WAY

Pharmacy Work Flow Manager by Holon Solutions

Integrate resources across your pharmacies to ensure consistency and help balance workloads.

The Simplest and Most Compliant Pharmaceutical Waste Program Available

Clinicians simply put both the non-hazardous and the RCRA hazardous pharmaceutical waste in the same black container.* Clean Harbors, the largest hazardous waste incineration company in North America, will pickup, transport, dispose of, and keep you in full compliance. We own every step of the process and don’t broker your waste to someone else, like your red container medical waste provider has to. Ensuring compliance to waste segregation protocols can be difficult, both to train staff and to enforce. This can put your hospital at risk for regulatory action.

Cleanroom wipers look white but are they really clean?

Cleanroom wipers are made by hand, but people are the greatest source of contamination. A standard wiper adds great risk through introduction of dirt, particles and other unwanted intruders into your cleanroom.

  

Sterile Vertex, The Ultra-Clean Wiper. Texwipe developed a patent-pending, fully-automated system to wash, cut and pack wipers without human hands. The robotic technology guides each wiper through its production and ensures consistency from wiper to wiper, bag to bag and lot to lot.

Narcotic BoxPicker Automated Storage and Retrieval by Swisslog Narcotic BoxPicker is the innovative medication storage and retrieval solution providing vault-like secure storage for controlled substances, bulk medications and supplies with the capacity of completing up to 300 transactions per hour. President:

Mike Hoganson Founded: 1915 Employees: >250 Phone: (800) 525-1841 Address: Swisslog Healthcare Solutions 10825 East 47th Avenue Denver, CO 80239 Website: www.swisslog.com Email: healthcare@swisslog.com

Company Background

Swisslog Healthcare Solutions began in 1915 as The Grover Company, a provider of pneumatic tube systems for local Detroit businesses. Over the years the company has transformed into a leading provider of integrated logistics solutions for healthcare facilities around the world. Its comprehensive product portfolio ranges from pharmacy automation to medication management and materials transport systems. Swisslog’s solutions optimize customers’ production, logistics and distribution processes in order to increase flexibility, responsiveness and quality of care while minimizing logistics costs. Decades of experience in the development and implementation of integrated logistics solutions have led more than 2,000 hospitals throughout North America to install Swisslog products. The North American Division of Swisslog Healthcare Solutions is based in Denver, CO.

Product Overview

Narcotic BoxPicker is an automated pharmacy warehouse unlike anything else on the market today. It is THE alternative to vertical carousels and pharmacy vaults. It provides high-density storage and retrieval of medications and supplies in non-refrigerated, refrigerated and dual temperature models. BoxPicker provides added security by limiting access to only a single bin at a time during all loading and dispensing operations. Designed to meet regulatory requirements for controlled substances, the narcotics are stored separately from non-controlled substances in secure, tamper-proof modules. This unique solution to narcotic storage and retrieval allows the pharmacy to pick controlled substances and other medications at a rate up to 3 times faster than a carousel, averaging 5 picks per minute and up to 300 transactions an hour. BoxPicker utilizes scan in and out practice, and as a result provides extremely accurate inventory control while optimizing pharmacy workflow.

indicates controlled substance and provides a visual indication of the location of the medication within BoxPicker. The internal robotic arm quickly picks the necessary bin and delivers it to a secured drawer. Once in place, a light indicates which drawer the bin was delivered to and unlocks the drawer. Access is limited to a single drawer at a time to eliminate picking errors. The technician is asked to take a blind inventory. The date, time and user information for each transaction is tracked and available in reporting or other proactive means. The barcode is then scanned for verification and further inventory tracking.

Features & Benefits

• Requires blind inventory for tracking and reporting • Limits access to one storage bin at a time • Picking speed of more than 3 to 1 when compared to carousels • Bio-authentication • Stand-alone alarm system • Emergency access in event of power outage • Requires no floor reinforcement • Software tracks inventory, access and interfaces directly with hospital information systems

Configuration

• Option for multiple operator stations • Modular for customized configuration and easy expansion • Refrigerated, non-refrigerated and dual temperature options • USP 797 Clean Room Application

Lipoderm® Transdermal Bases by PCCA Elegant, proven delivery of “medications in a cream for your patients.”

CEO:

David Sparks Founded: 1981 Employees: 250+ Toll Free Phone: (800) 331-2498 Phone: (281) 933-6948 Fax: (281) 933-6948 Address: 9901 South Wilcrest Drive Houston, TX 77099 Website: www.pccarx.com

Company Background Three decades ago, a physician encountered a patient who required an anti-nausea medication that was no longer commercially available. The physician challenged a Houstonarea pharmacist to compound the medication. The prescription was a success, and the pharmacist saw that others in his field faced similar demands to help patients who required compounded medications. This network of pharmacists, united by a commitment to meet patient needs, was the foundation of PCCA (Professional Compounding Centers of America). Today, PCCA has become the independent compounding pharmacist’s complete resource for fine chemicals, equipment, devices, flavors, ACPE-accredited training and education, pharmacy software, marketing, business and pharmacy consulting assistance. Our membership includes more than 3,900 independent community pharmacists in the United States, Canada, Australia, and other countries around the world.

transdermal needs because it is a stable system that does not separate upon refrigeration and has great resiliency in the presence of ionic substances. • L ipoderm ActiveMax™ has the benefits of original Lipoderm with an added stabilizing system to provide resiliency in the presence of high salt loads. How do you know when/if you need Lipoderm ActiveMax? If you are using over 20% actives in a formula and experiencing separation or liquification. For example, 30% Ketamine HCl in Lipoderm ActiveMax demonstrated superior physicochemical attributes, remaining stable and elegant for six months. •A nhydrous Lipoderm utilizes similar technology to the wellstudied and popular base, Lipoderm, and is an anhydrous version for actives that are unstable in water. It is also free of gluten, casein, egg, wheat-germ, paraben, butylated hydroxytoluene (BHT) and soy. This base utilizes rapeseed-based phospholipids, and may provide a better alternative for autistic patients than traditional Lipoderm. It also has a neutral odor and silky feel. • Lipoderm High Molecular Weight™, the latest in transdermal technology, is focused on the delivery of large molecular weight proteins. Lipoderm HMW does not contain lecithin, phospholipids or eggs, and could be an appropriate choice for autistic patients. This base has a low resiliency for salt forms of drugs, which cause the base to break. For more information and to download studies, check out www.pccarx.com/lipoderm.

Product Overview

LIPODERM® TRANSDERMAL BASES Available

exclusively from PCCA member pharmacies, the Lipoderm family of transdermal bases was independently tested, using the Franz Skin Finite Dose Model, and proven to deliver up to four drugs simultaneously through human skin in vitro. Also tested for allergic reactions – no adverse reactions were reported in an extensive two-month study. The Lipoderm family includes:

• Lipoderm is an elegant alternative to traditional PLO gels and contains a proprietary liposomal component that may increase the permeation of a variety of actives. Its touch and feel is smooth and creamy, unlike the tacky feel of PLOs. Many members have made the switch to Lipoderm for their SUMMER 2012

Remote Order Entry Pharmacy Services Rx RemoteSolutions (RemoteSolutions) RemoteSolutions provides safe, efficient, office-based medication order entry and verification services which meet The Joint Commission standards, HIPAA regulations, and applicable state licensing requirements.

Product Overview RemoteSolutions provides 24/7 office-based medication order entry and verification services. Many hospitals struggle to find adequate, cost effective coverage in the pharmacy after hours and when regular staff call in sick or take vacation. Overtime expenses are quite costly, add up quickly and are not the long term solution that many hospitals are seeking. RemoteSolutions provides assistance to your pharmacy operation after hours or in other times of need and eliminates the need to process overnight medication orders in the morning. Our highly trained pharmacy professionals adhere to your organizational and clinical policies, procedures and protocols. We provide an affordable service customized to fit the needs of each client’s pharmacy operation. Features & Options

Testimonials “ RemoteSolutions has enabled us to provide our 25 bed, critical access hospital, with pharmacy coverage 24 hours a day, seven days a week for the first time since opening. After switching from a different vendor to RemoteSolutions, not only did our after-hours pharmacy coverage increase, but our monthly cost for the service decreased by an average of 40%. We could not be more satisfied with coverage provided by the pharmacists, the daily handoffs and the monthly feedback and reports sent by RemoteSolutions.”

— 2 5 bed hospital in Minnesota

“ Having been through the process of after-hours telepharmacy service implementation two times previously, I know that getting this “right” can be maddening. Since every facility is different, the after-hours service must be extremely flexible and responsive to ensure implementation success. I can say without hesitation that RemoteSolutions has been extremely responsive to our needs, flexible with regard to processes and protocols, and has made this process as “painless” as possible. RemoteSolutions pharmacists are very “dialed-in” to doing what is best for our patients AND nursing staff. Complaints from nursing are virtually non-existent. The pharmacists are also very responsive to suggestions and direction from myself and my staff. We have been a client for 14 months at present and are very satisfied.”

— 7 5-100 bed hospital in Kentucky

Congratulates Phil Bunick on His Retirement

After 40 years of work in hospital pharmacy, and 36 years as a pharmacy director, Philip Bunick is retiring from his current job as Pharmacy Director at the UConn Health Center in Farmington this August. Bunick graduated from UConn in 1972, and is amazed at how much pharmacy has changed since then. He loves working with students because “they keep you in touch with what is currently happening in pharmacy.” Bunick believes that working with students is a give and take, and he benefits from the experience as much as they do. He says, “They have a lot of insight and refreshing ideas. It’s nice to get a new perspective.” Thanks to Bunick, the UConn Health Center now precepts a P1 and P4 student on rotation. Bunick is no stranger to working with students. In 20 plus years, working with up to ten students a year, he is constantly running in to past students, several of which have ended up working for him. “Those who make a good impression on their rotations have an advantage finding a job in this difficult job market,” he explains. Beginning in July, the pharmacy will begin a residency program in collaboration with the School of Pharmacy. He also assisted in the enhancement of clinical programs and the reestablishment of the anticoagulation clinic. His experiences have also made him an asset in selecting who will become accepted into the School of Pharmacy. For the past few years Phil has been involved with the interview process. He says he is extremely grateful that the School of Pharmacy asked him to be a part of this process since he believes that “the sky is the limit for pharmacy today” and the students selected will play a tremendous role in shaping the future of the profession.

Company Background Medi-Dose/EPS was founded in 1971 when Milton Braverman, a former pharmaceutical company Territory Manager, saw the need for inexpensive, manual unit dose packaging allowing a hospital to convert from traditional dispensing. He developed the Medi-Dose System to package, handle and dispense predetermined amounts of medication so they would be accessible for one regular dose. Because of the continued success of the Medi-Dose System, Medi-Dose/EPS expanded its product line to include the TampAlerT System offering tamper-evident liquid packaging, without the need for heat tunnels and accessory sealing equipment, and a full line of supplies and disposable products designed specifically for the pharmacy and health care professional.

Product Overview Our flagship software, MILT 3.0 by Medi-Dose, was designed to manage the printing of labels and packaging of unit dose medications. Using our Lid-Label Covers, LiquiDose, MiniLiquiDose, Butterfly and Line Tracing Labels, you can easily and clearly identify and, if desired, bar code all your medications. With MILT 3.0, we’ve greatly expanded the formatting options so you can design your labels any way you want. Bar Codes, Graphics, Special Fonts, Tall Man Lettering, Shapes, Logos and Symbols were all added to call attention to important dispensing information, helping minimize medication error. Bar Coding has never been easier than it is in MILT 3.0. All common types of bar codes are supported — 1-Dimensional (1-D, linear) and 2-Dimensional (2-D) Multi-Part codes have been included (e.g. PDF417 and Data Matrix). You can construct your bar code your way. Incorporate whatever information

your BPOC or BCMA system requires — NDC, Lot Numbers, Beyond-Use Dating, Hospital Codes, Wholesaler Numbers. In addition, MILT 3.0 supports our laser labels and our new line of direct thermal printer labels. Now, you can print just what you need and design your labels any way you want.

Product Overview Every healthcare institution is sure to feel the effects of drug shortages in the coming months; in fact, the amount of drug shortages has more than tripled since 2005. Approximately 80% of these products were sterile injectable medications. The shortages lead to increased cost for pharmacies to meet patient and physician needs, and much of the financial impact is felt in acute care divisions.

Testimonial

“ Cantrell Drug Company has been a lifesaver when it comes to drug shortages. A quick e-mail to Cantrell, and more often than not, they are able to provide the needed product. A couple of weeks ago, our metoclopramide injection supply was critically low. Cantrell saved the day by providing what commercial vendors could not. I am confident that the products we receive from Cantrell Drug Company are sterile and accurate because of the rigorous quality control tests that are performed on every batch. It is comforting to know that Cantrell Drug Company will provide a quality product at a fair price.” –– Angela N. Powell, PharmD, Director of Pharmacy, Baxter Regional Medical Center

Company Background

Texwipe is the worldwide leader in contamination control supplies and critical cleaning products including lint free cleanroom wipers, sterile products, Sterile IPA, cleanroom swabs, cleanroom paper and notebooks, adhesive mats and more. It all started in 1964 when Edward Paley invented a product to solve a problem that no one could see. The problem was microcontamination - minute particles that were creating errors on the disk and tape drives of the burgeoning mainframe computer industry. In inventing the first low-lint cloth for cleaning computer equipment, Mr. Paley created a company and invented an industry. In July 2001, The Texwipe Company, LLC was acquired by Illinois Tool Works, Inc. (ITW). Today, Texwipe is a worldwide leader in contamination control products and process materials for aerospace, automotive, biotechnology, food processing, data storage, medical device, pharmaceutical, semiconductor, telecommunications and other industries requiring critical manufacturing environments. ITW Texwipe’s broad product line for contamination control includes a complete array of wipers, swabs, mops, cleaning solutions and stationery for use in cleanroom environments. Products include Vectra® ultraclean sealed-border wipers, CleanPak® pre-wetted wipers, CleanTips® swabs engineered for cleanliness and consistency, Transplex® ESD-safe swabs, and TexWrite® cleanroom stationery. ITW Texwipe also develops custom products to address specific customer requirements. Headquartered in Kernersville, North Carolina, the company supports global customers with sales offices throughout North America, Europe and Asia. Manufacturing centers located in North Carolina, the Philippines and China provide the highest levels of quality and manufacturing technology. Texwipe is committed to technology, innovation, and service excellence to provide the most consistent and highest quality products, including the recent launch of the revolutionary Vertex® Cleanroom Wipers, the world’s first fully-automated manufactured wipers.

Product Overview

Your wiper may be white but does that mean its clean? Did you know that most cleanroom wipers are made by human hands and that people are the greatest source of contamination in the cleanroom? Texwipe realized this problem and spent years in R&D to create a fullyautomated, human-free manufacturing system - Vertex®. Vertex is the only wiper made using a fully-automated process, each and every step. Vertex is untouched clean. No People. From start to finish, this HandsFree system isolates the wiper material from human contact. Texwipe’s proprietary technology allows the wipers to be cleaned, sealed, inspected, stacked and packaged without any human intervention. No people, no hands, no added risk of contamination. HCS Medication Reconciliation Allowing for integration with any existing hospital information system, HCS Medication Reconciliation reduces clinician workload and increases patient safety by streamlining the medication management process. CEO:

Dwight Henderson, R.Ph Founded: 1983 Phone number: (334) 279-9711 Fax: (334) 279-0711 Address: 5755 Carmichael Parkway Montgomery, AL 36117 Website: www.hcsinc.net Email: info@hcsinc.net

Company Background

Health Care Systems, Inc. was founded in 1983 with a mission to develop the best clinical software applications for hospitals and health care facilities. Our innovative software solutions enable health care professionals to deliver the highest quality of patient care, while reducing costs and improving access to information. By integrating into existing hospital information systems, our health care applications can join multiple medical data systems, resulting in reduced delay, improved accuracy, and enhanced information. These benefits are just some of the reasons that HCS healthcare information systems are being used today in more than 400 hospitals and medical centers nationwide.

Product Overview

HCS Medication Reconciliation is an automated medication history retrieval and software solution that meets and exceeds the Joint Commission’s standard “to accurately and completely reconcile medications across the continuum of care.” Upon admission, clinicians are provided, within seconds, a detailed report of a patient’s prior medication history, including drug strength, quantity, and original fill and last refill dates. The solution alerts clinicians to possible medication interactions, duplicate drug therapies, compliance issues, and formulary interchanges. Clinicians can immediately create a drug order and/or input additional medications. Upon discharge, a similar view allows reconciliation of inpatient and home medications, enabling medication reorder and/or addition of new medications. Each step of the reconciliation process is supported electronically, and/ or with specialized reports. For example, the Medication List Worksheet displays a box to allow the physician to start an order and/or allow entry of additional medications at Admission, Transfer and Discharge. Software to accomplish this step is also available, eliminating the need for paper reports. Once the Prior Medication History is completed on Admission, the information is printed and/or sent electronically to the Clinical Information System. If the patient transfers levels of care, medication orders are printed or sent to the Clinical Information System as well as displayed electronically in the clinician’s choice of format.

HCS Medication Reconciliation provides the clinician a Discharge Medication Worksheet and/or on-screen display that contains the patient’s inpatient medications and home medications with a column for the physician to check to continue, indicate dispense amount, select refill times, add comments and e-sign. The physician can add new medications if applicable. In addition, Medication Education Monographs (in English and other languages) and Electronic Prescriptions can be generated. This facilitates patient counseling regarding any changes in discharge medication therapies compared to pre-admit therapies, and increases compliance. The physician may submit the Discharge Prescriptions by fax, paper or electronically to the patient’s pharmacy. Additionally, discharge instructions and the patient medication list is supplied to the patient and transmitted to the next level of care. HCS Medication Reconciliation has been proven to save time and increase accuracy during medication reconciliation: • 2.4 more critical medications identified during admission • 50% or more increase in computerized physician order entry • 51 minute reduction in time to reconcile • 23% increase in ordered critical medications

SEVA Direct Basic by IntraVena

SEVA Direct Basic offers an inexpensive, safety hazardous drug transfer convenience kit designed to improve hazardous drug safety and minimize IV line connections and disconnections for your patients. CEO:

Gale Thorne Jr. Founded: 2007 Employees: <10 Phone: (801) 510-8733 Fax: (801) 295-8471 Address: 1898 Chokecherry Drive Bountiful, UT 84010 Website: www.intravenaonline.com

Company Background

IntraVena, LLC (“IntraVena”) was founded in 2007 by Gale H. Thorne Jr. and Gale H. Thorne Sr. IntraVena focused primarily in the area of hazardous drug safety. IntraVena began selling its convenience kits into the hazardous drug safety market in 2008. In Q4 2010, IntraVena launched Vented SEVA, SEVA Direct Basic and SEVA Direct with Texium. SEVA is an acronym for Safety Enclosed Vial Adapter. SEVA is designed to minimize or eliminate accidental exposures to hazardous drugs. SEVA’s unique and simple approach helps prevent accidental exposure from a number of sources that are not addressed by any other known product on the market. These include: 1.) Residual hazardous drug found on vial exteriors, 2.) Vapors released during use and disposal and 3.) Vial breakage. SEVA is part of IntraVena’s overall approach to hazardous drug handling, called “Arrival to Disposal Safety”. In Q3 2011, IntraVena introduced an exciting new integral tethered alcohol swab feature to its SEVA Convenience Kits that provides easier, more efficient product use.

Product Overview

Approximately 85% of vials containing hazardous drugs are contaminated with hazardous drug residue on the outside of the vial, and the mere act of spiking a vial can release aerosols outside the vial. To address these and other common hazardous drug transfer issues, IntraVena developed SEVA convenience kits. SEVA Direct Basic, a part of the SEVA family of products, is designed to transfer hazardous drug from a vial directly to an IV bag without detaching a syringe from the vial access pathway. IntraVena’s SEVA Direct convenience kit offers a low cost, closed system hazardous drug transfer kit, which delivers measurably doses of hazardous drug directly from a vial to an IV bag. SEVA Direct is even more cost effective when used for multi-dose vial applications. Fluids are delivered from the vial to a measuring syringe, then to the IV bag through an integral stopcock. SEVA Direct Basic is a low cost solution for closed system hazardous drug transfer. It allows clinicians to use a needleless male luer connector of their choice. Each SEVA Direct Basic convenience kit provides an

enclosed vial adapter with tethered alcohol swab inside a medical grade, 4 mil, transparent, polyethylene bag. SEVA Direct Basic creates a barrier against hazardous drug on the exterior of the vial which may spread outside a laminar flow hood via touch contamination. The barrier also helps contain fluid and aerosol from release into a work environment. The tethered alcohol swab affixed to the vial adapter spike cover, within the SEVA Direct Basic bag, permits easy access to an alcohol swab from the bag exterior without compromising sterility of bag contents. An easy tug of the tether exposes the swab, thereby reducing steps in vial preparation and handling. SEVA Direct Basic uses the bag as an air reservoir, permitting high volume gas transfer without a need to exchange air from the exterior environment. With the aid of a vent on the vial spike, diluent may be added to lyophilized powder for easy drug reconstitution. The SEVA Direct Basic bag is clear, permitting clinicians to easily see vial contents and labeling. Importantly, the vented vial adapter only vents inside the bag, not into the exterior environment.

Additional Product Lines

• SEVA Direct with Texium® includes a unitized Texium® Male Luer Connector for convenience and ease of use. • Mini SEVA is a vial adapter for accessing hazardous drug vials requiring needle access. • Vented SEVA Convenience Kit is a hazardous drug transfer solution which operates like a conventional spike. The vial is contained inside a SEVA bag.
