Pharmacists are among the most trusted and most accessible of all healthcare professionals (HCPs).7,8 In addition, they are one of the most frequently visited members of a healthcare team.8,9 Many of the frequent visitors of community pharmacies are patients with cardiovascular disease (CVD).10 Statin therapy, the first-line option for high cholesterol, lowers levels of low-density lipoprotein cholesterol (LDL-C), or “bad” cholesterol, and has helped reduce the risk of developing life-threatening blockages in the arteries that can cause heart attacks and strokes.5,11-13 Despite the LDL-C–lowering effects of statin therapy, persistent CV risk remains.14 Other drugs, including but not limited to those listed below, are sometimes recommended in patients that cannot tolerate statins or in addition to statins for patients responding inadequately or with severe hypercholesterolemia.3,15

• Ezetimibe: An oral cholesterol absorption inhibitor taken in combination with statin therapy that has shown reduction in LDL-C levels and in the risk of heart attacks and ischemic strokes; however, it has not shown reduced risk of other CV events compared to when statin therapy is taken alone.16 • Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors: Recently shown to improve CV outcomes when added to statin therapy,17,18 these monoclonal antibodies are injectables and tend to be limited due to high-cost adoption.19,20 • VASCEPA (IPE): An oral medication containing an ethyl ester of omega-3 fatty acid that was recently FDA approved and clinically shown to reduce the risk of CV events.1,2 The American Diabetes Association has updated their 2019 guidelines to include IPE as a top-tier recommendation for CV risk reduction, and further recommendations to use IPE have been made by the European Society

Despite an overall trend showing an increase in the number of independent pharmacies that close their doors every year, new standalone stores continue to appear in communities across the U.S. The challenges these independent shops face include the effect of reimbursements on revenue, the cost of managing inventory, as well as difficulty parsing and acting on crucial profit and loss metrics. Key Stakeholders Impacting Hospital-Based Specialty Pharmacies: How to Forge Essential Relationships The importance of patients, providers, clinical staff, payors and pharmaceutical manufacturers Specialty pharmacy is a rapidly growing segment of drug spend, projected to comprise $250 billion in revenues by 2020. Hospitals and health systems are feeling the pressure to build or expand existing specialty pharmacy services in order to compete for patients with chronic complex diseases who require specialty medications. Q. Specialty drugs remain the most expensive drugs on the market. What spending trends do you foresee for specialty pharmacy in the future? Since the 1990s, the number of specialty drugs available has increased exponentially and dominated new drug developments. Specialty product spending growth is outpacing traditional product growth and now has approximately a 47% share of the total non-discounted spend.¹ As an example, in November 2019, specialty spend grew by 11.0% while traditional spend increased 0.7%. This increase has been fueled not only by approvals in more common “specialty” diseases such as oncology and hepatitis, but in rare or orphan diseases as well. We are beginning to see an increasing focus on specialty drug development for the treatment of diseases and conditions that were historically managed by small molecule therapies i.e., Alzheimer’s Disease, allergy, heart failure, stroke. In the next five years, oncology, autoimmune, and diabetes will drive therapeutic growth. In 2018, 59 New Active Substances were launched, including 28 orphan NAS’s, resulting in the biggest year for new specialty product launches in history.² The continued growth of specialty spending is driven primarily by the large number of new specialty and orphan drugs available and in the pipeline, as well as the rising prevalence of chronic diseases and an aging population.

Q. What are some of the biggest challenges faced by the specialty pharmacy industry in 2020? Limited Distribution Networks restrict the distribution channel for a specialty drug to one or a very small number of pharmacies. This strategy has advantages and disadvantages. It may help ensure the safe distribution of high-risk drugs to small patient populations, but it limits competition among specialty pharmacies and creates access challenges for patients, payers, and healthcare providers. Managed care access and carve-outs. Health plans and payers continue to limit access to participation in networks, as well as carve out and channel specialty drugs. Patients are forced to use specific specialty pharmacies, which can result in patients receiving medications from multiple pharmacies and not the pharmacy of

their or their physician’s choosing. This can result in confusion and challenges for both patients and healthcare providers who struggle to navigate complex healthcare benefits. Declining reimbursement rates and Direct and Indirect Remuneration (DIR) Fees. Though the specialty pharmacy

industry has grown substantially, labor and drug costs have increased, and reimbursements from health insurers and pharmacy benefit managers (PBMs) continue to decrease. Another challenge faced by the specialty pharmacy industry is Direct and Indirect Remuneration pharmacy fees, more commonly known as DIR fees. A loophole in the Medicare antikickback law allows health plans and PBMs to charge pharmacies fees to fill Medicare prescriptions. DIR fees are applied after the point-of-sale and collected months after the pharmacy has dispensed the drug and after a beneficiary has purchased the drug. Since 2013, fees on pharmacies have totaled $8.5 billion and have grown more than 1,600 percent ³. This is unsustainable and is pushing many independent pharmacies out of business. A recent report dated from June 2018 to July 2019 stated that more than 2,200 independent pharmacies have closed, with many directly blaming DIR fees as the reason.

Q. Can you identify some future trends in specialty pharmacy? Targeted therapies. One of the major trends in specialty pharmacy is managing targeted therapies. Nearly 70 percent of the specialty drugs in development have an identified biomarker associated with them. Biomarkers help providers match patients with the best therapy right from the start and help manufacturers demonstrate just how successful their drugs can be. Biomarkers and genomics are used to personalize treatment regimens and determine precisely which medications are best suited for the patient, resulting in superior clinical and cost-effective outcomes. Specialty pharmacy can serve as an invaluable resource to help identify the optimal targeted therapy and further demonstrate the value they deliver as medication therapy experts, and also demonstrate why they are a critical member of the healthcare delivery team!


value-based contracting moving forward. Though managing the cost of specialty products has focused mainly on drug prices in the past, value-based contracting seeks to link the price of a drug to clinical or economic performance. Specialty pharmacies fulfill a key role in providing high-touch patient care and in the coordination of care with healthcare providers, resulting in improved outcomes. This type of contracting will require a great deal of collaboration, coordination, and some financial risk, but will focus on quality care and services as opposed to cost alone. This is another tremendous opportunity for specialty pharmacy to make a difference.

Drug pricing reform. Legislative and regulatory activity is underway to address the high cost of prescription drugs, including specialty drugs. Various policies under consideration would directly reduce drug costs at the point those drugs are sold – whether at the counter or being directly provided to a specialty patient. For specialty patients, payment reform referred to as “pharmacy DIR reform” would bring immediate relief to specialty patients, reducing a senior’s co-pay costs on expensive therapies that do not have generic alternatives. Pharmacy DIR reform ensures all pharmacy price concessions are used by a plan to reduce a Medicare patient’s upfront cost. This reform also would support specialty pharmacies in being able to ensure they can provide the support services needed to improve patient outcomes through drug management.

Q. How do you see the role of the specialty pharmacist evolving? There is an active debate about giving pharmacists provider status under Medicare. This would allow pharmacists to be recognized and compensated for the services that they provide. Specialty pharmacists are an integral part of the patient’s overall healthcare team and help facilitate complete communication between the care team and the patient. They can play a larger

role in advising care and working alongside physicians and nurses on prescribing decisions and assisting with the transition of care. Their specialized training in chronic conditions can lead to impactful interventions, and reductions in hospitalizations and unnecessary healthcare costs.

Q. What is the role and the future of technology in specialty pharmacy? Specialty pharmacy technologies will continue to improve communication and engagement with patients and caregivers, improve workflows, drive adherence and improved clinical management, and identify medication safety and risk, which will demand improved dialogue between pharmacies and prescribers. Improved communication with patients and prescribers could significantly reduce medication mismanagement, potential side effects, and the risk of adverse drug events. As technology continues to advance, more data and information is shared between the entire healthcare team, including health plans, pharmacy benefits managers, providers, and manufacturers. This team approach will ultimately result in improved patient care and outcomes.

Q. Is there anything else you’d like to add that you haven’t already covered? With all of this in mind, it is essential that we do not lose sight of the fact that the patient is at the center of all we do. Specialty pharmacies are vital members of the patient’s healthcare team and will continue to evolve as needed to provide the optimal model of care for patients living with chronic illnesses and complex medical conditions, achieve superior clinical and economic outcomes, and expedite patient access to care. CARDIOVASCULAR RISK THE ROLE OF THE PHARMACIST

PHARMACISTS CAN HELP PREVENT THE NEXT CARDIOVASCULAR EVENT VASCEPA® (icosapent ethyl [IPE])—not your typical omega-3 fatty acid drug—could help millions of high-risk patients, including statin-treated patients and statin-intolerant patients, to reduce their cardiovascular (CV) risk.1,2 Multiple professional societies have updated guidelines or issued advisories recognizing IPE, including the American Diabetes Association, the European Society of Cardiology/European Atherosclerosis Society, the National Lipid Association, and the American Heart Association.3-6

Pharmacists are among the most trusted and most accessible of all healthcare professionals (HCPs).7,8 In addition, they are one of the most frequently visited members of a healthcare team.8,9 Many of the frequent visitors of community pharmacies are patients with cardiovascular disease (CVD).10 Statin therapy, the first-line option for high cholesterol, lowers levels of low-density lipoprotein cholesterol (LDL-C), or “bad” cholesterol, and has helped reduce the risk of developing life-threatening blockages in the arteries that can cause heart attacks and strokes.5,11-13 Despite the LDL-C–lowering effects of statin therapy, persistent CV risk remains.14 Other drugs, including but not limited to those listed below, are sometimes recommended in patients that cannot tolerate statins or in addition to statins for patients responding inadequately or with severe hypercholesterolemia.3,15

Encouragingly, studies have shown that during pharmacy visits, short educational interventions by pharmacists can positively influence their patients’ decision making, especially as it pertains to taking control and managing CV risk factors—highlighting the critical role of pharmacists in patient care.21 An example patient journey is presented to demonstrate the impact pharmacists have on patient outlook. MILLIONS TAKING STATIN-BASED STANDARD OF CARE EXPERIENCE PERSISTENT CVD

• Ezetimibe: An oral cholesterol absorption inhibitor taken in combination with statin therapy that has shown reduction in LDL-C levels and in the risk of heart attacks and ischemic strokes; however, it has not shown reduced risk of other CV events compared to when statin therapy is taken alone.16 • Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors: Recently shown to improve CV outcomes when added to statin therapy,17,18 these monoclonal antibodies are injectables and tend to be limited due to high-cost adoption.19,20 • VASCEPA (IPE): An oral medication containing an ethyl ester of omega-3 fatty acid that was recently FDA approved and clinically shown to reduce the risk of CV events.1,2 The American Diabetes Association has updated their 2019 guidelines to include IPE as a top-tier recommendation for CV risk reduction, and further recommendations to use IPE have been made by the European Society

Research has shown that despite being on a statin, patients continue to be at risk. Maybe you can talk to your doctor about that. Patient: Oh, really? I’ll ask during my upcoming checkup. Actually, I’ve heard about fish oils being good. Omega-3, right? Should I add that as a supplement to my medications? Pharmacist: Many fish oil supplements have not been proven to benefit the heart. The FDA does not regulate their manufacturing, purity, or content, and they have not shown evidence of efficacy or safety in clinical trials. VASCEPA is a prescription medication and is the only omega-3 fatty acid product to show CV benefit in a clinical trial.1.22 In December 2019, VASCEPA was granted a new indication based on the Reduction of Cardiovascular Events with Icosapent Ethyl–Intervention Trial (REDUCE-IT), which was conducted globally with an emphasis on a Western population and enrolled 8179 patients.1,22 The trial investigated the effects of VASCEPA on the occurrence of a major adverse CV event in adults 45 years of age or older who were on statin therapy with well-controlled LDL-C levels between 40 mg/dL and 100 mg/dL and elevated TG levels (90% of enrolled patients had TG levels between 150 mg/dL and 500 mg/dL), and either established CVD (71%) or diabetes and at least one additional CV risk factor (29%).1,22 Patients were randomized 1:1 VASCEPA 4 g/day (2 g twice a day with food; n=4089) or placebo (n=4090).1,22 The median follow-up duration was 4.9 years.1,22 VASCEPA 4 g/day administration resulted in a significant reduction of 25% in risk of CV events relative to placebo (hazard ratio [HR] 0.75, 95% confidence interval [CI] 0.68–0.83; p<0.001), evident after approximately 1 year of treatment.1,22 Treatment with VASCEPA for 1 year also significantly lowered LDL-C and TG levels from baseline compared to placebo. The median change

in TG from baseline after 1 year of taking VASCEPA was -39 mg/dL (-18%) compared to 5 mg/dL (2%) in the placebo group.1 Moreover, the median change in LDL-C from baseline after 1 year of taking VASCEPA was 2 mg/dL (3%) compared to 7 mg/dL (10%) in the placebo group.1 VASCEPA has a well-established safety profile.22 Common adverse reactions that occurred in the VASCEPA group during REDUCE-IT at a frequency of at least 3% and at least 1% more frequently than in the placebo group included musculoskeletal pain, peripheral edema, constipation, gout, and atrial fibrillation.1 VASCEPA has been associated with increased risks of bleeding and atrial fibrillation/flutter, the latter being more prevalent in patients with a previous history of atrial fibrillation or flutter.1,2 It should be noted that the exact mechanisms of action contributing to the reduction of CV events with VASCEPA are not completely understood but are likely multi-factorial.1

FDA-APPROVED VASCEPA IS NOT SUBSTITUTABLE WITH GENERIC OMEGA-3 FATTY ACID PRODUCTS CV outcomes studies of prescription omega-3 mixture products, containing docosahexaenoic acid (DHA), have failed to demonstrate CV benefit on top of statins.22-31

The results of VASCEPA in the REDUCE-IT trial cannot be extrapolated to generic omega-3 fatty acid products, which have failed to demonstrate an impact on the persistent risk of CV events in large-scale outcomes trials.22-27 However, it should be noted that no head-to-head trials have been conducted comparing VASCEPA and these generic omega-3 fatty acid drugs, and that crosstrial comparisons are subject to differences in populations, primary outcomes, and other trial design aspects.

acid eicosapentaenoic acid (EPA).1,28 Unlike VASCEPA, generic omega-3 fatty acid drugs contain both EPA and DHA.29 While use of either VASCEPA or generic omega-3 fatty acid drugs have demonstrated lower TG levels, DHA-containing products may raise LDL-C in patients with elevated TG.1,23-27,30,31 Moreover, DHA-containing products are not FDA approved for co-administration with statins to affect lipid, lipoprotein, or inflammation parameters with the aim of reducing CV mortality or morbidity.

It still stands that VASCEPA is the first and only medication indicated to reduce CV risk in high-risk patients approved for treatment.1 VASCEPA is highly refined, naturally derived, and consists primarily of the omega-3 fatty
