THE CHALLENGE Mechanical failures and uneven cooling of medical refrigerators — a major pain point for staff at Lehigh Valley Physicians Group (LVPG) — increased product waste, revenue losses, labor, and down-production while delaying patient contact. “The refrigeration units we were using were malfunctioning on us. We had many issues with the refrigeration and the way they delivered cold air inside the unit,” said Dave Bowers, Lead Biomed Technician at LVPG, who services all the equipment in the nurse-supported physicians’ offices within the network. With more than 140 practices and 47 specialties serving eastern Pennsylvania’s BethlehemAllentown-Easton metroplex, LVPG, a subsidiary of Lehigh Valley Health Network (LVHN), needed to find a reliable solution — and fast.

THE CHALLENGE Mechanical failures and uneven cooling of medical refrigerators — a major pain point for staff at Lehigh Valley Physicians Group (LVPG) — increased product waste, revenue losses, labor, and down-production while delaying patient contact. “The refrigeration units we were using were malfunctioning on us. We had many issues with the refrigeration and the way they delivered cold air inside the unit,” said Dave Bowers, Lead Biomed Technician at LVPG, who services all the equipment in the nurse-supported physicians’ offices within the network. With more than 140 practices and 47 specialties serving eastern Pennsylvania’s BethlehemAllentown-Easton metroplex, LVPG, a subsidiary of Lehigh Valley Health Network (LVHN), needed to find a reliable solution — and fast.

CASE STUDY THE DECISION A stable temperature is critical to maintaining product efficacy when storing high-dollar biologics and other medical products that require refrigeration. However, certain variables can threaten temperature uniformity and product stability — one of the most crucial being the delivery of cold air into a concentrated area. Uneven or inconsistent air distribution decreases the potential for temperature homogeneity throughout refrigeration unit’s internal casing. Such fluctuations in air flow increase the likelihood for out-of-range readings, which could jeorpardize product effectiveness —regardless of their shelf level. Temperature instability always demands immediate attention. A temperature excursion often pulls clinical staff away from patient care in order to follow specific protocols necessary to ensure product quality and efficacy. As a result, refrigerator failures prove as critical as inconsistent temperatures.

In the instance of an out-of-temperature reading, mechanical malfunction, or process failure, the integrity and potency of vaccines and other biologics could be compromised. “Either way, your vaccine could be weak. You might have to discard everything in your refrigerator,” explains Julie Kaszuba, RN, Clinical Procedure and Products Nurse. The cabinets in LVPG’s existing refrigerators employed top-mounted hanging evaporators inside the storage cabinet, which concentrated cold air at the top. Bowers and his biomedical team explored a variety of options capable of sustaining cold chain specification requirements. However, each option utilized a similar hanging evaporator air distribution system. Fearing similar mechanics might create all-too-familiar challenges with temperature instability, they continued their search for a solution. Bowers and Kaszuba were then introduced to Follett’s medical grade upright refrigerators. Kaszuba found Follett’s receptivity helpful in finalizing the team’s product selection. “They are very customer-oriented and very open to feedback: ‘What did you like about the product? “What could you improve in time?’ They listen to recommendations to make the product better.” So receptive are the manufacturers to feedback, in fact, that Follett will be implementing one of Bowers’ suggestions to offer larger units bearing the same controls as the manufacturer’s smaller, under-the-counter refrigerators to make the units more user-friendly for smaller practices.

THE RESULTS Bowers noticed an immediate difference in temperature stability after the Follett units were installed. The medications remained within temperature regardless of shelf level. Follett’s unique back-plenum air distribution circulated air more evenly to multiple levels within the cabinet, providing temperature uniformity that ensured product efficacy. An additional feature Bowers’ team found useful is Follett’s compatibility with third party external alarm systems that send alerts to the user’s email or phone. The simple installation process proved to be another major time and money-saver.

The benefit is two- fold: “Nurses don’t have to spend time worrying about having to check failures or checking to see whether the product is stored properly,” Bowers noted.

“The modular refrigeration unit can be unbolted, swapped, and unplugged within an hour by a mechanical person, but it doesn’t have to be a refrigeration specialist,” Bowers pointed out. He also noted that the unit installation does not require a refrigeration specialist, which makes the setup process more user-friendly.
THE CONCLUSION Having a product that supports best practice plays a significant role in providing quality care. For a seamless roll-out, Bowers aims to make sure LVPG incorporates renovations and upgrades planning for floor space to accommodate Follett’s slightly larger refrigerator. Although no exact timeline has been established, Bowers eventually plans to replace all older refrigerator models with Follett units throughout the LVPG system. “Any time we add a new practice or renovate a practice, if they don’t have it, we make sure it gets put in as part of the budget,” Bowers stressed. “Also, the external alarm system allows us to monitor the temperature closely and make recommendations to management to swap out systems sooner. If we see a pattern of out-or-range temperatures, we try to get the old units changed out to Follett.” “Generally speaking, any time that there is not waste, there is a win. Any time waste is avoided, it’s speaking to a level of quality and following a best practice,” Bowers concluded. Kaszuba echoed Bowers’ sentiments on quality, stating, “I think clinically, it’s about safety, so your medications are only as good as maintaining the efficacy of your products.”

n Picking Stations and Work Stations New for 2016 are the H+H System picking stations and work stations. Picking stations organize and simplify the restocking of drugs for your exchange trays. By having the storage of items (both small dosages and bulk) in close proximity to the work surface, increased efficiency is achieved. Different height trays with customizable divider configurations allow the picking station to fit your specific needs. Our work stations, with an IT Configuration or work top, are ideal compliments for your scanning stations.
n Exchange Trays H+H System exchange trays give hospital personnel the ability to standardize their medication storage in all medical carts and drawers. Standard heights include 1⅜", 2 5⁄16", 4", and 5". Standard size trays are available, but custom sizes can be produced for any quantities. Flexible dividers on the inside of the tray allow the end user to compartmentalize the tray based on their needs. If the need for more storage space arises for a particular item, just slide the divider and a larger storage compartment is now created. Trays can be produced in different colors, which allow it to be quickly identified for different hospital departments. n FlexModul The FlexModul product line is designed to give the user ideal storage for bulk goods. Our side panels can be mounted inside an existing casework and allows the trays to be hinged 10 or 30 degrees for viewing while still being stored horizontally. This allows for the storage to be optimized in the existing space. Trays are available in 2", 4", and 8" heights in either gray ABS plastic or clear polycarbonate plastic. FlexModul dividers can be added to the inside of trays to further divide the space. Labeling options are also available so the items are easily and quickly identified. n FlexShelf FlexShelf is an expandable open rack storage system that allows for products to be stored in a viewable manner. When accompanied with the FlexModul trays, the system can be adjusted as your storage needs change. Accessories such as fixed shelves, pull-out shelves, pull-out drawers, and solid worktops allow for numerous configurations and the perfect solution for any need. Additional storage columns can be added at any time, as needed. For a mobile storage option, castors can be used instead of the adjustable leveling feet. This can be used as a distribution cart when restocking exchange trays on your medical carts.

2016 Preceptor of the Year The Fairleigh Dickinson University School of Pharmacy & Health Sciences is proud to acknowledge Dr. Ralph Bury as our 2016 Preceptor of the Year! Dr. Bury is currently the pharmacy manager at Walgreens Store #3146 located in Newark, NJ. He was nominated for this award by the Class of 2016 and was honored at our school’s annual White Coat Ceremony this past August. He has participated in a wide array of community events including HIV awareness and education, flu and meningitis clinics, and senior outreach programs. Dr. Bury encourages his students to think outside the box and has them travel on field trips to see the variety of roles within the retail setting including, but not limited to, Hepatitis C, HIV, and transplant specialties. Since the inception of our program, he has proudly precepted 20 FDU students and counting. Dr. Bury has been praised by students for being a great preceptor who always encourages students and finds unique opportunities for them to learn!

It is a totally closed system, and its polymer construction avoids breakage and potential injury. Its design eliminates the usual timeintensive steps associated with reconstituting multiple 10 gram vials of drugs, resulting in fewer manipulations, which, in turn, reduces the risk of contamination of product that is administered to a patient. SmartPak® is compatible with commerciallyavailable pumps, tubing sets, cassettes, and syringes.
Features • SmartPak® is composed of two sterile bags. • An inner bag is in direct contact with sterile drug product and is made of an additive-free polymer to ensure maximum stability and shelf life. • Sealed double port on the bottom allows for introduction of diluent and subsequent withdrawal of the bag contents. • A labeled, filled bag is placed into an outer foil bag. • This outer bag protects the inner bag from light, moisture, and oxidation, maximizing stability of the drug product. The following will briefly explain our procedures and service capabilities. n What to Expect on Inventory Day Upon arrival, the Capital Inventory team will be introduced to the pharmacy and staff. Our team leader and a pharmacy representative
will map the pharmacy and its respective areas. Team members are then assigned to the designated areas and proceed entering each NDC number and QTY into a hand-held data-gathering computer. Once all areas are completed, the raw inventory data is then transmitted to an independent team for review and analysis. n What to Expect Post Inventory Day Items will be priced according to GPO/Wholesaler Cost Data and Direct Purchase/Secondary Wholesaler Cost Data. Upon review and analysis, the data will be adjusted, compiled, and valuated. Finally, the inventory data will be delivered electronically as an Excel file. n What to Expect After Receipt of Inventory Data If it’s identified that adjustments are needed, we have an easy stepby-step way to apply the indicated changes. When submitted, the adjustments can be made and returned within 24 to 48 hours. The easy-to-follow, step-by-step illustrated instructions allow the user to set up assays within minutes. In addition to FDA licensure, the Pyrosate® Kit is now available in sensitivities of 0.03 EU/mL, 0.125 EU/mL and 0.25 EU/mL, in both 10-pack and 30-pack formats. With the introduction of this licensed version, in addition to research and water testing, Pyrosate is ideal for dialysis centers, compounding pharmacies, and pharmaceutical and medical device companies.
Performing the Test
The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.
Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage.
