Antimicrobial/Antibiotic Drug Case Study Study of 180 U.S. Hospitals on Antibiotic Over-Prescribing Impact Reveals Steps U.S. Hospitals Can Take to Combat Antibiotic Overuse and Reduce Use by 11.4%

THE CHALLENGE: Though antibiotics have transformed the practice of medicine, the Centers for Disease Control and Prevention (CDC) warn that 20-50% of all antibiotics prescribed in U.S. acute care hospitals are either unnecessary or inappropriate. The misuse of antibiotics has contributed to the growing problem of antibiotic resistance, which has become one of the most serious threats to public health.1 More than two million people are infected with antibiotic-resistant organisms each year, resulting in approximately 23,000 deaths annually. 2 According to the World Health Organization (WHO), antimicrobial resistance threatens the effective prevention and treatment of an ever-increasing range of infections caused by bacteria, parasites, viruses and fungi.3 In response, the Joint Commission published a new Medication Management (MM) standard for hospitals, critical access hospitals and nursing care centers, which became effective January 1, 2017, emphasizing the need to reduce use of inappropriate antimicrobials in all health care settings, and requiring hospitals to have an antimicrobial stewardship program that is based on current scientific literature.4 The Centers for Medicare and Medicaid Services (CMS) proposed new rule (June 2016) requires hospitals to reduce antibiotic use in order to participate in Medicare or Medicaid and states that hospitals must appoint designated, qualified leaders for those antimicrobial stewardship programs.5,6

Antimicrobial/Antibiotic Drug Case Study Study of 180 U.S. Hospitals on Antibiotic Over-Prescribing Impact Reveals Steps U.S. Hospitals Can Take to Combat Antibiotic Overuse and Reduce Use by 11.4%

THE CHALLENGE: Though antibiotics have transformed the practice of medicine, the Centers for Disease Control and Prevention (CDC) warn that 20-50% of all antibiotics prescribed in U.S. acute care hospitals are either unnecessary or inappropriate. The misuse of antibiotics has contributed to the growing problem of antibiotic resistance, which has become one of the most serious threats to public health.1 More than two million people are infected with antibiotic-resistant organisms each year, resulting in approximately 23,000 deaths annually. 2 According to the World Health Organization (WHO), antimicrobial resistance threatens the effective prevention and treatment of an ever-increasing range of infections caused by bacteria, parasites, viruses and fungi.3 In response, the Joint Commission published a new Medication Management (MM) standard for hospitals, critical access hospitals and nursing care centers, which became effective January 1, 2017, emphasizing the need to reduce use of inappropriate antimicrobials in all health care settings, and requiring hospitals to have an antimicrobial stewardship program that is based on current scientific literature.4 The Centers for Medicare and Medicaid Services (CMS) proposed new rule (June 2016) requires hospitals to reduce antibiotic use in order to participate in Medicare or Medicaid and states that hospitals must appoint designated, qualified leaders for those antimicrobial stewardship programs.5,6 To support hospital and health systems in meeting the Joint Commission standard and the CMS proposed rule, Comprehensive Pharmacy Services (CPS) launched the first of its kind Antimicrobial Stewardship Solution, based on an expansive study of 180 U.S. hospitals through its Antimicrobial Stewardship Pilot Program.

THE ANTIMICROBIAL STEWARDSHIP PILOT PROGRAM The CPS Antimicrobial Stewardship Pilot Program studied 180 hospital facilities across the U.S. from January 1, 2014 through December 31, 2014. The pilot included 45 facilities with antibiotic drug spend greater than $10 per patient per day and 135 facilities with drug spend less than $10 per patient per day. The program targeted achieving a 10% reduction in antibiotic use. Combined, these health facilities spent $23,736,031.26 on antibiotics in 2014, and by the end of the study, antibiotic expenditures dropped in every category.

RESULTS: To achieve 10% reduction in antibiotic use, the 45 facilities with antibiotics greater than $10 per metric had to spend $2,373,603 less on antibiotics during the evaluation period, compared to the baseline. By applying CPS’ Antimicrobial Stewardship guidelines for a one year period, participating facilities realized significant reductions in antibiotic consumption both clinically and financially, even with 3 to 4 % drug inflation occurring. Savings were $2,709,204.34, a reduction of 11.4%. As seen below, all facilities combined saved $3,386,111.29 from the baseline period.

RECOMMENDATIONS FOR AN ANTIMICROBIAL STEWARDSHIP PROGRAM The best performing sites in the pilot had the following minimum elements in place that constitute a successful antimicrobial stewardship program. These four elements are based on proprietary components in the CPS Antimicrobial Stewardship program. 1) A multidisciplinary team that includes pharmacy, and an identified antimicrobial stewardship leader 2) An antimicrobial stewardship policy and associated procedures that include the requirement that metrics be reported for recording and evaluation 3) A stewardship education and training initiative for clinicians and other relevant staff 4) Antibiogram in place Following the pilot, CPS developed its Antimicrobial Bundle that is designed to address all of the antibiotic stewardship requirements and regulations from CMS, CDC and the Joint Commission. The Antimicrobial Bundle’s education initiative offers a Pharmacist Antimicrobial Stewardship Certificate (PhAST) that includes more than twenty hours of comprehensive and intensive antibiotic stewardship education sessions.

Congratulations IPPE I Preceptor of the Year, Ryan Ades! The Pharmacy Program at Manchester University College of Pharmacy, Natural and Health Sciences awarded our first Preceptor of the Year awards in spring 2016. We would like to congratulate Ryan Ades of Meijer Pharmacy as our inaugural IPPE I Preceptor of the Year recipient. Excerpts from Ryan’s student nomination include: “I had a phenomenal preceptor who not only simplified pharmacy practice for a beginner like myself, but also increased my passion for the profession. Ryan went well beyond his job by taking personal interest in both my practical education as well as my classroom education. He always made an effort to stay updated on what I was learning in school each week, greatly solidifying my knowledge by often quizzing me on topics the professors had covered. Most importantly, whenever he noticed me doing activities like counting pills or shelving, he would tell me to stop and would instead give me tasks such as figuring out drug interactions, calculating drug dosing or observing his counseling. In fact, before explaining his own reasoning and advice in detail, Ryan would first ask what medication I would recommend based on what the patient had said. What I loved and respected most about my preceptor is that rather than treating me as a first year student of minimal knowledge, Ryan treated me as he would any of his pharmacist colleagues by valuing my opinion as well as my potential as a future pharmacist.” Recipients receive a trophy and an 18-month membership to the Indiana Pharmacists Alliance. CONGRATULATIONS PRECEPTORS OF THE YEAR Masood Malik, PharmD, MBA (IPPE 1- Community Practice)

Dr. Masood Malik is currently a Pharmacy Manager and Clinical Pharmacist for Costco Pharmacy, where he has served in this capacity for the last 20 years. He is actively involved in Costco’s clinical and training programs where he plays a key role in immunizations, diabetic, and asthma managements programs. Prior to joining Costco he owned and operated an independent pharmacy for nine years. Dr. Malik started his pharmacy career as a intern-pharmacist then later as a staff pharmacist at Little Company of Mary Hospital. Dr. Malik graduated from the School of Pharmacy at Idaho State University in 1981 with a Doctor of Pharmacy Degree. He also received an MBA in Executive Management from the University of Phoenix. He is passionate about better patient care, clinical training, and quality of education for future pharmacists.

Richard Carvotta, RPh, MBA (IPPE 2- Institutional Practice)

Richard Carvotta is currently the Director of Pharmacy Services at St. John’s Regional Medical Center and St. John’s Pleasant Valley Hospital. Dr. Carvotta graduated from the University of Arizona in 1972 and went on to complete the Wharton-LDI Fellowship in Philadelphia. In 1996 , Dr. Carvotta earned his MBA at Cal Lutheran University. Dr. Carvotta has been a practicing pharmacist for over 40 years and serves on a number of Pharmacy Advisory Boards to help shape the future of the pharmacy profession. “I have come to realize the importance of continued learning and how that translates to helping patients get well and keep them safe from harm. And more importantly, how those lessons learned need to be shared with all those who have chosen to become a part of this great profession.”

The Ben and Maytee Fisch College of Pharmacy is proud to recognize Mr. Stacy Cody, RPh, as the student-selected Independent Preceptor of the Year for 2017. Mr. Cody is the owner and manager of Cody Drug, a community independent pharmacy located in the East Texas town of Sulphur Springs. Mr. Cody spends a great deal of time mentoring each of his student pharmacy interns, and has demonstrated a genuine interest in their professional development. His primary motto is that every customer should be treated with the same kind of care that you would give your family. “When you look at individuals like you do your own grandmother,” he says, “ your perspective changes.” Additionally, Mr. Cody reinforces a need for compassion in pharmacy practice, and attributes that as a key factor in his success as a pharmacist. He said, “If you want to be a pharmacist, the most important thing you need to have is love. Love for your customers.” A rotation with Mr. Cody is not simply about “doing stuff” in a pharmacy. He treats all of his student interns like potential new hires, and he holds them to a higher standard. He emphasizes the need for strong knowledge and communication skills, and works with his students to build their professionalism and character. Dr. Bellanger precepts pharmacy students on clinical rotations at the Children’s Hospital of San Antonio for the Christus Health System and the Brazil exchange students for the University of the Incarnate Word and Prati, Donaduzzi Pharmaceutical Company in Brazil.

Dr. Renee Bellanger received her B.S. in pharmacy from the University of Louisiana at Monroe and a PharmD degree from the University of Texas at Austin and the University of Texas Health Science Center at San Antonio. She completed an ASHPaccredited residency in hospital pharmacy at Harris HospitalMethodist in Fort Worth, TX and a residency in pediatric pharmacy practice at the University of Oklahoma. She has maintained board certification in Nutrition Support Pharmacy since 1992. Dr. Bellanger is recognized by her students for teaching and helping the students progress as individuals in making clinical decisions for their patients. In working with her students in Brazil Dr. Bellanger holds weekly meetings on Skype allowing the student’s to contact her for feedback and advice on their assigned projects. She displays integrity, commitment, and insight into building the pharmacy program at the Feik School of Pharmacy. She has made tremendous improvements with her innovative thinking in the development of the Brazil Exchange program for the fourth year pharmacy students for their advanced internships. She continually upholds the pharmacy school vision for innovation and quality.
