THE CHALLENGE When it comes to neonates and pediatric patients, one size does not fit all. Dosing is typically based upon the patient’s weight and, to a lesser extent, age and/or body surface area. To manage these various dosage requirements, doses are custom tailored and subsequently prepared manually for each patient. This individual repackaging of oral liquid medications tends to be a time-intensive process. However, there are opportunities to take advantage of clinically accepted, weightbased dosage ranges. There are situations where a given medication may be safely administered based on a weight or age range. In this situation, batching of doses and repackaging in corresponding aliquots is not only financially beneficial (i.e. repurposing individuals to perform other tasks), it is preferred.

THE CHALLENGE Oklahoma State University Medical Center (OSUMC) is a 195-bed teaching hospital located in downtown Tulsa. CASE STUDY Implementation of Auto SyringeFilling Technology Improves Pediatric Pharmacy Performance Automated unit-dose packaging and labeling of oral liquid medications improves efficiencies and patient care in pediatric pharmacy.

One big driver behind pharmacy automation is return on investment in the form of time savings, as well as the ability to free up pharmacists and technicians to perform other tasks within the pharmacy. The ultimate priority, of course, is enhanced patient care and safety. When a new product can make a positive impact on these considerations, it warrants attention. Combined with the company’s companion Auto-Wrap® Syringe Labeling system, the Auto-Draw® provides the ideal oral liquid medication packaging and labeling solution.

THE CHALLENGE When it comes to neonates and pediatric patients, one size does not fit all. Dosing is typically based upon the patient’s weight and, to a lesser extent, age and/or body surface area. To manage these various dosage requirements, doses are custom tailored and subsequently prepared manually for each patient. This individual repackaging of oral liquid medications tends to be a time-intensive process. However, there are opportunities to take advantage of clinically accepted, weightbased dosage ranges. There are situations where a given medication may be safely administered based on a weight or age range. In this situation, batching of doses and repackaging in corresponding aliquots is not only financially beneficial (i.e. repurposing individuals to perform other tasks), it is preferred. For Enrico Ligniti, Pharm.D., Director, Pediatric Pharmacy Service Line for Cohen Children’s Medical Center in Queens, New York, utilizing the Auto-Draw® and AutoWrap® Oral Syringe Filling Labeling System to batch-fill weight-range–based doses seemed like a potential opportunity for increasing efficiency and improving safety in his pediatric pharmacy.

CASE STUDY THE AUTO-WRAP® SYRINGE LABELING SYSTEM Medical Packaging offers a companion system designed to pair perfectly with the Auto-Draw®. The Auto-Wrap® Syringe Labeling System automates the syringelabeling process and is compatible with nearly any kind of oral or injectable syringe from ½ mL to 60 mL. It provides all the information that is required on a label and is extremely legible, while still allowing visualization of the amount drawn in the syringe.

THE DECISION Most oral liquid medications are manufactured in bulk-sized bottles that must be divided into single-unit doses. Cohen Children’s Medical Center is part of Northwell Health’s extensive network of 21+ hospitals and provides care for a high volume of patients. “We typically administer 900 to 1,000 oral doses a day,” Ligniti said, “and about 70% to 75% of those are either in oral-dose liquid form or solid oral-dosage form. The majority have to be repackaged for use.” Cohen Children’s places a significant emphasis on building their clinical informatics technology to reflect formulary decisions in two main areas – the provider order-entry and clinical decision support tools, which offer suggested weight-range dosing and dosage-range checks. “We created clinical decision support within our clinical informatics environment,” said Ligniti. “When a provider goes in to search for a medication, the system will make a recommendation based on patient-specific parameters such as age and weight. When a pharmacist verifies an order, they have the ability to accept a default dosage form or perfect to another one. Embedding age or weight-based dosage range methodology, when clinically appropriate, will allow item perfection to a select group of predetermined items, thus allowing the advantages that come with batching and pre-drawn dosage forms." Ligniti saw that the Auto-Draw® system could contribute to streamlining the process of adopting standardized weight-range–based doses by more efficiently and reliably preparing syringes ahead of time. Additionally, barcoded labels allow for scanning at the bedside, thus enhancing patient safety at the point of medication administration. Part of the Auto-Draw’s® success is its simplicity. “It’s very intuitive,” said Ligniti. “Quite frankly, it’s taken very little time to train the staff in order to utilize the technology.” The Auto-Draw® system features a user-friendly touch screen. Setup involves drawing an empty syringe to the desired graduation mark and then tapping the syringe to the screen; this accurately sets the draw to be repeated for the duration of that filling session. “Our goal is the provision of exceptional pharmaceutical care to all of our neonatal and pediatric patients,” Ligniti said. “Care can only be optimized when we harmonize evidence-based clinical practice standards, informatics, and operational capabilities. [We chose] the Auto-Draw® [because it] is one of those important pieces of equipment that helps us operationalize practice standards and marry them to information technology and patient safety practice expectations.”

THE RESULTS The Auto-Draw® technology optimizes care by providing efficiencies in several ways. “We no longer have to spend time [in the pharmacy] drawing up individual doses,” Ligniti said. “For select items, we are able to generate and turn around product much more quickly compared to what was previously a slow manual process.” A stock of pre-drawn, pre-labeled doses also enables Cohen Children’s to utilize automated dispensing cabinets with greater efficiency. “We can reduce the number of patient-specific doses in patient medication cassettes, which were formerly filled with a day’s worth of medications.” Ligniti said. “Now, we fill the automated dispensing cabinet once a day with the required amounts of standard doses, thus alleviating some of the cart-fill burden.” In addition to enhanced patient safety, more accurate bill-upon-dispensing functionality is also possible via barcode through the companion Auto-Wrap® Syringe Labeling System. The filled syringes are automatically labeled with barcodes, so the nurse can scan the product at the bedside for product confirmation, which accordingly populates the eMAR and subsequent charge capture.

THE CONCLUSION For select items, repurposing of staff members, secondary to implementation of this technology, has resulted in a 25% decrease in preparation time. “In essence, we are able to repurpose some technician time to other departmental needs,” Ligniti said, “and that time is now redirected to other responsibilities in the department, which is fiscally beneficial.” Cohen Children’s has integrated the Auto-Draw® system, and Ligniti is working toward adopting this technology to benefit Northwell’s network of more than 21+ hospitals. “We are in the beginning stages of rolling out the Auto-Draw® system locally,” said Ligniti, “and we are also in the process of expanding it to our affiliated system hospitals, where we anticipate seeing similar gains in efficiency across the network.

Congratulations APPE Preceptor of the Year, Roger Nutt! The University of Arkansas for Medical Sciences College of Pharmacy is proud to name Roger Nutt “Preceptor of the Year” for the 2016-2017 academic year.

Roger has had a huge impact on the future of our students. Not only does he go above and beyond as a preceptor and teacher, he genuinely enjoys being a part of it all. Not to mention the students think so much of him as they voted for him to win our Preceptor of the Year award for 2017. Roger also volunteers his time for the experiential’s High Honors Award Program. Roger has served as a preceptor since the initiation of the program for fourth year students. He says he has always enjoyed this aspect of his job, taking pride in teaching the next generation of pharmacists. The way he sees it, he learns just as much from the students as they learn from him. He is able to bestow knowledge from years of experience in the management of community and hospital pharmacy and the students Roger Nutt keep him on his toes in all the new medications coming down the pipeline and the ever changing practices of pharmacy. Teaching the pharmacists of tomorrow is not a responsibility that Roger takes lightly and this is made obvious by his passion and dedication to precepting!

Manchester University would like to congratulate Jaymee Gaspar, 2017 APPE Preceptor of the Year! Excerpts from Jaymee’s student nomination include: “Jaymee wasn’t only a great preceptor, but also a great mentor. Her commitment to teaching/mentoring, empowered me to do more and build myself in a pharmacy setting that I wasn’t confident with. Jaymee built my confidence in ambulatory care by letting me sit in her seat on the first day to see patients. You rarely see any preceptor who could trust a student from the first day by letting them do their job, but Jaymee did. Jaymee strived to utilize every available opportunity she got to teach me, even if it was during a patient appointment. Her teaching technique is to let a student practice pharmacy while she is there to guide them. Jaymee always had questions for me about each patient. She cared about my therapy decisions and corrected me when I was wrong by asking me to look it up. After I come back with an answer, she will spend more time discussing it with me. Jaymee was available to help me whenever I had a question during my rotation hours or after. Because of these efforts, Jaymee has earned not only the preceptor of the year award, but also the respect and gratitude of those for whom and with whom she served, her students, her patients and her peers at the VA clinic.” Recipients receive a trophy and an 18-month membership to the Indiana Pharmacists Alliance. This preceptor is very special to Roosevelt College of Pharmacy, as he is a member of our 2014 Inaugural Class, and the first alumnus to receive a Preceptor of the Year Award from us. Students repeatedly refer to him as a true mentor. We are extremely appreciative of what he has given back to Roosevelt College of Pharmacy.

THE CHALLENGE Oklahoma State University Medical Center (OSUMC) is a 195-bed teaching hospital located in downtown Tulsa. When John Bury, Pharm.D., stepped into the OSUMC Director of Pharmacy role in 2014, the refrigeration equipment he inherited was functional, but clearly older. “The units had been there probably 25 years or more. They were solid, but then one gave out,” Bury recalled. “I knew we could manage being down one refrigerator, but if we were to lose another — well, you’re talking about a lot of high-dollar items subject to loss.”


CASE STUDY VACCINES FOR CHILDREN (VFC) In 2009, the Centers for Disease Control and Prevention (CDC) issued vaccine storage requirements to ensure that the low- or no-cost vaccines provided through the federally funded Vaccines for Children (VFC) program are properly managed and stored. In 2012, the CDC issued additional guidance regarding all vaccine handling and storage. The Oklahoma State University Medical Center is proud to be a VFC provider. Follett helps them easily comply with the requirements. “One of the reasons we chose the medicalgrade units is our involvement in VFC,” said pharmacy director John Bury. “They have certain requirements for storage including continuous temperature monitoring.” Follett refrigerators meet and exceed the CDC’s stipulations for participation in the program, including the use of: • Stand-alone, dedicated vaccine-only refrigerators and freezers • Temperature measurement with an accuracy of ± 1 °F (± 0.5 °C) • Option for a detachable ISO 17025 calibrated product temperature probe with certificate of calibration testing Constant internal monitoring records can be downloaded, and detailed data logs are automatically compiled for easy reference by date and time, making compliance with any regulatory standards a zero-effort endeavor.

THE DECISION Bury thoroughly researched the available options on the market — as well as the practical matter of upkeep. “Early on, when we were doing the analysis, I talked to our maintenance group. They would be the ones servicing the units,” he said, “and Follett came up very high on their list.” Other departments in the hospital had previously adopted Follett refrigerators. The OSUMC maintenance team, familiar with the brand, cited the ease of working on the units and their known longevity. A key feature that set Follett apart from a maintenance standpoint is the ability to remove the entire top-mounted refrigeration module for routine service or repair. This design allows a replacement module to be easily installed if needed, essentially eliminating unit downtime.

THE RESULTS Beyond the maintenance and reliability plusses, choosing Follett resulted in notable upgrades to features that benefit the pharmacy every day. Perhaps the most powerful advantage of the Follett units is their superior ability to maintain consistent and precise temperature control. “One of the things you notice looking at the older logs with staff tracking temperatures two times a day, is that you can see fluctuations,” Bury said. “With the Follett products, there is very little fluctuation.” Temperature recovery is another strong advantage, particularly for a busy pharmacy department. “On the old units, you would grab your items and the unit would start to alarm because of temperature drop, and it would only slowly recover,” Bury said. “If I’m in the fridge now, and I’m batching, I don't worry as much about temperature fluctuations as I know the unit is quick to recover.” This caliber of temperature consistency and recovery doesn’t happen by accident. Follett upright refrigerators and freezers feature the industry’s most advanced airflow design, utilizing unique plenums that deliver cold air directly to each storage level. Multiple pathways for cool air ensure consistent temperature throughout the unit, regardless of loaded shelves, drawers, and large or odd-sized packaging. Cooled air is distributed simultaneously to multiple levels and locations throughout the unit, as opposed to being pumped in via one primary entry point and reliance on dissipation. This comprehensive delivery of cool air to every point within the unit removes any concerns about temperature consistency, leaving more focus for other tasks.

"The temperature that [the Follett unit] says it is, is the temperature throughout the whole machine,” Bury said. “This offers peace of mind more than anything. When [a refrigeration unit] is working like it should, it just makes your life so much easier. When you have a system that doesn’t work, you have to spend a lot of time trying to mitigate fluctuations.” Other Follett features include modern technological functionality. Intuitive, full-color, 7-inch touchscreens allow users easy access to programmable tools and settings, such as temperature alarming. Some Follett units also offer WiFi connectivity, which allows for easy remote monitoring. The 24/7 visibility of temperature excursions gives pharmacy managers the ability to make any necessary corrections before sensitive inventory is affected.

THE CONCLUSION OSUMC ended up replacing all of their pharmacy-dedicated refrigerators, including three upright 25-cubic-foot units and 19 smaller units. “All of the units that store medications in our hospital are now medical-grade,” Bury said, “and, specifically, they are all Follett units.” New practice and quality standards for handling hazardous drugs in healthcare settings, known as USP 800, are slated for official implementation on December 1, 2019. These standards require dedicated storage for hazardous medications in order to promote patient safety, worker safety, and environmental protection. “We don’t do a ton of chemo, but enough to need a stock on hand,” Bury said. OSUMC now has a 5-cubic-foot undercounter Follett unit on site, already addressing the new requirement for secure, separate storage. The medical center is also participating in beta testing of Follett’s newest double-door refrigerators. The double-door units are larger, providing more flexibility for storing expanded inventory without sacrificing efficiency. The plenum airflow feature is just as reliable in the larger units. “Even though the double-door unit is bigger, it’s still excellent at maintaining consistent temperatures,” Bury said.

Trays can be produced in different colors, which allow it to be quickly identified for different hospital departments. n FlexModul The FlexModul product line is designed to give the user ideal storage for bulk goods. Our side panels can be mounted inside an existing casework and allows the trays to be hinged 10 or 30 degrees for viewing while still being stored horizontally. This allows for the storage to be optimized in the existing space. Trays are available in 2", 4", and 8" heights in either gray ABS plastic or clear polycarbonate plastic. FlexModul dividers can be added to the inside of trays to further divide the space. Labeling options are also available so the items are easily and quickly identified. n FlexShelf FlexShelf is an expandable open rack storage system that allows for products to be stored in a viewable manner. When accompanied with the FlexModul trays, the system can be adjusted as your storage needs change. With the introduction of this licensed version, in addition to research and water testing, Pyrosate is ideal for dialysis centers, compounding pharmacies, and pharmaceutical and medical device companies.

Performing the Test

The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.

Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage. With one click you can access descriptions, HCPCS, revenue codes, and billable units of measure to add drugs to both the formulary and chargemaster. n Determine Correct Multipliers: Since medication dosages are often different than billable units, hospitals need to ensure multipliers are correct for optimal reimbursement. The BET detects unsafe levels of microbial cell wall debris from live or dead Gram-negative bacteria that cause fever and symptoms of septic shock. Until now, BET required skilled analysts and manipulation of cumbersome reagents. The Endosafe® nexgen-PTS™ is a handheld spectrophotometer that utilizes FDA-licensed LAL cartridges to deliver accurate, real-time endotoxin results in 15 minutes at the point of sample collection. The USP/BET compliant cartridges require no microbiology training, eliminate the need to prepare endotoxin standards, and are compatible with suitable dilutions of all common compounded sterile products. This rapid, on-site solution eliminates bottlenecks, improves sample

management, and most importantly assures that products are free of endotoxin (pyrogen) within limits set by the Pharmacopeia. To ensure that our clients get the most out of the Endosafe® nexgen-PTS™, Charles River offers additional support through method development and training. n Microbial Identification for Environmental Monitoring Programs Confident microbial and particle measurement is critical for an environmental monitoring program to ultimately confirm the security of the compounding area. Accurately identifying an organism to the species, and many times to the strain level, facilitates tracking of the potential origin of the contamination and prevents delays in product release and completion of investigations. Charles River offers comprehensive contract microbial testing services from our FDA-registered, cGMP-compliant laboratories. We have supported QC testing for over 1,000 global facilities within the biopharmaceutical, medical device, and nutraceutical industries with our Accugenix® bacterial/fungal identification and strain typing services. Offering a 98% accurate identification rate and 99% on-time delivery, we have tested and identified more microorganisms than any other company or service laboratory in the industry. n Sterility Testing Sterility testing is used to demonstrate the presence or absence of extraneous viable contaminating microorganisms in samples. This testing should be applied to substances, preparations, or articles which, according to the Pharmacopoeia, are required to be sterile. Charles River provides sterility testing through our Celsis® ATPbioluminescence technology that is compliant with the requirements of the USP, EP, and current FDA regulations. Sterility testing is conducted by direct inoculation or membrane filtration methods and can be performed in an isolator or cleanroom environment. In conjunction with the sterility test, a bacteriostasis/fungistasis test is performed to assess whether the test article is inhibitory to the growth of microorganisms. n Microbial Limits Testing Microbial limits testing is performed on pharmaceutical products and medical devices in order to monitor the levels of microbial organisms present during processing and handling. The information provided by this test can be used to help determine the sterilization dose for the product or device. This assay is designed primarily to allow quantitative enumeration of bacteria and fungi that may grow under aerobic conditions. Our Celsis® rapid microbial method reduces the long incubation times necessary for a quantitative assay with a qualitative, ATP-based bioluminescence automated analysis, a proven gold standard for product screening and rapid release.

Product Overview Our cost effective disposable products are available for various needs in the compounding pharmacy. Aseptic technique validation kits are available for low, medium, and high risk settings. We also sell multiple ala carte sizes of vials, bags, tubes, and syringes of growth media for facilities looking to create a custom media fill test that mimics their day to practice. In regard to environmental monitoring/gloved fingertip sampling, our EnviroTest™ paddle is ideal for both. The rectangle shape and built in hinge allow for testing critical areas such as edges and corners of a hood and behind latch handles. Sterility testing products are available in both USP 71 approved methods of testing. For the full filtration method, the QT Micro™ and QT Junior™ systems work for both small and large volume solutions. For the direct inoculation methods, our TuffTest 2™ product is ideal. All Q.I. Medical growth media is challenged with a battery of USP specified organisms and lot specific Certificate of Analysis (CofA) are available for download.
