Summa Health and Leiters Collaborate to reduce opioid use.

BACKGROUND For a patient, the statistics paint a grim picture regarding the effects of opioids. Every day, more than 115 people die from opioid overdoses.1 A one day use of an opioid prescription may pose a six percent risk of long-term opioid use, and dependency can start within three days of initial opioid use.2

Opioid addiction invades homes, schools, and communities, leaving devastated lives in its wake. Summa Health, a nonprofit integrated healthcare delivery system in Northeast Ohio, is more committed than ever to conquer the opioid crisis. To support this commitment, it has set a goal to eliminate opiates from its operating rooms completely.3 While Summa Health has already made significant headway on achieving this goal, additional help combating opioid use came from Thomas Mark, M.D. Chair, Department of Anesthesia, with the concept of a compounded multi-drug nerve block using three non-opioid drugs: bupivacaine HCI, dexamethasone sodium phosphate, and epinephrine.4 “Summa Health’s intent for this combination drug product is to reduce opioid use, improve patient outcomes and satisfaction, minimize complications, and shorten hospital stays,” said Dr. Mark. Initially, Dr. Mark collaborated with his pharmacy colleagues, John Feucht, Vice President of Pharmacy Services, and Glenn Huth R.Ph., Director of Pharmacy Operations, to compound this formulation in-house. However, as demand for the combination product increased, it brought new challenges for the hospital and pharmacy. Quality assurance, increased regulatory compliance, capital expenditures, drug shortages, and a national scarcity of pharmacy technicians make it difficult for hospitals, surgery centers, and clinics to compound higher volumes of complex combination drugs in-house. Furthermore, the increased expectations of regulatory agencies, accreditation organizations, and consumer groups lead many pharmacists to outsource their drug compounding to a high-quality 503B organization. Summa Health realized that collaboration was essential for success in reaching its goal to eliminate opiates from its operating rooms. The team at Summa Health understood that

Shields Health Solutions (SHS) recently partnered with Optum Advisory Services to evaluate the impact of the SHS integrated specialty pharmacy model on total costs of care for Medicare Advantage patients.

HYPOTHESIS SHS’s integrated specialty pharmacy (SP) model reduces total health care costs measured on a per member per month (PMPM) basis. METHODOLOGY Optum proprietary de-identified Normative Health Information claims database was used to identify patients enrolled in Medicare Advantage and pharmacy insurance coverage in 2018 and 2019. To be included in the data set, patients had to fill at least one self-administered specialty prescription in 2018. Patients were assigned to groups (as seen in the table on next page.) Total health care costs (including medical and pharmacy spending) were then compared. The authors used Centers for Medicare & Medicaid Services Hierarchical Condition Categories risk scores to adjust for differences in disease states, comorbidities, and demographic factors between patient groups.

Helmer Scientific Moses H. Cone Memorial Hospital’s Inpatient Pharmacy Prepares for Regulation Changes and Enhances Workflows The Moses H. Cone Memorial Hospital, located in Greensboro, North Carolina, underwent a full inpatient pharmacy renovation to accommodate the addition of the new Cone Health Women’s & Children’s Center at Moses Cone Hospital. The pharmacy renovations were aligned to meet or exceed the recently revised compounding standards, USP <795>, <797>, and <800>, set by the United States Pharmacopeial Convention (USP). Two cleanroom workflow challenges from the previous pharmacy were addressed in the renovation; placement of a stand-alone refrigerator in the anteroom which required technicians to leave the cleanroom to retrieve drugs for compounding, and visibility to the refrigerator which was blocked from both the cleanroom staff and the technicians in the main pharmacy.

Increasing Efficiency With an Interlocking Pass-Thru Refrigerator

An interlock pass-thru refrigerator from Helmer was chosen to enhance their cleanroom workflows. The pass-thru sits between the positive pressure cleanroom and the main, non-classified pharmacy space. The location of the interlock pass-thru allows access to both the cleanroom personnel and the main pharmacy staff, reducing risk of contamination and optimizing the way drugs get to patients from a fast-paced, high-volume cleanroom. We asked Kevin Hansen, PharmD, MS, BCPS, Assistant Director of Pharmacy at Moses H. Cone Memorial Hospital, what advice he has for other facilitates going through a cleanroom renovation. urgent demand for the safe storage of new treatments and vaccines, but we had to do so all while redesigning our business and manufacturing processes to ensure our team could do so safely.” Because of Helmer’s investment in R&D and focus on designing medical-grade units that meet current and emerging guidelines, Helmer was ready to support customers with cold storage solutions that helped providers test, treat, and vaccinate patients during the COVID-19 pandemic.

Emerging Standards for Vaccine Storage A study completed by the Department of Health and Human Services in 2010 confirmed that vaccine stored by 76% of providers was stored at inappropriate and unsafe temperatures. As a result, new CDC recommendations for vaccine storage emerged, and hospitals and health departments quickly ramped up orders to safely store and administer vaccines. In addition to current CDC guidelines, a new vaccine storage standard is emerging to further define performance characteristics of cold storage units to safeguard vaccines. The NSF Joint Committee on Vaccine Storage has drafted new testing protocols and other recommendations to ensure vaccine storage safety. Manufacturers, including Helmer Scientific, will be able to submit their units for third-party testing and certification to this NSF standard once it is approved, which is expected in 2021. One of Helmer’s key differentiators is to focus solely on developing products for clinical and healthcare applications. Helmer Scientific cold storage products are designed and tested to meet existing guidelines, as well as emerging standards, such as NSF. “We are committed to serving healthcare providers that rely on our solutions to safely store critical medications and vaccines,” said King. “As part of that commitment we have prepared for emerging NSF vaccine standards through industry partnership and participation in the NSF Joint Committee.”

What is Helmer Scientific preparing for in the future? Looking Ahead As the COVID-19 vaccine rollout continues and the U.S. approaches the end of this pandemic, Helmer is preparing for what’s next. “The impact of COVID-19 will continue for years to come. I think a new awareness of the importance of vaccines has been brought to light,” said King. Supporting medication and vaccine compliance and safety, as well as ongoing investments to ensure quality and reliability, will continue to drive Helmer Scientific’s ongoing investment in R&D and new technologies. Beyond investments in the business, Helmer remains committed to their consultative approach with customers. “Customer loyalty remains our top priority,” said King. “We are here to protect what matters most to the world’s caregivers, so they can do what they do best — care for their patients and save lives.”

Product Overview Our cost-effective, disposable products are available for various needs in the compounding pharmacy. Aseptic technique validation kits are available for low, medium, and high-risk settings. CAS E S TU DY SUMMA HEALTH AND LEITERS COLLABORATE TO REDUCE OPIOID USE BACKGROUND For a patient, the statistics paint a grim picture regarding the effects of opioids. Every day, more than 115 people die from opioid overdoses.1 A one day use of an opioid prescription may pose a six percent risk of long-term opioid use, and dependency can start within three days of initial opioid use.2 Opioid addiction invades homes, schools, and communities, leaving devastated lives in its wake. Summa Health, a nonprofit integrated healthcare delivery system in Northeast Ohio, is more committed than ever to conquer the opioid crisis. To support this commitment, it has set a goal to eliminate opiates from its operating rooms completely.3 While Summa Health has already made significant headway on achieving this goal, additional help combating opioid use came from Thomas Mark, M.D. Chair, Department of Anesthesia, with the concept of a compounded multi-drug nerve block using three non-opioid drugs: bupivacaine HCI, dexamethasone sodium phosphate, and epinephrine.4 “Summa Health’s intent for this combination drug product is to reduce opioid use, improve patient outcomes and satisfaction, minimize complications, and shorten hospital stays,” said Dr. Mark. Initially, Dr. Mark collaborated with his pharmacy colleagues, John Feucht, Vice President of Pharmacy Services, and Glenn Huth R.Ph., Director of Pharmacy Operations, to compound this formulation in-house. However, as demand for the combination product increased, it brought new challenges for the hospital and pharmacy. Quality assurance, increased regulatory compliance, capital expenditures, drug shortages, and a national scarcity of pharmacy technicians make it difficult for hospitals, surgery centers, and clinics to compound higher volumes of complex combination drugs in-house. Furthermore, the increased expectations of regulatory agencies, accreditation organizations, and consumer groups lead many pharmacists to outsource their drug compounding to a high-quality 503B organization. Summa Health realized that collaboration was essential for success in reaching its goal to eliminate opiates from its operating rooms. The team at Summa Health understood that

This case study details the extensive efforts that Mark, Feucht, and Huth applied to find an FDAregistered 503B outsourcing partner that could efficiently and effectively manufacture and deliver the opioid-free nerve block agent to their health system.

DECISION TO OUTSOURCE The Summa Health team defined the necessary criteria and conducted comprehensive due diligence that ultimately led them to an innovative, reliable partner. However, the path presented problems. Feedback from prospective 503B partners ranged from a lack of capabilities and confidence to formulate the combination drug product with the requisite quality and testing to vendors not having the operational and production resources to meet demand. After expanding the outreach, the team’s research led them to Leiters — an FDA-registered 503B outsourcing provider of high-quality ready-to-use (RTU) compounded sterile preparations (CSPs). Leiters state-of-the-art facility located in Denver, Colorado follows cGMP and employs a multidisciplinary team with deep experience in cGMP, sterile pharmaceutical product development, manufacturing, compounding, and repackaging that ensures all products are released under the highest quality and regulatory compliance standards. “We required a compounding pharmaceutical company that received high grades in quality, compliance, consistency, service, and standardization. RESULTS Summa Health and Leiters have collaborated for over two years on this compounded nerve block agent. Leiters has consistently provided Summa with a high-quality compounded formulation in a RTU pre-filled syringe that has allowed Summa to provide their patients with an opioid-free drug. By outsourcing the compounding of this drug to Leiters, Summa Health has realized many positive improvement in their pharmacy operations. They entail: • Improved shelf life. Q. What does environmental monitoring look like in your pharmacy settings? It looks different depending upon the setting but is important and required in each. In all settings we continuously monitor the temperature of the ambient spaces, refrigerators, and freezers. We also monitor humidity of our storage and medication preparation areas.

Q. How does environmental monitoring factor into your packaging and shipping processes? We have worked with a third-party vendor to validate our shipping models to ensure that all medications sent to our patients stay within the acceptable temperature range for the duration of the shipment. This includes both refrigerated and room-temperature products.

Q. What considerations are made when packaging a medication shipment? In North Dakota, we have extremely cold winters and our summers can also see extreme heat. Depending on the time of year, our packing methods may be quite different to keep our products at ideal temperatures. We worked to validate our refrigerated and room temperature medication shipments in a variety of conditions to determine a reliable standard packing method for each season.

Q. What technology do you use to support environmental monitoring in your facility? All of our medication storage areas are under constant humidity and temperature monitoring. This data is available on a health system dashboard and alerts are pushed through to our facilities and leaders when readings are trending out of range. This allows our teams to activate a corrective action to keep our medications in proper storage conditions.

Q. How are you contacted if there is a problem with environmental readings? Our refrigeration monitors relay a text message to the leaders anytime a refrigerator or freezer is out of range. This includes the readings to allow us to make an informed, immediate decision any time of day or night. If room temperature or humidity is out of the range, our facilities team will either contact the area directly or if after hours, the leader will be called at home to initiate a corrective action.

Q. What technology and materials do you use in shipping? To initially configure our pack-out methods we include electronic data trackers in the test shipments which record and transmit temperature for the duration of the shipment. This allows us to adjust our pack out materials until we achieve the desired temperature


Q. How do you determine the safe limits for temperature and humidity? We look to USP 659 Packaging and Storage Requirements, USP 795 for nonsterile compounding, as well as specific product manufacturer guidance.

Q. Why is environmental monitoring important in your work? We need to be sure that the medications we give our patients are in perfect condition. Our patients trust that we are giving them medication that has been handled properly and is going to be effective. Tight environmental monitoring allows us the confidence to know we are meeting this expectation.

Q. Who in your health system is involved with environmental monitoring? Our facilities team is in charge of the maintenance and monitoring of environmental controls. Leaders of all

Analysis shows that integrated specialty pharmacy model is associated with lower total cost of care1

Shields Health Solutions (SHS) recently partnered with Optum Advisory Services to evaluate the impact of the SHS integrated specialty pharmacy model on total costs of care for Medicare Advantage patients.

HYPOTHESIS SHS’s integrated specialty pharmacy (SP) model reduces total health care costs measured on a per member per month (PMPM) basis. METHODOLOGY Optum proprietary de-identified Normative Health Information claims database was used to identify patients enrolled in Medicare Advantage and pharmacy insurance coverage in 2018 and 2019. To be included in the data set, patients had to fill at least one self-administered specialty prescription in 2018. Patients were assigned to groups (as seen in the table on next page.) Total health care costs (including medical and pharmacy spending) were then compared. The authors used Centers for Medicare & Medicaid Services Hierarchical Condition Categories risk scores to adjust for differences in disease states, comorbidities, and demographic factors between patient groups.

OVERVIEW Greenville Memorial Medical Center (GMMC) is the 814-bed flagship facility of Greenville Health System (GHS), which is one of 12 hospitals that is now part of Prisma Health. Serving a variety of patient populations throughout the upstate and midlands regions of South Carolina, GMMC is committed to delivering the best possible patient care while promoting a safe, healthy, and efficient work environment for staff. This commitment holds true with regards to controlled substance waste disposal. In 2018, as the opioid epidemic continued to grow, GMMC, always cognizant of transforming health care for the benefit of the communities it serves, re-evaluated its processes to mitigate drug diversion, to ensure its controlled substance waste disposal methods were safe, compliant, and easy to implement. According to Jay Terry, Manager of Pharmacy Operations at GMMC, “We were familiar with Stericycle’s work because the company already handled and disposed of the hospitals’ other pharmaceutical waste. Since Stericycle’s waste management programs complement each other, organization leaders felt it made good sense to shift controlled waste disposal to Stericycle.” With the help of Stericycle’s CsRx® Controlled Substance Waste Service, GMMC standardized its approach to controlled substance waste management, lessening the staff’s burden involved in the effort and implementing a reliable and proactive program that cost-effectively mitigates the risk of diversion, while preserving the environment.

S O LV I N G C H A L L E N G E S Before turning to Stericycle, GMMC used another waste management company for controlled waste disposal and found several challenges with the equipment. First, the hospital’s pharmacy staff were tasked with servicing the waste containers, which entailed periodically emptying them, replacing batteries, setting up new receptacles, and so on. “Not only was this onerous, it also pulled staff away from their patient-focused duties, which was frustrating for both patients and staff,” said Terry. In addition, because the receptacles were relatively small, they often overflowed, resulting in an increased diversion risk as well as a safety hazard. By switching to CsRx, GMMC was able to remove the burden of servicing the containers from staff. Trained Stericycle technicians pick up the containers at predetermined times, and staff members do not need to be involved in the process. At the time of pickup, technicians add a solidifying agent to the waste to make it solid prior to shipping, preserving safety and further preventing diversion. They then replace used containers with new ones that are ready for immediate use. Once the technician removes the waste from the facility, it is completely disposed of through incineration, which is recommended by the Environment Protection Administration (EPA) and the Drug Enforcement Agency (DEA). Along with its superior service and reliability, GMMC was also attracted to Stericycle’s simple-to-use containers that automatically deactivate controlled substances on contact without requiring batteries or other tools. The specially designed receptacles render waste unusable, curtailing the chances of diversion. Because Stericycle technicians proactively pickup and dispose of the receptacles, they never overflow, maintaining a safe working environment.

CAS E S TU DY R E S U LT S By switching to Stericycle’s CsRx service, GMMC now has a streamlined and reliable process for disposing of controlled substance waste. “Not only has the program been cost-effective and easy to implement, but the day-to-day aspects do not pull staff away from patients because Stericycle provides an end-to-end service,” said Pearson. The program has reduced the risk of drug diversion through its proactive disposal processes and its use of waste containers that are engineered to mitigate theft. The service is environmentally friendly as well, using incineration to limit the possibility of controlled substances leaching into the surrounding environment. $80 MILLION IMPACT TO BOTTOM LINE The University of California at San Francisco (UCSF) reviewed all aspects of pharmacy operations to uncover long-term value drivers. This hospital, ranked among the best hospitals by U.S. News, didn’t hesitate to transform when needed. Transformation required change, and change meant confronting challenges to provide the best outcomes for patients, specifically involving their pharmaceutical needs. Leaders at UCSF believed that evolving the practice of pharmacy through management, optimization, business principles, and a radical redesign would result in a best-in-class pharmacy, giving them an unparalleled competitive advantage among health systems. Achieving the gold standard in pharmacy involved generating new revenue streams, eliminating inefficiencies, and contributing to excellence in patient experience. Moving forward, leaders specifically referred to the word radical to remind them to implement change in an unconventional and sweeping manner to elevate its pharmacy enterprise.

CHALLENGE UCSF wasn’t immune to the cost and efficiency challenges found in many pharmacy operations. A one-size-fits-all solution would not work at an institution with more than 20 locations throughout San Francisco and beyond. UCSF is an industry of its own, contributing to the intellectual rigor and economic vitality of San Francisco. The extensive offerings of UCSF’s therapeutics — medications, medical devices, and diagnostic tests reside across multiple specialty areas. Organizationally, the pharmacy structure needed to support and compliment the overall care delivery and complex patient populations served by UCSF. UCSF leaders knew they had to question old processes and assumptions and exhaustively examine pharmacy positions and operations. At the time, job vacancies and standardizations needed focus to keep pace with UCSF’s new acquisitions and innovations. Drug pricing strategies and overall cost management needed assessment. And ambulatory staff required increased pharmacy support. The pharmacy enterprise had devolved into an undervalued and underutilized asset. This status was not the norm for the over-achieving UCSF global enterprise.

DECISION & APPROACH UCSF’s response was to lead and provide solutions. As a result, they initiated an intensive planning effort to transform pharmacy operations to add value to care delivery and to improve financial contribution to the organization. The pharmacy value improvement initiative involved assessing organizational structure, operations, financial stability, compliance, and safety across all points in the organization. The first move involved UCSF establishing a multidisciplinary steering committee to provide project oversight. The team members represented hospital senior leadership, finance, nursing, informatics, legal, compliance, and value improvement.

this initiative. This consulting company in addition to a national profile and deep pharmacy expertise had a history of several successful pharmacy projects with UCSF. The committee engaged Visante because they had demonstrated their capabilities time and again by moving to quickly identify and solve issues. Trust and results were the deciding factors in turning to Visante to help transform UCSF healthcare through pharmacy. “Visante is known to accelerate the business of pharmacy and provide a sustainable proposition. This is a partnership like no other, and is an integral part of UCSF in many ways,” said Sheila Antrum, Chief Operating Officer; Senior Vice President, UCSF Health. Visante delivered 12 statements of work that detailed the rationale and approach to achieve UCSF’s strategic imperatives on pharmacy. Before developing these improvement initiatives, Visante conducted a comprehensive program assessment and gap analysis. Following the assessment, Visante defined a budget and created scorecards to track outcomes and financial performance, which the steering committee monitored monthly. The decision on the statements of work reinforced the following four themes with clear deliverables:

FORTIFYING THE ORGANIZATIONAL STRUCTURE Visante immediately put interim leadership in place to move on urgent projects quickly, while assessing staffing requirements and developing processes. The team at Visante worked alongside UCSF executives to help recruit, coach, and develop staff in pharmacy leadership and operations. “People are the biggest assets and we will invest in our team,” said Sheila Antrum, Chief Operating Officer; Senior Vice President, UCSF Health. Having a detailed understanding of the culture at UCSF and the specific skills necessary for success, Visante tapped into its vast network of hospital and health system pharmacy contacts to hire a new UCSF pharmacy leadership team. The investment in personnel covered every level of the new organizational chart. The new structure emphasized culture, patient care, efficient performance, and financial accountability.

CONCENTRATING ON COMPLIANCE Visante experts assessed, addressed, and prioritized medication-related compliance activities. From clinic licensure to controlled substance diversion assessment, the Visante approach complemented UCSF’s well-developed system compliance team to tighten compliance around medication use even tighter. Visante also provided program management and analyst support for the UCSF 340B program which delivered insight on evolving regulations and guidance on implementing needed processes, ensuring transparency, compliance, and optimization of program financial performance. For infusion and sterile compounding, the Visante team prepared UCSF for compliance with USP Chapters 795, 797, 800, and 825. Visante assisted the staff with preparing for inspections by the CMS, CA Board of Pharmacy, DEA, and Joint Commission. The Visante team also support the physical and operational upgrade of the UCSF Pharmacy Central Service Center (CPSC) to achieve licensure by the CA State Board of Pharmacy as a central hospital packaging operation allowing the center to operate as a true CPSC for the system.

APPLYING BUSINESS RIGOR TO PHARMACY OPERATIONS Pharmacy informatics played a crucial role for Visante to strengthen existing practices and expand growth areas that could produce higher returns on investments. Visante provided a team of Epic certified programmers to address multiple informatics projects that were behind schedule.

At the end of 12 months, I’m happy to report that all of the CPSC initiatives have been successfully completed. It is now a fully functioning central hospital pharmacy packaging operation licensed appropriately. Visante oversaw a new drug pricing scheme, reviewed drug utilization and purchasing strategies and revenue cycle strategies, and optimized dispensing cabinet inventory. These multiple initiatives resulted in a net margin enhancement of $38 million. With sterile compounding, business decisions included acquiring community-based oncology infusion practices to create a network of sites around the Bay area. To support these new operations Visante worked to establish a contract with a local 503A pharmacy operation that could centrally fill and transport the drugs to the community sites. "Working with Visante was a true partnership. We experienced big growth in our specialty pharmacy. Uncovered inventory and supply chain opportunities. We overcame hurdles in 340B to where the performance is now extremely strong. Throughout this initiative, the focus was always on our patients and our staff,” said Desi Kotis, Chief Pharmacy Executive, UCSF Health; Associate Dean UCSF School of Pharmacy.

PUTTING PATIENTS FIRST One of the most patient-oriented needs that pharmacy can address is eliminating wait times for authorizations and refills. Visante dove deeper into timing concerns by interviewing each clinic to understand their unique needs. The team then created a plan to centralize functions by using pharmacy technicians and pharmacists working under a set protocol for refill management. "Physicians were spending too much time with the management of refilling prescriptions, detracting from seeing patients. It was a big dissatisfier and an inefficient process for patients. Our plan involved transitioning that process from physicians to pharmacy, emphasizing the philosophy that medical professionals should be practicing at the top of their license,” said James Jorgenson, CEO Visante, Inc. UCSF is steadfast in the philosophy that no patient should go without needed medications regardless of their financial condition. To ensure this promise, a new program with a dedicated fulltime medication assistance specialist was implemented and integrated with the UCSF Community Benefit Program.

RESULTS This multi-year plan sets a trajectory to drive transformation while managing costs and increasing revenue. The initiatives involved intentional disruption in rethinking where and how UCSF operated all its pharmacy areas. The bottom line, the collaboration between UCSF and Visante built a more vital pharmacy platform by redesigning the organizational structure and instilling strong quality and value initiatives to guide the new team. The enhanced interdisciplinary pharmacy support and interactions established financial performance exceeding targets with a 30:1 return on investment with Visante’s engagement and approach. Specifically, at the conclusion of the initial 12-month engagement, the improvements to the University Medical Center’s net operating margin from pharmacy operations exceeded $80 million and the organization improved service delivery, safety and compliance around medication use.

CONCLUSION The multidisciplinary steering committee accepted responsibility for the economic risk of innovation and improved operations. The pragmatic risk-taking approach demonstrated that transformation happens with a patient-centered mindset and a business within a business philosophy.
