In addition, they are one of the most frequently visited members of a healthcare team.8,9 Many of the frequent visitors of community pharmacies are patients with cardiovascular disease (CVD).10 Statin therapy, the first-line option for high cholesterol, lowers levels of low-density lipoprotein cholesterol (LDL-C), or “bad” cholesterol, and has helped reduce the risk of developing life-threatening blockages in the arteries that can cause heart attacks and strokes.5,11-13 Despite the LDL-C–lowering effects of statin therapy, persistent CV risk remains.14 Other drugs, including but not limited to those listed below, are sometimes recommended in patients that cannot tolerate statins or in addition to statins for patients responding inadequately or with severe hypercholesterolemia.3,15

of Cardiology/European Atherosclerosis Society, National Lipid Association, and American Heart Association.3-6
Encouragingly, studies have shown that during pharmacy visits, short educational interventions by pharmacists can positively influence their patients’ decision making, especially as it pertains to taking control and managing CV risk factors—highlighting the critical role of pharmacists in patient care.21 An example patient journey is presented to demonstrate the impact pharmacists have on patient outlook. MILLIONS TAKING STATIN-BASED STANDARD OF CARE EXPERIENCE PERSISTENT CVD
• Ezetimibe: An oral cholesterol absorption inhibitor taken in combination with statin therapy that has shown reduction in LDL-C levels and in the risk of heart attacks and ischemic strokes; however, it has not shown reduced risk of other CV events compared to when statin therapy is taken alone.16 • Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors: Recently shown to improve CV outcomes when added to statin therapy,17,18 these monoclonal antibodies are injectables and tend to be limited due to high-cost adoption.19,20 • VASCEPA (IPE): An oral medication containing an ethyl ester of omega-3 fatty acid that was recently FDA approved and clinically shown to reduce the risk of CV events.1,2 The American Diabetes Association has updated their 2019 guidelines to include IPE as a top-tier recommendation for CV risk reduction, and further recommendations to use IPE have been made by the European Society

“ Liberty Software is easy to learn, has lots of great user options, and the support is phenomenal!”
How long does it take to teach new employees how to operate your existing pharmacy software? Pharmacies using Liberty have found that new employees can often begin filling scripts immediately, with little to no training. It’s just that simple. A well-designed pharmacy system should make your employees more productive and happier. When you do need help, it’s only a phone call or mouse click away.
Benefits
• Organize Your Pharmacy • Fill More Scripts Without Adding Staff • Improve Customer Service • Streamline Your Workflow • Manage and Improve Profits • Increase Patient Safety

Pharmacy Point of Sale Built In
For your pharmacy, point-of-sale software needs to be more than cash drawer management for your front-end. For your patients, it should enable your staff to provide better service. For you, it should result in better accountability, assistance with HIPAA compliance, and improved pharmacy management. These are values that not just any point-of-sale package can deliver. That’s why we developed ours from the ground up, designed especially for community pharmacies’ needs. Liberty’s point of sale is not simply an add-on to your pharmacy system, but a completely integrated solution. SPRING 2020

Company Background
Founded in 1998, with a mission to help pharmacies understand and use technology-based tools to run a successful business, Retail Management Solutions is now one of the most respected point-of-sale providers in the industry. RMS’ POS systems are created exclusively for pharmacies. We listen to our customers, partners, and industry requirements to provide timely, pointof-sale based solutions to solve the challenges faced by pharmacies today. At RMS, we believe that partnership is at the heart of successful pharmacy/vendor relationships. We aim to help customers work with the partners they choose, while providing top of the line point-of-sale solutions and robust integrations.
Product Overview
RMS offers a range of point-of-sale products that are both flexible and scalable to meet the needs of any pharmacy. While our easy-to-use till application is at the forefront of day-to-day activities in the pharmacy, we know that running a successful pharmacy goes far beyond collecting a payment. There are many ways in which an RMS system can help your pharmacy. n At the Register RMS customers have freedom to choose from over 30 pharmacy system partners, all with integrations that make prescription sales fast, easy, and accurate. Signatures for HIPAA, safety caps, prescription acknowledgement, etc. are captured at the time of sale and are always easily accessible when needed for reporting and audits. Built in date of birth verification processes and additional prescription for pickup notifications bring added patient safety and convenience to every transaction. Beyond prescription sales, OTC product sales are as simple as scanning the item barcode. Custom items can also be created for easy sale of unique products, gift items, etc. Sales can be processed in many different ways to meet the unique needs of your pharmacy. Whether you need standard retail lanes, an option for drive-thru payments and signatures, curbside pickup solutions, or an
easy way to track home deliveries, you can mix and match to create the perfect technology scheme. As with our pharmacy system integrations, we also offer flexibility in your choice of credit card processors. Choose the partner that works best for you with all the functionality you need, including processing of FSA, EMV, and NFC transactions. Pharmacies working through PCI compliance will also find end-to-end and validated point-to-point encryption capabilities, which represent the most secure options for processing credit cards today. n Improving Customer Relationships Improving patient outcomes and increasing customer satisfaction are goals at the heart of most pharmacy businesses today. Nutrient Depletion notifications at the register help you to improve patient wellness by making supplement recommendations based on prescription induced nutrient depletion. Integrated customer loyalty aids you in strengthening customer relationships, including advanced promotion options and rewards options that you can customize to suit your needs. You can also support charitable giving easily through any RMS system, strengthening your ties with the community you serve. n Behind the Scenes Streamlining pharmacy operations is no small task. Point-of-sale applications extend beyond the customer interaction to help you run your business more efficiently. For pharmacies struggling to manage retail departments, RMS’ inventory management solutions open the door for advanced management of front-end products. Wholesaler interfaces allow automated price updates and a streamlined purchasing and receiving process. Integrated shelf labels keep your shelf prices and system prices in sync. And advanced product reporting can help you reduce overhead costs by identifying products that aren’t selling, enabling you to take a more agile approach to the products you carry and make the most of your retail space. A host of additional reports, both canned and customizable, are also available from cash management, to employee performance, to A/R balances, and more. RMS systems give you the data you need to drive important decisions and run a more profitable and efficient pharmacy. n When You Need Support All customers have access to the RMS support line 24 hours a day, seven days a week. The RMS customer center is available at any time to request support, access the customer knowledge base, and more.

DOSIS Systems’ Month-to-Month Subscription – Autonomous Efficiency Guaranteed DOSIS Systems offers autonomous workflow that partners with pharmacies to ensure improved efficiency, productivity, and bottom-line by automating over 75% of their production. When our partner pharmacies thrive, we thrive!

Company Background DOSIS Systems is seeing pharmacies all over the country improve their efficiencies in workflow, increase production volumes, and become more profitable. We have a proven track record of automating up to 80% of a pharmacy’s blister card volume. This includes pharmacies that are currently producing over 250,000 blister cards per month. In order to do this, we focus on each pharmacy’s needs. This allows us to offer the best solution for each partner pharmacy and increase their efficiencies where it matters most. Product Overview DOSIS Systems starts with customer care to better understand your pharmacy’s workflow and efficiencies. We then integrate a seamless system to match your needs using the flexibility of our technology; this includes custom integrations with your inhouse software. Knowing that your workflow and integrations are efficient, we introduce autonomous blister card towers that produce a 24/7 product that becomes the muscle of your growth. From this system, we can continue to analyze your production volume to ensure that you are getting the most out of DOSIS Systems and generating more revenue, causing you to thrive!
SNF patients with DOSIS Systems filling over 200,000 of these cards every month (dailies and cycles) – DOSIS automatically prints color-coded med-pass information on every label in this system based on the prescription data. A 48,000-card-permonth pharmacy is getting 37,000 cards per month via DOSIS Systems and is actively converting away from the cycle-fill model because it is more efficient for them to fill on demand using their integrated DOSIS automation – they have already eliminated pre-packs from their workflow “to reduce inventory and extra touches.” Using DOSIS Systems to automate filling cards and printing time-pass labels, an 18,000-card pharmacy doubled in size without adding any staff on the floor. At another pharmacy doing 180,000 cards per month with 75% filled by DOSIS, the return-to-stock feature allows them to efficiently track and re-use cards filled for scripts that are canceled before shipping. For a pharmacy filling 97,000 on-demand cards each month, the prepack manager feature extends the productivity of the DOSIS Systems by automatically restocking pre-pack card inventory after hours – DOSIS produces 58,000 cards each month, and the pharmacy is open less than 60 hours per week. Every month, DOSIS Systems fill over 2-million cards in pharmacies large and small; SNF, mental health, and corrections; dailies, cycles, pre-packs; with and without med-pass and auxiliary label information; single- and multiple-tablet per blister; calendarstart and straight-fill.
Features & Options By customizing the capacity, features, and workflow integration, DOSIS Systems can make a tremendous impact in a wide variety of pharmacies. For example, a 55,000-cards-per-month pharmacy serving mental health patients is producing 35,000 cards per month in DOSIS automation while providing different filling and labeling formats for their various customer facilities. Another partner pharmacy is filling 250,000 cards per month for

Company Background Our purpose is to serve the nation with the single most trusted and capable health information network, built to increase patient safety, lower costs, and ensure quality care. Since 2001, Surescripts has led the movement to turn data into actionable intelligence, and convened the Surescripts Network Alliance™ to enhance prescribing, inform care decisions, and advance the healthcare industry. Product Overview Specialty Patient Enrollment automates the workflow for specialty medications, helping pharmacists to align benefit details, clinical information, and medication costs to quickly and reliably get patients on their medications. With Specialty Patient Enrollment, you get the patient’s prescription, prior authorization, and patient enrollment forms together in your pharmacy’s workflow. • Use a secure electronic connection to prescribers nationwide, with attachment of forms and clinical information for seamless care coordination in workflow. • Receive more complete and accurate patient enrollment forms directly in the pharmacy workflow, eliminating the need for multiple faxes and calls with prescribers. • Seamlessly embed Specialty Patient Enrollment and related forms into your pharmacy’s software platform, bringing these advanced capabilities to your pharmacists and patients. • Get your patients the critical medications they need, faster and more efficiently.
Product Specifications n Step 1: At the point of prescribing, Specialty Patient Enrollment populates the enrollment form with information pulled directly
from the electronic health record. The enrollment form can be sent directly to your pharmacy with no intervention needed from prescribers or their staff. Your pharmacy receives the prescription and enrollment form. n Step 2: Prior authorization required? If the prescriber’s office is relying on you to handle prior authorization, Surescripts includes that information in the enrollment form and ensures you have the data needed to complete the request. Questions for the prescriber? Bidirectional messaging helps you get answers without leaving your workflow or picking up the phone. n Step 3: Specialty Patient Enrollment gives you all the information required to dispense the medication and automatically populate the patient’s case file. It uses the prescriber order number to link the e-prescription with the enrollment form and confirms prior authorization electronically if needed, making for a streamlined workflow.
Testimonials “ T he complexities of specialty medications require pharmacies to gather additional information beyond the basics of strength, frequency, dose, and directions. Our collaboration with Surescripts is a complement to our existing solutions and further enhances our ability to reduce turnaround time, improve the prescriber experience, and ensure access to medication for patients.”
— Brian Seiz, President of Accredo
“ We are excited to be able to provide prescribers and their patients with a streamlined, automated workflow for specialty medications. Innovation that keeps clinicians off the phone and in their workflow saves significant time and resources in an area of the market that’s rightfully concerned about rising healthcare and prescription drug costs.”

CASE STUDY Grow Your Family of Pharmacies With Medicine Shoppe International, Inc. Smart independents achieve successful expansion through the power of co-branding with a respected name.
PRIMARY INTENDED OUTCOME
Jerry Barnette Managing Partner ~ Med-Save, A member of The Medicine Shoppe® Family
David Ruth Chief Operating Officer ~ Med-Save, A member of The Medicine Shoppe® Family
36
www.cardinalhealth.com/MSIfranchise
Despite an overall trend showing an increase in the number of independent pharmacies that close their doors every year, new standalone stores continue to appear in communities across the U.S. The challenges these independent shops face include the effect of reimbursements on revenue, the cost of managing inventory, as well as difficulty parsing and acting on crucial profit and loss metrics.
THE CHALLENGE There was a time when independent pharmacy owners found it fairly easy to be successful.
CASE STUDY THE CO-BRANDING ADVANTAGE Co-branding with The Medicine Shoppe® provides partners with powerful tools and support, including: • A flat $599 monthly fee to join the network. • Five-year licensing agreement with a 90-day out. • Use of the franchise brand name alone or co-branded with your pharmacy name. • Best-practice sharing from a recognized, high-performing franchise system. • Ability to offer patient care services like immunizations and point-of-care testing. • Business coaching from a dedicated pharmacy Franchise Business Consultant. • Digital marketing tools that drive refills and patient engagement.
Despite the increasing industry hurdles, after starting out with one store, Barnette quickly acquired four more in five years. “I realized I was really an entrepreneur at heart,” he said, “I enjoyed the growth more than being behind the counter.” His growing business, however, is guided by old-fashioned values. Barnette feels that the chain mentality puts profits over people and that customer service is ultimately neglected. Instead, he believes in the power of partnership. He now runs 17 independent pharmacies in Kentucky and Florida with the invaluable help of his partners David Ruth, chief operating officer, John McDaniel, chief executive officer, and David Kelly, who is in charge of expansion. Thirteen of those pharmacies are co-branded as Med-Save Pharmacies, A Member of The Medicine Shoppe® Family. An important part of their operating model is to hire a strong Pharmacist in Charge (PIC) at each location, "they are a critical team member and ensure we deliver the service and care consistent with our values," says Barnette.
THE DECISION Barnette had been a wholesale customer of Cardinal Health for many years. In 2014, a Franchise Business Consultant explained MSI's pharmacy co-branding option as the best of both worlds – combining the national recognition of The Medicine Shoppe® name with the local feel and touch of an independent store. This unique opportunity appealed to Barnette. “I saw it as a strong advantage over other independent pharmacies, while still remaining separate from the chain mentality,” he said. “Adding a new store as A Member of The Medicine Shoppe® Family brand means anyone driving by will know who and what we are.” Ruth agrees. “In our stores, ‘chain’ is a four-letter word, but this partnership gave us a cohesiveness and equity in branding and messaging across our stores.” Medicine Shoppe International's approach to partnering offers more than simple name recognition, however. Valuable built-in services include marketing support, business consulting, financial monitoring, and monthly reporting that would otherwise require time-consuming trials and tribulations.
THE RESULTS Co-branding as A Member of The Medicine Shoppe® Family has streamlined and expanded Barnette’s marketing efforts. “As a group, we are seeing an ability to market ourselves better; we are now basically a family of stores,” said Barnette. “The name enables us to market under one banner across Kentucky and as we expand in other areas.”

CASE STUDY The operational expertise of Medicine Shoppe International's Franchise Business Consultants and Franchise Development team provides important structure and guidance for pharmacy transitioning. Barnette and Ruth concur that the services and support they receive are second to none. “I knew how to run a pharmacy, but not necessarily how to run a business,” Ruth said. “They help in every direction, at every step of the way.” The MSI connection is also a boost for owners who are just starting out. “With MSI's help, you could grow your business from the beginning with the sense that someone is there looking over your shoulder,” Barnette said. “Their Franchise Business Consultants provide security in helping you plan, forecast, and monitor your business.” Their Franchise Development team also offers assistance in locating stores that are available for sale, saving pharmacists valuable time in shopping for new business opportunities.
THE CONCLUSION Choosing the right wholesale partner and franchise team is critical. “From the day I came on board, the Medicine Shoppe International Team treated us as partners, and we received great feedback and help through the co-branding agreement. They helped us develop a sense of community.” The positive experience has been encouraging. “It’s been an evolution for us,” Ruth said. “After we got our feet wet with a few co-branded stores, we saw that it was working well. We decided to increase the connection because of the comfort level we had already gained with The Medicine Shoppe® and Cardinal Health.” The information provided is not intended as an offer to sell or the solicitation of an offer to buy a franchise. Company Background
The Compliance Team, Inc. (TCT) is the USA’s first woman-owned and nurse-led nationally recognized healthcare accreditation organization. Accreditation process simplification (reform) has been our passion since our incorporation in 1994. Indeed, we believe that it should not be difficult or costly for providers to consistently demonstrate patient care delivery excellence. Our Exemplary Provider® Award programs are Medicare-approved and authorized by the Secretary of Health and Human Services to accredit Part A – patient-centered medical home and rural health clinics, as well as all Part B – durable medical equipment, prosthetics, orthotics, and supplies (DMEPOS) providers. In addition, we also offer a complete suite of pharmacy services accreditation programs covering: community and long-term care pharmacy, DMEPOS, infusion, sterile and non-sterile compounding (USP chapters 795, 797, and 800), in-store clinic, mail order, DSMT, telepharmacy, and Patient-Centered Pharmacy Home™; our program for top-of-license pharmacist services.
Process Simplification
TCT’s Safety-Honesty-Caring® quality assurance methods represent healthcare’s first “operations-based” accreditation process that simplifies accreditation by standardizing day-to-day patient care reporting practices. One-on-one mentoring and healthcare’s first plain-language service-specific quality standards combine with hundreds of valueadded extras that result in a dramatically simplified yet thoroughly comprehensive as well as scalable accreditation validation process.
The Details
Daily pharmacy operations are streamlined so that they become integral parts of a measurable quality improvement process. Participants need not change their business models. Friendly to chain drug stores and smaller independents alike.
A TCT advisor leads step-by-step implementation webinars that can be accessed and downloaded for future employee training. A subscription to Patient Quality Measurement™, our national patient satisfaction reporting and benchmarking database is also included, along with hundreds of document templates, regular update notices, great customer service, and much more.
FACS
What to look for in an Rx accreditation organization (AO). n Flexibility: The Compliance Team™ was the first AO to match its quality standards to an individual provider organization’s business model; not the other way around. We also offer a full line of “operationsbased” pharmacy services accreditation tracks that gives pharmacies the opportunity to use them as blueprints to expand into new impactful revenue streams. n Accessibility: The Compliance Team™ was the first AO to feature one-on-one on-going expert mentoring, coast-to-coast coverage during business hours, and 24/7 access to our national patient satisfaction benchmarking database and archived training webinars. Senior management and even our founder and CEO, Sandy Canally, RN, are available most days to take calls. n Consistency: The Compliance Team™ is no slouch when it comes to training our corps of accreditation advisors and office staff. Our customers benefit across the board by getting consistently accurate information and answers to their queries. Regular refresher webinars are reinforced through annual company-wide gatherings that keep us on top of our game. n Simplicity: The Compliance Team™ was the first AO to place an emphasis on accreditation process simplification starting with our industry leading plain language Safety-Honesty-Caring® quality standards that can be adapted to fit any and all pharmacy business models.

CASE STUDY Unleash your potential at cpssparx.com
Key Stakeholders Impacting Hospital-Based Specialty Pharmacies: How to Forge Essential Relationships The importance of patients, providers, clinical staff, payors and pharmaceutical manufacturers Specialty pharmacy is a rapidly growing segment of drug spend, projected to comprise $250 billion in revenues by 2020. Hospitals and health systems are feeling the pressure to build or expand existing specialty pharmacy services in order to compete for patients with chronic complex diseases who require specialty medications.
PATIENTS Patients can be overwhelmed by their complex medication regimens, potential drug side effects, and concerns over the cost of their drugs. Not addressing their concerns can lead to non-adherence. An innovative model created by the University of North Carolina Medical Center was able to improve adherence rates by providing the following support to patients: •
Patient education regarding treatment and side effects before and during treatment
•
Adherence monitoring using patient surveys
•
Adverse effect management to address side effects
In addition, to help with patient drug co-pays, take a look at patient assistance programs offered by pharmaceutical companies. Eligible patients can utilize manufacturer copy discount programs or patient assistance programs to lower out-of-pocket costs for their specialty medications.
PROVIDERS Communication is often fragmented in the healthcare environment where the fax machine and landlines are still used to communicate and coordinate care. A landmark study by Biologics found that the average cancer patient
46
CHALLENGES FACING HOSPITAL BASED SPECIALTY PHARMACY: •
High cost of specialty medications, most over $2,000 per fill
•
Fewer than two percent of patient populations requires these drugs
•
Complex administration and compliance issues
•
Manufacturers may choose to limit specialty drug distribution

saw more than 13 different healthcare providers within a one-year period. With all those moving parts, it is essential to keep providers in the loop regarding their patients who are being treated for chronic complex conditions. Providers want to know that their patients are receiving excellent care and that they will be informed regarding that care and any issues that may arise. Technology is improving information flow through electronic health records and health information exchanges. Healthcare technology startups are creating secure messaging platforms that are HIPAA compliant and enable care teams to communicate with each other. Keeping providers in the communication loop will be increasingly critical as specialty pharmacy services continue to grow.
CLINICAL STAFF Clinical staff are being tasked with even more work in the specialty pharmacy space. Traditionally, an oncology nurse already bears a heavy load of responsibilities, including: •
Review patients’ health history
•
Track laboratory, pathology and imaging studies
•
Communicate with physicians on patients’ behalf
•
Safely administering medications, fluids and cancer treatments
•
Help patients plan and manage symptoms throughout treatment
•
Translate complex medical terminology and answer questions
However, as specialty pharmacy exponentially increases in importance, oncology nurses also need to be prepared to include educating patients on their specialty pharmacy choices, completing prior authorization paperwork and liaising between specialty pharmacy and hospital or clinic. Pharmacists who are integrated into clinical roles can help reduce the burden on already overworked clinical staff. A specialty care model developed at Vanderbilt University Medical Center inserts pharmacists directly into the comprehensive management of patients on specialty medications. Not only does the Vanderbilt model reduce the demands on clinical staff, it also ensures patient adherence and coordinates care and collaboration with providers through the use of electronic medical records.
HOW TO MAKE A HOSPITAL SPECIALTY PHARMACY ATTRACTIVE TO PAYORS: •
Invest in IT and data analytics in order to track and report data and outcomes as required by payors
•
Achieve accreditation from two of these accrediting bodies Utilization Review Accreditation Commission (URAC) Accreditation Commission for Healthcare (ACHC) Center for Pharmacy Practice Accreditation (CPPA)

CASE STUDY Unleash your potential at cpssparx.com
PAYORS Payors are continually looking to better manage their drug spend as medication costs continue to rise and are tightening networks. They expect detailed tracking and reporting of financial, clinical, quality of life, and patient satisfaction scores. Payors are invested in reducing the total cost of care and seeing value for patient treatments. URAC is considered the “gold standard” of accreditation for specialty pharmacy and covers drug-drug interactions, call center performance, dispensing accuracy, distribution accuracy and prescription turnaround time. In the era of value-based care, pharmacies must show how their clinical services are impacting clinical, financial, quality of life and patient satisfaction measures. Accreditation ensures compliance with healthcare’s most comprehensive and rigorous standards, requiring specialty pharmacies to continually measure, monitor and evaluate the services they provide. Importantly, patients directly benefit from this accountability.
MANUFACTURERS Specialty drug manufacturers often restrict access to certain specialty drugs. While each manufacturer has different clinical and reporting requirements, several factors can help a specialty pharmacy secure limited distribution drug contracts: •
Clinical expertise from a knowledgeable care team that manages relationships between patients, caregivers, prescribers and payors
•
Accreditation
•
Technology to track and report financial, operational and clinical outcomes
•
Systems in place for medication handling, dispensing and monitoring requirements
•
Clinical support for specialty diseases or specific patient populations
•
Access to payor contracts
Pharmacies that can demonstrate expertise in specific patient populations or disease states may have an advantage. Having care coordinators, nurses and pharmacists in place who are trained in specific diseases demonstrates to manufacturers that the specialty pharmacy may be a good fit for access to the limited distribution drug. Additionally, providing excellent services to patients taking open distribution drugs can show the manufacturer that the organization is well aligned to take on a limited distribution drug.
CONCLUSION In order to be successful in the specialty drug world, pharmacies must understand and meet the needs of these five stakeholders. CPS SPARX can help your organization create a winning specialty pharmacy strategy that addresses the challenges that these stakeholders present and position your overall organization for success in the specialty pharmacy space.

Service Options
n On-Site Service: Let our experienced representatives take care of your entire return from start to finish. Your dedicated service representative will spend 2-3 hours scanning your prescription shelves for outdated items. Outdated merchandise is scanned into our proprietary software and your rep provides you with a complete inventory and credit estimate the day of your service. All items are boxed and sealed, and a FedEx pickup is scheduled for the next business day. Additional benefits include consultation regarding items with recently released generic equivalents, collection of Schedule II-V items and issuance of all necessary documentation, and collection of recalled items. n Mail-In Service: Do your returns on your own schedule by using our online portal to create returns, inventory items, and print return forms and prepaid UPS shipping labels. Simply scan the bar code or enter the
NDC and the drug information will populate. You can keep a running inventory until you’re ready to ship your items – your list is automatically saved. When you’re ready, submit your return and we’ll mail out a DEA 222 form if you are sending us Schedule II items. Print your prepaid shipping labels and give your boxes to any UPS driver. As soon as we process your return you’ll be able to view your inventory and credit estimate online. n Destruction Service: We provide a safe, simple, and compliant manner for compounding pharmacies, doctor’s offices, manufacturers, and distributors to dispose of unwanted and unsaleable products. Items are transported to an EPA approved, waste-to-energy incinerator where destruction is witnessed by at least two employees. Any hazardous waste is transported and destroyed by EPA licensed firms.
Online Reporting and Analysis
Our newly updated customer portal allows you to view your complete returns history with data down to the NDC level for every return we process for you. The dashboard shows your most recent return, a return history, and a snapshot of trends in your returnable versus non-returnable products. You can print or export return reports, controlled substance reports, proof of destruction, and copies of our checks. You can also manage any direct manufacturer credits or request us to track those for you if you aren’t sure you have received them.

& Q A Shelia Arquette, RPh
Executive Director, National Association of Specialty Pharmacy
Q. Specialty drugs remain the most expensive drugs on the market. What spending trends do you foresee for specialty pharmacy in the future? Since the 1990s, the number of specialty drugs available has increased exponentially and dominated new drug developments. Specialty product spending growth is outpacing traditional product growth and now has approximately a 47% share of the total non-discounted spend.¹ As an example, in November 2019, specialty spend grew by 11.0% while traditional spend increased 0.7%. This increase has been fueled not only by approvals in more common “specialty” diseases such as oncology and hepatitis, but in rare or orphan diseases as well. We are beginning to see an increasing focus on specialty drug development for the treatment of diseases and conditions that were historically managed by small molecule therapies i.e., Alzheimer’s Disease, allergy, heart failure, stroke. In the next five years, oncology, autoimmune, and diabetes will drive therapeutic growth. In 2018, 59 New Active Substances were launched, including 28 orphan NAS’s, resulting in the biggest year for new specialty product launches in history.² The continued growth of specialty spending is driven primarily by the large number of new specialty and orphan drugs available and in the pipeline, as well as the rising prevalence of chronic diseases and an aging population.
Q. What are some of the biggest challenges faced by the specialty pharmacy industry in 2020? Limited Distribution Networks restrict the distribution channel for a specialty drug to one or a very small number of pharmacies. This strategy has advantages and disadvantages. It may help ensure the safe distribution of high-risk drugs to small patient populations, but it limits competition among specialty pharmacies and creates access challenges for patients, payers, and healthcare providers. Managed care access and carve-outs. Health plans and payers continue to limit access to participation in networks, as well as carve out and channel specialty drugs. Patients are forced to use specific specialty pharmacies, which can result in patients receiving medications from multiple pharmacies and not the pharmacy of

their or their physician’s choosing. This can result in confusion and challenges for both patients and healthcare providers who struggle to navigate complex healthcare benefits. Declining reimbursement rates and Direct and Indirect Remuneration (DIR) Fees. Though the specialty pharmacy
industry has grown substantially, labor and drug costs have increased, and reimbursements from health insurers and pharmacy benefit managers (PBMs) continue to decrease. Another challenge faced by the specialty pharmacy industry is Direct and Indirect Remuneration pharmacy fees, more commonly known as DIR fees. A loophole in the Medicare antikickback law allows health plans and PBMs to charge pharmacies fees to fill Medicare prescriptions. DIR fees are applied after the point-of-sale and collected months after the pharmacy has dispensed the drug and after a beneficiary has purchased the drug. Since 2013, fees on pharmacies have totaled $8.5 billion and have grown more than 1,600 percent ³. This is unsustainable and is pushing many independent pharmacies out of business. A recent report dated from June 2018 to July 2019 stated that more than 2,200 independent pharmacies have closed, with many directly blaming DIR fees as the reason.
Q. Can you identify some future trends in specialty pharmacy? Targeted therapies. One of the major trends in specialty pharmacy is managing targeted therapies. Nearly 70 percent of the specialty drugs in development have an identified biomarker associated with them. Biomarkers help providers match patients with the best therapy right from the start and help manufacturers demonstrate just how successful their drugs can be. Biomarkers and genomics are used to personalize treatment regimens and determine precisely which medications are best suited for the patient, resulting in superior clinical and cost-effective outcomes. Specialty pharmacy can serve as an invaluable resource to help identify the optimal targeted therapy and further demonstrate the value they deliver as medication therapy experts, and also demonstrate why they are a critical member of the healthcare delivery team!
value-based contracting moving forward. Though managing the cost of specialty products has focused mainly on drug prices in the past, value-based contracting seeks to link the price of a drug to clinical or economic performance. Specialty pharmacies fulfill a key role in providing high-touch patient care and in the coordination of care with healthcare providers, resulting in improved outcomes. This type of contracting will require a great deal of collaboration, coordination, and some financial risk, but will focus on quality care and services as opposed to cost alone. This is another tremendous opportunity for specialty pharmacy to make a difference.
Drug pricing reform. Legislative and regulatory activity is underway to address the high cost of prescription drugs, including specialty drugs. Various policies under consideration would directly reduce drug costs at the point those drugs are sold – whether at the counter or being directly provided to a specialty patient. For specialty patients, payment reform referred to as “pharmacy DIR reform” would bring immediate relief to specialty patients, reducing a senior’s co-pay costs on expensive therapies that do not have generic alternatives. Pharmacy DIR reform ensures all pharmacy price concessions are used by a plan to reduce a Medicare patient’s upfront cost. This reform also would support specialty pharmacies in being able to ensure they can provide the support services needed to improve patient outcomes through drug management.
Q. How do you see the role of the specialty pharmacist evolving? There is an active debate about giving pharmacists provider status under Medicare. This would allow pharmacists to be recognized and compensated for the services that they provide. Specialty pharmacists are an integral part of the patient’s overall healthcare team and help facilitate complete communication between the care team and the patient. They can play a larger
role in advising care and working alongside physicians and nurses on prescribing decisions and assisting with the transition of care. Their specialized training in chronic conditions can lead to impactful interventions, and reductions in hospitalizations and unnecessary healthcare costs.
Q. What is the role and the future of technology in specialty pharmacy? Specialty pharmacy technologies will continue to improve communication and engagement with patients and caregivers, improve workflows, drive adherence and improved clinical management, and identify medication safety and risk, which will demand improved dialogue between pharmacies and prescribers. Improved communication with patients and prescribers could significantly reduce medication mismanagement, potential side effects, and the risk of adverse drug events. As technology continues to advance, more data and information is shared between the entire healthcare team, including health plans, pharmacy benefits managers, providers, and manufacturers. This team approach will ultimately result in improved patient care and outcomes.
Q. Is there anything else you’d like to add that you haven’t already covered? With all of this in mind, it is essential that we do not lose sight of the fact that the patient is at the center of all we do. Specialty pharmacies are vital members of the patient’s healthcare team and will continue to evolve as needed to provide the optimal model of care for patients living with chronic illnesses and complex medical conditions, achieve superior clinical and economic outcomes, and expedite patient access to care. Testimonial
“ Prescriptions ready for pick up will appear on the POS screen. When the initial Rx bar code is scanned, the patient’s name automatically appears. The nutritional supplements and any other general merchandise the customer wants to purchase can be scanned in the same transaction, and the system prompts the patient to sign for their prescriptions – saving us a massive amount of time so we can provide great customer service.”

REDUCING
~6.3 MILLION
OF WHICH HAVE CVD OR DIABETES (AND ≥2 RISK FACTORS)
~12 MILLION
OF WHICH HAVE TRIGLYCERIDE LEVELS ≥150 mg/dL
~39 MILLION PEOPLE ARE STATIN-TREATED
INDICATIONS AND LIMITATIONS OF USE
• VASCEPA® (icosapent ethyl) is indicated as an adjunct to maximally tolerated statin therapy to reduce the risk of myocardial infarction, stroke, coronary revascularization and unstable angina requiring hospitalization in adult patients with elevated triglyceride (TG) levels (≥150 mg/dL) and established cardiovascular disease or diabetes mellitus and 2 or more additional risk factors for cardiovascular disease • VASCEPA is indicated as an adjunct to diet to reduce TG levels in adult patients with severe (≥500 mg/dL) hypertriglyceridemia The effect of VASCEPA on the risk for pancreatitis in patients with severe hypertriglyceridemia has not been determined.
IMPORTANT SAFETY INFORMATION
• VASCEPA is contraindicated in patients with known hypersensitivity (e.g., anaphylactic reaction) to VASCEPA or any of its components Please see additional Important Safety Information on inside pages and back cover.

Pharmacist: Hi! How are you doing today? Patient: Same old. I’m here for my usual. Pharmacist: You know, you might want to ask your doctor about whether the statin you’re taking is enough for protecting your heart. Research has shown that despite being on a statin, patients continue to be at risk. Maybe you can talk to your doctor about that. Patient: Oh, really? I’ll ask during my upcoming checkup. Actually, I’ve heard about fish oils being good. Omega-3, right? Should I add that as a supplement to my medications? Pharmacist: Many fish oil supplements have not been proven to benefit the heart. The FDA does not regulate their manufacturing, purity, or content, and they have not shown evidence of efficacy or safety in clinical trials. As such, fish oil products are not approved to treat, cure, or prevent any medical condition. Patient: I didn’t know that! Well then, is there an alternative? Pharmacist: There is something else that is FDA approved that might help you. It’s called VASCEPA. Let me write it down for you. It’s been proven in a large clinical trial with over 8000 people and has been shown to reduce the risk of cardiovascular events by 25%, including heart attacks and strokes when added to a statin. Patient: Oh, I’m interested. I’ll be sure to ask my doctor about this and see if they’d recommend adding it to my treatment. Thanks! Pharmacist: No problem! If you like, I can call your doctor directly and talk to them about prescribing it.
IMPORTANT SAFETY INFORMATION (continued)
• VASCEPA was associated with an increased risk (3% vs 2%) of atrial fibrillation or atrial flutter requiring hospitalization in a double-blind, placebo-controlled trial. The incidence of atrial fibrillation was greater in patients with a previous history of atrial fibrillation or atrial flutter • It is not known whether patients with allergies to fish and/or shellfish are at an increased risk of an allergic reaction to VASCEPA. VASCEPA: THE FIRST AND ONLY FDA-APPROVED, ORAL, SINGLE-AGENT IPE WITH PROVEN CV REDUCTION VASCEPA is indicated as an adjunct to maximally tolerated statin therapy to reduce the risk of myocardial infarction, stroke, coronary revascularization, and unstable angina requiring hospitalization in adult patients with elevated triglyceride (TG) levels (≥150 mg/dL) and established CVD or diabetes mellitus and two or more additional risk factors for CVD.1 VASCEPA is also indicated as an adjunct to diet to reduce TG levels in adult patients with severe (≥500 mg/dL) hypertriglyceridemia.1
REDUCE-IT STUDY DESIGN1,22 Placebo-controlled, double-blind trial
8179 adults with: • Well-controlled LDL-C • Elevated TG levels
Statin + VASCEPA 4 g/d

71% secondary CVD prevention ≥45 yrs old + existing CVD
Median follow-up:4.9 years
VASCEPA is a prescription medication and is the only omega-3 fatty acid product to show CV benefit in a clinical trial.1.22 In December 2019, VASCEPA was granted a new indication based on the Reduction of Cardiovascular Events with Icosapent Ethyl–Intervention Trial (REDUCE-IT), which was conducted globally with an emphasis on a Western population and enrolled 8179 patients.1,22 The trial investigated the effects of VASCEPA on the occurrence of a major adverse CV event in adults 45 years of age or older who were on statin therapy with well-controlled LDL-C levels between 40 mg/dL and 100 mg/dL and elevated TG levels (90% of enrolled patients had TG levels between 150 mg/dL and 500 mg/dL), and either established CVD (71%) or diabetes and at least one additional CV risk factor (29%).1,22 Patients were randomized 1:1 VASCEPA 4 g/day (2 g twice a day with food; n=4089) or placebo (n=4090).1,22 The median follow-up duration was 4.9 years.1,22 VASCEPA 4 g/day administration resulted in a significant reduction of 25% in risk of CV events relative to placebo (hazard ratio [HR] 0.75, 95% confidence interval [CI] 0.68–0.83; p<0.001), evident after approximately 1 year of treatment.1,22 Treatment with VASCEPA for 1 year also significantly lowered LDL-C and TG levels from baseline compared to placebo. The median change

(5-point MACE was defined as myocardial infarction, stroke, coronary revascularization, unstable angina requiring hospitalization, and CV death)
in TG from baseline after 1 year of taking VASCEPA was -39 mg/dL (-18%) compared to 5 mg/dL (2%) in the placebo group.1 Moreover, the median change in LDL-C from baseline after 1 year of taking VASCEPA was 2 mg/dL (3%) compared to 7 mg/dL (10%) in the placebo group.1 VASCEPA has a well-established safety profile.22 Common adverse reactions that occurred in the VASCEPA group during REDUCE-IT at a frequency of at least 3% and at least 1% more frequently than in the placebo group included musculoskeletal pain, peripheral edema, constipation, gout, and atrial fibrillation.1 VASCEPA has been associated with increased risks of bleeding and atrial fibrillation/flutter, the latter being more prevalent in patients with a previous history of atrial fibrillation or flutter.1,2 It should be noted that the exact mechanisms of action contributing to the reduction of CV events with VASCEPA are not completely understood but are likely multi-factorial.1
VASCEPA’S EFFICACY1,22 reduction in risk of composite 1st occurrence of 5-point MACE when using VASCEPA relative to placebo
VASCEPA’S SAFETY PROFILE1 Common adverse reactions at a frequency of ≤3% and ≤1% more frequently in the VASCEPA group than in the placebo group include:
Musculoskeletal pain
IMPORTANT SAFETY INFORMATION (continued)
Peripheral edema
Constipation
Gout
Atrial fibrillation
• VASCEPA was associated with an increased risk (12% vs 10%) of bleeding in a double-blind, placebo-controlled trial. The incidence of bleeding was greater in patients receiving concomitant antithrombotic medications, such as aspirin, clopidogrel or warfarin • Common adverse reactions in the cardiovascular outcomes trial (incidence ≥3% and ≥1% more frequent than placebo): musculoskeletal pain (4% vs 3%), peripheral edema (7% vs 5%), constipation (5% vs 4%), gout (4% vs 3%) and atrial fibrillation (5% vs 4%)

FDA-APPROVED VASCEPA IS NOT SUBSTITUTABLE WITH GENERIC OMEGA-3 FATTY ACID PRODUCTS CV outcomes studies of prescription omega-3 mixture products, containing docosahexaenoic acid (DHA), have failed to demonstrate CV benefit on top of statins.22-31
The results of VASCEPA in the REDUCE-IT trial cannot be extrapolated to generic omega-3 fatty acid products, which have failed to demonstrate an impact on the persistent risk of CV events in large-scale outcomes trials.22-27 However, it should be noted that no head-to-head trials have been conducted comparing VASCEPA and these generic omega-3 fatty acid drugs, and that crosstrial comparisons are subject to differences in populations, primary outcomes, and other trial design aspects.
acid eicosapentaenoic acid (EPA).1,28 Unlike VASCEPA, generic omega-3 fatty acid drugs contain both EPA and DHA.29 While use of either VASCEPA or generic omega-3 fatty acid drugs have demonstrated lower TG levels, DHA-containing products may raise LDL-C in patients with elevated TG.1,23-27,30,31 Moreover, DHA-containing products are not FDA approved for co-administration with statins to affect lipid, lipoprotein, or inflammation parameters with the aim of reducing CV mortality or morbidity.
It still stands that VASCEPA is the first and only medication indicated to reduce CV risk in high-risk patients approved for treatment.1 VASCEPA is highly refined, naturally derived, and consists primarily of the omega-3 fatty
VASCEPA is IPE, the only FDA-approved EPA that has undergone a proprietary purification process to extract toxins, saturated fats, and LDL-raising DHA, and—once again—it is the only EPA approved to reduce CV risk.1,28

Temperature Intelligence Solutions by Temptime – Monitoring Tools and Systems for Medication Shipments, Storage, and Pharmacy Facilities Providing the tools and knowledge to accurately monitor temperature sensitive medication, ensuring they have not been exposed to temperature events that could impact their effectiveness in treating patients.

Anders Gustafson Founded: 1969 Employees: 7,400 Phone: (973) 630-6000 Fax: (973) 630-6100 Address: 116 The American Road Morris Plains, NJ 07950 Website: www.temptimecorp.com
Company Background In 2019 Zebra – an innovator at the edge of enterprise solutions with expertise in healthcare, among other industries – acquired manufacturer Temptime Corporation. More than 30 years ago, Temptime developed the first miniaturized technology to monitor time-temperature exposure of vials containing the oral polio vaccine. Since then the company has deepened its commitment to public health by expanding its suite of easy-to-use, patient-centric temperature monitoring solutions to include those designed for both pharmacy and patient use. Product Overview Through Temptime’s manufacturing facility, Zebra offers a comprehensive range of temperature monitoring solutions to meet the cold chain needs of specialty pharmacies and their patients. Their products include: • Low-cost, chemically-based heat and freeze indicators for blood products, vaccines, biologics, and other medication. • Heat and freeze indicators for monitoring medication during shipping. • Sophisticated electronic, wireless temperature and humidity monitoring systems with cloud-based data storage and sharing. By reducing the risk and cost associated with unpredictable temperature events in production, storage, and shipment – Zebra is striving to improve supply chain efficiency, global health, and patient care.
Product Specifications n Wireless Monitoring Systems The Temptime family of wireless Bluetooth enabled sensors monitor and record temperature data 24/7.
The W-200 is designed for facilities and warehouses, monitoring both temperature and humidity in medication storage areas to help with accreditation compliance and regulatory requirements. Data can be accessed remotely using a web-based application and customized alerts notify users instantly if any areas have exceeded temperature limits. The M-300 and S-400 programmable data loggers are ideal for monitoring temperatures during transport or storage, letting users: • View and share temperature data using a free mobile app. • Customize alarm limits, datalogging intervals, startup options, and more. • Store unlimited data on the cloud and generate reports with a complementary web app. n Performance Qualification Testing For specialty pharmacies, performance qualification (PQ) testing ensures that the critical and often lifesaving medications they send to patients have not been exposed to unsafe temperatures, which could impact their effectiveness. More than that, PQ testing is also required for certain pharmacy accreditations and participation in some pharmacy networks. These wireless monitoring devices help specialty pharmacies simplify PQ testing to optimize pack-outs. Customized ThirdParty PQ Testing services also help meet the requirements of pharmacy networks that make third-party validation testing a condition of participation. n TransTracker® Shipment Indicators TransTracker® visual temperature indicators monitor medication heat and freeze events during shipment and storage. They are simple to read and understand so patients can tell at-a-glance if medication has been exposed to extreme temperatures – cutting down on unnecessary, costly reshipments due to suspected temperature damage. These costeffective, single-use indicators also help specialty pharmacies comply with state regulations. SPRING 2020
